RESUMEN

Decision-Delivery interval when carrying out emergency caesarean section (EmCS) cannot be over-emphasized especially with regards to maternal and fetal outcome. There are variety of factors that may contribute to these intervals such as logistics, personal delay, delay in obtaining of informed consent, lack of blood, and availability of theatre space.Aim: To determine the decision-delivery interval and causes of delay in EmCS at the Rivers State University Teaching Hospital (RSUTH).Methods: It was prospective study conducted at the RSUTH between July 1, 2018 to January 31, 2019. Information was obtained using a self structured questionnaire and analyzed using version SPSS 25.Results: There were 481 patients admitted into labour ward for the period under review of which 71(14.8 %) had EmCS. The mean age was 31 years. The commonest indication for EmCS was Cephalopelvic disproportion (CPD) represented by 23 (32.4%) of the subjects. The average time for decision -delivery interval was 1 to 2 hours represented by 29 (40.8%). The shortest decision -delivery interval was less than 30 minutes 1(1.3%).The decision –delivery interval time greater than 5 hours were 9 (12.7%). The 3 commonest reasons for delay with respect to average decision –delivery intervals were personal delay 21 (20.8%), logistics 19 (18.8%) and lack of blood 13 (12.9%).Conclusion: The study revealed that the average decision -delivery interval was1 –2 hours represented by 40.5% of the subjects. This was relatively long when compared to developed countries of the world. The commonest reason for delay in carrying out EmCS was personal delay (20.8%). The commonest indication for EmCS was CPD (32.4%). Addressing the reasons for the decision –delivery interval will help improve our practice and reduce adverse effects to mother and baby

RESUMEN

Objective To study the effect of sitz bath with 1.6%concentrated brine solution and metronidazole on perianal infection in acute leukemia patients. Methods Seventy-six acute leukemia patients admitted from June 2010 to December 2014 with perianal infections were randomly divided into the control group and the observation group in equal number. The two groups were given routine nursing care and symptomatic supportive treatment . Apart from the basic nursing , the control group was treated with sitz bath with 10 min 1 ∶ 5000 potassium permanganate. The observation group was treated with sitz bath with 1.6% brine solution and 0.1%metronidazole two times every day and another one if defecation was done. The perianal infections of two groups were compared in 7 days. Result The total effectiveness in the observation group was significantly better than that of the control group (P < 0.05). Conclusions Sits bath with 1.6% brine solution and metronidazole two times a day is more effective than with 1 ∶ 5000 potassium permanganate. It is economic and convenient, safe and effective for the treatment of perianal infections and therefore worth popularizing in clinical application.

RESUMEN

Objective To systematically review the efficacy of hydromorphone hydrochloride injection for treatment of chronic pain. Methods Web of Science Proceedings and PubMed were searched for clinical trials involving the efficacy of hydromorphone for treatment of chronic pain, with no language or time limit.Evaluation indexes included visual analogue scale (VAS) score and the rate of pain control or relief. The studies were screened independently, and the data were extracted by two researchers.Meta-analysis was conducted using the Stata 10 software. Results Eleven studies involving 452 patients were included in our meta-analysis.VAS score was significantly decreased after treatment compared with that before treatment. For the patients with cancer pain, VAS score was significantly decreased after treatment with hydromorphone hydrochoride injection, and the rate of pain control or relief was increased when compared with the other opioid analgesics. Conclusion Hydromorphone hydrochloride injection can treat chronic pain, and it may provide better therapeutic effect than the other opioid analgesics for the patients with cancer pain.

RESUMEN

The aim of the study was to evaluate the clinical usefulness of N-terminal pro-brain natriuretic peptide [NT-Pro BNP] and nitric oxide [NO] with echocardiography [gold st and ard test] in diagnosis of canine cardiomyopathy. Out of 374 cases, 82 cases showing cardiovascular abnormalities including 16 cases of cardiomyopathy were further selected for st and ardization of biomarkers. Significant [P<0.01] increase in plasma NT-Pro BNP was found in dogs with cardiomyopathies. The plasma concentration of NO showed non-significant increase in comparison to healthy dogs. The sensitivity of NT-Pro BNP and NO was 72.20% and 62.50%, with specificity 93.90% and 89.40%, respectively when compared with echocardiography. The correlation between echocardiography and NT-Pro BNP was 89.30% with kappa value of 0.675 suggesting substantial agreement between the two tests. The positive and negative predictive value was estimated at 0.765 and 0.925, respectively. It is concluded that NT-Pro BNP and NO may be an alternate diagnostic tool for diagnosis of cardiomyopathy where the facility of echocardiography is not available. Key words: Cardiomyopathy, Echocardiography, N-terminal pro-brain natriuretic peptide, Nitric oxide

RESUMEN

OBJECTIVETo investigate the oral health-related quality of life (OHRQL) and related influencing factors in elderly patients.

METHODSA survey was conducted on a random probability sample of 250 elderly patients aged 55 years and over. The measures included the Chinese version of GOHAI, self-rated oral health, satisfaction with oral health and satisfaction with life. Socio-demographic and clinical information were collected. Multiple regression was used to analyze the related influencing factors.

RESULTSMean GOHAI scores of the sample was 12.26 +/- 7.67. Multiple regression equation was established. GOHAI scores = -6.477 + 0.309 x self-ratings of oral health +0.164 x number of missing teeth +0.237 x satisfaction with oral health +0.127 x satisfaction with life. The R square of the equation was 0.423.

CONCLUSIONSSelf-rated oral health, number of missing teeth, satisfaction with oral health and satisfaction with life are important variables influencing oral health-related quality of life in elderly patients.

RESUMEN

OBJECTIVETo establish a method for culturing dental follicle cells of rat and observe the growth characteristics of the cultured cells in vitro.

METHODSDental follicle and associated enamel organs were dissected from the first and second mandibular molars of 6-7-day-old rats, and then cultured in vitro. Purified dental follicle cells derived from the third or the fourth passage cells were utilized in the following experiments. The shape and ultrastructure of dental follicle cells were observed by light-microscopy and transmission electron microscopy. Immunocytochemistry was used to detect the expression of vimentin, type I collagen and fibronectin.

RESULTSThe cultured cells were fibroblastic in shape, elongate with processes, and transmission electron microscopy revealed that they contained electron-dense granules, an abundant rough endoplasmic reticulum, but did not form desmosomes. Vimentin, type I collagen and fibronectin were present in dental follicle cell.

CONCLUSIONThe dental follicle cells of rat could be successfully cultured in vitro and the cultured cells had the same characteristics of dental follicle cells of normal rat.
