RESUMEN

Purpose To investigate the features of 64-slice spiral computerized tomography (CT) on collateral circulation of pancreatic portal hypertension (PPH).Material and Methods the abdominal CT images of 52 patients with PPH confirmed by pathology the Affiliated Hospital of Southwest Medical University from August 2013 to May 2015 were analyzed retrospectively.The collateral circulations of portal vein were recorded.Results There were 21 (40.4%) patients with isolated splenic vein occlusion (ISVO),and 32 (59.6%) patients with non isolated splenic vein occlusion (NISVO) in the total 52 patients.There was no significant difference in the occurrence rate between ISVO and NISVO (x2=1.92,P>0.05).The main collateral pathways of PPH were pathway Type Ⅰ (28 patients)and pathway Type Ⅲ (22 patients),and 2 patients belonged to pathway Type Ⅱ.The patients in pathway Type Ⅰ or Ⅲ were significantly more than patients in pathway Type Ⅱ(P<0.01).There were 92.3% (48/52) of patients with gastroepiploic varicosity,which is significantly higher than other varicose veins (P<0.001).The significant difference was found between the patients (21.2%) with gastric fundus varicosity and the patients (1.9%)with inferior segment esophageal varices (x2=9.42,P<0.01).The patients with gastric fundus varicosity and right superior colic varicosity in NISVO were more than those in ISVO (x2=4.15,7.44,P<0.05).Conclusion For PPH patients,the valuable features of collateral circulation can be revealed by multi-slices CT.

RESUMEN

Objective To evaluate hepatic functional reserve and operational risks of primary hepatic carcinoma in Child A using functional CT.Methods In 128 cases of primary hepatic carcinoma of Child A undergoing hepatoectomy and identified by pathology, CT perfusion scanning and measurement of remannent hepatic volume were done before operation and whole patients were divided in-to acute hepatic failure group(AHF group,33 cases)and non-acute hepatic failure(non-AHF group,95 cases).All variables were ana-lyzed by one way analysis of variance(one-way ANOVA)firstly.The variables with significance (P<0.05)were analyzed with Step-wise Logistic regression further.Results One-way ANOVA result:There were significant difference between two groups in RHVS measured by CT,PVP,HBF,HBV,serum creatinine,thrombinogen activity,total bilirubin and intraoperative blood loss (P<0.05).The StepwiseLogistic regression analysis demonstrated that decreased RHVS and the lowed PVP were the independent risk factors of AHF complicated to hepatoectomy of primary hepatic carcinoma(P<0.01).Conclusion Hepatic functional reserve and operational risks of primary hepatic carcinoma could be j udged with functional CT before operation .

RESUMEN

Objective To study the pharmacokinetics difference of levofloxacin polymorphs in rats, evaluate the advantageous medical polymorph,and explore the effects of different polymorphs on clinical medicine. Methods Four crystal forms of levofloxacin were administered intragastrically to rats,and high performance liquid chromatography( HPLC)was used to measure the contents of levofloxacin in rat plasma. The pharmacokinetic parameters were calculated and compared Results After a single oral dose,the peak plasma concentration(Cmax)of crystal forms ofⅠ,Ⅱ,ⅢandⅣof levofloxacin was 6. 984,9. 692,9. 405,6.424 mg·L-1;the time to peak(tmax)was 0.6,0.9,1.0,1.0 h;the half-life(t1/2)was 4.207,2.97,4.857,1.695 h;theareaunderthecurve(AUC0→12h)was31.478,42.385,32.406,31.636mg·h·L-1. Conclusion Thereisnostatistically significant difference in pharmacokinetic parameters. However,compared with other crystal forms,plasma concentration of crystal form II is higher and maintained longer. Therefore,crystal form II of levofloxacin is an advantageous polymorph for medicine.

RESUMEN

Objective To explore the application value of multi-slice CT angiography (MSCTA)in posterior approach of upper cervical vertebrae.Methods Fifty-one patients with suspected or conformed upper cervical disease were collected and performed cer-vical CTA examination.Twenty three patients underwent internal fixation surgery of posterior upper cervical and reviewed with CTA examination.Volume Render (VR),Maximum Intensity Projection (MIP)and Multi-planar Reformation (MPR)were used to show the courser and anatomical variation of vertebra artery,the shape of vertebrae,the relationship between the vertebra artery and adjacent structure,advantage artery.The distribution of screw placement,including perforation of the anterior wall,the inner wall, the outer wall,the upper wall,the interior wall and the inner wall of occipital were observed by VR,MIP and MPR after surgery, especially the relationship between the screw and vertebra artery,the screw and spinal cord.Comparing the correlation between the screw placement and postoperative symptoms.SPSS1 9.0 statistical packages were used for statistical analysis.Results VR,MIP and MPR can show the courser and nearby space relationship of cervical artery.The number of advantage artery was 25,equilibra-tion was 26,dysplasia variation was 13,course variation was 4.The number of tiny pedicle was 1 6,including 9 cases of ipsilateral advantage artery.The postoperative data analysis of 23 patients showed that:C1 -20,C2 -36,C3 - 1 5,C4 -2,occipital 40.The number of accurate screw placement was 83.Different type of perforation was 30,including 3 screws closing to vertebra artery,1 screw closing to spinal cord.All 23 patients were asymptomatic after surgery.Conclusion MSCTA can reflect the morphology of cervical vertebral,the spatial relationship between cervical vertebral and surrounding structure,which can provide an accurate refer-ence for clinical screw placement and improve the accuracy .The postoperative CTA can show screw placement accurately,which has great reference for the evaluation of clinical success rate of surgery and post-surgery treatment.

RESUMEN

Objective To explore the feasibility and value of MSCT for the evaluation of hypophysoma before and after endonasal transphenoidal microsurgery.Methods 82 patients with hypophysoma intended to be treated by endonasal transphenoidal microsur-gery,were examined by 64-slice head CT angiography (CTA).All the images were transmitted to workstation and reconstructed by MIP、VR and MPR.The hypophysoma were typed according to the relationship between hypophysoma and its surrounding vessels. The shortest and longest diameter between internal carotid arteries of two sides,tumor length and width,the diameter between ex-ternal naris and the rear wall of sphenoid top and the length from external naris to the rear wall of tumor top were measured in VR images.Sphenoid division was observed in coronal plane image.Major vascular injury,bleeding,and residual tumor situation were observed on postoperative CT review.The surgery effect was evaluated by three associate professorwith SPSS 13.0.Results 39 ca-ses were type-A,30 were type-B,13 were type-C,tumor length ranged from 1.7 cm to 3.9 cm,width ranged from 1.3 cm to 2.5 cm,the diameter between external naris and the rear wall of sphenoid top and the length between external naris and the rear wall of tumor top were 6.9 cm~7.6 cm and 9 cm~12.2 cm.There are 1 5 cases of sphenoid without division,34 cases with one divi-sion,26 cases with two division and at least three division in 7 cases.The surgery effect was better in those which tumor size was relatively smaller compared to the diameter of the internal carotid artery.The evaluation of endoscopic endonasal transsphenoidal surgery with three types of pituitary adenomas had a significant difference (P <0.05).Conclusion MSCT can preferably reflect the spatial location relationship between pituitary adenimas and its surrounding structure,and it is effective evaluation methods for pre-operative and postoperative evaluation of transnasal microsurgical resection.

RESUMEN

[Objective]To research the effect of acetic ether extract from T. ramosissima on the skin-wounded mice. [Methods]The acetic ether extract of T. ramosissima was administrated to skin-wounded mice by gavage methods. [Results] The experimental groups in different dosages had significant difference ,compared to the control group, with the areas of wound healing 17.68 mm2, 17.90 mm2, 20.18 mm2 and 31.48 mm2,after the acetic ether extract of T. ramosissima administration to skin-wounded mice by gavage methods. [Conclusions] It was suggested that the acetic ether extract of T. ramosissima had some protective effects on improving the wound healing.

RESUMEN

Objective To explore cardioprotective effect of matrine on myocardial ischemia in hypercholesterolemia rats. Methods Myocardial infarction was induced by subcutaneous injection of isopreterenol (ISO, 85 mg/kg) once daily for two consecutive days in rats feeding cholesterol-rich diet for 4 weeks. Content of serum lipid, myocardial injury marker enzymes, lipid peroxidatase and activities of antioxidative enzymes in serum and/or heart tissues were measured, and cardiac function was evaluated. Results Administration of matrine (50, 100, and 200 mg/kg, respectively) decreased serum level of TC and TG, improved left ventricle (LV) contractile function (increased LVSP and +dp/dtmax) and LV diastolic function (decreased LVEDP and increased -dp/dtmax), depressed the levels of myocardial injury marker enzymes of lactic dehydrogenase (LDH) and creatine kinase (CK), promoted the activities of antioxidative enzymes of superoxide dismutase (SOD), catalyst (CAT), and glutathione peroxidase (GHS-PX), as well as decreased the content of lipid peroxidation product of malondialdehyde (MDA) in plasma and/or myocardial tissues in hypercholesterolemia rats with myocardial infarction. Histopathology examination demonstrated that matrine could attenuate ISO-induced myocardial infarction morphologically in hypercholesterolemia rats. Conclusion Our results suggest that cardioprotective effect of matrine on myocardial infarction in hypercholesterolemia rats is attributed to its ability to decrease the TC and TG content of serum, enhance the activities of antioxidative enzymes, and maintain the stability of myocardial cellular membranes.
