ABSTRACT

Remitting seronegative symmetrical synovitis with pitting edema(RS3PE),the inflammatory arthritis attacking mainly elderly males,is characterized by symmetrical synovitis with pitting edema of the dorsum of hands and feet and the absence of rheumatoid factor.RS3PE commonly accompanies malignant tumor,infections and other diseases.Here we report a case of RS3PE associated with lung malignancy and review other six cases to summarize the clinical features,treatment and prognosis.

ABSTRACT

OBJETIVOS: Comparar a curto prazo (04 semanas) a efetividade das infiltrações intra-articulares (IIA) guiadas por fluoroscopia (FC) e ultrassom (US) em pacientes com enfermidades reumáticas. MATERIAL E MÉTODOS: Foi realizado um estudo controlado e prospectivo em pacientes portadores de doenças reumáticas captados dos ambulatórios da Disciplina de Reumatologia da Universidade Federal de São Paulo (UNIFESP), Brasil. Critério de inclusão: adultos com indicação de IIA com corticosteróide por sinovite refratária. Todos os pacientes forma infiltrados com hexacetonide triancinolona (20 mg/mL) com doses variáveis, de acordo com a articulação estudada. RESULTADOS: Foram avaliados 71 pacientes (52 mulheres; 44 brancos), portadores de enfermidades reumáticas variadas. A média de idade era 51,9 ± 13 anos e 47 deles (66,2%) faziam uso de drogas modificadora do curso da doença (DMARD). Na análise global da amostra (71 pacientes) e na subanálise coxofemoral (23 pacientes), observou-se melhora estatística (p < 0,001) em ambos os grupos quanto à EVA de dor. Na análise global observou-se aumento significativo da flexão articular (p < 0,001) e um Δ de flexão maior a favor do grupo guiado por FC. A avaliação de melhora segundo Likert Scale mostrou diferença significativa (p < 0,05) entre os grupos na avaliação global, nas proporções inalterado e melhor, a favor do grupo guiado por US. Não foi observada diferença estatisticamente significante entre os grupos para qualquer outra variável. CONCLUSÃO: A IIA guiada por imagem melhorou a dor regional, a curto prazo, relacionada à sinovite de vários tipos de articulações. Para a grande maioria das variáveis avaliadas não houve diferença entre a efetividade da IIA guiada por US ou FC.


OBJECTIVE: Compare the effectiveness of ultrasound and fluoroscopy to guide intra-articular injections (IAI) in selected cases. MATERIAL AND METHODS: A prospective study in our outpatient clinics at the Rheumatology Division at Universidade Federal de São Paulo (UNIFESP), Brazil, was conducted to compare the short-term (4 weeks) effectiveness of ultrasound and fluoroscopy-guided IAI in patients with rheumatic diseases. Inclusion criteria were: adults with refractory synovitis undergoing IAI with glucocorticoid. All patients had IAI performed with triamcinolone hexacetonide (20mg/ml) with varying doses according to the joint injected. RESULTS: A total of 71 rheumatic patients were evaluated (52 women, 44 whites). Mean age was 51.9 ± 13 years and 47 of them (66.2%) were on regular DMARD use. Analysis of the whole sample (71 patients) and hip sub-analysis (23 patients) showed that significant improvement was observed for both groups in terms of pain (P < 0.001). Global analysis also demonstrated better outcomes for patients in the FCG in terms of joint flexion (P < 0.001) and percentage change in joint flexion as compared to the USG. Likert scale score analyses demonstrated better results for the patients in the USG as compared to the FCG at the end of the study (P < 0.05). No statistically significant difference between groups was observed for any other study variable. DISCUSSION AND CONCLUSION: Imaging-guided IAI improves regional pain in patients with various types of synovitis in the short term. For the vast majority of variables, no significant difference in terms of effectiveness was observed between fluoroscopy and ultrasoundguided IAI.

ABSTRACT

Tuberculosis of the appendicular skeleton is an uncommon infection by tubercle bacilli. Thirty percent of the skeletal tuberculosis involves joints, the knee being the third most common joint affected. We report a case of tuberculous synovitis of the knee joint in a 69 years old male. The diagnosis was done by Ziehl – Neelsen stain and culture on Lowenstein – Jensen medium of the synovial fluid along with X-ray finding of the knee joint. Though the sputum sample was negative for AFB, X-ray of the chest showed finding suggestive of old pulmonary tuberculosis. The patient was treated with the anti tubercular regimen and responded well .

ABSTRACT

OBJECTIVES: The aim of the present study was to investigate the effectiveness of Samarium153-particulate hydroxyapatite radiation synovectomy in rheumatoid arthritis patients with chronic knee synovitis. METHODS: Fifty-eight rheumatoid arthritis patients (60 knees) with chronic knee synovitis participated in a controlled double-blinded trial. Patients were randomized to receive either an intra-articular injection with 40 mg triamcinolone hexacetonide alone (TH group) or 40 mg triamcinolone hexacetonide combined with 15 mCi Samarium153-particulate hydroxyapatite (Sm/TH group). Blinded examination at baseline (T0) and at 1 (T1), 4 (T4), 12 (T12), 32 (T32), and 48 (T48) weeks post-intervention were performed on all patients and included a visual analog scale for joint pain and swelling as well as data on morning stiffness, flexion, extension, knee circumference, Likert scale of improvement, percentage of improvement, SF-36 generic quality of life questionnaire, Stanford Health Assessment Questionnaire (HAQ), Lequesne index, use of non-steroidal anti-inflammatory drugs or oral corticosteroids, events and adverse effects, calls to the physician, and hospital visits. RESULTS: The sample was homogeneous at baseline, and there were no withdrawals. Improvement was observed in both groups in relation to T0, but no statistically significant differences between groups were observed regarding all variables at the time points studied. The Sm/TH group exhibited more adverse effects at T1 (p<0.05), but these were mild and transitory. No severe adverse effects were reported during follow-up. CONCLUSION: Intra-articular injection of Samarium153-particulate hydroxyapatite (15 mCi) with 40 mg of triamcinolone hexacetonide is not superior to triamcinolone hexacetonide alone for the treatment of knee synovitis in patients with rheumatoid arthritis at 1 y of follow-up.

ABSTRACT

Chronic pain in the lumbar region is extremely common. Data from the US show that nearly 80% of the population develop this syndrome in the course of their lives. [1] [2]. Facet syndrome occurs as a result of synovitis or disruption that is expressed as low-back pain with pseudoradicular radiation [3]. Many methods were used for management of facet syndrome. In this study 40 patients with facet syndrome were chosen to be injected by corticosteroid., and were divided into 2 groups 20 patients each. One group was injected using fluoroscopic control in the operative theater and the second group was injected using C.T guided approach. Patients were evalualed for pain relief, at intervals, and it was concluded that C.T guided approach was more accurate and efficient in treating pain

ABSTRACT

La sinovitis con derrame articular presente en varias enfermedades reumáticas puede no responder satisfactoriamente a los corticoides intraarticulares y otras terapias. Con el objetivo de evaluar diferentes tratamientos, aplicamos metrotrexato intraarticular en 36 pacientes con sinovitis en rodillas y tobillos, con falla previa a otros tratamientos. Efectuamos control histológico y por escala visual análoga (EVA) pre y postratamiento, además de contrastar la VSG y otros parámetros de laboratorio. El medicamento se aplicó en diluciones al 10 y 15 por ciento adicionado con xilocáina. Analizamos las diferencias pre y postratamiento con prueba t pareada: vSG inicial: 36.22 ñ 6.83 vs. final de 25.29 ñ 6.8 (p<0.005)). La EVA inicial fue de 9.63 y final de 3.59 (p<0.001). La mejoría clínica y el grado de satisfacción de los pacientes con el tratamiento fue calificado como muy bueno

ABSTRACT

Descrevem-se os resultados do uso de uma formulaçäo de diclofenaco em microgrânulos de liberaçäo planejada, no tratamento de 28 pacientes com traumatismos e enfermidades agudas extra-articulares, compreendendo lombalgias, sinovites, tendinites, pericapsulites, fraturas e lesöes traumáticas. O medicamento foi administrado por via oral, em cápsulas de 100 mg, sendo a posologia de uma cápsula diária. O tempo de tratamento variou de cinco a 10 dias; entretanto, mais de 50% dos casos fizeram uso da droga somente durante cinco dias. Os pacientes foram avaliados comparando-se a intensidade dos sinais/sintomas (dor, edema, rigidez articular, limitaçäo dos movimentos etc.) registrada antes do tratamento com o alívio obtido ao final do estudo. Realizou-se também uma avaliaçäo global, cujos resultados foram satisfatórios em 96,4% dos casos (18 casos curados e nove casos melhorados) e insatisfatórios em apenas um caso, que permaneceu inalterado. A boa tolerância apresentada pelo produto é colocada em destaque, bem como os fatores galênicos e farmacocinéticos que podem explicar a variaçäo dos efeitos gastrotóxicos entre os antiinflamatórios

La sinovitis simétrica seronegativa remitente con edema blando (Síndorme R S3 P E por las iniciales en ingles) constituye una forma diferente de artritis que se observa en personas de edad, generalmente mayores de 60 años y se caracteriza por tener pronóstico excelente a pesar de iniciarse en forma aguda e incapacitante en la mayoría de los casos. Se presenta el resumen de dos historias que reunen los criterios clínicos establecidos para esta entidad.

ABSTRACT

Säo relatados os resultados obtidos em um estudo multicêntrico com o tenoxim, novo agente antiinflamatório näo-esteróide em patologias diversas. Participam da investigaçäo 185 pacientes e 50 investigadores. Foram registrados resultados satisfatórios em 96,2 por cento dos casos. Os eventos adversos, de leve intensidade em sua maioria, foram registrados em 14,1 por cento dos pacientes e só constituíram causa de abandono em cinco casos. A boa tolerabilidade da droga, aliada a uma eficácia superior com apenas uma dose diária, recomendam seu emprego nas afecçöes ortopédicas.

ABSTRACT

Se presentan las historias clinicas de 2 casos que llenan los requisitos del sindrome de sinovitis simetrica seronegativa remitente con edema blando(RS 3 PE), recientemente descrito por McCarty. Son los primeros casos de este sindrome que se publican en Colombia. Se revisan las caracteristicas clinicas, de laboratorio y de rayos X. Al tratamiento aceptado con antiinflamatorios no esteroideos y cloroquina, se sugiere agregar 7.5 mg/dia de prednisolona en dosis fraccionadas. Este tratamiento se prescibio en los casos presentados y se observo que acorta la evolucion de la enfermedad y produce alivio sintomatico rapido con efectos colaterales escasos leves.
