ABSTRACT

Introdução:A violência autoprovocada é um importante problema de saúde pública. Esse agravo produz impactos no campo da saúde do indivíduo, da família eda coletividade com desdobramentos sociais e econômicos. Objetivo:Analisar a mortalidade por violência autoprovocada em mulheres em idade fértil no estado do Rio Grande do Norte, Brasil, entre os anos de 2012 e 2021. Metodologia:Trata-se de um estudo ecológico com abordagem quantitativa e utiliza-se como base o estado do Rio Grande do Norte. Os dados foram coletados do Departamento de Informática do Sistema Único de Saúde, por meio das Informações em Saúde,nas seções de estatísticas vitais e população residente com a seleção sexo feminino e faixa etária de 10 a 49 anos.Resultados:Entre os anos de 2012 a 2021, no estado do Rio Grande do Norte,foram registrados 213 óbitos de mulheres em idade fértil por lesões autoprovocadas. Considerando o início e o final desse período, é possível destacar que a faixa etária de maior ocorrência de suicídio foi de 30 a 39 anos em 2012 e de 40 a 49 anos em 2021. Observou-se, nos anos avaliados, que as mulheres eram em sua maioria solteiras, de raça parda/preta e que a própria residência da vítima foi o local predominante para o desfecho da lesão autoprovocada. No que se refere à escolaridade e à relação do óbito com período de gravidez ou puerpério é preciso ressaltar o alto índice de "Não informada" e "Ignorada" nos registros.A taxa média de mortalidade por lesões autoprovocadas em mulheres em idade fértil entre 2012 e 2021 foi de 2,0 óbitos por cada 100.000 habitantes. Conclusões:Assim, conclui-se que o cenário da mortalidade por violência autoprovocada em mulheres em idade fértil no Rio Grande do Norte necessita de estratégias para prevenção do suicídio nessa faixa etária (AU).


Introduction: Self-inflicted injury is a major public health problem that impacts the health, social, and economic areas of individuals, their families, and society. Aim: To analyze mortality by self-inflicted injury in fertile women from the Rio Grande do Norte state between 2012 and 2021.Methodology: This ecologic and quantitative study collected vital statistics of women aged between 10 and 49 years. Data were obtained from the Health Information Systems of the Brazilian Health Informatics Department.Results: A total of 213 deaths of fertile women by self-inflicted injury were registered between 2012 and 2021. Considering the age groups, most deaths occurred between 30 and 39 years in 2012 and between 40 and 49 years in 2021. In addition, women were mostly single andwith brown or black skin color, and most of the self-inflicted injuries happened at their houses. Regarding education level and the relationship of death with pregnancy or postpartum, most registries presented a high incidence of "Not informed" or "Ignored" answers. Last, the mean mortality by self-inflicted injury in this population was 2.0 per 100,000 inhabitants between 2012 and 2021.Conclusions: Strategies must be implemented to reduce the mortality by self-inflicted injury of fertile women from the Rio Grande do Norte state (AU).


Introducción: La violencia autoinfligida es un importante problema de salud pública. Este problema tiene impactos en la salud del individuo, la familia y la comunidad con consecuencias sociales y económicas.Objetivo: Analizar la mortalidad por violencia autoinfligida en mujeres en edad fértil en el estado de Rio Grande do Norte, Brasil, entre los años 2012 y 2021.Metodología: Se trata de un estudio ecológico con enfoque cuantitativo y utiliza como base el estado de Rio Grande do Norte. Los datos fueron recolectados del Departamento de Tecnologías de la Información del Sistema Único de Salud, a través de Información en Salud, en las secciones de estadísticas vitales y población residente con la selección del género femenino y rango de edad de 10 a 49 años. Resultados: Entre los años 2012 y 2021, en el estado de Rio Grande do Norte, se registraron 213 muertes de mujeres en edad fértil por lesiones autoinfligidas. Considerando el inicio y final de este periodo, es posible resaltar que el grupo etario con mayor incidencia de suicidio fue el de 30 a 39 años en 2012 y el de 40 a 49 años en 2021. Se observó, en los años evaluados, que las mujeres eran en su mayoría solteras, de raza mestiza/negra y la propia residencia de la víctima era el lugar predominante para la autolesión. En lo que respecta a la educación y la relación entre muerte y embarazo o puerperio, es necesario resaltar el alto índice de "No informados" e "Ignorados" en los registros. La tasa media de mortalidad por autolesiones en mujeres en edad fértil entre 2012 y 2021 fue de 2,0 muertes por 100.000 habitantes. Conclusiones: Así, se concluye que el escenario de mortalidad por violencia autoinfligida en mujeres en edad fértil en Rio Grande do Norte requiere estrategias para prevenir el suicidio en este rango de edad (AU).

Esta pesquisa adaptou e analisou as evidências de validade da versão simplificada do Deliberate Self-Harm Inventory (DSHI-s) para o português. Participaram 160 estudantes do ensino médio. Aplicou-se um questionário sociodemográfico, o Questionário de Impulso, Autodano e Ideação Suicida na Adolescência (QIAIS-A) e a versão traduzida do DSHI-s (nomeado como Inventário de Autolesão Deliberada - reduzido - IAD-r). A análise dos dados foi conduzida por meio dos softwares SPSS e Factor. O resultado da análise fatorial exploratória indicou uma estrutura final para o IAD-r composta por 15 itens distribuídos em um fator e a escala apresentou boa consistência interna (alfa de Cronbach=0,95; ômega de McDonald=0,95). Constataram-se, também, evidências de validade convergente do IAD-r com o QIAIS-A (índice de correlação=0,775; p<0,001). Portanto, os presentes achados demonstraram evidências de validade do IAD-r, pois foram obtidas características psicométricas satisfatórias para mensuração de comportamentos autolesivos em adolescentes. This research adapted to Portuguese and analyzed the validity evidence of the simplified version of the Deliberate Self-Harm Inventory (DSHI-s). Participants included 160 high school students. A sociodemographic questionnaire, the Questionnaire of Impulse, Self-harm and Suicidal Ideation in Adolescence (QIAIS-A), and the translated version of the DSHI-s (Deliberate Self-Injury Inventory - reduced - IAD-r) were applied. Data analysis was conducted using SPSS and Factor. The result of the Exploratory Factor Analysis indicated a final structure for the IAD-r composed of 15 items and one factor, and the scale showed good internal consistency (Cronbach's Alpha=.95; McDonald's Omega=.95). There was also evidence of convergent validity of the IAD-r with the QIAIS-A (correlation index=.775; p<.001). Therefore, the current findings allow attesting that the IAD-r showed satisfactory validity evidence for measuring self-injurious behaviors in adolescents. (AU)


Esta investigación adaptó y analizó evidencias de validez de la versión simplificada del Deliberate Self-Harm Inventory (DSHI-s) para portugués. Participaron 160 estudiantes de secundaria. Se aplicó un cuestionario sociodemográfico, el Cuestionario de Impulso, Autolesiones e Ideación Suicida en la Adolescencia (QIAIS-A) y la versión traducida del DSHI-s (denominado Inventario de Autolesiones Deliberadas - reducido - IAD-r). El análisis de los datos se realizó con el software SPSS y Factor. El resultado del Análisis Factorial Exploratorio indicó una estructura final para el IAD-r con 15 ítems distribuidos en un factor, y la escala mostró una buena consistencia interna (Alfa de Cronbach=0,95; Omega de McDonald's=0,95). También hubo evidencias de validez convergente del IAD-r con el QIAIS-A (índice de correlación=0,775; p<0,001). Por lo tanto, los presentes hallazgos demuestran evidencias de la validez del IAD-r, ya que se obtuvieron características psicométricas satisfactorias para medir conductas autolesivas en adolescentes. ABSTRACT

It is a type of quantitative documentary research of descriptive and exploratory content in which studied the profile of adolescents with self-injurious behavior and the variables of risk and protection regarding the suicidal intent, reported in a Children and Youth Psychosocial Care Center (CAPS IJ) from a metropolitan region in the south of Brazil. Data from 139 assisted adolescents, admitted for self-injury, reported that self-injuries occurred predominantly at home (M=14,36 years; SD=1,63), with multiple episodes, using sharp objects with suicidal intent. The hierarchical binary logistic regression results point out that experiencing abuse in the present -using non-sharps objects- having severe injuries and practicing them in different places are risk variables. Whereas the existence of community and school social support networks are protective factors. There is a necessity of investment in the studies that explore the etiology of self-injuries and that provide support for cases of prevention, detection, and treatment. (AU)


Esta pesquisa documental quantitativa, de caráter descritivo e exploratório, investigou o perfil de adolescentes com comportamento autolesivo e variáveis de risco e proteção relacionadas à intenção suicida relatada em um Centro de Atenção Psicossocial Infantojuvenil (CAPS IJ) de uma região metropolitana do sul do país. Dados de 139 adolescentes (M = 14,36 anos; DP = 1,63), atendidos por autolesão, indicaram que estas ocorriam predominantemente em ambiente doméstico, com episódios múltiplos, utilização de objetos perfurocortantes e com intenção suicida. Os resultados da regressão logística binária hierárquica indicaram que sofrer violência no presente, utilizar outros objetos que não os perfurocortantes, apresentar lesões graves e praticá-las em locais diferentes são variáveis de risco, enquanto possuir redes de apoio comunitária e escolar são fatores de proteção. É necessário investir em estudos que investiguem a etiologia da autolesão e subsidiem ações de prevenção, detecção e tratamento. (AU)


Esta investigación documental cuantitativa, de carácter descriptivo y exploratorio, investigó el perfil de los adolescentes con conductas autolesivas y variables de riesgo y protección relacionadas con la intención suicida reportada, en un Centro de Atención Psicosocial Infantil y Juvenil (CAPS IJ) de una región metropolitana al sur de Brasil. Los datos de 139 adolescentes (M=14,36 años; DS=1,63), atendidos por autolesión, indicaron que las autolesiones se produjeron en un entorno doméstico, con múltiples episodios, uso de objetos punzantes y con intención suicida. Resultados de la regresión logística binaria jerárquica indicaron que sufrir violencia en el presente, utilizar objetos distintos de los punzantes, presentar lesiones graves y practicarlas en diferentes lugares son variables de riesgo, mientras que tener una red de apoyo comunitario y escolar son factores de protección. Es necesario invertir en estudios que investiguen la etiología de la autolesión y subvencionar acciones de prevención, detección y tratamiento. (AU)

ABSTRACT

El avance de la tecnología es constante, las redes sociales son de uso cotidiano y también una forma de vincularse con otros. La virtualidad llegó para quedarse, y con la pandemia por covid19 su uso se acrecentó. Esto implicó poner en marcha estrategias y herramientas para poder dar continuidad al trabajo clínico con niños y adolescentes. El presente trabajo da cuenta del recorrido terapéutico con una joven que presentaba autolesiones, tricotilomanía y trastornos alimenticios. Ante este cuadro de desvalimiento psíquico de la paciente, se advierten fallas en la constitución de la subjetividad y la perturbación en el sentimiento de estar vivo. A partir del relato de un tipo particular de juego en línea, los juegos de rol, se evidenció un cambio en la actitud de la paciente. La inclusión del material de dichos juegos en el trabajo terapéutico permitió desplegar elementos de su subjetividad AU


Technology's progress is constant. Social networks are the way of connecting with others daily. With the COVID 19 pandemic, virtuality increased noticeably and is here to stay. Different stra-tegies and tools were implemented in order to continue clinical work with children and teena-gers. This article develops the therapeutic work with a young woman who presented self-harm, trichotillomania (pulling out hair) and eating disorders. Given the patient ́s state of psychic hel-plessness, flaws in the constitution of subjectivity were noted, along with the disturbance in the feeling of being alive. Her sessions and stories were focused on role-playing online games. The inclusion of the mate-rial of those games in the therapy allowed to display elements of her subjectivity. Since then, a change in the patient's attitude was evidenced AU


L'avancement de la technologie est constant. Les réseaux sociaux sont le moyen quotidien de se connecter avec les autres. Avec la pandémie de COVID 19, la virtualité s'est développée rapidement et est là pour rester. Différentes stratégies et outils ont été mis en place afin de poursuivre le travail clinique auprès des enfants et des adolescentes. Cet article développe le travail thérapeutique auprès d'une jeunefemme qui présentait de l'automutilation, de la trichotillomanie (arracher les cheveux) et des troubles alimentaires. Face à ce tableau d'impuissance psychique du patient, on constate des défaillances dans la constitution de la subjectivité et des perturbations dans le sentiment d'être vivant.Ses sessions et ses histoires se sont concentrées sur les jeux de rôle en ligne. L'inclusion de ces jeux dans le travail thérapeutique a permis de montrer des éléments de leur subjectivité. De ces histoires de jeux, un changement dans l'attitude du patient était évident AU

Diferentes estratégias e ferramentas foram implementadas para dar continuidade ao tra-balho clínico com crianças e adolescentes. O presente trabalho desenvolve a jornada tera-pêutica com uma jovem que apresentava automutilação, tricotilomania (arrancar o cabelo) e transtornos alimentares. Diante desse quadro de desamparo psíquico do paciente, observam-se falhas na constituição da subjetividade e perturbação no sentimento de estar vivo. Suas sessões e histórias se concentravam em jogos de RPG online. A inclusão desses jogos no trabalho terapêutico permitiu evidenciar elementos de sua subjetividade. ABSTRACT

INTRODUCCIÓN: El Psicodiagnóstico interventivo, basado en las Consultas Terapéuticas propuestas por Winnicott, involucra, desde la primera sesión, intervenciones clínicas. Dado su empleo en diferentes contextos y poblaciones, instrumentos proyectivos como el Test de Apercepción Temática Infantil con Figuras Humanas son utilizados con la finalidad de explorar y comprender el mundo vivencial, y aspectos relevantes de la personalidad de niños y adolescentes, apoyando el trabajo clínico con adolescentes. OBJETIVO: Introducir, por medio del análisis del Psicodiagnóstico interventivo con la ayuda del CAT-H, la ilustración clínica de una adolescente con comportamiento de autolesión no suicida. MÉTODO: Se trata de un estudio clínico-teórico, con delineamiento de estudio de caso. Los datos fueron analizados a partir de la libre inspección del material y a la luz del abordaje psicoanalítico winnicottiana. RESULTADOS: Se evidenció el principal conflicto interno experimentado por el participante: sumisión frente al deseo de hacer lo que es propio de la época, así como la autoimagen vulnerable y el empleo de mecanismos de defensa primitivos dada la intensa sensación de impotencia. CONCLUSIÓN: El material clínico descrito presenta el uso del test más allá de su posibilidad diagnóstica, en calidad de mediador terapéutico, colaborando para la profundización en la queja y ofreciendo una experiencia de acogida y reflexión a la paciente, de manera similar la oferta del juego del garabato de Winnicott, es decir estableciendo contacto el campo emocional.


INTRODUÇÃO: O psicodiagnóstico interventivo, baseado nas Consultas Terapêuticas propostas por Winnicott, envolve, desde a primeira sessão, intervenções clínicas. Devido ao seu emprego em diferentes contextos e populações, instrumentos projetivos como o Teste de Apercepção Infantil com Figuras Humanas são utilizados para explorar e compreender o mundo vivencial, e aspectos relevantes da personalidade do público infanto-juvenil, subsidiando a atuação clínica com adolescentes. OBJETIVO: Apresentar a análise do psico diagnóstico interventivo com o auxílio do teste por meio da ilustração clínica de um adolescente com comportamento de autolesão não suicida. MÉTODO: Trata-se de um estudo teórico-clínico, com delineamento de estudo de caso. Os dados foram analisados a partir da livre inspeção do material e à luz da abordagem psicanalítica winnicottiana. RESULTADOS: Evidenciou-se o principal conflito interno vivenciado pelo participante: submissão versus desejo de fazer o que é próprio para a idade, assim como autoimagem vulnerável e emprego de mecanismos primitivos de defesa diante do intenso sentimento de desamparo. CONCLUSÃO: O material clínico descrito apresenta a utilização do teste além de sua possibilidade diagnóstica, como mediador terapêutico, colaborando para aprofundar a queixa e oferecendo uma experiência de acolhimento e reflexão ao paciente, à semelhança da proposta do jogo de rabiscos de Winnicott, ou seja, estabelecendo contato com o campo emocional.


INTRODUCTION: The interventive psychodiagnosis, based on the Therapeutic Consultations proposed by Winnicott, involves, from the first session, clinical interventions. Given its use in different contexts and populations, projective instruments such as the Children's Apperception Test with Human Figures are used in order to explore and understand the living world, and relevant aspects of the personality of children and adolescents, supporting clinical work with adolescents. OBJECTIVE: To present the analysis of the interventive psychodiagnosis with the help of the test through the clinical illustration of an adolescent with non-suicidal self-injury behavior. METHOD: This is a clinical-theoretical study, with a case study design. Data were analyzed from the free inspection of the material and in the light of the Winnicottian psychoanalytic approach. RESULTS: The main internal conflict experienced by the participant was evidenced: submission versus the desire to do what is proper for the age, as well as vulnerable self-image and employment of primitive defense mechanisms given the intense feeling of helplessness. CONCLUSION: The clinical material described presents the use of the test in addition to its diagnostic possibility, as a therapeutic mediator, collaborating to deepen the complaint and offering an experience of welcoming and reflection to the patient, similarly to the proposal of Winnicott's scribble game, which is, establishing contact with the emotional field.

ABSTRACT

OBJECTIVES@#To investigate the differences in non-suicidal self-injury (NSSI) behaviors between only-child and non-only-child adolescents with mood disorders.@*METHODS@#A three-stage sampling method was used to perform a cross-sectional survey of 529 adolescents, aged 12-18 years, who had mood disorders and NSSI behaviors. These adolescents were sampled from the outpatient service of 20 mental hospitals in 9 provinces of China from August to November 2020. A self-made questionnaire was used to collect general demographic data. The Functional Assessment of Self-Mutilation, Beck Scale for Suicide Ideation, Kessler Psychological Distress Scale, Stress Mindset Measure-General, Multidimensional Scale of Perceived Social Support, Multidimensional Students' Life Satisfaction Scales, and Rosenberg Self-Esteem Scale were used to collect the information on self-injury behaviors and psychological factors in these adolescents.@*RESULTS@#A total of 529 adolescents with mood disorders and NSSI behaviors were surveyed, among whom 375 were only-child adolescents and 154 were non-only-child adolescents. Compared with the non-only-child group, the only-child group had a significantly higher total score of Functional Assessment of Self-Mutilation (P<0.05) .The type and frequency of self-injury in the only-child group were significantly higher than those in the non-only-child group (P<0.05). Psychological analysis showed that compared with the non-only-child group, the only-child group had a significantly lower score of self-esteem (P<0.05) and significantly higher scores of psychological distress and depressive symptoms (P<0.05). The multiple linear regression analysis showed that the score of suicidal ideation was positively correlated with the frequency of NSSI behaviors in both only-child and non-only-child adolescents with mood disorders (P<0.05); in the only-child adolescents, the level of self-esteem was negatively correlated with the frequency of NSSI behaviors (P<0.05), and the score of stress perception was positively correlated with the frequency of NSSI behaviors (P<0.05); in the non-only-child adolescents, the score of anxious emotion was positively correlated with the frequency of NSSI behaviors (P<0.05).@*CONCLUSIONS@#Among the adolescents with mood disorders and NSSI behaviors, the only-child adolescents tend to have a higher frequency of self-injury and poorer mental health, and therefore, the only-child adolescents with mood disorders and NSSI behaviors need more attention.

OBJECTIVES@#Adolescents are at high risk of non-suicidal self-injury (NSSI). Currently, there is no clinical assessment tool for adolescent NSSI behaviors measurement with global consistency. The Ottawa Self-injury Inventory (OSI) is considered as a relatively comprehensive assessment tool for NSSI, but the questionnaire is discussed with excessive content and timecostly, which may affect the reliability of the measurement results for adolescent.Thus, this study, based on OSI, aims to revise the assessment tool for adolescent with NSSI that is suitable for both clinically and scientifically, referring to the diagnostic criteria for NSSI in the 5th Diagnostic and Statistical Manual of Mental Disorder (DSM-5).@*METHODS@#This study was led by the Second Xiangya Hospital of Central South University and collaborated with 6 mental health service institutions in China from August to December 2020. Adolescent aged from 12 to 24 years old who had self-injury behavior and met the DSM-5 diagnostic criteria for NSSI were continuously recruited in the psychiatric outpatient department or ward. After clinical diagnosis by an experienced attending psychiatrist or above, the general information and OSI were collected by questionnaires. SPSS 24.0 and AMOS structural equation model statistical softwares were used to conduct item analysis and exploratory factor analysis on the obtained data to complete the revision of the scale. Cronbach's alpha coefficient, split-half reliability, test-retest reliability, and content validity and structure validity were performed to analyze the reliability and validity and confirmatory factor analysis was carried out to test the structure validity for the revised scale.@*RESULTS@#A total of 234 adolescent with NSSI were enrolled, including 33 (14.1%) males and 201 (85.9%) females with the mean age of (16.2±2.6) years old. The most common clinical diagnoses were depression disorder (57.4%), bipolar disorder (20.9%), adolescent mood disorder (17.1%), etc. Nine items (item 2, 7, 11, 13, 23, 24, 10, 17, 18) in the functional scale of OSI were deleted according to extreme grouping method, correlation analysis, and principal component analysis in exploratory factor analysis. The revised functional scale for NSSI consisted of 15 items. The reliability analysis showed that the Cronbach's alpha coefficients of NSSI thought and behavior frequency, addiction characteristics, and function scales were 0.799, 0.798, and 0.835, respectively, and the split-half coefficients were 0.714, 0.727, and 0.852, respectively. The test-retest coefficients of the latter 2 scales were 0.466 and 0.560, respectively. The correlation coefficient between sub-items and total scores in each part of the scale showed good content validity. The exploratory factor analysis showed that a component was extracted from the frequency of thoughts and behaviors of NSSI, one component was extracted from the addictive characteristics, and three components were extracted from the functional part. The three functional subscales were social influence, external emotion regulation, and internal emotion regulation. The factor load of each item was >0.400.@*CONCLUSIONS@#The revised Chinese version OSI targeted the adolescent patients with mental disorders has relatively ideal reliability and validity. The scale shows high stability, dependability, and a reasonable degree of fit. It is a suitable assessment tool for clinical and scientific research on adolescent with NSSI.

ABSTRACT

Abstract Objective: to develop and validate educational material to strengthen adolescent health care on non-suicidal self-injury. Method: methodological research designed in three stages: (1) construction of the material based on a mixed study on needs related to the theme through social networks and an umbrella review on health care related to non-suicidal self-injury; (2) validation with 10 experts in mental health and/or self-inflicted violence selected through the Lattes Platform; (3) evaluation by the target public, with health professionals being invited, without restriction of training. Validation and evaluation data were collected by using a sociodemographic questionnaire and the Suitability Assessment of Materials for evaluation of health-related information for adults. We used descriptive statistics, content validity index, and Gwet's AC1 test. Results: the material obtained good general acceptance and reliability in the validation by the experts (AC1= 0.633; p=0.0000) and in the evaluation by the target public (AC1=0.716; p=0.0000). All professionals pointed out the personal contribution and educational potential of the material. Conclusion: we highlight the construction of science-based educational material to strengthen the health care for adolescents with non-suicidal self-injury.


Resumo Objetivo: elaborar e validar um material educativo para fortalecer a assistência em saúde aos adolescentes sobre a autolesão não suicida. Método: pesquisa metodológica delineada em três etapas: (1) construção do material a partir de estudo misto sobre necessidades ligadas à temática por meio das redes sociais e uma revisão guarda-chuva sobre a assistência relacionada à autolesão não suicida; (2) validação com 10 especialistas em saúde mental e/ou violência autoprovocada selecionados pela Plataforma Lattes; (3) avaliação pelo público-alvo, sendo convidados profissionais de saúde, sem restrição de formação. A coleta dos dados de validação e avaliação foi realizada por um questionário sociodemográfico e o Suitability Assessment of Materials for evaluation of health-related information for adults. Foi empregada a estatística descritiva, índice de validade de conteúdo e o teste AC1 de Gwet. Resultados: o material obteve boa aceitação geral e confiabilidade na validação pelos especialistas (AC1=0,633; p=0,0000) e na avaliação pelo público-alvo (AC1=0,716; p=0,0000). Todos os profissionais apontaram a contribuição pessoal e potencial educativo do material. Conclusão: destaca-se a construção de material educativo embasado em ciência para o fortalecimento da assistência aos adolescentes com autolesão não suicida.

ABSTRACT

Self-harm is a prevalent adolescence behavior, indicated as a public health problem. The objective was to understand how adolescents perceive and explain this behavior, considering family and school characteristics. It's a multiple case study, in which five self-harming adolescents participated, enrolled in a public school in Novo Hamburgo-RS, participated in the study. They answered the following instruments: Sociodemographic Data Sheet, Semi-Structured Interview, Family Support Perception Inventory, Youth Self-Reporting and Delaware School Climate Survey-Student. Each case was individually analyzed and cross-case synthesis was performed. Self-harm was indicated as a strategy to relief suffering and regulate emotions. Low familial support, lack of skills, and presence of internalizing and externalizing problems were evidenced, as well as negative evaluation of peer relations and student engagement. When faced with a complex phenomenon, it is important to contemplate individual and relational issues in interventions. (AU)


A autolesão é prevalente na adolescência e indicada como um problema de saúde pública. Objetivou-se compreender como adolescentes percebem e explicam esse comportamento, considerando características de seu contexto familiar e escolar. Trata-se de um estudo de casos múltiplos, do qual participaram cinco adolescentes que se autolesionavam, matriculados em uma escola pública de Novo Hamburgo-RS, que responderam aos instrumentos: Ficha de Dados Sociodemográficos, Entrevista Semiestruturada, Inventário de Percepção de Suporte Familiar, Inventário de Comportamentos Autorreferidos para Jovens de 11 a 18 Anos e Delaware School Climate Survey-Student. Analisou-se individualmente cada caso e realizou-se a síntese de casos cruzados. A autolesão foi indicada como uma estratégia para aliviar sofrimento. Evidenciou-se baixo suporte familiar, carência de competências e presença de indicadores de problemas internalizantes e externalizantes, assim como avaliação negativa das relações entre pares e do engajamento estudantil. Frente a um fenômeno complexo, faz-se importante contemplar questões individuais e relacionais nas intervenções. (AU)


La autolesión es frecuente en adolescencia y se indica como un problema de salud pública. El objetivo era comprender cómo adolescentes perciben y explican este comportamiento, considerando características de su contexto familiar y escolar. Es un estudio de caso múltiple, en el participaron cinco adolescentes con autolesión, matriculados en escuela pública en Novo Hamburgo-RS, que respondieron a los instrumentos: Hoja de Datos Sociodemográficos, Entrevista Semiestructurada, Inventario de Percepción de Apoyo Familiar, Auto informe para Jóvenes de 11 a 18 años y Delaware School Climate Survey-Student. Cada caso se analizó individualmente y se realizó una síntesis cruzada de casos. La autolesión se indicó como estrategia para aliviar el sufrimiento. Se evidenció poco apoyo familiar, falta de habilidades y presencia de indicadores de problemas de internalización y externalización, así como una evaluación negativa de las relaciones con los compañeros y la participación de los estudiantes. Ante un fenómeno complejo, es importante contemplar problemas individuales y relacionales en las intervenciones. (AU)

Resumen Hace muchos años se describió el ahora conocido efecto copycat. Se entiende como la reproducción del acto autolesivo, por parte de personas susceptibles, posterior a la exposición a la información en los medios de comunicación relacionada con un suicidio concretado (en noticias, redes sociales, etc.). A partir de la observación en la atención psicológica y psiquiátrica de los pacientes, nuestro grupo ha detectado que esta replicación en realidad forma parte de un espectro mucho más amplio, y que, consecuentemente, otras manifestaciones suelen pasar inadvertidas. Si se trazara un continuum de esas expresiones, en el extremo más severo, se encontraría el suicidio consumado luego de la exposición a dicho material, que es el componente más reconocido hasta la fecha (efecto copycat); no obstante, en otras zonas del espectro, también es posible que las personas experimenten distorsiones cognitivas, componentes obsesivos e intrusivos, ideación pasiva y activa de muerte e intentos no consumados de suicidios. El presente documento se propuso como objetivo describir este fenómeno y, a la vez, ejemplificarlo mediante la exposición de un caso. Resulta probable que el correcto reconocimiento y abordaje fenomenológico sean pasos básicos para establecer los tratamientos y las estrategias preventivas, que garanticen la seguridad de estas personas


Abstract: Long ago, the scientific literature informed about the now well-known copycat effect, understood as a reproduction of the self-injurious act by susceptible individuals after being exposed to suicide-related information in the media (news, social networks, etc.). From observations in psychological and psychiatric care of patients, our group has detected that these manifestations are actually part of a much broader clinical spectrum, where other manifestations can also be found. It would seem plausible that if a spectrum is defined, the most severe end would be consummated suicide, which is the well-recognized component to date (copycat effect); but it is also possible that there is a series of more diverse expressions in other patients after having been exposed to this information, which could include cognitive distortions, obsessive and intrusive phenomena, passive and active ideation of death and unsuccessful suicide attempts. Our purpose with this paper is to explain our observations and present a clinical vignette as an example. We believe that correct recognition and approach to these phenomena is fundamental to establish treatments and preventive strategies that guarantee the safety of this population.

O autor considera que o alarmante crescimento do número de suicídios de adolescentes constitui a ponta de um iceberg muito mais amplo: o sofrimento profundo, muitas vezes naturalizado, das gerações mais jovens. É um sofrimento em que o olhar sociológico costuma verificar o fracasso dos adultos na transmissão de um legado que permita a inclusão dos recém-chegados no corpo social. Nós, psicanalistas, registramos - paradoxalmente ­ um êxito nessa conjuntura inquietante: aquele que corresponde às tendências filicidas e destrutivas para com os jovens, as quais autores como Raskovsky, Winnicott e Lacan exploraram em suas análises. São examinadas algumas peculiaridades sintomáticas da adolescência atual. Examina-se a orientação fornecida por algumas observações de Freud e Lacan com o intuito de acomodá-las e processá-las no dispositivo analítico (AU)


The author considers that the alarming growth in the number of adolescent suicides is the tip of a much larger iceberg: the deep suffering, often accepted as natural, of the younger generations. ABSTRACT

Resumo O presente ensaio busca discutir as implicações do isolamento social devido à pandemia do COVID-19 para o uso intensivo da internet entre crianças e adolescentes e suas possíveis consequências para a prática de violências autoinflingidas. Discutimos brevemente o potencial ansiogênico e a reprodução de um "medo global" que se consolidam com a exposição maciça e sem mediação dos conteúdos consumidos, que podem aumentar as vulnerabilidades para estresse e ideações suicidas. Centramos nosso debate sobre práticas "recreativas", denominadas de "desafios" com poder autolesivo, realizados por adolescentes no site Youtube. Essa prática revelou-se crescente a partir das medidas de isolamento social. Nossa reflexão sobre esses riscos é feita a partir da perspectiva teórica da sociabilidade digital, e suas implicações nas interações de adolescentes mediadas pela internet.


Abstract This essay aimed to discuss the implications of social isolation due to the COVID-19 pandemic for the intensive use of the internet among children and adolescents and its possible consequences for the practice of self-inflicted violence. We briefly discussed the anxiogenic potential and the reproduction of a "global fear" that are consolidated with the massive and unmediated exposure of the content consumed, which can increase the vulnerabilities to stress and suicidal ideas. We centered our debate on "recreational" practices, called "challenges" with self-harm power, carried out by teenagers on the YouTube website. This practice has been shown to increase with the social isolation measures. Our reflection on these risks builds on the theoretical perspective of digital sociability, and its implications for the internet-mediated interactions of adolescents.

RESUMO. O suicídio pode ser compreendido como o desfecho decorrente de uma complexa interação de fatores e seu resultado gera intensos impactos na família e em toda sociedade. Considerando-se que para cada suicídio estima-se que cerca de 100 pessoas sejam afetadas, recebendo pouco ou nenhum suporte, e que o luto por suicídio tem especificidades que podem complicar o processo; a tarefa da intervenção após um suicídio se amplia como imprescindível. Este artigo descreve e fundamenta o processo de construção e manejo por iniciativa voluntária de profissionais da psicologia, de um grupo de apoio para sobreviventes/enlutados pelo suicídio na cidade de Maringá-PR, iniciado em setembro de 2016, com frequência mensal e gratuidade na participação. Os grupos de apoio representam recursos fundamentais de suporte emocional na posvenção, sendo considerados como espaço de escuta, reconhecimentos, legitimação e apoio a pessoas enlutadas ou intensamente impactadas pelo suicídio, assim, possibilitando a construção de sentidos para a perda e uma adaptação ao processo continuado de ressignificação.


RESUMEN. El suicidio puede ser comprendido como el desenlace indeseable resultante de una compleja interacción de factores y su resultado genera intensos impactos en la familia y toda sociedad. Considerando que para cada suicidio cerca de 100 personas son afectadas, aún así, éstas reciben poco o ningún soporte, y aún, que luto por suicidio tiene especificidades que pueden complicar el processo; la acción de la intervención después de un suicidio se amplía como imprescindible. Este artículo describe y fundamenta el proceso de construcción y manejo, por iniciativa voluntaria de profesionales de la Psicología, de un grupo de apoyo para sobrevivientes/enlutados del suicidio en la ciudad de Maringá-PR, Brasil, iniciado en septiembre de 2016 y mantenido con frecuencia mensual y gratuidad en la participación. Los grupos de apoyo representan recursos fundamentales de soporte emocional en la posvention, siendo considerados como espacio de escucha, reconocimiento, legimitación y apoyo a personas enlutadas o intensamente impactadas por el suicídio, así, posibilitando la construcción de sentidos para la pérdida y una adaptación al proceso continuado de resignificación.


ABSTRACT. Suicide can be understood as the undesirable outcome resulting from a complex interaction of factors, and its outcome generates intense impacts on the family and society as a whole. Considering that for each suicide about 100 people are affected, even so, these receive little or no support, and still, that mourning/grief for suicide has specificities that can complicate the process; the action of the intervention after a suicide is amplified as essential. This article describes and bases the process of construction and management, through a voluntary initiative of Psychology professionals, a support group for suicide survivors / bereaved in the city of Maringá, PR/Brazil, started in September 2016 and maintained monthly and free participation. The support groups represent fundamental resources of emotional support in the posvention, being considered space of listening, recognition, legitimation and support to people mourning/grief or intensely impacted by suicide, thus making it possible to construct meanings for loss and an adaptation to the ongoing process of re-signification.

La victimización de pares es un importante factor de riesgo para la salud mental de los adolescentes. Uno de sus correlatos más graves son las autolesiones y la ideación suicida. Pocos estudios, sin embargo, han examinado si la victimización predice las autolesiones y la ideación suicida, controlando el nivel de depresión. Se constituyó una muestra intencional de 532 adolescentes de Paraná, Argentina. El 41% era varón (Medad = 13.8 años, DE=1.6). El 69% de los adolescentes era testigo, el 19%, reportaba victimización, el 5%, reportaba agresión y el 7%, ambos. Las víctimas y el grupo de ambos presentaban altos niveles de autolesiones e ideación suicida. El género introducía diferencias en las autolesiones y la ideación suicida. La victimización resulta un predictor significativo de las autolesiones y la ideación suicida, luego de controlar el nivel de depresión de los adolescentes, prediciendo el 7% y 5% de su varianza, respectivamente


Peer victimization is a significant risk factor for adolescent mental health. One of the most severe correlatives of victimization is selfharm and suicidal ideation. Few studies, however, have examined whether victimization predicts self-harm and suicidal ideation above the level of previous depression. A sample of 532 teenagers was taken from three public middle schools in the city of Paraná, Entre Ríos. 41% were male, and the average age was 13.8 years. The results suggested that 69% were not involved, 19% victim, 5% perpetrator and 7% both. The victims of bullying and the group of both had a higher level of self-harm and suicidal ideation compared to those not involved. Females score higher than males in self-harm and suicidal ideation. Victimization was a predictor of self-harm and suicidal ideation over depression

Abstract Background Skin picking disorder is a prevalent disorder frequently comorbid with depression and anxiety, which is underdiagnosed mainly by dermatologists. Assessment of skin picking disorder is based on instruments influenced by the awareness about skin picking disorder and comorbid symptoms. To date, there is no validated instrument for Brazilian individuals nor an instrument to evaluate the severity of skin lesions in an objective way. Objectives Validate the Skin Picking Impact Scale for Brazilian Portuguese and create a photographic measurement to assess skin lesions. Methods The sample was assessed through the Skin Picking Impact Scale translated into Brazilian Portuguese, the Hamilton Anxiety Scale, the Beck Depression Inventory, and the Clinical Global Impression Scale. The patients' skin lesions were photographed. Photos were evaluated regarding active excoriation, crust/bleeding, exulceration, and linear lesions. Results There were 63 patients included. The Skin Picking Impact Scale translated into Brazilian Portuguese had good internal consistency (Cronbach's alpha = 0.88), which tests of goodness-of-fit, showing a suitable model. The reliability of photographic measurement was 0.66, with a high internal consistency (Cronbach's alpha = 0.87). Photographic measurement was not correlated with the Skin Picking Impact Scale, the Clinical Global Impression Scale, or comorbid symptoms. Study limitations Lack of a previously validated instrument to evaluate dermatillomania in the Brazilian population for comparison. Conclusion The Skin Picking Impact Scale validated in Brazilian Portuguese is a good instrument to evaluate skin picking disorder. Photographic measurement is a consistent way of assessing skin lesions, but it does not reflect the impact of skin picking disorder on the individual's life.

ABSTRACT

Las conductas lesivas no suicidas suponen un problema sanitario de creciente importancia, con una prevalencia en muestras comunitarias de adolescentes del 15-20 %. Las autolesiones pueden tener un significado diverso; se clasifican en intrapersonales o interpersonales; son un factor de riesgo y evolutivo, con inicio en la adolescencia temprana. Se presenta el caso de una adolescente de 12 años con factores de riesgo psicosociales, que acudió a la consulta por cortes autoinfligidos en los brazos y las piernas, realizados con una cuchilla de afeitar, detectados en el colegio. Se intervino potenciando la autoestima y en regulación de emociones con intervención multidisciplinar (sistema educativo, servicios sociales, psiquiatría infantojuvenil y pediatría), con una evolución positiva. Es vital su detección y abordaje con el paciente y su familia para evitar riesgos futuros, especialmente, de patología psíquica. La intervención se realiza desde la Atención Primaria, pero precisa la colaboración de otros profesionales.


Non-suicidal harmful behaviors pose a health problem of increasing importance, with a prevalence in community samples of adolescents of 15-20 %. Self-harm can have adverse meaning, qualifying in intrapersonal or interpersonal; they are a risk and evolutionary factor, with an age of onset in early adolescence. We present the case of a 12-year-old adolescent with psychosocial risk factors, who went to the consultation for self-cuts in arms and legs, made with a razor, detected in the school. It was enhanced her self-steem an guided in regulation of emotions with multidisciplinary intervention (educational system, social services, infantile-juvenile Psychiatry and Pediatrics), with a positive evolution. It is vital to detect and approach this issue with the patient and the family to avoid future risks, especially psychic pathology. ABSTRACT

Dentro de los daños intencionales en adolescentes, las autoagresiones se han convertido en los últimos años en un tema de interés clínico, social y público, en constante investigación. Una conducta autolesiva es toda conducta autodeliberada destinada a producir daño físico directo en el cuerpo, sin la intención de provocar la muerte. Objetivos: Describir los casos de pacientes con autoagresiones que fueron atendidos por el equipo de adolescencia (clínica pediátrica y salud mental) en el Hospital Garrahan desde el año 2015 al 2017. Diseño observacional, descriptivo y retrospectivo. Se incluyeron todos los adolescentes que fueron evaluados en conjunto por el servicio de adolescencia y salud mental. Resultados: Se atendieron 17 casos, rango de edad 12 a 16 años, 14 mujeres. Los motivos de consulta fueron variados, el principal mecanismo de autoagresión elegido fue cortes superficiales en antebrazo y piernas. La mayoría presentó como factor de riesgo principal conflictos familiares, coincidiendo con la bibliografía revisada. Un abordaje integral por personal capacitado en un servicio amigable es necesario para no perder la oportunidad de pesquisar estas situaciones. Así, mediante un trabajo interdisciplinario disminuir los riesgos que conllevan estas conductas (AU)


Within intentional harm in adolescents, over the last years selfinjury has become an issue of clinical, social, and public interest in ongoing research. Self-injurious behavior is all deliberate behavior aimed at producing direct physical harm to the body, without the intention to cause death. Objectives: To describe cases of patients with self-injurious behavior seen by the adolescents team (pediatrics and mental health) at Garrahan Hospital between 2015 and 2017. A retrospective, observational, descriptive study was conducted. All adolescents seen together by the Departments of Adolescence and Mental Health were included in the study. Results: 17 patients were evaluated, with ages ranging from 12 to 16 years; 14 were girls. Main complaints were varied. The main mechanism of self-injury were superficial cuts on the forearms and legs. The main risk factor in the majority of patients were family conflicts, as reported in the literature. A comprehensive approach by trained personnel from a friendly team is necessary so as not to lose the opportunity to screen this type of situations. An interdisciplinary approach may reduce the risks associated with these behaviors (AU)

ABSTRACT

Objective: To investigate the clinical differences between intermittent explosive disorder (IED) (disorder of aggression primarily directed towards others) and nonsuicidal self-injury (NSSI) (disorder of aggression predominantly directed towards the self) in order to better understand the different clinical subtypes of aggression. Methods: We used treatment-seeking samples to compare demographic and clinical correlates between 82 participants with IED and 55 participants with NSSI. Results: The IED group was older, more likely to be male, in a relationship, and employed than the NSSI group. With respect to clinical variables, the NSSI group had more severe depressive symptoms and more social adjustment difficulties. Regarding psychiatric co-morbidities, the IED group had higher rates of generalized anxiety disorder. On the other hand, the NSSI group had higher rates of major depressive disorder, agoraphobia, substance use disorder, and bulimia nervosa. Conclusions: Individuals with NSSI may benefit from better management of psychiatric comorbidities, specifically depressive symptoms and social adjustment difficulties. Conversely, the treatment of individuals with IED may be improved by targeting comorbid generalized anxiety disorder. Our results provide important insight for the development of tailored interventions for specific subtypes of aggression.

Resumen El propósito de esta revisión sistemática fue identificar los instrumentos creados o adaptados para evaluar la autolesión no suicida en adolescentes. Se utilizó la metodología PRISMA. Dos revisores independientes analizaron las propiedades psicométricas de los instrumentos publicados en inglés o español desde 1990 a 2016 considerando criterios de calidad estandarizados. Las bases utilizadas fueron PsycINFO, PubMed, ISI Web of Knowledge, Scopus, SciELO, ScienceDirect y EBSCO. Se seleccionaron 18 estudios que crearon o adaptaron 11 instrumentos. La mayoría desarrollados en Estados Unidos y Canadá y ninguno en América Latina. Varios estudios no presentan evidencia de las propiedades psicométricas de los instrumentos. Siete de los 18 estudios obtuvieron al menos una puntuación positiva. El ABUSI y el ISSIQ-A obtuvieron la mayor cantidad de puntuaciones positivas. Las limitaciones de este estudio fueron que la búsqueda se realizó solamente en siete bases de datos y en los idiomas inglés y español. Se sugiere mejorar el reporte de las propiedades psicométricas de los instrumentos de autolesión no suicida en adolescentes y desarrollar adaptaciones a países de América Latina que permitan hacer comparaciones internacionales.


Abstract The purpose of this systematic review was to identify the instruments created or adapted to assess non-suicidal self-injury (NSSI) among adolescents. The Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) methodology was used. Two individual reviewers analyzed the psychometric properties of instruments published in English or Spanish from 1990 to 2016 considering standardized quality criteria. The PsycINFO, PubMed, ISI Web of Knowledge, Scopus, SciELO, ScienceDirect, and EBSCO databases were consulted. Eighteen studies that created or adapted 11 instruments were selected. Most were developed in the United States or Canada, and none were developed in Latin America. Several studies presented no evidence of the psychometric properties of their instruments. Seven of the 18 studies obtained at least one positive score. The Alexian Brothers Urge to Self-Injure Scale (ABUSI) and the Impulse, Self-harm, and Suicide Ideation Questionnaire for Adolescents (ISSIQ-A) obtained the highest positive scores. The limitation of this study is that only seven databases were employed for the literature search in English and Spanish. The reporting of the psychometric properties of NSSI instruments among adolescents should be improved, and adaptations to Latin American countries should be developed for international comparisons.
