ABSTRACT
Introducción: la pandemia por COVID-19 afectó negativamente los sistemas de salud. Las fracturas vertebrales osteoporóticas y el aislamiento social se relacionan con mayor morbimortalidad. Objetivos: caracterizar la morbilidad de las fracturas vertebrales osteoporóticas y evaluar una posible relación entre morbilidad y nivel de aislamiento social se-cundario al estado de pandemia. Material y métodos: estudio observacional, analítico y transversal. Resultados: se incluyeron en el estudio 45 adultos. La fractura fue mayorita-riamente lumbar con una evolución superior de 3 meses. El 35% presentaba seguimiento, el 48% había recibido tratamiento para osteoporosis y el 48% presentaba fracturas previas documentadas. El 46% refirió falta de accesibilidad al sistema. Se evaluó el aisla-miento social con la escala sociofamiliar de Gijón, que evidenció una situación buena en el 75% y un deterioro social intermedio/severo en el 24%. El Índice de Oswestry mostró una discapacidad mínima/moderada en el 66% y severa o mayor en el 33%. Se evaluó el dolor por la Escala análoga visual (VAS) y se obtuvo un VAS mayor de 5 en el 57%. Al comparar el grupo de situación sociofamiliar buena con el de deterioro social intermedio/severo se observó una diferencia en multipli-cidad de fracturas (p 0,030), hipovitaminosis D (p 0,045) y falta de accesibilidad (p 0,029). En discapacidad y dolor no hubo diferencias. Conclusión: el grupo con mayor aislamiento presentó una enfermedad más severa en términos de multiplicidad de fracturas e hipovitaminosis D; esto podría indicar una asociación entre aislamiento social secundario al estado de pandemia y morbilidad por las fracturas vertebrales. Introduction: the COVID-19 pandemic had a negative impact on healthcare systems. Osteoporotic vertebral fractures and social isolation have a significant morbidity in our setting. Objectives: to characterize the morbidity of osteoporotic vertebral fractures and evaluate a potential relationship between morbidity and the level of social isolation secondary to the pandemic. Material and methods: observational, analytical and cross-sectional study. Results: forty-five adults were included. Fractures were mostly lumbar with a history of over 3 months. Thirty-five percent (35%) had been followed-up, 48% had been treated for osteoporosis and 48% had previous documented fractures. Forty-six percent (46%) reported lack of accessibility to healthcare. Social isolation was measured using Gijón Ìs social-familial evaluation scale, which showed a good situation in 75% of cases and an intermediate/severe social deterioration in 24%. According to the Oswestry index, disability was minimal/moderate in 66% of cases and severe or worse in 33%. Pain was assessed using the Visual Analogue Scale (VAS), with a score greater than 5 recorded in 57% of patients. When comparing the group with a good social-familial situation vs. the group with intermediate/severe social deterioration, differences were found in multiplicity of fractures (p 0.030), hypovitaminosis D (p 0.045) and lack of accessibility (p 0.029). No differences were found in disability and pain. Conclusion: the group with higher levels of isolation exhibited more severe disease in terms of multiplicity of fractures and hypovitaminosis D, which might suggest an association between social isolation secondary to the pandemic and morbidity due to vertebral fractures. INTRODUCTION: Recent studies have shown that low vitamin D levels constitute a potential risk factor for the development of cognitive impairment. The present study aimed to investigate the association between vitamin D levels and the suspicion of cognitive impairment in Chilean older adults. MATERIAL AND METHOD: We performed a cross-sectional study, including 1,287 participants ≥ 65 years (56.8% were women, age range 65 to 97 years) from the Chilean National Health Survey. Cognitive impairment was assessed using the Mini Mental State Examination (MMSE). Participants were classified into three groups according to their vitamin D levels (> 29 ng/ml sufficient, 12-29 ng/ml deficit, and < 12 ng/ml severe deficit). The association between vitamin D levels and cognitive impairment was explored using logistic regression analysis, adjusted for confounding factors. RESULTS: The prevalence of vitamin D deficit and vitamin D severe deficit was 37.7% and 21.0%, respectively. Compared to older adults with sufficient levels of vitamin D, those with severe deficits had a 94% (OR: 1.94 [95% IC: 1.27; 1.66], p = 0.002) higher odds of cognitive impairment (unadjusted model). Adjusting according to sociodemographic factors, lifestyle, adiposity, sunlight exposure, and multimorbidity slightly attenuated the association to 61% (OR: 1.61 [95%IC: 1.03; 2.19], p = 0.046), but remain significant. CONCLUSION: A severe deficit of vitamin D was associated with higher odds of cognitive impairment in Chilean older adults independent of major confounding factors. Future studies are needed to provide causal evidence between vitamin D and the suspicion of cognitive impairment.

Se determinó el nivel de 25-hidroxivitamina D ([25(OH)-D]) al ingreso y tras suplementación con 100.000 UI semanales de colecalciferol, durante 3 o 5 semanas, en casos de insuficiencia ([25(OH)-D] entre 20 ng/mL y 29,9 ng/mL) o deficiencia ([25(OH)-D] < 20 ng/mL), respectivamente. Se determinó la prevalencia de hipovitaminosis D, el porcentaje de normalización de [25(OH)-D], y los efectos adversos. RESULTADOS Se evaluaron 56 pacientes; 98,2% presentó hipovitaminosis D, y 28 (73,7%) y 10 (26,3%) presentaron déficit e insuficiencia, respectivamente. Tras la suplementación, 92,1% alcanzaron niveles [25(OH)-D] normales. Ningún paciente presentó efectos adversos. DISCUSIÓN La prevalencia de deficiencia de vitamina D en nuestra población fue mayor a la reportada en la literatura. Comprobamos que un esquema de suplementación en altas dosis de vitamina D es seguro, y más efectivo que los previamente recomendados. Este esquema de suplementación puede ser implementado en futuros estudios randomizados. CONCLUSIÓN La prevalencia de hipovitaminosis D en pacientes adultos chilenos con fractura de tibia fue alta (98,2%). El esquema de suplementación con vitamina D propuesto fue efectivo y seguro.

ABSTRACT Objective: This study aimed to assess vitamin D (25OHD) levels in individuals who underwent an examination at a private laboratory (between latitudes 14° and 22° south) over 14 years, stratified by sex, age, and epidemiological profiles, and determine variations in the number of tests performed over the years. Materials and methods: All records of 25OHD tests performed at a private clinical laboratory in Brazil were analyzed. This retrospective cross-sectional study included patients stratified by sex (female or male), age range (0-17, 18-40, 41-59, and ≥ 60 years), and year of testing. The final sample size was 193,725 patients. Categorical variables are presented as absolute and relative frequencies and numerical variables as means ± standard deviation. Comparisons between groups were performed using the equality of proportions test. Results: The number of tests performed steeply increased since 2010. More tests were performed in female individuals (73.3%) and individuals aged 41-59 years (32.2%). Most samples (68.0%) demonstrated sufficient vitamin D status. Women had a higher incidence of vitamin D deficiency than men (33.1% and 26.6%, respectively; p < 0.001). Individuals aged ≥ 60 years had the highest incidence of vitamin D deficiency (68.4%), while individuals aged 0-17 years had the lowest (32.2%) (p < 0.001). Conclusion: Despite increased testing and attention given to vitamin D in recent years, our study demonstrates high levels of deficiency in a country with geographical conditions favorable to its production.

Objective: To explore current vitamin D status and influential factors of vitamin D deficiency and insufficiency among children under 7 years of age in 11 provinces, autonomous regions or municipalities of China. Methods: According to the "province-city-hospital" sampling technical route, a total of 1 531 healthy children under 7 years of age were sampled from 11 provinces, autonomous regions or municipalities in China by the cluster random sampling method from November 2020 to November 2021. The demographic information, family conditions, behavior and living habits and feeding behaviors were collected using unified questionnaire. Serum 25-hydroxyvitamin D(25(OH)D) levels were measured by liquid chromatography-tandem mass spectrometry. Serum 25(OH)D<30 nmol/L was considered deficient and 30-50 nmol/L was considered insufficient. With 25(OH)D≤50 nmol/L as the dependent variable, multivariate Logistic regression was applied to analyze the association between vitamin D deficiency and insufficiency and potential influential factors. Results: The prevalence of vitamin D deficiency and insufficiency among children under 7 years of age in 11 provinces, autonomous regions or municipalities of China was 14.0% (215/1 531), 3.8% (25/664) and 21.9% (190/867) in 0-<3 and 3-<7 of age years, respectively. Compared to children aged 0-<3 years, children aged 3-<7 years had a 2.6-fold increased risk of vitamin D deficiency and insufficiency (OR=3.60, 95%CI 1.93-6.72, P<0.001). Frequent sunlight exposure (OR=0.46, 95%CI 0.29-0.73, P=0.001), vitamin D supplementation (sometimes, OR=0.33, 95%CI 0.21-0.51, P<0.001; daily, OR=0.20, 95%CI 0.11-0.36, P<0.001) and infant formula intake(4-7 times per weeks, OR=0.43, 95%CI 0.28-0.68, P<0.001) were protective factors for vitamin D deficiency and insufficiency. Conclusion: Vitamin D deficiency and insufficiency are common among children under 7 years of age in 11 provinces, autonomous regions or municipalities of China, which is affected by age, sunlight exposure, vitamin D supplementation and infant formula intake.

This study aims to investigate factors associated with serum 25-hydroxyvitamin D [25(OH)D] concentration in Brazilian adults considering sociodemographic and lifestyle factors, as well as vitamin D-related single nucleotide polymorphisms (SNPs). This is a cross-sectional study (n = 491; 34-79y; 251 women), nested within a prospective cohort (Pró-Saúde Study). Associations between serum 25(OH)D and sociodemographic characteristics, diet, use of supplement, physical activity, season of blood collection, body fat, skin type, sun exposure index, and SNPs CYP2R1-rs10741657 and GC-rs2282679 were explored by multiple linear regression. The prevalence of serum 25(OH)D < 50nmol/L was 55%. Serum 25(OH)D was lower among women (β = -4.38; 95%CI: -8.02; -0.74), those with higher visceral fat (β = -4.02; 95%CI: -5.92; -2.12), and those with AC and CC genotypes for GC-rs2282679 (β = -6.84; 95%CI: -10.09; -3.59; β = -10.63; 95%CI: -17.52; -3.74, respectively). Factors directly associated with serum 25(OH)D included summer (β = 20.14; 95%CI: 14.38; 25.90), intermediate skin type (β = 6.16; 95%CI: 2.52; 9.80), higher sun exposure (β = 0.49; 95%CI: 0.22; 0.75), vitamin D intake (β = 0.48; 95%CI: 0.03; 0.93), and physical activity (β = 4.65; 95%CI: 1.54; 7.76). Besides physical activity, diet, and sun exposure, non-modifiable factors, such as GC genotypes must be considered when evaluating vitamin D insufficiency in mixed-race populations. Moreover, high visceral fat in association with poorer vitamin D status deserve attention given that both conditions are unfavorably related with chronic and acute health outcomes.
Este estudo teve como objetivo investigar fatores associados com as concentrações séricas de 25-hidroxivitamina [25(OH)D] em adultos brasileiros de acordo com fatores sociodemográficos e de estilo de vida, assim como de polimorfismos de nucleotídeo único (SNPs) relacionados à vitamina D. Este é um estudo transversal (n = 491; 34-79 anos; 251 mulheres) aninhado em uma coorte prospectiva (Estudo Pró-Saúde). Associações entre a 25(OH)D sérica e características sociodemográficas, consumo alimentar, uso de suplementos, atividade física, estação do ano na coleta da amostra de sangue, gordura corporal, fototipo de pele, índice de exposição solar e SNPs CYP2R1-rs10741657 e GC-rs2282679, explorados por regressão multilinear. A prevalência de 25(OH)D sérica < 50nmol/L foi 55%. A concentração sérica de 25(OH)D foi menor entre mulheres (β = -4,38; IC95%: -8,02; -0,74), indivíduos com mais gordura visceral (β = -4,02; IC95%: -5,92; -2,12) e genótipos AC e CC para GC-rs2282679 (β = -6,84; IC95%: -10,09; -3,59 e β = -10,63; IC95%: -17,52; -3,74, respectivamente). Os fatores associados diretamente à 25(OH)D sérica incluíram os meses de verão (β = 20,14; IC95%: 14,38; 25,90), fototipo intermediário (β = 6,16; IC95%: 2,52; 9,80), maior exposição solar (β = 0,49; IC95%: 0,22; 0,75), ingestão de vitamina D (β = 0,48; IC95%: 0,03; 0,93) e atividade física (β = 4,65; IC95%: 1,54; 7,76). Além de atividade física, dieta e exposição solar, fatores não modificáveis, tais como variantes do gene GC devem ser considerados na avaliação da deficiência de vitamina D em populações miscigenadas. Além disso, merece atenção a associação entre a gordura visceral elevada e o pior estado de vitamina D, uma vez que ambas as condições implicam em desfechos de saúde desfavoráveis, tanto crônicos quanto agudos.

Se investigaron las asociaciones entre concentración sérica 25(OH)D y características sociodemográficas, ingesta alimentaria, uso de suplementos, actividad física, estación del año de recogida de muestras de sangre, grasa corporal, tipo de piel, índice de exposición al sol, y SNPs CYP2R1-rs10741657 y GC-rs2282679 mediante una regresión múltiple lineal. La prevalencia sérica 25(OH)D < 50nmol/L fue 55%. La 25(OH)D sérica fue menor entre las mujeres (β = -4,38; IC95%: -8,02; -0,74), quienes tenían alta grasa visceral (β = -4,02; IC95%: -5,92; -2,12), genotipos AC y CC para GC-rs2282679 (β = -6,84; IC95%: -10,09; -3,59 y β = -10,63; IC95%: -17,52; -3,74, respectivamente). Los factores directamente asociados con la concentración sérica 25(OH)D incluyeron verano (β = 20,14; IC95%: 14,38; 25,90), tipo de piel intermedia (β = 6,16; IC95%: 2,52; 9,80), más alta exposición al sol (β = 0,49; IC95%: 0,22; 0,75), toma de vitamina D (β = 0,48; IC95%: 0,03; 0,93) y actividad física (β = 4,65; IC95%: 1,54; 7,76). Además de la actividad física, dieta y exposición al sol, los factores no modificables, tales como genotipos GC, necesitan tenerse en cuenta cuando se está evaluando la insuficiencia de vitamina D en poblaciones mestizas. Asimismo, las implicaciones de la asociación de una alta grasa visceral con un estatus más pobre de vitamina D merece que se le preste atención, puesto que ambas condiciones de salud están relacionadas desfavorablemente con resultados de salud graves y crónicos.

Resumo O objetivo deste artigo é analisar a insuficiência de vitamina D e fatores associados em idosos assistidos na atenção primária à saúde. Trata-se de um estudo transversal que avaliou 533 idosos (≥ 60 anos) em três cidades do estado de São Paulo, Brasil. Foi avaliada a 25-hidroxivitamina D (25 OH D) sérica por quimioluminescência. Os fatores avaliados foram condições sociodemográficas (sexo, faixa etária, etnia, escolaridade, renda, estado civil), de saúde (doenças referidas), composição corporal (IMC, circunferência da cintura), estilo de vida (atividade física e tabagismo) e exposição solar (finalidade, duração, frequência e horário de exposição, partes expostas, uso de protetor solar, tipo de pele). A prevalência de insuficiência foi de 64,5%, com associação para sexo feminino, etnia não brancos/não declarados, baixo peso, circunferência da cintura elevada (risco para DCV) e inatividade física. Houve associação negativa para exposição solar habitual de mãos, braços e pernas, durante atividade de lazer, deslocamentos diários e atividade física e entre as 9h e 15h. Os achados mostram a relevância de fatores como sexo, etnia, composição corporal, atividade física e hábitos de exposição solar na alta prevalência de níveis inadequados de vitamina D em idosos.
Abstract This article aims to analyze vitamin D insufficiency and factors associated among older adults using primary health care services. This is a cross-sectional study that evaluated 533 older adults individuals (≥ 60 years old) in three cities in the state of São Paulo, Brazil. Serum level of 25-hydroxyvitamin D (25-OHD) was evaluated by chemiluminescence. The factors evaluated were sociodemographic information (sex, age group, ethnicity, education, income, marital status), health conditions (reported diseases), body composition (BMI, waist circumference), lifestyle (physical activity and smoking), and sun exposure (purpose, duration, frequency, time of exposure, exposed body parts, use of sunscreen, skin type). The prevalence of vitamin D insufficiency was 64.5%, presenting association with female participants, non-white/unreported ethnicity, low weight, high waist circumference (risk for CVD - cardiovascular disease), and physical inactivity. Negative association was observed with habitual sun exposure of hands, arms and legs, during leisure activities, daily commuting and physical activity, and between 9 am and 3 pm. The findings show the relevance of factors such as sex, ethnicity, body composition, physical activity, and sun exposure habits in the high prevalence of inadequate levels of vitamin D among older adults.

Abstract Low bone health is associated with vitamin D deficiency in older individuals; however, this association is not well established in adults. The aim of the study was to analyze the association between serum concentrations of 25-hydroxyvitamin D and bone health in adults by systematic review and meta-analysis. The search was carried out in the LILACS, PubMed, Scopus, Web of Science, ScienceDirect databases from March 2017 to October 2018 with adult individuals (20-59 years). Bone health was evaluation performed through dual X-ray absorptiometry and serum concentrations of 25(OH)D. The random effect model was used to analyze data from bone mineral content and bone mineral. Random effects models were used and the sources of heterogeneity were explored by means of meta-regression. Thirty-five articles were selected. There was positive correlation between vitamin D and bone health in most of the evaluated sites. Correlation was observed in the analysis of subgroups for lumbar spine among men. When stratified, the studies presented high heterogeneity, which was explained by the sample size, mean serum vitamin D levels and risk of bias. Vitamin D is positively correlated to bone health in adult individuals.
Resumo A baixa saúde óssea está associada à deficiência de vitamina D em indivíduos mais velhos; no entanto, isso não está bem estabelecido em adultos. O estudo objetivou-se analisar a associação entre concentrações séricas de 25-hidroxivitamina D e baixa saúde óssea em adultos por revisão sistemática e metanálise. A pesquisa foi realizada nas bases LILACS, PubMed, Scopus, Web of Science, ScienceDirect de março de 2017 a outubro de 2018 com indivíduos adultos (20-59 anos). A avaliação da saúde óssea foi realizada através da absorciometria dupla de raios X e concentrações séricas de 25(OH)D. O modelo de efeito aleatório foi utilizado para analisar dados do conteúdo mineral ósseo e densidade mineral óssea. Modelos de efeitos aleatórios foram utilizados e a heterogeneidade foi explorada por meio de meta-regressão. Trinta e cinco artigos foram selecionados. Houve correlação positiva entre a vitamina D e a saúde óssea na maioria dos locais avaliados. Observou-se correlação na análise de subgrupos da coluna lombar entre homens. Quando estratificados, os estudos apresentaram alta heterogeneidade, explicada pelo tamanho da amostra, pelos níveis séricos médios da vitamina e pelo risco de viés. A vitamina D está positivamente correlacionada com a saúde óssea em indivíduos adultos.

Abstract Objective: To determine the prevalence of vitamin D deficiency in children in the South- Central Region of Costa Rica. Methods: A cross-sectional study was performed with Costa Rican children aged 1-7 years from the South-Central Region of the country belonging to the Nutrition and Education Centers and Children's Nutrition and Comprehensive Care Centers. Serum samples were collected from all participants between August 2014 and May 2016 and stored at -80 ° C. Vitamin D status was assessed by immunoassay on an ARCHITECT Plus i1000 instrument. Descriptive statistics were performed with the SPSS statistical software package (V20, IBM Corp). A value of p˂0.05 was considered significant. Spearman and Pearson correlation were also performed to study the association between vitamin D status, anthropometric and hematological variables. Results: Atotal of 428 sampleswere analyzed.According to the cut-off points establishedby the Endocrine Society, 4.9% of the children tested presented deficiency, 50.2% had insufficiency and 44.9% had vitamin D sufficiency. The mean concentration of 25-hydroxyvitamin D in the studied population was 29.7 ng/mL (SD 6.5) in boys and 29.8 ng/mL (SD 7.0) in girls. A high prevalence of hypovitaminosis D (55.1 %) was found, but only 7.9 % of the children presented 25-hydroxyvitamin D ≤ 20 ng/mL. No correlation was found between vitamin D status and any of the evaluated anthropometric or hematological variables. Conclusions: More than half of the young population presented hypovitaminosis-D. Therefore, in order to overcome this situation, the recommendation is to supplement the population with vitamin D and improve its fortification in widespread accessible food in Costa Rica.
Resumen Objetivo: Determinar la prevalencia de la deficiencia de vitamina D en niños de la Región Central Sur de Costa Rica. Métodos: Se realizó un estudio transversal con niños de 1-7 años de la Región Central Sur de Costa Rica pertenecientes a los Centros de Educación y Nutrición y Centros de Atención Integral y Nutrición Infantil. Se recolectaron muestras de suero de todos los participantes entre agosto de 2014 y mayo de 2016 y se almacenaron a -80 ° C hasta su uso. Se evaluó las concentraciones de vitamina D mediante un inmunoensayo con el instrumento ARCHITECT Plus i1000. Los análisis estadísticos descriptivos se realizaron con el paquete de software estadístico SPSS (V20, IBM Corp). Se consideró significativo un valor de p <0,05. También se realizó la correlación de Spearman y Pearson para estudiar la asociación entre el estatus de vitamina D con variables antropométricas y hematológicas. Resultados: Se analizaron un total de 428 muestras. Un 4,9% de los niños presentaban deficiencia, 50,2% insuficiencia y el 44,9% suficiencia de vitamina D, según los puntos de corte establecidos por Endocrine Society. La concentración media de 25-hidroxi vitamina D en la población estudiada fue de 29,7 ng/mL (DE 6,5) en niños y de 29,8 ng/ mL (DE 7,0) en niñas. Se encontró una alta prevalencia de hipovitaminosis D (55,1%), pero solo el 7,9% de los niños presentó 25- hidroxi vitamina D ≤ 20 ng/mL. No se encontró correlación del estatus de vitamina D con ninguna de las variables antropométricas o hematológicas evaluadas. Conclusiones: Más de lamitad de la población infantil evaluada presentó hipovitaminosis D. Por lo tanto, se recomienda suplementar y mejorar la fortificación con vitamina D en alimentos ampliamente accesibles en la población costarricense.

ABSTRACT In recent years the immunomodulatory actions of vitamin D, a steroid hormone, have been extensively studied. In 2020, due to the COVID-19 pandemic, the question arose as to 25(OH)D status would be related to susceptibility to SARS-CoV-2 infection, since several studies pointed out a higher prevalence and severity of the disease in populations with low levels of 25(OH)D. Thus, we investigated the 25(OH)D levels in adults "Detected" positive for SARS CoV-2 by RT-PCR (reverse transcriptase polymerase chain reaction) test, and in negative controls, "not Detected", using the Fleury Group's examination database, in Sao Paulo, Brazil. Of a total of 14.692 people with recent assessments of 25(OH)D and RT-PCR tests for COVID-19, 2.345 were positive and 11.585 were negative for the infection. The groups did not differ in the percentage of men and women, or in the age distribution. There were no differences in the distribution of 25(OH)D between the two groups (p = 0.08); mean 25(OH)D of 28.8 ± 21.4 ng/mL and 29.6 ± 18.1 ng/mL, respectively. In the specific population studied, clinical, environmental, socioeconomic and cultural factors should have greater relevance than 25(OH)D in determining the susceptibility to COVID-19.

Abstract Objective To evaluate the prevalence of vitamin D deficiency in obese children and adolescents when compared to eutrophic controls. Methods Systematic review with meta-analysis covering studies with patients aged 0-18 years old diagnosed with obesity and vitamin D deficiency and control group of eutrophic patients. The studies were retrieved in the PubMed, Embase, and LILACS databases in December 2019. The search used the terms "obesity" in combination with "pediatric population" and "vitamin D". Results Through the search 3155 articles were retrieved; and after analysis, 20 studies were selected according to the study objectives. A total of 24,600 children and adolescents were included. Through meta-analysis, the relative risk for the association between obesity and vitamin D deficiency in the pediatric population was 1.41 (95% CI: 1.26-1.59) (I² = 89%, p < 0.01). Conclusion Children and adolescents with obesity have higher risk of vitamin D deficiency.

La vitamina D es esencial para el funcionamiento del organismo. Su deficiencia puede estar asociada tanto a patologías óseas, como a otras afecciones sistémicas. La prevalencia de la hipovitaminosis está aumentando, en todas las edades, incluidos niños y adolescentes. El objetivo del presente estudio fue evaluar los niveles sanguíneos de vitamina D en niñas colombianas, analizando también la dieta, la actividad física y la exposición solar. Se realizó un estudio descriptivo transversal con 52 niñas sanas prepúberes de Pasto (Colombia), entre 7 y 10 años. Se analizaron los niveles de albúmina, calcio, fósforo, magnesio, calcitriol (1,25 dihidroxicolecalciferol) y calcidiol (25 hidroxicolecalciferol). Se realizaron encuestas sobre hábitos alimentarios, actividad física y exposición solar. El 51,1% de las niñas evaluadas presentó insuficiencia de 25-OH-VITD y el 40% presentó deficiencia (< 20 ng/mL). Ninguna niña se encontraba en desnutrición u obesidad, el 10% se encontraba en riesgo de bajo peso (IMC ≤ -1DE y > -2DE), el 4 % presentaba sobrepeso (≥ +1DE y < +2DE), y el 34 % se encontraban en riesgo de talla baja (T/E: -1 y -2 DE). La ingesta media de calorías/día fue inferior a las recomendadas. Se observó una ingesta deficiente de vitamina D, calcio y magnesio (p > 0,05), así como de fibra (p > 0,05). Se pone de manifiesto una deficiencia de vitamina D en las niñas evaluadas a pesar de tener una actividad física y una exposición solar adecuadas. Además, se observan ingestas deficientes de fibra, calcio, magnesio y vitamina D. Habría por tanto que asegurar la ingesta e incluso suplementar para evitar problemas de salud en la edad adulta(AU)
Vitamin D is essential for the body to function. Its deficiency can be associated with bone pathologies as well as other systemic conditions. The prevalence of hypovitaminosisis increasing, in all ages, including children and adolescents. The objective of this study was to evaluate blood levels of vitamin D in Colombian girls, also analyzing diet, physical activity and sun exposure. A descriptive cross-sectional study was carried out with 52 healthy prepubertal girls from Pasto (Colombia), between 7 and 10 yearsold. The levels of albumin, calcium, phosphorus, magnesium, calcitriol (1.25 dihydroxycholecalciferol) and calcidiol (25 hydroxycholecalciferol) were analyzed. Surveys were conducted one a ting habits, physicalactivity and sun exposure. 51.1% of the girls evaluated presented 25-OH-VITD insufficiency and 40% presented deficiency (<20 ng / mL). None of the girls were under nourished or obese, 10% were at risk of low weight (BMI ≤ -1SD and> -2SD), 4% were overweight (≥ + 1DE and <+ 2DE), and 34% were they were at risk of short stature (T / E: -1 and -2 SD). The average calorie intake / day was lower than recommended. A deficient intake of vitamin D, calcium and magnesium (p> 0.05), as well as fiber (p> 0.05) was observed. A vitamin D deficiency is evident in the girls evaluated despite adequate physical activity and sun exposure. In addition, deficient intakes of fiber, calcium, magnesium and vitamin D are observed. Therefore, it would be necessary to ensure the intake and even supplement to avoid health problems in adulthood(AU)

OBJECTIVES@#To investigate vitamin D nutritional status in children after outbreak of coronavirus disease 2019 (COVID-19), as well as the effect of strict epidemic prevention and control measures for the COVID-19 epidemic on vitamin D nutritional status in children.@*METHODS@#A total of 7 460 children who underwent routine physical examinations from February to August, 2020 and had normal results were retrospectively enrolled as the observation group, and 10 102 children who underwent routine physical examinations from February to August, 2019 (no epidemic of COVID-19) and had normal results were enrolled as the control group. The serum level of 25-hydroxy vitamin D [25(OH)D] was compared between the two groups. The children in the observation and control groups who underwent physical examinations in March and April were selected as the epidemic prevention subgroup (@*RESULTS@#The observation group had a lower serum level of 25(OH)D than the control group in March and April (@*CONCLUSIONS@#Strict prevention and control measures for the COVID-19 epidemic may lead to a significant reduction in vitamin D level in children, especially school-aged and adolescent children. It is recommended to timely monitor vitamin D level in children, take vitamin D supplements, and increase the time of outdoor sunshine as far as possible under the premise of adherence to epidemic prevention regulations.

La tasa de hospitalización por crisis de asma en niños chilenos se ha duplicado en los últimos años. Diversos estudios muestran una asociación entre hipovitaminosis D y crisis de asma. El objetivo principal del presente estudio fue describir la prevalencia de déficit e insuficiencia de vitamina D (vitD) en escolares asmáticos hospitalizados por crisis de asma y evaluar su estado nutricional. METODOLOGÍA: Estudio analítico, transversal, que incluyó a todos los pacientes de 5 años o más que se hospitalizaron por crisis de asma durante un año calendario en el Servicio de Pediatría del Hospital Clínico de la Universidad de Chile. Se aplicó una encuesta que incluyó datos sociodemográficos y clínicos, se evaluó el nivel de control del asma mediante el cuestionario Asthma Control Test (ACT), se determinó el estado nutricional y se midió la concentración de vitD. RESULTADOS: Se observó que 6 de cada 10 pacientes presentaba déficit o insuficiencia de vitD (22,8 ng/ml ± 10,5), encontrándose una asociación positiva entre concentración de vitD y ACT. La malnutrición por exceso estaba presente en casi la mitad de los pacientes. Aquellos pacientes que se hospitalizaron por más de 3 días tenían una concentración sérica de vitD significativamente menor. CONCLUSIONES: La hipovitaminosis D afectó a la mayoría de los niños hospitalizados por crisis de asma. Se encontró una asociación significativa entre concentración de vitD con días de hospitalización y control de la enfermedad medido por ACT.
Asthma hospitalization rates in Chilean children has doubled in recent years. Multiple studies show an association between hypovitaminosis D and asthma attacks. OBJECTIVES: To determine the prevalence of vitamin D (vitD) deficiency and insufficiency in asthmatic school age children hospitalized for asthma crisis and to evaluate their nutritional status. METHODS: Cross-sectional, analytical study that included patients 5 years of age or older hospitalized for asthma attacks during one calendar year, in the Pediatric Service of the University of Chile Clinical Hospital. A survey was conducted that included sociodemographic and clinical data, the level of asthma control was evaluated using the Asthma Control Test questionnaire (ACT), the nutritional status was determined and the concentration of vitD was measured. RESULTS: 6 out of 10 patients had either Vit D deficiency or insufficiency (22,8 ng/ml ± 10,5), showing a positive association between vitD concentration and ACT. Malnutrition due to excess was present in almost half of the patients. Children hospitalized for more than 3 days had a significantly lower vitD serum concentration. CONCLUSIONS: Most of the patients had hypovitaminosis D. An association was found between vitD concentration and hospital stay and between vitD concentration and asthma control, measured by ACT .

ABSTRACT Hypovitaminosis D is now considered a pandemic, especially among more vulnerable populations and in HIV-infected subjects, with 80% presenting levels below 30 ng/mL. As there is no consensus on the more adequate dosage needed to correct such deficiency, the objective of this study was to evaluate 25 (OH) vitamin D supplementation in HIV-1 patients deficient of vitamin D. A total of 73 HIV-1-infected patients were included, drawn from a cohort of 435 patients; 37 patients were randomized to the active group, supplemented once a week with 50,000 UI vitamin D by mouth (group 1) and 36 to the placebo group (group 2). The study period ranged from June 2016 to September 2017. Variables involved in vitamin D metabolism and risk factors associated with hypovitaminosis were evaluated. The mean age was 45 years and 31.5 % were women. Vitamin D supplementation was effective in normalizing serum levels after six months in group 1 (mean 35 ng/mL compared to 21 ng/mL for the placebo group; p= 0.04). No patient reached blood levels considered toxic (>100 UI). Efavirenz use can negatively influence vitamin D levels and supplementation is necessary as a likely adjunct to improving CD4+ T cells, resulting in greater effectiveness of the treatment. A weekly oral dose of 50,000 IU of vitamin D was sufficient to normalize the vitamin deficiency, safely and with good adherence among persons living with HIV/AIDS in Brazil.

OBJECTIVES: To evaluate the mean concentration of 25-hydroxivitamin D [25(OH) D] and prevalence of hypovitaminosis D in individuals residing in Rio de Janeiro, Brazil. METHODS: The data of 80,000 consecutive individuals who had 25(OH) D measurements performed by electrochemiluminescence between 1/2/2018 and 2/5/2018 were selected. Patients who reported the use of therapies/supplements were excluded. Levels of 25(OH) D ≥20 ng/mL (ages <60 years) and ≥30 ng/mL (ages ≥60 years) were considered adequate. RESULTS: We analyzed the data of 24,074 individuals (1-95 years old, 64.7% female). Descriptive curves showed that, in both sexes, the mean values of 25(OH) D decreased from the first years of life until adolescence, then slightly increased, and then tended to stabilize during adulthood. Levels of 25(OH) D <20 ng/mL were observed in 6% of girls versus 3.6% of boys and in 13.6% of adolescent girls versus 12.6% of adolescent boys and 11% of adults. The percentage of seniors with serum levels of 25(OH) D <20 ng/mL was 13.6% in women and 12.7% in men; 53.2% of women and 50.6% of men had levels <30 ng/mL. CONCLUSIONS: Mean 25(OH) D values were higher in children and lower in adolescents and women. Approximately 90% of non-seniors and presumably healthy residents of the urban metropolitan region of Rio de Janeiro presented satisfactory levels of 25(OH) D during the summer months; however, in over half of the elderly, the serum concentrations of 25(OH) D were inadequate. Therefore, strategies for the prevention of hypovitaminosis D should be considered in the senior population.

Resumen Introducción: La relación entre 25-OH-vitamina D y el sistema inmune en pacientes con enfermedad renal crónica es objeto de atención. Objetivos: Evaluar la prevalencia de la deficiencia de vitamina D en pacientes en hemodiálisis e investigar la asociación entre la vitamina D y proteína C reactiva ultrasensible (PCRus), índice neutrófilo/linfocito (INL) e índice plaqueta/linfocito (IPL). Método: Estudio transversal de 80 pacientes en hemodiálisis, divididos en dos grupos: un nivel sérico de 25-OH-vitamina D < 20 ng/mL se consideró como deficiencia de vitamina D y ≥ 20 ng/mL, como normal. Con el análisis de correlación de Spearman se definió la relación entre los parámetros. Resultados: 40 % de los pacientes presentó deficiencia de vitamina D. Hubo diferencias significativas entre los grupos en PCRus (p = 0.047), INL (p = 0.039), IPL (p = 0.042) y tratamiento con análogos de vitamina D (p = 0.022). La vitamina D tuvo una correlación negativa significativa con PCRus (p = 0.026), INL (p = 0.013) e IPL (p = 0.022). Conclusiones: La deficiencia de vitamina D fue de 40 %. Los niveles de PCRus, INL e IPL fueron significativamente más altos ante deficiencia de vitamina D. Se encontró correlación inversa significativa entre vitamina D y PCRus, INL e IPL.
Abstract Introduction: The relationship between 25-OH-vitamin D and the immune system in patients with chronic kidney disease is a subject of attention. Objectives: To assess the prevalence of vitamin D deficiency in patients on hemodialysis and to investigate the association between vitamin D, ultra-sensitive C-reactive protein (US-CRP), neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR). Method: Cross-sectional study of 80 patients on hemodialysis, divided into two groups: a serum 25-OH-vitamin D level < 20 ng/mL was considered to be vitamin D deficiency and a serum level ≥ 20 ng/mL was regarded as normal. The relationship between the parameters was defined with Spearmans correlation analysis. Results: 40 % of the patients had vitamin D deficiency. There were significant differences between groups in US-CRP (p = 0.047), NLR (p = 0.039), PLR (p = 0.042) and treatment with vitamin D analogues (p = 0.022). Vitamin D had a significant negative correlation with US-CRP (p = 0.026), NLR (p = 0.013) and PLR (p = 0.022). Conclusions: The prevalence of vitamin D deficiency was 40 %. The values of US-CRP, NLR and PLR were significantly higher in the presence of vitamin D deficiency. A significant inverse correlation was found between vitamin D levels and US-CRP, NLR and PLR.

Abstract Objectives: To measure the prevalence of vitamin D deficiency (through the 25-hydroxyvitamin D metabolite) in pediatric patients using antiepileptic drugs. Source of data: Meta-analysis of studies identified through search in the PubMed, Embase, LILACS, and Cochrane Library databases, on February 19, 2019. Summary of data: A total of 748 articles were identified, 29 of which were relevant to the objectives of this study. The prevalence of vitamin D deficiency found was 0.32 (95% CI = 0.25-0.41; I 2 = 92%, p < 0.01). In the subgroup analyses, the most significant results were observed in the group of patients using cytochrome P450-inducing antiepileptic drugs, with a prevalence of 0.33 (95% CI = 0.21-0.47; I 2 = 86%, p < 0.01) and, considering the study design, in the subgroup of cohort studies, with a prevalence of 0.52 (95% CI = 0.40-0.64; I 2 = 76%, p < 0.01). Conclusions: Taking into account the deleterious effects of vitamin D deficiency on the bone health of individuals using antiepileptic drugs, it is suggested to include in their care 25-hydroxyvitamin D monitoring, cholecalciferol supplementation, and treatment of the deficiency, when present.

Fonte dos dados: Metanálise de estudos identificados por meio de pesquisa nas bases de dados Pubmed, Embase, LILACS e Cochrane em 19 de fevereiro de 2019. Síntese dos dados: Foram identificados 748 artigos, dos quais 29 mostraram-se relevantes aos objetivos deste estudo. A prevalência de deficiência de vitamina D encontrada foi de 0,32 (IC 95% = 0,25-0,41) (I2 = 92%, p < 0,01). Nas análises por subgrupos, os resultados mais expressivos foram observados no grupo de pacientes em uso de fármacos antiepilépticos indutores do citocromo P450, que apresentou prevalência de 0,33 (IC 95% = 0,21-0,47) (I2 = 86%, p < 0,01). Considerou-se o delineamento dos estudos, no subgrupo de estudos de coorte, com prevalência de 0,52 (IC 95% = 0,40-0,64) (I2 = 76%, p < 0,01). Conclusões: Levando-se em consideração os efeitos deletérios da deficiência de vitamina D na saúde óssea dos sujeitos em uso de fármacos antiepilépticos, sugere-se incluir em seu atendimento, o monitoramento de 25-hidroxivitamina D, suplementação com colecalciferol e tratamento de deficiência quando existente.

ABSTRACT Objective: To determine the prevalence of vitamin D deficiency (VDD) in exclusively breastfed infants at the Aga Khan University Hospital Nairobi, Kenya (AKUHN). The relationships between 25-hydroxyvitamin D; 25OHD, parathyroid hormone (PTH), maternal vitamin D supplementation, and sunlight exposure were also determined. Subjects and methods: Blood from 98 infants was assayed for 25OHD, calcium, phosphate, and PTH. Socio-demographic and clinical characteristics were analyzed using descriptive statistics and inferential analysis (p < 0.05). Results: The prevalence of VDD (25OHD <12 ng/mL), vitamin D insufficiency (VDI, 25OHD 12-20 ng/mL) and vitamin D sufficiency (VDS, 25OHD >20 ng/mL) was 11.2% (95% CI 8.0%-14.4%), 12.2% (95% CI 8.9%-15.5%), and 76.5% (95% CI 72.3%-80.8%) respectively. There was no difference in the mean age, head circumference, length, or weight of infants in VDD, VDI, and VDS groups. PTH was elevated when 25OHD was <12 ng/mL and normal when 25OHD was between 12-20 ng/mL. 25OHD and PTH were normal in infants whose mothers received vitamin D supplements. Infants who received <30 minutes/day of exposure to sunlight were 5 times more likely to have VDI than infants who received ≥30 minutes/day (p = 0.042). Conclusions: The prevalence of VDD in exclusively breastfed infants at AKUHN is low. The current national policy that recommends exclusive breastfeeding of infants in the first 6 months of life appears to be effective in staving off vitamin D deficiency but those infants with < 30 minutes sunlight exposure may benefit from low dose supplemental vitamin D during times of low sunlight exposure.

ABSTRACT The effects of vitamin D on the musculoskeletal system are well established. Its deficiency causes osteomalacia, secondary hyperparathyroidism, and an increased risk for fractures and falls. However, clinical and experimental evidence points to extra-skeletal actions of vitamin D, including on immune and respiratory systems. Thus, during this COVID-19 pandemic, a possible deleterious role of vitamin D deficiency has been questioned. This paper aims to present a brief review of the literature and discuss, based on evidence, the role of vitamin D in the lung function and in the prevention of respiratory infections. Relevant articles were searched in the databases MEDLINE/PubMed and SciELO/LILACS. The mechanisms of vitamin D action in the immune system response will be discussed. Clinical data from systematic reviews and meta-analyses show benefits in the prevention of respiratory infections and improvement of pulmonary function when vitamin D-deficient patients are supplemented. At the time of writing this paper, no published data on vitamin D supplementation for patients with COVID-19 have been found. Vitamin D supplementation is recommended during this period of social isolation to avoid any deficiency, especially in the context of bone outcomes, aiming to achieve normal values of 25(OH)D. The prevention of respiratory infections and improvement of pulmonary function are additional benefits observed when vitamin D deficiency is treated. Thus far, any protective effect of vitamin D specifically against severe COVID-19 remains unclear. We also emphasize avoiding bolus or extremely high doses of vitamin D, which can increase the risk of intoxication without evidence of benefits.
