RESUMEN

This was a pilot project which conducted at a primary care Health Centre [IBn Sina] to identify and register all chronic subjects wirh non-communicable diseases,. The importance of this step which is usually and should be the first step in the staged approach to improve their health care. This step is often neglected. For, without this step, care can be delivered, but the number of lost cases is high. Surveying the baseline data of all these subjects, would be the second step in this staged approach in order to improve their health care. The use of well structured protocols and flow charts can help achieve the second step. The importance of the audit wast to obtain a baseline data of the chronic cases and identify the magnitude of the problem that could in the future plan to recommend NCDs clinic. They will, also, help decision makers at local and higher level better plan for human resources, management and medications. To register and label any patient of chronic non-communicable disease, and obtain a baseline data of the chronic cases seen in the Health Centres and to identify the magnitude of the chronic illness in the catchment area. Data was collected over a period of one year, January 2000 to January 2001. A print-out form which was made ready that was given to all doctors after explanation. All what was needed from the doctor after identifying his patients with NCD was to enter their name into form provided along with their CPR number and to tick the problem encountered. The rest of the jop was of the medical secretary to trace the family folder of the identified subjects to record their data, and by inspection prescriptions reaching the pharmacy. Data were entered into pre-designed data base programme using a PC.A chronic problem register was compoled. A total of 1220 patients were identified as having chronic problems of NCDs at Ibn Sina Health Centre, representing all the registered clients at the Health Centre [15,836] population, giving a prevalence rate of 7.7% of the registered patients at Ibn Sina Peevalence of the selected and registered problems were also calculated and identified. A large number of cases with chronic illnesses are seen by doctors in the Health centres. The health centres is overwhelmed by the number of patients seeking care, which demoralizes and frustrates health care givers and doesn't bring optimal care to patients. It is important to structure the service so that those whose condition is complicated and/or poorly controlled receive the required extra attention and are not 'lost' among the far greater number of people with uncomplicated and easily controlled NCDs. This project need to identify the problem of chronic illness in the health centre

The use of ketamine 1 mg/kg and droperidol 0.05 mg/kg has no significant haemodynamic effect in patients with end stage renal disease [ESRD]. The factors preventing Bahraini medical graduates from joining the Department of Anesthesiology were studied in 49 Bahraini residents at Salmaniya Medical Center and Bahrain Defence Force Hospital. It was shown that the working conditions and lack of respect and consideration for the anesthesiologist are the main factors responsible for not attracting graduates to take anesthesiology as a career
