RESUMEN

Objective:To investigate the status of caregivers' benefit finding of senile dementia patients in Shiyan city, Hubei Province, and to analyze its influencing factors, so as to provide basis for carrying out targeted intervention measures. Methods:Totally 252 caregivers of senile dementia patients who visited Taihe Hospital, Shiyan People′s Hospital and Sinopharm Dongfeng General Hospital in Shiyan City, Hubei Province from July 2019 to July 2021 were selected as the research objects by convenience sampling method. The general Information Questionnaire, the Caregiver benefit finding questionnaire, the General self-efficacy Scale (GSES) and the Perceived Social Support Scale (PSSS) were used. The Pearson correlation analysis method was used for the correlation between caregivers' self-efficacy, social support and perceived benefit. Catreg was used to analyze the influencing factors of caregivers' benefit finding in senile dementia patients. Results:The total score of caregiver benefit questionnaire for 252 caregivers of senile dementia patients was (104.74 ± 14.82), the item "made me realize the significance of my health to my family and society" received the highest score (4.70 ± 0.55), while the item "made me quit bad habits" received the lowest score (3.22 ± 1.17). The GSES and PSSS scores of 252 caregivers of senile dementia patients were (24.74 ± 7.02) and (54.21 ± 13.32), which were positively correlated with the benefit finding ( r=0.565, 0.459, both P<0.01). The catreg analysis showed that the age of the caregiver, the average monthly income of the family, self-efficacy and social support were the influential factors of the perceived benefit level of the caregiver of senile dementia patients ( F values were 3.30-104.92, all P<0.05), which could explain 42.4% of the variance. Conclusions:The benefit finding of caregivers of senile dementia patients still needs to be improved. Medical staff should pay attention to the evaluation of benefit finding of caregivers of senile dementia patients, and provide personalized intervention measures to relieve caregivers' care pressure, maintain their physical and mental health, and improve the care quality of caregivers of senile dementia patients.

RESUMEN

ObjectiveTo study the anatomical abnormalities of basal ganglia and research their influence on depression status in patients with post stroke depression (PSD)by diffusion tensor imaging (DTI) of MRI.MethodsPatients with basal ganglia infarction were recruited, and divided into groups of PSD and non depression control group by Hamilton Depression Rating Scale (HAMD) assessment. All the patients were evaluated with National Institute of Health Stroke Scale ( NIHSS). And the patients were checked by DTI sequence.Fractional anisotropy (FA),average diffusion coefficient (ADC) values and the number of nerve fiber were measured in bilateral caudatum, pallidum, putamen and thalamus.ResultsThe score of NIHSS (6.29 ± 3.45 ) was significantly higher in PSD group than that in non-depression group (3.95 ± 1.90 ;t =2.219,P =0.036). No significant difference was found between the two groups for the DTI data of the basal ganglia nuclei ( t =0.056-1.618,all P > 0.05 ). Compared with contralateral construction (0.40 ± 0.02 ),the FA value decreased in the left putamen ( 0.37 ± 0.03 ) in the PSD group ( t =2.243,P =0.045 ).By Spearman correlations analysis, the HAMD score was positively correlated with NIHSS score ( r =0.464,P =0.017 ),and negatively correlated with the FA values of left pallidum (r=-0.563,P=0.005),right pallidum (r=-0.416,P=0.035) and left putamen (r =-0.428,P =0.029).Conclusions The occurrence of PSD was associated with neurological functional deficit following basal ganglia infarction. The depression level was correlated with the increasing of NIHSS score, the reductions in bilateral pallidum and left putamen FA values. This research contributes to evaluation of the PSD status in patients with basal ganglia infarction.

RESUMEN

Objective To evaluate the efficiency of normal sacral nerve root transposition in repair of the sacral plexus root avulsion. Methods A total of 30 adult SD rats were chosen and divided into three groups,ie,group A(the sciatic nerve received no repair),group B(the autologous sacral plexus root nerve was bridged with the right L6 nerve root by the translocation of the left L6)and group C (the right L5 nerve root nerve was bridged by the translocation of the left L6),10 rats per group. The left side of the rats was used as the control side and the right one as the experimental side.Twelve weeks after operation, the rats in each group were selected for the histomorphological observation of the nerves under the microscope and the electron microscope. The models were evaluated by observing the survival rates of the rats,BBB scores, electron microscope weight and muscle fiber CSA(cross section area)of double biceps femoris, triceps surae and tibial muscle. Results Twelve weeks after operation, the BBB scores in groups B and C was higher than that in group A,with statistical difference(P<0.01)between three groups.A remarkable improvement was found in the ratio of weight and muscle fiber CSA of double biceps femoris, triceps surse and tibial muscle. The repair efficiency in the group C was better than that in the group B.In the group B,the biceps femoris, triceps surae and tibial muscle recovered at different degrees. The biceps femoris recovered the best, when a great deal of myelinated nerve fiber regeneration was observed under the microscope and the electromicroscope.Electromyography revealed the volatility in the muscles of three groups, with larger peak value for the proximal biceps femoris and the triceps muscle but smaller peak value for the distal anterior tibial muscle. Conclusions L6 transposition combined with auto-graft of nerve root or without the auto-graft can reconstruct the partial function of the sciatic nerve in the paraplegia rats, when the latter has the better effect.

RESUMEN

Objective To assess whether 5-HTR1A C( - 1019) G and GNβ3 C825T gene polymorphisms are associated with post-stroke depression (PSD) and explore the genetic mechanism of the pathogenesis of post-stroke depression. Methods All 159 patients with first stroke were divided into the PSD group and the control group according to HAMD scores. Their genotypes were determined with polymerase chain reaction and allele-specific restriction enzyme analysis. Results The frequency of 5-HTR1A (-1019) GG genotype(8/53,15. 1% ), G allele (44/106,41.5%)and GNβ3 825T allele(68/106,64. 2% ) were significantly higher in the post-stroke depression group than in the controls (5/106,4.7% ;35/212, 16. 5%; 113/212, 53.3%; ×2 = 23.204, 23. 655, 3. 392, all P < 0. 05 ). Combined genotype analysis showed that individuals with both 5-HTR1A ( - 1019) G and GNβ3 825T allele ( OR =4. 980,95% CI 2. 429-10. 210,P =0. 000) had a higher risk than those with 5-HTR1 A (-1019) G allele ( OR = 3. 589,95% CI 2. 113-6. 096, P = 0. 000) or GNβ3 825T allele ( OR = 0. 638,95% CI 0.395-1. 031 ,P =0. 042) only for post-stroke depression. Conclusion The 5-HTR1A C( - 1019)G and GNβ3 C825T polymorphisms are predisposing genes of post-stroke depression. Our data also suggest a significant interaction between the 5-HTR1A ( - 1019)G allele and GNβ3 825T allele in post-stroke depression.

RESUMEN

Objective To establish an animal model of sacral nerve root avulsion in rats and e-valuate its efficiency. Methods A total of 20 adult SD rats (either sex) were chosen at random to es-tablish the sacral nerve root avulsion model by avulsing the fight L4-6 nerve roots out of intervertebral fo-ramina without laminectomy. The left side was set as control group. The models were evaluated in aspects of survival rate, Basso-Beattie-Bresnahan (BBB) scores, somatosensory evoked potential (SEP), horse radish peroxidase (HRP) tracing, bilateral weight and cross section area (CSA) of muscle biceps femo-ris, fiber of triceps surae and anterior tibial muscle. Results Of all, 19 rats were survived but one died, with survival rate of 95.0%. The BBB score was (10.78+3.15) points in experimental group and 21 points in control group. The success rate of establishing animal model was 89.5% ,for there detected no SEP in bilateral cerebral cortex of the wounded extremity of 17 rats. HRP showed positive reaction in the spinal segment of L4-6 in two rats but negative in 17, with success rate of 89.5%. There was statistical difference in aspects of weight and muscle fiber CSA of double biceps femoris, triceps surae and anterior fibial muscle between experimental group and control group. Electron microscope found denervation chan-ges including muscle fiber atrophy, nucleus shifting to (center and muscle satellite cell. Conclusion A-vulsion of L4-6 sacral nerve root out of vertebral canal is a feasible and ideal method to establish the avui-sion model of sacral nerve root injury in rats.

RESUMEN

BACKGROUND: Absorbable materials for internal fixation have some advantages such as simple surgery, rapid recovery and no need to second surgery, and have been accepted in the circle of orthopaedics, and had been applied in the clinic. However, the system experiment evidences are not reliable.OBJECTIVE: To investigate whether the absorbable material (the absorbable tension band, absorbable stick or screw) is an acceptable alternative to stainless steel wire with Kirschner wire by comparing their biomechanical properties in the fixation of patella transverse fractures.DESIGN, TIME AND SETTING: The randomized controlled study of biomechanical analysis was performed at the Laboratory of Institute of Biomechanics, Shanghai University from March 2007 to January 2008.MATERIALS: Eight fresh patellae specimens were collected, with the complete patella, articular capsule and patellar ligament (3.0-4.0 cm) and quadriceps femoris (4.0-5.0 cm), and then divided into two groups at random, AO steel wire group (n = 2) and absorbable tension band group (n = 6).METHODS: Eight fresh patellae specimens were divided into two groups at random, AO steel wire group (n = 2) and absorbable tension band group (n = 6) to create models of transverse fractures, which were separately fixed by absorbable tension band, steel wire tension band + Kirschner wire to measure their biomechanics function. If fracture gap reached 1.0 mm, the fixation failed.MAIN OUTCOME MEASURES: The changes in patellar ligament tensile force, quadriceps femoris tensile force, patellar displacement, quadriceps femoris displacement, patellar ligament displacement and patella tensile strain were measured under external load.RESULTS: Under external load, no significant difference was determined in total displacement between the absorbable tension band group and AO steel wire group (9%-10% difference). No significant difference was detected in patellar strain between absorbable tension band group and AO steel wire group (9%-10% difference). The patellofemoral joint force was similar between the absorbable tension band group and AO steel wire group. When internal fixation displaced 1 mm, the load was 587 N in the absorbable tension band group, and 650 N in the AO steel wire group (P > 0.05).CONCLUSION: The absorbable material is an acceptable alternative to stainless steel wire with Kirschner wire in the fixation of patella transverse fractures, with the advantages of good biological function and good stability at the broken ends of fractured bone.

RESUMEN

AIM: To observe the curative effect of titanium wiring in treatment of the fractures of Neer Ⅱ type distal clavicle and dislocation of Tossy Ⅲ type acromioclavicular joint. METHODS: From March 2004 to December 2006, 37 cases with Neer Ⅱ type distal clavicle fractures or Tossy Ⅲ type acromioclavicular joint dislocation were treated with titanium wiring in Changzheng Hospital, Second Military Medical University of Chinese PLA. Titanium wiring (produced by Sofamor-Danek Company in USA) had good histocompatibility, intensity of tension and flexibility. The broken coracoclavicular ligament was repaired directly by titanium wiring in all cases. The outcomes were evaluated according to Herscovici's criteria. RESULTS: All the cases were followed up for 6 to 24 months. All the dislocated acromioclavicular joints returned to normal 2 weeks after operation. Excellent and good rate of shoulder function was 100% according to Herscovici standard. X-ray imaging showed all clavicle fractures united, acromioclavicular joint had no redislocation or titanium wiring breaking. The incisions of all cases had no obvious swelling and exudation. CONCLUSION: When Neer Ⅱ type distal clavicular fracture and Tossy Ⅲ type acromioclavicular joint dislocation are treated with titanium wiring, the acromioclavicular joint can exercise earlier and recover quickly.

RESUMEN

<p><b>OBJECTIVE</b>To summarize surgical treatments and their corresponding curative effects on sciatic nerve injuries.</p><p><b>METHODS</b>Surgical treatments on sciatic nerve injury were performed in 28 patients from January 1990 to July 2000. The treatments included neurolysis, neurolysis plus partial nerve anastomosis, nerve anastomosis and nerve transplantation. The curative effect was evaluated according to Sunderland criteria.</p><p><b>RESULTS</b>Of 28 cases, 22 patients were followed up with a follow-up period of 13 months to 5 years (average 30 months). Of 22 nerves, 7 were excellent, 5 good, 7 fair and 3 poor, with an excellence rate of 54.5%.</p><p><b>CONCLUSIONS</b>The fair results of sciatic nerve injury are related to its structural character. Surgical exploration should be performed if nerve function does not recover 3 months after primary operation and if Tinel's sign and electromyogram show no signs of nerve regeneration. Electrophysiological monitoring in the operation is useful in electing surgical methods and predicting the results of nerve anastomosis.</p>

RESUMEN

Objective To explore the clinical application of reconstruct ion interlocking nail in treating ipsilateral fractures of femoral neck and shaf t. Methods Twelve cases of ipsilateral fractures of femoral neck and shaft trea ted with reconstruction interlocking nail by static locking were reviewed. The d iaplasis of the fracture was done in a close manner or in an open manner but wit h a little incision, and the two sides of the fracture were locked with nails by a three-dimensional aim implement. Two lag screws were inserted into the femor al neck at a 15 degree dip angle. Results Eleven cases were followed up (average 12.3 months). Their long term effects were evaluated according to MaYuanzhang e valuation criteria: 5 cases gained excellent clinical results, 4 good, 2 fair; t he excellent and good rates were 81.8% (9/11). The average period of femoral n eck fracture healing was 5.2 months and that of shaft fracture healing 6.7 month s .72.7 % of the patients(8/11) showed fracture healing at one stage. There were no necrosis of femoral head, no femoral neck nonunion or malunion, or no n ail rupture. Conclusions Reconstruction interlocking nail is a principal way in treating ipsilateral fractures of femoral neck and shaft because of its effecti ve fixation and excellent clinical effect. Anatomy reduction of femoral neck and non-open reduction should be emphasized in treatment of femoral neck fractures , while close nail insertion, limited reaming and static locking should be a rou tine way in treatment of femoral shaft fractures.

RESUMEN

0.05).Both femur bone lengths in 3.2mm group were shorter than those in the 1.5 mm and 2.2 mm groups. No abnormalities of cartilage cells were observed, without obvious histological evidence of inflammation;only nonspecific foreign body reaction and macrophage infiltration were noticed. Conclusion: PDLLA rod itself does not cause any disturbance to the development of the epiphyseal plate. The growth disturbance of epiphyseal plate depends on the area of injured epiphyseal plate;a injured area over 7% of epiphyseal plate may cause the growth disturbance of epiphyseal plate.

RESUMEN

Objective To compare the results of treatment with biodegradable ligaments and metallic fixation for total acromioclavicular joint dislocation. Methods Twenty eight patients with total acromioclavicular joint dislocation were treated with two different materials, 1) by Kirschner pin and steel wire or screw and steel wire tension band (MF group,16 cases), and 2) reconstructed by artificial coracoclavicular ligament and coracoacromial ligament (BF group,12 cases). There were 17 males and 11 females. All patients had acute dislocation. The time from injury to hospital admission varied from 0-8 days (average, 1.5 days). Results All patients were availble for an average duration of follow up of 39.5 months (range,8-70 months). The results were evaluated by radiographic representations and joint function according to Lazcano's standards. In BF group, 10 patients were assesed as good and 2 as fair. In MF group, 13 patients were assesed as good, 2 as fair and 1 as poor. There were no statistic differences between these two methods. Conclusion Biodegradable ligament fixation is believed to be a good simple method for the treatment of acromioclavicular joint dislocation without the necessity of a second operation for removal of implants.
