ABSTRACT

Introducción: El colgajo de reposición coronal constituye una técnica ventajosa para el tratamiento de la recesión periodontal. Cuando esa técnica es combinada con diferentes biomateriales los resultados son superiores desde el punto de vista de la cobertura radicular y la estabilidad en el tiempo. Objetivo: Evaluar la efectividad de la membrana de fibrina rica en plaquetas combinada con el colgajo de reposición coronal en la cobertura radicular. Métodos: Se realizó una investigación cuasiexperimental con pacientes atendidos en la consulta de Periodoncia de la Facultad de Estomatología de Villa Clara durante el periodo comprendido entre marzo del 2017 y mayo del 2019. La población de estudio estuvo constituida por 41 pacientes que presentaban recesión periodontal y requerían recubrimiento radicular mediante la técnica del colgajo de reposición coronal. Se empleó un muestreo no probabilístico intencional por criterios y la muestra quedó constituida por 26 pacientes que cumplieron los criterios establecidos para el estudio. Se establecieron sitios de estudio y sitios de control. Las variables estudiadas fueron: edad, sexo, extensión longitudinal de la recesión, cobertura radicular, clasificación de Miller, anchura y ganancia de la encía insertada, categorías de evaluación y efectividad del tratamiento. Resultados: En el 92,45 por ciento de los dientes ubicados en los sitios de estudio predominó la categoría de evaluación favorable. El porcentaje de efectividad del tratamiento fue superior en los sitios de estudio en un 33,96 por ciento con relación a los controles. Conclusiones: Se consideró más efectivo el tratamiento de la membrana de fibrina rica en plaquetas asociada al colgajo de reposición coronal(AU)


Introduction: Coronally repositioned flap constitutes an advanced technique for the treatment of periodontal recession. When combined with different biomaterials the results are much more encouraging regarding root coverage and stability in time. Objective: to assess the effectiveness of the platelet-rich fibrin membrane combined with coronal repositioned flap in the root coverage. Methods: A quasi-experimental research was carried out in patients treated at the periodontal office of the Dentistry faculty in Villa Clara province from March 2017 to May 2019. The population was constituted by 41 patients who suffered from periodontal recession and required root coverage using coronally repositioned flap. An intentional non-probabilistic sample was used resulting in 26 patients with the established criteria. Study and control sites were established. Variables such as age, sex, recession length, root coverage, Miller's classification, width and gaining of the attached gingiva, assessment categories and effectiveness of treatment were studied. Results: A favorable assessment category was predominant in 92,45 percent of teeth located in the study sites. The percentage of effectiveness of treatment was higher in the study sites in 33,96 percent in relation to the control sites . Conclusions: The treatment of the platelet-rich fibrin membrane associated with coronally repositioned flap was considered more effective(AU)

SUMMARY OBJECTIVE To assess the patient's understanding of the informed consent form before and after plastic surgery. METHODS This was a prospective analytical descriptive study that utilized a questionnaire on informed consent before and after plastic surgery procedures. RESULTS Comprehension of informed consent was higher before surgery than after surgery (p=0.016; question 15). The higher the scholarity, the higher the comprehension (s=0.151; p=0.045) before surgery (question 4). For the other questions, it was not possible to find a difference in the pattern of understanding and in the association with the educational attainment level after surgery (s=0.180; p=0.046; question 1). CONCLUSIONS: The patients' level of comprehension of the details, outcomes, possible complications, and postoperative evolutions of surgical procedures, as stated by the informed consent form, is high.

Summary OBJECTIVE: This study aimed to compare the publications authored by plastic surgeons with those from other specialties' surgeons on patient-reported outcomes of oncoplastic surgery. METHODS: A review was carried out on the Medline database, emcompassing five years (2015-2020). Studies about partial breast reconstruction after conservative treatment, immediate or delayed, by any technique, which presented patient-reported outcomes, were included. RESULTS: We found 292 articles, from which 142 met the eligibility criteria. Publications were stratified into groups 1 (plastic surgeons) and 2 (other surgical specialties), and also into groups A (only plastic surgeons), B (only other specialties) and C (both), and compared statistically. Most publications (60.6%) were attributed to specialties other than plastic surgery. Nineteen percent had only plastic surgeons as authors, 50% only other specialties' surgeons, and 31% had both. There was no difference between groups regarding the impact factor of the journals in any of the stratifications, and the majority was published in journals with impact factor ≤2. CONCLUSION: In the last years, surgeons from specialties other than plastic surgery published more about the results of the oncoplastic surgery reported by the patients. There was no statistical difference between the groups regarding the impact factor of the journals.

Las quemaduras químicas en cuero cabelludo, que se producen en lugares públicos como salones de belleza o peluquerías causadas por mezclas de sustancias activas como persulfatos y peróxido de hidrógeno, secundarias a la decoloración de cabellos, producen graves secuelas de alopecias en pacientes jóvenes. Se trata de un caso clínico, de quemadura química, espesor completo, extensa, en cuero cabelludo. Productos utilizados en forma cotidiana en salones de belleza, peluquerías o domicilios, que tiene estrecha relación con el daño. Resolución del caso con colgajos locales, con tiempos de internación y quirúrgicos cortos, en tiempos de COVID. Enfoque de la falta de control de sustancias usadas en peluquerías, pocos casos publicados y secuelas psicosociales importantes, con pronta mejoría de calidad de vida y reinserción social


Chemical burns in scalp after hair bleaching are produced in public places such as hairdressing salons and are caused by the combination of active agents like persulfate and hydrogen peroxide. The burns leave severe sequels of alopecia in young patients. This is about a clinical case of a chemical burn in the sculp which is full thickness and has a great large. Products used on a daily basis in hairdressing salons or in the domiciles have a close connection with the hurt. In times of COVID the case was resolved with local flaps, and short period of hospitalization and surgical. Focus on the lack of control in the usage of substances made by hairdressing salons, the existence of few published cases and the main psychosocial sequels, a speedy recovery in the quality of life and social reintegration

ABSTRACT

Este estudio demuestra la utilidad del colgajo inervado de Tripier en pacientes con diferentes etiologías, ya sean lesiones oncológicas, postraumaticas o lesiones causadas por iatrogenia. León Tripier, en 1889, describió un colgajo musculocutáneo inervado basado en el músculo orbicular, en donde utiliza un exceso de piel del párpado superior y orbicular para reconstruir los defectos del párpado inferior. Las evaluaciones preoperatorias y los procedimientos fueron realizados por un solo equipo quirúrgico en 34 pacientes, con edades comprendidas entre 18 y 72 años. El estudio se realizó en un período de 3 años, desde noviembre de 2016 a noviembre de 2019. Se pudo concluir que el colgajo musculocutáneo inervado de Tripier es una excelente alternativa para el cubrimiento de defectos que se presentan en la lamela anterior del párpado inferior. Su disección es fácil y rápida, presenta un buen aporte vascular y nervioso y es segura, lo cual es un factor importante, ya que la población de mayor demanda son pacientes de avanzada edad.


This study demonstrates the utility of the Tripier's innervated flap in patients with different etiologies, be they oncological, post-traumatic lesions or injuries caused by iatrogenesis. León Tripier in 1889 described an innervated musculocutaneous flap, based on the orbicularis muscle, where he uses excess skin from the upper eyelid and orbicularis to reconstruct defects of the lower eyelid. Preoperative evaluations and procedures were carried out by a single surgical team, and for this, 34 patients were taken, aged between 18 and 72 years. The study was conducted over a 3-year period from November 2016 to November 2019. It was concluded that Tripier's innervated musculocutaneous flap is an excellent alternative for covering defects that occur in the anterior lamella of the lower eyelid. Its dissection is easy and fast, and it also has a good vascular and nervous contribution, being safe, bearing in mind that it is an important factor, since the population in greatest demand is elderly patients.

ABSTRACT

El problema de la conformidad del ser humano con las características específicas de su cuerpo puede tener su origen en un problema estético o en un aspecto de carácter puramente físico que limita de manera marcada el desarrollo de sus actividades diarias. Ambos pueden tener solución mediante una cirugía, especialmente la estética. El objetivo de este trabajo es reafirmar que la cirugía plástica estética resuelve verdaderos problemas de salud tanto física como mental al reconocer la labor de los especialistas de cirugía plástica como evaluadores y consejeros de pacientes que acuden a consulta en busca de ayuda en nuestra provincia. Se revisó la literatura nacional e internacional sobre el tema y se concluyó que la cirugía plástica con fines estéticos constituye una solución, al satisfacer las expectativas tanto de las personas que reciben el tratamiento, como de los especialistas que realizan el trabajo.


The problem of dissagrement of the human being with the specific characteristics of his body may have its origin in an esthetical problem or in a purely physical character affair which limits, in a marked way, the development of his daily activities. Both may be solved through a surgery, specially, a esthetic surgery. The objective of this work is to reaffirm that the plastic esthetic surgery solves real physical or mental health problems, so as to recognize the work of plastic surgery specialists as evaluators and advisers of patients visiting the provincial department who look for help. National and international literature on the topic was reviewed and it was concluded that plastic surgery with esthetic aims is a solution, as it satisfies the expectatives either from people searching treatment, as from the specialists who give the service.

INTRODUCCION: los biopolímeros son macromoléculas cuyo uso como sustancias de relleno con fines estéticos ha ido en aumento en los últimos años. Esto ha generado un incremento de complicaciones por alogenosis iatrogénica de difícil tratamiento. OBJETIVO: describir los hallazgos intraoperatorios en las pacientes con alogenosis iatrogénica intervenidas quirúrgicamente en el Departamento de Cirugía Plástica del Hospital Hermanos Ameijeiras. METODO: se realizó un estudio descriptivo, prospectivo, longitudinal y monocéntrico con 15 pacientes femeninas que recibieron tratamiento quirúrgico para extracción de sustancias modelantes desde enero 2017 a diciembre 2019. RESULTADOS: el rango de edad predominante fue entre los 19 y 29 años con un 66.6% y entre los 30 y 40 años de edad con un 33.3%. El procedimiento quirúrgico fue realizado bajo anestesia general en el 33.3% de las pacientes; de ellos el 20% fue en la región mamaria, 40% glúteos y 40% genitales y pubis. Se empleó anestesia local en el 66.6%, distribuidas en 20% en la región frontal y glabelar, 30% en los párpados y 50% en los labios. El 100% mostró hallazgos intraoperatorios similares. CONCLUSIONES: independientemente del producto inyectado, la región anatómica y la expresión clínica de la enfermedad, los hallazgos transoperatorios son los mismos. Tanto en las zonas más inyectadas que fueron la región glútea, genital y púbica como en la de menor frecuencia que correspondió a las zonas frontales y glabelar, la extracción del producto resultó en múltiples nódulos en forma de perlas de pequeño tamaño y cavernas.


INTRODUCTION: biopolymers are macromolecules whose use as fillers for aesthetic purposes has been increasing in recent years. This has generated an increase in complications due to iatrogenic alogenosis that is difficult to treat. OBJECTIVE: to describe the intraoperative findings in patients with iatrogenic alogenosis who underwent surgery at the Department of Plastic Surgery of the Hermanos Ameijeiras Hospital. METHODS: a descriptive, prospective, longitudinal, single-center study was conducted with 15 female patients who received surgical treatment for removal of modeling substances from January 2017 to December 2019. RESULTS: the predominant age range was between 19 and 29 years old with 66.6% and between 30 and 40 years old with 33.3%. The surgical procedure was performed under general anesthesia in 33.3% of the patients; of them 20% were in the breast region, 40% buttocks and 40% genitalia and pubis. Local anesthesia was used in 66.6%, distributed in 20% in the frontal and glabellar region, 30% in the eyelids and 50% in the lips. 100% showed similar intraoperative findings. CONCLUSIONS: regardless of the product injected, the anatomic region and the clinical expression of the disease, the transoperative findings are the same. Both in the most injected areas which were the gluteal, genital and pubic region and in the less frequently injected areas which corresponded to the frontal and glabellar areas, the extraction of the product resulted in multiple nodules in the form of small pearls and caverns.

Introducción. El empleo del plasma rico en plaquetas (PRP) o plasma autólogo con una cifra de plaquetas superior a la del plasma basal, ha tomado un auge notorio para estimular la regeneración tisular en las afecciones musculoesqueléticas y de tejidos blandos. El objetivo de este estudio fue revisar sistemáticamente la eficacia del tratamiento con PRP en algunas lesiones musculoesqueléticas y de tejidos blandos en cirugía plástica y maxilofacial, así como en odontología y dermatología, con su nivel de evidencia y grado de recomendación. Metodología. Se utilizaron los buscadores PubMed, Google Académico y la Biblioteca Cochrane con terminología MeSH. Se analizaron un total de 44 artículos que cumplieron los criterios de inclusión y exclusión. Resultados. El análisis de los estudios encontró que el nivel de evidencia fue de 1 en el 75%, nivel 2 en el 15,9%, nivel 3 en el 6,8%, y nivel 4 en el 2,2%. El grado de recomendación se distribuyó entre bueno y favorable; el 40,9% de los estudios reflejaron grado A, 50% grado B y 9% grado C. No obstante, el 50% de los estudios reportaron en sus conclusiones que hay una deficiencia en el diseño metodológico, con inconsistencia en sus resultados. Conclusión. Estos hallazgos señalan que en la evidencia encontrada se observan resultados contradictorios, la mayoría de ellos no muestran una prueba firme que apoye el empleo rutinario del PRP, porque sus diseños y poder estadístico son de baja calidad. Hace falta unificar criterios y diseños de investigación con evidencia científica altamente recomendable, para que el uso del PRP como agente ortobiológico en estas patologías, pueda ser ampliamente aceptado


Introduction. The use of platelet-rich plasma (PRP), autologous plasma with a number of platelets higher than that of basal plasma, after having been subjected to some extraction and concentration process, has taken a noticeable rise to stimulate tissue regeneration in musculoskeletal and soft tissue disorders. The objective of this study is to systematically review the efficacy of treatment with PRP in some musculoskeletal and soft tissue injuries, in plastic and maxillofacial surgery, as well as in dentistry and dermatology, with their level of evidence and grade of recommendation. Methodology. The search was performed using MeSH terms in PubMed, Google Scholar and the Cochrane Library. A total of 44 articles that met the inclusion and exclusion criteria were analyzed. Results. Analysis of the studies found that the level of evidence was level 1 in 75%, level 2 in 15.9%, level 3 in 6.8%, and level 4 in 2.2%. The grade of recommendation was distributed between good and favorable; 40.9% reflected grade A, 50% grade B, and 9% grade C. However, 50% of the studies reported in their conclusions that there is a deficiency in the methodology, with inconsistency in their results. Conclusion. These findings indicate that the evidence found shows contradictory results, most of them do not show a firm proof that supports their routine use, because their designs and statistical power are of low quality. It is necessary to unify criteria and research designs with highly recommended scientific evidence, so that the use of PRP as an orthobiological agent in these pathologies could be widely accepted

Diversas fontes de dados têm identificado a labioplastia ou ninfoplastia, dedicada à redução dos pequenos lábios, como uma intervenção estética muito procurada por mulheres e adolescentes brasileiras, sendo que o Brasil é o país que mais faz este tipo de cirurgia. Este ensaio discute como se promove e quem produz a divulgação desta imagem de aumento da demanda e do número de cirurgias realizadas. Está embasado em análise de artigos em periódicos científicos, material de imprensa e outros documentos; e ancorado no cruzamento entre os estudos sociais da ciência e da tecnologia, os estudos de gênero e a antropologia do corpo e da saúde. Por meio deste recorte, analisa como o campo da cirurgia plástica, ou mais precisamente seus representantes mais atuantes na mídia e na produção bibliográfica, pode atuar na criação de determinadas demandas de intervenção, por meio da promoção de normas classificatórias, tecnologias biomédicas e divulgação de resultados com ênfase nos benefícios projetados. Conclui que, apesar da falta de consenso sobre o que seria a hipertrofia dos pequenos lábios, a cirurgia tem sido muito indicada e praticada e contribui para a (re)produção de padrões estritos associados ao gênero feminino e a uma certa noção de "normalidade" corporal. Poucas referências mencionam que a divulgação das técnicas operatórias e mesmo deste tipo de cirurgia no consultório médico seria um fator relevante para entender o fenômeno. Sugere que o papel de médicos/as na promoção desta possibilidade de intervenção estética cirúrgica que, como qualquer prática deste tipo, não é isenta de riscos e efeitos inesperados, precisaria ser alvo de um debate mais aprofundado.


Various data sources have identified labiaplasty or nymphoplasty, aimed at the reduction of the labia minora, as an aesthetic intervention in high demand by Brazilian women and adolescents, with Brazil as the world's leading country in the performance of this surgery. This essay discusses how and by whom the dissemination of this image is promoted and produced, with the increase in demand and in the number of surgeries performed. The study is based on an analysis of scientific journals, press stories, and other documents. The approach employs a cross-analysis of social studies of science and technology, gender studies, and anthropology of the body and health. The article analyzes how the field of plastic surgery, or more precisely its more active representatives in the media and in research output, can act to create certain demands for surgical intervention through the promotion of classificatory norms, biomedical technologies, and the publication of results with emphasis on the projected benefits. Despite the lack of consensus on the definition of hypertrophy of the labia minora, the surgery has been widely indicated and practiced and contributes to the (re)production of narrow standards associated with female gender and a certain notion of the "normal" body. Few references mention that publicizing the surgical technique and even this type of surgery in the physician's office may be a relevant factor for understanding the phenomenon. The article suggests that the physician's role in the promotion of this possibility for aesthetic surgical intervention, like any such practice, is not free of risks and unexpected effects and should be the target of a more in-depth debate.


Diversas fuentes de datos han identificado la labioplastia o ninfoplastia, dedicada a la reducción de los labios menores, como una intervención estética muy solicitada por mujeres y adolescentes brasileñas, siendo Brasil el país donde más se realiza este tipo de cirugía. Este ensayo discute como se promueve y quién dirige la divulgación de esta imagen de aumento de la demanda y número de cirugías realizadas. Está basado en el análisis de artículos en periódicos científicos, material de prensa y otros documentos; y anclado en el cruce entre los estudios sociales de ciencia y tecnología, estudios de género y antropología del cuerpo y de la salud. Mediante esta reducción, analiza como el campo de la cirugía plástica, o más precisamente sus representantes más activos en los medios y producción bibliográfica, pueden actuar en la creación de determinadas demandas de intervención, mediante la promoción de normas clasificatorias, tecnologías biomédicas y divulgación de resultados con énfasis en los beneficios proyectados. Concluye que, a pesar de la falta de consenso sobre lo que sería la hipertrofia de los labios menores, la cirugía ha sido muy recomendada y practicada, así como que contribuye a la (re)producción de patrones estrictos asociados con el género femenino y a una cierta noción de "normalidad" corporal. Pocas referencias mencionan que la divulgación de las técnicas operatorias, incluidas las de este tipo de cirugía, en el consultorio médico sería un factor relevante para entender el fenómeno. Sugiere que el papel de médicos/as en la promoción de esta posibilidad de intervención estética quirúrgica que, como cualquier práctica de este tipo, no está exenta de riesgos y efectos inesperados, necesitaría ser objeto de un debate más profundo.

Resumen La impresión 3D es una tecnología interesante en constante evolución. También conocida como manufactura aditiva, consiste en la conversión de diseños digitales a modelos físicos mediante la adición de capas sucesivas de material. En años recientes, y tras el vencimiento de múltiples patentes, diversos campos de las ciencias de la salud se han interesado en sus posibles usos, siendo la cirugía plástica una de las especialidades médicas que más ha aprovechado sus ventajas y aplicaciones, en especial la capacidad de crear dispositivos altamente personalizados a costos accesibles. Teniendo en cuenta lo anterior, el objetivo del presente artículo es describir los usos de la impresión 3D en cirugía plástica reconstructiva a partir de una revisión de la literatura. Las principales aplicaciones de la impresión 3D descritas en la literatura incluyen su capacidad para crear modelos anatómicos basados en estudios de imagen de pacientes, que a su vez permiten planificar procedimientos quirúrgicos, fabricar implantes y prótesis personalizadas, crear instrumental quirúrgico para usos específicos y usar biotintas en ingeniería tisular. La impresión 3D es una tecnología prometedora con el potencial de implementar cambios positivos en la práctica de la cirugía plástica reconstructiva en el corto y mediano plazo.


Abstract 3D printing is an interesting technology in constant evolution. Also known as additive manufacturing, it consists of the conversion of digital designs into physical models by successively adding material layer by layer. In recent years, and after the expiration of multiple patents, several fields of health sciences have approached this type of technology, plastic surgery being one of the medical specialties that has taken advantage of its benefits and applications, especially the ability to create highly customized devices at low costs. With this in mind, the objective of this work is to describe the uses of 3D printing in reconstructive plastic surgery based on a literature review. The main applications of 3D printing described in the literature include its ability to create anatomical models based on patient imaging studies, which in turn allow planning surgical procedures, manufacturing custom implants and prostheses, creating surgical or instrumental simulators, and using bioinks in tissue engineering. 3D printing is a promising technology with the potential to cause positive changes in the field of reconstructive plastic surgery in the short and medium term.

Resumen Un capítulo novedoso es la atención y promoción por parte de médicos especialistas de aspectos relacionados con procedimientos estéticos, más que con la salud. La aspiración humana de la búsqueda de la belleza personal ha generado nuevos escenarios en la labor médica. El Comité de Ética y Transparencia en la Relación Médico Industria (CETREMI) de la Academia Nacional de México ha revisado esta circunstancia y emite recomendaciones tanto a los médicos como a los productores y potenciales consumidores de procedimientos estéticos.


Abstract A novel chapter in current medical settings is the promotion and attention of esthetic aspects rather than health issues by health professionals. The human aspiration related to the search for personal beauty has generated new scenarios in medical practice. The Committee on Ethics and Transparency in the Physician-Industry Relationship (CETREMI) of the National Academy of Medicine of Mexico has analyzed this phenomenon and has issued recommendations directed both to medical professionals and to producers and potential consumers of esthetic procedures.

Resumen Introducción: La región periorbitaria es la primera en exhibir los efectos del envejecimiento, siendo el exceso de piel del párpado superior el signo más frecuente. Actualmente las técnicas quirúrgicas de blefaroplastia superior son más conservadoras, logrando resultados estéticos satisfactorios y naturales. La técnica con realce de volumen es una técnica poco agresiva en la cual se respeta el músculo y grasa periorbitaria. Objetivo: Describir la técnica de blefaroplastia superior con realce de volumen, sus beneficios y resultados estéticos. Material y Método: Estudio descriptivo retrospectivo. Se analizaron pacientes sometidos al procedimiento entre agosto de 2016 y octubre de 2017. Resultados: Se describe la técnica quirúrgica. Se operaron 56 pacientes con un promedio de edad de 57 años. 75% correspondían a sexo femenino. Se utilizó anestesia local en 59% pacientes. Se complementó con otras cirugías en un 57%. De éstas 19% corresponden a rinoplastías, 31% a frontoplastías, 47% a blefaroplastias inferiores, entre otras. No se presentaron complicaciones intraoperatorias ni postoperatorias mayores. Conclusión: La técnica de blefaroplastia superior con realce de volumen es una técnica conservadora con excelentes resultados estéticos y funcionales. Al conservar el músculo orbicular y los tejidos blandos subyacentes se genera un aumento de volumen en la zona del supratarso, realzando la belleza de la región periorbitaria.


Abstract Introduction: The periorbital region is the first to exhibit the effects of aging. The excess of skin on the upper eyelid is the most frequent sign. Nowadays upper blepharoplasty techniques are more conservative, achieving satisfactory aesthetic results. The volume enhancement technique is a non-aggressive technique in which muscle and periorbital fat are preserved. Aim: Describe the upper blepharoplasty with volume enhancement technique, its benefits and aesthetic results. Material and Method: The surgical technique is described. Retrospective descriptive study. Patients studied were between august 2016 and october 2017. Results: 56 patients underwent surgery with an average age of 57 years. 75% were female. 59% were performed under local anesthesia. 57% had complementary surgeries. These were rhinoplasties (19%), frontoplasty (31%), lower blepharoplasty (47%) among others. There were no mayor intraoperative or postoperative complications. Conclusion: The upper blepharoplasty with volume enhancement technique is a conservative technique with excellent aesthetic and functional outcomes. By preserving the orbicularis muscle and the underlying soft tissues, a volume increase is achieved in supra tarsal area, enhancing the beauty in the periorbital region.

Introducción: ¿La mastoplastia de aumento realizada por un cirujano no especialista en cirugía plástica debe ser vista como un acto de intrusismo o de competencia médica? Objetivo: Presentación de casos de mastoplastias de aumento realizadas por un ginecólogo mastólogo, su comparación con la bibliografía publicada por reconocidos cirujanos plásticos y discusión de la situación actual del tema. Material y método: Se describen retrospectivamente casos de mastoplastia de aumeeto recolectados en diez años de experiencia unipersonal, entre julio 2010 y abril de 2020, comparando resultados cosméticos y complicaciones con casuística publicada. Resultados: Se realizaron 144 casos de mastoplastia de aumento. Hubo 2.08% de complicaciones tempranas y 6.25% de complicaciones tardías, con 94.4% de satisfacción cosmética de las pacientes. No hubo diferencias significativas respecto a tasa de complicaciones quirúrgicas respecto a la serie comparada, encontrando equivalente el resultado cosmético. Conclusiones: La mastoplastia de aumento realizada por un cirujano mastólogo capacitado en el área cosmética, debería aceptarse dentro del alcance de su competencia, creando un marco regulatorio científico y legal adecuado.


Introduction: Can an augmentation mammoplasty performed by a surgeon who is not a plastic surgery specialist be cosidered as an act of presessional intusion, or within the scope of his competence? Objetive: Report of a gynecologist and breast specialist's experience comparing results and complications to the reported case studies published by renowned plastic surgeons and an argument about the topic. Material and method: The author reports retrospectively his ten years experience (january 2010 - april 2020) in augmentation mammoplasty. Cosmetic results and complications were compared with published reports. Results: 144 augmentation mammoplasties were performed between january 2010 and april 2020. There were 2.08% of early complications, 6.25% of late complication, and 94.4% of the patients were satisfied with the cosmetic result. The results show no significative difference about complications and an equivalent cosmetic satisfaction compared to the reported experience published. Conclusions: Augmentation mammoplasty surgery performed by a breast surgeon trained in cosmetic procedures must be considered within the scope of his competence, thus developing an adequate medical and legal regulatory framework.

ABSTRACT

ABSTRACT: A variety of periodontal plastic surgery techniques have been proposed to correct aesthetic and functional problems of periodontal tissues. The most common procedure uses the connective tissue graft from the palate. However, patient discomfort and the limited quantity of palatal tissue results in many cases where the surgeon uses connective tissue substitutes. In this case report, we describe the use of a human dermal matrix for gingival volume augmentation in the lower incisors with a novel modified VISTA technique stabilizing the flap with sutures and cyanoacrylates, avoiding comorbidity of a second surgical site. The follow up at 6 months showed an increase in gingival thickness and a reduction in recessions length.

Introdução: Atualmente com o aumento das gastroplastias redutoras (cirurgia bariátrica) e grande perda ponderal, há também um aumento na procura destes pacientes pela cirurgia plástica. Uma das características destes pacientes é o excesso de tecido dermogorduroso que causa deformidades, principalmente nos membros inferiores, acometendo a região trocantérica. Objetivo: Relatar um caso de dermolipectomia trocantérica em paciente pós-cirurgia bariátrica, onde a lipoaspiração por si só não resolveria a correção da deformidade. Relato de caso: IPMS, sexo feminino, 55 anos, histórico de gastroplastia redutora (cirurgia bariátrica) com grande perda ponderal. Relata desconforto importante com a lipodistrofia e extensa flacidez em região trocantérica bilateral. Após avaliação pela equipe, foi optado por realizar dermolipectomia trocantérica bilateral, em janeiro de 2019, no Serviço de Cirurgia Plástica Osvaldo Saldanha. Discussão: Nos casos de lipodistrofia trocantérica com deformidades graves, a cicatriz da dermolipectomia em relação à deformidade é favorável quanto a escolha desta técnica, pois a lipoaspiração poderá agravar ainda mais a deformidade, sendo mandatório utilizar a técnica de dermolipectomia trocantérica para corrigila. Quanto à lipodistrofia com deformidade moderada há dúvida entre a relação do benefício e a deformidade resultante, sendo aplicada a técnica de acordo com a necessidade do paciente e, por fim, nos casos de lipodistrofia com deformidade leve, opta-se pela lipoaspiração devido à correção ser realizada sem grandes cicatrizes aparentes. Conclusão: Portanto, a lipoaspiração tem benefício nos casos de adiposidade localizada, limitando as indicações da técnica de dermolipectomia, em especial na região trocantérica, sem invalidá-la para casos selecionados, como o descrito neste relato de caso.


Introduction: Currently, with the increase in reducing gastroplasty (bariatric surgery) and the great weight loss, there is also an increase in the demand of these patients for plastic surgery. One of the characteristics of these patients is the excess of dermal adipose tissue that causes deformities, especially in the lower limbs, affecting the trochanteric region. Objective: To report a case of trochanteric dermolipectomy in a patient after bariatric surgery, where liposuction alone would not solve the correction of the deformity. Case report: IPMS, female, 55 years old, history of reducing gastroplasty (bariatric surgery) with great weight loss. She reports significant discomfort with lipodystrophy and extensive flacidity in the bilateral trochanteric region. After the team's evaluation, it was decided to perform bilateral trochanteric dermolipectomy, in January 2019, at the Plastic Surgery Service Osvaldo Saldanha Discussion: In cases of trochanteric lipodystrophy with severe deformities, the scar of dermolipectomy in relation to the deformity is favorable in terms of the choice of this technique, since liposuction may further aggravate the deformity, and it is mandatory to use the trochanteric dermolipectomy technique to correct it. Conclusion: Therefore, liposuction is beneficial in cases of localized adiposity, limiting the indications for the dermolipectomy technique, especially in the trochanteric region, without invalidating it for selected cases, as described in this case report.
