RESUMEN

OBJECTIVES@#To investigate the effect of progressive pre-disconnection of urethral mucosal flap during transurethral plasmakinetic enucleation of prostate (TUPEP) on early recovery of urinary continence.@*METHODS@#Clinical data of patients with benign prostatic hyperplasia (BPH) admitted in Zhujiang Hospital of Southern Medical University during February and May 2022 were collected. All the patients underwent TUPEP, and the progressive pre-disconnection of urethral mucosal flap was performed in the procedure. The total operation time, enucleation time, postoperative bladder irrigation time and catheter indwelling time were recorded. Urinary continence was evaluated 24 h, 1 week, and 1, 3, 6 months after the removal of urinary catheter.@*RESULTS@#All surgeries were successfully completed at one time with less intraoperative bleeding, and there were no complications such as rectal injury, bladder injury or perforation of prostate capsule. The total operation time was (62.2±6.5) min, the enucleation time was (42.8±5.2) min, the postoperative hemoglobin decrease by (9.5±4.5) g/L, the postoperative bladder irrigation time was (7.9±1.4) h, and the postoperative catheter indwelling time was 10.0 (9.2, 11.4) h. Only 2 patients (3.6%) had transient urinary incontinence within 24 h after catheter removal. No urinary incontinence occurred at 1 week, and 1, 3, 6 months after operation, and no safety pad was needed. The Qmax at 1 month after operation was 22.3 (20.6, 24.4) mL/s, international prostate symptom scores were 8.0 (7.0, 9.0), 5.0 (4.0, 6.0) and 4.0 (3.0, 4.0) at 1, 3 and 6 months after surgery, and quality of life scores at 1, 3 and 6 months after surgery were 3.0 (2.0, 3.0), 2.0 (1.0, 2.0) and 1.0 (1.0, 2.0), all of these indicators were better than those before surgery (all P<0.01).@*CONCLUSIONS@#In the treatment of BPH, the application of progressive pre-disconnection of urethral mucosal flap in TUPEP can completely remove the hyperplastic glands and promote early recovery of postoperative urinary continence with less perioperative bleeding and decreased surgical complications.

RESUMEN

To investigate the effect and mechanism of metformin on intestinal epithelial barrier injury in ulcerative colitis. A cell model of colitis was established by co-culture of human colon cancer cell line Caco-2 and human monocyte cell line THP-1. The colitis model cells were treated with metformin at concentration of for Flow cytometry was used to detect Caco-2 cell apoptosis, and Western blotting was used to detect the protein expression of tight junction proteins and endoplasmic reticulum stress-related proteins. After metformin treatment, the apoptosis rate of Caco-2 cells was decreased from (14.22±2.34)% to 0.61)% (=3.119, <0.05), and the expression levels of tight junction protein-1 and claudin-1 increased (=5.172 and 3.546, both <0.05). In addition, the expression levels of endoplasmic reticulum-related proteins glucose regulated protein (GRP) 78, C/EBP homologous protein (CHOP) and caspase-12, as well as the phosphorylation level of PRKR-like endoplasmic reticulum kinase (PERK) and eukaryotic translation initiation factor 2α (eIF2α) decreased (all <0.05). Metformin may alleviate the intestinal epithelial barrier damage in colitis by reducing intestinal epithelial cell apoptosis and increasing the expression of tight junction proteins, which may be associated with the inhibition of endoplasmic reticulum stress-induced apoptotic pathway.

RESUMEN

Objective To evaluate the advantages of confocal laser endomicroscopy(CLE)for function repairing of ulcerative colitis(UC). Methods Thirty patients with UC who were diagnosed and treated in the First Affiliated Hospital of Zhejiang University Medical College between July 2014 and December 2016 were enrolled in the study group. The control group consisted of 10 patients who were diagnosed as having colonic polyps with colonoscopy and underwent polypectomy in the same period. Both groups were examined with white light endoscopy and CLE,then the fluorescein leakage score was compared. Results There were 10 out of 30 cases in the study group whose whole intestinal mucosa were judged as normal under white light endoscopy. In the other 20 cases,there were parts of intestinal mucosa being judged as abnormal. All of the 10 cases in the control group were with normal colon mucosa. There was a significant difference on fluorescein leakage score by CLE between the study group which showed normal by white light endoscopy and the control group(P<0.05). The fluorescein leakage score by CLE was significant difference between the abnormal intestinal segment and normal intestinal segment in 20 patients with active UC(P<0.05). Spearman correlation analysis showed that there was non-ranked correlation between the fluorescein leakage score of CLE and histopathological findings of biopsy in UC patients with normal mucosa of the left colon under white light endoscopy(rs=0.394,P>0.05). Conclusion In the process of mucosal healing of UC patients,structural repair can be found firstly through CLE. Mucosal healing under white light endoscopy cannot represent the functional recovery. CLE is more effective than the histopathology in the evaluation of mucosal barrier function.

RESUMEN

<p><b>OBJECTIVE</b>To compare the diagnostic value of double-balloon endoscopy (DBE) and multi-detector row computed tomography (MDCT) for small intestinal tumors.</p><p><b>METHODS</b>The clinical data of 88 patients with small intestinal tumor who underwent DBE and MDCT examinations during January 2010 and December 2016 was retrospectively analyzed. The diagnostic value of DBE and MDCT for small intestinal tumor was compared.</p><p><b>RESULTS</b>The positive rate of small intestinal tumors by DBE (92.0%, 81/88) was significantly higher than that of MDCT (75.0%, 66/88,<0.01). The positive rates for malignant small intestinal tumors by DBE and MDCT were the same (91.1%), but the rate of histological qualitation was higher in DBE than that in MDCT (94.1% vs 76.5%,=6.331,<0.05); the positive rates for benign tumors or the tumors <3 cm in diameter by DBE were significantly higher than those by MDCT (96.8% vs 48.4%, 88.2% vs 47.1%, respectively,=18.235 and 13.170, both<0.01); also DBE had a higher positive rate for small intestinal tumor presenting gastrointestinal bleeding (97.2% vs 77.8%,=6.222,<0.05).</p><p><b>CONCLUSIONS</b>The diagnostic value of small intestinal tumor by DBE is significantly higher than that of MDCT. The combination of two methods may improve the diagnostic accuracy for small intestinal tumor.</p>

<p><b>OBJECTIVE</b>To explore the platelet count in prediction of the efficacy of infliximab for active Crohn's disease.</p><p><b>METHODS</b>A total of 74 patients with Crohn's disease, including 50 males and 24 females with mean age of 27 years, receiving infliximab therapy from January 2011 to June 2015 were retrospectively included. Clinical variables including routine blood test, erythrocyte sedimentation rate, C-reactive protein and Crohn's disease activity index were measured before treatment and after 3rd, 6th treatments.</p><p><b>RESULTS</b>Compared with baseline, the Crohn's disease activity index score, white blood cell count, platelet count, erythrocyte sedimentation rate, C-reactive protein significantly decreased (all (P<0.01), while hemoglobin and red blood cell hematocrit increased (P<0.01) in patients after 3 treatments of infliximab. These variables were further improved after 6 treatments. Further analysis showed that platelet count was positively associated with disease severity of Crohn's disease. Fifty-three patients achieved clinical remission (Crohn's disease activity index score<150) after 3 treatments of infliximab. Logistic regression analysis showed that baseline platelet count, Crohn's disease activity index, and disease course were three predictors for clinical remission of Crohn's disease.</p><p><b>CONCLUSION</b>Platelet count is positively associated with disease severity of Crohn's disease, and baseline platelet count is an important predicator for the early response of infliximab therapy.</p>

Objective To investigate the correlation between capsule endoscopy Crohn′s disease activity index (CECDAI ) , capsule endoscopy Lewis score , simplified Crohn′s disease activity index (sCDAI) ,C reactive protein (CRP) ,erythrocyte sedimentation rate (ESR) and Crohn′s disease (CD) activity .Methods From April 2009 to April 2014 , 120 patients having received capsule endoscopy examination and diagnosed as CD were collected . The data of CRP and ESR were analyzed . The CD severity was scored by CECDAI ,Lewis score and sCDAI .Spearman correlation was performed to analyze the correlation between CECDAI ,Lewis score ,sCDAI ,CRP and ESR .Results Among all the patients , CECDAI had a good correlation with Lewis score (r=0 .645 , P0 .05) . sCDAI had a good correlation with CRP and ESR (r=0 .438 and 0 .429 ,both P0 .05) .Conclusions Lewis score ,sCDAI ,CRP and ESR can reflect disease activity of CD patients in a certain degree .CECDAI is the most suitable index in evaluating the disease activity of CD patients .Therefore ,it is necessary to attach importance to capsule endoscopy examination .

RESUMEN

<p><b>OBJECTIVE</b>An optimal automatic selection method of permissible source region is proposed to reduce the ill-conditioned and ill-posed problems in the reconstruction of the light source in bioluminescence tomography.</p><p><b>METHODS</b>The 2D images captured by CCD are mapped into surface light irradiance distribution based on the light propagating model. The relation matrix between the source and light distribution is obtained by finite element method. Permissive source region is determined by using the automatic selection method proposed in this paper, and then Tikhonov regularization is applied to reconstruct the light source.</p><p><b>RESULTS</b>The center point distance between the optimal permissible source region and true source is 1.26 mm, and the center point error of the reconstructed light source and true source is 0.47 mm, the volume error is 9.13 mm3.</p><p><b>CONCLUSION</b>The optimal permissive source region selection strategy is effective to locate the permissive source region close to the true source, and reduces the reconstructed error due to subjective orientation of permissible source region. This proposed method is the basis of high precision source reconstruction in bioluminescence tomography.</p>

RESUMEN

In order to choose a fast and efficient real-time method in beta wave information extraction, we compared the result and the efficiency of the information separation of both fast Fourier transform (FFT) and wavelet transform of EEG beta band in the present paper. Our work provides the basis for the EEG data come from the real-time health assessment of 3DTV. We took the EEGs of 5 healthy volunteers before, after and during the process of watching 3DTV and meanwhile recorded the results. The trends of the relative energy and the time cost of two methods were compared by using both the FFT and wavelet packet transform (WPT) which was to extract the feature of EEG beta wave. It demonstrated that (1) Results of the two methods were consistent in the trends of watching 3DTV; (2) Results of the differences in two methods were consistent before and after watching 3DTV; (3) FFT took less time than the wavelet transform in the same case. It is concluded that the results of both FFT and Wavelet transform are consistent in feature extraction of EEG, and a fast method to work with the large quantities of EEG data obtained in the experiments can be offered in the future.

RESUMEN

Reconstruction of 3D structure of an object from 2D views plays an important role in plastic surgery and orthopedics. This method doesn't need camera to do specific movements, such as translation or rotation independently. It only needs a hand-hold camera arbitrarily to take a few pictures, and apply the geometry relationship among the three views to obtain the projective reconstruction of the object. Then, it needs to introduce cheirality constraint in stratified reconstruction to determine the search area of the infinity plane, and finally achieve the camera intrinsic parameters calibration, and complete the metric reconstruction. This model has also been reconstructed with mouse and keyboard response coordinates to observe the model from different angles. Experiments with both pictures of object and face pictures show that the proposed method is very robust and accurate.

RESUMEN

Dynamic causal modeling (DCM) is a spatio-temporal renewable network model. As an analytical method of causality of functional integration in fMRI, DCM is applied to study the effective connectivity. The neuro-imaging time series of activated regions were put into DCM, and the trial-bound inputs were used as perturbations to the designed model. DCM was used in combination with Bayesian estimation to evaluate the intrinsic connectivity among selected neurons. Bayes factors were used to compute different neuro-physiological models with intrinsic connectivity structures, and then were used to select the optimal model. The selected regions in this mental calculation task are the left superior parietal lobule (SPL), the left inferior parietal lobule (IPL) and the left middle frontal gyrus (MFG). Finally, the connected network in conformity to physiological significance was obtained.

RESUMEN

The popularity of magnetic resonance imaging over other imaging disciplines depends predominantly on its high spatial resolution, soft tissue contrast, no hard fake trace and no radiation injury. However, the ghost artifacts produced during a magnetic resonance imaging would degrade the image badly and affect the precise orientation to focus. This paper introduced the process of studies for reduction of ghost artifact resulting from EPI imaging theory and motion of subject. A lot of researches indicated that how to reduce the ghost effectively is still a challenging task. Researchers should persist in seeking for new method to solve the difficult problem.

The artifacts in magnetic resonance imagines(MRI) seriously degrade the imagine quality and affect the exact diagnosis. The reasons that may cause artifacts in magnetic resonance imaging and the suppressing methods are presented in this paper. One of the intractability appeared in researches is how to restrain all kinds of artifacts quickly and efficiently. It is important to search for new methods to solve this problem. It may provide reliable basis for medical observation and clinical diagnosis.

RESUMEN

The electrocardiac signals exhibit obvious unsteadiness and contain many noises in the processing of detecting. In this paper, a wavelet transform, which can be located in the fields of time and frequency, is introduced to decompose the ECG signals on multi-scales. In characteristic scales, the electrocardiac signals can be decomposed. After removal of various noises, the electrocardiac signals are formed again perfectly. This method provides more accurate ECG data for use in medical diagnosis.

RESUMEN

The study of pulse wave, including its quantitative analysis and propagation, is one of the hot topics in the fields of clinical medicine, such as how to detect cardiovascular diseases and make early diagnosis without harm. In this paper are discussed a new type of automatic cardiovascular diagnosis method and the corresponding test instrument. The cardiovascular system parameters can be achieved through the wave changes, related data processing and analyzing. This system includes piezo-film transducer, signal amplifier, data collecting and data analyzing equipment. Various pulse diagrams can be shown in omnidirectional and multifunctional ways. The measurement error can be reduced effectively by introducing Mini-wave analysis in the design of software. The research indicates that combination of this equipment with the traditional Chinese medicine will provide wide practical application.
