RESUMEN

Introduction: Diabetes is the most common metabolic disorder in Iran. Due to the nature of the disease, the patient's cooperation in self care is very important. One of the most effective and affordable ways to empower patients is education, which, to be beneficial, must be provided in accordance with related standards. The current study was conducted to investigate accordance of time of discharge diabetic patients with the standards of education for diabetics Type 2 in hospitals, affiliated in Yazd University of Medical Sciences

Materials and Methods: At discharge, data on patient education provided by nurses to 385 diabetic patients, was documented using the "nursing diabetic patient education-discharge time check list" and were analyzed using descriptive statistics and SPSS 20

Results: Conformity rate of discharge nursing education in patients with Diabetes Type 2 with standards was 14.5%, which indicated a low quality of education. Conformity rate in the dimensions of "insulin therapy" was 34.7%, in "foot care education" was 6.75%, in "diet programs" was 1.8%, in "physical activity" was 0.1%, in "prevention of infection" was 1.8%, and in "oral antihyperglycaemics drugs" was 13.3%

Conclusion: Based on the poor compliance with the standards of education for diabetics Type 2 patients, application of nursing clinical guidelines during discharge for type 2 diabetic patients and clinical supervision by nursing managers for optimizing quality of care is recommended

RESUMEN

ICU patients are needy to medical and nursing services. According to Global estimates, the mortality rate is high in these wards because of deterioration of the patient and the severity of the disease. So the grading systems are designed to predict recovery, mortality rate and classifying of these patients. The SOFA scoring system is one of these scoring systems. So, this study was conducted to determine the efficiency of SOFA scoring system in predicting of mortality rate and stay length in intensive care unit for patients of alzahra hospital in Isfahan. This descriptive study was conducted on 107 patients in intensive care unit of Al-Zahra Hospital in 2012 during a 4-month period through convenience sampling. The instruments for data gathering consisted of demographic information form and SOFA check list. For data analysis independent t-test and Pearson correlation were used. The findings showed that there is significant difference between SOFA score at the beginning of admission and death of patient and between the SOFA score on the following days and mortality rate [P<0.0001]. But mortality rate in the beginning of admission was more significant than other days. There is significant relationship between sofa score in beginning of admission and stay lenght [P=0.07], but there is no significant relationship between SOFA score on the days after admission and length of stay [P=0.14]. The result of current study showed that SOFA scoring system is appropriate instrument to predicting mortality rate and stay length of patients in intensive care unit.

RESUMEN

The process of communicating with psychiatric patients by nursing students can indicate the extent of their social interaction in the future. Since communicating with these patients is more difficult than others and is often associated with anxiety, the present study was designed to determine the effect of communication skill with psychiatric patients using Wilkinson- Shimizu method and traditional method on anxious thoughts of nursing students. It was a quasi-experimental study with before-after design. The sample consisted of 51 undergraduate nursing students in Najafabad branch of Azad University of Isfahan who were recruited during 2011-2012 academic year by purposive sampling and then were randomly assigned into either experimental or control groups during their mental health clinical practice. The experimental group received training about Wilkinson- Shimizu method and the control group received training by traditional method. Data was collected by the Persian version of Anxious Thought Inventory [AnTI] and analyzed by SPSS-PC [v.17] using mean and standard deviation of the groups and paired t-test. The mean score of anxious thoughts significantly decreased in experimental group [p<0/05]. In control group, the mean score of anxious thoughts significantly increased [p=0/004]. Before and after difference of mean scores of anxious thoughts after training with two methods was significant [P=0/005]. The findings revealed that using Wilkinson- Shimizu method for communication with psychiatric patients affects anxious thoughts of nursing students. Therefore, it is recommended that managers of nursing schools pay more attention to teaching these communication skills and clinical practice about communication with psychiatric patients in nursing curriculum

Since body image can have an effective role in educational progress, therefore, in this research we have tried to investigate the relationship between body image satisfaction and academic field of study of female students. In this descriptive-correlative research 360 female students of Islamic Azad University branch of Najafabad were selected by simple sampling from various fields and were investigated by BSQ34 questionnaire. Data was analyzed by S.P.S.S software version 17 and descriptive analysis, Kruskal Wallis, Mann-Whitney U and Spearman correlation tests were used. Findings of this research showed that 34.4% of the university students had moderate-poor body image and there was a meaning relation between economic status and satisfaction from body image. Among different fields, students of physical training had the least attention to their appearance and art students had the most. Since most of the university students in this research were not satisfied with their appearance and as, paying attention to ones outward appearance can affect different aspect of life, including educational status and also regarding that universities are among the places having vital role in process of forming and improving youth problems therefore it seems that necessary teachings and training should be included in universities syllabus to improve the satisfaction of university students from their body image

RESUMEN

Nurses' participation in clinical affairs improves patient care and reduces costs. Also, the lack of correct and timely decisions can prolong the treatment and care for patients and create problems. The aim of study was to determine the effect of nurses' participation in patient weaning from mechanical ventilator and on its outcomes on patients hospitalized in intensive care units in Esfahan Al-Zahra Hospital. This is a clinical trial in which 50 patients connected to mechanical ventilators for more than 48 h were selected through convenient sampling and were randomly divided into two groups of 25 subjects. In the Intervention group patients were prepared for weaning by trained nurses three times in every 24 hours using Burn's wean assessment program in three shifts. Control group weaning followed the conventional care unit procedures. Findings were analyzed using descriptive and inferential statistical methods [t-test, chi-square and Fisher's exact]. The findings showed that length of MV [P=0.03], length of staying time in intensive care units [P=0.006] and the weaning failure from mechanical ventilator [P=0.01] were all significantly different in the two groups; however, the average arterial oxygen saturation 2 hours after removal from mechanical ventilation was not significantly different between intervention and control group. The results showed that nurse participation in the weaning process of the patients from mechanical ventilator is a safe method in day time and shortens the length of MVT and length of staying in ICU and, reduces the weaning failure compared to the conventional methods.
