ABSTRACT
Introdução: A gestão de informações associadas à multimorbidade na atenção hospitalar é relevante para o planejamento de estratégias de prevenção de agravos à saúde em pacientes de maior risco, a fim de oportunizar a organização de sistemas de saúde de modo eficiente. Objetivo: O presente trabalho objetivoudelinear o caminho formal percorrido por pacientes com e sem multimorbidade, considerando o uso da rede pública de saúde após a internação hospitalar. Materiais e métodos: Realizou-se um estudo quantitativo, transversal, descritivo, utilizando dados primários de 445 pacientes internados em um hospital universitário, no ano de 2018. Os dados foram coletados através de análise do prontuário médico e entrevista telefônica. Os resultados foram analisados por meio de frequência absoluta e relativa. Resultados: Desenvolveu-se um fluxograma, representando os pontos da rede de saúde utilizados pelo paciente após a alta hospitalar, segundo multimorbidade. Verificou-se uma alta prevalência de encaminhamento (com multimorbidade (CM) 93,52%; sem multimorbidade (SM) 97,71%) e comparecimento na atenção secundária à saúde (CM 86,15%; SM 89,63%), um baixo encaminhamento (CM 42,45%; SM 36,27%) e comparecimento na atenção primária à saúde (CM 61,29%; SM 64,81%), e considerando os 3 níveis de atenção juntos, houve um baixo comparecimento (CM 17,98%; SM 21,89%) para ambos os grupos investigados. Discussão: Entende-se que a semelhança de comparecimento em todos os pontos da rede por ambos os grupos é um problema, por tratar de forma igual populações desiguais e consequentemente com necessidades diversas. Conclusões: Sinaliza-se a importância de maior incentivo ao acompanhamento de pacientes com multimorbidade na rede primária de saúde, especialmente no período de pós-alta hospitalar, e a necessidade de fortalecimento da Rede de Atenção à Saúde.
Introduction: Managing information related to multimorbidity in hospital care is crucial for planning strategies to prevent health problems in high-risk patients in order to enable healthcare systems to be organized more efficiently. Objective: To describe the formal healthcare network path for patients with and without multimorbidity in relation to the use of public healthcare networks after discharge. Materials and Methods: A quantitative descriptive cross-sectional study was conducted with primary data from 445 patients admitted to a university hospital in 2018. Data were collected through analysis of medical records and telephone interviews. Results were analyzed by absolute and relative frequency. Results: A flowchart was developed representing the healthcare network locations used by the patient after hospital discharge based on morbidity. It was possible to confirm the existence of a high prevalence of referral (with multimorbidity (WM): 93.52%, without multimorbidity (WOM): 97.71%) and secondary care attendance, low prevalence of referral (WM: 42.45%, WOM: 36.27%) and primary care attendance (WM: 61.29%, WOM: 64.81%). When putting the three levels of healthcare together, low attendance (WM: 17.98% - WOM: 21.89%) was observed in both groups under study. Discussion: Similar attendance at all healthcare network locations is problematic as these are unequal populations and thus, with different needs. Conclusions: It is important to encourage follow-up of patients with multimorbidity in the primary care network, especially in the period after hospital discharge and strengthen the healthcare network.

Objetivo: Describir la ruta formal recorrida por los pacientes con y sin multimorbilidades con base en el uso de la red pública de salud después de la hospitalización. Materiales y métodos: Se realizó un estudio cuantitativo, transversal y descriptivo con datos primarios de 445 pacientes ingresados en un hospital universitario en 2018. Los datos se recolectaron mediante el análisis de historias clínicas y entrevistas telefónicas. Los resultados se analizaron a través de frecuencia absoluta y relativa. Resultados: Se elaboró un flujograma que representa los puntos de la red de atención en salud utilizados por el paciente tras el alta hospitalaria según la morbilidad. Se confirmó la existencia de una alta prevalencia de remisión (con multimorbilidad (CM): 93.52%, sin multimorbilidad (SM): 97.71%) y asistencia a atención secundaria, baja remisión (CM: 42.45% - SM: 36.27%) y asistencia a atención primaria (CM: 61.29% - SM: 64.81%); al incluir los tres niveles de atención en conjunto se evidenció una baja asistencia (CM: 17.98% - SM: 21.89%) en ambos grupos investigados. Discusión: Se considera problemático la similitud en la asistencia en todos los puntos de la red por parte de ambos grupos, por tratarse de poblaciones desiguales y, en consecuencia, con necesidades diferentes. Conclusiones: Se señala la importancia de incentivar el seguimiento de los pacientes con multimorbilidades en la red de atención primaria, especialmente en el periodo posterior al alta hospitalaria y la necesidad de reforzar la red de atención en salud.

Objetivo: analisar as condições de saúde de mulheres idosas quilombolas residentes em uma cidade do interior do estado do Maranhão, Brasil. Método: Trata-se de um estudo transversal, de base domiciliar, realizado em 11 comunidades remanescentes de quilombolas no Município de Bequimão, Maranhão. Realizou-se o censo da população de idosos quilombolas, que foi representada por 132 mulheres idosas ≥60 anos. Os dados foram analisados no programa Stata® versão 14, com técnicas de estatística descritiva. Resultados: a mediana das idosas foi de 70 a 79 anos, (61,4%) se declarava na cor/raça preta; (32,6%) eram viúvas, 53,8% das mulheres não sabem ler e escrever. Quanto à situação de saúde, em relação às condições crônicas, 60,6% referiam acometimento por duas ou mais, 38,8% realizaram examepreventivo para câncer de colo doútero há pelo menos três anos. A maioria das idosas nunca realizou uma mamografia (67,2%) e nunca tinha sido submetida a um exame clínico das mamas (56,9%). Conclusão: A maioria das idosas quilombolas vivem em precária situação socioeconômica, sanitária e de saúde, entre eles, a prevalência de multimorbidade foi elevada. (AU)
Objective: to analyze the health conditions of elderly quilombola women living in a city in the interior of the state of Maranhão, Brazil. Method: This is a cross-sectional, household-based study conducted in 11 remaining quilombola communities in the Municipality of Bequimão, Maranhão. A census of the quilombola elderly population was carried out, which was represented by 132 elderly women ≥60 years of age. The data were analyzed in the Stata® version 14 program, with descriptive statistics techniques. Results: the median of the elderly women was 70 to 79 years old, (61.4%) declared themselves as black in color/race; (32.6%) were widows, 53.8% of the women cannot read and write. Regarding their health status, 60.6% reported having two or more chronic conditions, 38.8% had had a cervical cancer preventive exam at least 3 years ago. Most elderly women had never had a mammogram (67.2%) and had never been submitted to a clinical examination of the breasts (56.9%). Conclusion: Most elderly quilom-bola women live in precarious socioeconomic, sanitary, and health situations; among them, the prevalence of mul-ti-morbidity was high. (AU)

Objetivo: Descrever o perfil de óbitos por COVID-19 no município de Rio Grande, Rio Grande do Sul, Brasil. Metodologia: Trata-se de um estudo transversal, descritivo, com dados oriundos do banco de óbitos da Vigilância Epidemiológica, registrados no período de março a dezembro de 2020. Resultados: Dos 194 óbitos, a maioria era do sexo masculino (63,4%), com 60 anos ou mais (82,5%), de cor da pele branca (82,5%), residentes na região central histórica do município (11,3%). Quanto à ocupação, a maior ocorrência de óbitos foi entre os aposentados (69,5%), seguido por comerciante ou autônomo (17,7%). Com relação às morbidades, 38,7% tinha cardiopatias, 29,4% hipertensão arterial sistêmica, 28,0% diabetes mellitus e praticamente a metade dos indivíduos tinha multimorbidade (49,0%). Conclusões: Com a identificação do perfil de óbitos por COVID-19 no município de Rio Grande no período de março a dezembro de 2020, esses dados podem contribuir para auxiliar os gestores no planejamento de ações estratégicas e educativas de prevenção e combate à COVID-19, principalmente no direcionamento de grupos prioritários nas campanhas de vacinação (AU)
Objective: Describe the COVID-19 death profile in the city of Rio Grande, Rio Grande do Sul, Brazil. Methods: This is a cross-sectional descriptive study using data from the Epidemiological Surveillance service deaths database of deaths notified from March to December 2020. Results: Of the 194 deaths, most were male (63.4%), aged 60 years or more (82.5%), of white skin color (82.5%), and living in the central historic district of the city (11.3%). With regard to occupation, the highest occurrence of deaths was among retirees (69.5%), followed by tradesmen or the self-employed (17.7%). Regarding morbidities, 38.7% had heart disease, 29.4% hypertension, 28.0% diabetes mellitus, and practically half of the individuals had multiple morbidities (49.0%). Conclusions: We identified the profile of COVID-19 deaths in the city of Rio Grande in the period from March to December 2020. These data can help health service managers to plan strategic and educational actions to prevent and combat COVID-19, mainly by targeting priority groups in vaccination campaigns

ABSTRACT BACKGROUND: Due to the coronavirus disease 2019 (COVID-19) pandemic, access to healthcare services may have become difficult, which may have led to an increase in chronic diseases and multimorbidity. OBJECTIVES: To assess the incidence of multimorbidity and its associated factors among adults living in the state of Rio Grande do Sul, Brazil. DESIGN AND SETTING: Cohort study conducted in Rio Grande do Sul, Brazil. METHODS: We included data from the two waves of the Prospective Study About Mental and Physical Health (PAMPA). Data were collected via online questionnaires between June and July 2020 (wave 1) and between December 2020 and January 2021 (wave 2). Multimorbidity was defined as the presence of two or more diagnosed medical conditions. RESULTS: In total, 516 individuals were included, among whom 27.1% (95% confidence interval, CI: 23.5-31.1) developed multimorbidity from wave 1 to 2. In adjusted regression models, female sex (hazard ratio, HR: 1.97; 95% CI: 1.19-3.24), middle-aged adults (31-59 years) (HR: 1.78; 95% CI: 1.18-2.70) and older adults (60 or over) (HR: 2.41; 95% CI: 1.25-4.61) showed higher risk of multimorbidity. Back pain (19.4%), high cholesterol (13.3%) and depression (12.2%) were the medical conditions with the highest proportions reported by the participants during wave 2. CONCLUSION: The incidence of multimorbidity during a six-month period during the COVID-19 pandemic was 27.1% in the state of Rio Grande do Sul, Brazil.

Abstract Several studies are limited by verifying the level of physical activity with questionnaires and not through objective measurement in older adults. This article aims to analyze the association between a low level of physical activity with accelerometry) and mobility limitation in older adults. A population-based cross-sectional study conducted with 543 older adults. Multiple regression analysis was performed using hierarchical analysis, grouping the variables into two blocks ordered according to the precedence with which they acted on the outcomes. Among the evaluated older adults, 13.7% presented mobility limitations and among these 60.39% were in the low level of physical activity group. Older adults with a low level of physical activity (OR = 3.49 [2.0 - 6.13]), aged 75 and over (OR = 1.97 [1.03 - 3.72]), living without a partner (OR = 2.01 [1.09 - 3.68]), having difficulty performing basic (OR = 2.49 [1.45 - 4.28]) and instrumental (OR = 2.28) [1.18 - 4.36]) activities of daily life, and multimorbidity (OR = 2.06 [1.04 - 4.08]) were independently associated with mobility limitation. A low level of physical activity increases the chance of mobility limitation in older adults, regardless of sociodemographic and clinical variables.
Resumo Vários estudos são limitados por meio da verificação do nível de atividade física com questionários, mas não possuem medidas objetivas em adultos mais velhos. O objetivo deste artigo é analisar a associação entre um baixo nível de atividade física e limitação de mobilidade em idosos. Um estudo transversal de base populacional realizado com 543 idosos. A análise múltipla da regressão foi realizada usando a análise hierárquica, agrupando as variáveis em dois blocos ordenados de acordo com a precedência com que atuaram sobre os resultados. Entre os idosos avaliados, 13,7% apresentaram limitações de mobilidade e entre estes 60,39% estavam no baixo nível de atividade física. Idosos com um baixo nível de atividade física (OR = 3,49 [2,0 - 6,13]), com idade igual ou superior a 75 anos (OR = 1,97 [1,03 - 3,72]), vivendo sem parceiro (OR = 2,01 [1,09 - 3,68]), dificuldade de viver sem um parceiro (OR = 2,01 [1,09 - 3,68]), dificuldades com atividades básicas (OR = 2,49 [1,45 - 4,28]) e as atividades instrumentais (OR = 2.28) [1.18 - 4.36]) atividades da vida do dia a dia e multimobilidade (OR = 2,06 [1,04 - 4,08]) foram associadas independentemente à mobilidade. Um baixo nível de atividade física aumenta a possibilidade de limitação da mobilidade em adultos idosos, independentemente das variáveis sociodemográficas e clínicas.

ABSTRACT BACKGROUND: Multimorbidity due to non-communicable chronic diseases (NCDs) constitutes a significant challenge for healthcare systems. To attenuate its impacts, it is essential to identify the sociodemographic determinants of this condition, which can discriminate against population segments that are more exposed. OBJECTIVE: To identify associations between multimorbidity conditions and sociodemographic indicators among Brazilian adults and older adults. DESIGN AND SETTING: Cross-sectional telephone-based survey in 26 Brazilian state capitals and the federal district. METHODS: The Vigitel 2013 survey was used, with data collected via a questionnaire. The outcome was multimorbidity (2, 3 or 4 NCDs), and the exposures were sociodemographic indicators (age, sex, skin color, marital status and education). The analysis consisted of multinomial logistic regression (odds ratio), stratified by age. RESULTS: Among adults, multimorbidity comprising two, three or four diseases was associated with advancing age (P < 0.001); two and three diseases, with having a partner (P = 0.004 and P < 0.001, respectively); and two, three or four diseases, with lower education (P < 0.001). Among older adults, two, three or four diseases were associated with female sex (P < 0.001); three diseases, with living with a partner (P = 0.018); two diseases, with black skin color (P = 0.016); and two or three diseases, with lower education (P < 0.001). CONCLUSIONS: To control and prevent multimorbidity, strategies for individuals with existing chronic diseases, with partners and with lower education levels are needed. Particularly for adults, advancing age should be considered; and for older adults, being a woman and having black skin color.

Resumo: O comportamento sedentário emerge como um importante determinante da saúde da pessoa idosa, no entanto, sua relação com a multimorbidade e seus padrões de acometimento em estudos epidemiológicos têm sido pouco explorados na população brasileira. Tais associações poderão auxiliar na elaboração de políticas públicas visando à modificação desse comportamento. Assim, o objetivo deste estudo foi avaliar a associação entre comportamento sedentário com multimorbidade e seus padrões em idosos brasileiros. Estudo transversal, com 43.554 idosos da Pesquisa Nacional de Saúde de 2019. O comportamento sedentário autorreferido foi categorizado em < 3; 3-6; e > 6 horas por dia. A presença de multimorbidade e seus padrões foram analisados pelo autorrelato da coexistência de duas ou mais doenças crônicas, sendo que os três padrões estabelecidos consideraram as doenças com características clínicas similares: (1) cardiopulmonar; (2) vascular-metabólico; e (3) mental-musculoesquelético. As associações foram realizadas pela regressão logística binária. Os idosos que despendiam 3-6 horas/dia em comportamento sedentário tiveram maiores chances (OR = 1,39; IC95%: 1,29; 1,50) de apresentar multimorbidade, padrão vascular-metabólico (OR = 1,39; IC95%: 1,29; 1,50) e mental-musculoesquelético (OR = 1,11; IC95%: 1,00; 1,24). Quando o comportamento sedentário foi > 6 horas/dia, houve maiores chances de multimorbidade (OR = 1,58; IC95%: 1,43; 1,74) e dos padrões cardiopulmonar (OR = 1,73; IC95%: 1,33; 2,27), vascular-metabólico (OR = 1,49; IC95%: 1,35; 1,64) e mental-musculoesquelético (OR = 1,15; IC95%: 1,01; 1,31), quando comparados àqueles que ficavam até 3 horas/dia. Dessa forma, evidenciou-se que tempos elevados em comportamento sedentário são fatores de risco relevantes para a ocorrência de multimorbidade e seus padrões em idosos.
Abstract: Sedentary behavior emerges as an important determinant of health in elderly persons, but its relationship to multimorbidity and its patterns in the Brazilian population have received little attention in epidemiological studies. Such associations can assist the elaboration of public policies aimed at modification of this behavior. This study thus aimed to assess the association between sedentary behavior and multimorbidity and its patterns in elderly Brazilians. This was a cross-sectional study of 43,554 elderly Brazilians in the Brazilian National Health Survey, 2019. Self-reported SB was categorized as < 3; 3-6; and > 6 hours a day. The presence of multimorbidity and its patterns were analyzed by self-report of two or more chronic diseases, where the three established patterns considered the diseases with similar clinical characteristics: (1) cardiopulmonary; (2) vascular-metabolic; and (3) mental-musculoskeletal. The associations were measured by binary logistic regression. Elderly persons that spent 3-6 hours/day in sedentary behavior had lower odds (OR = 1.39; 95%CI: 1.29; 1.50) of presenting multimorbidity, vascular-metabolic pattern (OR = 1.39; 95%CI: 1.29; 1.50), and mental-musculoskeletal pattern (OR = 1.11; 95%CI: 1.00; 1.24). Sedentary behavior > 6 hours/day was associated with higher odds of multimorbidity (OR = 1.58; 95%CI: 1.43; 1.74) and the cardiopulmonary (OR = 1.73; 95%CI: 1.33; 2.27), vascular-metabolic (OR = 1.49; 95%CI: 1.35; 1.64), and mental-musculoskeletal (OR = 1.15; 95%CI: 1.01; 1.31) patterns when compared to elders with 3 hours/day of sedentary behavior. This showed that extensive time in sedentary behavior is a relevant risk factor for multimorbidity and its patterns in the elderly.
Resumen: El comportamiento sedentario emerge como un importante determinante de salud en personas ancianas, no obstante, su relación con la multimorbilidad y los patrones de enfrentamiento a la misma en estudios epidemiológicos se han investigado poco en la población brasileña. Tales asociaciones podrán apoyar en la elaboración de políticas públicas, con vistas a la modificación de ese comportamiento. Por ello, el objetivo de este estudio fue evaluar la asociación entre comportamiento sedentario con multimorbilidad y sus patrones en ancianos brasileños. Es un estudio transversal, con 43.554 ancianos de la Encuesta Nacional de Salud de 2019. El comportamiento sedentario autoinformado se categorizó en < 3; 3-6; y > 6 horas por día. La presencia de multimorbilidad y sus patrones se analizaron mediante el autoinforme de la coexistencia de dos o más enfermedades crónicas, siendo que los tres patrones establecidos consideraron las enfermedades con características clínicas similares: (1) cardiopulmonar; (2) vascular-metabólica y (3) mental-musculoesquelética. Las asociaciones fueron realizadas mediante regresión logística binaria. Los ancianos que pasaban 3-6h/día en comportamiento sedentario tuvieron mayores oportunidades (OR = 1,39; IC95%: 1,29; 1,50) de presentar multimorbilidad, patrón vascular-metabólico (OR = 1,39; IC95%: 1,29; 1,50) y mental-musculoesquelético (OR = 1,11; IC95%: 1,00; 1,24). Cuando el comportamiento sedentario fue > 6 horas/día hubo mayores oportunidades de multimorbilidad (OR = 1,58; IC95%: 1,43; 1,74), y de los patrones cardiopulmonar (OR = 1,73; IC95%: 1,33; 2,27), vascular-metabólico (OR = 1,49; IC95%: 1,35; 1,64) y mental-musculoesquelético (OR = 1,15; IC95%: 1,01; 1,31), cuando se compara con aquellos que estaban hasta 3 horas/día. De esa forma, se evidenció que tiempos elevados en comportamiento sedentario se mostraron factores de riesgo relevantes para la ocurrencia de multimorbilidad y sus patrones en ancianos.

Objective: To explore the relationship between obesity status and death stratified by different multi-morbidity status in older adults in China. Methods: Data for older Chinese adults aged ≥65 years were from Chinese Longitudinal Healthy Longevity Survey (CLHLS). Multi-morbidity patterns based on 13 chronic conditions were explored using exploratory factor analysis. Cox models were used to examine relationships between obesity status and death stratified by disease count and multi-morbidity patterns at baseline, respectively. Besides, obesity status was defined by baseline body mass index and waist circumference. Results: A total of 6 272 participants were included in the analyses. Multi-morbidity including cardio-metabolic, sensory perception and other patterns were identified. For those without any chronic condition, compared with those without central obesity, central obesity was associated with a higher risk for death (HR=1.66, 95%CI:1.04-2.66). For those only with one chronic condition, compared with normal weight, underweight was associated with a higher risk for death (HR=1.41, 95%CI: 1.10-1.80). For those with multi-morbidity, compared with normal weight, underweight increased the risk for death (HR=1.19, 95%CI:1.05-1.34). Compared with those without central obesity, central obesity decreased the risk for death (HR=0.88, 95%CI:0.78-0.99). Conclusions: Relationships between obesity status and death varied by multi-morbidity status in older adults in China. Underweight and non-central obesity were associated with increased risks for death in older adults with only one chronic disease or multi-morbidity. Therefore, it is necessary to pay attention to multi-morbidity status in the management of obesity in older adults and provide effective targeted body weight management plan.

Objective: To assess the prevalence, pattern and outcome of multimorbidity in elderly patients with acute coronary syndrome (ACS). Methods: Secondary analysis was performed based on the data from the BleeMACS registry, which was conducted between 2003 and 2014. We stratified elderly patients (≥65 years) according to their multimorbidity. Multimorbidity was defined as two or more chronic diseases in the same individual. Kaplan-Meier methods were used to estimate 1 year event rates for each endpoint, and comparisons between the study groups were performed using the log-rank test. The primary endpoint was net adverse clinical events (NACE), which is a composite of all-cause mortality, myocardial infarction, or bleeding. Results: Of 7 120 evaluable patients, 6 391 (89.8%) were with morbidity (1 594 with 1, 2 156 with 2, and 2 641 with ≥3 morbidity). Patients with morbidity were older, percent of female sex and non-ST-elevation acute coronary syndromes and implantation rate with drug-eluting stents and blood creatine level were higher compared to patients without morbidity. Compared with the patients without morbidity, the proportion of participants with oral anticoagulant increased in proportion to increased number of morbidities (5.8% vs. 6.4% with 1 morbidity, 7.3% with 2 morbidities, 9.0% with ≥3 morbidities, P trend<0.01) and the proportion of participants with clopidogrel prescription decreased in proportion to increased number of morbidity (91.9% vs. 89.7% with 1 morbidity, 87.9% with 2 morbidities, 88.6% with ≥3 morbidities, P trend = 0.01). During 1 year follow-up, compared with those with no morbidity, the hazard ratio (HR) and 95% confidence interval (CI) of risk of NACE for those with 1, 2, and ≥ 3 morbidities was 1.18 (0.86-1.64), 1.49 (1.10-2.02), and 2.74 (2.06-3.66), respectively (P < 0.01). Multimorbidity was not associated with an increased risk of bleeding of various organs (P>0.05). Conclusion: Multimorbidity is common in elderly patients with ACS. These patients might benefit from coordinated and integrated multimorbidity management by multidisciplinary teams.

Objective To clarify the hotspots and trends of multimorbidity research and to provide evidence for further research in China. Methods Papers on multimorbidity were retrieved from PubMed and Web of Science (from inception to August 11,2021).BICOMB and gCLUTO were used for bibliometric and clustering analysis,and CiteSpace was employed for analysis of authors and citations,and burst detection of keywords. Results The research on multimorbidity has been on the rise.Among the authors,Mercer SW published the most papers on this topic and Fortin M was the most cited author.Karolinska Institute topped the institutions in the number of published papers,and the paper published in Lancet by Barnett K in 2012 was the most cited.A total of 75 high-frequency keywords were extracted,on the basis of which seven research hotspots were summarized:epidemiology (including the prevalence and trend),medication (involving polypharmacy,medication compliance,etc.),medical expenditure (including cost and medical services),aging (such as elderly patients,frailty,and disability),psychology (involving mental health,social support,etc.),multimorbidity management (such as the treatment,primary health care,and integrated care),and comorbidity of cardiovascular and metabolic diseases (involving obesity,stroke,diabetes,etc.). Conclusions Multimorbidity is concerned as a major health threat and public health problem worldwide.The management of multimorbidity is more complex than that of one disease,which thus faces more challenges.Therefore,researchers,health care providers,and policy-makers should underscore it.

BACKGROUND@#Few studies have assessed the relationship between multimorbidity patterns and mortality risk in the Chinese population. We aimed to identify multimorbidity patterns and examined the associations of multimorbidity patterns and the number of chronic diseases with the risk of mortality among Chinese middle-aged and older adults.@*METHODS@#We used data from the China Kadoorie Biobank and included 512,723 participants aged 30 to 79 years. Multimorbidity was defined as the presence of two or more of the 15 chronic diseases collected by self-report or physical examination at baseline. Multimorbidity patterns were identified using hierarchical cluster analysis. Cox regression was used to estimate the associations of multimorbidity patterns and the number of chronic diseases with all-cause and cause-specific mortality.@*RESULTS@#Overall, 15.8% of participants had multimorbidity. The prevalence of multimorbidity increased with age and was higher in urban than rural participants. Four multimorbidity patterns were identified, including cardiometabolic multimorbidity (diabetes, coronary heart disease, stroke, and hypertension), respiratory multimorbidity (tuberculosis, asthma, and chronic obstructive pulmonary disease), gastrointestinal and hepatorenal multimorbidity (gallstone disease, chronic kidney disease, cirrhosis, peptic ulcer, and cancer), and mental and arthritis multimorbidity (neurasthenia, psychiatric disorder, and rheumatoid arthritis). During a median of 10.8 years of follow-up, 49,371 deaths occurred. Compared with participants without multimorbidity, cardiometabolic multimorbidity (hazard ratios [HR] = 2.20, 95% confidence intervals [CI]: 2.14 - 2.26) and respiratory multimorbidity (HR = 2.13, 95% CI:1.97 - 2.31) demonstrated relatively higher risks of mortality, followed by gastrointestinal and hepatorenal multimorbidity (HR = 1.33, 95% CI:1.22 - 1.46). The mortality risk increased by 36% (HR = 1.36, 95% CI: 1.35 - 1.37) with every additional disease.@*CONCLUSION@#Cardiometabolic multimorbidity and respiratory multimorbidity posed the highest threat on mortality risk and deserved particular attention in Chinese adults.

ABSTRACT OBJECTIVE To estimate the prevalence of multimorbidity in older adults in São Paulo, Brazil. METHODS A cross-sectional study based on the 2015 ISA-Capital population-based survey, with a subsample of 1,019 older adults aged ≥ 60 years old. Multimorbidity was categorized considering two or more chronic diseases, based on a previously defined list. The data were analyzed in univariate and multiple models with Poisson regression. RESULTS The prevalence of multimorbidity was 40% (95%CI: 36.6-43.8), being higher in women (PR a = 1.95 [compared to men]; 95%CI: 1.58-2.40), in individuals aged ≥ 75 years old (PR a = 1.25 [compared to individuals aged ≥ 60 to 64 years old]; 95%CI: 1.01-1.60), in Black people (PR a = 1.28 [compared to White people]; 95%CI: 1.04-1.59), in high-income people (PR a = 1.27 [compared to low income]; 95%CI: 1.09-1.50) and in former smokers (PR a = 1.30 [compared to those who never smoked]; 95%CI: 1.05-1.60), and lower in smokers (PR a = 0.72 [compared to those who never smoked]; 95%CI: 1.09-1.50). CONCLUSION The prevalence of multimorbidity was lower than that reported in most of the reviewed studies, but consistently associated with gender, age, race/skin color, smoking habit and socioeconomic status. The standardization of conceptual and methodological criteria for estimation is a challenge to relieve problems in the planning and management of health care systems for older populations.
RESUMO OBJETIVO Estimar a prevalência de multimorbidade em idosos em São Paulo, Brasil. MÉTODOS Estudo transversal, aninhado ao inquérito de base populacional ISA-Capital, de 2015, com sub amostra de 1.019 idosos com 60 anos ou mais. A multimorbidade foi categorizada considerando duas ou mais doenças crônicas, a partir de uma lista previamente definida. Os dados foram analisados em modelos univariado e múltiplo com a regressão de Poisson. RESULTADOS A prevalência de multimorbidade foi de 40% (IC95% 36,6-43,8), sendo maior nas mulheres (RP a = 1,95 [comparado com homens]; IC95% 1,58-2,40), nos indivíduos com 75 anos ou mais (RP a = 1,25 [comparado com indivíduos de 60 a 64 anos]; IC95% 1,01-1,60), nos pretos (RP a = 1,28 [comparado com brancos]; IC95% 1,04-1,59), nas pessoas de alta renda (RP a = 1,27 [comparado com baixa renda]; IC95% 1,09-1,50) e nos ex-fumantes (RP a = 1,30 [comparado com quem nunca fumou]; IC95% 1,05-1,60) e menor nos que se declararam fumantes (RP a = 0,72 [comparado com quem nunca fumou]; IC95% 1,09-1,50). CONCLUSÕES A prevalência de multimorbidade foi inferior à reportada na maioria dos estudos revisados, mas há consistência sobre sua associação com sexo, idade, cor da pele, tabagismo e nível socioeconômico. A padronização de critérios conceituais e metodológicos para a sua estimação é um desafio para mitigar problemas no planejamento e gestão de sistemas de saúde para populações cada vez mais envelhecidas.

A multimorbidade tem sido cada vez mais comum em todo o mundo. O acesso a serviços de saúde eficazes e equitativos que atendam às necessidades das doenças crônicas não-transmissíveis (DCNT) ainda é limitado em muitos países. Nesse sentido, o presente estudo teve por objetivo identificar a relação entre a multimorbidade e a utilização de diferentes serviços de saúde e avaliar a associação entre fatores contextuais e individuais e o uso de serviços de saúde em indivíduos com multimorbidade no Brasil de 1998 a 2013. Trata-se de um estudo em painéis utilizando dados do suplemento de saúde da Pesquisa Nacional de Amostras por Domicílios de 1998, 2003 e 2008 e dados da Pesquisa Nacional de Saúde realizada em 2013. A população de estudo foi composta por adultos com idade maior ou igual a 18 anos e classificados como tendo multimorbidade (presença de dois ou mais problemas crônicos em um mesmo indivíduo). Foram considerados três desfechos de utilização de serviços de saúde: busca por serviços de saúde nos últimos quinze dias, consulta médica nos últimos doze meses e hospitalizações nos últimos doze meses. Modelos de regressão de Poisson estratificados por sexo foram utilizados para estimar as razões de prevalência brutas e ajustadas e seus respectivos intervalos de confiança de 95% para cada desfecho de uso de serviços de saúde e multimorbidade, por ano. Modelos de regressão de Poisson multinível em dois níveis foram utilizados para estimar as razões de prevalência ajustadas por variáveis independentes individuais e contextuais e seus respectivos intervalos de confiança a 95%, para os três desfechos, por ano de painel. Houve aumento da prevalência de procura por serviços de saúde e consultas médicas nos últimos 12 meses entre 1998 e 2013, independentemente da classificação de multimorbidade. A prevalência de hospitalizações diminuiu ao longo do período do estudo e foi duas vezes maior em indivíduos com multimorbidade. Ter multimorbidade aumentou a utilização de serviços de saúde para os três desfechos em estudo, sendo mais expressivo entre os homens. Fatores individuais relacionados ao uso de serviços de saúde se modificaram segundo tipo de serviço de saúde investigado. Características contextuais de vulnerabilidade social, organização dos serviços de saúde e índices de saúde mostraram influência na busca por serviços de saúde e nas hospitalizações, não apresentando associação com consultas médicas. Este estudo constatou que indivíduos com multimorbidade apresentam níveis mais elevados de utilização de serviços de saúde. Além disso, revelou que o uso de serviços de saúde está associado de forma variável com os fatores predisponentes, capacitantes e de necessidade em pessoas com multimorbidade. Esses resultados apontam para a necessidade de integrar todos os fatores individuais e contextuais no planejamento do acesso à saúde e, assim, reduzir as desigualdades no acesso aos serviços de saúde nessa população.
Background: Multimorbidity has been increasingly common throughout the world. Access to effective and equitable health services that address the needs of chronic non-communicable diseases (NCDs) is still limited in many countries. In this sense, the present study aimed to identify the relationship between multimorbidity and the use of different health services and to assess the association between contextual and individual factors and the use of health services in individuals with multimorbidity in Brazil from 1998 to 2013. Methods: This is a panel study using data from the health supplement of 1998, 2003, and 2008 National Survey of Samples by Households and data from the National Health Survey conducted in 2013. The study population consisted of adults of age 18 years of age or older and classified as having multimorbidity (presence of two or more chronic problems in the same individual). Three health service utilization outcomes were considered: search for health services in the last fifteen days, medical consultation in the last twelve months, and hospitalisations in the last twelve months. Poisson regression models stratified by sex were used to estimate the crude and adjusted prevalence ratios and their respective 95% confidence intervals for each health service use and multimorbidity outcome per year. Two-level multilevel Poisson regression models were used to estimate the prevalence ratios adjusted for individual and contextual independent variables and their respective 95% confidence intervals, for the three outcomes, by panel year. Results: There was an increase in the prevalence of demand for health services and medical consultations in the last 12 months between 1998 and 2013, regardless of the multimorbidity classification. The prevalence of hospitalisations decreased over the study period and was twice as high in individuals with multimorbidity. Having multimorbidity increased the use of health services for the three outcomes under study, being more expressive among men. Individual factors related to the use of health services changed according to the type of health service investigated. Contextual characteristics of social vulnerability, organization of health services, and health indices influenced the search for health services and hospitalisations, with no association with medical appointments. Conclusion: This study found that individuals with multimorbidity have higher levels of use of health services. Furthermore, it revealed that the use of health services is variably associated with predisposing, enabling, and need factors in people with multimorbidity. These results point to the need to integrate all individual and contextual factors in planning access to health and, thus, reduce inequalities in access to health services in this population.

Métodos: Estudo transversal da linha de base de um estudo de intervenção. Foram realizados exames bioquímicos, antropométricos, hemodinâmicos e entrevistas, entre outubro de 2018 e março de 2019, em Vitória. Testes estatísticos apropriados, conforme o delineamento da amostra, foram realizados utilizando-se o software SPSS versão 21.0, e adotando-se p<0,05. Resultados: Foram avaliados 262 ACS com idade média de 46,1±9,3 anos. Observaram-se elevados percentuais de pré-diabetes (22,9%), diabetes mellitus (17,2%), hipertensão arterial (37,0%), obesidade (39,8%), hipercolesterolemia (57,3%), hipertrigliceridemia (27,1%), multimorbidade (40,8%), sedentarismo (60,9%) e uso de ansiolíticos/antidepressivos (22,5%). Cerca de 40% dos ACS apresentaram três ou mais morbidades. Conclusão: Foram observados elevados percentuais de doenças crônicas, multimorbidade, sedentarismo e uso de ansiolíticos/antidepressivos em ACS de Vitória.

Métodos: Estudio transversal de la línea de base de un estudio de intervención. Se realizaron clínicos y entrevistas entre octubre de 2018 y marzo de 2019 en Vitória. Se realizaron las pruebas estadísticas adecuadas utilizando el software SPSS versión 21.0, adoptando p<0,05. Resultados: Se evaluaron 262 ACS, con una edad promedio de 46,1±9,3 años. Hubo altas prevalencias de prediabetes (22,9%), diabetes mellitus (17,2%), hipertensión arterial (37,0%), obesidad (39,8%), hipercolesterolemia (57,3%), hipertrigliceridemia (27,1%), multimorbilidad (40,8%), inactividad física (60,9%) y uso de ansiolíticos/antidepresivos (22,5%). Aproximadamente el 40% tenía tres o más enfermedades. Conclusión: Se observaron altos porcentajes de enfermedades crónicas, multimorbilidad, sedentarismo y uso de ansiolíticos/antidepresivos en los ACS de Vitória.
Objective: To assess health conditions, health care and lifestyle habits of community health workers (CHW) in Vitória, Espírito Santo, Brazil. Methods: This was a cross-sectional study using baseline data from an intervention study. Biochemical, anthropometric and hemodynamic examinations and interviews were carried out between October 2018 and March 2019 in Vitória. Appropriate statistical tests, in accordance with the sample design, were performed using SPSS software version 21.0, adopting p<0,05. Results: We assessed 262 CHWs with a mean age of 46.1±9.3 years. High prevalence of prediabetes (22.9%), diabetes mellitus (17.2%), hypertension (37.0%), obesity (39.8%), hypercholesterolemia (57.3%), hypertriglyceridemia (27.1%), multimorbidity (40.8%), physical inactivity (60.9%) and use of anxiolytics/antidepressants (22.5%) was found. Some 40% of the CHWs had three or more morbidities. Conclusion: High percentages of chronic diseases, multimorbidity, sedentary lifestyle and use of anxiolytics/antidepressants were found in CHWs in Vitória.

Background and objectives: multimorbidity can generate disabilities, when associated with work, it affects the reduction of participation in the workforce, job turnover and early retirement. Thus, the objective was to estimate the prevalence and identify factors associated with multimorbidity in higher education institution workers. Method: this is a cross-sectional, quantitative study carried out with workers from a higher education institution (n=629) in the city of Ponta Grossa, PR, Brazil. For data collection, questionnaires with instruments from the Ministry of Health were used. The dependent variable referred to the presence of multimorbidity, and the independent variable referred to sociodemographic and work characteristics, use of health services, self-perceived health, presence of symptoms, lifestyle and eating habits. Chi-square test and logistic regression were performed. Results: the prevalence of multimorbidity was 53%, and it was associated with age (OR=2.99), overweight (OR=1.77), pain (OR=4.54), self-rated general health (OR=2.08) and self-rated oral health (OR=2.30) (p<0.05). Individuals with multimorbidity seek more follow-up by a Basic Health Unit (OR=0.54) and perform routine medical consultations more frequently (OR=0.83) (p<0.05). Conclusion: a high prevalence of multimorbidity was observed in the assessed workers, with a statistical association with biological factors, lifestyle, self-perceived health and access to health services. In this way, it is possible to outline strategies aimed at reestablishing workers' health, improving their quality of life.(AU)
Justificativa e Objetivo: a multimorbidade pode gerar incapacidades, quando associada ao trabalho, afeta a redução da participação na força de trabalho, rotatividade de empregos e aposentadoria precoce. Assim, objetivou-se estimar a prevalência e identificar fatores associados à multimorbidade em trabalhadores de instituições de ensino superior. Método: trata-se de um estudo transversal, quantitativo, realizado com trabalhadores de uma instituição de ensino superior (n=629) da cidade de Ponta Grossa, PR, Brasil. Para coleta de dados, foram utilizados questionários com instrumentos do Ministério da Saúde. A variável dependente referiu-se à presença de multimorbidade e a variável independente referiu-se às características sociodemográficas e laborais, utilização de serviços de saúde, autopercepção de saúde, presença de sintomas, estilo de vida e hábitos alimentares. Teste do qui-quadrado e regressão logística foram realizados. Resultados: a prevalência de multimorbidade foi de 53% e esteve associada à idade (OR=2,99), excesso de peso (OR=1,77), dor (OR=4,54), autoavaliação geral de saúde (OR=2,08) e -avaliação da saúde bucal (OR=2,30) (p<0,05). Indivíduos com multimorbidade procuram mais acompanhamento em Unidade Básica de Saúde (OR=0,54) e realizam consultas médicas de rotina com maior frequência (OR=0,83) (p<0,05). Conclusão: observou-se alta prevalência de multimorbidade nos trabalhadores avaliados, com associação estatística com fatores biológicos, estilo de vida, autopercepção de saúde e acesso aos serviços de saúde. Dessa forma, é possível traçar estratégias voltadas ao restabelecimento da saúde do trabalhador, melhorando sua qualidade de vida.(AU)
Justificación y Objetivo: la multimorbilidad puede generar discapacidades, cuando asociada al trabajo incide en la reducción de la participación laboral, la rotación laboral y la jubilación anticipada. Así, el objetivo fue estimar la prevalencia e identificar factores asociados a la multimorbilidad en trabajadores de instituciones de educación superior. Método: se trata de un estudio transversal, cuantitativo, realizado con trabajadores de una institución de enseñanza superior (n=629) de la ciudad de Ponta Grossa, PR, Brasil. Para la recolección de datos, se utilizaron cuestionarios con instrumentos del Ministerio de Salud. La variable dependiente se refirió a la presencia de multimorbilidad, y la variable independiente a las características sociodemográficas y laborales, uso de servicios de salud, autopercepción de salud, presencia de síntomas, estilo de vida y hábitos alimentarios. Se realizaron pruebas de chi-cuadrado y regresión logística. Resultados: la prevalencia de multimorbilidad fue del 53% y se asoció con la edad (OR=2,99), el sobrepeso (OR=1,77), el dolor (OR=4,54), la autopercepción de salud general (OR=2,08) y la autopercepción. salud bucal valorada (OR=2,30) (p<0,05). Los individuos con multimorbilidad buscan más seguimiento por una Unidad Básica de Salud (OR=0,54) y realizan consultas médicas de rutina con mayor frecuencia (OR=0,83) (p<0,05). Conclusión: se observó una alta prevalencia de multimorbilidad en los trabajadores evaluados, con asociación estadística con factores biológicos, estilo de vida, autopercepción de salud y acceso a los servicios de salud. De esta forma, es posible delinear estrategias encaminadas a restablecer la salud de los trabajadores, mejorando su calidad de vida.(AU)

ABSTRACT BACKGROUND: Associations between behaviors and individual chronic diseases have been demonstrated. However, the relationship between time spent on sedentary behavior and multimorbidity remains less clear. OBJECTIVE: To identify the predictive power of various intensities of physical activity versus sedentary behavior, as discriminatory factors for cardiometabolic multimorbidity (cardiovascular diseases and diabetes) in the elderly. DESIGN AND SETTING: Cross-sectional study in different residential census tracts and residential households in Florianópolis (SC). METHODS: The participants were 425 elderly people (65% women) from the EpiFloripa Aging study in 2014. Sociodemographic variables and self-reported incidence of cardiovascular diseases and diabetes were obtained via a questionnaire. Light physical activity (LPA), moderate-to-vigorous physical activity (MVPA) and sedentary behavior (SB) were measured using accelerometers. The analyses were stratified according to sex and included a diagnosis for interpretation. Behaviors were taken into consideration if their predictive power in terms of area under the receiver operating characteristic (ROC) curve was greater than 0.50. The time cutoff point was defined from sensitivity and specificity. RESULTS: For older adult men with diabetes, the predictive value of MVPA for absence of multi-morbidity was an area of 0.75 (95% confidence interval, CI: 0.538-0.962), and a cutoff of 17 minutes per day. Older adult women with diabetes had an area of 0.71 (95% CI: 0.524-0.866) and a cutoff of 10 minutes per day. LPA and SB did not present predictive values. CONCLUSION: The time spent on MVPA is a predictor of absence of multimorbidity in elderly people with diabetes, for both sexes.

Abstract The objective was to identify the impact of social distance in the management of noncommunicable diseases (NCD) in the adult population from the state of Rio Grande do Sul, southern Brazil. This is an ambispective, population-based cohort study. Descriptive analysis and Poisson regression models were used and the results were reported as prevalence ratio and 95% confidence intervals. From a total of 1,288 participants, 43.1% needed medical care and 28.5% reported impaired management of NCDs during social distance. Female sex, age between 18 and 30 years old, living in the Serra region (central region of the state), people with depression and multimorbidity were more likely to have impaired management of NCDs. Being physically active reduced the probability of having impaired management of NCD by 15%. Reduced monthly income was associated with the difficulty in accessing prescription medicine and avoidance of seeking in-person medical assistance. Depression was associated with difficulties in accessing medications, while avoidance of seeking in-person medical assistance was more likely for people with multimorbidity, arthritis/arthrosis/fibromyalgia, heart disease, and high cholesterol.
Resumo O objetivo foi identificar o impacto do distanciamento social no manejo das doenças crônicas não transmissíveis (DCNT) na população adulta do estado do Rio Grande do Sul, sul do Brasil. Este é um estudo de coorte ambispectivo de base populacional. Foi utilizada análise descritiva e modelos de regressão de Poisson, os resultados são apresentados em razão de prevalência e intervalos de confiança de 95%. De um total de 1.288 participantes, 43,1% necessitaram de cuidados médicos e 28,5% relataram manejo prejudicado de DCNT durante o distanciamento social. Sexo feminino, idade entre 18 e 30 anos, residente na região da Serra (região central do estado), pessoas com depressão e multimorbidade apresentaram maior chance de manejo prejudicado das DCNT. Ser fisicamente ativo reduziu a probabilidade de ter gerenciamento prejudicado de DCNT em 15%. A redução da renda mensal esteve associada à dificuldade de acesso a medicamentos prescritos e a deixar de buscar atendimento médico presencial. A depressão foi associada a dificuldades de acesso a medicamentos, enquanto deixar de buscar assistência médica pessoalmente foi mais provável para pessoas com multimorbidade, artrite/artrose/fibromialgia, doenças cardíacas e colesterol alto.

INTRODUCTION: Aging is a physiological process associated to decreased functional capacity and the presence of diseases, especially chronic noncommunicable diseases. OBJECTIVE: To analyze the prevalence and factors associated to the multimorbidity of elderly in a teaching hospital, according to sociodemographic characteristics, lifestyle and use of services in the health care network. METHODS: Descriptive and inferential cross-sectional study, conducted with 144 hospitalized patients 60 years of age or more, from January to June 2018, in a university hospital in the state of Paraná. Data were collected 30 days after hospital discharge by telephone interview. The dependent variable was the occurrence of multimorbidity and the independent variables were: sociodemographic characteristics, lifestyle and use of hospital health services. Logistic regression analysis was performed. RESULTS: It was found that 55% of the elderly had multimorbidity. The elderly who were more chances to have multimorbidity where there low education (OR=16.29; CI:2.75-96.42), non-white (OR=2.34; CI:1.005.50) hospitalized longer (4 to 7 days: OR=6.91; CI:2.40-19.96; more than 7 days: OR=3.03; CI:1.00-9.22), who scheduled to return to the hospital (OR=18.99; CI: 1.30-277.87), and that after discharge they needed help from someone to follow the medical recommendations (OR=3.16; CI:1.38-7.22). CONCLUSION: It was identified a high prevalence of multimorbidity, and important factors associated to multimorbidity in hospitalized elderly, with emphasis on education; color; hospitalization time; scheduling of return to hospital after discharge; need for help from someone (family member/caregiver), without post-discharge, to follow medical recommendations.
INTRODUÇÃO: O envelhecimento é um processo fisiológico associado à diminuição da capacidade funcional e presença de doenças com destaque para as crônicas não transmissíveis. OBJETIVO: Analisar a prevalência e os fatores associados à multimorbidade de idosos internados em um hospital de ensino, segundo características sociodemográficas, estilo de vida e utilização de serviços na rede de atenção à saúde. MÉTODO: Estudo transversal, quantitativo, desenvolvidos junto a 144 pacientes com 60 anos ou mais internados, no período de janeiro a junho de 2018, em um hospital universitário do estado do Paraná. Os dados foram coletados 30 dias após a alta hospitalar por entrevista telefônica. Considerou-se como variável dependente a ocorrência de multimorbidade e como independentes: características sociodemográficas, estilo de vida e utilização de serviços de saúde hospitalares. Realizou-se análise de regressão logística. RESULTADOS: Verificou-se que 55% dos idosos apresentaram multimorbidade. Apresentaram maiores chances de dispor de multimorbidade idosos analfabetos e com menos de 6 anos de estudo (OR=16,29; IC:2,75-96,42), de cor não branca (OR=2,34; IC:1,00-5,50), que ficaram maior tempo internados (4 a 7 dias: OR=6,91; IC:2,40-19,96; mais de 7 dias: OR=3,03; IC:1,00-9,22), que agendaram retorno ao hospital (OR=18,99; IC:1,30-277,87), e que pós alta precisaram de ajuda de alguém para seguir as recomendações médicas (OR=3,16; IC:1,38-7,22). CONCLUSÃO: Identificou alta prevalência de multimorbidade e importantes fatores associados à multimorbidade em idosos internados, com destaque para escolaridade; cor; tempo de internação; agendamento de retorno ao hospital após a alta, e; necessidade de ajuda de alguém, no pós alta, para seguir as recomendações médicas.

ABSTRACT Background: Multimorbidity is common among adults and associated with socioeconomic deprivation, polypharmacy, poor quality of life, functional impairment, and mortality. Objectives: To identify the frequency of multimorbidity among older adults inpatients with neurological disorders (NDs), stratify clusters of chronic comorbidities associated with NDs in degrees, and verify whether multimorbidity was associated with demographic data, readmission, long length of hospital stay (LOS), and hospital mortality in this population. Methods: We enrolled patients aged ≥60 years successively admitted to a tertiary medical center with NDs between January 1, 2009, and December 31, 2010. Results: Overall, 1,154 NDs and 2,679 comorbidities were identified among 798 inpatients aged ≥60 years (mean: 75.76±9.12). Women comprised 435 (54.51%) of patients. Multimorbidity was detected in 92.61% (739) of patients, with a mean of 3.88±1.67 (median: 4.0), ranging from 2 to 10 chronic diseases. Patients with epilepsy, dementia, and movement disorders had the highest degrees of clusters of chronic morbidities (>50% of them with ≥5 chronic disorders), followed by those with cerebrovascular and neuromuscular disorders. Multimorbidity was associated with long LOS (p<0.001) and readmission (p=0.039), but not with hospital mortality (p=0.999). Conclusions: Multimorbidity was preponderant among older adults inpatients with NDs, and NDs had a high degree of associated chronic comorbidities. Multimorbidity, but not isolated NDs, was associated with readmission and long LOS. These results support ward-based, neurohospitalist-directed, interdisciplinary care for older adults inpatients with NDs to face multimorbidity.
RESUMO Introdução: A multimorbidade é comum entre idosos e está associada a privação socioeconômica, polifarmácia, má qualidade de vida, déficit funcional e mortalidade. Objetivos: Identificar a frequência da multimorbidade entre pacientes idosos hospitalizados com doenças neurológicas (DN), estratificar combinações de comorbidades crônicas associadas às DN em graus e verificar se a multimorbidade foi associada a dados demográficos, readmissão, longo tempo de internação (TDI) e mortalidade hospitalar nessa população. Métodos: Foram incluídos pacientes com ≥60 anos sucessivamente admitidos com DN em um centro médico terciário entre 1º de janeiro de 2009 e 31 de dezembro de 2010. Resultados: Um total de 1.154 DN e 2.679 comorbidades foram identificados entre 798 pacientes com idade ≥60 anos (média: 75,76±9,12). Mulheres representaram 435 (54,51%) dos pacientes. A multimorbidade foi detectada em 92,61% (739) dos pacientes (média de 3,88±1,67; mediana de 4), variando de 2 a 10 doenças crônicas. Pacientes com epilepsia, demência e distúrbios do movimento apresentaram os maiores graus de morbidades crônicas (>50% deles com ≥5 doenças crônicas), seguidos por doenças cerebrovasculares e neuromusculares. A multimorbidade foi associada com longo TDI (p<0,001) e readmissão (p=0,039), mas não com mortalidade hospitalar (p=0,999). Conclusões: A multimorbidade foi preponderante entre os pacientes idosos internados com DN, as quais tiveram altos graus de comorbidades crônicas. A multimorbidade, mas não as DN isoladas, foi associada a readmissões e longo TDI. Esses resultados respaldam uma assistência interdisciplinar para idosos hospitalizados com DN em enfermarias lideradas por neurologistas hospitalistas para enfrentar a multimorbidade.

Introdução: A ocorrência de multimorbidade, a qual corresponde ao acúmulo de doenças crônicas, é considerada um problema de saúde pública e suas consequências na saúde bucal carecem de evidências científicas em grandes populações. Objetivo: Avaliar o impacto da multimorbidade nas condições de saúde bucal em idosos brasileiros. Método: Realizou-se um estudo transversal e de base populacional, utilizando a base de dados da Pesquisa Nacional de Saúde. A presença de multimorbidade em idosos foi considerada quando o mesmo possuía um diagnóstico de duas ou mais doenças crônicas. Com relação às condições de saúde bucal, essas foram coletadas a partir das variáveis estudadas na referida pesquisa. O teste Qui-quadrado foi utilizado para a análise dos dados e em seguida as razões de prevalência foram ajustadas por meio da regressão múltipla de Poisson. Resultados: Participaram 11.697 idosos e, desses, 53,1% possuíam multimorbidade. Na análise multivariada, observou-se que a presença de multimorbidade em idosos predispôs a um relato negativo da autopercepção de saúde bucal (p=0,025), à dificuldade de se alimentar devido a problemas dentários (pï¼0,001), a perder totalmente os dentes superiores (pï¼0,001) e a escovar os dentes ou próteses pelo menos uma vez ao dia (p=0,025). Conclusão:Diante dos resultados, conclui-se que a maioria das condições de saúde bucal, assim como a autopercepção das mesmas apresentam-se piores na presença de multimorbidade na população idosa (AU).
Introduction:The occurrence of multimorbidity, which corresponds to the accumulation of chronic diseases, is considered a public health problem and its consequences on oral health lack scientific evidence in large populations.Objective:To evaluate the impact of multimorbidity on oral health conditions in elderly Brazilians.Methods:A cross-sectional and population-based study was carried out, using the database of the National Health Survey. The presence of multimorbidity in the elderly was considered when they had adiagnosis of two or more chronic diseases. Regarding oral health conditions, these were collected from the variables studied in the National Health Survey. The Chi-square test was used for data analysis and then the prevalence ratios were adjusted using Poisson multiple regression.Results:11,697 elderly people participated and, of these, 53.1% had multimorbidity. In the multivariate analysis, it was observed that the presence of multimorbidity in the elderly predisposed to a negative report of self-perceived oral health (p=0.025), the difficulty of eating due to dental problems (pï¼0.001), to lose teeth completely (pï¼0.001)and brushing teeth or dentures at least once a day (p=0.025). Conclusions:In view of the results, it is concluded that most oral health conditions, as well as their self-perception, are worse in the presence of multimorbidity in the elderly population (AU).
Introducción:La aparición de multimorbilidad, que corresponde a la acumulación de enfermedades crónicas, se considera un problema de salud pública y sus consecuencias sobre la salud bucalcarecen de evidencia científica en grandes poblaciones. Objetivo:Evaluar el impacto de la multimorbilidad en las condiciones de salud bucal en los brasileños de edad avanzada. Método:Se realizó un estudio transversal y basado en la población, utilizando la base de datos de la Encuesta Nacional de Salud. Se consideró la presencia de multimorbilidad en los ancianos cuando tenían un diagnóstico de dos o más enfermedades crónicas. En cuanto a las condiciones de salud bucal, se obtuvieron de las variables estudiadas en el Encuesta Nacional de Salud. La prueba de Chi-cuadrado se usó para el análisis de datos y luego las razones de prevalencia se ajustaron mediante regresión múltiple de Poisson. Resultados:Participaron 11.697 personas mayores y, de estos, 53.1% tenían multimorbilidad. En el análisis multivariante, se observó que la presencia de multimorbilidad en los ancianos predispone a un informe negativo de autopercepción de la salud bucal (p=0,025), la dificultad de comer debido a problemas dentales (pï¼0,001), para perder los dientes por completo (pï¼0,001) y cepillarse los dientes o las dentaduras postizas al menos una vez al día (p=0,025).Conclusiones:En vista de los resultados, se concluye que la mayoría de las afecciones de salud bucal, así como su autopercepción, son peores en presencia de multimorbilidad en la población de edad avanzada (AU).
