Screener, a board game supplemented with online resources, was introduced and distributed by the Brazilian Society of Pharmacology and Experimental Therapeutics to postgraduate programs as an instructional tool for the process of drug discovery and development (DDD). In this study, we provided a comprehensive analysis of five critical aspects for evaluating the quality of educational games, namely: 1) description of the intervention; 2) underlying pedagogical theory; 3) identification of local educational gaps; 4) impact on diverse stakeholders; and 5) elucidation of iterative quality enhancement processes. We also present qualitative and quantitative assessments of the effectiveness of this game in 11 postgraduate courses. We employed the MEEGA+ online survey, comprising thirty-three close-ended unipolar items with 5-point Likert-type response scales, to assess student perceptions of the quality and utility of Screener. Based on 115 responses, the results indicated a highly positive outlook among students. In addition, we performed a preliminary evaluation of learning outcomes in two courses involving 28 students. Pre- and post-quizzes were applied, each consisting of 20 True/False questions directly aligned with the game's content. The analysis revealed significant improvement in students' performance following engagement with the game, with scores rising from 8.4 to 13.3 (P<0.0001, paired t-test) and 9.7 to 12.7 (P<0.0001, paired t-test). These findings underscore the utility of Screener as an enjoyable and effective tool for facilitating a positive learning experience in the DDD process. Notably, the game can also reduce the educational disparities across different regions of our continental country.

Canine visceral leishmaniasis (CVL) caused by Leishmania (Leishmania) infantum is transmitted by phlebotomine sandflies and a major zoonotic disease in Brazil. Due to the southward expansion of the disease within the country and the central role of dogs as urban reservoirs of the parasite, we have investigated the occurrence of CVL in two municipalities Erval Velho and Herval d'Oeste in the Midwest region of Santa Catarina state. Peripheral blood samples from 126 dogs were collected in both cities and tested for anti-L. infantum antibodies by indirect enzyme linked immunosorbent assay (ELISA) and indirect immunofluorescence reaction (IIF) and for the presence of parasite DNA by polymerase chain reaction (PCR) in peripheral blood. From examined dogs, 35.71% (45/126) were positive for at least one of the three tests and two (1.6%) were positive in all performed tests. Twelve dogs (9.5%) were positive for both ELISA and IIF, while 21 dogs were exclusively positive for ELISA (16.7%), and 15 (11.9%) for IIF. L. infantum k-DNA was detected by PCR in 9 out of 126 dogs (7.1%) and clinical symptoms compatible with CVL were observed for 6 dogs. Taken together, these results indicate the transmission of CVL in this region, highlighting the needs for epidemiological surveillance and implementation of control measures for CVL transmission in this region.

Maternal anxiety symptoms in the perinatal period might have long-term health effects on both the mother and the developing child. Valerian is a phytotherapeutic agent that is widely used for the treatment of anxiety. This study investigated the effects of valerian treatment in postpartum rats on maternal care, toxicity, and milk composition. Postnatal development, memory, and anxiety behavior in the offspring were also assessed. Postpartum Wistar rats received the valerian (500, 1000, or 2000 mg·kg-1·day-1) by oral gavage. Clinical and biochemical toxicity was evaluated with commercial kits. Maternal behavior was observed daily. Milk composition was analyzed by colorimetric methods. Physical and neuromotor tests were used to analyze postnatal development. Anxiolytic activity was assessed by the elevated plus maze, and memory was evaluated by the step-down inhibitory avoidance task. Maternal toxicity and care behavior were not altered by the treatment, while only the highest dose promoted a significant increase of lactose, and the doses 1000 and 2000 mg·kg-1·day-1 promoted a reduction of protein contents in milk. Postnatal development was similar in all offspring. Adult offspring did not display altered anxiety behavior, while long-term memory was impaired in the female adult offspring by maternal treatment with 1000 mg·kg-1·day-1. These results suggested that high doses of valerian had significant effects on important maternal milk components and can cause long-term alterations of offspring memory; thus, treatment with high doses of valerian is not safe for breastfeeding Wistar rat mothers.

Abstract It is a fact that the regions that cultivate the most maize crop do not have fully adequate technologies to measure productivity losses caused by irregularities in water availability. The objective of this study was to evaluate the physiological characteristics of maize hybrids tolerant (DKB 390) and sensitive (BRS 1030) to drought, at V5 growth stage and under water restriction, in order to understand the mechanisms involved in the induction of tolerance to drought by chitosan in contrasting maize genotypes. Plants were cultivated in pots at a greenhouse, and chitosan 100 ppm was applied by leaf spraying. The water restriction was imposed for 10 days and then leaf gaseous exchange and chlorophyll fluorescence were evaluated. The tolerant hybrid (DKB 390) showed higher photosynthesis, stomatal conductance, carboxylation efficiency, electron transport rate, and non-photochemical quenching when chitosan was used. Plants from tolerant genotype treated with chitosan were more tolerant to water stress because there were more responsive to the biopolymer.


Resumo As regiões que cultivam milho como cultura principal ainda não possuem tecnologias adequadas para mensurar as perdas na produtividade decorrentes na disponibilidade irregular de água. O objetivo desse estudo foi avaliar as características fisiológicas de híbridos de milho tolerante (DKB 390) e sensível (BRS1030) à seca, no estádio de crescimento V5 e sob restrição hídrica, para compreender os mecanismos envolvidos na indução de tolerância à seca pela quitosana em genótipos contrastantes. As plantas foram cultivadas vasos na casa de vegetação e a quitosana 100 ppm foi aplicada por pulverização foliar. A restrição hídrica durou 10 dias e foram avaliadas as trocas gasosas e a fluorescência da clorofila. O híbrido tolerante (DKB 390) apresentou maior fotossíntese, condutância estomática, eficiência de carboxilação, taxa de transporte de elétrons e quenching não fotoquímico quando aplicada a quitosana. As plantas do genótipo tolerante tratadas com quitosana foram mais tolerantes ao déficit hídrico porque foram mais responsivas ao biopolímero.

Abstract Passion fruits species presents a larger economic importance for in natura consumption and industry. The main way to propagate these species is by seeds; however, the aril presence and environmental conditions may inhibit the seeds germination. In this sense, the objective of this study was to evaluate the effect of aril removal methods and light conditions on P. edulis var. flavicarpa seeds quality and germination. There were used five methods to remove the seeds aril: (1) friction on steel mesh sieve for 5 minutes; (2) friction with coarse sand on steel mesh sieve for 5 minutes; (3) immersion in lime solution at 10% (w/v), for 30 minutes; (4) fermentation in sugar solution at 10% (w/v), during 48 hours; and (5) natural pulp fermentation, during five days. The seeds were submitted to germination test at darkness and lightness, as well as, to length seedlings and emergence test. Regardless the aril removal method, the germination percentage was lower at lightness than darkness highlighting the natural pulp fermentation treatment, which presented 86% of seeds germinated. On the other hand, the aril removal methods did not affect the seedlings emergence, with emergence percentages ranged from 88 to 94%.


Resumo As espécies de maracujá apresentam grande importância econômica, tanto para o consumo in natura, quanto para a indústria. A principal maneira de propagar essas espécies é por sementes; contudo, a presença de arilo e condições ambientais pode inibir a germinação. Nesse sentido, objetivou-se estudar o efeito de métodos de remoção do arilo e luminosidade na qualidade e germinação das sementes de Passiflora edulis var. flavicarpa. Cinco métodos foram empregados para a remoção do arilo: (1) fricção em peneira de malha de aço por cinco minutos; (2) fricção com areia grossa em peneira de malha de aço por cinco minutos; (3) imersão em solução de cal a 10% (m/v), por 30 minutos; (4) fermentação da polpa em solução de açúcar a 10% (m/v), por 48 horas; e (5) fermentação natural da polpa, por cinco dias. As sementes foram submetidas ao teste de germinação no escuro e no claro, teste de comprimento de plântulas e teste de emergência. Independente do método de remoção do arilo, a porcentagem de germinação no claro apresentou-se menor em comparação ao escuro, com destaque para o tratamento de fermentação natural da polpa, o qual apresentou 86% de sementes germinadas. Por outro lado, os métodos de remoção do arilo não afetaram a emergência das plântulas, com porcentagem de emergência variando entre 88 a 94%.

Esta pesquisa foi realizada com o objetivo de se conhecer a variabilidade genética de 12 loci de microssatélites em galinhas crioulas Canela-Preta. Foram coletadas amostras de sangue de 118 galinhas crioulas Canela-Preta, provenientes de três municípios do estado do Piauí (Oeiras, Queimada Nova e Teresina). Após extração do DNA, foram utilizados marcadores para 12 loci de microssatélites: LEI0192, LEI0209, LEI0212, LEI0217, LEI0221, LEI0234, LEI0237, LEI0248, LEI0258, MCW0081, MCW0183 e MCW0213, que foram amplificados pela técnica de reação em cadeia da polimerase (PCR). Foram obtidos 408 alelos (somando os alelos dos 12 loci), com os fragmentos variando entre 50 e 460 pares de bases. O número de alelos variou de 15 (MCW0081) a 52 (LEI0212), com média de 31,5 alelos por locus. A média de heterozigosidade esperada e o conteúdo de informações polimórficas foram, respectivamente, 0,887 e 0,909. Foram observados desvios no equilíbrio de Hardy-Weinberg e valores positivos do índice de fixação com excesso de homozigotos. Os microssatélites utilizados mostraram-se polimórficos e podem ser usados para investigações genéticas em galinhas Canela-Preta. As galinhas dos plantéis avaliados apresentam grande variabilidade gênica, o que as qualifica como importante fonte de recursos genéticos e, consequentemente, faculta a utilização delas em programas de melhoramento genético animal.(AU)


The aim of this study was to analyze the genetic variability of twelve microsatellite loci in native Canela-Preta chickens. Blood samples were collected from 118 chickens of the breed from five properties in three cities (Oeiras, Queimada Nova and Teresina) of Piauí state. After the DNA extraction, markers were used for twelve microsatellite loci: LEI0192, LEI0209, LEI0212, LEI0217, LEI0221, LEI0234, LEI0237, LEI0248, LEI0258, MCW0081, MCW0183, and MCW0213 that were amplified by polymerase chain reaction technique (PCR). The results showed a total of 408 alleles (adding alleles from the 12 loci) with the fragments ranging between 50 and 460 base pairs, the number of alleles ranged from 15 (MCW0081) to 52 (LEI0212) with an average of 31,5 alleles per locus. The average expected heterozygosity and PIC were, respectively, 0.887 and 0.909. Deviations were observed in the Hardy-Weinberg equilibrium and positive values of the fixation index with excess of homozygotes. It is concluded that the used microsatellites are polymorphic and can, therefore, be used for genetic research in Canela-Preta chickens. The birds of the analyzed cores present great genetic variability, which qualifies them as an important source of genetic resources, which could be used for future animal breeding programs.(AU)

Abstract Several experimental surveys were carried out in the Northeastern Atlantic, Madeira archipelago from 1991 to 2008 to explore new fisheries resources. This study examined the selectivity of bottom and floating traps and the analysis of yield-per-recruit (YPR) and biomass-per-recruit (BPR) providing helpful insight to the management of the shrimp Plesionika narval. A total of 28,262 specimens were sampled and the analysis of length at first capture returned higher values when using floating traps indicating that these traps are more selective, exerting less pressure on the resource. The YPR and BPR analysis showed that the stock is under exploited for the studied area and suggests that the use of floating traps in the commercial fisheries of P. narval is recommended, which will allow a higher maximum allowable limit of exploitation and greater yield. The results suggest that P. narval has the potential to support a viable and sustainable fishery using floating traps.


Resumo Vários cruzeiros de investigação foram realizados no Atlântico Nordeste, arquipélago da Madeira entre 1991 e 2008 visando a exploração de novos recursos pesqueiros. O presente estudo contemplou a comparação da seletividade de pesca entre armadilhas de fundo e flutuantes e a análise de rendimento por recruta (YPR) e biomassa por recruta (BPR) facultando informação essencial para a gestão do camarão Plesionika narval. Um total de 28,262 especimens foram amostrados e a análise de comprimento de primeira captura retornou valores mais elevados aquando da utilização de armadilhas flutuantes, indicando que essas armadilhas são mais seletivas e exercem menos pressão sobre o recurso. A análise YPR e BPR mostrou que o estoque se encontra sob explorado para a área de estudo e recomenda a utilização de armadilhas flutuantes na pesca comercial de P. narval, que permitirão um limite máximo de exploração com rendimento superior. Os resultados sugerem que P. narval tem o potencial para suportar uma pescaria viável e sustentável mediante a utilização de armadilhas flutuantes.

Abstract It is a fact that the regions that cultivate the most maize crop do not have fully adequate technologies to measure productivity losses caused by irregularities in water availability. The objective of this study was to evaluate the physiological characteristics of maize hybrids tolerant (DKB 390) and sensitive (BRS 1030) to drought, at V5 growth stage and under water restriction, in order to understand the mechanisms involved in the induction of tolerance to drought by chitosan in contrasting maize genotypes. Plants were cultivated in pots at a greenhouse, and chitosan 100 ppm was applied by leaf spraying. The water restriction was imposed for 10 days and then leaf gaseous exchange and chlorophyll fluorescence were evaluated. The tolerant hybrid (DKB 390) showed higher photosynthesis, stomatal conductance, carboxylation efficiency, electron transport rate, and non-photochemical quenching when chitosan was used. Plants from tolerant genotype treated with chitosan were more tolerant to water stress because there were more responsive to the biopolymer.


Resumo As regiões que cultivam milho como cultura principal ainda não possuem tecnologias adequadas para mensurar as perdas na produtividade decorrentes na disponibilidade irregular de água. O objetivo desse estudo foi avaliar as características fisiológicas de híbridos de milho tolerante (DKB 390) e sensível (BRS1030) à seca, no estádio de crescimento V5 e sob restrição hídrica, para compreender os mecanismos envolvidos na indução de tolerância à seca pela quitosana em genótipos contrastantes. As plantas foram cultivadas vasos na casa de vegetação e a quitosana 100 ppm foi aplicada por pulverização foliar. A restrição hídrica durou 10 dias e foram avaliadas as trocas gasosas e a fluorescência da clorofila. O híbrido tolerante (DKB 390) apresentou maior fotossíntese, condutância estomática, eficiência de carboxilação, taxa de transporte de elétrons e quenching não fotoquímico quando aplicada a quitosana. As plantas do genótipo tolerante tratadas com quitosana foram mais tolerantes ao déficit hídrico porque foram mais responsivas ao biopolímero.

Aiming to contribute to the development of alternative control methods of the coffee leaf miner, Leucoptera coffeella (Guérin-Mèneville & Perrottet, 1842) (Lepidoptera: Lyonetiidae), a search for plants able to produce active substances against this insect was carried out, with species collected during different periods of time in the Alto Rio Grande region, (Lavras, Minas Gerais, Brazil). Coffee leaves containing L. coffeella mines were joined with 106 extracts from 77 plant species and, after 48 hours, the dead and alive caterpillars were counted. The extracts from Achillea millefolium, Citrus limon, Glechoma hederacea, Malva sylvestris, Mangifera indica, Mentha spicata, Mirabilis jalapa, Musa sapientum, Ocimum basiculum, Petiveria alliaceae, Porophyllum ruderale, Psidium guajava, Rosmarinus officinalis, Roupala montana, Sambucus nigra and Tropaeolum majus showed the highest mortality rates.


Visando contribuir para o desenvolvimento de métodos alternativos de controle do bicho-mineiro-do-cafeeiro, Leucoptera coffeella (Lepidoptera: Lyonetiidae), buscou-se selecionar plantas coletadas em diferentes épocas na região do Alto Rio Grande, (Lavras, Minas Gerais, Brasil) que contenham substâncias ativas contra este inseto. Folhas de cafeeiro com minas intactas de L. coffeella foram colocadas em contato com 106 extratos provenientes de 78 espécies vegetais e, após 48 horas, contaram-se as lagartas vivas e mortas. Os extratos de Achillea millefolium, Citrus limon, Glechoma hederacea, Malva sylvestris, Mangifera indica, Mentha spicata, Mirabilis jalapa, Musa sapientum, Ocimum basiculum, Petiveria alliaceae, Porophyllum ruderale, Psidium guajava, Rosmarinus officinalis, Roupala montana, Sambucus nigra e Tropaeolum majus, provocaram os maiores índices de mortalidade.

During thyroid tumor progression, cellular de-differentiation may occur and it is commonly accompanied by metastatic spread and loss of iodine uptake. Retinoic acid (RA) administration might increase iodine uptake in about 40 percent of patients, suggesting that RA could be a promising therapeutic option for radioiodine non-responsive thyroid carcinoma, although a prospective study with a long-term follow-up has not been reported. This was a clinical prospective study assessing the value of 13-cis-RA in patients with advanced thyroid carcinoma and its impact on major outcomes such as tumor regression and cancer-related death with a long-term follow-up of patients submitted to radioiodine (131I) therapy after RA administration. Sixteen patients with inoperable disease and no significant radioiodine uptake on post-therapy scan were selected. Patients were treated orally with 13-cis-RA at a dose of 1.0 to 1.5 mg·kg-1·day-1 for 5 weeks and then submitted to radioiodine therapy (150 mCi) after thyroxine withdrawal. A whole body scan was obtained 5 to 7 days after the radioactive iodine therapy. RECIST criteria were used to evaluate the response. An objective partial response rate was observed in 18.8 percent, a stable disease rate in 25 percent and a progression disease rate in 56.2 percent. Five patients died (62.5 percent) in the group classified as progression of disease. Progression-free survival rate (PFS) ranged from 72 to 12 months, with a median PFS of 26.5 months. RA may be an option for advanced de-differentiated thyroid cancer, due to the low rate of side effects.

O objetivo deste estudo foi avaliar a característica reológica (espalhabilidade) e quantificar os princípios ativos (flavonoides) de preparações semissólidas (pomadas) contendo extratos de Calendula officinalis L. (Asteraceae). A partir da droga vegetal dessa espécie, foram obtidos dois tipos de extratos preparados por percolação na proporção de 1:5 utilizando-se mistura hidroalcóolica (etanol:água 62:38) e hidroglicólica (propilenoglicol:água 9:1). As pomadas contendo esses extratos, além de excipientes lanolina e vaselina, foram preparadas por duas técnicas distintas: solução (a quente) e suspensão (a frio). Análises para determinar a espalhabilidade através das relações entre área, esforço limite e teor de flavonoides foram realizadas a partir de amostras das pomadas preparadas. Os resultados demonstraram que as pomadas contendo extratos hidroalcoólicos e hidroglicólicos, preparadas por suspensão (a frio), exibiram as maiores relações entre área e esforço limite (0,34 e 0,33 mm2 g-1, respectivamente). No entanto, as pomadas preparadas com extratos hidroalcoólicos por suspensão resultaram nos maiores valores de flavonoides totais (0,004% m/m calculados como hiperosídeos).


The aim of this study was to assess the rheological properties (spreadability) and assay the active ingredients (flavonoids) in semi-solid preparations (ointments) containing extracts of marigold (Calendula officinalis L. - Asteraceae). The herbal drug from this species was separated from the dried inflorescence and two types of extracts were prepared by percolation at a ratio of 1:5, one with an ethanol:water mixture (62:38) and the other with propylene glycol: water (9:1). Ointments were prepared from these extracts, lanolin and vaseline, by two different techniques: solution (hot) and suspension (cold). Samples of each ointment were analyzed, to determine the spreadability, as the ratio between area of spread and limiting applied weight, and the flavonoid content. The results showed that ointments prepared by cold suspension, containing hydroalcoholic and hydroglycolic extract, exhibited the highest area:limiting weight ratios (0.34 and 0.33 mm2 g-1 , respectively). Secondly, ointments prepared by suspension, with hydroalcoholic extracts, had the highest total flavonoid content (0.004% w/w, calculated as hyperoside).

The main objectives of the present study were to investigate the clinical and laboratory features of meningococcal disease in the city of Rio de Janeiro, Brazil, during the overlap of 2 epidemics in the 1990s. We conducted a study of a series of cases of meningococcal disease admitted in a Meningitis Reference Hospital. All clinical isolates available were analyzed by means of microbiological epidemiological markers. In 1990, Neisseria meningitidis serogroup B:4,7:P1.19,15, 1.7,1 sulfadiazine-resistant of the ET-5 complex emerged causing epidemic disease. Despite mass vaccination campaign (VaMengoc B+C®), the ET-5 clone remained hyperendemic after the epidemic peaked. In 1993 to 1995, an epidemic of serogroup C belonged to the cluster A4 overlapped, with a significant shift in the age distribution toward older age groups and an increase of sepsis. Serogroup C epidemics are a recurrent problem in Rio de Janeiro, which can be hindered with the introduction of a conjugate vaccine. We hope the data presented here brings useful information to discuss vaccines strategies and early management of suspected cases.

The aim of this work was to evaluate the antibacterial activity of Brazilian plants extracts against fish pathogenic bacteria. Forty six methanolic extracts were screened to identify their antibacterial properties against Streptococcus agalactiae, Flavobacterium columnare and Aeromonas hydrophila. Thirty one extracts showed antibacterial activity.


O objetivo deste trabalho foi avaliar a atividade antibacteriana de extratos de plantas brasileiras contra bactérias patogênicas para peixes. A atividade antibacteriana de quarenta e seis extratos metanólicos de plantas foi avaliada contra os agentes Streptococcus agalactiae, Flavobacterium columnare e Aeromonas hydrophila. Trinta e um extratos apresentaram atividade antibacteriana.

Insulin receptor substrate-1 (IRS-1) is the main intracellular substrate for both insulin and insulin-like growth factor I (IGF-I) receptors and is critical for cell mitogenesis. Thyrotropin is able to induce thyroid cell proliferation through the cyclic AMP intracellular cascade; however, the presence of either insulin or IGF-I is required for the mitogenic effect of thyroid-stimulating hormone (TSH) to occur. The aim of the present study was to determine whether thyroid IRS-1 content is modulated by TSH in vivo. Strikingly, hypothyroid goitrous rats, which have chronically high serum TSH levels (control, C = 2.31 ± 0.28; methimazole (MMI) 21d = 51.02 ± 6.02 ng/mL, N = 12 rats), when treated with 0.03 percent MMI in drinking water for 21 days, showed significantly reduced thyroid IRS-1 mRNA content. Since goiter was already established in these animals by MMI for 21 days, we also evaluated IRS-1 expression during goitrogenesis. Animals treated with MMI for different periods of time showed a progressive increase in thyroid weight (C = 22.18 ± 1.21; MMI 5d = 32.83 ± 1.48; MMI 7d = 31.1 ± 3.25; MMI 10d = 33.8 ± 1.25; MMI 14d = 45.5 ± 2.56; MMI 18d = 53.0 ± 3.01; MMI 21d = 61.9 ± 3.92 mg, N = 9-15 animals per group) and serum TSH levels (C = 1.57 ± 0.2; MMI 5d = 9.95 ± 0.74; MMI 7d = 10.38 ± 0.84; MMI 10d = 17.72 ± 1.47; MMI 14d = 25.65 ± 1.23; MMI 18d = 35.38 ± 3.69; MMI 21d = 31.3 ± 2.7 ng/mL, N = 9-15 animals per group). Thyroid IRS-1 mRNA expression increased progressively during goitrogenesis, being significantly higher by the 14th day of MMI treatment, and then started to decline, reaching the lowest values by the 21st day, when a significant reduction was detected. In the liver of these animals, however, a significant decrease of IRS-1 mRNA was detected after 14 days of MMI treatment, a mechanism probably involved in the insulin resistance that occurs in hypothyroidism. We transplanted 47 patients with Fanconi anemia using an alternative source of hematopoietic cells. The patients were assigned to the following groups: group 1, unrelated bone marrow (N = 15); group 2, unrelated cord blood (N = 17), and group 3, related non-sibling bone marrow (N = 15). Twenty-four patients (51 percent) had complete engraftment, which was not influenced by gender (P = 0.87), age (P = 0.45), dose of cyclophosphamide (P = 0.80), nucleated cell dose infused (P = 0.60), or use of anti-T serotherapy (P = 0.20). Favorable factors for superior engraftment were full HLA compatibility (independent of the source of cells; P = 0.007) and use of a fludarabine-based conditioning regimen (P = 0.046). Unfavorable factors were > or = 25 transfusions pre-transplant (P = 0.011) and degree of HLA disparity (P = 0.007). Intensity of mucositis (P = 0.50) and use of androgen prior to transplant had no influence on survival (P = 0.80). Acute graft-versus-host disease (GVHD) grade II-IV and chronic GVHD were diagnosed in 47 and 23 percent of available patients, respectively, and infections prevailed as the main cause of death, associated or not with GVHD. Eighteen patients are alive, the Kaplan-Meyer overall survival is 38 percent at ~8 years, and the best results were obtained with related non-sibling bone marrow patients. Three recommendations emerged from the present study: fludarabine as part of conditioning, transplant in patients with <25 transfusions and avoidance of HLA disparity. In addition, an extended family search (even when consanguinity is not present) seeking for a related non-sibling donor is highly recommended.

Quadriplegic subjects present extensive muscle mass paralysis which is responsible for the dramatic decrease in bone mass, increasing the risk of bone fractures. There has been much effort to find an efficient treatment to prevent or reverse this significant bone loss. We used 21 male subjects, mean age 31.95 ± 8.01 years, with chronic quadriplegia, between C4 and C8, to evaluate the effect of treadmill gait training using neuromuscular electrical stimulation, with 30-50 percent weight relief, on bone mass, comparing individual dual-energy X-ray absorptiometry responses and biochemical markers of bone metabolism. Subjects were divided into gait (N = 11) and control (N = 10) groups. The gait group underwent gait training for 6 months, twice a week, for 20 min, while the control group did not perform gait. Bone mineral density (BMD) of lumbar spine, femoral neck, trochanteric area, and total femur, and biochemical markers (osteocalcin, bone alkaline phosphatase, pyridinoline, and deoxypyridinoline) were measured at the beginning of the study and 6 months later. In the gait group, 81.8 percent of the subjects presented a significant increase in bone formation and 66.7 percent also presented a significant decrease of bone resorption markers, whereas 30 percent of the controls did not present any change in markers and 20 percent presented an increase in bone formation. Marker results did not always agree with BMD data. Indeed, many individuals with increased bone formation presented a decrease in BMD. Most individuals in the gait group presented an increase in bone formation markers and a decrease in bone resorption markers, suggesting that gait training, even with 30-50 percent body weight support, was efficient in improving the bone mass of chronic quadriplegics.

Blood pressure pattern was analyzed in 12 complete quadriplegics with chronic lesions after three months of treadmill gait training. Before training, blood pressure values were obtained at rest, during treadmill walking and during the recovery phase. Gait training was performed for 20 min twice a week for three months. Treadmill gait was achieved using neuromuscular electrical stimulation, assisted by partial body weight relief (30-50 percent). After training, blood pressure was evaluated at rest, during gait and during recovery phase. Before and after training, mean systolic blood pressures and heart rates increased significantly during gait compared to rest (94.16 ± 5.15 to 105 ± 5.22 mmHg and 74.27 ± 10.09 to 106.23 ± 17.31 bpm, respectively), and blood pressure decreased significantly in the recovery phase (86.66 ± 9.84 and 57.5 ± 8.66 mmHg, respectively). After three months of training, systolic blood pressure became higher at rest (94.16 ± 5.15 mmHg before training and 100 ± 8.52 mmHg after training; P < 0.05) and during gait exercise (105 ± 5.22 mmHg before and 110 ± 7.38 mmHg after training; P < 0.05) when compared to the initial values, with no changes in heart rate. No changes occurred in blood pressure during the recovery phase, with the lower values being maintained. A drop in systolic pressure from 105 ± 5.22 to 86.66 ± 9.84 mmHg before training and from 110 ± 7.38 to 90 ± 7.38 mmHg after training was noticed immediately after exercise, thus resulting in hypotensive symptoms when chronic quadriplegics reach the sitting position from the upright position.
