ABSTRACT

Hemimandibular hyperplasia is responsible for aesthetic, functional, motor, and psychosocial impairments. With an uncertain etiology, it occurs due to the imbalance of regulatory growth factors present in the cartilaginous layer of the condyle. The report aims to describe decision-making based on specific complementary exams combined with the appropriate surgical intervention for the condition. A 33-year-old female patient presented at a private clinic with the main complaint of "asymmetric face." A scintigraphic evaluation was requested to objectively confirm the interruption of condylar growth, ruling out the hypothesis of Osteochondroma and condylectomy. After prior orthodontic preparation, tomographic exams were used to create a prototype that was printed after planned bone movements. Through the reconstruction, it was concluded that the existing asymmetry would not be entirely corrected through orthognathic intervention alone, necessitating also an osteotomy to remove part of the base of the body and mandibular angle. This was done through the creation of a cutting guide, allowing for reanatomization without the need for submandibular extraoral access, thus avoiding a scar on the female face. The patient is 60 months postoperative, with no complaints and satisfied. Therefore, the increasingly positive influence of virtual planning on optimizing surgical outcomes for professionals is evident.

ABSTRACT

Introduction: Efforts to combat tuberculosis (TB) usually focus on early, rapid diagnosis and effective treatment to break the chain of transmission of Mycobacterium tuberculosis. However, in the last few years, coinciding with the syndemic TB/COVID-19 association, more and more evidence has proved the serious clinical, functional and psycho-social sequelae that TB can cause. This condition has been defined as Post-Pulmonary Disease Tuberculosis (PTLD) and it affects approximately one-third of the patients who survive TB, facing persistent respiratory symptoms with episodic exacerbations, chronic respiratory failure, emotional disorders and psychosocial challenges that negatively impact their life quality, meaning a high catastrophic cost. Objective: Provide a shared, guiding and scientifically valid model to promptly diagnose, evaluate and treat patients with PTLD (prevention, diagnosis, treatment and possible rehabilitation). Methodology: It is documentary research that includes systematic reviews, meta-analysis, observational studies and the guidelines that have existed in recent years in this regard, added to an evaluation by experts, with the purpose of adapting them to local conditions of each Latin American country. Conclusions: Considering the global and, particularly, the Latin American burden of TB, and the estimated burden of PTLD, the development of a consensus document on this topic is urgent. Therefore, we think ALAT recommendations will provide the basis for the formulation and adoption of national specific guidelines for the management of PTLD in Latin America.

ABSTRACT

Esthetic dentistry is based on aesthetic principles with logical, also intuitive procedures and techniques, in order to plan a harmonic smile. With its more consecutive current profile linked to the numerous tools available on the market, it is possible to design procedures prioritizing the success of the treatment restoring the function and harmony with greater safety and predictability. The digital flow includes software and automated equipment that can optimize clinical time, presenting benefits for both, the dental surgeon and the patient. Currently, the DSD (digital smile design) has been used as a resource of great prominence for Rehabilitative Dentistry, because it allows a more detailed planning in each stage of the aesthetic rehabilitation treatment with the use of drawings and reference lines, in order to compare between the images generated before and after the procedure, assessing whether they are in accordance with the planning and desires of the professional and the patient, or if modifications are necessary. With this, the objective of the present work was to carry out a literature review of the advances in the digital workflow in aesthetic oral rehabilitation. Analyzing the influence on the quality of care linked to the comfort of patients. A literature review was carried out in PubMed, Scielo and Wiley Library databases, using the descriptors "Digital Smile Design", "Dental Esthetic", "Esthetic Treatment". Articles were searched between 2009 and 2022. Based on the literature revie, it was possible to conclude that the technique presents itself as a valuable tool for restorative treatments, optimizing the understanding of aesthetic difficulties and increasing the acceptance of the treatment by the patient. And, behind these results, there are several variables influencing them, such as greater predictability of procedures, reducing the time for completion in addition to more conservative works, with preserved tooth structure.

ABSTRACT

Background: International literature has evidenced that Deaf people have been disadvantaged during the COVID-19 pandemic; however, there is currently little research published within the South African context. Objectives: This study investigated the ways in which the COVID-19 pandemic and its consequent response measures impacted Deaf adults in Cape Town. Method: Using a descriptive approach, semi-structured, qualitative interviews were held with 15 Deaf adults in Cape Town, South Africa. Participants were purposively selected through a local Deaf organisation. Data were analysed using thematic analysis. Results: Data revealed the challenges experienced when accessing information, the impact of communication barriers on daily life, and how the response measures impacted access to healthcare. Conclusion: The findings of this study demonstrate how the needs of the Deaf community were overlooked and their voices disregarded during the planning of the national pandemic response, ultimately having detrimental consequences. Therefore, the authors argue for greater inclusion of Deaf representatives to ensure equal access to information and resources, especially during a crisis. Contribution: This study contributes to the growing body of knowledge on the consequences of the COVID-19 pandemic in the field of disability and insights can inform both future research and interventions to promote equity and inclusion for Deaf people.

ABSTRACT

Aim: This study examined the relationship between children's intelligence quotient (IQ) and their levels of dental fear and anxiety(DFA) during their first dental visit. It also evaluated the influence of parental general anxiety on children's anxiety during this visit.Materials and methods: Eighty-eight children, aged 612 years, who had never visited a dentist before were conveniently recruited. Recruited children were subjected to the Wechsler Intelligence Scale for Children III (WISC III) and the Children's Fear Survey Schedule-Dental Subscale (CFSS-DS) to examine their IQ and dental fear and anxiety, respectively. Concerning parental general anxiety, General Anxiety Disorder-7 (GAD-7) was utilized. Children's behavior at their first dental visit was assessed utilizing Frankl's behavior rating scale. The Arabic-validated versions of WISC III, CFSS-DS, and GAD-7 were used. The correlations were examined using Spearman's rank-order correlation coefficient (P<0.05). Results: There was a negative correlation between children's IQ score and their DFA levels. There was a positive association between children's DFA and their parent's general anxiety levels. There was a favorable positive correlation between the degree of cooperation and the children's IQ scores during their first dental visit. Conclusion: This study reveals the common occurrence of fear and anxiety in children during dental visits between the ages of 6 and 12. The findings highlight the complex interplay between cognitive abilities, parental influence, and dental experiences in children. It emphasizes the importance of addressing both child and parental anxiety to promote positive dental visits.

ABSTRACT

Background: Health system planning and monitoring rely on routine data collection, analysis and utilisation. However, underdeveloped countries need more data for decision-making. South Sudan's data management framework only partially functions, with delayed data collection and inaccurate data. The study examined the factors affecting data quality in Maridi County, South Sudan, aiming to improve resource forecasting and equitable health service delivery. Objective: The study sought to identify the obstacles and opportunities for improving data quality in health information systems (HIS) in Maridi County, Western Equatoria State, South Sudan. Methods: A cross-sectional study involving 106 respondents was conducted on 12 healthcare facilities in Maridi County. Statistical Package for the Social Sciences (SPSS) version 25 was used for descriptive, factor and thematic analysis to understand data quality, focussing on behavioural, organisational and technical aspects. Result: The study revealed that insufficient motivation, negative staff attitudes, excessive workloads, a lack of cooperation, personnel insufficiency, inadequate supervision, feedback and training influenced data quality. These factors were interrelated, with over 50% of variables showing weak to strong correlations. Set of standard indicators correlated with the presence of standard data collection tools (r = 0.51). Regular feedback from the County Health Department linked with completeness (r = 0.63) and the training of personnel on health management information systems (HMIS) and completeness resulted in moderate association (r = 0.488). Conclusion: Staff motivation, optimal staffing, training, regular feedback, and continuous supervision are crucial for maintaining the appropriate skill set for data quality. Contribution: Data quality can be achieved when standard tools and human resources are maintained and are evenly distributed.

ABSTRACT

Objective To evaluate the effect of a mobile health (mHealth) intervention on early retention of female sex workers in human immunodeficiency virus (HIV) pre-exposure prophylaxis services in the United Republic of Tanzania. Methods The study involved 783 female sex workers: 470 from Dar es Salaam who were given the Jichunge mHealth application (app) in addition to standard HIV pre-exposure prophylaxis (intervention arm), and 313 from Tanga who received pre-exposure prophylaxis alone (control arm). Participants were recruited using respondent-driven sampling and followed up for 12 months. Early retention was defined as attending a pre-exposure prophylaxis follow-up clinic within 28 days of an appointment scheduled for 1 month after starting treatment. To assess if the Jichunge app led to higher retention, we conducted intention-to-treat and per-protocol analyses using a regression model adjusted by inverse probability weighting. Findings Early retention in HIV pre-exposure prophylaxis care was observed in 27.6% (130/470) of participants in the intervention arm and 20.1% (63/313) in the control arm. In the adjusted, intention-to-treat analysis, early retention was observed in 29.4% in the intervention arm and 17.7% in the control arm (risk difference: 11.8 percentage points; 95% confidence interval: 5.318.3). Conclusion Early retention in HIV pre-exposure prophylaxis care was significantly greater among female sex workers in the United Republic of Tanzania who used the Jichunge app than in those who did not. Nevertheless, more than two thirds of sex workers using the application did not attend follow-up services after 1 month, suggesting that additional interventions are needed.

ABSTRACT

Introduction: Complex distal femur fractures AO/AOT type 33C3.3 constitute a challenge for orthopedists due to the difficulty of their treatment and associated complications. The surgical planning and procedure use condylar plates; but these are associated with loss of fixation and collapse of the reduction. Objective: To describe preoperative planning for an AO/AOT 33C3.3 distal femur fracture with 3D bioprinting and computer reconstruction. Case report: The case of a 34-year-old male patient is reported. He has comminuted left fracture of the distal femur, AO/AOT type 33C3.3, due to a traffic accident. Surgical planning and treatment were successfully performed with 3D biomodel printing and reconstruction. Based on the patient's tomographic images, the main fragments, the reduction sequence, the number and position of the implants to be used were identified. Conclusions: Preoperative planning is a critically important stage in managing complex fractures. Conventional techniques can be optimized with computer-assisted surgery and reconstruction with 3D printed biomodels. This novel proposal will allow the appropriate use of materials, optimal reduction sequence, better stability of the fracture and lower risk of surgical complications(AU)

ABSTRACT

Gummy smile is commonly known for exposing the gums to the smile, thereby increasing the crowns elevation of the elements. Regarding a condition, it presents itself in different ways and its identification is fundamental for the development of a treatment plan. The clinician can be done using a dynamic and digital approach to image treatment, such as software, imaging exams, various 3D virtual models and elaborate guides, improving the quality and precision of treatment, offering benefits to patients. Therefore, this study aimed to demonstrate the importance of digital processes in planning and correcting the gummy smile. Patient complained of disproportion of the smile, facing the regions of premolar crowns and incisors of the smile, facing the exposure of the gingiva when smiling. After clinical and imaging exams, the diagnostic studies were diagnosed as passive eruption, type I (phenotype and specific). The choice of choice was remodeling associated with surgery treatment and surgery treatment, thus providing a greater precision in the treatment, being a double guide elaborated. Therefore, it is possible that the digital is possible a predictability of execution, compatibility in relation to patient and professional care, in addition to the greater possibility of individualization planning than it allows, making the results more planned as possibilities and exactly the iatrogenic(AU)

ABSTRACT

Having a harmonious smile is one of the priorities of patients in dentistry, who seek professionals in the area to improve the aesthetic requirements of both teeth and gums. With the various technological advances, the dental surgeon was able to expand his diagnostic and prognostic capacity, being able to carry out a more judicious intervention planning and with greater possibility of success. Through this integrative literature review, we propose to list in detail what are these characteristics that can disharmonize a smile and we also present how to restore the harmony of the smile through dental procedures and elements. A search was carried out in the electronic portals PubMed, Biblioteca Virtual em Saúde (BVS), Ebsco host and Scielo, using the descriptors: "smile", "periodontal plastic", "aesthetics", "black triangles" and "interdental papilla", in addition to of its related terms in Portuguese, including publications from 2017 to 2022. It was possible to observe that through a multifactorial framework, the esthetics and harmony of the smile can be compromised by gingival, facial and dental factors. Thus, assertive planning must be carried out by a trained professional, who identifies the points that interfere positively and negatively in the balance of the smile, with the purpose of generating clinical solutions. With a multidisciplinary approach, it can be confirmed that it is possible to restore the harmony of the smile and achieve patient satisfaction, through plastic surgical and restorative procedures.

ABSTRACT

Executive functions (EF) refer to the set of skills that, in an integrated way, coordinate behavior and cognition. Thus, the commitment in developing EF is linked to several negative outcomes throughout life. Therefore, the assessment of these abilities in childhood is essential to identify and prevent possible harm in adult life. This study aimed to investigate evidences of validity of the Computerized Test for Evaluation of Executive Functions (TAFE) by the age criterion and by the pattern of correlation between TAFE measures and other EF measures. To this end, a sample of 51 children, between 4 and 10 years old, enrolled in a private school in the municipality of Goiânia (GO), from preschool to the 4th year of elementary school, was used. As instruments, in addition to TAFE, the tasks of Corsi Block, Trails Test A and B, Preschool Trails Test, Preschool Stroop Test and the Digits Subtest of the Wechsler Intelligence Scale were used. Kruskal Wallis statistical analyzes were conducted to verify the evidence of validity by age criteria and Spearman's nonparametric correlation analysis to assess evidence of convergent validation. The instrument investigated was effective to discriminate between different age groups, thus being sensitive to the development of EF. The results obtained in TAFE were correlated with those obtained in other tests that also assess FE, showing patterns of convergence. Therefore, the analysis of the results provides validation evidence to TAFE, derived from different investigation methods.(AU)

ABSTRACT

This article aims to understand the conceptions of professionals from the management and services of the Unified Health System (SUS) on Permanent Education in Health (EPS), as well as its challenges and potential. A focus group was used for data collection, followed by a lexical analysis of the descending hierarchical classification type using the Iramuteq software. The results delineated four classes: a) EPS - understandings and expectations; b) obstacles to EPS; c) ETSUS and EPS by courses and training; and d) EPS devices: potentialities and challenges. Participants pointed out misunderstandings about EPS, when equating it with Continuing Education (CE) focused on content transfer, with negative repercussions on EPS practice. The risk of centralizing the person responsible for implementing this proposal, which should be collective and shared among different actors, is discussed. Therefore, a collaborative production is claimed for, which can circulate among those involved, so that each one experiences this place and appropriates the complexity of interactions provided by Permanent Education in Health.(AU)
