Objetivo: Relatar a experiência de uma profissional assistente social, com o apoio da rede socioassistencial do município de Belo Horizonte, no acompanhamento a uma mãe em situação de vulnerabilidade social. Métodos: As informações foram compiladas em agosto de 2017 através dos atendimentos e de registros dos relatos da mãe a cerca da sua trajetória de privações, violações de direitos desde a infância e da percepção negativa da sua relação com os meios de proteção do Estado. Resultados: A internação hospitalar da mãe e bebê permitiu o adequado acolhimento e acompanhamento da equipe de saúde, que gerou a identificação das demandas e a articulação com equipamentos de apoio disponíveis na rede socioassistencial. Favorecendo assim, o fortalecimento e promoção da família através da inserção nas políticas públicas de saúde, habitação e assistência social, bem como sendo instrumentos para a garantia de direitos da mãe e do bebê. Conclusão: Desta forma, conclui-se que o trabalho em rede afirmou sua potencialidade na articulação do cuidado integral a saúde e na garantia de direitos do cidadão, mediante a construção de um projeto terapêutico que considere as singularidades e necessidades de cada sujeito individualmente.(AU)

To report the experience of a social worker, with the support of the social assistance network of the city of Belo Horizonte, in the monitoring of a mother in a situation of social vulnerability. Methods: The information was compiled in August 2017 through the consultations and records of the mother's reports about her trajectory of deprivation, violations of rights since childhood and the negative perception of her relationship with the State's means of protection. Results: The hospital admission of the mother and baby allowed the adequate reception and monitoring of the health team, which generated the identification of the demands and the articulation with support resources available in the social assistance network. This strategy made it possible to strengthen and promote the family through its insertion in health, housing and social assistance public policies, as well as contributed to the guarantee of rights of mother and baby. Conclusion: In this way, it is concluded that the network work affirmed its potential in articulating integral health care and guaranteeing citizens' rights, through the construction of a therapeutic project that took into account the singularities and needs of each individual subject.
