RESUMEN

BACKGROUND: Medical skin care is essential for the treatment of skin diseases all over the world. Medical skin care is also part of medical practice and this must be differentiated from the simple skin care that is given for normal healthy skin. OBJECTIVE: We wanted to discuss medical skin care and the related medical devices and legal issues. METHODS: We reviewed the related laws and regulations, we consulted experts and associations and we analyzed the result of the survey. RESULTS: Legally, medical skin care and simple skin care are well classified. However, many illegal procedures are still performed by non-medical personnel and many adverse effects have been reported as a result. Furthermore, there are no legal restrictions for the performer based on the grade of each medical skin care procedure. CONCLUSION: For the best results and safe procedures, medical skin care must be performed by approved medical equipment under the supervision of a physician or medical personnel. Continuous control and guidance by the government is strongly needed.

BACKGROUND: Recently, narrow-band UVB phototherapy is introduced for vitiligo treatment. It is easier to be applicable because of no need for topical and/or systemic photosensitizers, and comparably or more effective than PUVA without systemic side effects. Moreover, it is more safely applicable to children and pregnant women. However, to date, there is not so large series of clinical reports about its use in vitiligo treatment as to evaluate therapeutic effects. OBJECTIVE: This study was to evaluate the effects of narrow-band UVB phototherapy for vitiligo in various aspects of clinical parameters. METHODS: Treatment of patients in this study with narrow-band UVB was started between November 2001 and November 2002. Narrow-band UVB phototherapy was given as monotherapy once or twice a week. The starting dose was 280-300mJ/cm2, with dose increments at each subsequent treatment. RESULTS: 78 patients were able to be evaluated in this study. Their ages ranged from 3 to 67 years (mean, 33.7 years). 69 patients had Fitzpatrick skin type IV, 5 had skin type III and 4 had skin type V. 36 of the 78 patients (46.2%) achieved more than 75% repigmentation (grade 4). The better repigmentation grade, the shorter vitiligo duration (p=0.0464). Compared with other sites, lesions on the hands and feet showed poorer improvement(grade 1: 10/13 and 4/4 respectively, p= 0.0015). Adverse effects were limited and transient. CONCLUSION: Narrow-band UVB therapy is an effective and safe therapy for vitiligo. Long-term adverse and therapeutic effects compared to other treatment modalities remain to be determined.

RESUMEN

Progressive macular hypomelanosis is an idiopathic acquired hypopigmentary disorder first described in 1988. The disorder is characterized by ill-defined, non-scaly, round to oval, hypopigmented patches symmetrically localized on the trunk. No clinical symptoms have been observed. Currently, diagnosis is made on clinical grounds and any histological, laboratory investigations are not diagnostic. This disorder does not respond to conventional treatment and its clinical course is variable. We experienced a case of progressive macular hypomelanosis that was successfully treated with narrow band UVB phototherapy.

RESUMEN

Atopic dermatitis is a chronic dermatitis which accompanies severe pruritus. It has various modalities of treatment and yet ones that resist to temporizing treatments may be difficult to treat. Thus, we report a case of a severe adult type atopic dermatitis that resisted the established treatments, however, was treated successfully by wet-wrap dressing with the literature reviews.

BACKGROUND AND OBJECTIVES: Multiple techniques of ossicular replacement have been advocated for hearing gain in the setting of chronic otitis media. The purpose of this study is to investigate hearing gain in the middle ear surgery according to the surgical technique and the materials. MATERIALS AND METHOD: Ninety-nine cases with ossicular replacement with mastoidectomy in the Konkuk University Hospital from 1995 to 2001 were reviewed retrospectively. Postoperative hearing gains were compared with the two methods of mastoidectomy and the two materials of ossicular replacement separately. RESULTS: A successful hearing gain was defined as a postoperative air-bone gap less than 30dB. According to this criterion, 47% of intact canal wall mastoidectomy cases and 43% of open cavity mastoidectomy cases were successful. Thirty-one percent of ossicular replacement case using cartilage-perichondrium and 62% using hydroxyapatite-PORP were successful. The extrusion rate was zero % in ossicular replacement using cartilage-perichondrium, and 57% in ossicular replacement using PORP. CONCLUSION: There was a significant differences in hearing gain and extrusion rate by using two different materials for ossicular replacement such as cartilage and hydroxyapatite-PORP (p0.05).

BACKGROUND AND OBJECTIVES: Cisplatin is frequently used in the treatment of various forms of malignancies. It's therapeutic efficacy, however, is limited due to the occurrence of sensorineural hearing loss. Little is known about the course of hearing loss after cessation of cisplatin administration. We observed the cochlear duct morphology with normal and cisplatin treated animals. MATERIALS AND METHODS: Healthy rats (strain Sprague-Dawly, weighting 80-100 mg) were used for all experiments. Total 15 rats were selected. They were divided into two groups, a treated group and a control. Four survival groups (n=3, respectively) were assigned as the treated group. After treatment with cisplatin, each survival group was sacrificed 1, 4, 10, 20 days. Except for the normal control (n=3), twelve animals were treated with cisplatin by daily I.P. injection of 1.5 mg/kg for 8 consecutive days. RESULTS: One day after cessation of cisplatin administration, outer hair cells (OHCs) loss and stria vascularis were degenerated especially in the basal turn. At 4, 10, 20 days, the OHCs and stria vascularis morphology of the survival group were similar to those of the survival group at one day. CONCLUSION: Our findings suggest cisplatin ototoxicity is predominantly involved in the basal turn of cochlear duct. This finding was significantly correlated with high frequency hearing loss of cisplatin ototoxicity.

BACKGROUND AND OBJECTIVES: Quinolone-induced arthropathy is an unusual toxic effect observed in juvenile animals of multiple species. Such toxic effects are usually found in joint cartilages. The reports of chondrotoxic potential of quinolones in juvenile animals were mainly performed in weight bearing joints such as hips and knees. The purpose of this study was to investigate the morphological changes in the incudomalleolar joint of immature rats dosed with ciprofloxacin, and to draw relationship between the incudomalleolar joint pathology and hearing. MATERIALS AND METHODS: An experimental group of 21-day-old rats were administrated intraperitoneally with 200 mg ciprofloxacin/kg for 10 days. The hearing threshs of the control and experimental animals were determined with auditory brainstem response (ABR). Their ossicles were obtained and fixed, and we observed them with light microscopy (LM) and transmission electron microscopy (TEM). RESULTS: The ABR threshs were increased in the ciprofloxacin administrated group, and the increased latency of the first waves were identified in the ciprofloxacin administrated group, but there were no differences in the interval of wave I-III. The LM findings of incudomalleolar joint in the ciprofloxacin administrated group showed cartilage lesions such as disrupted articilar surface, irregular cellularity of the chondrocytes, matrix swelling, cleft formation, decreased matrix staining. The TEM findings of the incudomalleolar joint cartilage in the ciprofloxacin administrated group showed decreased bundle-like matrix and cell process, condensed nucleus, and vacuolated cytoplasm. CONCLUSION: From the results of recent works and the present study, we suggest that ciprofloxacin may induce morphological changes in the incudomalleolar joint, and that such morphological changes may induce conductive hearing loss.

RESUMEN

Seborrheic keratosis is a benign epidermal lesion, that commony develops in the aged. It exhibits a remarkable spectrum of histologic variations. We evaluated one hundred and one cases of seborrheic keratoses according to the histologie typing by Lever and Schaurnburg-Lever. The most common histologic type was the acanthotic type(50.5%), and the next. most common one was hyperkeratotic type(33.7%). The rest of the caces were relatively uncommon, irritated type(6.9%), mixed type(4.9%), clonal type(3.9%), adenoidal type(2.0%), and melanoacanthoma(1.0%). In 2 cases, mixed type was found in a lesion.

RESUMEN

We report a case of Mondor's disease, or superficial thrombophlebitis of the chest wall, which occured in a 35-year-old female after a mild blunt trauma of the right chest wall. She had 0.3 × 25 cm, firm, tender, bifurcated cord-like lesions on the right chest and upper abdomen for two weeks. The histopathologic findings corresponded to the “thrombus organization” stage of Mondor's disease, characterized by numerous small areas of recanalization with marked surrounding fibrosis. The patient was treated with non-steroidal antiinflammatory drugs and warm compresses. Within 7 weeks, the cord-like lesions virtually disappeared.

RESUMEN

We report a case of eosinophilic pustular folliculitis in a 18-year-old woman who had numerous mild pruritic erythematous plaques with follicular papules and pustules covering almost her whole body. Histopathologically intrafollicular, perifollicular and perivascular eosinophilic infiltrates were present with mild spongiosis. The blocd eosinophil count was 9%. The patient responded well to systemic administration of corticosteroid and dapsoni.

RESUMEN

We report a case of adenoid squarnous cell carcinoma occurred in a 64-year-old male. The patient had a single, centrally ulcerated nodule with an indurated border on the superior helical rim of the left ear. The histopathologic finding showed a squamous cell carcinoma infiltrating der mis as multiple gland-like structures. The wall of gland-like structure was composed of one to several cell layers and the lumen was filled with dyskeratotic, acantholytic cells.
