RESUMEN

Background:Preterm birth is the most common of neonatal morbidity and mortality. 85% of neonatal morbidity and mortality is due to premature birth. A large number of studies demonstrate an association between specific organisms and preterm delivery. Material And Methods:A retrospective cohort study of pregnant women who received antenatal care between august 2017 and November 2018 at our tertiary care hospital. 70 Cases with high risk for preterm labor were included in the study and 70 cases with term labor in the control group. High vaginal swabs were collected from the posterior fornix of the vagina. Result:This study shows that there is no association of preterm labor with age, parity and numbers of previous abortions. There are high chances of preterm birth in patients with history of previous preterm birth. 48(68%) patients with preterm labor had vaginal infection in which bacterial vaginosis is more common. Other contributing factors for preterm delivery: Anaemia and lower socio-economic status. Conclusion:In this study, a significant difference in the presence of vaginal infection in patients of preterm labor and term labor was found (P<0.05).Women at risk for preterm birth or preterm premature rupture of membranes because of vaginal infections should be screened, diagnosed, treated , reevaluated, and re-treated if necessary.

Purpose: To report the surgical outcomes in six patients of Helveston syndrome using a 揻our oblique� procedure. The popular methods for surgical management include superior rectus recessions alone or combined with superior oblique tenectomy. However, large angle exotropia correction would entail a higher risk of anterior segment ischemia when the superior rectus needs to be operated along with the horizontal recti. Hence, we evaluated the long-term results of this uncommon procedure. Methods: This was a retrospective review of six patients diagnosed to have manifest dissociated vertical deviation (DVD) with A pattern exotropia with bilateral superior oblique over action. All patients underwent horizontal muscle recessions/resections for exotropia along with bilateral posterior tenectomy of the superior oblique with inferior oblique anterior transpositioning. Results: The median age was 10 years (Range 5� years). The mean postoperative follow-up was 26 � 14.02 months (Range 12� months). The mean reduction in exotropia was from 36.5 � 21.06 PD (Range 15� PD) to 6.1 � 3.06 PD (Range 3� PD). The procedure corrected the A pattern from a mean 23 � 7 PD (Range 15� PD) to 7.6 � 3.2 PD (Range 3� PD). The average DVD in the right eye reduced from 14 � 4.3 PD (Range 8� PD) to 5.3 � 1.2 PD and in the left eye from 14.33 � 3.6 PD (Range 10� PD) to 4.1 � 1.1 PD. The DVD asymmetry reduced from 6.33 � 3.4 PD to 1.5 � 1.3 PD. Conclusion:擣our oblique� procedure with horizontal muscle surgery seems to be an effective method for significantly correcting the A pattern as well as reducing the DVD with good long-term outcome in our case series.

RESUMEN

The study was carried out to determine the effectiveness and safety of the infrared 1.48 microm laser in cleavage stage mouse embryo biopsy, compared to the conventional acid Tyrode's solution. One hundred and thirty cryopreserved cleavage stage mouse embryos were included in the study. Fifty embryos were biopsied by acid Tyrode's solution. Forty-seven embryos were biopsied by the infrared 1.48 microm laser. Thirty-three embryos were incubated without biopsy as the control group. Thirteen of 50 embryos in the acid Tyrode's group and 16 of 47 in the laser assisted group became cavitating morulae on day 4, meanwhile 23 of 33 in the control group reached this stage. The blastocyst formation of acid Tyrode's, laser assisted and control group were 94.0, 97.8 and 100.0 per cent, respectively. The hatching rate of acid Tyrode's solution, laser assisted and control group were 78.7, 84.7 and 63.6 per cent, respectively. No significant difference in blastocyst formation and hatching rate was found. The percentage of grade I blastocysts in control group (96.9%) was significantly higher than those in acid Tyrode's solution (68.0%) and the laser assisted group (76.0%). There was no significant difference in the percentage of grade 1 blastocysts between the acid Tyrode's solution and the laser assisted group. In conclusion, the infrared 1.48 microm wavelength laser may be an alternative to acid Tyrode's solution in embryo biopsy.

RESUMEN

Amyloidosis is a generic term for a heterogeneous group of disorders associated with deposition of protein in an abnormal fibrillar form. Amyloidosis can be hereditary or acquired, localised or systemic and potentially lethal or merely an incidental finding. We report a case of a male who was totally asymptomatic and was incidentally detected to have lung amyloidosis. He was not given any treatment but kept under observation., His chest radiograph (CXR) and high resolution computed tomograph (HRCT) of the chest cleared spontaneously after one year and the patient continues to remain asymptomatic even after two years.

RESUMEN

A 49-year-old woman was diagnosed as autoimmune hepatitis and started on steroids and azathioprine. Subsequently, she developed fever; chest radiograph showed lower lobe nodular opacities. Bronchoalveolar lavage and transbronchial lung biopsy confirmed the diagnosis of lymphocytic interstitial pneumonitis.

RESUMEN

We report a case of a 3-year-old child with Prader-Willi syndrome who had severe sleep disordered breathing with daytime hypersomnolence. His daytime blood gases showed type II respiratory failure. He was effectively treated with continuous positive airway pressure (CPAP) and has used this form of therapy for 2 years now with improvement in daytime somnolence, improved mental acuity and normalisation of daytime blood gases.

RESUMEN

Post cardiac injury syndrome (PCIS) is known to occur following myocardial infarction, cardiac surgery, blunt chest trauma, percutaneous left ventricular puncture and pace-maker implantation. The diagnosis is one of exclusion. We report a case of PCIS following cardiac surgery who showed false positive IgG, IgM antibodies to antigen A60 of Mycobacterium tuberculosis in pleural fluid.
