Introducción. La adecuada sedación y analgesia es fundamental en el tratamiento de pacientes que requieren asistencia ventilatoria mecánica (AVM). Se recomienda la utilización de protocolos y su monitoreo; son dispares los resultados reportados sobre adhesión e impacto. Objetivos. Evaluar el impacto de la implementación de un protocolo de sedoanalgesia sobre el uso de benzodiacepinas, opioides y evolución en la unidad de cuidados intensivos pediátricos (UCIP), en pacientes que requieren AVM mayor a 72 horas. Métodos. Estudio tipo antes-después, no controlado, en la UCIP de un hospital pediátrico. Se desarrolló en 3 etapas: preintervención de diagnóstico situacional (de abril a septiembre de 2019), intervención y posintervención de implementación del protocolo de sedoanalgesia, educación sobre uso y monitorización de adherencia y su impacto (de octubre de 2019 a octubre de 2021). Resultados. Ingresaron al estudio 99 y 92 pacientes en las etapas pre- y posintervención, respectivamente. Presentaron mayor gravedad, menor edad y peso en el período preintervención. En la comparación de grupos, luego de ajustar por gravedad y edad, en la etapa posintervención se reportó una reducción en los días de uso de opioides en infusión continua (6 ± 5,2 vs. 7,6 ± 5,8; p = 0,018) y los días de uso de benzodiacepinas en infusión continua (3,3 ± 3,5 vs. 7,6 ± 6,8; p = 0,001). No se observaron diferencias significativas en los días de AVM y en los días totales de uso de benzodiacepinas. Conclusión. La implementación de un protocolo de sedoanalgesia permitió reducir el uso de fármacos en infusión continua.


Introduction. Adequate sedation and analgesia is essential in the management of patients requiring mechanical ventilation (MV). The implementation of protocols and their monitoring is recommended; mixed results on adherence and impact have been reported. Objectives. To assess the impact of the implementation of a sedation and analgesia protocol on the use of benzodiazepines, opioids, and evolution in the pediatric intensive care unit (PICU) in patients requiring MV for more than 72 hours. Methods. Before-and-after, uncontrolled study in the PICU of a children's hospital. The study was developed in 3 stages: pre-intervention for situational diagnosis (from April to September 2019), intervention, and post-intervention for implementation of a sedation and analgesia protocol, education on use, and monitoring of adherence and impact (from October 2019 to October 2021). Results. A total of 99 and 92 patients were included in the study in the pre- and post-intervention stages, respectively. Patients had a more severe condition, were younger, and had a lower weight in the preintervention period. After adjusting for severity and age, the group comparison in the post-intervention stage showed a reduction in days of continuous infusion of opioids (6 ± 5.2 versus 7.6­5.8, p = 0.018) and days of continuous infusion of benzodiazepines (3.3 ± 3.5 versus 7.6 ± 6.8, p = 0.001). No significant  differences were observed in days of MV and total days of benzodiazepine use. Conclusion. The implementation of a sedation and analgesia protocol resulted in a reduction in the use of continuous infusion of drugs.

Introducción: El manejo de los síntomas refractarios con dexmetomidina (DXM) es una alternativa en pacientes oncológicos en la atención al final de la vida (AFV) lo que puede convertirse en un gran desafío, ante la necesidad de iniciar sedación paliativa. El objetivo del presente estudio es determinar el efecto de la DXM comparado con el midazolam (MDZ) en el control de síntomas refractarios en pacientes con cáncer avanzado AFV, como parte de una estrategia asistencial de sedación paliativa. Metodología: Estudio observacional, longitudinal, se revisaron los informes médicos en las historias clínicas de pacientes hospitalizados con diagnóstico de cáncer avanzado terminal y síntomas refractarios (dolor, disnea y delirio), que recibieron sedación paliativa con DXM o MDZ. Se utilizaron los registros de Escala de Evaluación de Síntomas de Edmonton - revisada (ESAS-r) para intensidad de síntomas y la Escala de Sedación/Agitación de Richmond (RASS) para respuesta a la sedación. Resultados: Un total de 35 pacientes recibieron DXM y 18 MDZ. El dolor (32 %) y delirio (30 %) fueron los síntomas refractarios más identificados. Dolor intenso en 89.3 % antes del inicio de DXM. A las 72 horas, el grupo DXM presentó disminución de la intensidad del delirio a leve y moderado, mientras que la mayoría de los pacientes del grupo MDZ fallecieron antes de las 72 horas. Conclusiones: El uso clínico de la DXM permite un enfoque multimodal, ampliando su utilidad en la atención al final de la vida para el manejo del dolor y delirio, ya que permite un estado de respuesta para la interacción con los miembros de la familia, función relevante en cuidados paliativos.


Introduction: Managing refractory symptoms with dexmedetomidine (DXM) is an alternative for cancer patients in end-of-life care (EOLC), which can become a great challenge, given the need to start palliative sedation. This study aims to determine the effect of DXM compared to midazolam (MDZ) in controlling refractory symptoms in patients with advanced EOLC cancer as part of a palliative sedation care strategy. Methodology: In this observational, longitudinal study, the medical reports in the medical records of hospitalized patients diagnosed with terminal advanced cancer and refractory symptoms (pain, dyspnea, and delirium) who received palliative sedation with DXM or MDZ were reviewed. The Edmonton Symptom Assessment Scale-revised (ESAS-r) scores for symptom intensity and the Richmond Sedation/Agitation Scale (RASS) for response to sedation were used. Results: A total of 35 patients received DXM, and 18 received MDZ. Pain (32%) and delirium (30%) were the most identified refractory symptoms. Intense pain in 89.3% before DXM started; at 72 hours in 64.3%, the pain was reported as mild. Delirium was severe in 77.7%, DXM was used in 59.3%, and MDZ was used in 40.7%. At 72 hours, the DXM group presented a decrease in delirium intensity to mild and moderate, while most of the patients in the MDZ group died before 72 hours. Conclusions: The clinical use of DXM allows a multimodal approach, expanding its usefulness in end-of-life care for managing pain and delirium since it provides a state of response for interaction with family members, a function relevant in palliative care.

Introduction: Every year, millions of children and adolescents undergo surgery, 50%-75% of them experience fear and anxiety. Children are particularly susceptible to stress and anxiety surrounding surgery as a result of their cognitive development, previous experiences, and knowledge about healthcare; this leads to additional interventions to prevent and reduce these symptoms. Objective: To evaluate the effectiveness of family-centered educational interventions in the children's and adolescents' anxiety, pain, and behaviors and their parents' anxiety during the perioperative period. Methods: This review will follow the Joanna Briggs Institute guidelines for systematic reviews of effectiveness and will consider those studies (experimental and quasi-experimental) in which perioperative educational interventions have been applied to children and adolescents and their parents; these studies measured children and adolescents' pain, anxiety, and behaviors, as well as their parent's anxiety. An initial search of MEDLINE and CINAHL will be followed by a second search for published and unpublished studies from January 2007 on, available in English, Spanish and Portuguese. After all full texts are retrieved, the methodological quality assessment and data extraction will be independently and critically evaluated by two reviewers, and the data will then be presented in a tabular format. An explanatory synthesis will accompany the results. Whenever possible, a meta-analysis will be performed, and a Grading of Recommendations, Assessment, Development, and Evaluation Summary of Findings will be presented. Expected Results: This review will provide guidance on how family-centred educational interventions can be used as a resource to manage anxiety, pain, and behavior in children, adolescents and their relatives during the perioperative processes.


Introducción: Cada año, millones de personas menores y adolescentes se someten a cirugía, de las cuales entre el 50-75 % experimenta miedo y ansiedad. Las niñas y los niños son particularmente susceptibles al estrés y la ansiedad que rodea a la cirugía, como resultado de su desarrollo cognitivo, experiencias previas y conocimiento de la salud, lo que requiere intervenciones para prevenir y reducir estos síntomas. Objetivo: Esta revisión tiene como objetivo evaluar la efectividad de las intervenciones educativas familiares centradas en la ansiedad, el dolor y los comportamientos de las personas menores y adolescentes y de sus progenitores en el período perioperatorio. Métodos: Esta revisión seguirá las pautas del Instituto Joanna Briggs para revisiones sistemáticas de efectividad y considerará estudios experimentales y cuasiexperimentales en los que las intervenciones educativas perioperatorias para medir el dolor, la ansiedad y los comportamientos en niñas, niños y adolescentes y la ansiedad de sus progenitores. Se ha realizado una búsqueda inicial limitada de MEDLINE y CINAHL. Además, una segunda búsqueda de estudios publicados y no publicados de enero de 2007 disponibles en inglés, español y portugués. Una vez recuperados los textos completos, dos revisores evaluarán críticamente, de forma independiente, la calidad metodológica y la extracción de datos y se presentarán en forma de tabla. Una síntesis narrativa acompañará a los resultados y, si es posible, se realizará un metanálisis y se presentará un Grading of Recommendations, Assessment, Development and Evaluation. Crianças/adolescentes são particularmente suscetíveis ao stress e ansiedade em torno da cirurgia devido ao seu desenvolvimento cognitivo, experiências anteriores e conhecimento que possuem sobre os cuidados de saúde, necessitando de intervenções para a prevenção/redução destes sintomas. Objetivo: Avaliar a eficácia de intervenções educacionais centradas na família na ansiedade, dor e comportamentos de crianças/adolescentes e ansiedade dos pais no período perioperatório. Métodos: Esta revisão seguirá a metodologia do Instituto Joanna Briggs para revisões sistemáticas de eficácia e considerará estudos (experimentais e quase-experimentais) em que as intervenções educacionais perioperatórias tenham sido aplicadas a crianças/ adolescentes e seus pais e avaliadas a dor, ansiedade e comportamento em crianças/adolescentes e ansiedade dos pais como resultados. Uma pesquisa inicial limitada de MEDLINE e CINAHL foi realizada. Será seguida por uma segunda busca por estudos publicados e não publicados de janeiro de 2007 disponíveis em inglês, espanhol e português. Abstract Background: Service quality in anesthesiology has been frequently measured by morbidity and mortality. This measure increasingly considers patient satisfaction, which is the result of care from the client's perspective. Therefore, anesthesiologists must be able to build relationships with patients, provide understandable information and involve them in decisions about their anesthesia. This study aimed to evaluate the peri-anesthetic care provided by the anesthesia service in an ambulatory surgery unit using the Heidelberg Peri-anaesthetic Questionnaire. Methods: This cross-sectional study used the Heidelberg Peri-anaesthetic Questionnaire to evaluate 1211 patients undergoing ambulatory surgery. We selected questions that showed a greater degree of dissatisfaction and correlated them with patient characterization data (age, sex, education, and ASA physical status), anesthesia data (type, time, and prior experience), and surgical specialty. Results: Questions in which patients tended to show dissatisfaction involved fear of anesthesia and surgery, feeling cold, the urgent need to urinate, pain at the surgical site, and the team's level of concern and speed of response in relieving the patient's pain. Conclusion: The Heidelberg Peri-anaesthetic Questionnaire proved to be a useful tool in identifying points of dissatisfaction, mainly fear of anesthesia and surgery, feeling cold, the urgent need to urinate, pain at the surgical site, and the team's level of concern and speed of response in relieving the patient's pain in the population studied. These were correlated with patient, anesthesia, and surgical variables. This allows the establishment of priorities at the different points of care, with the ultimate goal of improving patient satisfaction regarding anesthesia care.

ABSTRACT

La enfermedad de Graves es un proceso inmunomediado en el que autoanticuerpos se dirigen contra el receptor de tirotrofina. Por su acción estimulante sobre la glándula tiroides, se genera crecimiento glandular difuso y aumento de la hormonogénesis. Se caracteriza por el comienzo subagudo de síntomas constitucionales, neuromusculares, cardiovasculares, gastrointestinales y oculares, seguidos en algunos casos de la aparición de manifestaciones cutáneas como la dermopatía tiroidea o mixedema. En pediatría la enfermedad de Graves es infrecuente (aunque es la causa más frecuente de hipertiroidismo), pero la cronología de aparición de los síntomas está bien descrita; es rara la aparición de dermopatía en ausencia de otros síntomas de hipertiroidismo y sin afectación ocular. Se presenta el caso de una paciente de 15 años con dermopatía tiroidea por enfermedad de Graves sin oftalmopatía ni otros síntomas de hipertiroidismo clínico asociados.


Graves disease is an immune-mediated process characterized by the presence of autoantibodies to thyrotropin receptors. Its stimulating action on the thyroid gland causes diffuse glandular growth and increased hormone production. Graves disease is characterized by a subacute onset of non-specific, neuromuscular, cardiovascular, gastrointestinal, and eye symptoms, sometimes followed by skin manifestations, such as thyroid dermopathy or myxedema. In pediatrics, Graves disease is rare (although it is the most frequent cause of hyperthyroidism). However, the chronology of symptom onset has been well described; the development of dermopathy in the absence of other symptoms of hyperthyroidism and without eye involvement is rare. Here we describe the case of a 15-year-old female patient with thyroid dermopathy due to Graves disease without eye disease or other associated clinical symptoms of hyperthyroidism.

Abstract Background Emergence Delirium (ED) is a combination of disturbance of perception and psychomotor agitation that is common in pediatric patients after general anesthesia, especially at preschool age. Since the effect of ED on the length of stay has been studied in adults but infrequently in children, the aim of this study was to investigate the relationship between ED and length of stay in this population. Methods A single center, retrospective, observational study was carried out in children who underwent tonsillectomy or adenotonsillectomy. The Pediatric Anesthesia Emergence Delirium (PAED) scale was used to assess ED. In addition to the time to hospital discharge (time frame 24 hours), drugs used, comorbidities, early postoperative complications, and pain were investigated if potentially associated with the complication. Results Four hundred sixteen children aged from 1.5 to 10 years (183 female, 233 male) were included. ED occurred in 25.5% of patients (n = 106). Patients were divided into the ED group and the No-ED group. The discharge time was similar in both groups. No significant differences were observed in the frequency of postoperative complications. The use of fentanyl or dexmedetomidine did not affect ED occurrence. The frequency of pain was greater in the ED group, both in the recovery room and in the ward (p= 0.01). Conclusions The occurrence of ED in children after tonsillectomy/adenotonsillectomy did not extend the length of stay.

ABSTRACT

La ecografía es una herramienta segura, portátil, económica y de fácil acceso. Los médicos anestesiólogos pueden beneficiarse con esta herramienta diagnóstica rápida y precisa en su práctica habitual. Existen múltiples áreas potenciales donde la ecografía desempeña un papel importante, para la orientación de intervenciones a ciegas e invasivas, el diagnóstico de condiciones críticas y la evaluación de posibles variaciones anatómicas que pueden conducir a la modificación del plan anestésico. Esta revisión narrativa describe las principales aplicaciones de la ecografía en anestesia, las técnicas ecoguiadas y las tendencias actuales del manejo anestésico perioperatorio del paciente quirúrgico. Se realizó una búsqueda en las bases de datos PubMed y Cochrane, se incluyeron artículos originales, estudios aleatorizados y de revisión, en español y en inglés, publicados entre 2017-2021. El uso de ecografía ha entrado en el campo de la medicina del dolor, anestesia regional y del intervencionismo analgésico durante la última década, e incluso es el estándar de la práctica, por tanto, la capacitación y un adecuado aprendizaje en la ecografía deben ser parte del plan de estudios de cualquier programa de anestesiología


Ultrasound is a safe, portable, inexpensive, and easily accessible tool. Anesthesiologists can benefit from this fast and accurate diagnostic tool in their routine practice. There are multiple potential areas where ultrasound plays an important role in the guidance of blind and invasive interventions, diagnosis of critical conditions, and assessment of possible anatomical variations that may lead to modification of the anesthetic plan. This narrative review describes the main applications of ultrasound in anesthesia, ultrasound-guided techniques, and current trends in the perioperative anesthetic management of the surgical patient. A search was conducted in PubMed and Cochrane databases. Original articles, randomized and review studies in Spanish and English published between 2017-2021 were included. The use of ultrasound has entered the field of pain medicine, regional anesthesia, and interventional analgesia during the last decade and is even the standard of practice. Therefore, training and adequate learning in ultrasound should be part of the curriculum of any anesthesiology program

Abstract Background Postdural puncture headache (PDPH) is a common complication of neuraxial techniques which delays patients' discharge. Sphenopalatine ganglion block (SPGB) is a safe bedside technique with comparable efficacy to Epidural Blood Patch, the gold-standard treatment. There is no evidence on the ideal timing for SPGB performance. We aimed to evaluate the difference between early versus late SPGB concerning efficacy, symptom recurrence and hospital length of stay. Methods We present an observational study with 41 patients diagnosed with PDPH who were submitted to SPGB with ropivacaine 0,75%. The study sample (n = 41) was divided in two groups: an early (less than 24 hours after diagnosis) and a late (more than 24 hours after diagnosis) SPGB group. Pain was evaluated 15 minutes after the block and follow up occurred daily until patients were discharged. Patients' demographic characteristics, neuraxial technique, timing of SPGB, qualitative pain relief and post-SPGB length of stay were registered and analyzed with SPSS statistics (v26) software. Results Early SPGB resulted in a significant reduction in length of stay (p = 0,009) and symptom recurrence (p = 0,036), showing equally effective pain relief, compared to late SPGB. Conclusions SPGB was equally effective in both groups. Data showed that early SPGB reduces length of hospital stay and symptom recurrence, which potentially allows early resumption of daily activities and a reduction in total health costs.

ABSTRACT

Abstract The regional techniques for axillary analgesia are well established. However, few studies have investigated surgical anesthesia. In this report, extensive debridement of axillary necrotizing fasciitis, including the posteromedial region of the right arm, performed under exclusive regional anesthesia in a patient with probable difficult airway is described. The procedure was accomplished under a Serratus Plane Block (SPB) and supraclavicular brachial plexus block, guided by ultrasound, and with venous sedation. We observed satisfactory anesthesia 15 minutes after the intervention, efficient intraoperative pain control and within the following 24 hours. Surgical axilla anesthesia is feasible with the described blocks.

Introducción: el tratamiento del dolor es un derecho humano y constituye un pilar de los cuidados paliativos (CP). Este síntoma en niños suele ser subestimado e insuficientemente tratado. Objetivo: conocer la prevalencia del dolor y describir el perfil de uso de fármacos analgésicos, coadyuvantes y procedimientos invasivos en niños asistidos en la Unidad de Cuidados Paliativos Pediátricos del Centro Hospitalario Pereira Rossell (UCPP-CHPR) durante el período 2019-2021. Metodología: se realizó un estudio observacional, descriptivo y retrospectivo mediante revisión de historias clínicas. Resultados: se incluyeron 317 niños, 58% de sexo masculino, con una mediana de edad 6,9 años. Eran portadores de enfermedades neurológicas severas no evolutivas 64%, utilizaban prótesis o tecnología médica 51%. Se encontró registro de presencia de dolor en 35%, de tipo crónico 87%, mixto 55% y de fuentes múltiples 54%. Se detectó uso de escala para evaluación del dolor en 61%, la más utilizada fue r-FLACC. En el grupo de niños con dolor se encontró prescripción de analgésicos en 43% (48/111) y de coadyuvantes 87% (97/111), gabapentina en 78. En todos la vía de administración fue la oral/enteral. Se encontró uso off label de fármacos en 79% y polifarmacia en 82%. Se registraron efectos adversos en 10%. Conclusión: un tercio de los niños asistidos por la UCPP-CHPR, presentaba registros de presencia dolor. La mayoría de tipo crónico, mixto y de fuentes múltiples. Se encontró amplio uso de escalas validadas para evaluación del dolor y alta prescripción de coadyuvantes en relación a la de analgésicos.


Introduction: pain treatment is a human right and a pillar of palliative care (PC). This symptom in children is often underestimated and insufficiently treated. Objective: learn about the prevalence of pain and describe the analgesic drugs' usage profile, adjuvants and invasive procedures in children assisted in the Pediatric Palliative Care Unit of the Pereira Rossell Hospital Center (UCPP-CHPR) during the period 2019-2021. Methodology: observational, descriptive and retrospective study based on the review of medical records. Results: 317 children were included, 58% male, with a median age of 6.9 years. 64% were carriers of severe non-progressive neurological diseases, 51% used prosthetics or medical technology. A record of the presence of pain was found in 35%, chronic type 87%, mixed 55% and multiple sources 54%. The use of a pain assessment scale was detected in 61%, the most used was r-FLACC. In the group of children with pain, analgesics were prescribed in 43% (48/111) and adjuvants in 87% (97/111), gabapentin in 78. In all of them, the administration route was oral/enteral. Off-label use of drugs was found in 79% and polypharmacy in 82%. Adverse effects were recorded in 10%. Conclusion: a third of the children assisted by the UCPP-CHPR showed records of pain presence. Most chronic type, mixed and multiple sources. We found a vast use of validated scales for pain assessment and high prescription of adjuvants in relation to analgesics.


Introdução: o tratamento da dor é um direito humano e constitui um pilar dos Cuidados Paliativos (CP). Este sintoma em crianças é geralmente subestimado e insuficientemente tratado. Objetivo: conhecer a prevalência da dor e descrever o perfil do uso de medicamentos analgésicos, adjuvantes e procedimentos invasivos em crianças atendidas na Unidade de Cuidados Paliativos Pediátricos do Centro Hospitalar Pereira Rossell (UCPP-CHPR) durante o período de 2019-2021. Metodologia: foi realizado um estudo observacional, descritivo e retrospectivo por meio de revisão de prontuários. Resultados: foram incluídas 317 crianças, 58% do sexo masculino, com idade mediana de 6,9 anos. 64% eram portadores de doenças neurológicas graves não evolutivas, 51% usavam próteses ou tecnologia médica. Registro da presença de dor foi encontrado em 35%, do tipo crônica 87%, mista 55% e de origem múltipla 54%. A utilização de escala para avaliação da dor foi detectada em 61%, sendo a mais utilizada a r-FLACC. No grupo de crianças com dor, a prescrição de analgésicos foi encontrada em 43% (48/111) e adjuvantes em 87% (97/111), gabapentina em 78. Ao todo, a via de administração foi oral/enteral. Uso off-label de medicamentos foi encontrado em 79% e polifarmácia em 82%. Efeitos adversos foram registrados em 10%. Conclusão: um terço das crianças atendidas pela UCPP-CHPR apresentou registro da presença de dor. A maioria do tipo crônica, mista e de fontes múltiplas. Encontrou-se ampla utilização de escalas validadas para avaliação da dor e elevada prescrição de coadjuvantes em relação aos analgésicos.

Introducción: el conocimiento del cannabis medicinal data de hace más de 10.000 años y tiene sus orígenes en el continente asiático y en la medicina oriental. En el último tiempo ha surgido especial interés en su uso terapéutico, y Uruguay desde 2013 cuenta con una ley que regula la tenencia y uso de la planta de cannabis. Objetivo: caracterizar el uso de los derivados de cannabis medicinal (DCM) en una población de usuarios uruguayos. Metodología: se realizó un estudio observacional, prospectivo, en una población de usuarios de DCM, a través de encuestas telefónicas. Los datos obtenidos se analizaron utilizando Excel®, mediante estadística descriptiva. Resultados: se incluyeron 32 usuarios entre 29 y 78 años, la mayoría de sexo femenino. La forma farmacéutica más utilizada fue el aceite y la principal indicación fue para el tratamiento del dolor. Se observó una disminución en la intensidad del dolor postratamiento. El principal efecto adverso observado fue la sequedad de boca. Conclusiones: es el primer estudio nacional en caracterizar el uso de DCM artesanal. Se incluyeron 32 usuarios de DCM artesanal. La principal indicación de DCM fue para el tratamiento del dolor, siendo la artrosis su principal causa. Todos los usos fueron para indicaciones no aprobadas si se compara con sus equivalentes industrializados. Predominó el uso en mujeres adultas. Se destacó una gran expectativa frente al inicio del uso. Los eventos adversos observados estuvieron dentro de los esperados y de entidad leve.


Summary: Introduction: knowledge on medical cannabis is over 10,000 years old and stems from Asian and Eastern medicine. In recent years, special interest on its therapeutic use has arisen, and in 2013 Uruguay passed a law to regulate possession and use of cannabis plants. Objective: to characterize the use of medicinal cannabis derivatives in a population of Uruguayan users. Method: observational, prospective study in a population of medicinal cannabis derivatives users through telephone surveys. Data obtained were analysed with Excel®, by using descriptive statistics. Results: thirty two users were included in the study, between 29 and 78 years old, most of which were female. The most widely used pharmaceutical form was oil and the main indication was to treat pain. A decline in pain intensity after treatment was observed. The main adverse effect observed was dry mouth. Conclusions: this is the first national study to characterize the use of artisanal medicinal cannabis derivatives. Thirty two users of artisanal medicinal cannabis derivatives were included in the study. The main indication for artisanal medicinal cannabis derivatives was the treatment of pain, arthrosis being the main cause. All users followed non-approved indications if compared to industrialized equivalents. The use was more extended among women. Great expectation upon initiation of use was noticed. Adverse events observed were mild and the expected.


Introdução: o conhecimento da cannabis medicinal remonta a mais de 10.000 anos e tem suas origens no continente asiático e na medicina oriental. Nos últimos tempos, surgiu um interesse especial em seu uso terapêutico e, desde 2013, o Uruguai possui uma lei que regula a posse e o uso da planta de cannabis. Objetivo: caracterizar o uso de derivados de cannabis medicinal (DCM) em uma população de usuários uruguaios. Metodologia: Foi realizado um estudo observacional, prospectivo, em uma população de usuários de DCM, por meio de inquéritos telefônicos. Os dados obtidos foram analisados ​​no programa Excel®, por meio de estatística descritiva. Resultados: foram incluídos 32 usuários entre 29 e 78 anos, a maioria do sexo feminino. A forma farmacêutica mais utilizada foi o óleo e a principal indicação foi para o tratamento da dor. Observou-se diminuição da intensidade da dor pós-tratamento. O principal efeito adverso observado foi boca seca. Conclusões: este é o primeiro estudo nacional a caracterizar o uso de DCM artesanal. Foram incluídos 32 usuários artesanais de DCM. A principal indicação da CMD foi para o tratamento da dor, sendo a osteoartrite sua principal causa. Todos os usos foram para indicações não aprovadas quando comparados aos seus equivalentes industrializados. Predominou o uso em mulheres adultas. Houve uma grande expectativa desde o início do uso. Os eventos adversos observados estavam dentro do esperado e de entidade leve.

ABSTRACT

A pandemia da covid-19 impôs transformações no cotidiano mundial, em âmbito micro e macroestrutural. Seu impacto psicológico desestabiliza e evidencia desigualdades e vulnerabilidades psicossociais brasileiras. Configura-se como um estudo de perspectiva crítica, com base na Psicologia Sócio-histórica, com o objetivo de mapear os posicionamentos da Psicologia, vindos de diferentes campos, diante das ações de saúde mental. Para tanto, utiliza-se o site do Conselho Federal de Psicologia para a análise de 62 documentos, que resultaram em dois eixos de produção crítica: 1) a relação da Psicologia com o Conselho Federal de Psicologia; e 2) da Psicologia com a sociedade. Revela-se o abismo social entre segmentos da sociedade brasileira; formas de exclusão da população carcerária; violência doméstica contra as mulheres e as crianças; dificuldades de acesso a estratégias sociais, na educação e na saúde, e de superação dos impasses acirrados com a infecção global pelo novo coronavírus. Conclui-se que a diversidade de públicos, temáticas, áreas de atuação e referenciais teóricos materializa um compromisso crítico e científico da Psicologia.(AU)


The COVID-19 pandemic imposed transformations in the world daily life, at the micro and macrostructural levels. Its psychological impact destabilizes and highlights Brazilian inequalities and psychosocial vulnerabilities. This is a critical perspective study, based in socio-historical Psychology, aiming to map the positions of Psychology, from different fields, in the face of mental health actions. To this end, the Federal Council of Psychology website is utilized to analyze 62 documents, which resulted in two axes of critical production: 1) the relation between Psychology and the Federal Council of Psychology; and 2) Psychology with society. They reveal the social gap between segments of Brazilian society; ways of excluding prison po7pulation; domestic violence against women and children; and difficulties in accessing social strategies, in education and health, and in overcoming impasses aggravated by the global infection by the new coronavirus. In conclusion, the diversity of public, themes, areas of professional performance, and theoretical references materialize Psychology's critical and scientific commitment.(AU)


La pandemia del COVID-19 provocó transformaciones globales en lo cotidiano a nivel micro y macroestructural. Su impacto psicológico desestabiliza y destaca las desigualdades y vulnerabilidades psicosociales en Brasil. Esta es una investigación en la perspectiva crítica, basada en la psicología sociohistórica, con el objetivo de mapear las posiciones de la Psicología, procedentes de diferentes campos, frente a las acciones de salud mental. Para este fin, se utiliza el sitio web del Consejo Federal de Psicología para el análisis de 62 documentos, lo que resultó en dos ejes de producción crítica: 1) la relación de la Psicología con el Consejo Federal de Psicología; y 2) de la Psicología con la sociedad. Se revelan la brecha social entre los segmentos de la sociedad brasileña; las formas de exclusión de la población carcelaria; la violencia doméstica contra las mujeres y los niños; y las dificultades para acceder a las estrategias sociales, en la educación y la salud, para superar los impasses agravados por la infección global por el nuevo coronavirus. ABSTRACT

O artigo parte da aposta na pesquisa como uma prática em movimento, na composição de territórios do ato de pesquisar como uma experiência. Inspiradas(os) na força disruptiva do torcer-retorcer dos Parafusos (folguedo sergipano, derivado de práticas de resistência à escravidão dos povos pretos), propomos um método-pensamento de inventar modos de ver, dizer e narrar a partir daquilo que está ao nosso redor, uma posição em relação ao mundo e a si mesmo, engendrando a ideia de que este mundo não é um dado, mas um efeito de nossas práticas. A partir dessa perspectiva, intentamos interpelar as próprias práticas discursivas e não discursivas da Psicologia, em favor de abrir trilhas nas quais esse saber possa refundar sua própria história, acentuando suas descontinuidades e heterogeneidades e, com isso, uma atitude de fazer frente às tendências colonialistas, agora em suas versões neoliberais. Esse modo de pesquisar se faz por: operar uma ideia-método genealógico-cartográfica e uma escrita polifônica; produzir máquinas de guerras nômades, minoritárias, pós-identitárias, decoloniais; e inventar com aquilo que está ao nosso redor em favor de saberes e fazeres das redes locais, que, tais como a planta mangabeira, não se deixam domesticar.(AU)


The article starts from the research as a moving practice; in the composition of territories of the act of researching as an experience. Inspired by the disruptive force of the twisting-twining of the Screws (revelry from Sergipe, derived from practices of resistance to the slavery of black people), we propose a thought-method of inventing ways of seeing, saying, and narrating from what is around us, a position in relation to the world and itself, engendering the idea that this world is not a given, but an effect of our practices. From this perspective, we try to question the very discursive and non-discursive practices of Psychology, in favor of opening trails, in which this knowledge can refund its own history, accentuating its discontinuities and heterogeneities and, thus, an attitude of facing the colonialist tendencies, now in their neoliberal versions. This way of searching is done by: operating a genealogical-cartographic idea-method and polyphonic writing; producing nomadic, minority, post-identity, decolonial war machines; and inventing with what is around us, in favor of knowledge and practices of local networks, which, like the mangabeira plant, do not allow domestication.(AU)


Este artículo parte de la investigación como práctica en movimiento, en la composición de territorios del acto de investigar como experiencia. Inspirándonos en la fuerza disruptiva de torsión-retorsión de los Tornillos (juerga de Sergipe, derivada de prácticas de resistencia a la esclavitud de los negros), proponemos un método de pensamiento para inventar formas de ver, decir y narrar a partir de lo que nos rodea, una posición en relación con el mundo y con él mismo, el cual constituye la idea de que este mundo no es un hecho, sino un efecto de nuestras prácticas. Desde esta perspectiva, pretendemos cuestionar las prácticas discursivas y no discursivas de la Psicología en favor de abrir caminos, en las cuales este conocimiento pueda refundar su propia historia, acentuando sus discontinuidades y heterogeneidades y, así, una actitud de hacer ante las prácticas colonialistas, ahora desde una perspectiva neoliberal. Este modo de investigación se realiza mediante: el funcionamiento de un método-idea de escritura genealógica, cartográfica y polifónica; la producción de máquinas de guerra nómadas, minoritarias y posidentitarias; y la composición con el tema que nos rodea a favor del conocimiento y las redes locales que no puede ser domesticada, como el árbol mangabeira.(AU)

ABSTRACT

O objetivo deste estudo foi refletir sobre os efeitos da não adesão ao tratamento para a equipe de saúde e sobre as ações/reações da equipe que podem causar a não adesão ao tratamento. A amostra foi composta por 10 profissionais de saúde. O instrumento de coleta de dados foi uma entrevista semiestruturada. O material coletado foi submetido à análise temática, e discussão foi baseada na psicanálise. Como resultado, verificou-se que os profissionais relacionaram a não adesão às carências percebidas nos pacientes. Também foi identificada a presença de um ciclo de encaminhamentos, o qual, por vezes, significava uma tentativa de eliminar um incômodo (a não adesão), mas, em contrapartida, o causava. Verificou-se também a presença de confusão entre cuidado e controle, produzindo relações permeadas por desconfiança, verificação e correção. Percebeu-se, ainda, relação entre não adesão e frustração, seja porque o tratamento é insuficiente para evitar o sofrimento do paciente, seja pelo desconforto advindo da não cooperação do paciente. Ao final, como efeitos para a equipe, evidenciou-se a presença de profissionais envolvidos por um discurso de frustração, desvalorização e impotência. Como efeitos da equipe, verificou-se que profissionais também podem produzir aquilo de que se queixam, pelos lugares subjetivos que delineiam e cristalizam. A partir disso, problematiza-se o sentido que a não adesão pode assumir, e é importante considerá-la como um sinal que pode revelar os percalços (e as possíveis resoluções) do contrato relacional entre paciente e equipe.(AU)


The objective of this study was to reflect about the effects of non-adherence to the treatment for the health team and about the actions/reactions of the team that may can cause the non-adherence to the treatment. The sample consisted of 10 health professionals. The data collection instrument was a semi-structured interview. The material collected was submitted to thematic analysis, and the discussion was based on psychoanalysis. As a result, it was verified that the professionals related non-adherence to needs perceived on patients. The presence of a referral cycle was also identified, which, sometimes, meant an attempt to eliminate a nuisance (the non-adherence) but, instead, caused it. It was also verified the presence of confusion between care and control, producing relations permeated by distrust, verification, and correction. The link between non-adherence and frustration was also observed, either due to the treatment being insufficient to avoid the suffering of the patient; or by the discomfort from the non-cooperation of the patient. At the end, as effects for the team, professionals involved by a discourse of frustration, devaluation, and impotence were evidenced. As effects of the team, it has been found that professionals can also produce what they complain about, by the subjective places that were delineate and crystallize. Thus, we problematize the meaning non-adherence may assume, and considering it a signal that can reveal the mishaps (and possible resolutions) of the relational contract between patient and team is important.(AU)


El objetivo de este estudio fue reflexionar sobre los efectos de la no adherencia al tratamiento para el equipo de salud y sobre las acciones/reacciones del equipo que pueden causar la no adherencia al tratamiento. La muestra estuvo conformada por diez profesionales de la salud. El instrumento de recolección de datos fue una entrevista semiestructurada. El material recolectado fue sometido a análisis temático, y se utilizó el psicoanálisis para discutir el material. El resultado constató que los profesionales entendieron la no adherencia como una carencia/necesidad de los pacientes. También se identificó la presencia de un ciclo de derivación, que a veces significó un intento de eliminar una molestia (falta de adherencia), pero que puede generar el problema. También se verificó la presencia de confusión entre cuidado y control, produciendo relaciones permeadas de desconfianza, verificación y corrección. También se observó el vínculo entre la no adherencia y la frustración, ya sea porque el tratamiento es insuficiente para evitar el sufrimiento del paciente o por la incomodidad que produce la falta de cooperación del paciente. Al final, como efectos para el equipo, se evidenciaron profesionales envueltos por la frustración, la devaluación y la impotencia. Como efectos del equipo, se constató que los profesionales también pueden producir lo que quejan desde los lugares subjetivos que fueron delineados y cristalizados. Así se discute el sentido que puede asumir la no adherencia, y es importante considerarla como señal de los percances (y posibles resoluciones) del contrato relacional entre paciente y equipo.(AU)

Objetivo: conhecer os sentimentos maternos frente a dor no recém-nascido durante procedimentos invasivos. Método: revisão integrativa cuja elaboração da questão norteadora foi baseada na estrutura mnemônica PiCo (População, Fenômeno de Interesse e Contexto): "Quais são os sentimentos maternos gerados frente a pratica de procedimentos invasivos que podem causar dor em seus filhos recém-nascidos? A busca dos estudos foi realizada nas seguintes bases de dados: lilacs, medline, pubmed, scielo e embase, com a combinação dos descritores em português, espanhol e inglês: mães, sentimentos, procedimentos invasivos; criança, dor, experiências, unidade de terapia intensiva, internação, hospitalar. Sem limite temporal. A coleta ocorreu de setembro a outubro de 2022. Resultados: a busca nas bases de dados resultou em 398 artigos sendo apenas nove artigos selecionados, os quais contemplaram a pergunta de pesquisa. Conclusão: os sentimentos maternos frente aos procedimentos invasivos que causam dor em seus filhos envolvem intenso sofrimento expresso por momentos de angústias, ansiedade, estresse, tristeza, medo, impotência e rompimento da vinculação mãe e filho. Objective: to know maternal feelings regarding pain in newborns during invasive procedures. Method: integrative review whose elaboration of the guiding question was based on the PiCo mnemonic structure (Population, Phenomenon of Interest and Context): "What are the maternal feelings generated when facing the practice of invasive procedures that can cause pain in their newborn children? The search for studies was carried out in the following databases: lilacs, medline, pubmed, scielo and embase, with a combination of descriptors in Portuguese, Spanish and English: mothers, feelings, invasive procedures; child, pain, experiences, intensive care unit, admission, hospital. No time limit. The collection took place from September to October 2022. Results: the search in the databases resulted in 398 articles, with only nine selected articles, which included the research question. Conclusion: maternal feelings in the face of invasive procedures that cause pain in their children involve intense suffering expressed by moments of anguish, anxiety, stress, sadness, fear, impotence and disruption of the mother-child bond.

ABSTRACT

Introducción: La lesión del nervio ciático puede ocasionar desde parestesias e hipoestesias, hasta dolor neuropático severo y parálisis. La neurólisis suele mejorar la función, el dolor y la calidad de vida de los pacientes. El objetivo de este estudio fue evaluar los resultados clínico-funcionales de una serie de pacientes con dolor neuropático por lesión del nervio ciático que no respondieron al tratamiento conservador y fueron sometidos a neurólisis. Materiales y métodos: Se analizó retrospectivamente una serie de pacientes sometidos a neurólisis del nervio ciático mayor entre marzo de 2009 y junio de 2018. El dolor pre- y posoperatorio se evaluó mediante la escala analógica visual y la escala de Likert. El tipo de dolor posoperatorio se evaluó con el cuestionario DN4 y la calidad de vida relacionada con la salud, con el cuestionario SF-36.Resultados:Se incluyó a 8 pacientes. A los 32 meses de seguimiento promedio (rango 14-66), el dolor había mejorado notablemente (promedio de 3 y 1,88 puntos en las escalas analógica visual y de Likert, respectivamente). El cuestionario DN4 arrojó un promedio de 3,75 puntos (rango 2-7). Según el SF-36, la "salud física" fue la variable con peores resultados (promedio 30,15).Conclusiones: La neurólisis, cuando se indica a pacientes con mala respuesta al tratamiento conservador, es un método que alivia el dolor y mejora la calidad de vida de los pacientes con dolor neuropático secundario a una lesión del nervio ciático. Introduction: Sciatic nerve injury may cause paresthesias and hypoesthesias, severe neuropathic pain, and paralysis. Neurolysis can improve function, pain, and quality of life for these patients. The objective of this paper is to evaluate the clinical-functional outcomes of a series of patients with neuropathic pain due to sciatic nerve injury that was refractory to conservative treatment in whom neurolysis was performed. materials and methods:A retrospective case series of patients operated on for neurolysis of the greater sciatic nerve between March 2009 and June 2018 was analyzed. Preoperative and postoperative pain were evaluated using the visual analog scale (VAS) and the Likert scale. The type of postoperative pain was evaluated using the DN4 questionnaire, and the health-related quality of life was measured with the SF-36 questionnaire. Results: Eight patients were included. All patients evolved with a notable improvement in pain, with an average of 3 and 1.88 points on the VAS and Likert scales, respectively. The mean follow-up was 32 months (range 14­66). The DN4 questionnaire showed an average of 3.75 points (range 2­7). According to the SF-36, "Physical Health" was the variable with the worst results, with an average of 30.15. Conclusions: Sciatic nerve neurolysis in patients with neuropathic pain due to sciatic nerve injury and poor response to conservative treatment may improve pain and quality of life. ABSTRACT

Abstract Introduction Knee osteoarthritis (KOA) is a prevalent pathology in older adults, with physical, social and psychological impacts that reduce their quality of life. Objective Analyze the correlation between KOA in older individuals and biopsychosocial aspects such as pain, functional limitation, dependence, anxiety and depression. Methods A systematic review of English and Portuguese articles published on the Cochrane Library, PUBMED/MEDLINE, SciELO and Web of Science databases between 2016 and 2021, selected according to inclusion and exclusion criteria. Results After cross-checking specific to each database, studies were selected by reading the title (2,304), abstract (136) and full article (72), with 30 ultimately included based on the eligibility criteria. Conclusion Knee osteoarthritis has a significant negative impact on patients' lives and is correlated with pain, functional limitation, dependence, anxiety and depression, compromising interpersonal relationships, physical and mental status and causing disability due to chronic pain, making osteoarthritis (OA) a public health issue with a high financial, physical and emotional cost for patients.


Resumo Introdução A osteoartrite de joelho é uma patologia prevalente em idosos, impactando suas vidas em um contexto físico, social e psicológico, o que ocasiona redução na qualidade de vida desses indivíduos. Objetivo Analisar a correlação entre a osteoartrite de joelho em idosos e aspectos biopsicossociais como dor, limitação funcional, dependência, ansiedade e depressão. Métodos Revisão sistemática de artigos publicados nas bases de dados Cochrane Library, PUBMED/MEDLINE, SciELO e Web of Science, entre 2016 e 2021, em português e inglês. Após isso, realizou-se a seleção dos artigos de acordo com os critérios de inclusão e exclusão. Resultados Após cruzamentos específicos em cada base, selecionaram-se os artigos por meio da leitura do título (2.304), do resumo (136) e do artigo na íntegra (72). Assim, 30 artigos foram selecionados de acordo com os critérios de elegibilidade. Conclusão Confirmou-se que a osteoartrite de joelho impacta negativamente a vida dos pacientes e está correlacionada com dor, limitação funcional, dependência, ansiedade e depressão, prejudicando as relações interpessoais, afetando condições físicas e mentais e gerando incapacidade devido à dor crônica, tornando a osteoartrite uma questão de saúde pública de alto custo financeiro, físico e emocional aos pacientes.

Abstract Introduction: Smartphone use has become a popular social communication phenomenon worldwide. Its excessive use can compromise daily routines and habits, which is associated with sleep disorders, stress, anxiety and pain. Therefore, the university student stands out, as they has a lifestyle in which it is necessary to reconcile daily activities with curriculum activities, aggravating psychosocial factors. Objective: To investigate whether smartphone addiction influences sleep quality, anxiety, depression and pain in university students. Methods: We carried out an analytical cross-sectional study. For data collection, the following self-administered questionnaires were used: Smartphone Addiction Inventory (SPAI-BR), to assess smartphone dependence; Pittsburgh Sleep Quality Index (PSQI), to evaluate sleep quality; Hospital Anxiety and Depression Scale, to assess anxiety and depression (HADS), where it was subdivided into HADS-A for anxiety and HADS-D for depression; and Numeric Rating Scale (NRS) to determine physical pain intensity. The sample consisted of 301 university students studying physiotherapy and physical education at the State University of Northern Paraná (UENP). They were divided according to the score obtained in the SPAI-BR between the "regular" group (up to 6 points) and "predisposed" to smartphone dependence (7 or more points). Results: The comparisons were statistically significant in favor of the regular group: the predisposed group obtained a higher score for the questionnaires used with an average NRS of 2.37 points, average HADS-D of 9.05 points and average HADS-A of 6.01 points. Differences between groups were statistically significant: NRS, p = 0.018; HADS-A, p = 0.001; HADS-D p = 0.001; and PSQI, p = 0.001. Conclusion: The university students analyzed in this study classified as predisposed were more prone to being addicted to their smartphone, and they were more likely to have anxiety, with a worse quality of sleep and with a greater intensity of pain.


Resumo Introdução: O uso de smartphones se tornou um fenômeno social mundialmente popular de comunicação. Seu uso excessivo pode comprometer as rotinas e hábitos diários, que estão associados aos distúrbios do sono, estresse, ansiedade, algias; logo, destaca-se o universitário, que apresenta um estilo de vida em que é preciso conciliar as atividades diárias com as curriculares, agravando fatores psicossociais. Objetivo: Investigar se a dependência do uso de smartphone influencia a qualidade de sono e os níveis de ansiedade, depressão e dor em universitários. Métodos: Trata-se de um estudo transversal analítico. Para a coleta dos dados foram utilizados os questionários autoaplicáveis Inventário de Dependências do Smartphone (SPAI-BR), Escala de Pittsburgh (PSQI), Escala Hospitalar de Ansiedade e Depressão (HADS), sendo este subdividido em HADS-A (ansiedade) e HADS-D (depressão), e Escala Numérica da Dor (END). A amostra foi composta por 301 universitários da Universidade Estadual do Norte do Paraná, dos cursos de fisioterapia e educação física. Os estudantes foram divididos de acordo com o escore obtido no SPAI-BR entre grupo regular (até 6 pontos) e pré-disposto à dependência do uso de smartphone (7 ou mais pontos). Resultados: As comparações foram estatisticamente significativas a favor do grupo regular; sendo assim, o grupo pré-disposto obteve uma pontuação pior nos questionários utilizados, sendo a média END de 2,37 pontos, a média HADS-D de 9,05 e a média HADS-A de 6,01. Os valores de intensidade de dor entre os grupos foram de p = 0,018; HADS-A: p = 0,001; HADS-D: p = 0,001; PSQI: p= 0,001. Conclusão: Os universitários classificados como pré- dispostos apresentaram uma maior propensão à dependência do smartphone, além de maior chance de terem ansiedade com uma pior qualidade de sono e maior intensidade de dor.

Opiorphin is a pentapeptide, which could be isolated from human fluids and has a decreasing effect on pain. Aim: Since lichen planus is a chronic mucocutaneous disease, which causes pain or burning feeling in the oral mucosa, this study aimed to compare salivary opiorphin levels of oral lichen planus (OLP) patients with healthy subjects. Methods: This case-control study, was performed on 24 patients with OLP lesions and 21 healthy subjects. After collecting unstimulated saliva, opiorphin levels were compared between two groups through statistical analyses. Results: There was not any significant difference between OLP patients and healthy subjects according to salivary opiorphin concentration (p=0.378). Also, in the OLP group, opiorphin concentration was not significantly different between males and females (p=0.601). Analytical analysis could not show any remarkable difference between various severity of OLP lesions regarding to salivary opiorphin levels (p=0.653). Conclusion: In this study, salivary opiorphin levels was not significantly different between patients with OLP and healthy subjects; however, more studies are suggested for better assessment of salivary opiorphin levels in various types of OLP lesions and its correlation with pain severity
