RESUMEN

OBJECTIVE@#To understand the clinical characteristics about sequence diagnosis and treatment of oral complications in patients with Sjögren's syndrome (SS) through retrospective analysis, and to provide some guidance for clinical work.@*METHODS@#Some SS patients who underwent oral sequence management in the Department of General Dentistry, Peking University School and Hospital of Stomatology from January 2015 to September 2021 were enrolled. For the SS patients included in this study, a comprehensive oral examination was performed, including parotid region examination, oral mucosal exa-mination, dentition examination, dental examination, periodontal examination, unstimulated salivary flow rate, Candida infection and radiological imaging examination. According to the examination results, the patients were given fluoride application, antifungal treatment, root canal therapy, direct filling repair, and indirect repair treatment in sequence and the results recorded.@*RESULTS@#A total of 9 patients with SS, with 4 primary SS patients (pSS) and 5 secondary SS patients (sSS) were enrolled in the study. For all the 9 patients, the average age was (49.2±16.2) years and the median xerostomia duration 5 years. The unstimulated salivary flow rate of the 9 patients was all less than 1 mL/10 min. Eight of the 9 cases was diagnosed as oral Candidiasis, with positive salivary Candida culture result (>200 cfu/mL), and 1 of the 9 cases was not. The average decay, missing, filling teeth (DMFT) was 24.8±4.2; the average decay, missing, filling tooth surfaces (DMFS) was 59.2±21.9, the average incisal caries was 2.5±1.3, and the average number of crown restorations at baseline was 4.5±3.6. All the 9 SS patients were applied with topical fluoride usage, and 8 were prescribed with antifungal treatment. One sSS patient was conducted with filling restoration treatment, one pSS patient was conducted with full mouth rehabilitation, and the remaining 7 patients were conducted with direct filling combined with fixed repair treatment. The average 3.2 full crown restorations in 6 patients had to be removed and restored because of secondary caries, and 3 of the 9 patients underwent implant denture restorations finally.@*CONCLUSION@#Management of oral complications in SS patients needs to be carried out in sequence. A comprehensive examination and diagnosis should be carried out first, followed by infection control, and then restoration of oral function at last.

OBJECTIVE@#To observe the short-term efficacy, long-term efficacy and safety of acupuncture for chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS).@*METHODS@#Forty-two patients with CP/CPPS were randomly divided into an acupuncture group (21 cases, 1 case dropped off) and a sham acupuncture group (21 cases). The patients in the acupuncture group were treated with acupuncture at bilateral Zhongliao (BL 33), Huiyang (BL 35), Shenshu (BL 23) and Sanyinjiao (SP 6); the needling depth of Zhongliao (BL 33) and Huiyang (BL 35) was 60 to 80 mm, while Shenshu (BL 23) and Sanyinjiao (SP 6) was directly punctured of 30 mm. The patients in the sham acupuncture group were treated with acupuncture at non-acupoints, including points 2 cm next to Shenshu (BL 23), Zhongliao (BL 33) and Huiyang (BL 35), and the midpoint of the connecting line between the spleen meridian and the kidney meridian. All the non-acupoints were treated with directly puncture of 2 to 3 mm. The needles were left for 30 min in both groups, once every other day in the first four weeks, three times a week, and twice a week in the next four weeks, totally 20 treatments. Before treatment, after treatment and in follow-up of 24 weeks after treatment completion, the National Institutes of Health-chronic prostatitis symptom index (NIH-CPSI) score and urinary flow rate were observed in both groups; the clinical efficacy and safety were evaluated.@*RESULTS@#Compared with those before treatment, the pain and discomfort scores, urination symptoms scores, quality of life scores and total scores of NIH-CPSI in both groups were reduced after treatment in the two groups (P<0.01), while each item score and total score of NIH-CPSI in the acupuncture group were reduced in follow-up (P<0.01, P<0.05). After treatment and in follow-up, each item score and total score of NIH-CPSI in the acupuncture group were lower than those in the sham acupuncture group (P<0.05, P<0.01). After treatment, the maximum and average urinary flow rates in the acupuncture group were higher than those before treatment (P<0.05), and the average urinary flow rate in the acupuncture group was higher than that in the sham acupuncture group (P<0.05). The total effective rate was 75.0% (15/20) in the acupuncture group, which was higher than 42.9% (9/21) in the sham acupuncture group (P<0.05). No significant adverse reactions were observed in the two groups, and there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).@*CONCLUSION@#Acupuncture could effectively alleviate the clinical symptoms, improve quality of life, and has a sustained, safe and reliable therapeutic effect in patients with CP/CPPS.

Objective: To explore the composition of bacteria in lower respiratory tract of patients with pneumoconiosis and dust exposure, and to compare and analyze the difference and correlation between them. Methods: From May 2020 to January 2021, a prospective multicenter cross-sectional study was conducted to select patients with pneumoconiosis who underwent bronchoalveolar lavage treatment at the Respiratory and Critical Care Medical Department of the 920th Hospital of the Joint Support Force and the Respiratory Department of Tongren Hospital in Kunming, as well as the population of dust recipients. A total of 24 patients with pneumoconiosis (pneumoconiosis group) were included, and 16 dust exposed individuals (dust exposed group) were used as controls. Two groups of patients' alveolar lavage fluid were collected. The 16SrRNA gene V3-V4 sequencing technology and bioinformatics analysis platform were used to measure and analyze the differences in microbial structure composition and associations between bacterial communities. Results: Compared with the dust exposed group, the top 5 bacterial phyla in the alveolar lavage fluid level of patients with pneumoconiosis were the same, followed by Proteobacteria, Firmicutes, Bacteroidetes, Fusobacteria, and Actinobacteria. Compared with the dust exposure group, the pneumoconiosis group patients belong to the top 5 genera of horizontal flora abundance, which are different. The dust exposure group is respectively: Pseudomonas, Proctor, Streptococcus, Achromobacter, and Neisseria. The pneumoconiosis group is respectively: Pseudomonas, Achromobacter, Streptococcus, Ralstonia, and Proctor. The Alpha diversity analysis results showed that compared with the dust exposed group, the level of bacterial diversity in the pneumoconiosis group was difference (P<0.05), and there was no statistically significant difference in bacterial evenness (P>0.05) ; Beta diversity showed differences in microbial community structure between the two groups (P<0.05 ). Single factor microbial association network analysis showed that there was a high correlation between Firmicutes and Bacteroidetes in the pneumoconiosis and dust exposed groups and other species, showing a positive correlation; The correlation between Proteobacteria and other species is high, showing a negative correlation. Conclusion: The structure and relative abundance of bacteria in lower respiratory tract were different between patients with pneumoconiosis and dust exposure, and the diversity of bacteria in lower respiratory tract increased in patients with pneumoconiosis, which may be related to disease status.

OBJECTIVE@#To observe the effect of electronic moxibustion on memory function in the patients with amnestic mild cognitive impairment (aMCI).@*METHODS@#A total of 59 aMCI patients were randomized into an electronic moxibustion group (30 cases) and a placebo moxibustion group (29 cases). In the electronic moxibustion group, the electronic moxibustion was applied to Baihui (GV 20), Dazhui (GV 14), Mingmen (GV 4) and Taixi (KI 3), 45 ℃ in temperature, 20 min each time. The treatment was given once a day, 5 times a week. The treatment for 4 weeks was as one course and 2 courses were required totally. In the placebo moxibustion group, the moxa-free patch was used, 38 ℃ in temperature. The acupoint selection and the treatment frequency were same as the electronic moxibustion group. Before and after treatment, Rivermead behavior memory test (RBMT) was adopted to evaluate the global memory function of the patients in the two groups and the N-back task test was adopted to evaluate working memory function separately. Additionally, the mini-mental state examination (MMSE) and its immediate memory, Montreal cognitive assessment (MoCA) and its delay recall were adopted to evaluate the global cognitive function and memory function@*RESULTS@#In the electronic moxibustion group, after treatment, RBMT score, N-back accuracy rates, MMSE and MoCA scores and the scores of immediate memory and delay recall were improved significantly as compared with those before treatment (<0.01). In the placebo moxibustion group, the accuracy rates of 1-back and 2-back task and the scores of immediate memory and delay recall were improved obviously as compared with those before treatment (<0.05, <0.01). After treatment, the improvements of RBMT score, the accuracy rates of N-back task and MMSE and MoCA scores in the electronic moxibustion group were higher than those in the placebo moxibustion group (<0.05).@*CONCLUSION@#Electronic moxibustion improves memory function in the patients with amnestic mild cognitive impairment.

In the aspects of the acupoint prescription, acupuncture techniques and typical cases, the characteristics and experience of professor in the clinical treatment of vascular dementia with acupuncture and moxibustion were introduced. Vascular dementia is treated on the base of etiology, focusing on the brain as the root cause, with the reinforcing and the promoting as the dominant principle of acupuncture. The treatment is emphasized on tonifying the kidney to fill up marrow, strengthening the spleen to nourish the brain and promoting the collateral circulation to improve the intelligence. Concerning to the general situation of the disease, regulating mind stressed in treatment. In clinical practice, Baihui (GV 20) is the chief acupoint to benefit the brain. -source points and the eight influential points are predominated to tonify the functions of organs and fill up vessels. Moxibustion is adopted to resolve the stagnation and promote circulation in meridian as well as regulate and blood circulation. Specially, the moxibustion technique for resolving the stagnation and promoting circulation in meridian achieves the satisfactory therapeutic effects in clinical treatment.

·AIM: To evaluate the safety and postoperative complications of femtosecond laser - assisted small incision lenticule extraction (SMILE). ·METHODS: Retrospective case series. A total of 1127 patients (2 236 eyes) who were treated with SMILE for myopia or myopia astigmatism between June 2016 and May 2017 were enrolled in this study. Eyes that developed postoperative complications were noted and identified. The incidence, risk factors, management and prognosis were analyzed. The follow-up was 6mo. ·RESULTS: The rate of postoperative complications was 8. 05%, included diffuse lamellar keratitis (3. 31%), delayed visual acuity (2. 59%), minor interface residue (0. 63%), and ghost images ( 1. 52%). These complications had an impact on best corrected visual acuity (BCVA) at 3mo in only 1 eye with decentered ablation and was re-treated with topography-guided laser- assisted subepithelial keratomileusis (LASEK). Good visual outcomes were achieved in all eyes finally. · CONCLUSION: Although few eyes suffered postoperative complications, SMILE is an acceptable safe surgery. Careful surgical skill, appropriate surgical parameter, and rational postoperative medication can decrease the risk of complication.

Objective To investigate clinical features of tremors in PD patients with rapid eye movement sleep behavior disorder (RBD) and their response to medication. Methods PD patients were divided into PD with clinically possible (cp) RBD (PD+cpRBD) and PD without cpRBD (PD-cpRBD) according to RBDSQ scale. Hoehn & Yahr (H-Y) grade ,UPDRS-Ⅲ scale and MDS-UPDRS tremor scale were used to evaluate the motor function of the patients and amplitude of rest, kinetic and postural tremors of the first affected limbs. The re-sults were compared between these two groups including the features of the general information and tremor as well as the maximal improvement rate of UPDRS-Ⅲ and MDS-UPDRS tremor scores to acute levodopa chal-lenge test. Results A total of 42 PD patients with tremors were enrolled in this study, including 19 patients in PD+cpRBD group and 23 patients in PD-cpRBD group. There was no significant difference in sex, age, age of onset, duration of disease, UPDRS-Ⅲ score and H-Y grade between two groups (P>0.05). The tremor scores of the "off" state were significantly higher in PD+cpRBD group than in PD-cpRBD group (t=2.379,P=0.022). Compared with PD-cpRBD group, the progression of tremor from one side to other side was faster(u=-2.133,P=0.033)and the amplitude scores of rest and kinetic tremors were higher in PD+cpRBD group (rest tremor :u=-2.956,P=0.003;kinetic tremor:u=-2.657,P=0.008). The maximal improvement rate of UPDRS-Ⅲ and tremor score was significantly higher in PD-cpRBD group than in PD+cpRBD group after taking levodopa/ benserazide (200/50 mg) (UPDRS-Ⅲ:u=-3.134,P=0.002;tremor score: t=-3.189,P=0.003). Conclusion The present study has demonstrated severe tremors especially rest and kinetic tremors and poor response to levodopa in PD patients with cpRBD.

Objective By detecting the distribution and homology analysis of Acinetobacter baumanii in the ICU ward,to con-trol nosocomial infection,provide theoretical basis to take effective measures.Methods From January 2016,20 patients in ICU of Shaanxi Provincial People's Hospital were taken into group.The samples of sputum and surrounding environment samples were cultured.The homology of Acinetobacter baumannii was analyzed by MALDI-Biotyper software,antimicrobial susceptibility test was analyzed by K-B.Results 27 strains of Acinetobacter baumannii were detected,mainly comes from sputum,hands of medical staffs and the environment,homology analysis results showed that the 27 strains of Acinetobacter baumannii was divided into two clusters(Ⅰtype 1 1 strains,Ⅱ type 1 6 strains),most of Acinetobacter baumannii were multi-resistant bacteria,except for the polymyxin B,minocycline and SCF.Conclusion The ways of transmission of Acineto-bacter baumanii in ICU were by medical personnel hand,pollution of the medical equipment and so on,strengthening the dis-infection and reasonable application of antimicrobial agents were taken advantageous for the prevention and control of acine-tobacter baumannii infection and transmission.

Objective To observe the effect of Rule Granules on the breast tissue pathology,ovarian hormone and receptor expression of mammary glands hyperplasia rats; To explore its mechanism of action. Methods The model of mammary gland hyperplasia was established by intramuscular injection of estradiol benzoate for 20 days and intramuscular injection of progesterone for 5 d. 66 female SD rats were randomly divided into 6 groups, including blank group, model group, acupuncture group, TCM high-, medium- and low-dose groups, with 11 rats in each group. Acupuncture group chose group A (Wuyi, Hegu, on both sides, Tanzhong) and group B (Tianzong, Ganshu, Zusanli, on both sides), and the two groups were used alternately, one group per day. TCM groups received 1.1 g/mL, 0.55 g/mL,and 0.11 g/mL dose of Rule Granules for gavage,once a day.Abdominal aorta blood was collected after continuous treatment of 30 d. The contents of serum E2and P were detected through ELISA method, at the same time, the second left side of the breast was removed. The changes of the breast tissue was observed under light microscope, and the ER and PR positive expression was observed by SABC immunohistochemical method. Results Compared with the blank group, hyperplasia of mammary gland in the model group rats was more obvious, and serum E2, ER and PR positive expression of integral increased significantly (P<0.01), but the content of P decreased significantly (P<0.01); compared with the model group, hyperplasia of mammary gland was improved in all treatment groups, and serum E2, ER and PR positive expression of integral decreased significantly (P<0.05); the content of P in acupuncture group increased significantly (P<0.01), and the acupunture group and TCM low-dose group were the most obvious. Conclusion Rule Granules can effectively improve hyperplasia of mammary gland organization structure in rats caused by exogenous estrogen and progesterone, with TCM low-dose group and acupuncture group having the best efficacy. Its mechanism is related to reducing the contents of serum E2and ER, and PR positive expression in breast tissue, and increasing the content of serum P.

Objective:To study the biological characteristics of new retrograde filling materials iRoot BP plus and iRoot FS.Methods:(1 )The roots were cut into 3 mm in length,and the root canals were pre-pared to 1 mm in diameter,followed by being filled with iRoot BP plus,iRoot FS,or mineral trioxide ag-gregate (MTA).The specimens were immersed in simulated body fluid (SBF).The ability of minerali-zation in vitro was detected through three studies.First,the mineralization of specimens was analyzed through scanning electron microscope observations and energy dispersive X-ray spectrometer.Then,the pH of SBF was monitored using pH meter.(2)The extracts were gained by immersing blocks of iRoot BP plus,iRoot FS,and MTA (8 mm diameter and 2 mm height)into dulbecco’s modified eagle medium (DMEM).The effects of the extracts on proliferation of MG63 cells were detected through MTT assay. The gene expression level of alkaline phosphatase (ALP)was analyzed by quantitative real-time PCR, and the expression of ALP activity was observed by ALP activity staining.Results:(1 )The formation of minerals could be observed on the surfaces of iRoot BP plus,iRoot FS,and MTA at the end of 24 h,and there were more amounts of apatite aggregated after 14 days.The values of Ca/P ratios of apatites were 1.43,1.39,and 1.51,respectively.(2)The pH of SBF could be raised to 8.09 ±0.07,7.91 ± 0.06,and 8.1 1 ±0.06,respectively,significantly higher than the blank.(3)The extracts of iRoot BP plus,iRoot FS,and MTA of dilutions of 1∶5 and 1∶10 presented no effect of proliferation of MG63 cells. (4)iRoot BP plus and iRoot FS could significantly up-regulated the levels of ALP messenger RNA ex-pression,while there was no obvious difference in ALP staining among the iRoot BP plus,iRoot FS, MTA,and the blank.Conclusion:The present study shows that iRoot has displayed good mineralization capability in vitro and capability to promote differentiation and mineralization of MG63 cells,inferring that iRoot may have good bioactivity.

<p><b>OBJECTIVE</b>To investigate the clinical characteristics and surgical treatment of penile Paget's disease.</p><p><b>METHODS</b>We retrospectively analyzed the treatment and follow-up data of 10 cases of penile Paget's disease surgically treated in Jiangsu Provincial Government Hospital and Jiangsu Provincial People's Hospital from 2008 to 2012.</p><p><b>RESULTS</b>All the 10 patients received expanded local resection of the lesion with reconstruction of the defects with scrotal skin flaps or free skin flaps from the thigh. All surgeries were successful and the postoperative course was uneventful with complete graft survival and no lymph node metastasis. IIEF scores obtained before and 1 -2 months after surgery showed no statistically significant differences in the penile erectile function (P = 0.229), sexual orgasm (P = 0.761), and sexual satisfaction (P = 0.801) of the patients.</p><p><b>CONCLUSION</b>When penile skin lesions suggest the possibility of Paget's disease, biopsy should be performed and surgery should follow as soon as possible. The ideal surgical option is expanded local resection of the lesion with reconstruction of the defects with scrotal skin flaps or free flaps according to the patient's specific conditions.</p>

Objective The CYP19 gene product enzyme aromatase mediates the conversion of the androgen testosterone to es -tradiol.The aim of this study is to investigate whether the CYP19A1 gene single nucleotide polymorphism (SNP) is associated with the susceptibility to polycystic ovary syndrome (PCOS) and serum hormone levels. Methods We conducted a case-controlled study, which included 373 PCOS patients and 313 healthy controls.We genotyped SNP rs2899470 in the subjects using the polymerase chain re-action-restriction fragment length polymorphism ( PCR-RFLP) method and analyzed the frequencies of genotypes and alleles as well as the association of different genotypes with age , menarchal age, body mass index (BMI), and serum levels of hormones. Results The gen-otypic distributions of rs2899470 GG, TG, and TT in the PCOS women were 44.5%, 49.6%, and 5.9%, respectively, significantly dif-ferent from 39.3%, 48.6%, and 12.1%in the healthy controls (P=0.013).The frequency of the G allele was 69.3%in the former, remarkably higher than 63.6%in the latter (P=0.025).The rs2899470 genotypic frequencies were associated with the serum E 2/T lev-els in the PCOS patients. Conclusion SNP rs2899470 in the CYP19A1 gene is associated with the susceptibility to PCOS , and so is the genotype of rs2899470 with serum E2/T levels, which may be attributed mainly to the reduced activity of aromatase .

RESUMEN

<p><b>OBJECTIVE</b>To explore the long-term survival and prognosis of prostate cancer patients after treated by androgen deprivation therapy.</p><p><b>METHODS</b>We conducted a follow-up study of 124 patients with prostate cancer treated by androgen deprivation therapy, and compared the survival times of the patients with different pathological grades and clinical characteristics using Kaplan-Meiers survival curves.</p><p><b>RESULTS</b>The mean survival time of the 124 patients after androgen deprivation therapy was 5. 912 years, with the median survival time of 7.81 years. The patients with bone metastases showed a shorter survival time than those with non-bone metastasis (P = 0.04). Pathological grades and PSA levels were not prognostic factors. No significant differences were found in the mean survival time between those died of prostate cancer (n = 35) and those from other factors (n = 23) (P = 0.50).</p><p><b>CONCLUSION</b>Bone metastasis is an important prognostic factor in advanced prostate cancer following androgen deprivation therapy, which is more significantly correlated with the survival time of the patients than tumor grades and clinical classification.</p>

RESUMEN

Objective To investigate the association between functional polymorphism IVS8-poly (T) in cystic fibrosis transmembrane conductance regulator (CFTR) gene and prostate cancer risk.Methods A population-based case-control study was conducted including 230 prostate cancer patients and 230 agematched controls.PCR-STRP was used to analysis the IVS8-poly (T) tract in CFTR gene.Results The prevalences of the low expression allele,the IVS8-5T,in prostate cancer patients and controls were 1.52 % (7/460) and 5.22 % (24/460),respectively (x2 =12.489,P =0.002).Logistic regression analysis confirmed the 5T/7T genotype was in protective association with prostate cancer risk (OR =0.232,P =0.003,95 % CI0.090-0.599).Conclusion The low expression CFTR IVS8-5T allele contributes to a reduced risk of prostate cancer in Chinese Han population,and may be a protective factor against prostate cancer.

RESUMEN

<p><b>OBJECTIVE</b>To study the chemical constituents contained in Acorus calamus.</p><p><b>METHOD</b>The chemical constituents were separated and purified by various chromatographic methods including silica gel, ODS, HPLC and Sephadex LH-20, and their structures were identified on the basis of analysis on spectroscopic data.</p><p><b>RESULT</b>Ten compounds were separated from A. calamus and identified as 1beta, 4beta, 7alpha-trihydroxyeudesmane (1), bullatantriol (2), teuclatriol (3), threo-1', 2'-dihydroxyasarone (4), erythro-1', 2'-dihydroxyasarone (5), (+)-de-4'-O-methyleudesmin (6), (+)-de-4'-0-methylmagnolin (7), (+)-eudesmin (8), (+)-magnolin (9) and beta-sitosterol (10), respectively.</p><p><b>CONCLUSION</b>Compounds 1-2,4-9 were separated from this plant for the first time. Specifically, compounds 1-2,6-9 were obtained from Acorus genus for the first time.</p>

RESUMEN

Objective To investigate the risk factors for conversion of laparoscopic cholecystectomy (LC) to laparotomy.Methods In 1020 LC patients,36 patients with conversion of LC to laparotomy were chosen as the case group,108 patients with successful LC were chosen as the control group.Univariate analysis and Logistic multivariate regression model were used to analyze the risk factors for conversion of LC to laparotomy.Results Age > 65 years (0R=3.234,95% CI:0.532-6.853),course of disease > 72h (OR =2.342,95% CI:0.568 ~ 5.656),history of upper abdominal operation (OR =2.453,95% CI:0.345-7.453),thickness of gallbladder wall ≥ 6 mm (OR =2.453,95% CI:0.453-6.343),white bloodcell count > 15.0 × 109/L (OR =4.532,95% CI:0.535-8.329) were risk factors for conversion of LC to laparotomy.Conclusion Preoperative comprehensive evaluation the risk factors and selecting suitable program have important clinical significant in reducing the rate of conversion LC to laparotomy.

RESUMEN

Background Clinical effectiveness of intavitreal injection of glucocorticosteroid for macular edema has been verified,especially triamcinolone acetonide(TA).However,the efficacy and safety of combination of TA with macular laser grid photocoagulation for macular edema is concerned. Objective This clinical trial was to evaluate the efficacy and safety of intravitreal injection of TA combined with macular laser grid photocoagulation in the treatment of macular edema. Methods A case-cohort study was designed.One hundred and twenty eyes of 120 patients with macular edema from diabetes or retinal vein occlusion were included in this study.The patients were randomized into trial group and control group,with the matched age,course,visual acuity,intraocular pressure (IOP).The patients of the trial group received intravitreal injection of TA combined with macular laser grid photocoagulation,and those of the control group were managed with macular laser grid photocoagulation only.Best corrected visual acuity ( BCVA),optical coherence tomography(OCT),fundus fluorescein angiography(FFA) and IOP were examined before TA injection and 1 week,1 month,3 months,6 months after treatment and compared among different time points between two groups.Written informed consent was obtained from each patient prior to entering this trial. Results Compared with TA injection before,the BCVA was significantly elevated in the trial group 1 week,1 month,3 months and 6 months after TA injection( all P=0.000),however,no obvious improvement of visual acuity was found in the control group before and after treatment at any time point (P>0.05 ).At various time points,the visual acuity was significantly improved in the trial group than the control group (P =0.037,0.000,0.002,0.046 ).Macular thickness was significantly decreased at various time points after TA injection in comparison with before TA injection in the trial group(all P=0.000),but no significant change in macular thickness in the control group between before and after treatment at any time point( P>0.05 ).Macular thickness was lower in the trial group compared with the control group at various time points after treatment ( P<0.05 ).Recurrence of macular edema was seen in 7 eyes ( 1 1.67% ) 4-6 months,and the IOP raise( >21 mmHg)was found in 11 eyes( 14.1% )after TA injection in the trial group.Conclusions Intravitreal injection of TA combined with macular laser grid photocoagulation can be an effective method in the treatment of macular edema.However,recurrence of macular edema or increase of IOP may occur in a few patients within 6 months after TA injection.A long-term follow-up should be performed for the evaluation of efficacy and safety after intravitreous injection of TA.

<p><b>OBJECTIVE</b>To evaluate the clinical significance of prostate-specific antigen (PSA) screening in early detection of prostate cancer in Chinese men.</p><p><b>METHODS</b>PSA screening was performed in 8562 asymptomatic men who had been enrolled for health checkup and all were > or = 50 years old. Prostate biopsy was recommended for those with a serum PSA level > or = 4.0 ng/ml. The pathological and clinical features of the patients with prostate cancer detected by the PSA screening were compared with that of 82 clinically diagnosed prostate cancer patients during the same period.</p><p><b>RESULTS</b>Of the 8562 asymptomatic men, 719 had PSA levels > or = 4.0 ng/ml and biopsy was performed in 295 of them. Fifty-eight prostate cancers were detected. The biopsy rate was 41.0% and positive detection rate was 19.7%. The overall age distribution in the screening group and the clinical groups was not significantly different (P = 0.176). However, 41.4% (24/58) of the patients in screening group were > 75 years old, and significantly more than that in the clinical group (25.6%, P = 0.0491). The proportion of the patients with PSA levels > or = 20 ng/ml in the screening group was significantly less than that in the patients of the clinical group (44.8% vs. 75.6%, P = 0.0002). Whether in the patients whose age was > 75 years old (P < 0.05) or < or = 75 years old (P = 0.0002), the patients in the screening group had significantly lower Gleason scores < 7 (60.3% vs. 34.1%, P = 0.002), more T1 or T2 tumor (87.9% vs. 26.8%, P < 0.0001) and more chance to receive radical prostatectomy (50.0% vs. 18.3%, P < 0.0001) than the patients in the clinical group did. However, the distributions of PSA levels at diagnosis and biopsy Gleason scores were not significantly different between the above mentioned two groups (P > 0.05).</p><p><b>CONCLUSION</b>Prostate-specific antigen (PSA) screening is useful for early detection of prostate cancer in Chinese men aged > or = 50 years. The patients detected by PSA screening usually show a lower PSA level, Gleason scores and early clinical stage disease, and have more chance for radical prostatectomy than the clinically diagnosed patients.</p>
