Resumen Antecedentes: El lupus eritematoso sistémico (LES) es una enfermedad auto inmunitaria crónica multisistémica con diversas manifestaciones clínicas. Siendo las mujeres la pobla ción vulnerable y con mayor afectación a nivel neurológico, al presentar mayor riesgo de convulsiones. Las manifestaciones neuropsiquiátricas ocurren en etapas tempranas de la enfermedad y del diagnóstico, ya que pueden presentarse junto con manifestaciones sistémicas o no. La frecuencia de manifestaciones neuropsiquiátricas en el lupus eritematoso sistémico se ha descrito del 14 al 75%, siendo las alteraciones cognitivas uno de los grandes síntomas a destacar1. La cual puede ir acompañada de trastornos afectivos de tipo depresión y ansiedad. Ya que la psicosis secundaria a LES se remarca por su baja prevalencia (10%)2, los estudios de laboratorio nos suelen orientar hacia el diagnóstico definitivo, siendo los anticuerpos ribosomales P los que se han relacionado más específicamente con la psicosis lúpica. La resonancia magnética es la prueba de elección y las lesiones cerebrales están dominadas por hiperintensidades de materia blanca en forma de punción3. En el siguiente reporte de caso, presentamos a una paciente de 20 años, la cual contaba con antecedentes de esteatosis hepática diagnosticado, diabetes tipo MODY y resección de ovario derecho por teratoma maduro de 9 años de evolución, pero sin antecedentes psiquiátricos de importancia para el momento de su valoración. Sin embargo, de forma aguda presentó un brote psicótico caracterizado por ideas delirantes de grandiosidad y referencia, así como alteraciones conductuales, cognitivas y afectivas. Por las que tuvieron que acudir a hospital de 3er nivel durante el periodo de contingencia sanitaria en el 2020. Tras el antecedente de presentar infección por SARS-CoV-2 tres meses antes de su patología neuropsiquiátricas. Se sospechó en síntomas neurológicos secundarios a infección por COVID-19, así como patología psiquiátrica aislada. Por lo que se realizó abordaje de estudio de primer brote psicótico, diagnosticándose lupus eritematoso sistémico con manifestaciones neuropsiquiátricas. El tratamiento se basó en un bolo de metilprednisolona y antipsicóticos, luego modificada por terapia con corticoesteroides orales y antipsicótico de depósito. Conclusión: El lupus eritematoso sistémico con manifestaciones neuropsiquiátricas es una presentación poco frecuente del padecimiento, por la amplia variación en la aparición de este, los pacientes con síntomas psiquiátricos en contexto de hospital general deben de ser tomados en cuenta para abordajes extensos4. De la misma forma, el tener este conocimiento del caso podrá ampliar nuestro conocimiento sobre las complicaciones de esta patología reumatológica. Y una de sus complicaciones más graves como la psicosis lúpica para poder realizar un mejor abordaje del primer brote psicótico en hospitales generales, donde la valoración de un especialista puede ser más complicada para mejorar las condiciones médicas de estos pacientes.


Abstract Background: Systemic lupus erythematosus is a chronic multisystemic autoimmune disease with diverse clinical manifestations. Women are the most vulnerable population and have the greatest neurological involvement with a higher risk of seizures. Neuropsychiatric manifestations occur in early stages of the disease and diagnosis since they can occur together with systemic manifestations or not. The frequency of neuropsychiatric manifestations in systemic lupus erythematosus has been described from 14 to 75%; being cognitive alterations one of the major symptoms to highlight. Which, in the same way can be accompanied by affective disorders such as depression and anxiety. Since psychosis, secondary to SLE, stands out for its low prevalence (10%), laboratory studies usually guide us towards a definitive diagnosis, being ribosomal P antibodies the ones that have been more specifically related to lupus psychosis. MRI is the test of choice and brain lesions are dominated by punctate white matter hyperintensities. In the following case report, we present a 20-year-old patient who had a history of diagnosed hepatic steatosis, MODY type diabetes and resection of the right ovary for mature teratoma of 9 years of evolution; but with no psychiatric history of importance at the time of her evaluation. However, she acutely presented a psychotic outbreak characterized by delusions of grandiosity and reference; as well as behavioral, cognitive, and affective alterations. For which she had to go to a 3rd level hospital during the period of health contingency in 2020. After a history of SARS-CoV-2 infection three months before her neuropsychiatric pathology, neurological symptoms secondary to COVID-19 infection were suspected, as well as isolated psychiatric pathology. Therefore, a study approach of the first psychotic outbreak was performed, diagnosing systemic lupus erythematosus with neuropsychiatric manifestations. Treatment was based on a bolus of methylprednisolone and antipsychotics; later modified by therapy with oral corticosteroids and depot antipsychotic. Conclusion: Systemic lupus erythematosus with neuropsychiatric manifestations is an infrequent presentation of the disease, because of the wide variation in its appearance, patients with psychiatric symptoms in a general hospital setting should be considered for extensive approaches. In the same way, having this knowledge of this case may broaden our knowledge about the complications of this rheumatologic pathology. And one of its most serious complications such as lupus psychosis to be able to make a better approach to the first psychotic outbreak in general hospitals, where the assessment of a specialist can be more complicated.

With the advancement in the study of keratoconjunctivitis sicca and the scope of its treatment, punctal plugs are being widely used for the therapeutic management of dry eye disease. With the emergence of 3D printing in medicine, 3D printing of punctal plugs that have an inbuilt drug delivery system and also that can be personalized from patient to patient according to their punctum size can be a great therapeutic option. Another benefit of the device is that its printing takes a short period of time and is cost-effective. This study aimed at making an open source design and 3D printing an efficient model of a punctal plug with an inbuilt drug delivery system that can be eventually used for the treatment of various ocular diseases that require frequent drug instillation or blockage of the nasolacrimal pathway. The 3D design for the punctal plug was made using the open source application, FreeCAD, and slicing was done using the application ChituBox. After that, the plugs were printed using the LCD printer Crealty LD-002R. The material used was resin that was compatible with the Crealty LD-002R. Punctal plugs with satisfactory results were printed using the LCD printer. The punctal plugs showed suitable structure and were also easily reproduced in the 3D printer without any complications or setbacks.

This study was aimed to isolate and characterize antibiotic producing fungi from the soil environment within Ahmadu Bello University main campus Samaru, Zaria. Soil samples were collected from five different locations within Ahmadu Bello University main campus for the Isolation of fungi. Spread plate method involving serial dilution technique was used for the Isolation using Saboraud Dextrose Agar (SDA). Six species of fungi were isolated from the soil samples and then characterized microscopically and macroscopically. The fungi were Aspergillus niger, Aspergillus fumigatus, Aspergillus sp, Penicillium sp, Fusarium sp (P14) and Fusarium sp (P15). Sensitivity test using Staphylococcus aureus, Escherichia coli and Klebsiella pneumoniae as test pathogens was employed to determine the ability of the fungal isolates to produce antimicrobials. All the fungal isolates were found to inhibit the growth of at least one of the test pathogens except Fusarium sp (P14). Aspergillus niger produces zones of inhibition of 9mm, 5mm and 6mm against Staphylococcus aureus, Escherichia coli, and Klebsiella pneumoniae, Aspergillus fumigatus produces zones of inhibition of 5mm against Escherichia coli and Klebsiella pneumoniae. Aspergillus sp produced zones of 6mm against Staphylococcus aureus and Escherichia coli, Penicillium sp which produces a zone of 10mm, 7mm and 6mm against Staphylococcus aureus, Escherichia coli and Klebsiella pneumoniae. The findings of this study show that antibiotic-producing fungus are prevalent in the soil of Ahmadu Bello University's main campus in Samaru, Zaria, and that these strains could be used by pharmaceutical companies to produce antibiotics from local sources.

Resumen Las enfermedades cardiovasculares y la depresión se consideran problemas de salud pública, la depresión es un factor de riesgo para pacientes con insuficiencia cardiaca (IC) por sus implicaciones clínicas. El objetivo de este estudio fue describir dos modalidades de intervenciones psicoterapéuticas en el abordaje de la depresión en pacientes con IC. Métodos de búsqueda: Se realizaron búsquedas en las bases especializadas MEDLINE, PubMed, PsycINFO, Web of Science, CINAHL y Cochrane Library de 2010 a 2020. Criterios de selección: Se seleccionaron los artículos por título y resumen, publicados en los últimos 10 años, escritos en castellano o inglés y se eliminaron los registros repetidos. Obtención de los datos: Tres autores recopilaron la información de los estudios que se documentan. Resultados principales: Se incluyeron 4 estudios clínicos con 911 pacientes, se realizaron en 4 países en los últimos 10 años y se implementaron diferentes métodos de intervenciones digitales y visitas domiciliarias, los efectos de estos tratamientos han sido estadísticamente significativos para reducir los puntajes de depresión autoinformada en pacientes con IC que recibieron la intervención psicoterapéutica en alguna de estas modalidades a comparación de aquellos pacientes que solo recibieron un tratamiento estándar. Conclusiones: Debido al número limitado de estudios y de los diferentes métodos de intervención no se identifica un tratamiento estándar de la depresión en IC en estos formatos de atención, por lo que se recomienda más investigación en este campo de estudio.


Abstract Cardiovascular disease and depression are considered public health problems, depression is a risk factor for patients with heart failure (HF) for its clinical implications. The objective of this study was to describe two modalities of psychotherapeutic interventions in the approach to depression in patients with HF. Search methods: We searched the specialized databases MEDLINE, PubMed, PsycINFO, Web of Science, CINAHL and Cochrane Library from 2010 to 2020. Selection criteria. Articles were selected by title and abstract., published in the last 10 years, written in Spanish or English and repeated registrations were deleted. Obtaining the data: Three authors collected information from the studies being documented. Main results: We included 4 clinical studies with 911 patients, were conducted in 4 countries in the last 10 years and implemented different methods of digital interventions and home visits, the effects of these treatments have been statistically significant to reduce self-reported depression scores in patients with HF who received psychotherapeutic intervention in any of these modalities compared to those patients who only received a treatment. Conclusions: Due to the limited number of studies and different methods of intervention, a standard treatment of HF depression in these care formats is not identified, so more research in this field of study is recommended.

Preventive programs are standardized behavioral interventions that either stop or reduce course of diseases. Indeed, it is known that much of public health programs are synonymous to preventive medicine, which constitute major theme in primary healthcare. The objective of this study was to provide preliminary summaries of research programs, which determined nutritional status on visual impairment (VI) among children,utilization of ITNs and reduction of HIV/AIDS infection in Nigeria. The studies adopted intervention approaches. In Delta state, antioxidant vitamins and ferrous sulphate were administered to participants, to assess impact on visual impairment. In health education on utilizationof ITNs in Bayelsa and Rivers states; as well as on HIV/AIDS infection in Northern Nigeria. Statistically, the studies followed a quantitative method. Baseline percentages of vitamin A and C deficiencies, anaemia, and VI differed with respective post-intervention levels. Post-intervention result showed that 92.1% owned ITNs, improved knowledge of causes and consequences of malaria. On HIV, community-based television, and radio awareness programs on the use of condom influenced the reduction of HIV/AIDS infection by up to 30%. In conclusion, the preliminary reports indicatethatantioxidant vitamins and iron supplementation ameliorated anaemia and VI. Public health awarenesscampaign influenced behavioural change wheel on use of ITNs as well asuse of condom for the reduction of HIV/AIDS infection in Nigeria.

objectives: This study aimed to examine the association between Family Adaptability, Partnership, Growth, Affection and Resolve (Family APGAR) and HIV treatment outcomes. Design: A cross-sectional study using the Family APGAR questionnaire Setting: The study was conducted in Kumasi, Ghana, at the Komfo Anokye Teaching Hospital and the Kwame Nkrumah University of Science and Technology Hospital Participants: Consenting HIV-positive patients who had been on treatment for at least 12 months were recruited. Main outcome measures: The Family APGAR questionnaire was administered, and relevant data were extracted from hospital records and analysed using STATA® software. The relationship between Family APGAR and treatment outcomes was determined using Chi-squared tests or Fisher's exact test. Results: Approximately 70.1% of 304 participants were females with a mean age of 41.8 years (±9.9). At treatment initiation, 47.4% of the patients presented at World Health Organisation (WHO) clinical stages I and II and had a CD4 count ≥ 200 cells/mm3 . Females were less likely (Odds Ratio= 0.52; 95% CI=0.31 ­ 0.90, p = 0.018) to report late for treatment compared with the males. After 12 months of treatment, approximately 70% recorded undetectable viral load. Patients with functional families constituted 70.4%, which had a statistically significant relationship with viral load (p = 0.041). Conclusion: HIV care providers should incorporate family functionality evaluation into clinical practice and provide early essential support to enhance treatment outcomes

Resumo: Introdução: O curso de graduação em Medicina expõe os alunos a uma quantidade significativa de estresse, o que pode gerar consequências negativas para o aprendizado, a motivação e o contato com os pacientes. A falta de empatia na relação médico-paciente pode dificultar a adesão ao tratamento e os resultados nele. Algumas técnicas e práticas de medicina integrativa (mente e corpo) têm sido indicadas para auxiliar no manejo e na redução do estresse. Essas intervenções que envolvem práticas de meditação e que já são utilizadas em escolas médicas podem auxiliar no desenvolvimento da empatia e na visão da integralidade do cuidado. Objetivo: Este estudo teve como objetivo investigar o efeito de um programa de Redução de Estresse e Desenvolvimento da Empatia na Medicina (Redemed©) no nível de empatia de estudantes de graduação em Medicina. Método: O programa foi composto por oito encontros de duas horas cada, envolvendo práticas de meditação, posturas de ioga e atividades de grupo direcionadas ao aperfeiçoamento de interações interpessoais. O grupo intervenção foi composto por 47 alunos, e o grupo controle, por 40 estudantes. Utilizou-se a Escala Jefferson de Empatia Médica, versão para estudantes (JSPE-S), para avaliar o nível de empatia antes e depois da intervenção. Resultado: O aumento do nível de empatia no grupo que recebeu a intervenção foi significativo quando comparado ao grupo controle (p: 0,000). Conclusão: A participação no programa Redemed© se mostrou eficaz no aumento da empatia entre estudantes de graduação de um curso de Medicina.


Abstract: Introduction: The undergraduate medical course exposes students to a significant amount of stress, which can have negative consequences for learning, motivation and contact with patients. Lack of empathy in the doctor-patient relationship can hinder adherence and treatment results. Some techniques and practices of integrative medicine (mind and body) have been indicated as aiding the management and reduction of stress. These interventions that involve meditation practices, and which are already used in medical schools, can help in the development of empathy and in the vision of comprehensive care. Objective: Investigate the effect of the Stress Reduction and Empathy Development in Medicine Program (REDEMED©) on empathy levels in undergraduate medical students. Method: The program consisted of eight 2-hour sessions with practices involving meditation, yoga postures, and group activities aimed at improving interpersonal interactions. The intervention group consisted of 47 students and the control group 40 students. Empathy was evaluated before and after intervention using the Jefferson Scale of Empathy - version for medical students (JSE-S). Result: Empathy level significantly increased in the group who received intervention compared to the control group (p: 0.000). Conclusion: Participation in the REDEMED© program showed an increase in empathy in undergraduate medical students.

BACKGROUND Leprosy is a chronic infectious disease, still endemic in many countries that may lead to neurological, ophthalmic, and motor sequelae if not treated early. Access to timely diagnosis and multidrug therapy (MDT) remains a crucial element in the World Health Organization's strategy to eliminate the disease as a public health problem. OBJECTIVES This systematic review aims to evaluate the accuracy of rapid point-of-care (POC) tests for diagnosis of leprosy. METHODS Searches were carried out in electronic databases (PubMed, EMBASE, CRD, Cochrane Library and LILACS) in April 2021 for patients with suspicion or confirmatory diagnostic of leprosy, classified in multibacillary (MB) or paucibacillary (PB) cases, performing rapid POC serological tests compared to clinical evaluation, smear microscopy and immunohistochemistry analysis. Methodological quality was assessed using the Quality Assessment of Diagnostic Accuracy Studies tool (QUADAS-2). A meta-analysis was undertaken to generate pooled estimates of diagnostic parameters, presenting sensitivity, specificity and diagnostic odds ratio (DOR) values. The review protocol was registered at PROSPERO, CRD # 42014009658. FINDINGS From 893 potentially relevant references, 12 articles were included reporting 16 diagnostic tests accuracy studies with 5395 individuals enrolled. Meta-analysis of NDO-LID and PGL-I tests data in MB patients showed sensitivity and specificity [95% confidence interval (CI)] of 0.83 (0.71-0.91), 0.91 (0.72-0.97); and 0.92 (0.86-0.96), 0.93 (0.78-0.98); respectively, with high heterogeneity among the studies. MAIN CONCLUSIONS Our results can inform policymakers regarding the possibility of implementing accurate, rapid POC tests for leprosy in public health services, especially within primary health care.

Resumen La cardiomiopatía de Takotsubo es una entidad caracterizada por disfunción ventricular aguda y transitoria, la cual está generalmente relacionada a un evento desencadenante (estrés emocional o físico) y que, por lo general, se presenta con disfunción sistólica regional del ventrículo izquierdo, aunque hasta en un 30% puede ser biventricular. Según su severidad, en algunos casos puede condicionar choque cardiogénico refractario a manejo con inotrópicos y vasopresores, por lo que para estos casos deben considerarse los dispositivos de asistencia circulatoria. Presentamos el caso de una paciente joven a quien se realizó cambio valvular pulmonar con prótesis biológica, la cual siete semanas posteriores a la cirugía acudió al servicio de urgencias con derrame pericárdico y fisiología de tamponade secundario a síndrome pospericardiotomía. Por tal motivo se le practicó ventana pericárdica, sin embargo durante el transquirúrgico presentó cardiomiopatía de Takotsubo biventricular que le condicionó choque cardiogénico con insuficiencia mitral y tricúspidea severas y refractariedad a tratamiento médico, así como a balón intraaórtico de contrapulsación (BIAC), por lo cual requirió soporte circulatorio con ECMO venoarterial durante 5 días.


Abstract Takotsubo cardiomyopathy is an entity characterized by acute and transient ventricular dysfunction, which is usually related to a triggering event (emotional or physical stress), and usually presents with regional systolic dysfunction of the left ventricle, however up to 30% may be biventricular. Depending on its severity in some cases the disease can condition refractory cardiogenic shock to management with inotropics and vasopressors, so for these cases circulatory assistance devices should be considered. We present the case of a young patient who had pulmonary valve change with biological prosthesis, which seven weeks after surgery went to the emergency department with pericardial effusion and tamponade physiology secondary to postpericardiotomy syndrome. For this reason pericardial window was practiced, however during the procedure she presented biventricular Takotsubo cardiomyopathy which conditioned cardiogenic shock with severe mitral and tricuspid regurgitation, and refractivity to medical treatment as well as intraaortic balloon pump, requiring circulatory support with venoarterial ECMO for 5 days.

OBJECTIVES: Positive fluid balance is frequent in critically ill patients and has been considered a potential biomarker for acute kidney injury (AKI). This study aimed to evaluate positive fluid balance as a biomarker for the early detection of AKI in critically ill patients. METHODS: This was a prospective cohort study. The sample was composed of patients ≥18 years old who stayed ≥3 days in an intensive care unit. Fluid balance, urinary output and serum creatinine were assessed daily. AKI was diagnosed by the Kidney Disease Improving Global Outcome criteria. RESULTS: The final cohort was composed of 233 patients. AKI occurred in 92 patients (40%) after a median of 3 (2-6) days following ICU admission. When fluid balance was assessed as a continuous variable, a 100-ml increase in fluid balance was independently associated with a 4% increase in the odds of AKI (OR 1.04; 95% CI 1.01-1.08). Positive fluid balance categorized using different thresholds was always significantly associated with subsequent detection of AKI. The mixed effects model showed that increased fluid balance preceded AKI by 4 to 6 days. CONCLUSION: These results suggest that a positive fluid balance might be an early biomarker for AKI development in critically ill patients.

Background: The use of pharmaceutical products in our day to day life is escalating and one cannot deny their presence in every household. Unfortunately not all the medicines that reach our homes get consumed. Unused, unwanted and expired drugs get accumulated over time and are generally disposed along with other household trash thus contributing to environmental pollution. This turns our attention towards the significance of eco-pharmacovigilance. Assessing the level of knowledge and awareness of students under training as health care professionals regarding safe disposal of pharmaceutical wastes can help us to partly assess the magnitude of the problem of inappropriate disposal methods and help us to plan and initiate steps to prevent the hazards caused by improper disposal of these items.Methods: A questionnaire based cross-sectional study was conducted among medical and paramedical students of a tertiary care teaching hospital using a pre-validated questionnaire from previous studies.Results: Majority of the participants were not aware of the possible hazards of improper disposal of pharmaceutical wastes. They expressed the need for awareness programs regarding the subject.Conclusions: The study revealed the practice of drug accumulation at home. The disposal methods opted by the participants were not the recommended methods. There is a need to address this issue through awareness programs at various levels.

Resumo: Introdução: O curso de graduação em Medicina expõe os alunos a uma quantidade significativa de estresse, o que pode gerar consequências negativas para o aprendizado, a motivação e o contato com os pacientes. Algumas técnicas e práticas têm sido indicadas para auxiliar no manejo e na redução do estresse, como é o caso da meditação que já é utilizada em escolas médicas. Este estudo avaliou os efeitos de um programa de redução do estresse e desenvolvimento da empatia em medicina (Redemed©) sobre a percepção de estresse de seus participantes e possíveis grupos de acolhimento. Método: Este é um ensaio quase experimental cuja amostra foi composta por 40 estudantes que compuseram o grupo controle e 47 alunos de um grupo de intervenção que participaram de oito encontros semanais de duas horas divididas em: 30 minutos de teoria sobre como o estresse influencia o estado de saúde, 60 minutos de vivências interpessoais e 30 minutos de ioga e meditação. Ambos os grupos, antes e depois do curso, responderam ao questionário sobre estresse percebido (PSS - Escala de Cohen). Resultados: Após os oito encontros semanais, o grupo intervenção apresentou melhora significativa (p = 0,030), demonstrando que a participação no curso Redemed© mostrou-se eficaz no controle do estresse entre os estudantes do presente estudo. Os alunos também foram questionados quanto à autopercepção sobre se sentirem ou não apoiados por outros grupos. Os três grupos de acolhimentos mais citados entre os alunos, tanto do grupo ativo como do controle, foram: amigos/família, centro acadêmico e a equipe do esporte que praticavam. Após as oito semanas, enquanto o grupo controle permaneceu com as mesmas indicações, no grupo intervenção foram citados: amigos/família, Redemed© e centro acadêmico. Conclusão: Este estudo mostrou que o programa Redemed©, com encontros durante oito semanas utilizando a meditação como sua técnica central, foi eficaz na redução do estresse percebido pelos estudantes de Medicina que participaram dessa intervenção quando comparado ao grupo controle (p = 0,000). As práticas integrativas e complementares podem ser uma ferramenta importante dentro das escolas médicas, de modo a levar os estudantes a lidar melhor com o estresse a que estão expostos ao longo do curso.


Abstract: Introduction: Undergraduate medical students experience a considerable amount of stress, which can negatively affect their learning, motivation and contact with patients. Some techniques and practices for stress management and reduction, such as meditation, have been recommended and used in medical schools. This study evaluated the effects of a Stress Reduction and Empathy Development Program in Medicine (REDEMED©) on participants' perception of stress and possible support groups. Method: This is a quasi-experimental trial whose sample comprised 40 students in a control group and 47 students in an intervention group. The students in the intervention group participated in eight weekly sessions lasting two hours each. The course hours were divided into: 30 minutes of theoretical content on how stress influences one's health, 60 minutes of interpersonal practices and 30 minutes of yoga and meditation practices. Both groups, before and after the program, answered the questionnaire on perceived stress (PSS - Cohen's scale). Results: After the eight weekly meetings, the intervention group showed significant improvement (p = 0.030), showing that participation in the REDEMED© course proved to be effective in stress control among the students in the study. The students were also questioned about their self-perception of whether or not they felt they were supported by any other groups. The three support groups most often referred to by the students, in both the intervention and the control group, were: friends/family, the Students' Union and their sports team. After eight weeks, while the control group still referred to the same groups, the intervention group mentioned friends/family, REDEMED© and the Students' Union. Conclusion: This study showed that the REDEMED© program, meeting for eight weeks using meditation as its central technique, was effective in reducing the stress perceived by medical students who participated in this intervention when compared to the control group (p = 0,000). Integrative and complementary practices can be an important tool within medical schools, empowering students to better cope with the stress they are exposed to throughout the course.

RESUMO Introdução Estudantes do curso de Medicina estão expostos a carga elevada de estresse, desencadeada por terem que lidar com adoecimento e morte dos pacientes, extensa carga horária, privação de sono, competitividade, cobrança, responsabilidade e medo de errar, entre outros fatores. Algumas técnicas e práticas como a meditação têm sido utilizadas para auxiliar no manejo e redução de estresse, já sendo utilizadas em escolas médicas. O estresse pode ativar componentes do sistema inflamatório, desencadeando uma série de doenças. As desordens causadas pelo estresse podem ser mensuradas por meio de marcadores sorológicos, sendo que os biológicos são os principais utilizados. Objetivo Avaliar os efeitos de um programa de práticas mente-corpo, Redução de Estresse e Desenvolvimento da Empatia na Medicina (Redemed©), nos níveis dos marcadores pró- e anti-inflamatórios de estudantes de medicina. Metodologia Trata-se de um estudo quase-experimental, composto por 86 estudantes, sendo 44 do grupo intervenção, que participaram do programa Redemed© com oito encontros semanais, englobando técnicas de meditação e exercícios de vivências interpessoais, e 42 estudantes do grupo controle. Ambos os grupos, antes e após o curso, coletaram sangue para análise dos marcadores: proteína C reativa (PCR), fator de necrose tumoral-alfa (TNF-alfa), interleucina 06 (IL06) e interleucina 10 (IL10). Resultados Neste estudo, não foi observada alteração estatisticamente significativa nas citocinas pró-inflamatórias: PCR, TNF-α e IL06. No entanto, a IL-10, que é uma citocina anti-inflamatória, apresentou uma variação positiva e estatisticamente significativa (p: 0,009). Ela tem sido utilizada em estudos com práticas integrativas e complementares a fim de demonstrar seus benefícios. Conclusão O programa Redemed© parece beneficiar os estudantes de Medicina por meio da modulação inflamatória e como grupo de acolhimento no qual eles puderam compartilhar seu estresse e treinar estratégias de enfrentamento. Este estudo, mesmo não tendo encontrado diferença estatística significativa nos marcadores pesquisados, com exceção da IL10, traz à tona este tema importante do grande estresse vivenciado por estudantes de Medicina e a necessidade de as escolas médicas terem maior cuidado com seus alunos, acolhendo e trabalhando o estresse desses estudantes de forma a reduzir e gerenciar melhor este fator de adoecimento em suas vidas.


ABSTRACT Introduction Medical students are exposed to high stress levels, triggered by having to deal with their illness and death of their patients, extensive workloads, sleep deprivation, competition, having to fulfil demanding duties, responsibility, and fear of making mistakes, among other factors. Some techniques and practices, such as meditation, are used in medical schools to help manage and reduce stress. Stress can activate components of the inflammatory system, triggering a series of pathologies. The disorders caused by stress can be measured by means of serological markers, with biological markers being the main ones used. Objective. To evaluate the effects of a program of mind-body practices, Reduction of Stress and Development of Empathy in Medicine (REDEMED ©), on pro- and anti-inflammatory markers of medical students. Methodology This is a quasi-experimental study, composed of 86 students: 44 in the intervention group, who participated in the REDMED© program with eight-weekly meetings that included meditation techniques and exercises of interpersonal experiences, and 42 students in the control group. Before and after the course, blood was collected from both groups for analysis of the markers: C-reactive protein (CRP), tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL06) and interleukin-10 (IL10). Results No statistically significant changes were observed in the proinflammatory cytokines CRP, TNF-α and IL06. However, IL-10, an anti-inflammatory cytokine, showed a positive and statistically significant variation (p: 0.009). It has been used in studies with integrative and complementary practices to demonstrate its benefits. Conclusion The REDEMED© program seems to benefit medical students through inflammatory modulation and as a group that provides a forum to share their stress and receive coaching in coping strategies. This study, even though it did not find a statistically significant difference in the markers studied, with the exception of IL10, raises the important theme of the high levels of stress that medical students experience, and the need for medical schools to take greater care of their students, addressing stress in order better manage it in their own lives.

@#<p style="text-align: justify;"><strong>OBJECTIVE: </strong>This paper presents the experiences of, and lessons learnt from, a seven-year government-academe-civil society collaboration in the development, implementation, and evaluation of a training program for Filipino physicians and rehabilitation practitioners on the management of drug dependence.</p><p style="text-align: justify;"><strong>METHODS:</strong> Review of memoranda, records, and reports relevant to the initiation and implementation of the training activity. Where relevant, we also perused internal notes and minutes of meetings written by the authors, who are also members of the training team.</p><p style="text-align: justify;"><strong>RESULTS:</strong> A tripartite collaboration between government, academe, and professional organizations developed a training program on the management of drug dependence for physicians and rehabilitation practitioners. Learner-centered approaches to education were adopted in the delivery of training content. Participation in the training is a prerequisite for government accreditation as rehabilitation professionals. A ladderized approach to the training was adapted, with participants first obtaining a broad introduction to the program, followed by in-depth focus on the assessment and management of drug dependency. This was done as a response to the perception that a single, two-week training program is insufficient to fully capacitate physicians and rehabilitation practitioners with the requisite knowledge and skills necessary to manage persons with drug dependence. Future plans include an executive course for established practitioners, and a course on community-based management of drug dependency.</p><p style="text-align: justify;"><strong>CONCLUSION:</strong> The current perspective on drug use and dependence is transitioning from a politico-legal issue to a public health concern. Attaining the sustainable development goals in 2030 will necessitate the development of a cadre of professionals who are, among others, capable of assessing and treating persons who suffer from drug dependence. The Philippine experience may serve as a model for other countries struggling with the drug menace.</p>

Objective: The objective of this study was to assess the efficacy of tamsulosin and finasteride monotherapies, and their combination in men with benign prostatic hyperplasia (BPH). Materials and Methods: This is a prospective single-blind randomized study of ninety men with BPH who were managed using drugs. The International Prostate Symptom Score (IPSS), peak urinary flow rate, and prostate volume were measured as parameters for assessment at the beginning, 3 months, and 6 months of the study. Results: The mean age of patients was 61.65 with a range of 44­81 years. There was a progressive and sustained improvement in the IPSS score in all patient groups with mean decrease at 3 months of 7.24 (42.59%), 7.60 (41.85%), and 7.24 (40.61%) and at 6 months of 8.14 (47.88%), 10.33 (56.88%), and 11.1 (62.25%) in the tamsulosin, finasteride, and combination groups, respectively. There was an increase in peak urinary flow rate in all groups with mean increase at 3 months of 0.98, 0.05, and 3.55 (ml/s) and at 6 months of 4.11, 0.87, and 3.74 (ml/s) in the tamsulosin, finasteride, and combination groups, respectively. There was a reduction in the prostate volume in the finasteride and combination groups at 6 months of 6.8 and 6.32 cm3, respectively, while the tamsulosin group recorded an increase. Conclusion: At the end of 6 months, tamsulosin monotherapy and combination therapy appear to be equally effective in the treatment of lower urinary tract symptoms BPH while finasteride monotherapy appears to be the least effective. Bothersome, side effects were more in patients taking finasteride alone or as combination therapy

BACKGROUND: Workplace violence (WPV) is becoming an issue that needs immediate attention in the United States, especially during this period as more states are adopting the "stand your ground laws to promote worker protection." This study was conducted to investigate how WPV has contributed to an unsafe environment for nurses and nursing assistants who work in long-term medical care facilities. METHODS: A structure questionnaire was used to collect data for the study. Three facilities were sampled and 80 nurses and certified nursing assistants participated in the study. Ninety-two percent (n = 74) were female and 8% (n = 6) were male. Approximately 62% were black or African American, approximately 33% were Caucasians, and only 2% were from other ethnicities. RESULTS: We found that 65% of the participants had experienced WPV while 41% believed that management shows little or no concern for their safety. Approximately 23% of respondents believed that reporting supervisor's WPV act is an unsafe action. In addition, 22% of those who reported that they have experienced WPV believed that the work environment is not safe to perform their duties. This significant difference in perception of workplace safety between those who had experienced WPV and those who had not was significant (t = 3.95, df = 158, p < 0.0001). CONCLUSION: WPV is an epidemic problem that affects all health-care professionals. The findings of this study could help long-term medical care facilities' management identify the areas to focus on mitigating, controlling, and/or eliminating incidents of WPV.

This study examined reproductive traits and growth rates of Heterandria bimaculata (Poeciliidae) in Cusuco National Park (CNP), a cloud forest reserve in northern Honduras, Central America. In CNP, H. bimaculata occurs in the absence of other fish species and major invertebrate predators along an approximately 1000 m elevation gradient. This allowed for the examination of trait variation along the gradient without the confounding effects of interspecific interactions or habitat patchiness. Heterandria bimaculata exhibited traits characteristic of a low-predation environment: balanced sex ratio, slow growth, late maturity and large female size. Females produced more, smaller eggs from upstream to downstream, but overall reproductive allocation remained constant along the gradient. Maximum male length and annual growth rates increased from upstream to downstream, but female growth showed no trend. The patterns of growth and reproductive allocation tradeoff are consistent with predicted response to a longitudinally-increasing productivity gradient in which food resources become more abundant downstream. Macrobrachium and Bellastoma could have caused some predation, but were sparse and patchily distributed. Fish density remained fairly constant among elevations; if food resources were limiting in upstream habitats, per-capita resource availability would be lower and density-dependent competition would drive selection for larger but fewer, more competitive offspring. Future work should quantify longitudinal changes in productivity and conduct experiments to decouple the effects of stream order and fish density dependence.


Este estudo analisou características reprodutivas e taxas de crescimento de Heterandria bimaculata (Poeciliidae) no Parque Nacional de Cusuco (CNP), uma reserva florestal nubosa no norte de Honduras, América Central. No CNP, H. bimaculata ocorre na ausência de outras espécies de peixes e grandes predadores invertebrados, ao longo de um gradiente de aproximadamente 1.000 m de altitude. Isto permitiu a análise de variação das características ao longo do gradiente sem a interferência dos efeitos de interações interespecíficas ou desconexão de hábitat. Heterandria bimaculata exibiu traços característicos de um ambiente de baixa predação: proporção sexual equilibrada, de crescimento lento, maturidade tardia e fêmeas de grande porte. As fêmeas produzem mais ovos menores de montante para jusante, mas a alocação reprodutiva global manteve-se constante ao longo do gradiente. O comprimento máximo dos machos e as taxas de crescimento anual aumentaram da montante para jusante, mas o crescimento as fêmeas não mostrou nenhuma tendência. Os padrões de crescimento e alteração de alocação reprodutiva são consistentes com a resposta prevista para um gradiente de produtividade que aumenta longitudinalmente, no qual os recursos alimentares se tornam mais abundantes a jusante. Macrobrachium e Bellastoma poderiam ter causado alguma predação, mas foram escassos e pouco distribuídos. A densidade de peixes manteve-se relativamente constante entre as elevações; se os recursos alimentares fossem limitantes em hábitats a montante, a disponibilidade de recursos per capita seria menor e a competição dependente da densidade iria conduzir a seleção para uma prole mais competitiva, maior mas menos numerosa. Trabalhos futuros deverão quantificar as mudanças longitudinais na produtividade e conduzir experimentos para dissociar os efeitos da ordem do riacho e dependência da densidade de peixes.

Renal histology results are very scarce in dengue-associated rhabdomyolysis patients developing acute kidney injury (AKI). We report a case of dengue fever-induced AKI associated to rhabdomyolysis with a renal biopsy showing acute tubular necrosis (ATN) and renal deposition of myoglobin. A 28-year-old patient who presented dengue fever (DF) complicated by severe AKI and rhabdomyolysis is described. The patient required hemodialysis for three weeks. A renal biopsy revealed ATN with positive staining for myoglobin in the renal tubuli. The patient was discharged with recovered renal function. In conclusion, this case report described a biopsy proven ATN associated to DF-induced rhabdomyolysis, in which renal deposition of myoglobin was demonstrated. We suggest that serum creatine phosphokinase should be monitored in DF patients to allow for an early diagnosis of rhabdomyolysis and the institution of renal protective measures.


Resultados de histologia renal são muito escassos em pacientes com rabdomiólise e injúria renal aguda (IRA) associada a dengue. Descrevemos caso de dengue complicado por rabdomiólise e IRA no qual a biópsia renal mostrou necrose tubular aguda (NTA) e deposição renal de mioglobina. Paciente de 28 anos que apresentou dengue complicado por IRA grave e rabdomiólise é descrito. Ele necessitou de diálise por três semanas. A biópsia renal mostrou NTA, com imunohistoquímica fortemente positiva para mioglobina nos túbulos renais. O paciente recebeu alta com recuperação da função renal. Em conclusão, descrevemos caso de dengue complicado por IRA e rabdomiólise, em que a biópsia renal mostrou NTA e deposição de mioglobina. Sugerimos que creatinofosfoquinase deve ser monitorizada em pacientes com dengue para permitir o diagnóstico precoce de rabdomiólise e a instituição de medidas protetoras para o rim.
