ABSTRACT

RESUMO: O artigo refaz o percurso de Freud desde a exclusão de das Ding até o surgimento da dimensão da objetalidade por meio da constituição da realidade estruturada como fantasia. Com Lacan, coloca-se em evidência o objeto a como causa do desejo no Seminário da Angústia, no qual se discute a caducidade dos objetos que estabelecem bordas pelas quais a libido irá se deslocar em busca do objeto perdido. Enfim, aborda-se o colapso da objetalidade na vigência do discurso do capitalista. Em particular, é sublinhado o engodo desse discurso que faz supor acessível o objeto perdido na forma dos gadgets que a cada vez renovam a promessa de mais gozar.


Abstract: The article retraces Freud's path from the exclusion of das Ding to the emergence of the dimension of objectality through the constitution of reality structured as fantasy. In Lacan´s Seminary of Anguish, it is highlighted the concept of object taken as cause of desire, resulting from his discussion of the downfall of the objects that establish the borders through which the libido will move in search of the lost object. At the end, the collapse of objectality caused by the capitalist discourse is examined. In particular, the deception of this discourse is underlined, as it makes it seem as if the lost object is accessible in the form of gadgets that each time renew the promise of more enjoyment.

ABSTRACT

A série inglesa Black Mirror desnuda o humano a partir da sua complexa relação com a tecnologia de uma forma que impacta e provoca o telespectador. Algo inquietante sobre nós é revelado através do espelho da série, que convida a refletir sobre um uso destrutivo das novas ferramentas e suas possíveis consequências. O autor aceita o convite e examina a partir das principais temáticas da série, o que em nosso funcionamento psíquico pode levar a um uso mortífero da tecnologia. Na busca desse objetivo, este trabalho propõe um diálogo entre as narrativas de Black Mirror e conceitos psicanalíticos de Freud, Meltzer, Bion e Green.(AU)


The English series Black Mirror exposes the human from its complex relationship with technology in a way that impacts and stirs the viewer. Something uncanny about us is revealed through series mirror, which invites us to reflect on the destructive use of new tools and their possible consequences.The author accepts the invitation and examines it from the main themes of the series, which in our psychic functioning can lead to a deadly use of technology. In the pursuit of this objective, this work proposes a dialogue between Black Mirror narratives and psychoanalytic concepts of Freud, Meltzer, Bion and Green.(AU)


La serie inglesa Black Mirror despoja al ser humano en su compleja relación con la tecnología de una manera que impacta y provoca al espectador. Algo inquietante acerca de nosotros se revela a través del espejo de la serie, que nos invita a reflexionar sobre el uso destructivo de nuevas herramientas y sus posibles consecuencias. El autor acepta la invitación y la examina a partir de los principales temas de la serie, que en nuestro funcionamiento psíquico pueden derivar en un uso letal de la tecnología. En la búsqueda de este objetivo, este trabajo propone un diálogo entre las narrativas de Black Mirror y los conceptos psicoanalíticos de Freud, Meltzer, Bion y Green.(AU)

ABSTRACT

Climatério é período de transição hormonal da mulher, com manifestações genitais e extragenitais que requerem ou não tratamento. Este estudo transversal analisou o conhecimento básico de mulheres de Curitiba-PR entre 45 e 60 anos sobre terapia de reposição hormonal através de questionário autoaplicado. Das 623 mulheres participantes a minoria fez ou fazia uso da terapia de reposição hormonal, sendo sintomas prevalentes dessa fase fogachos, distúrbio do sono, alteração do humor e diminuição da libido. Mais de 80% delas acreditavam que terapia de reposição hormonal pode melhorar qualidade de vida. No entanto, muitas disseram ter medo de iniciar esse tipo de tratamento. Maioria das entrevistadas foram orientadas sobre a menopausa, porém apenas uma parte delas foi instruída sobre benefícios e riscos da terapia. Esses dados corroboram a premissa de que ainda faltam orientações e medidas voltadas para o cuidado integral das mulheres no climatério


The climateric is a biological transition phase of the woman's life, being accompanied by genital and extra-genital manifestations whose symptons may, or may not, require treatment. The present transversal study intends to analyze the basic knowledge of the women between 45 and 60 years old about the hormone replacement therapy through a self-administered questionnaire. It has been observed that only few women have used or use hormone replacement therapy and that the most prevalent symptoms on this phase are hot flushes, sleep disturbance, mood changes and decreased libido. More than 80% of the interviewees believe that the hormone replacement therapy can improve life quality. However, many reported being afraid to initiate such treatment. The majority of the interviewees have been previously oriented about the menopause, but only a part of them have been warned about the benefits and risks of the said therapy. These data corroborate the premise that there is a lack of guidance and measures aimed at the integral care of women in the climateric

ABSTRACT

Resumo O término do ciclo fértil das mulheres está historicamente atrelado a sua capacidade reprodutiva, demarcando de maneira significativa o início do processo de envelhecimento. O climatério, momento de transição entre o período fértil e a menopausa (que é a última menstruação), é percebido em nossa sociedade como uma questão médica. O objetivo deste artigo é analisar como os hormônios, considerados produtos atuantes em dinâmicas humanas, agenciam modos de subjetivação entre mulheres que passam pela menopausa na nossa sociedade, que valoriza a produtividade e a juventude. Nesta pesquisa, inspirada na cartografia de controvérsias proposto por Bruno Latour, foram analisados sete vídeos disponibilizados no Youtube pela indústria farmacêutica Bayer que abordavam direta ou indiretamente a "reposição hormonal" para mulheres. Identificamos que as mudanças hormonais na velhice são compreendidas como um desequilíbrio, causador de calores, problemas de libido, osteoporose, problemas cardíacos, secura vaginal etc. Tais percepções levam à busca por soluções médicas, como a reposição hormonal, para que o corpo alcance novamente o equilíbrio perdido com o envelhecimento. Concluímos que os hormônios são prescritos pelos médicos com a promessa de as mulheres se manterem jovens e sexualmente atraentes para seus parceiros do sexo masculino dentro de uma perspectiva que reitera o machismo e a heteronormatividade na sociedade.(AU)


Abstract Historically, the end of women' fertile years is intrinsically associated with their reproductive capacity, marking the beginning of their aging process. In our society, the climacteric (transition period between fertility and menopause, consisting of the last menstruation) is perceived as a medical question. Thus, this article aimed to discuss how hormones, considered as active products in human dynamics, act as subjectivation modes among women who undergo menopause in a society such as ours, which values youth and productivity. Inspired by Bruno Latour's cartography of controversies, this research analyzed seven videos made available by the Bayer pharmaceutical company on YouTube, approaching "hormone replacement" for women either directly or indirectly. The results indicate that age-related hormonal changes are understood as an imbalance that triggers hot flashes, decreased libido, osteoporosis, heart diseases, and vaginal dryness. Such understanding drives the search for medical interventions aiming to regain the body balance lost with aging. Thus, doctors prescribe hormones under the guise of keeping women young and sexually attractive to their male partners, reiterating a logic of machismo and heteronormativity within our society.(AU)


Resumen El final del ciclo fértil femenino históricamente relacionado a la capacidad reproductiva marca de manera significativa el inicio del proceso de envejecimiento de las mujeres. El climatérico, momento de transición entre el período fértil a la menopausia caracterizado por la última menstruación, es percibido por nuestra sociedad como una cuestión médica. El objetivo de este artículo es analizar cómo las hormonas, consideradas como productos activos en la dinámica humana, actúan en los modos de subjetivación entre las mujeres, que atraviesan la menopausia, en una sociedad que valora la productividad y la juventud. En esta investigación, inspirada en la cartografía de controversias propuesta por Bruno Latour, se analizaron siete vídeos disponibles en YouTube por la industria farmacéutica de Bayer, los cuales plantean directa o indirectamente la terapia de "reemplazo hormonal" para las mujeres. Se identificó que los cambios hormonales en las personas mayores se entienden como un desequilibrio que provocaría calores, problemas en la libido, osteoporosis, problemas cardíacos, sequedad vaginal, etc. Tales hallazgos implican en la búsqueda de orientación médica para que el cuerpo alcance de nuevo el equilibrio perdido con el envejecimiento. Se concluye que las hormonas son prescriptas por médicos con la promesa de que las mujeres seguirán siendo jóvenes y sexualmente atractivas para su pareja masculina, dentro de una perspectiva que reitera el machismo y la heteronormatividad de nuestra sociedad.(AU)

ABSTRACT

Resumo Objetivo analisar o conceito de disfunção sexual em mulheres com câncer do colo do útero submetidas a tratamento radioterápico. Método estudo de análise de conceito fundamentado no referencial metodológico proposto por Walker e Avant, o qual é composto por oito etapas: seleção do conceito; determinação dos objetivos da análise conceitual; identificação dos possíveis usos do conceito; determinação dos atributos críticos ou essenciais; construção de um caso modelo; construção de casos adicionais; identificação dos antecedentes e consequentes do conceito; e definição das referências empíricas. Resultados a amostra final resultou em 11 artigos, que identificaram 4 atributos críticos, 18 antecedentes e 07 consequentes do conceito, os quais permitiram a construção de uma definição conceitual e elaboração de um caso-modelo, com a finalidade de exemplificar o contexto social no qual o conceito encontra-se inserido. Conclusão e implicações para prática alterações na estrutura corporal, déficit de conhecimento, diminuição da frequência das relações sexuais, dispareunia e indiferença do parceiro são alguns dos principais antecedentes e consequentes da disfunção sexual na população estudada. Entretanto, esses fatores ainda são pouco conhecidos, o que limita a realização de ações de promoção, prevenção e reabilitação sexual, sobretudo no campo da enfermagem.


Resumen Objetivo analizar el concepto de disfunción sexual en mujeres con cáncer de cuello uterino sometidas a tratamiento radioterápico. Método estudio de análisis de concepto basado en el marco metodológico de Walker y Avant, compuesto por ocho etapas: selección del concepto; determinación de los objetivos del análisis conceptual; identificación de posibles usos del concepto; determinación de atributos críticos o esenciales; construcción de un caso modelo; construcción de casos adicionales; identificación de antecedentes y consecuentes del concepto; y definición de referencias empíricas. Resultados La muestra final resultó en 11 artículos: 04 críticos, 18 antecedentes y 07 consecuentes del concepto, lo que permitió la construcción de una definición conceptual y la elaboración de un caso modelo con el fin de ejemplificar el contexto social en que se inserta el concepto. Conclusión e implicaciones para la práctica Cambios en la estructura corporal, déficit de conocimiento, disminución de la frecuencia de las relaciones sexuales, dispareunia y la indiferencia de la pareja son algunos de los principales antecedentes y consecuentes de la disfunción sexual en la población estudiada. Sin embargo, estos factores aún son poco conocidos, lo que limita la realización de acciones de promoción, prevención y rehabilitación sexual, especialmente en el campo de la enfermería.


Abstract Objective to analyze the concept of sexual dysfunction in women with cervical cancer undergoing radiotherapy. Method a concept analysis study based on the methodological framework proposed by Walker and Avant, which consists of eight stages: concept selection; determination of the objectives of the conceptual analysis; identification of possible uses of the concept; determination of critical or essential attributes; construction of a model case; construction of additional cases; identification of the antecedents and consequences of the concept; and definition of empirical references. Results the final sample resulted in 11 articles, which identified 4 critical attributes, 18 antecedents and 07 consequences of the concept, which allowed the construction of a conceptual definition and the elaboration of a model case in order to exemplify the social context in which the concept is inserted. Conclusion and implications for practice changes in body structure, lack of knowledge, decreased frequency of sexual intercourse, dyspareunia, and partner's indifference are some of the main antecedents and consequences of sexual dysfunction in the studied population. However, these factors are still little known, which limits the performance of actions of promotion, prevention and sexual rehabilitation, especially in the field of nursing.

ABSTRACT

Este artigo registra alguns resultados da pesquisa realizada durante o atendimento de dois pacientes no Projeto de Atendimento Temporário de Psicoterapia de Urgência, oferecido por uma universidade privada. Trata inicialmente do aumento das manifestações psíquicas patológicas resultantes da "urgência subjetiva" produzida pela pandemia da Covid-19 como ameaça à manutenção da vida. Destaca os principais pontos da obra freudiana concernentes à importância da organização anal da libido e das fezes como objeto de satisfação pulsional. O complexo de castração é responsável por permitir que os objetos adquiram a significação fálica capaz de equivaler as fezes ao dinheiro. O dinheiro é um dos objetos para veicular as fixações decorrentes da fase anal de organização da libido. Acrescenta que a relação de cada sujeito com o dinheiro pode se apresentar na transferência como exigência de satisfação pulsional autoerótica. A repetição indica o lugar do sujeito e do Outro na fantasia inconsciente. Assim, a gratuidade pode se tornar um veículo de gozo caso não seja simbolizada a existência de um outro tipo de ganho: para o paciente, conhecimento sobre seu modo de gozo; para o analista em formação, uma oportunidade de aprender algo que desconhecia; para o supervisor, uma investigação pertinente à sua pesquisa


Cet ouvrage reproduit certains résultats de la recherche menée au cours des soins de deux patients dans le cadre du Projet temporaire de soins de psychothérapie d'urgence, offert par une université privée. Il traite d'abord de l'augmentation des manifestations psychiques pathologiques résultant de l'«urgence subjective¼ produite par la pandémie covid-19 comme une menace au maintien de la vie. Il met en évidence les principaux points du travail freudien concernant l'importance de l'organisation anale de la libido et des excréments comme objet partiel de satisfaction motrice. Le complexe de castration est responsable de permettre aux objets d'acquérir une signification phallique capable d'assimiler les excréments à l'argent. L'argent est l'un des objets pour transmettre les fixations découlant de la phase anale de l'organisation de la libido. Il stipule que la relation de chaque sujet avec l'argent peut se présenter en transfert comme une exigence de satisfaction de pulsion autoérotique. La répétition de cette exigence indique la place du sujet et de l'Autre dans la fantaisie inconsciente. Ainsi, la gratuité peut devenir un vecteur de jouissance si l'existence d'un autre type de gain n'est pas symbolisée : pour le patient, la connaissance de son mode de jouissance ; pour l'analyste en formation, l'occasion d'apprendre quelque chose qu'il ne savait pas; pour le superviseur, une investigation capable d'avancer ses recherches


This article reports some results of the research conducted during the care of two patients in the Temporary Emergency Psychotherapy Care Project, offered by a private university. It initially deals with the increase in pathological psychic manifestationsresulting from the "subjective urgency" produced by the Covid-19 pandemic as a threat to the maintenance of life. It highlights the main points of freudian work concerning the importance of anal organization of libido and feces as a partial object of drive satisfaction. The castration complex is responsible for allowing objects to acquire phallic significance capable of equating feces to money. Money is one of the objects to convey the fixations arising from the anal phase of organization of libido. It states that the relationship of each subject with money can present itself in transfer as a requirement of autoerotic drive satisfaction. The repetition of this requirement indicates the place of the subject and the Other in unconscious fantasy. Thus, gratuity can become a vehicle of jouissance if the existence of another type of gain is not symbolized: for the patient, knowledge about his mode of enjoyment; for the analyst in training, an opportunity to learn something he did not know; for the supervisor, an investigation to contribute to his research

Este estudo teve por objetivo comparar variações de parâmetros andrológicos e comportamentais de touros Nelore de diferentes faixas etárias, calcular seu potencial reprodutivo (PR) e propor uma nova tabela de classificação por pontos, de acordo com as médias atualmente alcançadas por eles nas características estudadas. Foram utilizados dados de 6162 exames andrológicos de touros da raça Nelore, entre 12 e 80 meses de idade, em regime de monta natural. O número de touros classificados como aptos consistiu em 88,9% dos animais avaliados (n=5480), sendo 51,6% desses considerados excelentes (n=2827), 41,2% muito bons (n=2257) e 7,2% considerados bons (n=394). Entre os animais questionáveis (n=682; 11,1%), 79,6% foram classificados como inaptos temporários (n=542) e 20,4% (n=139) como animais descarte, de acordo com o exame andrológico, independentemente do teste da libido. O número de touros classificados como excelentes se reduziu para 752 (12,2%) quando dados de comportamento sexual foram incluídos para definição do seu PR. Concluiu-se que o uso de tabelas de classificação andrológica por pontos com atualizações técnicas beneficia a seleção mais apurada de touros Nelore. O teste da libido é ferramenta importante para a determinação do PR, o qual permite melhor aproveitamento dos reprodutores.(AU)


This study aimed to compare variations of andrological and behavioral parameters from Nelore bulls of different ages, to calculate their reproductive potential (RP) and propose a new classification table by points, considering current averages in each reproductive trait studied. Data were collected from 6162 breeding soundness examinations of Nelore bulls aged between 12 and 80 months, under natural mating. According to andrological parameters, regardless of the libido test, the number of bulls classified as approved was 88.9% (n= 5480), being 51.6% considered as excellent (n= 2827), 41.2% very good (n= 2257) and 7.2% considered as good (n= 394). Among the animals considered as questionable (n= 682; 11.1%), 79.6% were classified as temporarily reproved (n= 542) and 20.4% (n= 139) as discarded animals. The number of bulls classified as excellent decreased to 752 (12.2%) when sexual behavior data were included to define their RP. It was concluded that the use of tables for andrological classification by points with technical updates improves the reproductive selection of Nelore bulls. The libido test is an important tool for RP determination which provides better utilization of the sires.(AU)

ABSTRACT

Abstract: This study aims primarily to reflect on the role of symbiotic illusion in the construction of female identity in women with eating disorders (ED), using premises from psychoanalytic psychosomatics. Considering ED as psychopathologies related to the affective economy, Oedipal relations play crucial role in the process of identity constitution. Mother-daughter relationships in ED are shaped in the molds of symbiotic illusion, with weak father figures. We presented a case study to briefly illustrate the theoretical framework. The omissive posture of a father to interdict the symbiotic relation leads daughters to find themselves unable to libidinally invest in other objects.


Resumo: Este estudo teve como objetivo refletir sobre o papel da ilusão simbiótica na construção da identidade feminina em mulheres com transtornos alimentares (TAs), utilizando as premissas da psicossomática psicanalítica. Considerando-se os TAs como psicopatologias relacionadas à economia afetiva, as relações edípicas desempenham papel crucial no processo de construção de identidade. As relações mãe-filha nos TAs são moldadas pela ilusão simbiótica, com figuras paternas enfraquecidas. Nós apresentamos brevemente um estudo de caso para ilustrar os conceitos do referencial teórico. A postura omissa do pai para interditar a relação simbiótica com a mãe leva a filha à impossibilidade de investir libidinalmente em outros objetos.

ABSTRACT

RESUMEN El objetivo del estudio es discutir uno de los usos del crack denominado "virado", como estrategia de reducción de daños entre las personas que usan crack en Pernambuco, Brasil. Se realizó una investigación cualitativa y transversal. Desde marzo hasta agosto de 2016, se realizaron entrevistas semiestructuradas sobre la cultura del uso de crack a 39 personas que usan esta substancia. El límite de participantes se estableció por el criterio de saturación. Los datos se analizaron con la técnica de análisis de contenido. Las personas que participaron relataron que el virado es una manera distinta de utilizar el crack y, al comparar su efecto con el uso fumado o inhalado, mencionaron que el virado produce menos impacto en las relaciones interpersonales y en la libido, además de reducir el uso compulsivo de crack, cuestiones que se podrían considerar como estrategias de reducción de daños. Un aspecto negativo es que comparten los canutos para aspirar el virado, lo cual es una situación de riesgo para la transmisión de enfermedades infecciosas. Conocer la cultura del uso del crack en distintas formas y situaciones es imprescindible para la planificación y desarrollo de acciones de atención a la salud.


ABSTRACT The aim of the study is to discuss the use of crack in the form of "virado" as a harm reduction strategy in Pernambuco, Brazil. This is a cross-sectional study with a qualitative approach in which semi-structured interviews were conducted regarding aspects related to the culture of crack use with 39 crack users between March and August 2016. Participants were recruited using saturation criteria and data were analyzed through content analysis. Respondents discussed the use of "virado" and compared its effects in relation to crack, addressing improvement in interpersonal relationships, libido, and non-compulsive drug use, which can all be understood as harm reduction strategies. On the other hand, equipment sharing for the use of "virado" was identified as a high-risk practice with regards to the transmission of infectious diseases. Knowing about the culture of crack use in different contexts is essential in order to plan and develop health care actions.

O presente artigo tem por objetivo traçar uma genealogia possível da técnica psicanalítica apontando para a importância das dimensões histórica, política e social na construção da psicanálise. A partir do fenômeno do magnetismo animal, postulado por Franz Anton Mesmer, no século XVIII, passando pela hipnose e pela sugestão até alcançarmos a transferência na psicanálise, buscaremos remontar a influência como pressuposto ético-político que perpassa e, mesmo, possibilita todas essas práticas.(AU)


The present article aims to delineate a possible genealogy of the psychoanalytical technique indicating the importance of the historical, political and social dimensions in the construction of the psychoanalysis. From the animal magnetism phenomenon, postulated by Franz Anton Mesmer, in the eighteenth century, moving to hypnosis and suggestion until achieving the transference in psychoanalysis, we seek reassemble the influence as ethical-politic presuppose that passes through, and yet, makes possible all these practices.(AU)


El artículo tiene por objectivo trazar una genealogía possible de la técnica psicoanalítica marcando la importancia de las dimenciones históricas, política y sociales en la construcción de la psicoanálisis. A partir del fenômeno del magnetismo animal, propuesto por Franz Anton Mesmer, en siglo XVIII, pasando por la hipnosis y por la sugestión, hasta encuentrarmos la transferencia psicoanalítica, intentamos remontar la influencia como pressupuesto ético-político que atraviesa y possibilita todas esas practicas.(AU)

ABSTRACT

Abstract Introduction Sexual dysfunction is common in individuals with psychiatric disorders and under psychotropic medication such as antidepressants and antipsychotics. Several scales have been developed to assess sexual function in these patients. The Arizona Sexual Scale (ASEX) is a five-item rating scale that quantifies sex drive, arousal, vaginal lubrication/penile erection, ability to reach orgasm, and satisfaction from orgasm. We describe the translation and cross-cultural adaptation of the ASEX into the Portuguese language, with the goal of contributing to the assessment of sexual function in Portuguese-speaking psychiatric patients under treatment with psychotropic drugs. Methods The translation and cross-cultural adaptation process thoroughly followed the steps recommended by the Task Force of the International Society for Pharmacoeconomics and Outcomes Research (ISPOR), namely: preparation, forward translation, reconciliation, back-translation, back-translation review, harmonization, cognitive debriefing, review of cognitive debriefing, finalization, proofreading, and final version. Results The process was successfully completed and no major differences were found between the translation, reconciliation and back-translation phases, with only small adjustments being made. Conclusion The translation of the ASEX was completed successfully, following international reference guidelines. The use of these guidelines is a guarantee of a Portuguese version that is qualitatively and semantically equivalent to the original scale. This availability of this new scale version will enable studies evaluating the sexual function of Portuguese-speaking psychiatric patients. Future studies may assess the validity of the scale for Portuguese-speaking populations.


Resumo Introdução A disfunção sexual é comum em indivíduos com doenças psiquiátricas e sob o uso de medicações como antidepressivos e antipsicóticos. Várias escalas foram desenvolvidas para avaliar a função sexual desses doentes. A Arizona Sexual Scale (ASEX) é uma escala de cinco itens de avaliação que quantifica desejo sexual, excitação, lubrificação vaginal/ereção peniana, capacidade para atingir o orgasmo e satisfação com o orgasmo. Este artigo descreve o processo de tradução e adaptação transcultural da escala ASEX para a língua portuguesa, com o objetivo de contribuir para a avaliação da função sexual dos doentes medicados com fármacos psicotrópicos nos vários países onde se utiliza essa língua. Métodos A tradução e a adaptação transcultural seguiram de forma detalhada os passos recomendados pelo grupo de trabalho da International Society for Pharmacoeconomics and Outcomes Research (ISPOR), nomeadamente: preparação, tradução inicial, reconciliação, retroversão, revisão da retroversão, harmonização, teste cognitivo, revisão do teste cognitivo, finalização, leitura final e versão final. Resultados O processo foi completado com sucesso, e não foram observadas diferenças grandes entre as fases de tradução, reconciliação e retroversão, tendo sido feitos apenas pequenos ajustes. Conclusão A tradução da escala ASEX foi bem-sucedida, seguindo orientações internacionais de referência. A aplicação dessas orientações é a garantia de uma versão em língua portuguesa que é qualitativa e semanticamente equivalente à versão original da escala. A existência desta nova versão da escala permitirá estudos que avaliem a função sexual dos doentes em países nos quais se fale a língua portuguesa. Estudos futuros poderão atestar a validade da escala para essas populações.

ABSTRACT Objective To summarize current evidence regarding testosterone treatment for women with low sexual desire. Materials and methods The Female Endocrinology and Andrology Department of the Brazilian Society of Endocrinology and Metabolism invited nine experts to review the physiology of testosterone secretion and the use, misuse, and side effects of exogenous testosterone therapy in women, based on the available literature and guidelines and statements from international societies. Results Low sexual desire is a common complaint in clinical practice, especially in postmenopausal women, and may negatively interfere with quality of life. Testosterone seems to exert a positive effect on sexual desire in women with sexual dysfunction, despite a small magnitude of effect, a lack of long-term safety data, and insufficient evidence to make a broad recommendation for testosterone therapy. Furthermore, there are currently no testosterone formulations approved for women by the relevant regulatory agencies in the United States, Brazil, and most other countries, and testosterone formulations approved for men are not recommended for use by women. Conclusion Therefore, testosterone therapy might be considered if other strategies fail, but the risks and benefits must be discussed with the patient before prescription. Resumo A adicção sexual pode ser definida como quadro psicopatológico no qual o sexo é buscado de forma incontrolável, desenfreada, a despeito dos prejuízos emocionais, sociais e profissionais que suscita no cotidiano do adicto. Após o frenesi, a excitação e a adrenalina em ceder aos próprios impulsos urgentes e vorazes, o sex-addict percebe-se incapaz de gerenciar suas atuações sexuais, sendo consumido por sentimentos de vazio, fracasso e desespero. A problemática do prazer sexual "além do princípio de prazer", conjugada à da dor psíquica, nos conduz a uma interrogação mais rigorosa sobre a dimensão do masoquismo na vida subjetiva e sua relação com os estados destrutivos da psicossexualidade. Neste artigo, fundamentado sob o viés da psicanálise, procuramos examinar a estreita relação entre autodestruição e satisfação libidinal nos fenômenos próprios do quadro em questão.


Résumé La dépendance sexuelle peut être définie comme une condition psychopathologique dans laquelle le sexe est cherché de façon incontrôlable, effrénée, malgré les préjudices émotionnels, sociaux et professionnels causés à la vie quotidienne de le dépendent. Après la frénésie, l'excitation et l'adrénaline ressenties en s'adonnant à ses propres impulsions urgentes et voraces, le sex-addict se sent incapable de gérer ses performances sexuelles, consumés par des sentiments de vide, d'échec et de désespoir. Le problème du plaisir sexuel « au-delà du principe du plaisir ¼, combiné à celui de la douleur psychique, conduit à un questionnement plus rigoureux sur la dimension du masochisme dans la vie subjective et sa relation avec les états destructeurs de la psychosexualité. Dans cet article, basé sur la perspective de la psychanalyse, on cherche à examiner la relation étroite entre l'autodestruction et la satisfaction libidinale dans les phénomènes caractéristiques de la condition en question.


Resumen La adicción sexual puede ser definida como un cuadro psicopatológico en el cual se busca el sexo de forma incontrolable, desenfrenada, a pesar de los perjuicios emocionales, sociales y profesionales provocados en el cotidiano del adicto. Después del frenesí, de la excitación y de la adrenalina en ceder a los impulsos urgentes y voraces, el adicto sexual se percibe incapaz de gestionar sus actuaciones sexuales, siendo consumido por sentimientos de vacío, fracaso y desesperación. La problemática del placer sexual "más allá del principio de placer", combinada a la del dolor psíquico, nos conduce a una interrogación más rigurosa sobre la dimensión del masoquismo en la vida subjetiva y su relación con los estados destructivos de la psicosexualidad. Fundamentado en el sesgo del psicoanálisis, en este artículo nos proponemos analizar la estrecha relación entre la autodestrucción y la satisfacción libidinal en los fenómenos propios del cuadro en cuestión.


Abstract Sexual addiction can be defined as a psychopathological condition in which sex is sought in an uncontrollable, unrestrained manner, despite the emotional, social, and professional harm caused to the addict's daily life. After the frenzy, excitement, and adrenaline rush experienced in indulging in their own urgent and eager impulses, sexual addicts find themselves incapable to manage their sexual performances, being consumed by feelings of emptiness, failure, and despair. The issue of sexual pleasure "beyond the pleasure principle," combined with the issue of psychic pain, leads us to a more rigorous questioning about the dimension of masochism in subjective life and its relation with the destructive states of psychosexuality. In this article, based on the perspective of psychoanalysis, we seek to examine the close relation between self-destruction and libidinal satisfaction in the phenomena that are characteristic of the condition in question.

ABSTRACT

Finasteride is primarily used to treat benign prostatic hyperplasia (BPH) and male androgenetic alopecia (MAA). Five-alpha reductase inhibitors (5α-RIs) could induce male sexual dysfunction due to their effects on testosterone and dihydrotestosterone. There is evidence suggesting that 5α-RIs may independently increase the risk of erectile dysfunction (ED). However, many investigators believe that side effects of 5α-RIs will disappear with continuous treatment. Considerable controversy exists regarding the severity and persistence of side effects of finasteride on ED. The aim of this review was to summarize current research studies on finasteride associated with ED. The search strategy used each term of finasteride and ED against PubMed database to identify related studies. ED data reported from available trials for finasteride were summarized and reviewed. Although there is not enough evidence to prove the relationship between finasteride and ED, most studies in this review found that finasteride for BPH was correlated with ED. However, most studies included in this review revealed that finasteride for MAA was not correlated with ED. On the other hand, some studies reported side effects of finasteride associated with sexual dysfunction, including ED, male infertility, ejaculation problem, and loss of libido, even in MAA patients. Well-designed randomized controlled trials are needed to further determine the mechanism and effects of finasteride on ED. However, physicians should discuss with their patients possible long-term effects of finasteride on sexual function, although we do not have evidence showing that adverse events of sexual dysfunction are absolutely associated with 5α-RIs.

ABSTRACT

Testosterone has a variety of functions and is commonly used in older men to treat symptoms of hypogonadism, such as decreased libido, decreased mood and erectile dysfunction. Despite its positive effects on sexual function, it has a negative effect on fertility. Exogenous testosterone therapy can negatively affect the hypothalamic-pituitary gonadal axis and inhibit the production of follicle stimulating hormone and luteinizing hormone. The purpose of this review is to discuss the contraceptive properties of testosterone therapy and to discuss strategies to increase testosterone in men with the desire to preserve fertility.

ABSTRACT

PURPOSE: To characterize the population of hypogonadal men who presented to a tertiary academic urology clinic and evaluate risk factors for primary vs. secondary hypogonadism. MATERIALS AND METHODS: We evaluated all men with International Classification of Diseases-9 diagnosis codes R68.82 and 799.81 for low libido, 257.2 for testicular hypofunction, and E29.1 for other testicular hypofunction at a tertiary academic medical center from 2013 to 2017. We included men who had testosterone (T) and luteinizing hormone (LH) drawn on the same day. We classified men based on T and LH levels into eugonadal, primary, secondary, and compensated hypogonadism. Risk factors including age, body mass index (BMI) over 30 kg/m2, current smoking status, alcohol use greater than 5 days per week, and Charlson comorbidity index greater than or equal to 1 were investigated and measured in each group using the eugonadal group for reference. RESULTS: Among the 231 men who had both T and LH levels, 7.4%, 42.4%, and 7.4% were classified as primary, secondary, and compensated hypogonadism, respectively. Only elevated BMI was associated with secondary hypogonadism compared to eugonadal men (median BMI, 30.93 kg/m2 vs. 27.69 kg/m2, p=0.003). BMI, age, comorbidities, smoking, or alcohol use did not appear to predict diagnosis of secondary hypogonadism. CONCLUSIONS: Secondary hypogonadism appears to be the most common cause of hypogonadism among men complaining of low T and decreased libido at a tertiary academic medical center. Secondary hypogonadism is associated with elevated BMI and therefore obesity should be used as a marker to evaluate men for both T and LH levels.

ABSTRACT

Male sexual dysfunction refers to a phenomenon in which a man experiences difficulty at any stage during the process of sexual intercourse. In general, erectile dysfunction is regarded as the most representative form of sexual dysfunction, but various other diseases can also be categorized as male sexual dysfunction, including sexual arousal disorder, decreased libido, ejaculation disorder, and Peyronie's disease. Causes of sexual dysfunction include chronic diseases, such as diabetes, hypertension, dyslipidemia, and obesity. In addition, some medications, surgical procedures, and traumas can cause sexual dysfunction. However, aging is the most important cause of male sexual dysfunction. To diagnose and treat elderly patients who complain of male sexual dysfunction, it is first necessary to become familiar with the characteristics of sexual dysfunction in elderly men. The prevalence rates of metabolic syndrome, hypertension, diabetes, dyslipidemia, coronary artery disease, stroke, and depression are higher among elderly men than among younger men; furthermore, the elderly are at a higher risk for the development of kidney, hepatic, spinal cord, and neurological diseases. Notably, anti-hypertensive agents can affect erectile function in elderly men: sexual dysfunction may be severe or the response to treatment may be poor. For satisfactory treatment, spousal factors should also be considered.
