RESUMEN

Objectives@#The aim of the present study was to develop and evaluate the usability of a telemedicine system for management and monitoring of patients with diabetic foot. @*Methods@#This study was conducted in four phases. In the first phase, the information needs and characteristics required to design the telemedicine system were identified based on a literature review. Then, in a two-stage Delphi survey, 15 experts approved the identified information needs and characteristics. The prototype telemedicine system was then designed. In the third phase, system usability was evaluated through a semi-structured interview. In the fourth phase, users’ satisfaction with the designed system was analyzed. @*Results@#Out of 115 information needs and required characteristics, 95 were considered in the system design. Eight main pages for enabling patient-physician interactions and physician-physician interactions, monitoring the patient and controlling the disease process, providing medical consultation, and prescribing medications were considered. In the third phase, 26 distinct problems were identified. However, 75% of the participants were very satisfied with the system. @*Conclusions@#This study presents an attempt to design and evaluate a telemedicine system for the management and monitoring of patients with diabetic foot. In this system, patients receiving medical services or physicians who encounter rare cases can send the complete medical history, clinical test results, and videos and images related to the foot to specialist physicians. After examining the medical history or images and videos, the physician can provide the necessary medication prescriptions and laboratory tests or other recommendations.

Objectives: One of the most effective methods to describe speech disorders is the measurement of speech intelligibility. The speech intelligibility indicates the extent of acoustic signals that correctly speaker produces and hearer receives. The purpose of this study was to investigate the speech intelligibility in the Persian children with Down syndrome; age range was 3 to 5 years, who had spoken Persian.


Methods: This cross- sectional study investigates 12 children [6 girls and 6 boys] with Down syndrome who had referred to speech therapy clinic in Hamadan city and 12 normal children [6 girls and 6 boys] who went to the kindergarten in Hamadan city .The pictures of speech intelligibility test [in Persian language] were used to collect speech samples of participants. The participant's voice was recorded by voice recorder and was investigated in two age groups.


Results: The results of this study indicated the means of speech intelligibility was 92.25 for normal children and 35.08 for children with Down syndrome. The correlation between age and speech intelligibility for normal children was 0.866 and for children with Down syndrome was 0.352. The mean of speech intelligibility 2 for normal boys was 93 and for normal girls 91.5 and for boys with Down syndrome 34.66 and for girls with Down syndrome 35.5.


Discussion: The difference between normal children and children with Down syndrome was Significant. One of the factors that affect speech intelligibility for children with Down syndrome is difficulty with voluntarily programming, combining, organizing, and sequencing the movements necessary for speech.

RESUMEN

Cardio-Pulmonary Resuscitation [CPR[has been used by medical professionals to save the life of dying patients. To achieve more success rates in CPR, the timing factor is of great importance; as any delay in starting CPR will reduce the success rate. The aim of this study was to examine the impact of the time elapsed between CPR code announcement and start of resuscitation on CPR outcome. In this cross-sectional study, data was collected from June 2004 till June 2005 using checklists that were routinely used in CPR protocol. A total number of 327 cases of CPR were studied and the data was analyzed by SPSS software. Our results showed that 163 cases of CPR [49.9%] were immediately successful. Of those cases with immediate successful CPR, only 46 cases [28.2%] discharged from hospital and 117 cases [71.8%] died afterwards. In the other words, only 46 cases [14.06%] out of 327 patients who underwent CPR, discharged from hospital and the rest [85.94%] died. The mean time elapsed between CPR code announcement and the start of CPR operations was 1.63 minute and 1.87 minute in immediate successful and unsuccessful CPRs, respectively.
