RESUMEN

Objective:To investigate the feasibility of size-specific dose estimate of head based on pediatric patients′ age.Methods:A retrospective study was conducted on plain head CT scans of 210 patients in Tianjin Children′s Hospital from January to March in 2022. The volume CT dose index (CTDI vol) values, age and sex of the patients were recorded. The anteroposterior diameter (AP) and lateral diameter(LAT), aera and CT value in the middle slice of the scan region were measured on the workspace. According to the AAPM reports 220 and 293, the water equivalent diameter ( DW) of head, size-specific conversion factors ( fmeasuredH16), and size-specific dose estimate (SSDE measured) were calculated. The effective diameter ( Deff; WHO) was calculated for male and female individuals using the ralationship between the standard head circumference and age of children provided by World Health Organization (WHO). The effective diameter ( Deff; WHO) was converted into a water equivalent diameter ( DW; WHO), using a correlation established from the Deff and DWwhich were measured in 210 image sets of male and female patients. Then the corresponding fWHOH16 and SSDE WHO of head can be calculated. The correlation between DW and DW; WHO, fmeasuredH16 and fWHOH16, the correlation and consistency between SSDE measured and SSDE WHO, and the conversion relationship between age and fWHOH16 were analyzed for male and female patients. Results:A significant correlation were found between the measured DW and DW; WHO(female: r=0.917, P<0.01; male: r=0.873, P<0.01), fmeasuredH16 and fWHOH16(female: r=0.916, P<0.01; male: r=0.883, P<0.01), SSDE measured and SSDE WHO( r=0.991, P<0.01; male: r=0.992, P<0.01). The Bland-Altman test indicated that there was good coherence between SSDE measured and SSDE WHO. Maximal mean root-mean-square errors of SSDE measured and SSDE WHO were 5.61% in female and 5.25% in male. The conversion relationship between fWHOH16 and patient age in different genders was obtained by curve fitting. Conclusion:The SSDE of head in pediatric patients can be estimated simply and accurately by directly using the patient′s age.

Pulmonary infection is a common and frequent disease in children, which is caused by viruses, atypical pathogens, and mostly bacteria, seriously threatening children′s health.Bacteria are divided into Gram-positive bacteria and Gram-negative bacteria.The former mainly includes Streptococcus pneumoniae, Staphylococcus aureus, Streptococcus pyogenes, etc.In this article, pediatric chest imaging techniques and 4 typical imaging features of Gram-positive cocci pneumonia in children were summarized aiming providing imaging information for clinical diagnosis, assessment of efficacy and prognosis of Gram-positive cocci pneumonia.

RESUMEN

Objective To analyze the imaging features of Menkes disease in children, and in order to identify the diagnostic value and improve the knowledge of it. Methods The imaging findings of 9 Menkes disease patients confirmed by the clinical and laboratory examinations were retrospectively analyzed. All the patients were male aged from 2.0 to 7.0 months with a median age of 3.0 months. The clinical manifestations including seizures, psychomotor retardation, dystonia accompanied with light complexion and curly hair. The laboratory examinations showed low serum copper and ceruloplasmin levels. All the patients were performed with plain MR scan and MR angiography, 7 cases were performed with chest X?ray examinations and 4 cases were performed with bone X?ray examinations and 1 were performed with hand X?ray examinations. Results The plain MR scan showed white matter lesions in 8 cases, abnormality signal intensity of basal ganglia in 4 cases, subdural effusion in 2 cases. 8 cases had cerebral and cerebellar atrophy at different degrees. The MR angiography showed abnormal tortuosity of the main intracranial arteries. The chest X?ray of 5 in 7 cases showed that the front end of bilateral ribs were spherical and enlarged. 4 cases were performed with bone X?ray presenting, 3 out of 4 cases showed the widened metaphysis of tibia, fibula, femoral, humerus ulna and radius with the shape like cup?mouth and the formation of bony spur locally. The X ray examination of 1 case with connecting palm showed symphysodactylia. Conclusion Brain MR, X?ray of chest and bone showed imaging features of Menkes disease. MR angiography finding was the most distinctive feature, which has important value in the diagnosis of Menkes disease.

Objective To develop the finite element model of six-year-old child occupant lower extremity with higher biofidelity and validate the model of knee joints,as well as analyze the biomechanical responses of growth plate under frontal impact load and injury mechanisms of the knee joint by using this model.Methods The sixyear-old child occupant lower extremity with growth plate was modeled based on children's anatomy and CT images,and corresponding material properties of the lower extremity model were assigned.The model was validated according to biomechanical experiments by Kerrigan et aL and Haut et aL and then was used to analyze the injury results of growth plate with different material properties.Results The model validation was qualified by comparing the curves from the experimental and simulation results.The growth plates at knee regions could change injury patterns of the child occupant lower extremity fracture.The material properties of growth plate could affect threshold of axial damage of the femur as well as relative position of the fracture.Conclusions The validated model can be used for related study and application on biomechanical responses and injury mechanisms of sixyear-old child occupant lower extremities.

Objective To evaluate 3D CT features of normal anatomy,anatomic variations and fractures of occipital squama.Methods The 3D CT features on MIP,VR images were analyzed retrospectively in 589 pediatric cases.The normal anatomy,anatomic variations and fractures of occipital squama were observed respectively,and the differential diagnostic features including the individual location,appearance and extension were analyzed.Results Four hundred and thirty-three patients (75.2%) showed normal anatomy,including 154 patients with adult occipital anatomical features,279 patients with posterior intraoccipital synchondrosis,and 37 patients with Kerckring-supraoccipital synchondrosis.When cases with recent trauma history were excluded,113 patients (19.1%) showed anatomic variants,including unpenetrating sutures and penetrating sutures.The former could be subdivided to Mendosal sutures in 23 cases,superior median fissures in 19 cases,and midline supraoccipital fissures in 4 cases,while the latter could be subdivided to the interparietal bone variations in 54 cases,wormian bones in 23 cases,and accessory bones in 7 cases.Two or more variations coexisted in 33 cases.The occipital squama fractures were shown in 34 cases (5.6%),including linear fractures in 27 cases,comminuted fractures in 3 cases,with depression fracture in one case,separation of cranial sutures in 3 cases,and other fractures associated with variants in 3 cases.The fractures were sharp,or jagged,without limitation of the occification.Conclusion There are different 3D CT features of normal anatomy,anatomic variations and fractures of occipital squama in children,which are important for making the accurate diagnosis.

ObjectiveTo explore the feasibility,efficacy and cosmetic effect of three-dimensional conformal external beam partial breast irradiation (EB-PBI) after breast-conserving surgery for the selected Chinese early stage breast cancer patients.MethodsFrom June 2003 to December 2010,Forty-four early stage breast cancer patients underwent EB-PBI after breast-conserving surgery.Twenty patients had CT simulation scan in moderate deep inspiration breathing hold,and twenty-four patients in free breathing.EB-PBI was planned and delivered by three-dimensional conformal radiotherapy (3DCRT)with four noncoplanar beams.The prescribed dose was 3.40 Gy per fraction in thirty-nine patients and 3.85 Gy per fraction in five patients,twice per day at an interval of at least six hours,in five consecutive days.Results The number of patients with follow up time of 2,3 and 5 years were 39,31 and 16.Grade 1 acute radiationinduced dermatitis was observed in 17 patients (39%) at three months.Cosmesis was good or excellent in all cases at six months after radiotherapy and in 95% cases at two years after radiotherapy.The 2-,3-and 5-year local control rates were 100%,99% and 94%,respectively.The 2-,3-,and 5-year survival rates were all 100% and no metastases occurred.Conclusions EB-PBI delivered by 3DCRT is feasible for selected Chinese early stage breast cancer patients after breast-conserving surgery.The cosmetic effect,local control rate and long-term survival rate are satisfactory,and acute radiation toxicity is very low.

RESUMEN

In this study, the mature peptide sequence of a pectin lyase gene A was amplified from Aspergillus niger strain EIM-6 by using RT-PCR reverse transcription technique. The cloned gene was then inserted into a Pichia pastoris expression vector pPIC9k to produce the recombinant expression plasmid pPIC9K-pelA. By using electric shocks, we successfully transformed the recombinant pPIC9K-pelA into Pichia pastoris GS115. The activity of the engineered strain reached to 2.3 U/mL after induction with the final concentration of 1.5% methanol. SDS-PAGE analysis revealed that the pPIC9K-pelA transformant had an additional protein band of approximately 38 kD, which was not present in the control. There were no significant differences between the recombinant and native pectin lyase with regard to their hydrolysis activities.

Objective: To survey the content of serum total cholesterol, LDL and HDL in pa- tients with gastric cancer and to investigate the correlation between serum total cholesterol, LDL, HDL and the differentiation of gastric cancer. Methods: 143 patients with gastric cancer were selected and assigned into 3 groups according to pathology grouping after operation: 1) the well-differentiated group, consisting of 16 patients with papillary adenocarcinoma, and 43 patients with tubi-form well-differentiated adenocarcinoma; 2) the moderately differentiated group, including 39 patients with tubiform moderately differentiated adenocarcinoma, 3) the poorly differentiated group, including 20 patients with poorly differentiated adenocarcinoma, and 14 patients with mucinous adenocarcinoma, and 11 patients with signetring cell carcinoma. In addition, 30 patients with gastric ulcer and 30 health adults were selected as control. Veinous blood of every member was collected, and then the content of serum total cholesterol was surveyed by enzymic method, and the contents of serum LDL and HDL were measured by precipitation method. Results: The level of serum total cholesterol, LDL and HDL before operation in patients with gastric cancer decreased significantly than those with gastric ulcer and normal people (P0.05). There was statistically significance among different differentiation in patients with gastric cancer. The differences of levels of serum total cholesterol, LDL and HDL were statistically significant in gastric cancer of different differentiated groups. [Objective] Aneurysmal bone cyst of the ealeaneus is an uncommon entity. The purpose of the report is to describe aneurysmal bone cyst of the ealcaneus seen in a child.[ Methods] The clinical, radiological and histopathologic findings of a boy with aneurysmal bone cyst of the calcaneus were retrospectively analyzed with reference to literature. [ Results ]The tumor mainly involved the right calcaneus. He was treated successfully after the curettage and bone grafting. Histopathological examination revealed an aneurysmal bone cyst of the right calcaneus. He is now disease-free for 2 years, and no sign of recurrence was found. [ Conclusion] Aneurysmal bone cyst of the calcaneus is an uncommon entiy. To simple aneurysmal bone cyst,curettage and bone grafting are effective therapic method.

We retrospectively analyzed clinical and imaging data of 26 children with split cord malformations (SCMs). Based on Pang's classification, 14 SCMs were defined as type Ⅰ and 12 as type Ⅱ.Neural function was markedly improved in 20 patients postoperatively. Three of 4 children who did not undergo surgical treatment had neural function deteriorated. Two children lost follow-up. We suggest that Pang's Classification of SCMs may be useful in describing pathological changes and guiding surgical procedure; imaging examine (including MRI, CT and X-ray) would play a significant role in confirmed SCMs diagnosis; and surgical operation should focus on eliminate and prevent spinal cord damnification.

RESUMEN

OBJECTIVE To investigate the distribution and resistance rate of clinical isolates from neonatal sepsis in Hubei hospitals.METHODS The antimicrobial susceptibility of clinical isolates from(neonates) blood culture derived from sixteen hospitals in Hubei from 2000 to 2004,was retrospectively analyzed and tested by KirbyBauer(method).RESULTS A total of 1 019 strains were isolated from blood specimens,Gram-positive and Gram(-negative)(bacteria) accounted for 93% and 7% of these isolates,respectively.The main isolates were coagulase(-negative) staphylococci(CNS,79.8%),Enterococcus spp(7.0%),Staphylococcus aureus(SAU4.5%),Klebsiella spp(1.7%),Stenotrophomonas maltophilia(1.2%),and Escherichia coli(0.9%).71.3% CNS and 19.6% SAU were meticillin resistant,46.5% enterococci resistant to ampicillin and almost strains were susceptible to(vancomycin)(except E.gallinarum).ESBLs were detected in isolates of 56% E.coli and 35% K.pneumoniae.S.maltophilia was susceptible to trimethoprim(-sulfamethoxazole) and levofloxacin with resistance rate of 8.3% and 0%.In strains of other Gram-negative rods,resistance rate to imipenem,amikacin and fluoroquinolones were detected between 0% to 15%,and to cephalosporins Ⅰ-Ⅳ were between 20% to 70%.CONCLUSIONS Gram-positive cocci are important pathogens in neonatal sepsis,antimicrobials should be used on the result of(antimicrobial) susceptibility test.It is more important to reduce health care-associated(nosocomial) infections.

RESUMEN

Chitosan-carboxymethyl-chitosan complex film was prepared by freeze drying. Some tests in vivo and in animal were employed, in order to evaluate it on biology. All results indicated that the film has not only good surface compatibility but also good structural compatibility. It can be more suitable for GTR technology.

Objective:To determine the relationship between brachia l artery blood pressure and radial artery blood pressure in children and whether blood pressure measured from the radial artery can replace blood pr essure measured from the brachial artery. Methods:Brachial and rad ial artery pressures in 105 children were measured using an electronic sphygmomanometer.Results:There was no significant difference between the average values of blood pressures of the brachial and radial arteries in the 105 children studied (P>0.05). Linear regression and correlation analyses showed that there was a positive linear correlation between the systolic pressure of the b rach ial artery and the radial artery in the 105 children, as well as a posit ive linear correlation between the diastolic pressures of the brachial and the radial arteries. Conclusion:Blood pressure values from the radial artery can completely replace blood pressure values from the brachial artery.

Objective To evaluate the usefulness of CT and US in the diagnosis of gastrointestinal involvement in children with rheumatoid purpura. Methods Abdominal CT and US findings in 14 cases of clinical and laboratory confirmed of rheumatoid purpura were analyzed. There were 9 males and 5 females with the age ranged from 6 months to 14 years. All patients was hospitalized because of acute abdominal emergency, and the clinical presentations included paroxysm of abdomen pain, vomiting, without skin rashes and subcutaneous hemorrhage. CT examination was done in 7 cases, and contrast enhancement was performed in 1 case. Abdominal ultrasound was done in 9 cases. Both CT and US were done in 2 cases. Results All patients demonstrated multiple or single areas of bowel-wall thickening, localized intraluminal narrowing, mesenteric edema, and liquid in abdominal cavity by CT and ultrasound. Conclusion Abdominal CT and US examination have important values in differential diagnosis of acute abdominal disorders in children patients with rheumatoid purpura. Before appearing skin rashes, classical picture can help for the diagnosis and clinical treatment. Abdominal CT and US examination can also find the intussusception, bowel obstruction, and perforation in rheumatoid purpura.

Objective To evaluate the value and limitation of Gd-DTPA enhanced MRI for neuroblastoma staging in children. Methods Twelve cases of neuroblastoma proved by operation or bone marrow aspiration were examined by gadolinium-enhanced MRI. The age ranged from seven months to five years, mean 3.7 years. Eight tumors originated from adrenal, and four from posterior mediastinum. Conventional sequences, double dose gadolinium-enhanced MRI, and 3D CEMRA were used in all patients. Six cases were examined by CT in same time. Imaging staging on surgic-histopathological-based International Neuroblastoma Staging System (INSS) was performed. Results Six patients were staged by CT, including stage Ⅰ-Ⅱ in 2 cases, stage Ⅲ in 4 cases, and stage Ⅳ in none. Twelve patients were staged by conventional MRI, including stage Ⅰ-Ⅱ in 2 cases, stage Ⅲ in 9 cases, and stage Ⅳ in 1 case. Twelve patients were staged by double dose gadolinium-enhanced MRI, including stage Ⅰ-Ⅱ in 1 case, stage Ⅲ in 1 case, and stage Ⅳ in 10 cases. Conclusion Gadolinium-enhanced MRI was a single best imaging modality for neuroblastoma, most useful for distal to diaphragm metastasis, dumbbell tumor intraspinal extension, and bone marrow metastasis that was not detected by aspirate examination. Enhancement MRI was important in evaluating the therapy and was also helpful in assessing the therapeutic efficacy and relapse. 3D CEMRA helps demonstrate large vascular encasement and tumor erosion into important organs, and it is useful in assessing the respectability. Long examination time and lack in showing the characteristic calcium were the limitations.

Objective:To understand the levels of serum cholesterol in patients with gastric cancer and gastric ulcer before and after operation,and to study the relationship between the level of serum cholesterol and the tissues differential degree of gastric cancer.Methods:To survey the content of serum cholesterol in patients with gastric cancer and patients with gastric ulcer confirmed clinically before and after operation,looking through the medical records of the patients and taking notes about the tissues differentiated degree of the gastric cancer,then making the statistical analysis.Results:Comparing with the normal control group,the level of serum cholesterol in gastric cancer decreased obviously before operation,there was obvious statistical difference(P0.01),and increased after operation,comparing with themselves before operation was statistical difference(P0.05),and no statistical difference comparing with the normal control group after operation.There was statistical difference between the higher(group A)and the lower(group B)differentiated degree of gastric cancer tissues(P0.05)before operation and no statistical difference after operation.After operation the levels of serum cholesterol in group A and group B were all increased.Comparing with the level of serum cholesterol before operation each other,there was obvious statistical difference(P0.01)in group B and no statistical difference(P0.05) in group A.Conclusion:The level of serum cholesterol in patients with gastric cancer decreased before operation and increased after operation,especially in progressive gastric cancer.There was correlation between the level of serum cholesterol and the tissues differentiated degree of gastric cancer.The lower the tissues differentiated degree of gastric cancer was,the lower before operation and the higher after operation the level of serum cholesterol were.

Objective: To evaluate biological materials tanned with a compound method for clinical performance of biovalves. Methods: The tissue calcium concentration was compared between biological materials tanned simply with amylic aldehyde and those compoundly tanned with amylic aldehyde, cation fat, chromium trioxide and glycerol. The histological morphology, the ultra-structure and tensile strength of the biological materials were tested and the content of hydroxyls in the materials was measured with the Fourier infrared spectrum detector. The prosthetic biological cardiac valves made of the materials were tested with an accelerated wearing device extracorporeally. Results: The tissue calcium concentration was lower in the compoundly tanned materials. In these materials, the collagenous fibers were dense and lined up in order. The cellular structure was maintained. The materials were pliable at the contracting temperature of 86℃ to 90℃. The tensile strength of the materials was 19.9 to 25.9 N/mm 2 and the extension rate was 43.4% to 46.0%. The peak of the hydroxyls decreased significantly at the wave length of 1741.11 cm -1 in the Fourier infrared spectrum. The swine aortic valve experienced 3.892?10 8 cycles in the accelerated wearing tests, and the bovine pericardial valve, 3.888?10 8 cycles. By estimation, the biovalves will last for about ten years. Conclusion: Compound tanning can increase firmness and softness of the materials and may play an important role in anticalcification of the biovalves.

Stability constant of ascorbic acid a nd lea d complex was measured by pH potentiometry at different tempcraturcs and concentrations of monoammonium glycyrrhizinate (MG ).At a concentration of 5.0?10-4mol/L and at biolugical condition, lgk1 = 8.72 and lgk2 =6.91. The result suggestod that the stability constant can be imptoved at decreased temperatures and increased concentrations of MG.
