Abstract: Syringocystadenoma papilliferum and tubular apocrine adenoma are rare benign sweat gland tumors. Syringocystadenoma papilliferum occurs alone or in association with other tumors. Although it is rare, the association of tubular apocrine adenoma with syringocystadenoma papilliferum developing in a sebaceous nevus on the scalp is well documented. However, the co-existence of these two tumors without the background of a sebaceous nevus has not been frequently reported. Syringocystadenoma papilliferum and tubular apocrine adenoma may have a histopathological overlap, but a few cases of a syringocystadenoma papilliferum combined with a tubular apocrine adenoma have been reported. Herein we describe an unusual case of syringocystadenoma papilliferum co-existing with a tubular apocrine adenoma located on the back of a 14-year-old patient in the absence of a pre-existing sebaceous nevus.

Abstract: Hidroacanthoma simplex is a rare intraepidermal neoplasia that arises from the acrosyringial portion of the eccrine duct. Malignant transformation of hidroacanthoma simplex is reported in the literature and the treatment is performed with wide excision or Mohs micrographic surgery. We report the first case successfully treated with cryosurgery with a long-term follow up.

ABSTRACT The authors report the first case in Brazilian literature of orbital apocrine hidrocystoma with immunohistochemistry confirmation. The tumor had been growing slowly and progressively, there was no history of impaired vision, diplopia, watering, discharge, or prior trauma. There was no proptosis and extraocular mobility was normal. The radiologic study by orbital computerized tomography revealed an extraconal nodule, with partially defined limits, with cystic and solid areas in the superomedial right orbit. An anterior orbitotomy, with full excision of the tumor was performed. A histopathology examination revealed apocrine hidrocystoma and immunohistochemistry confirmed the diagnosis. After surgery, the patient had total remission of symptoms.

RESUMO Os autores relatam o primeiro caso de hidrocistoma apócrino orbitário com confirmação imunohistoquímica. O tumor apresentou crescimento lento e progressivo, sem relato de baixa de acuidade visual, diplopia, epífora, secreção ou trauma prévio. Não houve proptose ou alteração da motilidade extraocular. O exame de imagem por tomografia computadorizada da órbita revelou uma lesão nodular, extraconal, de limites imprecisos, com áreas císticas e sólidas no seu interior, na região súpero-medial de órbita direita. O paciente foi submetido à orbitotomia por via anterior, com exérese integral da tumoração. O exame histopatológico revelou o diagnóstico de hidrocistoma apócrino e a imunohistoquímica confirmou o diagnóstico. Após a cirurgia, o paciente teve regressão total dos sintomas.

Carcinoma microcístico anexial é uma neoplasia de glândulas sudoríparas écrinas incomum, localmente agressivo. Facilmente confundido com tumores anexiais benignos e com alto índice de recidiva após tratamento incorreto. O objetivo é divulgar aos cirurgiões plásticos a importância do conhecimento e manejo correto desta afecção com utilização da cirurgia micrográfica de Mohs. Foi realizada revisão da literatura de 1982 a janeiro de 2014 e análise prospectiva de um caso de carcinoma microcístico anexial conduzido na Clínica de Cirurgia Plástica do Hospital Felício Rocho em março de 2011, utilizando a cirurgia micrográfica de Mohs. Descrito em 1982, menos de 700 casos de carcinoma microcístico anexial relatados na literatura mundial. O caso apresentado foi tratado com o uso da cirurgia micrográfica de Mohs, com seguimento 3 anos sem recidiva. Podemos concluir que a cirurgia micrográfica de Mohs é um excelente arsenal terapêutico, com menor índice de recidiva e preservação de tecidos vitais.

Microcystic adnexal carcinoma is an uncommon, locally aggressive eccrine sweat gland tumor that is easily confused with benign adnexal tumors and has a high rate of recurrence after incorrect treatment. The present study was aimed at increasing awareness on the importance of knowing and properly managing this disease among plastic surgeons by using Mohs micrographic surgery. A literature review from 1982 to January 2014 and a prospective case study of a microcystic adnexal carcinoma treated with Mohs micrographic surgery, at the plastic surgery clinic of Felício Rocho Hospital in March 2011, were performed. First described in 1982, microcystic adnexal carcinoma now has at least 700 cases reported in the international literature. The case reported herein was treated with Mohs micrographic surgery, including a 3-year follow-up without recurrence. In conclusion, Mohs micrographic surgery is an excellent therapeutic tool to preserve vital tissues and ensure a reduced recurrence rate.

Eccrine syringofibroadenoma is a rare benign adnexal neoplasm derived from cells of the acrosyringium of eccrine sweat glands. ESFA usually manifests as a solitary nodule on the extremities of elderly patients, but it may also present as papules, nodules or plaques. Its clinical appearance is nonspecific and malignant neoplasms should beconsidered in the differential diagnosis. However, histopathological findings are typical. The main treatment is surgical excision. In order to illustrate a typical presentation of the tumor, we report a case of solitary eccrine syringofibroadenoma, including the surgical treatment used and its result.

INTRODUÇÃO: O hidrocistoma palpebral écrino corresponde a lesão cística rara, usualmente de pequenas dimensões, não ultrapassando o diâmetro de 10 mm. Sua abordagem usualmente é por excisão simples e biópsia. É alto o índice de recidiva local. MÉTODOS: Paciente com múltiplos hidrocistomas bilaterais, vários maiores que 10 mm. Sob anestesia local, foi realizada incisão infraciliar tradicionalmente usada para blefaroplastia inferior e retirada dos hidrocistomas com cápsula íntegra. RESULTADOS: Paciente com excelentes resultados estéticos e sem recidivas até o presente momento, mais de seis meses após o procedimento. CONCLUSÃO: A incisão infraciliar de blefaroplastia inferior é uma técnica promissora para exérese de hidrocistomas maiores que 10 mm. Mais estudos devem ser realizados para confirmação do bom resultado estético-funcional desta técnica.

INTRODUCTION: Eccrine eyelid hidrocystoma is a rare cystic lesion, usually small and not exceeding 10 mm in diameter. Its treatment is usually by simple excision and biopsy. The rate of local recurrence is high. METHODS: A patient presented with multiple bilateral hidrocystomas, several larger than 10 mm. Under local anesthesia, an infraciliary incision traditionally used for lower blepharoplasty was performed, and hidrocystomas with intact capsules were withdrawn. RESULTS: The patient had excellent esthetic results, with no recurrence for more than six months after the procedure. CONCLUSION: The infraciliary lower blepharoplasty incision is a promising technique for excising hidrocystomas larger than 10 mm. More studies should be conducted to confirm the good esthetic and functional outcomes of this technique.

Os hidrocistomas écrinos são lesões raras, císticas e benignas, que resultam em deformidades nas regiões palpebrais bilateralmente. Vários tratamentos são citados na literatura, porém nenhum deles é considerado padrão de referência. Deve-se somar as impressões obtidas à avaliação clínica e aos recursos disponíveis. Paciente de 73 anos, feminino, apresentando tumorações em regiões palpebrais bilaterais, de crescimento lento, com obstrução parcial do campo de visão e ectrópio. Foi submetida a dois procedimentos cirúrgicos para ressecção, em 2007 e em 2008. Apresentou melhora significativa do contorno palpebral bilateral, com boa simetria entre as regiões. O exame histopatológico concluiu: hidrocistoma écrino. O tratamento cirúrgico para a polipose palpebral bilateral relacionada ao hidrocistoma écrino mostrou-se uma modalidade que pode apresentar bons resultados estéticos e funcionais, sendo reprodutível, e sem causar maiores morbidades pós-operatórias ao paciente.

Eccrine hidrocystomas are rare lesions, cysts, or benign tumors, which lead to bilateral deformities in the eyelid areas. Several treatments are described in the literature. However, none of them has been established as the gold standard. Hence, it becomes necessary to consider the contribution of different opinions and available resources to clinical evaluation. A 73-year-old female patient presented with slow-growing tumors on both eyelids, which consequently led to partial visual field obstruction and ectropion. She underwent 2 surgical resections, one in 2007 and the other in 2008. As a result, she showed significant improvement of the bilateral eyelid contours and satisfactory symmetry between the areas. Histopathological diagnosis indicated eccrine hidrocystoma. The surgical treatment of bilateral eyelid polyposis associated with eccrine hidrocystoma proved to be a reproducible procedure that may ensure satisfactory aesthetic and functional results, without causing major postoperative morbidities to the patient.

Relato de caso de hidrocistoma palpebral e apresentação de técnica cirúrgica na conduta terapêutica dos tumores císticos benignos da pálpebra. Os hidrocistomas são tumores benignos relativamente frequentes nas pálpebras, principalmente, na pálpebra inferior, com maior prevalência no sexo feminino e a partir da quarta década de vida. O diagnóstico é clínico e, em casos de lesão única, a conduta cirúrgica é o tratamento de escolha. Deve-se descrever melhor a técnica cirúrgica utilizada, pois traz bons resultados estéticos e menor risco de recidiva.

This report describes the case of a hidrocystoma of the eyelid and the surgical technique used in the therapeutical management of benign cystic lesions of the eyelids. Hidrocystomas are relatively common benign lesions of the eyelids, principally the lower eyelid. They are more common in females over thirty years of age. Diagnosis is clinical and when there is a single lesion, surgery is the treatment of choice. The surgical technique used should be described in greater detail, since it offers good esthetic results and a low risk of recurrence.

Introducción: Las glándulas ecrinas (sudoríparas) se encuentran en todos los sitios de la piel y son más abundantes en las palmas de las manos y plantas de los pies. El hidradenocarcinoma nodular es un tumor raro de las glándulas ecrinas, maligno y agresivo. El objetivo de este trabajo es informar de un caso de hidradenocarcinoma perianal. Caso clínico: Mujer de 75 años de edad, diabética, hipertensa e hipotiroidea. Al examen físico se observó y palpó tumoración perianal pequeña que llegaba hasta canal anal; se tomó biopsia con resultado de hidradenoma perianal. Se llevó a cabo rectosigmoidoscopia hasta 25 cm, normal. Tomografía computarizada: adenopatías de 1 cm en región inguinal e iliaca derecha. Se realizó resección amplia de la tumoración. El informe de patología fue acrospiroma ecrino maligno. Actualmente la paciente se encuentra en tratamiento complementario con quimioterapia. Conclusiones: En pacientes que debuten con lesiones malignas metastásicas en región inguinal debe pensarse en origen del conducto anal.

BACKGROUND: Eccrine glands (sweat glands) appear in all sites of the skin and are more abundant in hands and feet. Nodular hidradenocarcinoma (NH) is a rare malignant and aggressive tumor of the eccrine glands. The objective of this study is to report a case of perianal hidradenocarcinoma. CLINICAL CASE: We present the case of a 75-year-old female with diabetes, hypertension, and hypothyroidism. Physical examination revealed a small perianal tumor that was palpated near the anal canal. Biopsy was done. Pathology report revealed perianal hidradenoma. Rectosigmoidoscopy was normal. Computed tomography showed 1-cm adenopathies in inguinal and right iliac regions. Extensive resection of the tumor was done. Definitive pathology report was malignant eccrine acrospiroma. The patient underwent adjuvant chemotherapy in the Oncology Service and died 1 month later. CONCLUSIONS: In patients with a first symptom of metastases in the inguinal region, suspicion must be directed to the anal canal.

El poroeorcinoma ecrino es un tumor cutáneo maligno infrecuente que deriva de las glándulas sudoríparas ecrinas y que afecta fundamentalmente a personas de edad avanzada. Se localiza con mayor frecuencia en extremidades inferiores, cara y cuero cabelludo y son potencialmente agresivos, tanto localmente como a distancia. Reportamos el caso de una paciente con diagnóstico de Porocarcinoma Ecrino Pigmentado en cuero cabelludo, que fue tratado con extirpación total de la lesión.

Eccrine porocorcinoma is a rare malignant skin tumor derived from eccrine sweat glands. It affects primarily older people and is found more frequently in lower extremities, face and scalp, and is potentially aggressive, both locally and through methastasis. We describe o patient diagnosed with pigmented eccrine porocarcinoma of the scalp, and treated with total excision of the lesion.
