RESUMEN

Objective:To investigate the surgical outcome and prognostic factors of para-split laminotomy for removal of lumbar spinal canal tumors.Methods:Retrospectively review the clinical data of 35 patients suffering lumbar spinal canal tumors, who underwent the para-split laminotomy for tumor resection in Department of Neurosurgery, Clinical Medical College of Yangzhou University from October 2016 to August 2019, including 16 males and 19 females, and the age was(40.1±10.6)years. Intraoperative blood loss, operation time, tumor resection, tumor pathological results, perioperative complications were observed. Follow-up situations, including tumor recurrence, bony fusion of laminae and spinal stability. Follow-up using outpatient examination and telephone interview was performed by the end of August 2022. The JOA back pain scoring system was used to evaluate the neurological function of the spinal cord, and paired t-test were performed to compare the overall preoperative and postoperative spinal cord neurological function scores. Linear regression and multiple linear regression were used to analyze the prognostic factors. Measurement data of normal distribution were expressed as mean±standard deviation ( ± s), and the comparison before and after operation was performed by paired t-test. Mearsurement data of skewed distribution were expressed as M( Q1, Q3). Count data were expressed as cases. Results:The tumors of 35 patients were resected completely. The median blood loss was 100(75, 140)mL and the average operative duration was (181.1±42.7) min. The postoperative pathological results were as follows: 24 neurilemmomas, 6 meningiomas, 4 ependymomas and 1 neurofibroma. There were no surgery-related complications occurred. The postoperative follow-up ranged from 36 to 69 months, with no tumor recurrence or spinal instability, and bony fusion of laminae seen in some patients on CT imaging. The overall spinal cord neurological function scores of pre and post operation were(19.5±3.4)versus(25.4±2.2), Paired t-test analysis revealed a significant difference between the overall postoperative spinal cord neurological function scores and the preoperative scores, and the postoperative scores were better than the preoperative scores( P<0.05). Multiple linear regression analysis showed a positive correlation between preoperative JOA scores and postoperative JOA scores, and postoperative JOA scores has negative correlation with tumor volume and the age at the time of operation ( P<0.05). Conclusion:Para-split laminotomy with less damage to the posterior spinal structures can effectively improve the neurological function of the spinal cord and protect the stability of the lumbar spine in patients with lumbar spinal canal tumors, and the better the preoperative neurological function of the spinal cord, the better the prognosis of patients, and the smaller the tumor volume, the better the prognosis.

Objective To explore the differences of curative effect and short-term prognosis to severe traumatic brain injury patients with three different early postoperative nutritional supports.Methods A retrospective study was performed on 60 severe traumatic brain injury patients received in Neurosurgical Intensive Care Unit of Northern Jiangsu People's Hospital from July 2014 to July 2016.A total of 60 cases were randomly divided into the early enteral nutrition support therapy group,the early parenteral nutrition group,and the early compound nutrition group.The clinical indicators such as basic clinical characteristics before treatment,the nutrition data in two weeks,the length of stay in the Neurosurgical Intensive Care Unit,complications and GCS scores between the three groups were observed and analyzed.Results The indicators of early compound nutrition group were fasting blood-glucose (5.74 ± 0.64) mmol/L,prealbumin(203.80 ± 10.45) mg/L,total serum protein(61.99 ± 1.34) g/L,blood hemoglobin (114.53 ± 2.69) g/L,C-reactive protein(0.37 ± 0.06) mg/dl.The length of stay in Neurosurgical Intensive Care Unit was (11.6 ± 0.42) days in the compound nutrition group while those in the early enteral nutrition group was (13.20 ±0.42) days and those in the early parenteral nutrition group was(14.65 ± 0.42) days.The postoperative complications of the compound nutrition group were significantly lower than other two groups.The GCS scores of early compound nutrition group was(11.40 ± 1.60),which was the best in three groups.The differences were statistically significant (P < 0.05).Conclusions Early compound nutrition support has an exact curative effect on postoperative severe traumatic brain injury patients in Neurosurgical Intensive Care Unit.It can obviously improve the nutrition status of patients with less complications,shorter length of stay in Neurosurgical Intensive Care Unit,higher safety and lower degree of coma,worth clinical promotion.

Objective To investigate the value of bispectral index and intracranial pressure monitoring in evaluation of postoperative consciousness and short-term prognosis in patients with severe traumatic brain injury.Methods A simple random sampling method was used in 30 cases of brain trauma coma patients in the People's Hospital of Northern Jiangsu Province from January 2014 to December 2014.Glasgow coma scale (GCS) were evaluated at intervals of 8 hours,GCS were recorded for 3 days after surgery,The bispectral index value and intracranial pressure value are recorded at the same time.According to GCS,the patients were divided into two groups,group A (3≤GCS≤5) and group B(5 < GCS≤8).The 21-days natural survival rate was counted.The Kaplan-Meier Method and Log-rank test were used to analyze the relationship between Bispectral index and intracranial pressure.Results The bispectral index value of group A was 45.3 ± 3.8,the intracranial pressure value of group A was (18.6 ± 2.8) mmHg,The bispectral index value of group B was 32.2 ± 8.2,the intracranial pressure value of group B was (33.4 ±4.6) mmHg.The 21-days survival rate of group A was 55%,and 21-days survival rate of group B was 30%.The Spearman rank correlation method was used to analyze the values.Bispectral index value was positively correlated with coma degree after operation in patients with severe traumatic brain injury,and was negatively correlated with intracranial pressure value (r =0.532,P < 0.05;r =0.521,P < 0.05).The more severer the patient's craniocerebral injury.,the more severer the condition,the higher the intracranial pressure,the lower the bispectral index value,the worse the prognosis is.Conclusion Combined monitoring of bispectral index and intracranial pressure has high application value in evaluating the coma degree and prognosis of patients with severe craniocerebral injury after operation.

Objective:To explore the antipyretic effect on rabbit fever induced by endotoxin and the anti-inflammatory effect on the increase of mouse abdominal cavity capillary permeability caused by 0. 6% glacial acetic acid of Bazhong gardenia. Methods:The rab-bit fever model was made by ear margin vein injection of endotoxin in physiological saline solution, and the mouse inflammation model of abdominal cavity capillary permeability increase was established by 0. 6% glacial acetic acid solution. The animals were respectively intragastrically administrated with gardenia water-soluble effective parts and 70% alcohol-soluble effective parts. The rabbit body tem-perature and mouse celiac fluid absorbance values were detected. The effects of gardenia from different habitats were studied and com-pared. Results:The antipyretic effect of water-soluble effective parts from Bazhong gardenia extract was better than that of gardenia ex-tract from Jiangxi, Hubei and Jiangjin (P0. 05). Bazhong gardenia could inhibit the increase of abdominal blood capillary permeability in mouse inflammation model, and the effect was better than the other gardenia, however without significant difference (P>0. 05). Con-clusion:Bazhong gardenia shows better antipyretic effect compared with that from other habitats.

RESUMEN

The new edition of Pharmacology of Chinese Materia Medica has broken through the previous mode of the teaching materials. It classified traditional Chinese medicinal herbs into three categories: the herbs for radical cure depending on syndrome differentiation, the herbs for etiological treatment aimed at pathogenesis, and the herbs for symptomatic treatment, combined diseases differentiation with syndrome differentiation, showing active significance in enlightening, guiding the Chinese medicine theory of combining traditional Chinese and Western medicine's compiling, teaching, learning and research applications.

RESUMEN

<p><b>OBJECTIVE</b>To evaluate the clinical therapeutic effect of electric acupuncture (EA) at Sishencong (EX-HN 1) on insomnia.</p><p><b>METHODS</b>Two hundred and seventy-six patients were randomly assigned to 2 groups, 138 in each group, the EA group treated with EA at Sishencong, and the control group with oral administration of Tianmeng Capsule. The treatment course for both groups was 3 weeks. The quality and related parameters of sleep before and after treatment were evaluated with a multi-channel sleep detector.</p><p><b>RESULTS</b>After treatment, the quality of sleep was improved in both groups (P < 0.05), as compared with before treatment, the difference in related parameters was significant respectively (P<0.05 or P <0.01), however, the improvement in the EA group was superior to that in the control group (P < 0.01).</p><p><b>CONCLUSION</b>EA at Sishencong has obvious effect on insomnia.</p>

RESUMEN

<p><b>OBJECTIVE</b>To observe the protective effects of Denghuang injection on cerebral infarction in animal.</p><p><b>METHOD</b>The study evaluated the protective effects of Denghuang injection on cerebral infarction through 3 experiments: 1. ligating the arteria cerebri media of dog; 2. blocking the internal carotid artery of rat; 3. ligating the common carotiol artery of gerbil.</p><p><b>RESULT</b>The results showed Denghuang injection could reduce the cerebral infarction area after the dog's arteria cerebri media was ligated, and also could restrain the AKP&CK increase. The injection could reduce the cerebral infarction area of rat after the internal carotid artery was bloked by thrombus, and the mark of behavior and nerve symptom was better than that in the control group. Denghuang injection could obviously reduce the water content in pallium of gerbil after the common carotiol artery was ligated, and also could increase the amount of living pyramidal neuron in hippocampus, and reduce the MDA&LDH increase in pallium slurry.</p><p><b>CONCLUSION</b>Denghuang injection can obviously protect animal on cerebral infarction.</p>

RESUMEN

<p><b>OBJECTIVE</b>To investigate the protective effects of Huperzine A, a potent acetylcholinesterase inhibitor, against the hypoxic ischemic brain damage (HIBD) of the cognitive and morphology in the neonatal rats.</p><p><b>METHODS</b>Postnatal 7 days old rats were given vehicle or Huperzine A (0.05 mg/kg or 0.1 mg/kg, i.p.) following HIBD (unilateral carotid artery ligation followed by hypoxia) or sham operation, and then tested the learning ability and memory in the Morris water maze (MWM) from 36 to 40 postnatal days. The performance in MWM (escape latency, probe time) were recorded to evaluate the learning and memory dysfunction. At the end of MWM trials, the rats were decapitated and their brains were histologically analyzed. The tissue loss in different brain regions including striatum, cortex, and hippocampus were analyzed by image analysis system. The CA(1) subfield neurons numbers were counted to evaluate the brain damage. The acetylcholinesterase histochemistry staining was used to determine the activity of acetylcholinesterase in different brain regions.</p><p><b>RESULTS</b>Compared with sham-operated group, HIBD rats with the vehicle treatment displayed significant tissue losses in the hippocampus (including CA(1) neurons), cortex, and striatum, as well as severe spatial memory deficits (escape latency: 44 s vs 30 s, P < 0.05, probe time: 14 s vs 40 s, P < 0.01). Huperzine A treatment (0.1 mg/kg) resulted in significant protection against both HI-induced brain tissue losses and spatial memory impairments (mean escape latency: 34 s vs 44 s, P < 0.05, probe time: 35 s vs 14 s,P < 0.01). However, Huperzine A treatment (0.05 mg/kg) did not show any significant improvement of spatial memory impairments (mean escape latency: 45 s vs 44 s, P > 0.05, probe time: 17 s vs 14 s, P > 0.05), but moderate to severe brain tissue losses. There was a pronounced reduction of CA(1) neuron density in ipsilateral hemisphere of vehicle-treated group and 0.05 mg/kg Huperzine A group compared with contralateral hemisphere or ipsilateral hemisphere of sham-operated group and 0.1 mg/kg Huperzine A group (72 vs 232, P < 0.01, 72 vs 229, P < 0.01, respectively). There was a close linear correlation between the CA(1) neurons cell number and the mean escape latency for 5 d acquisition trials (r = 0.777, P < 0.01).</p><p><b>CONCLUSION</b>The unilateral HI brain injury in a neonatal rat model was associated with cognitive deficits, and that Huperzine A treatment may be protective against both brain injury and spatial memory impairment. Huperzine A showed a therapeutic potential for the treatment of hypoxic-ischemic encephalopathy (HIE) caused by the perinatal asphyxia.</p>

RESUMEN

Hypoxanthine,the pre_substance of uric acid,was used to create mouse model with hyperuricemia.The dose_effect relationship and time_effect relationship of hypoxanthine during the model establishment were observed.The effect of sex and xanthine oxidase inhibitor on the establishment of mouse model was studied.The results showed this method is effective for the establishment of a stable mouse model with hyperuricemia.

RESUMEN

Objective To investigate the pharmacodynamic action of Yunkang Capsule(YKC).Methods The animals were divided into YKC groups(at high-,moderate-and low-dosage respectively),diphenidol control group,model control group and blank control group.The action of YKC on vertigo and thrombosis,hemorrheologic indexes,piamatral microcirculation and free radicals of the animal models were observed.Results YKC could significantly prolong the latency of vertigo,reduce wet weight of thrombus,decrease blood viscosity,plasma viscosity and hematocrit,promote the dilation of piamatral micro-vessels,increase the amount of interweave microdot,increase plasma SOD activity and decline plasma MDA content of animal models.Conclusion YKC has actions of counteracting vertigo and thrombosis,improving hemorrheology and microcirculation,and clearing away free radicals.

RESUMEN

D (A:the amount of water added. B: the alcohol concentration in the fluidextract of herbs. C: decoction time, D: decoction times). The optimum decoction condition obtained was: adding water (12 times as much as medicine), decocting twice 1.5 hours each time, merging the filtrate, concentrating the merged filtrate into extract with a relative density of 1.20 (measured at 85℃), then adding alcohol slowly, and making the concentration of alcohol in the fluidextract come to 80%. Conclusion: The optimized process is stable and feasible.

AIM:To study the chemical constituents of Erigeron Injection by HPLC. METHODS: Under the guidance of HPLC analysis,two constituents were separated and purified by the aid of column chromatography with silica gel,and identified by UV、 IR、 MS、 NMR respectively and documentary data. RESULTS: Two compounds from semi-finished products of Erigeron Injection were identified as caffeic acid(Ⅰ) and chlorogenic acid(Ⅱ). CONCLUSION: Two main peaks in HPLC chromatogram of Erigeron Injection are Ⅰ and Ⅱ.

RESUMEN

OBJECTIVE: To optimize the preparation process of Xiangsha Liujun Granule. METHODS: By the orthogonal design. taking the rates of recovery of volatile oil, Ginsenoside Re and qualified granules as the indexes, we optimized the oil extracting conditions, the decocting conditions and the conditions of granulation by spayer. RESULTS: The optimum oil ex- tracting conditions for four kinds of herbs(Pericarpium Citri Reticulatae etc.) were that water was 30 times of herbs in weight, soaking time was 3 hours and extracting time 15 hours. The optimum decocting conditions for four kinds of herbs(Fuling etc.) were that water was 15 times of herbs in weight, soaking time was 3 hours and decocting time 4 hours, herbs were decocted twice. The optimum conditions of granulation by sprayer were that the fliud extract was 2 times of Dextrin in weight, the rel- ative density of the fliud extract was 1. 30(measured at 25℃), the voltage of sprayer was 120 volts, the period of shaking bag was 65 times(shaking bag back and forth twice in each period). CONCLUSION: The results of the experiment make it clear that the preparation process of Xiangsha Liu jun Granule is stable and feasible.
