ABSTRACT Objective Currently programmed cell death protein 1 (PD-1) inhibitors in combination with other therapies are being evaluated to determine their efficacy in cancer treatment. However, the effect of PD-ligand (L) 1 expression on disease outcomes in stage III (EC III) non-small cell lung cancer is not completely understood. Therefore, this study aimed to assess the influence of PD-L1 expression on the outcomes of EC III non-small cell lung cancer. Methods This study was conducted on patients diagnosed with EC III non-small cell lung cancer who underwent treatment at a tertiary care hospital. PD-L1 expression was determined using immunohistochemical staining, all patients expressed PD-L1. Survival was estimated using the Kaplan-Meier method. Relationships between variables were assessed using Cox proportional regression models. Results A total of 49 patients (median age=69 years) with EC III non-small cell lung cancer and PD-L1 expression were evaluated. More than half of the patients were men, and most were regular smokers. The patients were treated with neoadjuvant chemotherapy, surgery, or sequential or combined chemotherapy and radiotherapy. The median progression-free survival of the entire cohort was 14.2 months, and the median overall survival was 20 months. There was no significant association between PD-L1 expression and disease progression, clinical characteristics, or overall survival. Conclusions PD-L1 expression was not correlated with EC III non-small cell lung cancer outcomes. Whether these findings differ from the association with immune checkpoint inhibitors remains to be addressed in future studies.

Abstract The present study evaluated 56 patients diagnosed with Chronic Lymphocytic Leukemia (CLL) and a control group of 44 clinically healthy subjects with no previous history of leukemia. Genetic expressions of AKT and microRNAs were evaluated by quantitative PCR (qPCR). A significant increase in AKT gene expression in patients when compared to controls was observed (p = 0.017). When the patients were stratified according to Binet subgroups, a significant difference was observed between the subgroups, with this protein kinase appearing more expressed in the B+C subgroup (p = 0.013). Regarding miRNA expression, miR-let-7b and miR-26a were reduced in CLL patients, when compared to controls. However, no significant differences were observed in these microRNA expressions between the Binet subgroups (A versus B+C). By contrast, miR-21 to miR-27a oncogenes showed no expression difference between CLL patients and controls. AKT protein kinase is involved in the signaling cascade that occurs with BCR receptor activation, leading to increased lymphocyte survival and protection against the induction of cell death in CLL. Thus, increased AKT protein kinase expression and the reduction of miR-let-7b and miR-26a, both tumor suppressors, may explain increased lymphocyte survival in CLL patients and may be promising markers for the prognostic evaluation of this disease.

Chronic lymphocytic leukemia (CLL) is a blood cancer characterized by the accumulation of clonal B-lymphocytes. This study evaluated the mRNA gene expression of miR-15a, miR-16- 1, ZAP-70, and Ang-2 by qPCR, as well as the plasma levels of Bcl-2 by Elisa immunoassay, in CLL patients and healthy controls. Significant differences were observed when comparing patients and controls regarding miR-15a (p < 0.001), miR-16-1 (p < 0.001) mRNA, Ang-2 gene expression, and Bcl-2 plasma levels (p < 0.001). When stratified by risk, differences were maintained with a significantly reduced expression in high-risk patients. A positive correlation was observed between miR-15a and platelets (R2 = 0.340; p = 0.009) as well as between Bcl-2 and leukocytes (R2 = 0.310; p = 0.019). Conversely, negative correlations were observed between ZAP-70 and platelets (R2 = - 0.334; p = 0.011), between miR-15a and lymphocytes (R2 = - 0.376; p = 0.004), as well as between miR-16-and lymphocytes (R2 = - 0.515; p = 0.00004). The data suggest that a reduction in miR-15a and miR-16-1 expressions, in addition to an overexpression of Bcl-2, are associated with the reduction in apoptosis and, consequently, to a longer survival of lymphocytes, thus contributing to lymphocyte accumulation and aggravation of the disease. By contrast, Ang-2 expression was significantly higher in A than in B + C Binet groups. This context leads to the speculation that this biomarker should be investigated in more robust studies within populations with a still relevantly indolent form of the disease in an attempt to identify those patients with a greater potential for an aggravation of the disease

ABSTRACT Objectives: Non-small cell lung cancer (NSCLC) is an incidental and aggressive type of cancer. Although curative treatment can be offered, the recurrence rate is relatively high. Identifying factors that have a prognostic impact may guide changes in the staging system and recommendations for adjuvant therapy. The aim of this study was to evaluate the impact of microvascular invasion on the 5-year overall survival (OS) of patients with resected NSCLC treated at a reference cancer center. Methods: This retrospective, observational cohort study included patients diagnosed with early-stage NSCLC (clinical stages I-IIIA), treated with curative-intent surgery at the Brazilian National Cancer Institute between 2010 and 2016. Results: The dataset comprised 91 surgical patients, mostly females and white, with a mean age of 62 years (range between 29-83). Cases were distributed as stages I, II, and III in 55%, 29%, and 16%. Adenocarcinoma was the predominant histological subtype (67%), and microvascular invasion was present in 25% of the patients. The 5-year OS probability was 60% (95% CI, 48.3-68.9). Among all characteristics, advanced stages (p = 0.001) and the presence of microvascular invasion (p< 0.001) were related to a worse 5-year OS. After adjusting for age group and pathological stage, the presence of microvascular invasion was associated with a 4-fold increased risk of death (HR 3.9, 95% CI, 1.9-8.2). Conclusion: The presence of microvascular invasion was an independent factor related to worse survival and, therefore, should be routinely assessed in resected specimens.
RESUMO Objetivos: O câncer de pulmão não pequenas células (CPNPC) é um tipo incidental e agressivo de câncer. Embora o tratamento curativo possa ser oferecido, a taxa de recidiva é relativamente alta. A identificação de fatores que têm impacto prognóstico pode orientar mudanças no TNM e recomendações para terapia adjuvante. O objetivo deste estudo foi avaliar o impacto da invasão microvascular na sobrevida global (SG) em 5 anos de pacientes com CPNPC ressecado tratados em um centro de referência em câncer. Métodos: Este estudo de coorte retrospectivo e observacional incluiu pacientes diagnosticados com CPNPC em estágio inicial (estágios clínicos I-IIIA), tratados com cirurgia com intenção curativa no Instituto Nacional de Câncer entre 2010 e 2016. Resultados: Foram incluídos 91 pacientes tratados com cirurgia, a maioria mulheres e brancos, com média de idade de 62 anos (variação entre 29-83). Os casos foram distribuídos em estágios I, II e IIIA em 55%, 29% e 16%. Adenocarcinoma foi o subtipo histológico predominante (67%), e a invasão microvascular esteve presente em 25% dos pacientes. A probabilidade de SG em 5 anos foi de 60% (IC 95%, 48,3-68,9). Dentre todas as características analisadas, estágios mais avançados (p = 0,001) e a presença de invasão microvascular (p < 0,001) foram relacionados a uma pior SG em 5 anos. Após ajustar para faixa etária e estágio patológico, a presença de invasão microvascular foi associada a um aumento de 4 vezes no risco de morte (RR 3,9, IC 95%, 1,9-8,2). Conclusão: A presença de invasão microvascular foi um fator independente relacionado a uma pior sobrevida e, portanto, deve ser avaliada rotineiramente em espécimes ressecados.

ABSTRACT Objective Since the rising of coronavirus disease 2019 (COVID-19) pandemic, there is uncertainty regarding the impact of transmission to cancer patients. Evidence on increased severity for patients undergoing antineoplastic treatment is posed against deferring oncologic treatment. We aimed to evaluate the impact of COVID-19 pandemic on patient volumes in a cancer center in an epicenter of the pandemic. Methods Outpatient and inpatient volumes were extracted from electronic health record database. Two intervals were compared: pre-COVID-19 (March to May 2019) and COVID-19 pandemic (March to May 2020) periods. Results The total number of medical appointments declined by 45% in the COVID-19 period, including a 56.2% decrease in new visits. There was a 27.5% reduction in the number of patients undergoing intravenous systemic treatment and a 57.4% decline in initiation of new treatments. Conversely, there was an increase by 309% in new patients undergoing oral chemotherapy regimens and a 5.9% rise in new patients submitted to radiation therapy in the COVID-19 period. There was a 51.2% decline in length of stay and a 60% reduction in the volume of surgical cases during COVID-19. In the stem cell transplant unit, we observed a reduction by 36.5% in length of stay and a 62.5% drop in stem cell transplants. Conclusion A significant decrease in the number of patients undergoing cancer treatment was observed after COVID-19 pandemic. Although this may be partially overcome by alternative therapeutic options, avoiding timely health care due to fear of getting COVID-19 infection might impact on clinical outcomes. Our findings may help support immediate actions to mitigate this hypothesis.
RESUMO Objetivo Desde o surgimento da pandemia da doença pelo coronavírus 2019 (COVID-19), há incerteza quanto ao impacto da transmissão para pacientes com câncer. As evidências sobre o aumento da gravidade para pacientes submetidos a tratamento antineoplásico são contra o adiamento do tratamento oncológico. Nosso objetivo foi avaliar o impacto da pandemia de COVID-19 em volumes de pacientes em um centro oncológico, em um epicentro da pandemia. Métodos Os volumes de pacientes ambulatoriais e de internação foram extraídos do banco de dados de prontuários eletrônicos. Dois intervalos foram comparados: períodos pré-COVID-19 (março a maio de 2019) e pandemia COVID-19 (março a maio de 2020). Resultados O número total de consultas médicas diminuiu 45% no período pandemia COVID-19, inclusive com redução de 56,2% nas novas consultas. Houve redução de 27,5% no número de pacientes em tratamento sistêmico intravenoso e de 57,4% no início de novos tratamentos. Por outro lado, ocorreram aumento de 309% em novos pacientes submetidos a regimes de quimioterapia oral e elevação de 5,9% em novos pacientes submetidos à radioterapia no período pandemia COVID-19. Observaram-se queda de 51,2% nos dias de internação e redução de 60% no volume de casos cirúrgicos durante a COVID-19. Na unidade de transplante de células-tronco, a redução foi de 36,5% nos dias de internação e de 62,5% nos transplantes de células-tronco. Conclusão Foi observado declínio significativo no número de pacientes em tratamento de câncer após a pandemia de COVID-19. Embora isso possa ser parcialmente superado por opções terapêuticas alternativas, evitar cuidados de saúde oportunos devido ao medo de contrair COVID-19 pode impactar nos resultados clínicos. Nossos resultados podem ajudar a apoiar ações imediatas para mitigar essa hipótese.

ABSTRACT Objective: To evaluate clinical features and complications in patients with bowel endometriosis submitted to hormonal therapy. Methods: Retrospective study based on data extracted from medical records of 238 women with recto-sigmoid endometriosis treated between May 2010 and May 2016. Results: Over the course of follow-up, 143 (60.1%) women remained in medical treatment while 95 (39.9%) presented with worsening of pain symptoms or intestinal lesion growth (failure of medical treatment group), with surgical resection performed in 54 cases. Women in the Medical Treatment Group were older (40.5±5.1 years versus 37.3±5.8 years; p<0.0001) and had smaller recto sigmoid lesions (2.1±1.9 versus 3.1±2.2; p=0.008) compared to those who had failed to respond to medical treatment. Similar significant reduction in pain scores for dysmenorrhea, chronic pelvic pain, cyclic dyschezia and dysuria was observed in both groups; however greater reduction in pain scores for dyspareunia was noted in the Surgical Group. Subjective improvement in pain symptoms was also similar between groups (100% versus 98.2%; p=0.18). Major complications rates were higher in the Surgical Group (9.2% versus 0.6%; p=0.001). Conclusion: Patients with recto-sigmoid endometriosis who failed to respond to medical treatment were younger and had larger intestinal lesions. Hormonal therapy was equally efficient in improving pain symptoms other than dyspareunia compared to surgery, and was associated with lower complication rates in women with recto-sigmoid endometriosis. Medical treatment should be offered as a first-line therapy for patients with bowel endometriosis. Surgical treatment should be reserved for patients with pain symptoms unresponsive to hormonal therapy, lesion growth or suspected intestinal subocclusion.
RESUMO Objetivo: Avaliar características clínicas e complicações em pacientes com endometriose intestinal submetidos ao tratamento hormonal. Métodos: Dados de prontuários de 238 pacientes com endometriose de retossigmoide tratadas entre maio de 2010 e maio de 2016 foram coletados para este estudo retrospectivo. Resultados: Durante o período de acompanhamento, 143 (60,1%) mulheres mantiveram tratamento clínico, enquanto 95 (39,9%) tiveram piora dos sintomas de dor ou aumento da lesão intestinal (grupo falha de tratamento clínico), sendo 54 submetidas ao tratamento cirúrgico. As mulheres no Grupo Tratamento Clínico eram mais velhas (40,5±5,1 anos versus 37,3±5,8 anos; p<0,0001) e tinham lesões intestinais menores (2,1±1,9 versus 3,1±2,2; p=0,008) em comparação ao grupo falha de tratamento clínico. Redução significativa e semelhante do escore de dor na dismenorreia, dor pélvica crônica, disquezia cíclica e disúria cíclica foi observada nos Grupos Tratamento Clínico e Cirúrgico. Dispareunia, no entato, teve uma redução maior no Grupo Cirurgia. A redução subjetiva dos sintomas dolorosos também foi semelhante entre os Grupos Clínico e Cirúrgico (100% versus 98,2%; p=0,18). O Grupo Tratamento Cirúrgico foi relacionado a uma maior taxa de complicações graves (9,2% versus 0,6%; p=0,001) em comparação ao Grupo Tratamento Clínico. Conclusão: Falha no tratamento clínico em pacientes com endometriose de retossigmoide foi observada em mulheres mais jovens que tinham lesões intestinais maiores. O tratamento clínico hormonal foi igualmente eficaz na melhora dos sintomas de dor, exceto dispareunia, em comparação ao tratamento cirúrgico em mulheres com endometriose intestinal, mas com menor taxa de complicações. O tratamento clínico deve ser oferecido como primeira opção em pacientes com endometriose intestinal, enquanto o tratamento cirúrgico deve ser reservado para pacientes sem melhora nos sintomas de dor com tratamento hormonal, progressão das lesões ou suspeita de suboclusão intestinal.

ABSTRACT BACKGROUND: High resolution anorectal manometry (HRAM-WP) allows more simplified, objective, and uniform data acquisition and interpretation of the test results. OBJECTIVE: To validate a HRAM under water perfusion (Alacer Biomédica) with a 24-channel probe and to compare the results of anorectal manometry with other systems. METHODS: Individuals without critical evacuation disorders were selected. Patients with incontinence, anal surgery, dyssynergia or sphincter injury were excluded. The test was performed with an Alacer Biomédica 24 channel manometry system under water perfusion, with a probe configured with 6 levels of 4 radial channels, separated from each other by 0.8 mm. The mean pressures for the functional channel were determined, in states of rest (RMP), contention effort (CMP) and evacuation effort (EEMP). The pressure extension of the sphincter was also tabulated in cm. The results were compared with those available in recent literature. RESULTS: Fifty patients were studied (20 men; 30 women). Overall, the following results were obtained: the RMP was 76.9±3.0 mmHg, the CMP was 194.2±9.4 mmHg, and EEMP was 88.2±3.7 mmHg. When classified according to the gender, for men: RMP was 72.2±3.0 mmHg, CMP was 229.5±17 mmHg, and EEMP was 91.4±7.0. For women, RMP was 79.8±4.0 mmHg, CMP was 170.7±8, and EEMP was 86.1±4.3 mmHg. The sphincter gauge extension for both genders was 3.1±0.09 cm (men 3.3±0.1; women 3.0±0.1). DISCUSSION: Studying HRAM-WP has become much easier. Non-mobilization of the sensor causes less discomfort and artefacts with a lower assessment time. In this study, small differential values between both sexes during rest were observed, highlighting a greater containment force in men. No difference in sphincter extension was noted. The results of this study are consistent with that of existing reports and with those obtained using solid state probes. CONCLUSION: The perfusion system yielded results similar to that of solid state systems. Further studies to evaluate parameters with respect to pelvic dyssynergia and incontinence need to be conducted. Additionally, to determine if the vector volume can furnish new information in terms of functional and anatomical aspects.
RESUMO CONTEXTO: Através da manometria anorretal de alta resolução (MARAR), a aquisição dos dados e a interpretação do exame tornaram-se mais simplificadas, objetivas e uniformes. OBJETIVO: Validar um sistema de MARAR sob perfusão de água (Alacer Biomédica), com sonda de 24 canais e comparar resultados dos exames de manometria anorretal com outros sistemas em trabalhos já publicados. MÉTODOS: Selecionados indivíduos sem distúrbio evacuatório importante. Excluídos pacientes com incontinência, cirurgia orificial, dissinergia, ou lesão esfincteriana. O exame foi realizado com sistema Alacer Biomédica de manometria sob perfusão de agua de 24 canais, com sonda configurada com 6 níveis de 4 canais radiais, distanciados entre si por 0,8 mm. Estabelecidas as pressões médias para o canal funcional, nos estados de repouso (PMR), no esforço de contenção (PMC) e no esforço evacuatório (PMEE). Também foi tabulada a extensão pressórica do esfíncter em cm. Comparou-se os resultados com os disponíveis em literatura recente. RESULTADOS: Foram estudados 50 pacientes (20 masc; 30 fem). No geral, foram encontrados os seguintes resultados: a PMR foi de 76,9±3,0 mmHg; PMC foi de 194,2±9,4 mmHg e; PMEE foi de 88,2±3,7 mmHg. Quando divididos por sexo: sexo masculino: PMR 72,2±3,0 mmHg; PMC: 229,5±17 mmHg e; PMEE 91,4±7,0. Sexo feminino: PMR 79,8±4,0 mmHg; PMC: 170,7±8; PMEE 86,1±4,3 mmHg. A extensão manométrica para ambos os sexos foi de 3,1±0,09 cm (masc 3,3±0,1; fem 3,0±0,1). DISCUSSÃO: A realização do estudo da MARAR ficou muito facilitada. A não mobilização da sonda provoca menos desconforto e artefatos, com menor tempo de estudo. Em nossa série há valores diferenciais pequenos entre os sexos durante o repouso, destacando-se maior força de contenção no sexo masculino. Não houve diferença para a extensão do esfíncter. Em relação à comparação com os estudos já publicados, mesmo com sondas de solid state, há uma proximidade de valores. CONCLUSÃO: O sistema de perfusão utilizado permitiu reproduzir resultados similares a sistemas solid state. Resta estabelecer parâmetros em casos de dissinergia pélvica, incontinência e esclarecer se o estudo pelo vetor volume pode trazer novas informações nos aspectos funcional e anatômico.

ABSTRACT Introduction: Described in 1983 and with a stable clinical application, the impact of Transanal Endoscopic Microsurgery on anorectal function remains unknown. The objectives of the present study were to evaluate the impact of Transanal Endoscopic Microsurgery on anorectal function according to clinical (Wexner score) and functional (anorectal manometry) evaluations before and after surgery. Method: Prospectively, 23 consecutive patients with rectal lesions were operated using TEO® equipment (Karl Storz, Tuttlingen, Germany). For all patients, the value of Wexner score was obtained before and after surgery (7, 30 and 90 days) and anorectal electromanometry was performed before surgery, and also postoperatively (30 and 90 days). Results: Fourteen patients were men. The mean age was 53.7 (24-81) years. The mean distance from the lesion to the pectineal line was 7 (2-15) cm. The histopathologic analysis revealed adenoma in 14 (61%); neuroendocrine tumor in 5 (21.7%); invasive carcinoma in 3 (13%); and hyperplastic polyp in 1 (4.3%) case. The mean duration of postoperative follow-up was 5 (3-7) months. Wexner score was significantly lower at 30 days compared to 7 days (Wilcoxon; p = 0.03). Rectal capacity was significantly lower 30 days after surgery and recovered at 90 days of surgery (ANOVA; p = 0.04). Conclusions: After Transanal Endoscopic Microsurgery, a modest impact on anorectal function can be observed. The transient impairment results from loss of rectal capacity and not from impairment of the anal sphincters, being completely resolved 90 days after surgery.
RESUMO Introdução: Descrita em 1983 e de sólida aplicação clínica, o impacto da Microcirurgia Endoscópica Transanal sobre a função anorretal permanece pouco conhecido. Os objetivos do presente estudo foram avaliar o impacto da Microcirurgia Endoscópica Transanal na função anorretal conforme avaliações clínica (Wexner score) e funcional (manometria anorretal) antes e após a cirurgia. Método: Prospectivamente, 23 pacientes consecutivos com lesões retais foram operados com o uso do equipamento TEO® (Karl Storz, Tuttlingen, Alemanha). Para todos os pacientes, o valor do escore de Wexner foi obtido antes e após a cirurgia (7, 30 e 90 dias) e a eletromanometria anorretal foi realizada antes da cirurgia e também no pós-operatório (30 e 90 dias). Resultados: Quatorze pacientes eram homens. A idade média foi 53,7 (24-81) anos. A distância média da lesão à linha pectínea foi de 7 (2-15) cm. A histopatológica revelou adenoma em 14 (61%), tumor neuroendócrino em 5 (21,7%); carcinoma invasivo em 3 (13%) e pólipo hiperplásico em 1 (4,3%) caso. A duração média do seguimento pós-operatório foi de 5 (3-7) meses. O escore de Wexner foi significativamente menor aos 30 dias em comparação com 7 dias (Wilcoxon; p = 0,03). A capacidade retal foi significativamente menor aos 30 dias após a cirurgia e recuperada aos 90 dias após a cirurgia (ANOVA; p = 0,04). Conclusões: Após Microcirurgia Endoscópica Transanal, um impacto modesto na função anorretal pode ser observado. O comprometimento transitório resulta de perda de capacidade retal e não por comprometimento dos esfíncteres anais cessando completamente 90 dias após a cirurgia.

ABSTRACT Background: Percutaneous drainage for pyogenic liver abscess has been considered the gold-standard approach for the treatment on almost of the cases. However, when percutaneous drainage fails or even in some especial situations, as multiloculate abscess, lobe or segment surgical resection can solve infectious clinical condition. Aim: To report a series of patients who underwent hepatectomy for pyogenic liver abscess performed by a single surgical team. Methods: Eleven patients were operated with ages ranging from 45-73 years (mean and median 66 years). There were eight men and three women. The etiologies were: idiopathic (n=4), biliary (n=2), radiofrequency (n=2), direct extension (n=1), portal (n=1), and arterial (n=1). The mean lesion diameter was 9.27 cm (6-20 cm). Results: The mean operation length was 180 min (120-300). The mean intra-operative blood loss was 448 ml (50-1500). Surgical approaches were: right hepatectomy (n=4), left hepatectomy (n=3), left lateral sectioniectomy (n=1), right posterior sectioniectomy (n=2), resection of S8 (n=1), and S1 (n=1). Postoperative morbidity rate was 30%, while mortality was null. Median hospital stay was 18 days (5-45). The median follow-up period was 49 months (13-78). There was single lesion recurrence. Conclusion: Hepatectomy can be done as exception approach for pyogenic hepatic abscess treatment; it is a good therapeutic option in special situations.
RESUMO Racional: A drenagem percutânea do abscesso hepático piogênico tem sido considerada padrão-ouro para o tratamento na maioria dos casos. Mais raramente, quando essa abordagem falha ou em casos de abscessos multiloculados, a ressecção daquele segmento ou lobo destruído pode resolver o contexto infeccioso. Objetivo: Relatar uma série de doentes com abscesso hepático que foram submetidos a hepatectomia. Método: Onze doentes foram operados. A idade variou de 45-73 anos (média e mediana 66). Foram oito homens e três mulheres. As causas foram: idiopática (n=4), biliar (n=2), radiofrequência (n=2), portal (n=1), arterial (n=1), e extensão direta (n=1). A média do tamanho das lesões foi 9,27 cm (6-20). Resultados A média de tempo cirúrgico foi de 180 min (120-300). A média de sangramento intra-operatório foi de 448 ml (50-1500 ml). Os procedimentos cirúrgicos foram: hepatectomia D (n=4), hepatectomia E (n=3), setorectomia lateral E (n=1), setorectomia posterior D (n=2), ressecção do S8 (n=1) e S1 (n=1). A morbidade pós-operatória foi 30%, enquanto a mortalidade foi nula. Mediana de internação foi de 18 dias (5-45). A mediana de seguimento foi 49 meses (13-78). Houve uma única recidiva de lesão. Conclusão: Embora a hepatectomia possa ser considerada conduta de exceção para tratamento do abscesso hepático piogênico, ela deve ser boa opção terapêutica em situações especiais.

ABSTRACT Background: Laparoscopic distal pancreatectomy has been the choice for resection of distal pancreas lesions due many advantages over open approach. Spleen preservation technique seems minimizes infectious complications in long-term outcome. Aim: To present the results of laparoscopic distal pancreatectomies with spleen preservation by Kimura´s technique (preservation of spleen blood vessels) performed by single surgical team. Methods: Retrospective case series aiming to evaluate both short and long-term outcomes of laparoscopic distal pancreatectomies with spleen preservation. Results: A total of 54 laparoscopic distal pancreatectomies were performed, in which 26 were laparoscopic distal pancreatectomies with spleen preservation by Kimura´s technique. Mean age was 47.9 years-old (21-75) where 61.5% were female. Mean BMI was 28.5 kg/m² (18-38.8). Mean diameter of lesion was 4.3 cm (1.8-7.5). Mean operative time was 144.1 min (90-200). Intraoperative bleeding was 119.2 ml (50-600). Conversion to laparotomy 3% (n=1). Postoperative morbidity was 11.5%. Postoperative mortality was null. Mean of hospital stay was 4.8 days (2-14). Mean time of follow-up period was 19.7 months (2-60). There was no neoplasm recurrence or mortality on evaluated period. There was no infectious complication. Conclusion: Laparoscopic distal pancreatectomy with spleen and splenic vessels preservation is feasible, safe, and effective procedure. This technique presented both low morbidity and null mortality on this sample. There were neither infectious complications nor neoplasm recurrence on long-term follow-up period.
RESUMO Racional: A pancreatectomia distal laparoscópica tem sido a abordagem de escolha para ressecção de tumores em pâncreas distal devido a muitas vantagens sobre a abordagem laparotômica. A técnica com preservação esplênica parece minimizar complicações infecciosas em longo prazo. Objetivo: Analisar os resultados das pancreatectomias distais laparoscópicas com preservação esplênica e dos vasos esplênicos. Método: Série de casos com 26 doentes operados para avaliar os resultados em curto e longo prazo. Resultados: Cinquenta e quatro pancreatectomias distais laparoscópicas foram realizadas nesse período, onde 26 foram pancreatectomias distais laparoscópicas com preservação esplênica pela técnica de Kimura. A média de idade foi 47,9 anos (21-75), onde 61,5% eram mulheres. A média do IMC foi 28,5 kg/m² (18-38,8). O diâmetro médio das lesões foi 4,3 cm (1,8-7,5). O tempo cirúrgico médio foi 144,1 min (90-200). A média de sangramento intra-operatório foi 119,2 ml (50-600). O índice de conversão foi 3% (n=1). A morbidade pós-operatória foi 11,5%, e a mortalidade nula. A média de internação hospitalar foi 4,8 dias (2-14). A média de seguimento foi 19,7 meses (2-60). Não houve recorrência de neoplasias e nem mortalidade durante esse período e também não houve complicações infecciosas em longo prazo. Conclusão: A pancreatectomia distal laparoscópica com preservação esplênica e dos vasos esplênicos é procedimento factível, seguro e eficaz no tratamento de neoplasias pancreáticas. Esta técnica apresentou baixa morbidade e mortalidade. Não houve complicações infecciosas e nem recidiva neoplásica em longo prazo.

ABSTRACT Background : The best treatment for anal fistula should extirpate infection and promote healing of the tract, whilst preserving the anal sphincter complex and full continence. Aim: To analyze the success rate after a modified technique for ligation of the intersphincteric fistula tract (LIFT) for patients with anal fistulas. Methods: A prospective (observational cohort study) Brazilian bi-institutional experience with a modified (ligation of the intersphincteric fistula tract without excision) LIFT technique was undertaken. A clinical database was settled for the following variables: age, gender, BMI, comorbidities, distance between external orifice and the anus, previous fistula surgery, type of fistula, operative time, intra- and postoperative complications, duration of follow-up, and success rate. Results: Between November 2015 and January 2017, 38 patients with transsphincteric fistulas were operated on using the modified LIFT procedure. Seventeen (44.7%) were men. Median age was 41 (18-67) years. Median BMI was 26.4 (22-38) kg/m2. Five (13.2%) had undergone previous surgery. The fistula was transsphincteric in all cases. Median follow-up was 32 (range, 14-56) weeks. Success was observed in 30 (79%) patients. Conclusions: The LIFT technique without excision of the fistula tract proved to be safe and effective for transsphincteric anal fistulas.
RESUMO Racional : O melhor tratamento para a fístula anal deve eliminar a infecção e promover a cicatrização do trato, preservando o esfíncter anal e a continência completa. Objetivo: Determinar a taxa de sucesso após o uso da técnica modificada de ligadura interesfincteriana do trato fistuloso (LIFT) para pacientes com fístulas anais. Métodos: Estudo de coorte observacional brasileiro bi-institucional com o procedimento LIFT modificado (ligadura interesfincteriana do trato fistuloso sem excisão). Foi estabelecida base de dados clínica para as seguintes variáveis: idade, gênero, IMC, comorbidades, distância entre o orifício externo e o ânus, operação anterior para fístula, tipo de fístula, tempo cirúrgico, complicações intra e pós-operatórias, duração do seguimento e taxa de sucesso. Resultados: Entre novembro de 2015 e janeiro de 2017, 38 pacientes com fístulas transesfincterianas foram operados com o procedimento LIFT modificado. Dezessete (44,7%) eram homens. A idade média foi de 41 (18-67) anos. O IMC médio foi de 26,4 (22-38) kg/m2. Cinco (13,2%) tinham sido submetidos à operação anterior. A fístula era transesfincteriana em todos os casos. O acompanhamento médio foi de 32 (14-56) semanas. Sucesso foi observado em 30 (79%) pacientes. Conclusões: A técnica LIFT sem excisão do trato da fístula provou ser segura e eficaz para fístulas anais transesfincterianas.

ABSTRACT Background: Laparoscopic hepatectomy has presented great importance for treating malignant hepatic lesions. Aim: To evaluate its impact in relation to overall survival or disease free of the patients operated due different hepatic malignant tumors. Methods: Thirty-four laparoscopic hepatectomies were performed in 31 patients with malignant neoplasm. Patients were distributed as: Group 1 - colorectal metastases (n=14); Group 2 - hepatocellular carcinoma (n=8); and Group 3 - non-colorectal metastases and intrahepatic cholangiocarcinoma (n=9). The conversion rate, morbidity, mortality and tumor recurrence were also evaluated. Results: Conversion to open surgery was 6%; morbidity 22%; postoperative mortality 3%. There was tumor recurrence in 11 cases. Medians of overall survival and disease free survival were respectively 60 and 46 m; however, there was no difference among studied groups (p>0,05). Conclusion: Long-term outcomes of laparoscopic hepatectomy for treating hepatic malignant tumors are satisfactory. There is no statistical difference in relation of both overall and disease free survival among different groups of hepatic neoplasms.
RESUMO Racional: A hepatectomia laparoscópica tem apresentado grande importância no tratamento das lesões hepáticas malignas. Objetivo: Avaliar o impacto dela realizada por uma única equipe em relação à sobrevida global e tempo livre de doença nos diferentes tumores malignos hepáticos. Métodos: Foram realizadas 34 hepatectomias laparoscópicas em 31 pacientes com neoplasia maligna. Os doentes foram distribuídos em: Grupo 1 - metástases colorretais (n=14); Grupo 2 - carcinoma hepatocelular (n=8) e Grupo 3 - metástases não-colorretais e colangiocarcinoma intra-hepático (n=9). As curvas de sobrevida e sobrevida livre de doença foram estimadas. Foram avaliadas também a taxa de conversão, morbidade, mortalidade e recorrência tumoral. Resultados: A taxa de conversão foi de 6%; a morbidade de 22%; a mortalidade pós-operatória de 3%; recorrência tumoral em 11 casos. As medianas de sobrevida global e de sobrevida livre de doença foram respectivamente de 60 e 46 m, contudo não houve diferença entre os grupos estudados (p>0,05). Conclusão: Os resultados em longo prazo da hepatectomia laparoscópica para o tratamento de tumores malignos hepáticos são satisfatórios. Não houve diferença estatisticamente significante quanto às sobrevidas global e livre de doença nos diferentes grupos de neoplasia tratada.

Abstract INTRODUCTION: The occurrence of leprosy reactions, a common event during treatment, may be mostly related to the action of multidrug therapy on Mycobacterium leprae. The clinical and laboratory monitoring of patients with reactions is important, since collecting data that assists in predicting the risk of reactions may help to prevent disability. METHODS: This was a sectional study, in order to correlate clinical and laboratory diagnosis with the number of reactions during treatment. Spearman's correlation was used to verify the degree of association between the assessed variables. RESULTS: This study was conducted with 211 patients with leprosy reactions during treatment of M. leprae. The borderline tuberculoid group was the most prevalent clinical form (74/211; 35.1%) and the type one reaction showed the highest frequency (136/211; 64.5%). It was observed that 73.5% (155/211) of reactions occurred within 3 months of the initiation of multidrug therapy. The diagnostic values, including the bacterial indices (BIs) of dermal smears (r = 0.21, p < 0.05) and skin biopsies (r = 0.20; p < 0.05), showed a positive correlation with the number of reactions during treatment. CONCLUSIONS: This research showed a positive correlation between bacillary load markers and the number of leprosy reactions. This study provided scientific support to future research aiming to elucidate the influence of antigenic load on the number of leprosy reactions during treatment.

ABSTRACT Purpose: To develop a rabbit experimental study to test the hypothesis that surgical repair of hypospadias with severe ventral curvatures might be completed in one stage, if a graft, such as buccal mucosa, could be placed over the tunica vaginalis flap used in corporoplasty for ventral lengthening, with the addition of an onlay preputial island flap to complete the urethroplasty. Materials and methods: The experimental procedure with rabbits included a tunica vaginalis flap for reconstruction of the corpora after corporotomy, simulating a ventral lengthening operation. A buccal mucosa graft was placed directly on top of the flap, and the urethroplasty was completed with an onlay preputial island flap. Eight rabbits were divided into 4 groups, sacrificed at 2, 4, 8 and 12 weeks postoperatively, and submitted to histological evaluation. Results: We observed a large number of complications, such as fistula (75%), urinary retention (50%) and stenosis (50%). There were two deaths related to the procedure. Histological evaluation demonstrated a severe and persistent inflammatory reaction. No viable tunica vaginalis or buccal mucosa was identified. Conclusions: In this animal model, the association of a buccal mucosa graft over the tunica vaginalis flap was not successful, and resulted in complete loss of both tissues.

ABSTRACT Background: Stapled hemorrhoidopexy is associated with less postoperative pain and faster recovery. However, it may be associated with a greater risk of symptomatic recurrence. We hypothesized that undertaking a limited surgical excision of hemorrhoid disease after stapling may be a valid approach for selected patients. Aim: To compare long-term results after stapled hemorrhoidopexy with and without complementation with closed excisional technique. Method: In a retrospective uni-institutional cohort study, sixty-five (29 men) patients underwent stapled hemorrhoidopexy and 21 (13 men) underwent stapled hemorrhoidopexy with excision. The same surgeons operated on all cases. Patients underwent stapled hemorrhoidectomy associated with excisional surgery if symptoms attributable to external hemorrhoid piles were observed preoperatively, or if residual prolapse or bulky external disease was observed after the firing of the stapler. A closed excisional diathermy hemorrhoidectomy without vascular ligation was utilized in all complemented cases. All clinical variables were obtained from a questionnaire evaluation obtained through e-mail, telephone interview, or office follow-up. Results: The median duration of postoperative follow-up was 48.5 (6-40) months. Patients with grades 3 and 4 hemorrhoid disease were operated on more frequently using stapled hemorrhoidopexy complemented with excisional technique (95.2% vs. 55.4%, p=0.001). Regarding respectively stapled hemorrhoidopexy and stapled hemorrhoidopexy complemented with excision, there was no difference between the techniques in relation to symptom recurrence (43% and 33%, p=0.45) and median interval between surgery and symptom recurrence (30 (8-84) and 38.8 (8-65) months, p=0.80). Eight (12.3%) patients were re-operated after stapled hemorrhoidopexy and 2 (9.6%), after hemorrhoidopexy with excision (p=0.78). Patient distribution in both groups according to the degree of postoperative satisfaction was similar (p=0.97). Conclusion: Stapled hemorrhoidopexy combined with an excisional technique was effective for more advanced hemorrhoid disease. The combination may have prevented symptomatic recurrence associated to stapled hemorrhoidopexy alone.
RESUMO Racional: A hemorroidopexia com grampeador está significativamente associada a menor dor pós-operatória e recuperação mais rápida. No entanto, há evidência de que possa ter maior taxa de recorrência sintomática. A hipótese é que empreender excisão cirúrgica limitada da doença hemorroidária após o grampeamento pode ser abordagem válida para pacientes selecionados. Objetivo: Avaliar os resultados comparativos a longo prazo após a técnica de grampeamento isolada ou associada à complementação por técnica excisional fechada. Método: Coorte retrospectiva, com 65 (29 homens) pacientes submetidos à hemorroidopexia por grampeamento e 21 (13 homens) a ela complementada por técnica excisional. Os pacientes foram submetidos à complementação do grampeamento se sintomas atribuíveis às hemorróidas externas fossem observados no pré-operatório, ou se prolapso residual ou doença externa volumosa existisse após o disparo do grampeador. A hemorroidectomia excisional fechada por eletrocauterização sem ligadura vascular foi utilizada em todos os casos de complementação. Todas as variáveis clínicas foram obtidos a partir de um questionário eletrônico, entrevista por telefone, ou em consulta. Resultados: A duração média do seguimento pós-operatório foi de 48,5 (6-40) meses. Os pacientes com graus hemorroidários 3 e 4 foram operados com mais frequência usando grampeamento complementado por técnica excisional (95,2% vs. 55,4% - p=0,001). Os pacientes operados por grampeamento sem e com complementação por técnica excisional, não houve diferença entre os grupos quanto a recidiva dos sintomas (43% vs. 33%, p=0,45, respectivamente) ou quanto ao intervalo médio entre a opração e a recorrência dos sintomas (30 (8-84) vs. 38,8 (8-65) meses, p=0,80). Oito (12,3%) pacientes foram reoperados após grampeamento isolado e 2 (9,6%), após grampeamento complementado por técnica excisional (p=0,78). O grau de satisfação pós-operatória foi semelhante entre as técnicas (p=0,97). Conclusão: A hemorroidopexia por grampeamento combinada com técnica excisional foi eficaz para a doença hemorróida mais avançada. A combinação pode ter impedido a recorrência sintomática associada a hemorroidopexia isolada.

Abstract: INTRODUCTION: Despite multidrug therapy, leprosy remains a public health issue. The intradermal Bacillus Calmette-Guérin (BCG) vaccine, Mitsuda test (lepromin skin test), and anti-phenolic glycolipid I (PGL-I) serology are widely used in leprosy studies and have shown great epidemiological value. METHODS: This longitudinal study evaluated the relative risks and benefits of these three tools by comparing results observed in household contacts (HHCs) of leprosy patients who developed leprosy with those of HHCs who did not in a population of 2,992 individuals monitored during a 10-year period. RESULTS : Seventy-five (2.5%) new leprosy cases were diagnosed, including 28 (0.9%) co-prevalent cases. Therefore, for the risk-benefit assessment, 47 (1.6%) HHCs were considered as truly diagnosed during follow-up. The comparison between healthy and affected contacts demonstrated that not only did BCG vaccination increase protection, but boosters also increased to 95% relative risk (RR) reduction when results for having two or more scars were compared with having no scars [RR, 0.0459; 95% confidence interval (CI), 0.006-0.338]. Similarly, Mitsuda reactions >7mm in induration presented 7-fold greater protection against disease development compared to reactions of 0-3mm (RR, 0.1446; 95% CI, 0.0566-0.3696). In contrast, anti-PGL-I ELISA seropositivity indicated a 5-fold RR increase for disease outcome (RR, 5.688; 95% CI, 3.2412-9.9824). The combined effect of no BCG scars, Mitsuda reaction of <7mm, and seropositivity to anti-PGL-I increased the risk for leprosy onset 8-fold (RR, 8.109; 95% CI, 5.1167-12.8511). CONCLUSIONS: The adoption of these combined assays may impose measures for leprosy control strategies.

Oncologic laparoscopic colectomy represents a fully validated surgical approach to the management of colorectal cancer. However, laparoscopic surgery for distal transverse and descending colon lesions remains a challenging procedure. A total laparoscopic approach to the left colectomy is an interesting option for critically ill patients although reports in the literature on this subject are scarce and its approach still not standardized because of its selective nature for indication. There are several advantages associated with conduction of totally laparoscopic approach to the left colon. Intracorporeal vessel sealing ensures an adequate lymph node dissection. Moreover, it enables the construction of a well-vascularized anastomosis. Ultimately, the occurrence of late wound complications are possibly reduced for the placement of a low abdominal incision exclusively used for specimen extraction. This paper aimed at describing our technique for a totally laparoscopic left colectomy for distal transverse and descending colon lesions.
A colectomia oncológica por videolaparoscopia representa uma abordagem cirúrgica validada no tratamento radical do câncer colorretal. No entanto, a cirurgia laparoscópica para lesões de cólon transverso e descendente distal persiste como um procedimento tecnicamente complexo. A abordagem totalmente laparoscópica para a realização da colectomia esquerda é uma opção interessante para pacientes selecionados, ainda que parcialmente avaliada na literatura e, sobretudo, carecendo de padronização técnica, devido à natureza seletiva de sua indicação. Existem várias vantagens associadas à realização da colectomia esquerda totalmente laparoscópica. A realização do controle vascular por videolaparoscopia assegura uma adequada extensão da dissecção linfonodal. Além disso, permite a construção de uma anastomose bem vascularizada. Finalmente, a ocorrência de complicações tardias da ferida pode ser reduzida devido ao emprego de uma incisão abdominal utilizada exclusivamente para a retirada do amostra cirúrgica. Este artigo teve como objetivo descrever uma proposta técnica de padronização da colectomia esquerda totalmente laparoscópica para lesões de cólon transverso e descendente distal.

Introduction: The laboratory diagnosis of paroxysmal nocturnal hemoglobinuria (PNH), disease that is categorized by reduced synthesis of glycosylphosphatidylinositol (GPI) anchor, is based on the detection of blood cells deficient in GPI-anchored proteins by flow cytometry. PNH clones have been detected in patients with aplastic anaemia (AA) and myelodysplastic syndrome (MDS), with therapeutic implications. Objectives: To validate a sensitive assay for detection of GPI-anchored protein-deficient cells, by flow cytometry, and to analyze the clone frequency in AA and MDS patients. Methods: Samples from 20 AA patients, 30 MDS patients and 20 adult volunteers (control group) were analyzed using monoclonal antibodies to CD16, CD24, CD55 and CD59 (neutrophils); CD14 and CD55 (monocytes); CD55 and CD59 (erythrocytes); besides fluorescent aerolysin reagent (FLAER) (neutrophils and monocytes) and lineage markers. The proportions of PNH cells detected in neutrophils and monocytes, using different reagent combinations, were compared by analysis of variance (ANOVA) and Pearson's correlation. Results: PNH cells were detected in five (25%) AA patients, and the proportions of PNH cells varied from 0.14% to 94.84% of the analyzed events. PNH cells were not detected in the MDS patients. However, by the analysis of these samples, it was possible to identify the technical challenges caused by the presence of immature and dysplastic circulating cells. Introdução: O diagnóstico laboratorial da hemoglobinúria paroxística noturna (HPN), doença caracterizada por deficiência de síntese da molécula de ancoragem glicosilfosfatidilinositol (GPI), baseia-se na detecção de células sanguíneas deficientes em proteínas ancoradas ao GPI, por citometria de fluxo. Clones de células com fenótipo HPN podem ser detectados em pacientes com anemia aplásica (AA) e síndrome mielodisplásica (SMD), com implicações terapêuticas. Objetivos: Validar técnica sensível para detecção de células HPN, por citometria de fluxo, e avaliar a frequência dos clones em pacientes com AA e SMD. Métodos: Amostras de 20 pacientes com AA, 30 pacientes com SMD e 20 voluntários (controles) foram analisadas, utilizando anticorpos monoclonais anti-CD16, CD24, CD55 e CD59 (neutrófilos); CD14 e CD55 (monócitos); e CD55 e CD59 (hemácias); além do reagente de aerolisina fluorescente (FLAER) (neutrófilos e monócitos) e marcadores de linhagem celular. A comparação do tamanho dos clones HPN detectados em neutrófilos e monócitos, pelas diferentes combinações de reagentes, foi realizada por análise de variância (ANOVA) e correlação de Pearson. Resultados: Em cinco (25%) pacientes com AA foram identificadas células HPN, em proporções entre 0,14% e 94,84% dos eventos analisados. O clone não foi detectado nos pacientes com SMD. Leprosy remains prevalent in Brazil. ErbB2 is a receptor for leprosy bacilli entering Schwann cells, which mediates Mycobacterium leprae-induced demyelination and the ERBB2 gene lies within a leprosy susceptibility locus on chromosome 17q11-q21. To determine whether polymorphisms at the ERBB2 locus contribute to this linkage peak, three haplotype tagging single nucleotide polymorphisms (tag-SNPs) (rs2517956, rs2952156, rs1058808) were genotyped in 72 families (208 cases; 372 individuals) from the state of Pará (PA). All three tag-SNPs were associated with leprosy per se [best SNP rs2517959 odds ratio (OR) = 2.22; 95% confidence interval (CI) 1.37-3.59; p = 0.001]. Lepromatous (LL) (OR = 3.25; 95% CI 1.37-7.70; p = 0.007) and tuberculoid (TT) (OR = 1.79; 95% CI 1.04-3.05; p = 0.034) leprosy both contributed to the association, which is consistent with the previous linkage to chromosome 17q11-q21 in the population from PA and supports the functional role of ErbB2 in disease pathogenesis. To attempt to replicate these findings, six SNPs (rs2517955, rs2517956, rs1810132, rs2952156, rs1801200, rs1058808) were genotyped in a population-based sample of 570 leprosy cases and 370 controls from the state of Rio Grande do Norte (RN) and the results were analysed using logistic regression analysis. However, none of the associations were replicated in the RN sample, whether analysed for leprosy per se, LL leprosy, TT leprosy, erythema nodosum leprosum or reversal reaction conditions. The role of polymorphisms at ERBB2 in controlling susceptibility to leprosy in Brazil therefore remains unclear.
