ABSTRACT

Objetivo:identificar os saberes de enfermeiras sobre icterícia neonatal. Método:pesquisa-ação, qualitativa, realizada em uma maternidade pública no Paraná. Nove enfermeiras participaram na etapa de diagnóstico, foram realizadas entrevistas semiestruturadas e análise de conteúdo. Nas etapas de ação e avaliação, participaram 49 profissionais, ocorreu observação participante, entrevista em grupo e análise de conteúdo. Resultados: emergiram três categorias: saberes de enfermeiras para identificação da icterícia neonatal; saberes para manejo adequado; saberes sobre as consequências da icterícia não tratada. Foi identificado pouco conhecimento sobre a fisiopatologia da doença, cuidados específicos e consequências da icterícia inadequadamente tratada. Foi desenvolvida uma capacitação sobre o tema em conjunto com a instituição. Conclusões: na primeira etapa, as enfermeiras demonstraram saber avaliar e identificar precocemente a icterícia, mas tinham pouco conhecimento sobre cuidados com a fototerapia e consequências do tratamento inadequado. São necessárias ações de educação permanente para qualificar a assistência aos recém-nascidos com icterícia


Objective: to identify the knowledge of nurses about neonatal jaundice. Method: qualitative action research conducted in a public maternity hospital in Paraná. Nine nurses participated in the diagnostic stage, semi-structured interviews and content analysis were performed. In the action and evaluation stages, 49 professionals participated, and participant observation, group interviews, and content analysis were conducted. Results: three categories emerged: knowledge of nurses for the identification of neonatal jaundice; knowledge for proper management; knowledge about the consequences of untreated jaundice. Limited knowledge was identified regarding the pathophysiology of the disease, specific care, and consequences of inadequately treated jaundice. Training on the subject was developed in collaboration with the institution. Conclusions: in the first stage, nurses demonstrated the ability to evaluate and identify jaundice early, but had limited knowledge about care with phototherapy and the consequences of inadequate treatment. Continuous education actions are necessary to improve the care for newborns with jaundice.


Objetivo: identificar los conocimientos de las enfermeras sobre la ictericia neonatal. Método: investigación-acción cualitativa, en una maternidad pública en Paraná. Participaron nueve enfermeras en el diagnóstico, mediante entrevistas semiestructuradas y análisis de contenido. En las etapas de acción y evaluación, participaron 49 profesionales, con observación participante, entrevistas grupales y análisis de contenido. Resultados: surgieron tres categorías: conocimientos sobre la identificación, manejo adecuado y consecuencias de la ictericia neonatal no tratada. Se identificó poco conocimiento sobre la fisiopatología, cuidados específicos y consecuencias de la ictericia no tratada adecuadamente. Se desarrolló una capacitación en conjunto con la institución. Conclusiones: en la primera etapa, las enfermeras demostraron saber evaluar e identificar precozmente la ictericia,pero tenían poco conocimiento sobre los cuidados con la fototerapia y las consecuencias del tratamiento inadecuado. Se necesitan acciones de educación continua para mejorar la atención a los recién nacidos con ictericia

INTRODUCTION: Hospitals may not be able to manage the chaotic environment during pandemics through classical management principles. Management can only be achieved with the help of effective leadership, appropriate flow of crisis management systems, teamwork, and collaboration. Nurse managers should have effective management skills. OBJECTIVE: To evaluate the perception of the nurse managers' practices by nurses such as resource management, organization, communication, psychosocial, care-training, and decision-making practices in the management of the pandemic based on the COVID-19 pandemic. METHODS: A quantitative, cross-sectional method was used, and a web-based online questionnaire was applied to collect data. This study included 182 nurses working in a public and private hospital in Istanbul. RESULTS: The nurses perceived the management practices of their managers at a moderate level. The leaders' resource management practices as low (16.5%), and medium (78.6%) perceive. There was a statistically significant difference (p<0.01) between the communication and team relations (p<0.05), resource management (p<0.01), care-training (p<0.05), and decision-making scores (p<0.01) of the nurses working in the private hospitals compared to those working in the public hospital. CONCLUSION: To effectively manage a complex and chaotic environment, nurse managers should trust their team, have speed and adaptation skills, be based on human factor principles, and provide a safe working environment. Education should be emphasized so that nurses can work in specialized areas and nurse managers can improve their management skills.

ABSTRACT

sO cenário advindo como consequência da pandemia de covid-19 acarretou diversas mudanças na vida dos profissionais de saúde. A percepção desse grupo torna-se indispensável para a compreensão do contexto, visto que esses trabalhadores são figuras essenciais no enfrentamento da doença. O objetivo deste estudo foi analisar a percepção de gestores e profissionais que atuaram durante a primeira onda da pandemia de covid-19 no município de Sobral (CE). Trata-se de uma pesquisa exploratório-descritiva, com abordagem qualitativa, feita por meio da aplicação de um instrumento de coleta de dados. O estudo foi realizado com gestores e trabalhadores que atuaram de março a dezembro de 2020 no município. A coleta foi feita no formato híbrido, com auxílio da plataforma Google Meet, fundamentada na metodologia snowball sampling e no método de saturação. Após a coleta, os discursos foram transcritos ­ utilizando a ferramenta Microsoft Word ­ e analisados ­ com suporte do software NVivo11. Foram contactados 68 profissionais. Identificou-se predominância final de enfermeiros, sexo feminino e atuantes na atenção terciária. Após análise e categorização, identificaram-se 12 unidades de registro. Foram dispostos os principais desafios e dificuldades acarretados para os processos de trabalho e suas subcategorias: a sobrecarga de trabalho e o desgaste físico; a reorganização dos serviços em resposta à nova realidade; o desconhecimento sobre a doença e seu impacto psicológico no fazer profissional; e o fator social e a dificuldade de seguimento da assistência na atenção primária. Por fim, conclui-se que os desafios identificados se inter-relacionam, repercutindo uns nos outros, e constata-se a importância de conhecer esses entraves para ações mais bem direcionadas e estratégicas.


The scenario that has arisen in response to COVID-19 has led to a variety of changes in the lives of healthcare providers. The perception of this group is essential to understanding this context since they are key figures in coping with the disease. This study aimed to analyze the perception of managers and professionals who worked during the first wave of the COVID-19 pandemic in the municipality of Sobral/CE. This is an exploratory-descriptive study with a qualitative approach that applied a collection instrument with managers and workers who worked from March to December 2020 in the municipality. Data were collected using the Snowball Smapling and the saturation method in a hybrid format using the Google Meet platform. After the collection, responses were transcribed using Microsoft Word and analyzed with the aid of NVivo11. In total, 68 professionals were contacted, with a final predominance of female nurses working in tertiary care. After analysis and categorization, 12 recording units were identified. The main challenges and difficulties for work processes and their subcategories include work overload and physical exhaustion; reorganization of services in response to the new reality; lack of knowledge about the disease and its psychological impact on professional practice; and the social factor and the difficulty of following up care in primary care. Finally, we conclude that the identified challenges are interrelated and have repercussions on each other, and that it is important to understand these obstacles to take more targeted and strategic actions.


El escenario que ha surgido en respuesta a la pandemia de la COVID-19 ha provocado diversos cambios en la vida de los profesionales de la salud. La percepción de este grupo es indispensable para comprender el contexto, ya que estos trabajadores son figuras esenciales en el enfrentamiento a la enfermedad. El objetivo de este estudio fue analizar la percepción de gestores y profesionales que actuaron durante la primera ola de la pandemia de la COVID-19 en el municipio de Sobral (Ceará, Brasil). Se trata de un estudio exploratorio-descriptivo, con enfoque cualitativo, realizado mediante la aplicación de una herramienta de recolección de datos. En esta investigación participaron gestores y trabajadores que actuaron de marzo a diciembre de 2020 en el municipio. Se realizó la recolección en formato híbrido, con ayuda de la plataforma Google Meet, basada en el método bola de nieve y en el método de saturación. Después, se transcribieron los discursos, utilizando la herramienta Microsoft Word, y se analizaron con el apoyo del software NVivo11. Se contactaron 68 profesionales. Se identificó predominio de enfermeros, mujeres y trabajadores de atención terciaria. Después del análisis y de la categorización, se identificaron 12 unidades de registro. Se presentaron los principales retos y dificultades que generan los procesos de trabajo y sus subcategorías: la sobrecarga de trabajo y el agotamiento físico; la reorganización de los servicios en respuesta a la nueva realidad; el desconocimiento de la enfermedad y su impacto psicológico en la práctica profesional; y el factor social y la dificultad de seguimiento de los cuidados en atención primaria. Finalmente, se concluye que los retos identificados están interrelacionados, impactándose entre sí, y se señala la importancia de conocer estos obstáculos para emprender acciones más específicas y estratégicas.

ABSTRACT

Objetivo: Caracterizar os aspectos trabalhistas das enfermeiras da Atenção Primária à Saúde nos municípios de tipologia urbana do estado da Bahia. Métodos: Estudo descritivo através de abordagem quantitativa a partir de informações obtidas com 32 enfermeiras, no período entre novembro de 2020 a maio de 2021, nos três maiores municípios do estado da Bahia. Resultados: Enfermeiras em sua maioria possuem vínculo trabalhista do tipo estatutário, com carga horária de 40 horas, atua há mais de um ano em unidades de saúde de modelo da Estratégia Saúde da Família, com diferenças salariais entre os municípios. Conclusão: Identificar aspectos trabalhistas das enfermeiras é de suma relevância, pois trazem implicações para melhoria das políticas de pessoal na Atenção Primária à Saúde. (AU)


Objective: To characterize the labor aspects of Primary Health Care nurses in urban municipalities of the state of Bahia. Methods: A descriptive study through a quantitative approach based on information obtained from 32 nurses, in the period between November 2020 and May 2021, in the 3 largest municipalities of the state of Bahia. Results: Most of the nurses have a statutory employment contract, work 40-hour shifts, and have been working for more than 1 year in health care units that follow the Family Health Strategy model, with wage differences among the municipalities. Conclusion: Identifying labor aspects of nurses is of utmost relevance, since they bring implications for improving personnel policies in Primary Health Care. (AU)


Objetivo: Caracterizar los aspectos laborales de las enfermeras de Atención Primaria de Salud en los municipios urbanos del estado de Bahia. Métodos: Estudio descriptivo a través de un enfoque cuantitativo basado en la información obtenida de 32 enfermeras, en el período comprendido entre noviembre de 2020 y mayo de 2021, en los tres mayores municipios del estado de Bahia. Resultados: La mayoría de las enfermeras tienen un contrato de trabajo legal, trabajan 40 horas y llevan más de 1 año en unidades sanitarias del modelo de la Estrategia de Salud Familiar, con diferencias salariales entre municipios. Conclusión: La identificación de los aspectos laborales de las enfermeras es de máxima relevancia, ya que aportan implicaciones para la mejora de las políticas de personal en Atención Primaria. ABSTRACT

Introduction: Primary health care is a strategy that involves the different health professionals and other agents in the system who can influence the social determinants that affect human wellbeing. Objective: To know the meanings about primary health care of nursing professionals practicing in this field in a municipality of Colombia. Materials and Methods: Qualitative, exploratory, descriptive study. The research was conducted in a State Social Company (ESE for its acronym in Spanish) that provides primary health care services. Data was collected through semi-structured interviews. The sample consisted of 13 professionals. IRaMuTeQ software was used to help data interpretation. Results: 'Nurse' was the most frequent word. The content analyzed was categorized into four classes: 'Meanings about primary health care', 'nursing activities in primary health care', 'barriers encountered in primary health care', 'primary health care organization and work team'. Conclusions: Primary health care meaning is understood as the entry point of users into the health system and the one responsible for coordinating their passage through other points of the healthcare network according to users' health needs. In addition, primary health care is understood as the level of care at which essential health actions focused on the early detection of health risks and complications are carried out, so that health interventions can be planned, implemented, and evaluated on that basis.

ABSTRACT

Background and Objective@#Professionals in the Philippines must finish compulsory Continuing Professional Develop-ment (CPD) units before renewing their registration ID card with the Professional Regulation Commission (PRC). CPD has always been significant in the field of nursing. It supports nurses and other healthcare professionals, and keeps them informed about their professional development so they may provide the best care possible for patients. This study sought to engage how nurses in Bontoc, Mountain Province would benefit in continuing professional development in terms of their professional growth, increase in income, network building, and improvement of personal capabilities.@*Methods@#This study used a quantitative descriptive study design using simple random probability sampling method. Descriptive analysis was used to characterize study participants. A modified validated survey tool was used to collect data on the nurses’ perceived CPD benefits. Data collection was done among 101 nurses employed in Bontoc General Hospital, Mountain Province State Polytechnic College, Bontoc Municipal Health Office, the Department of Education (DepEd) at Bontoc, from January to February 2022 through paper and pencil method. Nurses who did not consent to participate were excluded from the study. The UNP Ethics Review Committee reviewed the protocol and did not find any ethical inappropriateness with number 22-039. An F-test and a t-test were also conducted to determine the mean differences in perceived CPD benefits according to participants’ demographic characteristics. All these statistical analysis steps were performed using the IBM Statistical Package for the Social Sciences (SPSS) version 20.@*Results@#The majority of the 101 respondents were between the ages of 30 and 39, and had 1 to 5 years of service. The overall mean of perceived CPD benefits was very high (3.40-3.58 mean rating), with career development having the highest mean rating at 3.58. This only implies that the respondents had a positive perception of CPD engagement, particularly with the favorable impact on their career development. T-test and F-test (ANOVA) were both pivotal in reaching the objectives of this study. Across the demographic characteristics of the respondents such as age, civil status, sex, highest educational attainment, length of service CPD units earned, and membership to organization, there was no significant difference found in the level of perceived benefits of engaging in continuing professional development across demographic characteristics. Each demographic factors gained a p-value greater than 0.05. Statistically, a p-value greater than 5% level of significance would result to failing to reject the null hypothesis.@*Conclusion@#Given the foregoing results, the number of credit units obtained can be used to forecast how CPD will help nurses grow in their careers. To reap significant advantages and increases in career mobility and income, as well as, to some extent, the development of professional networks and personal capacities, nurses should take advantage of expanding best practice involvement and pertinent CPD engagements. It has been discovered that CPD resources, activities, and programs have an impact on professional development. It is recommended that there be support from both the government and employers to promote CPD for nurses throughout their entire nursing careers. Earning CPD units can help nurses build their confidence and elevate their status in the workplace, increasing their chances of getting promoted.

@#<strong>BACKGROUND</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The capability of nurse educators to teach and practice quality improvement (QI) is crucial in assisting students in their transition to becoming professionals. Understanding the strengths and limitations of nurse educators on QI can be a logical start to determine if they can keep up with the expectations.</p><strong>OBJECTIVES</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">This paper aimed to determine the nurse educators' knowledge, preparation, application, and participation in QI, as well as the differences when grouped according to years of teaching and BSN degree completion.</p><strong>METHODS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">A descriptive correlational design was utilized involving nurse educators from six nursing schools in Baguio City, Philippines. Only full-time nurse educators with official teaching load during the data gathering, regardless of academic background and position, were eligible. Faculty members who were on leave were excluded. A self-made tool (CVI 0.90, Cronbach's alpha 0.90) was used to gather data. Unpaired t-tests and ANOVA were used to determine the significant differences in the scores. Multiple regression was utilized to compute the relationship between knowledge, preparation, and participation in applying QI.</p><strong>RESULTS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">104 nurse educators responded. Results show that they are knowledgeable (x̄= 15.82; SD = 0.11), somewhat prepared (x̄= 2.93; SD = 0.08), and participated reasonably well (x̄= 2.77; SD = 0.11) on QI. Higher scores were given to the application of QI in the nursing courses (x̄= 2.44; SD = 0.08) and teaching-learning strategies (x̄= 0.83; SD = 0.07), compared with its application in improving Self as educators (x̄= 2.30; SD = 0.11), and improving student's learning outcomes (x̄= 2.13; SD = 0.11). Scores of nurse educators with more than ten years of teaching experience significantly differed in the extent of knowledge, application in teaching-learning strategies, and application of QI tools to improve Self and participation compared to those with less teaching experience. The scores did not significantly vary when grouped according to BSN degree completion. A positive relationship was observed between preparation and the application of QI tools to improve Self (p = 0.00). Data also showed a positive relationship between participation with application in nursing courses (p = 0.00), application of QI tools to improve Self (p = 0.00), and student learning outcomes. (p = 0.00).</p><strong>CONCLUSION</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The results of this study are encouraging and show the potential of nurse educators to apply quality improvement in the nursing curriculum. The adept use of technological tools in producing QI projects can augment the strength of nurse educators in meeting patient-centered care. Teaching experience can contribute to a better grasp of the concepts and maximum integration of quality improvement in nursing students' learning. Identifying, mobilizing, and supporting QI champions to spearhead the mentoring of new faculty members on QI may be a viable strategy to sustain a culture that values quality improvement. It further calls for the attention of educational institutions to develop policies to inform nurse educators in applying QI concepts.</p>

@#<p style="text-align: justify;" data-mce-style="text-align: justify;">Caring for ourselves mirrors our caring for others. This mirrored care theory serves as the embodiment of a nurse possessing a selfcare nursing competency capable of mirroring competent patient care. The key concept of the theory is self-care nursing competency. Nursing has been established within this theory to help nurses achieve patient care competencies and impact the nursing profession. Self-care nursing competency contains three domains: self-awareness, self-regarding behavior, and self love. Moreover, Hawthorne nurses are the nurse managers and leaders who must integrate and apply the self-care nursing competency and the concept of the mirrored care theory in the nursing curricula, policies, and programs. These managers and leaders should constantly monitor and appraise the system. Consequently, the mirrored care theory shall rationalize the inclusion of self-care nursing competency in the nursing scope and standards of practice. When nurses possess self-care nursing competency, they are more competent in caring for others since they develop a sense of purpose, decrease experiences of burnout and dissatisfaction, and increase their morale. With this, the quality of care given by the nurses to themselves reflects the quality of care given to their clients. Hence, the mirrored care theory shall serve as a basis of caring not only for the patients but for the nurses as well.<br></p>

@#<strong>PURPOSE</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The importance of culture for ensuring safe and high-quality nursing care cannot be overstated. However, despite the increase in nurse migration, the concept of acculturation in nursing has not been well defined. This study aimed to elucidate the concept of acculturation in immigrant nurses.</p><strong>DESIGN</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">The defining attributes of the concept were established using Walker and Avant's 8-step method. This article is due to the dissertation of the PhD degree course approved and defended at the Faculty of Nursing, Baqiyatallah University of Medical Sciences. In this study, the researcher started collecting data after receiving the Code of Ethics (IR.BMSU.REC.1401.114) from the Iran National Committee for Ethics in Biomedical Research. The researcher provided the participants with contact information and was always ready to answer their research queries.</p><strong>METHODS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">Without any time constraints, a search was conducted on databases including Scopus, Pub Med, Web of Science, SID, Magiran, Irandoc, and Oxford Medical Dictionary using the keywords 'acculturation' and 'immigrant nurses'. Atotal of 18 articles were analyzed based on the inclusion criteria.</p><strong>RESULTS</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">Immigrant nurses experience acculturation through two main attributes. One-way acculturation involves the impact of social values, norms, customs, and habits on the nurses. Two-way acculturation refers to the cultural exchange between the nurse and the new environment.</p><strong>CONCLUSION</strong><p style="text-align: justify;" data-mce-style="text-align: justify;">According to the study, immigrant nurses experience acculturation as being impacted by various aspects such as beliefs, values, norms, behaviors, and habits.</p>

ABSTRACT

O Sistema Único de Saúde tem enfrentado grandes desafios relacionados à formação, qualificação e organização do processo de trabalho dos profissionais de saúde. Neste momento, com a incorporação da Estratégia e-SUS Atenção Primária à Saúde, alinhada à Estratégia de Saúde Digital do Brasil, já em um patamar de cobertura bastante avançada de municípios com prontuário eletrônico, vivenciamos um cenário de qualificação permanente no uso de tecnologias digitais e uma frente promissora no enfrentamento dos obstáculos para prover saúde para todos e fixar profissionais em áreas mais afastadas dos grandes centros urbanos e nas regiões de maior vulnerabilidade social. No entanto, prover recursos tecnológicos não basta; é preciso estar atento aos profissionais que, de fato, fazem a transformação digital acontecer. Nesse sentido, a Secretaria de Atenção Primária à Saúde coordena uma agenda positiva e apresenta um projeto de retomada da construção do SUS com base na inovação tecnológica centrada nas pessoas. E, dentre as diversas estratégias, implanta um projeto nacional de educação permanente em saúde digital, que potencializa o uso qualificado, ético e responsável do Sistema e-SUS APS e da estratégia como um todo. O Educa e-SUS APS oferece trajetórias formativas para a adequação do perfil profissional e técnico às demandas e necessidades do cuidado primário, mediado por tecnologia e fortalecido pelo conjunto de inovações que compõem a saúde digital. Acreditamos que, quando capacitada e motivada, a força de trabalho da APS será capaz de transformar os mais diversos cenários do cuidado em dados, analisar as informações geradas e utilizá-las no aprimoramento do cuidado à saúde das pessoas, da gestão e transparência de suas ações. Além disso, poderá contribuir para ampliar a cobertura e a resolutividade da diversa carteira de serviços da APS, com o emprego de recursos de comunicação à distância, incorporados aos protocolos de cuidado. É o comprometimento de cada um, aliado à educação permanente para consolidação das boas práticas em saúde digital, que trará mudanças significativas na tomada de decisão baseada em informação confiável e oportuna, fomentando as melhores soluções para enfrentar os grandes desafios da atenção à saúde no país.

ABSTRACT

Objetivo: identificar a percepção de gestantes quanto à atuação do enfermeiro no pré-natal. Método: pesquisa fenomenológica que entrevistou 10 gestantes de um município de Minas Gerais, Brasil. Os depoimentos foram organizados em categorias e analisados segundo a fenomenologia social de Alfred Schütz e literatura temática. Resultados: emergiram as categorias "Assistência centrada em ações técnicas", "Fragilidade na comunicação entre enfermeiro e gestante" e a expectativa de "orientações qualificadas desenvolvidas mediante atividades coletivas". Considerações finais: os resultados suscitam a necessidade de valorizar os aspectos subjetivos e comunicacionais no pré-natal, para além das ações técnicas, com vistas a efetivação do vínculo entre os atores, ampliação da adesão e da qualidade do pré-natal. Para tal, recomenda-se atividades de educação permanente


Objective: to identify the perception of pregnant women regarding the role of nurses in prenatal care. Method: phenomenological research that interviewed 10 pregnant women in a city in Minas Gerais, Brazil. The statements were organized into categories and analyzed according to Alfred Schütz's social phenomenology and thematic literature. Results: the categories "Care centered on technical actions", "Fragility in communication between nurse and pregnant woman" and the expectation of "qualified guidelines developed through collective activities" emerged. Final considerations: the results raise the need to value subjective and communicational aspects in prenatal care, in addition to technical actions, with a view to establishing the bond between actors, expanding adherence and quality of prenatal care. To this end, continuing education activities are recommended


Objetivos: identificar la percepción de las gestantes sobre el papel del enfermero en el prenatal. Método: investigación fenomenológica que entrevistó a 10 mujeres embarazadas en una ciudad de Minas Gerais, Brasil. Los enunciados fueron organizados en categorías y analizados según la fenomenología social y la literatura temática de Alfred Schütz. Resultados: surgieron las categorías "Cuidado centrado en acciones técnicas", "Fragilidad en la comunicación entre enfermero y gestante" y la expectativa de "orientaciones calificadas desarrolladas a través de actividades colectivas". Consideraciones finales: los resultados plantean la necesidad de valorar los aspectos subjetivos y comunicacionales en el prenatal, además de las acciones técnicas, con miras a establecer el vínculo entre los actores, ampliando la adherencia y la calidad del prenatal. ABSTRACT

Objective: to evaluate the patient safety climate in Primary Health Care from the perspective of nurses working in the services. Method: a quantitative and cross-sectional study conducted with 148 nurses from a municipality in the state of São Paulo. The Brazilian version of the Primary Care Safety Questionnaire Survey and personal, professional, and organizational performance variables (intention to stay at work, job satisfaction, care quality, and frequency of incidents) were used. Parametric and non-parametric comparison tests and Spearman's correlation coefficient were performed, considering a 5% significance level. Results: the safety climate was positive, varying from 4.52 to 5.33 and differing across districts for workload (p=0.0214) and leadership (p=0.0129). The safety climate professional variables and dimensions differed in relation to the frequency of incidents. Teamwork and safety and learning system were strongly correlated with job satisfaction and moderately with perceived care quality. Conclusion: teamwork and safety and learning system stood out for their positive correlations with job satisfaction and care quality. A positive safety climate favors the involvement of Primary Care nurses to develop improvement plans aligned with the National Patient Safety Program.


Objetivo: evaluar el clima de seguridad del paciente en la Atención Primaria de la Salud, desde la perspectiva de los enfermeros de los servicios. Método: estudio cuantitativo y transversal, con 148 enfermeros de un municipio del Estado de São Paulo. Se utilizó la versión brasileña del Primary Care Safety Questionnaire Survey y variables personales, profesionales y de desempeño organizacional (intención de permanecer en el empleo, satisfacción laboral, calidad de la atención y frecuencia de incidentes). Se realizaron pruebas de comparación paramétricas y no paramétricas y coeficiente de correlación de Spearman, considerando un nivel de significación del 5%. Resultados: el clima de seguridad fue positivo, osciló entre 4,52 y 5,33; difirió entre distritos en carga de trabajo (p=0,0214) y liderazgo (p=0,0129). Las variables profesionales y dimensiones del clima de seguridad difirieron en la frecuencia de incidentes. El trabajo en equipo, el sistema de seguridad y el aprendizaje tuvieron una correlación alta con la satisfacción laboral y una moderada con la percepción de la calidad de la atención. Conclusión: el trabajo en equipo y el sistema de seguridad y aprendizaje se destacaron por su correlación positiva con la satisfacción laboral y la calidad de la atención. Un clima de seguridad positivo favorece la participación de los enfermeros de la atención primaria en el desarrollo de planes de mejora que sigan las indicaciones del Programa Nacional de Seguridad del Paciente.

Conclusão: trabalho em equipe e sistema de segurança e aprendizagem se destacaram pela correlação positiva com satisfação no trabalho e qualidade do cuidado. O clima de segurança positivo favorece o envolvimento dos enfermeiros da atenção primária para desenvolver planos de melhorias alinhados ao Programa Nacional de Segurança do Paciente.

ABSTRACT

Objective: to evaluate the association between different forms of childhood trauma and postpartum depression in Brazilian puerperal women. Method: this cross-sectional survey included 253 puerperal women who were evaluated using the Edinburgh Postnatal Depression Scale and the Childhood Trauma Questionnaire. Multivariate logistic regression analyses were performed to verify the association of different types of trauma and the co-occurrence of forms of abuse and neglect with postpartum depression. Results: postpartum depression was identified in 93 women (36.8%; 95% Confidence Interval: 30.8-42.7). All forms of childhood trauma assessed (emotional abuse, emotional neglect, physical abuse, physical neglect and sexual abuse) were independently associated with postpartum depression after adjusting for confounding variables. Emotional abuse remained associated with postpartum depression when the co-occurrence of all forms of childhood trauma was analyzed. Conclusion: the results suggest an association between the different forms of childhood trauma and postpartum depression. In this sense, childhood trauma is an indicator for Nursing professionals to screen for risk factors of postpartum depression during obstetric_follow-up.


Objetivo: evaluar la asociación entre diferentes formas de traumas en la infancia y depresión posparto en puérperas brasileñas. Método: en este estudio transversal se incluyó a 253 puérperas que fueron evaluadas con la Edinburgh Postnatal Depression Scale y el Childhood Trauma Questionnaire . Se realizaron análisis de regresión logística multivariados para verificar la asociación de diferentes tipos de trauma y la coocurrencia de formas de abuso y negligencia con la depresión posparto. Resultados: se identificó depresión posparto en 93 mujeres (36,8%; intervalo de confianza del 95%: 30,8-42,7). Todas las formas de traumas en la infancia que se evaluaron (abuso emocional, negligencia emocional, abuso físico, negligencia física y abuso sexual) estuvieron asociadas de manera independiente con la depresión posparto después de ajustar por variables de confusión. El abuso emocional siguió estando asociado con la depresión posparto cuando se analizó la coocurrencia de todas las formas de traumas en la infancia. Conclusión: los resultados sugieren una asociación entre las diferentes formas de traumas en la infancia y la depresión posparto. En este sentido, los traumas en la infancia son un indicador para que los profesionales de Enfermería detecten factores de riesgo para la depresión posparto en los controles obstétricos.

ABSTRACT

Objective: the aim of this study is to investigate the relationship between death distress, psychological adjustment, optimism, pessimism and perceived stress among nurses working during the COVID-19 pandemic. Method: this study was designed as cross-sectional/cohort. The population of the study involved 408 nurses from Northern Cyprus, which are registered as full members of the Nurse Council. The sample comprised 214 nurses, who volunteered to participate in the study. The study data was collected using a web-based online survey (Demographic form, the Coronavirus Stress Measure, The Optimism and Pessimism Questionnaire, The Brief Adjustment Scale-6, The Death Distress Scale). Results: the results indicated that perceived stress significantly and negatively predicted optimism (β = -0.21, p < 0.001) and pessimism (β = 0.38, p < 0.001). Perceived stress had significant and positive predictive effects on psychological adjustment (β = 0.31, p < 0.001) and death distress (β = 0.17, p < 0.01). Further analysis results revealed that pessimism mediates the association of stress with psychological adjustment and death distress; however, optimism only mediates the effect of stress on psychological adjustment among nurses. Conclusion: a low level of pessimism is effective in strengthening nurses' psychological adjustment skills againt perceived stress and death distress. Nurses should consider behavioral strategies to help reduce the level of pessimism during periods such as pandemics.


Objetivo: el objetivo de este estudio fue investigar la relación entre la ansiedad ante la muerte, el ajuste psicológico, el optimismo, el pesimismo y el estrés percibido entre los profesionales de enfermería que trabajaron durante la pandemia de COVID-19. Método: este estudio se diseñó como un estudio transversal/de cohortes. La población del estudio incluía a 408 profesionales de enfermería del Chipre del Norte registrados como miembros de pleno derecho del Consejo de Enfermería. La muestra consistió en 214 profesionales de enfermería que se ofrecieron voluntariamente a participar en el estudio. Los datos del estudio se recogieron a través de una encuesta online basada en la web (formulario demográfico, Medida de Estrés por Coronavirus, Cuestionario de Optimismo y Pesimismo, Escala Breve de Adaptación-6 y Escala de Sufrimiento por Muerte). Resultados: los resultados indicaron que el estrés percibido predecía significativa y negativamente el optimismo (β = -0,21, p < 0,001) y pesimismo (β = 0,38, p < 0,001). El estrés percibido tuvo efectos predictivos significativos y positivos sobre el ajuste psicológico (β = 0,31, p < 0,001) y angustia de muerte (β = 0,17, p < 0,01). Los resultados de otros análisis revelaron que el pesimismo es mediador en la asociación del estrés con el ajuste psicológico y la angustia de muerte; sin embargo, el optimismo sólo es mediador del efecto del estrés sobre el ajuste psicológico entre los profesionales de enfermería. Conclusión: un bajo nivel de pesimismo es eficaz para reforzar las habilidades de ajuste psicológico de los profesionales de enfermería frente al estrés percibido y la angustia por la muerte. Los profesionales de enfermería deberían considerar estrategias conductuales para ayudar a reducir el nivel de pesimismo durante periodos como las pandemias.

ABSTRACT

Objective: to build and validate a simulation-based education roadmap on suicide prevention in the virtual environment. Method: methodological research subdivided into a development and validation stage. The roadmap was built using a previously drafted template based on international guidelines on good clinical simulation practices and scientific literature on suicide prevention in the virtual environment. For validation, the roadmap was validated by experts through self-application of an assessment form with answers based on "adequate, fair, and inadequate", with a field for suggestions. Descriptive statistics and the Content Validity Index (CVI≥0.8) were used. Results: nine experts took part in the study, the majority of whom were nurses (66.7%), female (55.6%), with an average age of 42.22 years. All the items in the roadmap met the acceptance criteria (CVI≥0.8). Conclusion: this study provides a useful roadmap for teaching suicide prevention in the virtual environment.


Objetivo: elaborar y validar un guion de enseñanza basada en la simulación sobre la prevención del suicidio en el entorno virtual. Método: investigación metodológica subdividida en etapa de elaboración y validación. La elaboración se realizó a partir de un template creado previamente y basado en lineamientos internacionales sobre buenas prácticas de simulación clínica y literatura científica sobre prevención del suicidio en el entorno virtual. Para la validación los especialistas validaron el guion mediante la autoaplicación de un formulario de evaluación cuyas respuestas son: "adecuado, regular e inadecuado", con un campo para sugerencias. Se utilizó estadística descriptiva e Índice de Validez de Contenido (IVC≥0,8). Resultados: participaron nueve especialistas, la mayoría enfermeros (66,7%), mujeres (55,6%), con una edad promedio de 42,22 años. Todos los elementos del guion cumplieron con el criterio de aceptación (IVC≥0,8). Conclusión: este estudio proporciona un guion útil para la enseñanza sobre la prevención del suicidio en el entorno virtual.

ABSTRACT

Objective: to synthesize the main scientific evidence available on the factors associated with workaholism in nurses' mental health. Method: this is an integrative review carried out in seven databases. The sample consisted of 11 studies. The Level of Evidence classification followed the model described by Melnyk and Fineout-Overholt. Methodological quality was assessed using the Checklist for Analytical Cross-Sectional Studies. Data analysis and synthesis were carried out in a qualitative and descriptive manner, respectively. Results: the factors associated with workaholism were burnout, stress, anxiety, depression, sleep-related problems, low ability to concentrate and negative incidents at work, which affected the mental health of nurses. Conclusion: the synthesis revealed that workaholism was related to perceived stress at work, emotional exhaustion, depersonalization and anxious and depressive symptoms, which resulted in low professional effectiveness and poor sleep quality among workaholic professionals.


Objetivo: sintetizar las principales evidencias científicas disponibles sobre los factores asociados al workaholism en la salud mental de enfermeros. Método: revisión integrativa realizada en siete bases de datos. La muestra estuvo compuesta por 11 estudios. La clasificación del Nivel de Evidencia siguió el modelo descrito por Melnyk y Fineout-Overholt. La calidad metodológica fue evaluada por medio del Checklist for Analytical Cross-Sectional Studies . El análisis y síntesis de los datos se realizaron de manera cualitativa y descriptiva, respectivamente. Resultados: los factores asociados al workaholism fueron burnout , estrés, ansiedad, depresión, problemas relacionados con el sueño, baja capacidad de concentración e incidentes negativos en el trabajo, los cuales afectaron la salud mental de los enfermeros. Conclusión: la síntesis reveló que el workaholism presentó relación con el estrés percibido en el trabajo, la exhaución emocional, la despersonalización y los síntomas ansiosos y depresivos, lo que resultó en baja eficacia profesional y mala calidad del sueño entre los profesionales adictos al trabajo.

ABSTRACT Objective: To evaluate the workload and severity of patients in the Intensive Care Unit (ICU) with COVID-19. Method: Cross-sectional, analytical study carried out in the ICU of a private hospital. All patients over the age of 18 with a diagnosis of COVID-19 admitted from September 2020 to June 2021 were included. Workload assessed by the Nursing Activities Score (NAS), and severity by the Sequential Organ Failure Assessment. Descriptive and inferential analyses were performed. Results: 217 patients were included, mostly men, mean age 62.41 years, white, obese, non-smokers and sedentary. The average NAS was 84.79. Staffing was in line with legislation and NAS. NAS was not associated with severity. Severity was associated with higher age, gender, comorbidities, sedentary lifestyle, time on mechanical ventilation, hospitalization and death. Conclusion: Workload was high and not associated with severity or outcomes. Severity was associated with demographic and clinical conditions. This study shows the importance of staff sizing, with a view to promoting safety and quality of care.


RESUMEN Objetivo: Evaluar la carga de trabajo y la gravedad de los pacientes de la Unidad de Cuidados Intensivos (UCI) con COVID-19. Método: Estudio transversal y analítico realizado en la UCI de un hospital privado. Se incluyeron todos los pacientes mayores de 18 años con diagnóstico de COVID-19 ingresados entre septiembre de 2020 y junio de 2021. Carga de trabajo evaluada mediante la Nursing Activities Score (NAS), y gravedad mediante la valoración secuencial de fallo orgánico. Se realizaron análisis descriptivos e inferenciales. Resultados: Se incluyeron 217 pacientes, en su mayoría hombres, edad media 62,41 años, raza blanca, obesos, no fumadores y sedentarios. El NAS medio era de 84,79. Los niveles de personal se ajustaban a la legislación y al NAS. El NAS no se asoció con la gravedad. La gravedad se asoció a mayor edad, sexo, comorbilidades, sedentarismo, tiempo de ventilación mecánica, hospitalización y muerte. Conclusión: La carga de trabajo fue elevada y no se asoció a la gravedad ni a los resultados. La gravedad se asoció a las condiciones demográficas y clínicas. Este estudio muestra la importancia del dimensionamiento del personal, con vistas a promover la seguridad y la calidad de los cuidados.

Incluídos todos os pacientes maiores de 18 anos, com diagnóstico de COVID-19 admitidos de setembro de 2020 a junho de 2021. Carga de trabalho avaliado pelo Nursing Activities Score (NAS), e gravidade pelo Sequential Organ Failure Assessment. Realizado análises descritiva e inferencial. Resultados: Incluídos 217 pacientes, maioria homens, média de idade 62,41 anos, brancos, obesos, não tabagistas e sedentários. A média do NAS foi 84,79. O dimensionamento de pessoal estava em concordância com legislação e NAS. O NAS não foi associado a gravidade. Houve associação da gravidade com maior idade, sexo, comorbidades, sedentarismo, tempo de ventilação mecânica, internação e óbito. Conclusão: A carga de trabalho foi alta e não associada a gravidade e desfechos. A gravidade foi associada às condições demográficas e clínicas. Este estudo mostra a importância do dimensionamento de pessoal, com vistas à promoção da segurança e qualidade assistencial.

Introducción: La pandemia de COVID-19 afectó a toda la población, incluido el personal de salud, como las enfermeras. Estas, al vivenciar la enfermedad, adquirieron valiosas experiencias que les permitieron resignificar su existencia como seres en el mundo. Objetivo: Comprender las vivencias de las enfermeras con COVID-19 aisladas en el entorno familiar. Métodos: Se realizó un estudio cualitativo-fenomenológico desde la perspectiva existencialista de Heidegger, participaron 13 enfermeras que laboraban en hospitales de Lima-Perú; durante los meses de octubre 2020 a junio de 2021. La muestra fue por "saturación teórica". Se aplicaron entrevistas en profundidad por vía virtual, se utilizaron preguntas orientadoras, los testimonios fueron grabados y transcritos previa autorización. Para el tratamiento de los datos, estos se sometieron al análisis temático de Braun y Clarke. Resultados: Luego de la transcripción, codificación y reducción fenomenológica de los discursos, se obtuvieron dos categorías: 1. El "cuidar de" y el "estar-con" experiencia del aislamiento en el entorno familiar, subcategorías: Reacción frente al diagnóstico y Contagio familiar. Categoría 2: El "ser ahí" y la angustia al vivenciar la enfermedad por COVID-19 Subcategorías: Lecciones de vida y Significado de la pandemia. Conclusiones: Las enfermeras con COVID-19, en aislamiento en su hogar, vivenciaron lecciones que las hicieron revalorar la vida, la salud y el amor de la familia, lo que ha fortalecido su compromiso de brindar un cuidado humanizado; se destacan la fortaleza personal y la esperanza como elementos que les permite enfrentar el temor, la desesperación y el miedo a la muerte. Entre más se conoce el sujeto a sí mismo más grande es su actitud de cuidado, por lo que el cuidado representa un modo de ser esencial de la enfermera(AU)


Introduction: The COVID-19 pandemic affected the entire population, including the health personnel, such as nurses. Nurses, by living through the disease, acquired valuable experiences that allowed them to redefine their existence as beings in the world. Objective: To understand the experiences of nurses with COVID-19 and isolated in the family environment. Methods: A qualitative-phenomenological study was conducted from the existentialist perspective of Heidegger. Thirteen nurses working in hospitals in Lima, Peru, participated during the months from October 2020 to June 2021. The sample was obtained by "theoretical saturation." In-depth interviews were conducted virtually, using guiding questions; and the testimonies were recorded and transcribed with prior authorization. For data processing, they were subjected to Braun and Clarke's thematic analysis. Results: After transcription, coding and phenomenological reduction of the discourses, two categories were obtained: 1. "Taking care of" and "Being with" in related to the isolation experience in the family environment, with the subcategories of reaction to the diagnosis and family contagion; and 2. "Being there" and distress when experiencing the disease by COVID-19, with the subcategories of life lessons and meaning of the pandemic. Conclusions: Nurses with COVID-19 and isolated at home experienced lessons that made them revalue life, health and family love, which has strengthened their commitment to providing humanized care; personal strength and hope stand out as elements that allow them to face fear, despair and fear of death. The more any subject knows themselves, the greater their caring attitude, so that caring represents a nurse's essential way of being(AU)
