RESUMEN

Objective:To investigate the comfort of postoperative rectal cancer patients with indwelling anal tube and analyze the influencing factors, so as to provide a basis for the formulation of prevention and improvement measures. Methods:The subjective comfort self-report method of digital score and visual analog score was used to investigate and analyze 64 patients with indwelling anal canal in anorectal surgery. Results:The main discomfort of patients with indwelling anal tube were difficulty in sitting and getting in and out of bed, of which 43 cases (61.9%) reported moderate and severe sitting difficulty, and 30 cases (46.88%) reported moderate and severe difficulty in getting up and down the bed. The exposed length of anal canal was the influencing factor of discomfort ( Fvalues were 22.018, 18.213, P<0.01). Conclusion:Rectal cancer patients with indwelling anal tube have serious difficulties in sitting position and getting in and out of bed. Corresponding measures should be taken according to the influencing factors and effective intervention methods should be used to improve the comfort of the patients.

RESUMEN

Objective To compare the value of gynecologic imaging reporting and data system (GI-RADS) and risk of malignancy index 4 (RMI 4) in differential diagnosis of benign and malignant ovarian neoplasms. Methods Data of 342 patients with ovarian neoplasms confirmed by pathology were analyzed retrospectively. Taking pathological results as gold standards, GI-RADS 1-4a as benign tumor, GI-RADS 4b-5 as malignant tumor, RMI 4=450 as critical points of benign and malignant ones, the diagnostic performance of GI-RADS and RMI 4 were calculated and compared. Results The sensitivity, specificity, positive predictive value, accuracy, AUC of GI-RADS and RMI 4 for differentiating benign and malignant ovarian neoplasms were 70.71% (70/99)and 53.54% (53/99), 98.77% (240/243) and 95.47% (232/243), 95.89% (70/73) and 82.81% (53/64), 90.64% (310/342) and 83.33% (285/342), 0.91 and 0.81, respectively (all P<0.05). Conclusion GI-RADS classification is better than RMI 4 in differentiating benign and malignant ovarian neoplasms.

RESUMEN

Objective To investigate the causes of a suspected outbreak of lower respiratory tract infection (LR-TI)caused by carbapenem-resistant Acinetobacter baumannii (CRAB)in the general intensive care unit(ICU)of a hospital, and provide scientific evidence for effective control of healthcare-associated infection(HAI).Methods Epi-demiological investigation on patients infected with CRAB and on-site monitoring on ICU environmental hygiene from March 4 to 23,2014 were performed, active prevention and control measures were taken. Results A total of 7 patients developed CRAB infection, the total length of stay during epidemic period were 160 days, the infection density of LRTI was 43.75‰(7/160),4 of 5 patients at No.02 bed had CRAB LRTI.Univariate analysis revealed that adopting conventional sputum suction procedure was a risk factor;the qualified rate of bacterial count on the object surfaces was 31 .75% (20/63).CRAB was also isolated from outer frame of suction device of No.02 bed, which had the same antimicrobial pattern as CRAB from 7 patients’sputum. After taking a series of controlling measures, there was no CRAB infection case after March 24,2014,this outbreak was effectively controlled. Conclusion This suspected HAI outbreak may be caused by inadequate disinfection of outer frame of suction device contaminated by CRAB,and horizontally transmitted through manipulation of sputum suction by health care workers.Clean and disinfection of ICU environmental object surfaces is of great importance for preventing HAI.

RESUMEN

Objective: To examine effects of Platycodon grandiflorum (JG), Glycyrrhiza uralensis Fisch (GC) or the compound of JG with GC on the growth of cariogenic and periodontopathic bacteria in vitro . Methods: JG,GC and the compound were extracted with ?=65% ethanol respectively. The minimal inhibitory concentration(MIC)and minimal bactericidal concentration (MBC) of JG, GC or compound against S.mutans MT818, S. sobrinus 6715, P.gingivalis 381 and B.forsythus 43037 were measured by drug sensetivity test. Results: JG showed no effect on the growth of oral pathogens tested;GC inhibited S.mutans MT 8418 and S.sobrinus 6715 with MIC of 3.91 mg/ml.The compound inhibited the microorganisms with the MIC of 1.96 mg/ml against S.mutans MT8148 or S.sobrinus 6715, 3.91 mg/ml against P.gingivalis 381, 7.81 mg/ml against B.forsythus 43037 respectively. Conclusion: The compound of JG and GC has stronger inhibition and bactericidal effects on oral pathogens than the single medicine.
