ABSTRACT
Resumen Introducción: El bruxismo está definido como una actividad oral motora que consiste en el apretamiento o rechinamiento involuntario (rítmico o espasmódico no funcional) de los dientes, con una prevalencia de 5,9% a 49,6% en niños. Lo que podría contribuir a problemas clínicos asociados con la articulación temporomandibular. La asociación entre la disfunción temporomandibular (DTM) y el bruxismo en niños aún no está totalmente establecida. Objetivo: El objetivo de esta revisión de literatura es recopilar información actualizada de estudios que hayan abordado la asociación entre el bruxismo y la disfunción temporomandibular en niños de 3 a 12 años. Metodología: Se realizó una búsqueda en las bases de datos de Scopus, Pubmed y Scielo usando los descriptores "Niño", "Niños", "Preescolar", "Bruxismo", "Bruxismo del sueño", "Disfunción Temporomandibular"; y, "Síndrome de la Disfunción de Articulación Temporomandibular". Fueron incluidos artículos publicados en los últimos 15 años en idiomas español, inglés y portugués. Fueron incluidos estudios transversales, de cohorte, casos controles y ensayos clínicos aleatorizados. Resultados: Se evaluaron 242 artículos, de los cuales solo 11 fueron incluidos de acuerdo a los criterios de inclusión. Entre los estudios incluidos, dos de ellos no encontraron asociación entre DTM y bruxismo, tres encontraron una tendencia o probable asociación y seis encontraron una asociación positiva. Conclusiones: Existen estudios que relatan asociación entre el bruxismo y las disfunciones temporomandibulares en niños de 3 a 12 años, relacionando la presencia de signos y síntomas de la DTM con el bruxismo, pudiendo ser este último, un factor de riesgo para la presencia de DTM.
ABSTRACT Introduction: Bruxism is defined as an oral motor activity that consists of involuntary clenching or grinding (non-functional rhythmic or spasmodic) of the teeth, with a prevalence rate ranging from 5.9% to 49.6% in children, which could contribute to clinical problems associated with the temporomandibular joint. The association between bruxism in children and temporomandibular dysfunction (TMD) is not yet fully established. Objective: The objective of this literature review is to collect up-to-date information on studies that have addressed the association between bruxism and temporomandibular dysfunction in children ranging 3 - 12 years of age. Methods: A search was performed in the Scopus, PubMed and SciELO databases using the descriptors "Child", "Children", "Child, Preschool", "Bruxism", "Sleep Bruxism", "Temporomandibular Dysfunction" and "Temporomandibular Joint Dysfunction Syndrome." Articles published in the last 15 years in the Spanish, English and Portuguese languages were included. Results: A total of seventy-two articles were assessed. Only 11 articles were included according to the inclusion criteria. Among the included studies, two of them found no association between TMD and bruxism, three found a trend or probable association, and six found a positive association. Conclusion: There are studies that report an association between bruxism and temporomandibular dysfunctions in children ranging 3 - 12 years old, relating the presence of signs and symptoms of TMD with bruxism.

O objetivo do presente trabalho é apresentar um relato de caso de Disfunção Temporomandibular (DTM) dolorosa de origem muscular, tratado por meio de Agulhamento Seco (AS) e Massagem Facial (MF). Paciente do sexo feminino, M.R.B.A., de 47 anos, com diagnóstico de dor miofascial com referência do lado esquerdo e deslocamento de disco com redução do lado esquerdo, de acordo com os Critérios de Diagnóstico e Tratamento das Disfunções Temporomandibulares (DC/TMD). A paciente foi submetida a cinco sessões de AS e MF, executadas por fisioterapeuta experiente. Na primeira consulta, foi realizada a anamnese e exame físico para a obtenção de informações sobre qualidade, frequência, duração e intensidade da dor, bem como fatores de melhora, fatores de piora, fatores acompanhantes, fatores desencadeantes e tratamentos prévios. A intensidade da dor foi avaliada através de escala visual analógica e as intervenções com agulhamento seco foram iniciadas a partir da segunda consulta. Ao final das sessões, a paciente foi orientada quanto à continuidade de exercícios de relaxamento muscular e ao uso de placa interoclusal. Com base na escala adotada, o quadro doloroso geral da paciente teve evolução positiva de 6 a 0 após o tratamento, com destaque observado em seus pontos gatilhos de 10 e 8 para 6 e 3, para os lados esquerdo e direito respectivamente. Concluise que, após a aplicação da terapia proposta, a paciente obteve melhora com redução significativa em sua sintomatologia dolorosa em ambos os lados(AU)
The objective of this work is to present a case report of Dysfunction Painful temporomandibular joint (DTM) of muscular origin, treated with needling Dry (AS) and Facial Massage (MF). Female patient, M.R.B.A., 47 years old, with a diagnosis of myofascial pain with reference on the left side and displacement of disc with reduction on the left side, according to the Diagnostic and Treatment of Temporomandibular Disorders (DC/TMD). The patient underwent five sessions of AS and MF, performed by an experienced physiotherapist. At the first consultation, anamnesis and physical examination were carried out to obtain information on quality, frequency, duration and intensity of pain, as well as improvement factors, worsening factors, accompanying factors, triggering factors and previous treatments. The intensity of pain was assessed using a visual analogue scale and interventions with needling dry were started from the second consultation. At the end of the sessions, the patient was guided regarding the continuation of muscle relaxation exercises and the use of interocclusal splint. Based on the adopted scale, the patient's general painful condition was positive evolution of 6 to 0 after treatment, with emphasis on its points triggers from 10 and 8 to 6 and 3, for the left and right sides respectively. It is concluded that, after applying the proposed therapy, the patient improved with reduction significant in its painful symptoms on both sides

Objetivo: A pesquisa tem por objetivo verificar os limiares de repouso eletromiográfico dos músculos masseter e temporal em pacientes com disfunção temporomandibular (DTM) antes e após intervenção fonoaudiológica com e sem a utilização de bandagem elástica terapêutica. Métodos: A coleta contou com 14 participantes do sexo feminino, com idade entre 18 e 40 anos, com diagnóstico de DTM muscular ou mista. As pacientes foram divididas entre dois grupos classificados em: pacientes com bandagem associada à terapia tradicional (CB) e grupo de terapia tradicional (SB). As pacientes inicialmente foram avaliadas pelo exame de eletromiografia de superfície nas situações de contração voluntária máxima e repouso, e após quatro semanas de intervenção, foi realizada nova avaliação com os mesmos instrumentos. A análise dos dados ocorreu de forma quantitativa e qualitativa. Resultados:No grupo SB o músculo masseter direito apresentou aumento dos valores de repouso com significância, foi observado que o mesmo ocorreu para todos os músculos deste grupo, influenciando no equilíbrio da musculatura ipsilateral e contralateral, no entanto sem evidência estatística. O grupo CB não demonstrou valores estatísticos significativos, porém qualitativamente os valores de repouso muscular diminuíram e equilibraram-se de forma contralateral. Conclusão: Não foram observadas mudanças estatisticamente significantes nos limiares eletromiográficos durante repouso dos músculos masseter e temporal em ambos os grupos. Qualitativamente houve aumento dos valores eletromiográficos após terapia manual tradicional em todos os músculos do grupo SB. Com relação ao grupo CB, houve diminuição dos valores do repouso eletromiográfico após terapia, embora sem evidências estatísticas. (AU)
Purpose: Objective: The research aims to verify the electromyographic rest thresholds of the masseter and temporal muscles in patients with temporomandibular disorders (TMD) before and after speech therapy intervention with and without the use of therapeutic elastic bandage. Methods: The collection included 14 female participants, aged between 18 and 40 years, who had a diagnosis of muscular or mixed TMD. The patients were divided into two groups: with traditional therapy (CB) bandage and traditional therapy (SB) only group. The patients underwent initial evaluation, as well as surface electromyography in situations of maximum voluntary contraction and rest and at the end of the four weeks of intervention, a new evaluation was performed with the same instruments. Data analysis occurred quantitatively and qualitatively. Results: In the SB group, the right masseter muscle showed a significant increase in resting values. It was observed that the same occurred for all muscles in this group, influencing the balance of the ipsilateral and contralateral muscles, although without statistical evidence. The CB group did not show statistically significant values, but qualitatively the muscle rest values decreased and balanced in a contralateral way. Conclusion: No statistically significant changes were observed in the resting electromyographic thresholds of the masseter and temporalis muscles in both groups. Qualitatively, there was an increase in electromyographic values after traditional manual therapy in all muscles in the SB group. Regarding the CB group, there was a decrease in electromyographic resting values after therapy, although without statistical evidence. (AU)
Objetivo: La investigación tiene como objetivo verificar los umbrales electromiográficos de reposo de los músculos masetero y temporal en pacientes con trastornos temporomandibulares (TMD) antes y después de la terapia del habla con y sin el uso de venda elástica terapéutica. Métodos: La colección incluyó a 14 participantes mujeres, con edades entre 18 y 40 años, diagnosticadas con TTM muscular o mixta. Los pacientes fueron divididos en dos grupos clasificados en: pacientes con vendaje asociado a terapia tradicional (CB) y grupo de terapia tradicional (SB). Los pacientes fueron inicialmente evaluados mediante electromiografía de superficie en situaciones de máxima contracción voluntaria y reposo, luego de cuatro semanas de intervención se realizó una nueva evaluación con los mismos instrumentos. El análisis de datos se llevó a cabo cuantitativa y cualitativamente. Resultados: En el grupo SB, el músculo masetero derecho presentó un aumento significativo en los valores de reposo, se observó que lo mismo ocurrió para todos los músculos de este grupo, influyendo en el equilibrio de los músculos ipsilaterales y contralaterales, sin embargo, sin evidencia estadística. El grupo CB no mostró valores estadísticamente significativos, pero cualitativamente los valores de descanso muscular disminuyeron y se equilibraron contralateralmente. Conclusión: No se observaron cambios estadísticamente significativos en los umbrales electromiográficos en reposo de los músculos masetero y temporal en ambos grupos. Cualitativamente, hubo un aumento de los valores electromiográficos después de la terapia manual tradicional en todos los músculos del grupo SB. En cuanto al grupo CB, hubo una disminución de los valores electromiográficos de reposo después de la terapia, aunque sin evidencia estadística. (AU)

Introdução: As Disfunções temporomandibulares (DTM) incluem desordens dos músculos da mastigação, das articulações temporomandibulares e da inervação local, frequentemente associadas a dor orofacial e que resultam em mioartropatias do Sistema Mastigatório. A tendência atual tende a começar com tratamento conservador e progredir a procedimentos mais invasivos na falha dos tratamentos iniciais. Relato de caso: O presente relato visa mostrar o resultado de uma técnica invasiva para o tratamento de uma DTM grave, com a aplicação do ácido hialurônico e de corticoide através de uma punção guiado por ultrassonografia. A paciente apresentava dor crônica e perda importante de peso devido a limitação da abertura da boca. A RM demonstrou disfunção das ATMs, com sinais de deslocamento parcial do disco direito anteromedialmente. Foi realizada a aplicação bilateral intra-articular de ácido hialurônico e de corticoide através de uma punção guiado por ultrassonografia. Considerações Finais: A associação destas classes na punção de ATMs ainda não está bem estabelecida havendo necessidade de estudos complementares para avaliar eficácia, como este relato de caso, que se mostrou favorável com grande melhora clínica da paciente... Introduction: Temporomandibular dysfunctions (TMD) include disorders of the masticatory muscles, temporomandibular joints, and local innervation, often associated with orofacial pain and resulting in myoarthropathies of the masticatory system. The current trend tends to begin with conservative treatment and progress to more invasive procedures if the initial treatments fail. Case Report: The present report aims to show the result of an invasive technique for the treatment of a severe TMD, with the application of hyaluronic acid and corticoid through an ultrasound-guided puncture. The patient presented with chronic pain and significant weight loss due to limited mouth opening. MRI demonstrated TMJ dysfunction, with signs of partial anteromedial dislocation of the right disc. Bilateral intra-articular application of hyaluronic acid and corticoid was performed through an ultrasound guided puncture. Final considerations: The association of these classes in TMJ puncture is still not well established, and further studies are needed to evaluate efficacy, as in this case report, which proved favorable, with great clinical improvement for the patient... Introducción: Los trastornos temporomandibulares (TTM) incluyen trastornos de los músculos masticatorios, de las articulaciones temporomandibulares y de la inervación local, a menudo asociados a dolor orofacial y que dan lugar a mioartropatías del sistema masticatorio. La tendencia actual es comenzar con un tratamiento conservador y progresar hacia procedimientos más invasivos al fracasar los tratamientos iniciales. Informe de un caso: El presente informe pretende mostrar el resultado de una técnica invasiva para el tratamiento de un TTM severo, con la aplicación de ácido hialurónico y corticoide a través de una punción guiada por ecografía. El paciente presentaba dolor crónico y una importante pérdida de peso debido a la limitación de la apertura bucal. La RMN demostró una disfunción de la ATM, con signos de dislocación parcial del disco derecho anteromedialmente. Se realizó la aplicación intraarticular bilateral de ácido hialurónico y corticoide mediante una punción guiada por ecografía. Consideraciones finales: La asociación de estas clases en la punción de la ATM aún no está bien establecida y se necesitan más estudios para evaluar la eficacia, como en el reporte de este caso, que resultó favorable con gran mejoría clínica del paciente... A dor é a queixa principal dos pacientes com disfunção temporomandibular DTM- e seu controle deve ter prioridade absoluta no tratamento das DTM. A intervenção farmacológica tem sido utilizada por muitos anos. A escolha de um medicamento para o alívio da dor está diretamente relacionada ao estágio e a intensidade relatada pelo paciente, determinando que classes de drogas são eficazes, o período de utilização e efeitos colaterais. As principais classes medicamentosas utilizadas incluem: analgésicos, relaxantes musculares, anti-inflamatórios não esteroidais (AINES), antidepressivos, ansiolíticos, anticonvulsivantes, corticosteroides. O objetivo desta revisão é de abordar os principais agentes farmacológicos utilizados como coadjuvantes no tratamento de pacientes com sintomatologia dolorosa decorrente da DTM, além de analisar o raciocínio farmacológico envolvido na prescrição de cada classe de fármaco em cada tipo dor. Artigos entre 1955 e 2021, foram selecionados através das principais bases de dados como BV Salud, Scielo e Pubmed. Foram incluídos na lista de elegibilidade, artigos relacionados ao tema terapia medicamentosa no controle de dor em pacientes com DTM, dando ênfase às classes medicamentosas anteriormente mencionadas. Artigos que não possuíam texto completo ou o resumo não se referia diretamente ao tema foram excluídos. Em síntese, essa análise serve como forma de agregar conhecimento do ponto de vista teórico e auxiliar o prescritor na escolha do melhor fármaco para cada paciente, levando em consideração o tipo, duração e intensidade de cada caso, para que posteriormente, haja uma tratativa definitiva do caso.
Pain is the main complaint in patients with temporomandibular disorders - TMD - and its control should have absolute priority in the treatment of TMD. Pharmacological intervention has been used for many years. The choice of medication that will be used for pain relief is directly related to the stage and intensity reported by the patient, and these will determine which classes of drugs are effective, the period of use and the adverse effects. The usual drug classes used includes: analgesics, muscle relaxants, non-steroidal anti-inflammatory drugs (NSAIDs), antidepressants, anxiolytics, anticonvulsants, corticosteroidsThe objective of this eview is to approach the most common pharmacological agents used as adjuncts in the treatment of patients with painful symptoms resulting from TMD, and further to analyze the pharmacological logic. Articles between 1955 and 2021 were selected through the BV Salud, Scielo and Pubmed databases. Articles related to the topic "drug therapy in pain control in patients with TMD" will be included in the eligibility list, emphasizing the previously mentioned drug classes. Articles that didn't have full text or the abstract didn't directly refered to the topic were excluded. In this way, this literature review serves as a way of adding knowledge from a theoretic point of view and help the prescriber on choosing the best drug for each patient, taking into account the type, duration and intensity of each case, so that later, there is a definitive treatment of the case.

Temporomandibular disorder (TMD) is a term that covers a number of clinical problems involving the masticatory muscles, TMJ and all associated structures leading to signs and symptoms such as jaw pain, otalgia, headaches and limitation of function. In this context, TMD has been related to facial type and there are three distinct facial types (euryprosopic, mesoprosopic, and leptoprosopic). Objective: The aim of this study was to investigate the correlation between myofascial pain and facial types classified by the RDC/TMD Axis I. Material and Methods: this study was composed of 64 women aged between 12 and 49 years, using data obtained from two institutions. We used the anthropometric methodology, which meets the criteria of simplicity and reliability. We also applied the Brugsh Facial Index. The individuals were classified as euryprosopic (51.56%), mesoprosopic (12.50%) and leptoprosopic (35.94%), without statistical significance among the groups (p=0,3492). Results: there is no statistical difference between the age groups (p=0.2976) and no association between facial type and age range. Conclusion: this study found that there was a correlation between myofascial pain and facial types, with the predominance of euryprosopic faced women aged between 20 and 29 years when compared with other facial types and other age groups. A Disfunção Temporomandibular é um termo que cobre uma série de problemas clínicos envolvendo os músculos mastigatórios, ATM e todas as estruturas associadas que levam a sinais e sintomas como dor na mandíbula, otalgia, dores de cabeça e limitação de função. Nesse contexto, a DTM tem sido relacionada ao tipo facial que são classificados em três tipos distintos (euryprosopo, mesoprosopo e leptoprosopo). Objetivo: O objetivo deste estudo foi investigar a correlação entre a dor miofascial e os tipos faciais classificados pelo RDC/TMD Eixo I. Material e Métodos: este estudo foi composto por 64 mulheres com idade entre 12 e 49 anos, utilizando dados obtidos em duas instituições. Utilizou-se a metodologia antropométrica, que atende aos critérios de simplicidade e de confiabilidade. Também foi utilizado o Índice Facial de Brugsh. Os indivíduos foram classificados em euriprosopo (51,56%), mesoprosopo (12,50%) e leptoprosopo (35, 94%), sem significância estatística entre os grupos (p = 0,3492). Resultados: não houve diferença estatística entre as faixas etárias (p = 0,2976) e nenhuma associação entre tipo facial e faixa etária. Conclusão: este estudo constatou que houve correlação entre dor miofascial e tipos faciais, com predomínio de mulheres euriprosopo com idade entre 20 a 29 anos quando comparadas com outros tipos faciais e outras faixas etárias.(AU)

Abstract Temporomandibular joint dysfunction syndrome (TMD), is a collective term characterized by symptoms involving chewing muscles, temporomandibular joint and orofacial structures. The efficacy of low intensity laser (LLLT) Gallium arsenide, in combination with a non-steroidal anti-inflammatory drug (NSAID) was evaluated. The main objective was to evaluate the maximum mouth opening without pain (ABM), arthralgia in the joint capsule through visual analog scale (VAS), laterality, protrusion, joint noises and count of tablets ingested per group. A controlled clinical trial (double-blind-randomized) was carried out in 30 subjects, who presented DTM of arthrogenic etiology; 5 applications of LLLT were made with wavelength of 810 nm, output optical power of 100-200 mw, emission PW=Pulsed (1-10,000Hz), dose of 10 jouls-cm², time of 1.44 minutes in mouth closed and with the mouth half open. One more follow-up appointment per month. There were two groups: experimental and control group, where different variables were analyzed (ABM, laterality, protrusion, VAS and sociodemographic). In the control group, a supposed LT application (not active) was made, for later comparison. Pain-free ABM was assessed in all appointments in addition to the other clinical parameters. Repeated measures analysis was performed with mixed models. Thirty patients were included of which 28 finished the treatment, two of them were lost during follow-up. The groups were similar in all their baseline variables. There were no statistically significant differences when applying the final multiple regression analysis, in the ABM, or in any other of the clinical parameters analyzed. LT was not effective in treating arthrogenic DTM.
Resumen El síndrome de disfunción de la articulación temporomandibular (DTM) es un término colectivo caracterizado por síntomas que involucran músculos de la masticación, articulación temporomandibular y estructuras orofaciales. Se evaluó la eficacia del láser de baja intensidad (LLLT) Arseniuro de galio, en combinación con un antiinflamatorio no esteroideo (AINE). El objetivo principal fue evaluar la apertura bucal máxima sin dolor (ABM), la artralgia en cápsula articular a través de escala visual análoga (EVA), lateralidades, protrusión, ruidos articulares y conteo de tabletas ingeridas por grupo. Se realizó un ensayo clínico controlado (doble ciego-aleatorizado) en 30 sujetos, que presentaban DTM de etiología artrogénica; se les realizaron 5 aplicaciones de LLLT con longitud de onda de 810 nm, potencia óptica de salida de 100-200 mw, emisión PW=Pulsed (1-10,000Hz), dosis de10 jouls-cm², tiempo de1.44 minutos a boca cerrada y con la boca semiabierta. Una cita más de seguimiento al mes. Se tuvieron dos grupos: experimental y grupo control, donde se analizaron diferentes variables (ABM, lateralidades, protrusión, EVA y sociodemográficas). En el grupo control se hizo una supuesta aplicación LT (no activo), para posterior comparación. En todas las citas se valoró la ABM sin dolor además de los otros parámetros clínicos. Se realizó análisis de medidas repetidas con modelos mixtos. Se incluyeron 30 pacientes de los cuales 28 finalizaron el tratamiento, dos de ellos se perdieron en el seguimiento. Los grupos fueron similares en todas sus variables basales. No hubo diferencias estadísticas significativas al aplicar los análisis de regresión múltiple finales, en la ABM, ni tampoco en ningún otro de los parámetros clínicos analizados. El LT no fue eficaz en el tratamiento de la DTM de origen artrogénico.

Introdução: A disfunção temporomandibular é um problema de saúde pública bastante subnotificado. Além disso, o seu diagnóstico é complexo e exige o uso de instrumentos válidos e confiáveis para uso em estudos epidemiológicos. Objetivo: Validar o instrumento de diagnóstico epidemiológico da DTM (IDE/DTM), com aplicabilidade para estudos epidemiológicos nacionais. Metodologia: Trata-se de um estudo de validação, foram avaliadas a validade da estrutura interna através da Análise Fatorial Exploratória (AFE), confiabilidade (consistência interna, interobservadores e intraobservador) e a validade convergente do IDE/DTM usando os Critérios de Diagnóstico para Disfunção Temporomandibular (DC/TMD) como padrão-ouro. A amostra foi composta por 221 voluntários, 120 na etapa de AFE, 50 para o teste-reteste e 101 na validade convergente. Resultados: A AFE revelou 3 dimensões (DTM Muscular, DTM Articular e Diagnóstico diferencial) com todos os itens com cargas fatoriais > 0.4, que explicaram 73,3% da variância total. Boa consistência interna com alfa de Cronbach= 0,775. A confiabilidade variou entre substancial e excelente, reprodutibilidade interobservadores (Kappa=0,74-1,00, ICC=0,91-0,97; p< 0,001) e intraobservador (Kappa= 0,75-0,94, ICC=0,80-0,98; p< 0,001). O escore total do IDE/DTM apresentou critérios de diagnósticos válidos e satisfatórios segundo o DC/TMD (Kappa= 0,906; p<0,001), com capacidade para diferenciar indivíduos sem e com DTM com ponto de corte de 4,9 (Sensibilidade=1,0; Especificidade= 1,0; AUR=1,0), DTM Mista com ponto de corte de 14 ou mais (Sensibilidade=0,8; Especificidade=1,0; AUR=0,987), e DTM Muscular (Sensibilidade=1,0; Especificidade= 0,88; VPP= 0,89; VPN= 1,0) ou DTM Articular (Sensibilidade=0,95; Especificidade= 0,87; VPP= 0,83; VPN=0,96) com ponto de corte entre 5-13,9 pontos, sendo o diagnóstico obtido a partir da maior pontuação em cada dimensão (Muscular ou Articular). Conclusão: O IDE/DTM é um instrumento de avaliação válido e confiável, com propriedades psicométricas adequadas e satisfatórias, capaz de diagnosticar pessoas com DTM e classificar o subtipo da condição (Muscular, Articular e Mista) (AU).
Introduction: Temporomandibular disorders (TMD) is a highly underreported health problem. In addition, its diagnosis is complex and requires the use of valid and reliable instruments for use in epidemiological studies. Objective: To validate the Epidemiological Diagnostic Instrument for TMD (EDI/TMD), with applicability for national epidemiological studies. Methodology: This is a validation study, internal structure validity through Exploratory Factor Analysis (EFA), reliability (internal consistency, interobserver and intraobserver), and convergent validity of the EDI/TMD were assessed using the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) as the gold standards. The sample was composed of 221 volunteers, 120 in the EFA stage, 50 for the test-retest, and 101 for convergent validity. Results: AFE showed three factors (Muscles TMD, Joint TMD, and Differential Diagnosis) with all items having factor loadings > 0.4, which explained 73.3% of the total variance. Good internal consistency with Cronbach's alpha= 0.775. Reliability ranged from substantial to excellent, reproducibility interobserver (Kappa=0.74-1.00, ICC=0.91-0.97; p< 0.001) and intraobserver (Kappa= 0.75-1.00, ICC=0.80-0.98; p< 0.001). The EDI/TMD total score showed valid and satisfactory diagnostic criteria according to the DC/TMD (Kappa= 0.906; p<0.001), with the ability to differentiate between individuals without and with TMD with a cut-off point of 4.9 (Sensitivity=1.0; Specificity=1.0; AUR=1.0); Mixed TMD with a cut-off point of 14 or more (Sensitivity=0.8; Specificity=1.0; AUR=0.987); and either Muscles TMD (Sensitivity=1.0; Specificity= 0.88; PPV= 0.89; NPV=1.0) or Joint TMD (Sensitivity=0.95; Specificity= 0.87; PPV= 0.83; NPV=0.96), with a cut-off point between 5-13.9, with the diagnosis obtained from the highest score in each factor (Muscles or Joint). Conclusion: The EDI/TMD is a valid and reliable assessment tool, with adequate and satisfactory psychometric properties, capable of diagnosing people with TMD and classifying the subtype of the condition (Muscles, Joint, and Mixed) (AU).

Introduction: Neuromuscular deprogramming reduces the main symptoms such as pain by 70 to 90 percent in patients with temporomandibular dysfunction, but little information is available on the effect on quality of life and sleep. Objective: Determine the effect of neuromuscular deprogramming on quality of life and sleep in patients with temporomandibular dysfunction. Methods: 55 patients with temporomandibular dysfunction were included who were neuromuscularly deprogrammed (29 women and 26 men), with an average age of 34.9 ± 16.5 years. At the beginning and end of neuromuscular deprogramming, surveys were applied to assess the level of chronic pain, perception of quality of life related to oral health, perceived stress, quantity and quality of life, anxiety and depression. Results: In 37 patients (68 percent) pain was identified on examination, and it was confirmed in the chronic pain survey. The quality of life perception score was correlated with low sleep quality (r = 0.39; p = 0.008); pain score (r = 0.48; p = 0.003); anxiety (r = 0.55; p = 0.003) and depression (r = 0.41; p = 0.006). Neuromuscular deprogramming reduced patient-reported pain levels from 9.9 to 2.9 (p = 0.001), the percentage of patients with poor sleep quality from 60 percent to 29 percent (p < 0.0001), the quality of life score from 40.7 to 23.8 (p = 0.03), and perceived stress levels from 22.1 to 19.1 (p = 0.002). Conclusions: In patients with temporomandibular dysfunction, neuromuscular deprogramming reduces the level of pain. It is related to better perception in quality of life, higher quality of sleep and decreases perceived stress.
Introducción: La desprogramación neuromuscular reduce los síntomas principales como el dolor de 70 a 90 por ciento en los pacientes con disfunción temporomandibular, pero se dispone de escasa información sobre el efecto en la calidad de vida y sueño. Objetivo: Determinar el efecto de la desprogramación neuromuscular en la calidad de vida y sueño en pacientes con disfunción temporomandibular. Métodos: Se incluyeron 55 pacientes con disfunción temporomandibular que fueron desprogramados neuromuscularmente (29 mujeres y 26 hombres), con edad promedio de 34,9 ± 16,5 años. Al inicio y final de la desprogramación neuromuscular, se aplicaron las encuestas para evaluar el nivel de dolor crónico, percepción de calidad de vida relacionada con la salud oral, estrés percibido, cantidad y calidad de vida, ansiedad y depresión. Resultados: En 37 pacientes (68 por ciento) se identificó dolor a la exploración confirmado en la encuesta de dolor crónico. El puntaje de percepción de calidad de vida se correlacionó con baja calidad de sueño (r = 0,39; p = 0,008); el puntaje de dolor (r = 0,48; p = 0,003); ansiedad (r = 0,55; p = 0,003) y depresión (r = 0,41; p = 0,006). La desprogramación neuromuscular redujo los niveles de dolor referidos por el paciente de 9,9 a 2,9 (p = 0,001), el porcentaje de pacientes con pobre calidad de sueño de 60 por ciento a 29 por ciento (p < 0,0001), el puntaje de la calidad de vida de 40,7 a 23,8 (p = 0,03) y los niveles de estrés percibido de 22,1 a 19,1 (p = 0,002). Introduction: The presence of chronic pain and mood disorders can be related to the performance of intellectual and technical tasks. Objective: This study evaluated the correlation between anxiety level, chronic orofacial pain of temporomandibular disorders (TMD), and academic performance in dental students. Methods: One hundred ninety-five students (74 men and 121 women) answered the Spielberger's traitstate anxiety inventory to evaluate the level of anxiety; the Research Diagnostic Criteria for TMD (RDC/TMD) was used to analyze chronic orofacial pain of TMD, and the academic performance was evaluated through the average grade of all college subjects concluded by the students. Correlations between the presence of chronic orofacial pain of TMD, trait, and state anxiety levels, chronic pain grade (CPG), chronic pain intensity (CPI), and academic performance were tested using Pearson's correlation test. Results: The mean age of the students was 21.8 years (SD=2.3). Chronic TMD was observed in 37.5% of the students. The majority of students had moderate trait and state anxiety. A significant correlation was observed between traitanxiety level and CPG (r=0.148, p=0.044), and CPI (r=0.187, p=0.009). No significant correlation was found between academic grade and presence of chronic pain of TMD (r=0.041, p=0.571), trait (r=0.079, p=0.273) and state-anxiety (r=0.107, p=0.136). Conclusion: The CPG and CPI increase in participants with higher trait-anxiety levels, however, no significant correlation was found between academic performance and trait/state anxiety or chronic orofacial pain of TMD.

Objective: To screen the physical, psychological and behavioral factors related to patients with temporomandibular disorders (TMD) by using Axis Ⅱ assessment instruments of diagnostic criteria for TMD(DC/TMD). And to provide a reference to establish personalized diagnosis and treatment plans for TMD patients so as to prevent TMD and reduce predisposing factors. Methods: A total of 141 TMD patients, who were admitted in the Department of Oral and Maxillofacial Surgery in School and Hospital of Stomatology, Wuhan University from October 2018 to February 2021 were selected. There were 121 females and 20 males, with an average age of 30 years. A total of 90 healthy people were included as controls. A full-time psychologist conducted relevant questionnaire surveys. The questionnaires include general clinical survey forms and TMD symptom questionnaire. In addition, Axis Ⅱ assessment instruments include graded chronic pain scale, jaw functional limitation scale, oral behaviors checklist, patient health questionnaire-9 (depression), generalized anxiety disorder scale, patient health questionnaire-15 (physical symptoms), etc. The main observational indicators include: pain level, pain impact rates, overall classification of chronic pain, limited chewing function score, limited motor function score, limited communication function score, total jaw function restricted score, depression score, anxiety score, somatic symptom score and oral behavior score.The survey data were imported into SPSS 22.0 software for statistical analysis. Results: In the TMD group 60.3% (85/141) patients had various degrees of pain, 24.1% (34/141) of those with pain effect grades from 1 to 3 and 61.0% (86/141) showed chronic pain overall grades from Ⅰ to Ⅳ. The chewing function restricted score was 2.67(1.17, 4.25), motor function restricted score was 4.25(1.75, 6.12), communication function restricted score was 1.13(1.00, 2.25) and total jaw function restricted score was 2.56(1.47, 4.15) respectively. Patients with mild depression or above accounted for 59.6%(84/141), patients with mild anxiety or above accounted for 56.7%(80/141), 46.1%(65/141) patients had somatization symptoms. Statistical differences (P<0.05) were determined between TMD group and control group in various scores of jaw function, oral behavior grading, depression, anxiety, and physical symptoms. Physical symptoms had significantly statistical difference between different diagnostic classification(P<0.05). Meanwhile, among the different chronic pain levels in the TMD group, there were statistical differences in the various scales of mandibular dysfunction, depression, anxiety, and somatization. In the TMD group, other significant differences were noticed between males and females in terms of the average score of mouth opening, verbal and facial communication, the total score of mandibular dysfunction as well as physical symptoms (P<0.05). Conclusions: Compared with the healthy people, patients with TMD had more abnormal oral behaviors, different restriction of the mandibular functional activities. At the same time, depression, anxiety, and somatization were more serious. Patients with osteoarthritis and subluxation of temporomandibular joint were more likely to suffer physical symptoms. TMD patients suffering from pain had more severe mandibular dysfunction and symptoms of depression, anxiety, and somatization.

Abstract Objective: To assess the association between temporomandibular disorder (TMD) with sociodemographic factors, health-related factors, and oral conditions in adolescents. Material and Methods: This cross-sectional study was developed with 89 adolescents between 13 to 18 years. TMD diagnosis was obtained by the Research Diagnostic Criteria for Temporomandibular Disease (RDC/TMD) Axis I. Diagnoses of chronic pain, depression, presence of non-specific physical symptoms, including pain and anxiety, were obtained by the RDC/TMD Axis II. The feeling of happiness was measured by the Subjective Happiness Scale. Socio-economic and demographic characteristics were collected through self-administered questionnaires. Patients were examined for dental caries, dental trauma, malocclusion, and tooth wear. Parents answered a sociodemographic, economic, and general health questionnaire. Data were submitted to descriptive statistics, and a logistic regression model was used to assess the association between TMD and the socio-economic, demographic, health-related, and clinical variables. Results: TMD prevalence was 42%. TMD was associated to skin color (p=0.040), use of medications in the past year (p = 0.020) and previous dental trauma (p=0.030). Also, it tended to be associated with the presence of probable awake bruxism (p=0.053). Conclusion: Sociodemographic factors, health-related factors, and oral conditions play a role in TMD, with nonwhite adolescents, those who had used medications in the past year and/or had previous dental trauma having a greater chance of present this disorder (AU).

Introdução: As disfunções temporomandibulares apresentam uma variedade de sinais e sintomas que afetam a articulação temporomandibular, músculos da mastigação e estruturas relacionadas e muitos pacientes apresentam queixas cervicais. Objetivo: Verificar a correlação de dor à palpação dos músculos da mastigação (masseter, temporal anterior, pterigoideos lateral e medial) e digástrico, com a queixa de dor no pescoço; verificar a correlação de dor à palpação nos músculos da mastigação e digástrico, com dor à palpação no esternocleidomastoideo e trapézio, e se a força de correlação é diferente entre eles. Material e método: Foram avaliados 232 prontuários da clínica odontológica das Disfunções Temporomandibulares da Faculdade de Odontologia de Araçatuba, do período de 2011 a 2013. Os dados coletados foram submetidos à análise estatística, com alfa (α) = 0.01 para todos os casos, exceto digástrico quando associado ao trapézio (α) = 0.05. Resultado: A maioria dos pacientes era do sexo feminino. Houve correlação positiva entre dor à palpação nos músculos temporal, masseter, pterigoideo lateral, esternocleidomastoideo e trapézio, e queixa de dor no pescoço. Também houve correlação positiva entre a dor em todos os músculos da mastigação (masseter, temporal, pterigoideo lateral e medial) e digástrico e a dor no esternocleidomastoideo. Bem como a correlação de presença de dor nos músculos masseter, temporal, pterigoideo lateral e digástrico com dor no trapézio. A correlação de dor foi mais forte para o músculo esternocleidomastoideo, exceto para o pterigoideo lateral. Conclusão: Existe correlação positiva entre a queixa de dor à palpação nos músculos da mastigação, exceto pterigiodeo medial, e os músculos cervicais (esternocleidomastoideo e trapézio). A força de correlação entre a dor do masseter e temporal anterior com o esternocleidomastoideo é mais forte do que com o trapézio.
Introduction: Temporomandibular disorders present a variety of signs and symptoms that affect the temporomandibular joint, masticatory muscles and related structures, and many patients have cervical complaints. Objective: To verify the correlation of pain on palpation of the mastication muscles (masseter, anterior temporal, lateral and medial pterygoid), and digastric muscles with the complaint of neck pain; to verify the correlation of pain on palpation in the masticatory and digastric muscles with pain on palpation in the sternocleidomastoid and trapezius and if the correlation strength is different between them. Material and method: From 2011 to 2013, 232 medical records from the dental clinic of Temporomandibular Disorders of the Faculty of Dentistry of Araçatuba were evaluated. The data collected were submitted to statistical analysis, with alpha (α) = 0.01 for all cases, except digastric when associated with the trapezoid (α) = 0.05. Result Most patients were female. There was a positive correlation between pain on palpation in the temporal, masseter, lateral pterygoid, sternocleidomastoid and trapezius muscles, and complaints of neck pain. There was also a positive correlation between pain in all muscles of mastication (masseter, temporal, lateral and medial pterygoid), and digastric and sternocleidomastoid pain. As well as the correlation of the presence of pain in the masseter, temporal, lateral pterygoid and digastric muscles with pain in the trapezius. Pain correlation was strongest for the sternocleidomastoid muscle, except for the lateral pterygoid. Conclusion There is a positive correlation between the complaint of pain on palpation in the masticatory muscles, except Medial pterygoid, with the cervical muscles (sternocleidomastoid and trapezius). The strength of correlation between masseter and anterior temporal pain with the sternocleidomastoid is stronger than with the trapezius.

A disfunção da articulação temporomandibular é uma doença que afeta o osso, a cartilagem e os tecidos musculoesqueléticos associados, caracterizada pela presença de dor facial e articular, limitação de abertura de boca, travamento e ruídos articulares. O objetivo deste trabalho prospectivo, simples-cego, randomizado foi avaliar a melhora de abertura interincisal e dor articular em 28 pacientes. Neste ensaio clínico os pacientes foram divididos em 3 grupos aleatórios de acordo com a técnica de tratamento que foi utilizada. Os pacientes do grupo 1 foram submetidos a artrocentese isolada e os do grupo 2 e 3 a artrocentese seguida de infiltração de ácido hialurônico ou plasma rico em plaquetas (PRP), respectivamente. Os pacientes foram reavaliados para avaliação de abertura interincisal dor articular e qualidade de vida após 2 semanas, 1, 3 e 6 meses. Nenhuma diferença estatisticamente significativa foi observada entre os grupos para qualquer uma das alterações nos parâmetros EAVN ou medidas MAI(p< 0.05). Para a variável qualidade de vida os resultados foram clinicamente significativos em todos os grupos avaliados (p>0.05). Os três grupos avaliados apresentaram melhoras clínicas significativas(AU)
Temporomandibular joint dysfunction is a disease that affects bone, cartilage and associated musculoskeletal tissues, characterized by the presence of facial and joint pain, limited mouth opening, locking and joint noises. The aim of this prospective, single-blind, randomized study was to evaluate the improvement of interincisal opening and joint pain using a visual analog scale in 28 patients. In this clinical trial, patients were divided into 3 random groups according to the treatment technique that will be used. Patients in group 1 underwent arthrocentesis alone and those in groups 2 and 3 underwent arthrocentesis followed by infiltration of hyaluronic acid or platelet-rich plasma (PRP), respectively. Patients were reassessed for assessment of interincisal opening joint pain and quality of life after 2 weeks, 1, 3, and 6 months. No statistically significant differences were observed between the groups for any of the changes in VAS parameters or MAI measurements (p< 0.05). For the quality of life variable, the results were clinically significant in all groups evaluated (p>0.05). The three groups evaluated showed significant clinical improvements(AU)

Introduction Temporomandibular disorder (TMD) involves the mastication and temporomandibular articulation muscles. Completely toothless patients with prostheses should be thoroughly assessed since edentulism causes damage that can lead to TMD onset. Objective This study aims to assess the effect of the technical quality of total prostheses (TPs) on TMD occurrence in patients with bimaxillary TPs. Material and method 154 patients responded to a questionnaire to obtain personal data, information regarding prostheses, anamnestic index for TMD, and the prosthesis quality index. Result Of the 154 patients examined, 64 (41.6%) had no TMD, 61 (39.6%) had mild TMD, 23 (14.9%) moderate, and only 6 (3.9%) showed severe TMD. As for the quality of total prosthesis, 78 (50.6%) completely toothless individuals had satisfactory TPs while 76 (49.4) presented unsatisfactory prostheses. Regarding the time using total prosthesis, around 50% of individuals with up to 10 years use presented some degree of TMD and 70% of individuals with over 10 years presented TMD. Conclusion Considering the methodology used and the population studied, we conclude that the quality of the TPs did not influence the incidence of TMD.
Introdução A disfunção temporomandibular (DTM) é uma desordem que envolve os músculos da mastigação e da articulação temporomandibular. Pacientes desdentados totais e que utilizam protéses requerem uma avaliação criteriosa, já que o edentulismo leva a prejuízos que podem contribuir para o aparecimento de DTM. Objetivo O objetivo deste presente estudo foi avaliar o efeito da qualidade técnica das próteses totais (PTs) na incidência de DTM em pacientes portadores de PTs bi-maxilares. Material e método Foi aplicado um questionário a 154 pacientes contendo os dados pessoais, o histórico das próteses, o índice anamnésico para DTM e o índice de qualidade de prótese. Resultado Dos 154 pacientes examinados, 64 (41,6%) estavam livres de DTM, 61 (39,6%) possuíam DTM leve, 23 (14,9%), moderada e apenas 6 (3,9%) mostravam DTM severa. Quanto à qualidade da prótese total, 78 (50,6%) desdentados totais portavam PTs satisfatórias e 76 (49,4) estavam com próteses insatisfatórias. Quanto ao tempo de uso da prótese total, cerca de 50% dos indivíduos com até 10 anos de uso apresentaram algum grau de DTM e 70% dos indivíduos com mais de 10 anos acusaram a presença de DTM. Conclusão Considerando-se a metodologia empregada e a população estudada, pôde-se concluir que a qualidade das PTs não influenciou na presença de DTM.

RESUMO Objetivo identificar e sintetizar evidências sobre estratégias utilizadas no treino da mastigação e deglutição em indivíduos com disfunção temporomandibular e dor orofacial. Estratégia de pesquisa revisão de escopo desenvolvida com consulta nas bases de dados MEDLINE, LILACS, BBO, IBECS, BINACIS, CUMED, SOF, DeCS, Index Psi, LIPECS e ColecionaSUS (via BVS), Scopus, CINAHL, Embase, Web of Science, Cochrane e na literatura cinzenta: Biblioteca Digital Brasileira de Teses e Dissertações (BDTD), OpenGrey e Google Acadêmico. Critérios de seleção estudos quantitativos ou qualitativos, sem limite temporal e sem restrição de idioma, que continham os seguintes descritores ou palavras-chave: Articulação Temporomandibular, Síndrome da Disfunção da Articulação Temporomandibular, Transtornos da Articulação Temporomandibular, Dor Facial, Mastigação, Deglutição, Terapêutica, Terapia Miofuncional e Fonoaudiologia. Na primeira etapa, dois revisores fizeram a triagem independente dos estudos, por meio da leitura dos títulos e resumos. Na segunda etapa, os revisores leram, independentemente, os documentos pré-selecionados na íntegra. Em caso de divergência, um terceiro pesquisador foi consultado. Resultados as 11 publicações incluídas foram publicadas entre 2000 e 2018. As estratégias mais utilizadas foram o treino da mastigação bilateral simultânea, seguido da mastigação bilateral alternada. Na deglutição, foi proposto aumento do tempo mastigatório para reduzir o alimento em partículas menores e lubrificar melhor o bolo alimentar e treinos com apoio superior de língua. Conclusão o treinamento funcional demonstrou efetividade na reabilitação dos pacientes, embora não siga uma padronização e não seja realizado de forma isolada. Os estudos encontrados apresentam baixo nível de evidência. Considera-se fundamental a realização de estudos mais abrangentes e padronizados, como ensaios clínicos randomizados.

In the first stage, two reviewers independently screened the studies by title and abstract reading. In the second stage, the reviewers independently read the preselected documents in full text. In case of divergences, a third researcher was consulted. Results The 11 documents included in the review were published between 2000 and 2018. The mostly used training strategies were simultaneous bilateral mastication/chewing, followed by alternating bilateral mastication. In swallowing, increased mastication time was proposed to break food into smaller bits and better lubricate the bolus; training with upper tongue support was also indicated. Conclusion Functional training proved to be effective in rehabilitation, although it was not standardized or performed alone. The studies had low levels of evidence. It is essential to conduct more encompassing and standardized studies, such as randomized clinical trials.

RESUMO Objetivos Verificar o efeito da aplicação da bandagem elástica sobre músculos mastigatórios no alívio da dor, em comparação com outras intervenções, em indivíduos com disfunções temporomandibulares. Estratégia de pesquisa Busca nas bases de dados LILACS, IBECS, CINAHL, Scopus, Web of Science, Cochrane, Embase e MEDLINE. A pergunta norteadora, utilizando-se os elementos da estratégia PICOT (população, intervenção, comparador, outcome/desfecho, tipo de estudo) foi: "A bandagem elástica promove alívio da dor em indivíduos com disfunção temporomandibular?". Critérios de seleção Foram incluídos ensaios clínicos que fizeram uso da bandagem elástica em músculos mastigatórios de indivíduos com disfunção temporomandibular, publicados em português, inglês ou espanhol. Foram excluídos os artigos que não abordavam o método de aplicação e o desfecho "intensidade da dor". A avaliação da elegibilidade foi realizada pela leitura dos títulos e resumos, bem como pela leitura dos estudos na íntegra. Foram extraídas informações sobre ano de publicação, país de condução do estudo, idade e condição clínica da amostra, tratamento e resultados da avaliação da dor. Na metanálise, realizada por meio do método do inverso da variância, a média do valor indicado na escala visual analógica foi considerada como medida de efeito da intervenção. Resultados Foram localizadas, inicialmente, 344 referências, das quais, 3 foram selecionadas. Foram identificados resultados significativamente superiores na redução da dor, em uma semana de uso da bandagem, na comparação com outras abordagens conservadoras analisadas. Conclusão Considerando os artigos incluídos, a bandagem elástica apresentou resultados significativos para maior redução da dor na primeira semana. Porém, o número reduzido de estudos e a presença de vieses limitam os achados.
ABSTRACT Purpose To verify the effects on pain relief by applying kinesiology tape on the masticatory muscles, in comparison with other interventions, in people with temporomandibular disorders. Research strategy Search in LILACS, IBECS, CINAHL, Scopus, Web of Science, Cochrane, EMBASE, and MEDLINE. The research question, based on the PICOT components, was: "Does kinesiology tape applied on to masticatory muscles relieve the pain in people with temporomandibular disorders?". Selection criteria The included articles were clinical trials using kinesiology tape on masticatory muscles in people with temporomandibular disorders, published in Portuguese, English, or Spanish. Articles whose authors had not adopted this application method and pain intensity as an outcome were excluded. Eligibility was assessed by reading their titles, abstracts, and full texts. The following information was extracted: year of publication, the country where the study was conducted, age and clinical condition of the sample, treatment, and pain assessment results. The meta-analysis, using the inverse variance method, considered the mean value indicated in the visual analog scale as the intervention effect measure. Results Initially, 344 references were retrieved, of which three were selected. Significantly better pain relief results were identified after 1 week of using the tape, in comparison with the other conservative approaches analyzed. Conclusion Considering the included articles, the kinesiology tape had significant results with greater pain relief in the first week. However, small number of studies and their biases limit the findings.

Objective: Compare the clinical effectiveness of custom thermoformed occlusal splints (OS) alongside behavioral and self-care therapy (BST) in the management of myalgia of the masticatory muscles. Material and methods:A controlled clinical trial was conducted with a total of 46 subjects with a diagnosis of myalgia according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD). All subjects were treated with BST at the beginning of the study and were then randomized into four groups: behavioral and self-care control group; Thermoformed Tough-elastic splint group; Thermoformed Soft-elastic splint group, and non-occlusive splint group. Follow-ups were carried out at 2, 6, and 10 weeks, where it was evaluated: pain in the masticatory muscles, mandibular range of motion, mandibular functional limitation, and occlusal discomfort. Data were analyzed with Doornik and Hansen, ShapiroWilk, and ANOVA at p=0.05. Results: All the variables showed significant improvement (p<0.05) from the first follow-up and were maintained later. BST control group, as well as groups with BST associated with OS, were able to reduce pain and increase the mandibular range of motion without significant differences between them (p>0.05), while the Thermoformed Tough-elastic splint was the most efficient in terms of the mandibular functional limitation. The occlusal discomfort decreased over time, but without statistically significant differences in terms of time and design of OS. Conclusion: The addition of thermoformed OS to behavioral and self-care therapy does not have a significant impact on myalgia of the masticatory muscles. (AU)
Objetivo: Comparar a eficácia clínica das Placas Oclusais (PO) termoplásticas personalizadas associadas à Terapia Cognitiva Comportamental (TCC) na condução da mialgia dos músculos mastigatórios. Material e Métodos: Foi realizado um ensaio clínico controlado randomizado com um total de 46 participantes com um diagnóstico de mialgia de acordo com os Critérios de Diagnóstico das Desordens Temporomandibulares (DC/TMD). Todos os participantes foram tratados com a TCC, no início do estudo, e foram, depois, randomizados em quatro grupos: grupo controle Terapia Congnitivo Comportamental; grupo de placa termoplástica dura-soft, grupo de placa termoplástica soft, e grupo de placa sem cobertura oclusal. Foram realizados controles com 2, 6, e 10 semanas, onde foi avaliado: dor nos músculos mastigatórios, amplitude de movimento mandibular, limitação funcional mandibular, e desconforto oclusal. Os dados foram analisados com Doornik e Hansen, Shapiro-Wilk, e ANOVA a p=0,05. Resultados: Todas as variáveis mostraram melhora significativa (p<0,05) desde o primeiro controle e se mantiveram posteriormente. O grupo de controlo da TCC, bem como os grupos com TCC associada a PO, foram capazes de reduzir a dor e aumentar a amplitude de movimento mandibular sem diferenças significativas entre eles (p>0,05), enquanto que, a placa termoplástica dura-soft, foi a mais eficiente em termos da limitação mandibular funcional. O desconforto oclusal diminuiu ao longo do tempo, mas, sem diferenças estatisticamente significativas em termos de tempo e design da PO. Conclusão: A inclusão da Terapia Cognitivo Comportamental à PO termoplástica não tem um impacto significativo na mialgia dos músculos mastigatórios.(AU)

Resumen Objetivo. Comparar posición condilar y espacio articular en articulación témporo-mandibular, en pacientes con y sin disfunción témporo-mandibular mediante tomografía Cone Beam. Métodos. Criterios diagnósticos de investigación para trastornos témporo-mandibulares Eje II, n=50 pacientes ambos sexos, edad comprendida 18 - 27 años. Muestra aleatoria n= 50 pacientes equivalente a 100 articulaciones, se obtuvo: n=25 pacientes sin disfunción témporo-mandibular, considerados asintomáticos 25 con y sin disfunción témporo-mandibulares, considerados sintomáticos. Mediante tomografía Cone Beam de articulación témporo-mandibular boca abierta - boca cerrada se avaluó de forma manual los espacios interarticulares. Resultados. El espacio condilar anterior en pacientes con y sin disfunción témporo-mandibular no presentó diferencia significativa, p=0,30. La posición condilar tampoco mostró diferencia significativa p=0,58. En pacientes con y sin disfunción témporo-mandibular (sintomáticos) la posición central y posterior del cóndilo (35,2%), pacientes con y sin disfunción témporo-mandibular (asintomáticos) la posición anterior y central fue más significativa (37,0%); seguido de la posición posterior del cóndilo (26,1%). Conclusión. No existe diferencia significativa en la posición condilar y el espacio interarticular en pacientes sintomáticos y asintomáticos.
Abstract Aim. To compare the condylar position and joint space of the temporomandibular join in individuals with and without temporomandibular dysfunction, using cone beam computed tomography (CBCT). Methods. Research Diagnostic Criteria for Temporomandibular Disorders Axis II (n= 50) patients both sexes, age 18 - 27 years. Random sample (n=50) patients equivalent to 100 joints, obtaining: n-25 patients without temporomandibular dysfunction, considered asymptomatic and 25 patients with and without temporomandibular dysfunction, considered symptomatic. By tomography Cone Beam of open mouth temporomandibular joint - closed mouth was manually evaluated the interarticular spaces. Results. The anterior condylar position space and condylar position not significantly different in induvials with and without temporomandibular dysfunction temporomandibular disorders p=0,30 and p=0,58, respectively. Conclusions. There is no significant difference in the condylar position and intraarticular.

Objetivos: determinar a prevalência da disfunção temporomandibular (DTM) em pacientes de uma clínica escola de Odontologia, descrever o perfil epidemiológico da amostra analisada, caracterizar o grau dessas disfunções e identificar os sinais e sintomas orofaciais mais frequentes. Método: estudo epidemiológico, do tipo transversal, que avaliou pacientes da Clínica Escola de Odontologia da Universidade de Rio Verde, GO. A pesquisa foi realizada através de três questionários: Índice Anamnésico Simplificado de Fonseca (IASF), Questionário para Triagem para Dor Orofacial (QTDO), questionário sociodemográfico e econômico. Resultados: foram analisados 200 pacientes, dentre os quais 78,5% apresentaram algum grau de DTM, sendo o mais prevalente a DTM de leve intensidade, presente em 38% da amostra. Conclusão: nesse cenário, conclui- -se que a maioria dos participantes apresentou DTM, sendo que a intervenção multidisciplinar seria de suma relevância no tratamento, podendo oferecer uma abordagem mais completa e com melhores resultados.(AU)
Objectives: to determine the prevalence of TMD in patients at a dental school clinic, as well as describe its epidemiological profile for the evaluated sample, characterizing the degree of this dysfunction and identifying the most frequent orofacial signs and symptoms. Method: it is an epidemiological, cross-sectional study that evaluated patients at the School of Dentistry clinic, University of Rio Verde, GO, Brazil. The study was carried out using three questionnaires: Simplified Fonseca Anamnesis Index (SFAI), Orofacial Pain Screening Questionnaire (OPSQ), and the socio-demographic and economic questionnaire. Results: 200 patients were evaluated, among which 78.5% had some degree of TMD, the most prevalent being mild TMD, present in 38% of the sample. Conclusion: most participants had TMD, and multidisciplinary intervention would be of paramount importance in the treatment, being able to offer a more complete approach and with better results.(AU)
