RESUMEN

Objective:To investigate the relationship between iodine nutrition level and thyroid disease.Methods:Totally 299 patients with thyroid disease who were treated at Shandong Provincial Institute for Endemic Disease Prevention and Control from 2016 to 2018 were selected as case group which was further divided into Graves' disease group (GD group, 137 patients), chronic lymphocytic thyroiditis group (HT group, 90 patients) and thyroid nodule group (72 patients). At the same time, 75 healthy people with no history of thyroid disease, normal thyroid color ultrasound and thyroid function were selected as control group. Morning urine was collected and urinary iodine was detected by arsenic cerium catalytic spectrophotometry. Fasting venous blood was extracted, and serum levels of thyroid stimulating hormone (TSH), free triiodothyronine (FT 3), free thyroxine (FT 4), thyroid globulin antibody (TgAb) and thyroid peroxidase antibody (TPOAb) were detected by electrochemiluminescence method. Results:The difference of median urinary iodine in the 4 groups was statistically significant ( H = 42.530, P < 0.05). The medians urinary iodine in GD and HT groups (326.79, 341.91 μg/L) were higher than those of thyroid nodule group and control group (235.01, 187.32 μg/L, P < 0.05). The levels of TSH, FT 3 and FT 4 in GD group were compared with those of control group, and the differences were statistically significant ( P < 0.05). The positive rates of TgAb and TPOAb in HT group were significantly higher than those in GD, thyroid nodule and control groups, and the positive rates of TgAb and TPOAb in GD group were higher than those in thyroid nodule and control groups ( P < 0.05). Conclusions:GD and HT patients have excessive iodine nutrition, and high iodine intake may lead to the occurrence of these thyroid diseases (GD and HT). Thyroid function test combined with laboratory urinary iodine test can be used to diagnose thyroid diseases simply and quickly.

Objective:To investigate the iodine nutritional status of key population in waterbrone high iodine areas in Shandong Province and provide scientific basis for adjustment of control strategy.Methods:In 2018, the county (city, district, county for short) shall be the unit to carry out the monitoring work, and the administrative villages shall be the unit to determine the monitoring sites in the designated high-iodine areas. According to the results of the 2017 survey on drinking water iodine for residents of Shandong Province, each monitoring county sorted the administrative villages with a median water iodine above 100 μg/L according to the water iodine value and adopted a systematic sampling method. Each county took 5 administrative villages, and if there were less than 5 administrative villages, they were all extracted (if there were administrative villages with median water iodine above 300 μg/L, ensured at least one extraction). Among the monitoring sites, water iodine, children urinary iodine, salt iodine and thyroid volume, pregnant urinary iodine, and salt iodine were investigated. The "Arsenic Cerium Catalytic Spectrophotometry Method" (WS/T 107-2006) was used to detect the water iodine and urinary iodine contents; the B-ultrasound method was used to examine the thyroid volume of children; the semi-quantitative method was used to detect the salt iodine level of residents.Results:A total of 341 water samples were collected, the median water iodine was 131.15 μg/L, ranging from 7.10 to 1 054.00 μg/L. A total of 7 555 urine samples of children were tested and the median urinary iodine of children was 289.20 μg/L. The rate of goiter of children was 3.10% (234/7 555). A total of 1 996 urine samples of pregnant women were tested and the median urinary iodine of pregnant women was 179.90 μg/L. A total of 9 551 edible salt samples were collected from children and pregnant women, including 8 238 non-iodine salt samples and the consumption rate of non-iodine salt was 86.25% (8 238/9 551).Conclusions:The pregnant women are in a suitable state of iodine nutrition, the children's urinary iodine is at a higher than suitable level in waterbrone high iodine areas in Shandong Province. Therefore, it is still necessary to strengthen the efforts to improve water and reduce iodine, and strengthen the monitoring of iodine nutrition status of key populations.

Objective To evaluate the relationship between the degree of brain injury during the perioperative period of liver transplantation and postoperative cognitive dysfunction (POCD).Methods Thirtythree patients,undergoing elective liver transplantation,were enrolled in this study.Before induction of anesthesia (T0),at 5 min before blocking the portal vein (T1),5 min after opening the portal vein (T2),5 min after opening the hepatic artery (T3),and at 24 h after surgery (T4),blood samples were collected from the central vein for determination of the serum concentrations of S1O0β protein and neuron-specific enolase (NSE) by enzyme-linked immunosorbent assay.Patients were divided into POCD group and control group (group C) according to whether POCD happened within 7 days after surgery.Results Compared with the baseline value at T0,the serum concentrations of S100β protein were significantly increased at T2 and T3,and the serum concentrations of NSE was increased at T3 (P<0.05).There was no significant difference in serum concentrations of S100β and NSE at each time point between group POCD and group C (P>0.05).Conclusion The degree of brain injury during the perioperative period of liver transplantation is not the dominant factor for the development of POCD in the patients.

Objective To investigate the bacteriostasis effect of combination of Shuanghuanglian powder with piperacillin/tazobactam or cefoperazone/sulbactam for extensively drug-resistant Acinetobacter bauman-nii in vitro.Methods The minimum inhibitory concentration(MIC)and partial inhibitory concentration(FIC) index of 30 clinical isolates of Acinetobacter baumannii were measured with different concentrations of Shuan-ghuanglian,piperacillin/tazobactam,cefoperazone/sulbactam or combination.The effect of combined medica-tion was determined by FIC index.Results After shuanghuanglian combined with piperacillin/tazobactam,no strain showd synergistic effect;16.7% of the strains showed additive effect;83.3% of the strains showed ir-relevant effect;no strain showed antagonistic effect.After Shuanghuanglian combined with cefoperazone/sul-bactam,23.3% of the strains showed synergistic effect;73.3% of the strains showed additive effect;3.3% of the strains showed irrelevant effect,no antagonistic effect was shown.Conclusion The antibacterial effects of Shuanghuanglian and piperacillin/tazobactam were mostly irrelevant,while the antibacterial effect of Shuang-huanglian and cefoperazone/sulbactam are mostly synergistic and additive effect,w hich had better antibacterial effect to Acinetobacter baumannii in vitro.

Objective To summarize and analyze the epidemiological and clinical characteristics of patients with subacute thyroiditis (SAT),so as to provide a basis for prevention and treatment of the disease.Methods The epidemiological and clinical datas of 121 SAT patients,who were treated at the Shandong Provincial Institute for Endemic Disease Prevention and Control in 2015,were collected.The SAT patient's age,sex,lived in urban or rural areas,occupation and time of onset distributions were analyzed retrospectively.According to the clinical manifestations,the SAT patients were divided into types Ⅰ (mild/nodule type),Ⅱ (medium/normal),Ⅲ (severe/hyperthyroidism),and Ⅳ (delay),and clinical manifestations,auxiliary examination findings,treatment and outcome were summarized.Results Average age of the 121 cases of SAT patients was 41.36 years old,the youngest was 10 years old and the oldest was 76 years old.Most of the patients were 30-< 50 years old,which was accounted for 55.37% (67/121).Most of the patients were female,which was accounted for 90.08% (109/121).Most of the patients lived in rural areas,which was accounted for 59.50% (72/121).Majority of the SAT patients were farmers,which was accounted for 43.80% (53/121).Most onset of the disease was in November,accounted for 19.83% (24/121),and the least was in June,accounted for 2.48% (3/121),the peak of the incidence of SAT lied in autumn,and it was sporadic in the rest of the year.From the clinical classification,the mild type (61) and the medium type (46) were the largest part,a total of 107 cases,accounted for 88.43% (107/121);while the severe type had 5 cases,accounted for 4.13% (5/121);the delay type had 9 cases,accounted for 7.44% (9/121).The clinical manifestations were as follows:anterior neck discomfort was 109 cases,accounted for 90.08% (109/121);sore throat was 93 cases,accounted for 76.86% (93/121).Auxiliary examination findings were as follows:thyroid iodine taken rate reduced in 121 cases,accounted for 100.00% (121/121);thyroid nodule 96 cases,accounted for 79.34% (96/ 121);total number of white blood cells increased in 52 cases,accounted for 42.98% (52/121);neutrophil increased in 61 cases,accounted for 50.41% (61/121);blood sedimentation significantly speed up in 49 cases,accounted for 40.50% (49/121).All of the 121 patients were cured.Conclusions Majority of the patients are female,young aduhs and farmers.The peak of the incidence of SAT lies in autumn.The proportion of mild,medium type is the highest,we suggest this disease should be early diagnosis and treatment.

<p><b>OBJECTIVE</b>To examine the contribution of insulin related indices on the association between body fat and blood pressure among overweight adults.</p><p><b>METHODS</b>From April to May 2014, based on convenience sampling, we recruited overweight and obese volunteer participants aged 20-55 years living in Beijing at least 1 year through a strict examination by doctors in a physical examination center. In this study, we excluded the participants who reported suffering from any severe heart, lung, liver or kidney organic diseases, and abnormal development, disabilities, and secondary obesity caused by other disease. Also participants with use of antihypertensive drugs, hypoglycemic drugs and lipid lowering drugs were excluded for this study. A total of 1 221 participants were investigated in this study. With a simple self-designed questionnaire, the birthdates, sex, drug use, and disease history were examined. Participants' blood pressure (BP), percentage of body fat (PBF), glucose and fasting insulin level were measured. Mediation analysis was used to analyze the total effect of PBF on BP (c), the association between PBF and insulin related indices (a), and the mediation effect of serum fasting insulin level/HOMA-IR/HOMA-%S on relation between PBF and systolic/diastolic blood pressure (SBP/DBP).</p><p><b>RESULTS</b>PBF was positively associated with SBP (c=0.25 ± 0.05 and 0.19 ± 0.03 for male and female, respectively, P<0.001). In males, PBF was positively associated with fasting insulin level and HOMA-IR (a=0.28 ± 0.05 and 0.24 ± 0.05, P<0.001), and negatively associated with HOMA-%S (a=-0.29 ± 0.05, P<0.001); in females, PBF was positively associated with fasting insulin level, HOMA-IR (a=0.21 ± 0.04 and 0.20 ± 0.04, P<0.001), and negatively associated with HOMA-%S (a=-0.13 ± 0.04, P<0.001). In further mediation analysis for female participants, fasting insulin level/HOMA-IR/HOMA-%S played mediation roles in the relation between PBF and SBP, with ratio of mediation of 13.78%,18.3%, and 5.98%. Fasting insulin level/HOMA-IR also mediated the relation between PBF and DBP, with mediation ratio of 11.98% and 14.13%.</p><p><b>CONCLUSION</b>In overweight/obese female participants, insulin related indices mediated the relation between PBF and BP.</p>

RESUMEN

Mastering peripheral nerve block technique is a basic requirement for residents stan-dardization training. Ultrasound-guided nerve block has broken through the limitation of traditional blind operation, promoted the quality and safety of anesthesia, and improved the quality of the peripheral nerve block teaching. However, the safety of ultrasound-guided nerve block technique must be based on the speci-fication training. In the teaching experience, we have used theoretical study, simulation training and clinical practice to explore a new method for upgrading the quality of resident doctor training.

<p><b>OBJECTIVE</b>To explore the effect of weight-loss for overweight or obese population under the health management programs, and to provide evidence for the development of safe and effective weight-loss programs.</p><p><b>METHODS</b>738 participants who resided in Beijing longer than one year, under 22 to 55 years of age, with BMI≥24 kg/m(2) and without organic diseases were voluntarily recruited, from September 1(st) to October 15(th) 2013. All the participants were randomly divided into general management group or under health management group, in which all the subjects received intervention measures for 6 months. Anthropometry and body composition were measured at baseline and 6 months for all the subjects. Weight-loss effect from the health management programs was evaluated through analyzing the changes on weight and body fat.</p><p><b>RESULTS</b>618 participants completed the follow-up process, including 321 in the general management group and 297 in the health management group. 6 months after the intervention process, values of weight and body fat rate in the two groups appeared both significantly lower than that at the baseline levels. Difference before and after the intervention program was statistically significant (P<0.001). However, the differences of those with reduction value as 2.19 kg or 2.19% among health management group were higher than that in the general management group, which were 0.97 kg and 1.28% respectively, with difference statistically significant (P<0.001). The effective rate of 24.2% loss-weight and the 52.5% losing rate on body fat among the health management group were both higher than 11.8% and 34.3%, seen in the general management group, respectively, with difference statistically significant (P<0.001).</p><p><b>CONCLUSION</b>The healthy management programs could effectively control the weight and body fate rate among the overweight or obese adults.</p>

In male subgroup,PBF was negatively correlated with HDL-C level,but positively correlated with LDL-C level(β=-0.142 2,P<0.01 andβ=0.180 5,P<0.001),while in female subgroup,PBF was positively correlated with both TC level and LDL-C level(β=0.172 1,P<0.001 and β=0.233 5,P<0.001). With PBF controlled,TC and TG levels were positively correlated with DBP in both males and females(β=0.095 6,0.090 5 for males,β=0.117 6,0.083 1 for females,P<0.05),and TG level was positively correlated with SBP in females(β=0.127 2,P<0.001). Further analysis indicated there was a significant mediating effect of LDL-C on correlation between PBF and DBP in females,with the mediating effect value of 0.019 4 (P<0.05). The ratio of mediating effect was 9.26%. Conclusion PBF was positively correlated with blood pressure,but blood lipids had no mediating effect on the correlation between PBF and blood pressure in males,while LDL-C level had mediating effect on PBF and blood pressure in females.

RESUMEN

Objective To explore method of early diagnosis and treatment of adult patients with cerebral ischemic stroke after cardiovascular surgery.Methods The chnical data of 24 adult patients with cerebral ischemic stroke after cardiovascular surgery were retrospectively analyzed.Firstly,CT or MRI should be accomplished to determine the type of cerebral ischemic stroke as soon as patients' condition of circulation and respiration were stable.Secondly,the vital signs should be monitored closely,and the consciousness,pupil,respiratory and hmbs activity of the patients were observed.Thirdly,the patients' temperature of head should be reduced and be given dehydration,anticoagulation,cholesterol-lowering medication,brain nutrition drugs,beta receptor blockers and other drugs.Overall,the balance of fluid,electrolytes and acid-base were maintained in the course of treatment.Results Among the 24 patients,male was 66.7% (16/24).Early cerebral ischemic stroke occurred in 6 cases,delayed cerebral ischemic stroke occurred in 18 cases.Cerebral ischemic stroke happened in 12 patients who underwent coronary artery bypass grafting surgery,8 patients after cardiac valve replacement surgery,2 cases after artery dissection surgery and 2 patients after other surgery.Two cases were death during hospital stay,the mortality was 8.3% (2/24).Conclusion Adult patients with clinical manifestation of cerebral ischemic stroke after cardiovascular surgery should be diagnosed early as soon as possible,the treatment key of cerebral ischemic stroke is strict monitoring and comprehensive treatment.

Objective To evaluate the role of c-AMP-protein kinase A (cAMP-PKA) on the up-regulation of heme oxygenase-1 (HO-1) expression during lipopolysaccharide (LPS)-induced acute lung injury (ALI) in rats.Methods Forty-eight healthy male Sprague-Dawley rats,weighing 180-220 g,aged 2.5-3.0 months,were randomly divided into 4 groups (n =12 each)∶ normal control group (group C),ALI group (group ALI),H89 +ALI group (group H + ALI) and H89 group (group H).In group C,normal saline (solvent for LPS) 0.5 ml was injected via the femoral vein and normal saline (solvent for H89) 0.5 ml was injected subcutaneously 2 h later.In group ALI,10 mg/kg LPS 0.5 ml was injected via the femoral vein and normal saline 0.5 ml was injected subcutaneously 2 h later.In group H +ALI,10 mg/kg LPS 0.5 ml was injected via the femoral vein and 5 mg/kg H89 0.5ml was injected subcutaneously 2 h later.In group H,normal saline 0.5 ml was injected via the femoral vein and 5 mg/kg H89 0.5 ml was injected subcutaneously 2 h later.The rats were then sacrificed at 6 h after iv injection of LPS and the lungs were removed for microscopic examination and lung water content.The pathological changes of the lung were scored.The expression of HO-1 and PKA (by Western blot) and HO-1 mRNA (by RT-PCR) was detected.Results Compared with group C,the pathological score and lung water content were significantly increased,and the expression of HO-1,PKA and HO-1 mRNA was up-regulated in groups ALI and H +ALI (P <0.05),and no significant change was found in the parameters mentioned above in group H (P > 0.05).The pathological score and lung water content were significantly higher,and the expression of HO-1,AP-1 and HO-1 mRNA was significantly lower in group H + ALI than in group ALI (P < 0.05).Conclusion Activation of signaling pathway c-AMP-PKA is involved in the up-regulation of HO-1 expression during LPS-induced ALI in rats.

Objective To investigate the application of admission test as a medical management in the labor room.Methods Eight hundred and fifty singleton pregnant women underwent admission test in labor room at Women's Hospital,School of Medicine,Zhejiang University from Dec.2009 to Dec.2011 were enrolled into this study.They were divided into two groups:normal group (admission test was normal,n =658) and abnormal group (admission test was abnormal or undetermined,n =192).Deliveries and perinatal outcomes of them were observed.The difference between the two groups were compared with two independent sample t test and Chi-square test.Results There were no significant differences in the mean age at delivery of the women and mean birth weight of neonates between the two groups (all P>0.05).The cesarean section rate of normal group (34.2%,225/658) was lower than that (59.4%,114/192) of abnormal group (x2=3.93,P<0.05).The rates of prematurc rupture of membranes (72.4%,139/ 192),fetaldistress (42.2%,81/192),neonatal asphyxia (16.5%,27/192) and neonatal complications (31.8%,61/192) in abnormal group were significantly higher than those [31.0% (204/658),24.2% (159/ 658),0.2%(1/658) and 2.6% (17/658)] in normal group (x2 =105.78,40.84,52.54 and 151.92,P<0.01respectively).Conclusions Admission test is a good method to forecast placental function during labor and perinatal outcomes,which might play an important role in medical management in labor room.

Objective To analyze the feature of B cell epitope of soluble liver antigen(SLA) in autoimmune hepatitis. Methods Twenty-one autoimmune hepatitis(AIH) were enrolled the study. The control groups included 15 primary biliary cirrhosis(PBC) patients, 20 chronic virus hepatitis patients and 20 heaths. The serum incubated with coated one dimension of SLA peptide pools by ELISA for mapping the B cell epitopes. ELISPOT was performed to screen the SLA specific T cell response by observing the secretion of IFN-γ after PBMC simulated. Results Ten serum samples from AIH patients recognized the SLA peptide pools, the frequency was 47.62%, which was higher than PBC group, virus hepatitis group and normal control group, P value was 0.001, <0.001 and <0.001, respectively. The 10 positive serum dominant response to SLA 7 th pool(aa385-441), which laid in the unique no conservative region of the SLA molecular. T cell recognizing regions laid dominantly in first and second pool, the frequency was 30% and 40%, respectively, however the recognized frequency of 7th pool was 10%. Conclusion B cell linear epitopes lie in the sequence aa385-441, which is the unique unconservative region in the high conservative SLA molecular and overlaps with part sequence of T cell epitopes. The results show it is complex and interaction that SLA induced humoral-mediated immunity and cellular immunity to involve in AIH pathogenesis.

Objective To analyze the feature of SLA specific T cell response in AIH. Methods Thirtyone cases of AIH were enrolled by investigating T-cell reactivity against chemically synthesized peptides spanning the full SLA protein using ELISpot. The 31 cases of AIH included 10 anti-SLA/LP positive cases and 21 negative cases. The control groups included 30 PBC patients, 29 chronic viral hepatitis patients and 30 healthy cases. The secretion of IFN-γ after PBMC stimulated by SLA peptides was observed. Results Eighteen of 31 cases with AIH [56. 08 % ( 18/31 ) ] showed the positive response to SLA peptide pools, and only 1 of 30 patients with primary biliary cirrhosis (3.34%) and 1 of 29 patients with virus hepatitis ( 3.44% ), could be elicited responses by SLA peptide pools. There wasn't positive response in healthy eases. The response frequency to SLA peptides in AIH group was significantly higher than those in PBC cases, chronic viral hepatitis cases and healthy cases (X2 = 21. 295, 20. 655, 15.988, P < 0. 01 ). Amongst 18 AIH patients with positive responds to SLA pool, 8 antigen clusters including aa 1-44, aa 57-132, an 129-180,aa 177-196, aa 193-244, aa 241-268, aa 281-308 and aa 321-428 were highlighted. The mean number of recognized peptides was 6 (2-17), indicating the polyclonal feature. Fourteen of 18 AIH patients with positive response to SLA peptides were subjected to live function test simultaneously when PBMC were collected. There was significant correlation between the breadth of recognized poptides and AST ( logarithm, r = 0. 539, P = 0. 045). Conclusions SLA peptides can induce PBMC in peripheral blood of AIH patients to secrete IFN-γ and it is polyclonal response. The breadth of recognized peptides may reflect the degree of liver inflammation.

RESUMEN

6 OSAS patients wore the appliance. Polysomnographic monitoring and cephalometric analysis were performed before and after treatment to analyze the effect of the appliance. With the appliance on after one month, the patients felt better. The values of AHI decreased significantly(P<0.01), and the lowest SaO_2 increased significantly(P<0.01).Cephalometric analysis showed a widened airway(P<0.05)after treatment. The results showed that TMJ appliance is an easy and effective treatment of OSAS.

Objective To establish the enzyme-linked immunosorbent assay (ELISA) for the detection of serum alcohol dehydrogenase (ADH) antibody and evaluate its role in its diagnosis of autoimmune hepatitis( AIH ). Methods The reactivity between yeast ADH and human anti-ADH serum antibody was tested by Western blot analysis. ELISA was established using yeast ADH. The method was applied in serums of 67 AIH patients,94 primary biliary cirrhosis(PBC) patients, 199 chronic hepatitis B (CHB) patients, 132 chronic hepatitis(CHC) patients, 24 alcohol hepatitis disease(ALD) patients, 99 connective tissue disease(CTD) patients and 31 healthy individuals. The positive rate of ADH antibody in the patients and healthy individuals was measured. The χ2 test was used to compare the positive rates. Results The ELISA method for detecting human anti-ADH serum antibody was established successfully and the optimum reaction conditions were defined. Western blot showed that yeast ADH has cross reactivity with human anti-ADH antibody. The positive rate of anti-ADH antibody in the AIH group [59. 7% ,40/67 ] was higher than that in the normal control group(0,χ2 = 31. 271 ,P <0. 05), PBC group (6. 4% ,χ2 =54. 492,P <0. 05), CHB group( 14. 1% ,χ2 =54. 848,P <0. 05) ,CHC group(21.2% ,χ2 = 29.269,P<0.05), ALl) group ( 25. 0% ,χ2 =8.512,P <0.05)and CTD group ( 43. 4% ,χ2 =4.229, P <0. 05). Conclusions Compared with the PBC, CHB, CHC, ALD and CTD group, the anti-ADH antibody positive rate in the serums of AIH was significantly increased. The antibody may be helpful to the diagnosis of autoimmune hepatitis.

Objective To study the significance of antibody to double-stranded DNA (anti- dsDNA) in autoimmune hepatitis(AIH). Methods Anti-dsDNA were detected by immunological blotting. According to the anti-dsDNA, the patients were separated two groups: anti-dsDNA positive group and anti-dsDNA negative group. The clinical characteristics were compared between anti-dsDNA positive AIH group and negative group. Results Among the 43 serum samples, 13 (30.23%) eases with anti-dsDNA positive were found. The average level of AST(aspartate aminotransferase) and TBIL(total bilirubin) were (647.56 ±529.77) IU/L and (10.81± 8.08) rag/L, respectively, which was higher than that of the control group (P < 0.05). PTA (prothrombin activity) was 75.72%± 30.23 %, lower than that of the control group(P < 0.05). The liver cirrhosis rate in the anti-dsDNA positive AIH group was 61.5% (8/13), ,significantly higher than that of the control group. The time from the onset to liver cirrhosis was significantly shorter in an- ti-dsDNA positive AIH group than that of control group by the survival analysis and log rank test (P = 0. 0074). Conclusion AIH patients with anti-dsDNA positive generally associate with the serious liver damage, and prognosis is worse.

Objective:To clarify the treatment effects of T4K on Class II division 1 malocclusion.Methods:Twenty patients (10 girls and 10 boys) aged 8~10 years old with Class II division 1 malocclusion were treated with T4K appliance(Myofunctional Research Co., Queensland, Australia).A control group of 20 patients aged 9~11 years old with untreated Class II division 1 malocclusions was used to eliminate the possible growth effects. Lateral cephalograms were taken before and after treatment. Final cephalograms were taken (12.1?1.8) months after T4K application.The subjects in control group were observed for (11.2?2.1) months.The mean and standard deviations for cephalometric measurements were analyzed by paired-samples and independent-sample t-tests. Results:In the treated subjects significant changes including anterior rotation and sagittal growth of the mandible, increased SNB and facial height, reduced ANB, increased lower incisor proclination, upper incisor overjet reduction were observed.Conclusion:T4K appliance is effective in the treatment of class II division 1 malocclusion.
