RESUMEN

BACKGROUND@#This study aimed to develop a comprehensive instrument for evaluating and ranking clinical practice guidelines, named Scientific, Transparent and Applicable Rankings tool (STAR), and test its reliability, validity, and usability.@*METHODS@#This study set up a multidisciplinary working group including guideline methodologists, statisticians, journal editors, clinicians, and other experts. Scoping review, Delphi methods, and hierarchical analysis were used to develop the STAR tool. We evaluated the instrument's intrinsic and interrater reliability, content and criterion validity, and usability.@*RESULTS@#STAR contained 39 items grouped into 11 domains. The mean intrinsic reliability of the domains, indicated by Cronbach's α coefficient, was 0.588 (95% confidence interval [CI]: 0.414, 0.762). Interrater reliability as assessed with Cohen's kappa coefficient was 0.774 (95% CI: 0.740, 0.807) for methodological evaluators and 0.618 (95% CI: 0.587, 0.648) for clinical evaluators. The overall content validity index was 0.905. Pearson's r correlation for criterion validity was 0.885 (95% CI: 0.804, 0.932). The mean usability score of the items was 4.6 and the median time spent to evaluate each guideline was 20 min.@*CONCLUSION@#The instrument performed well in terms of reliability, validity, and efficiency, and can be used for comprehensively evaluating and ranking guidelines.

Objective@#To explore the association of the frequency of domestic violence witness in childhood with the quality of life (QOL) of college students, and to provide a reference for improving the quality of life of students who domestic violence witness in childhood.@*Methods@#In October 2018, a total of 4 034 college students from four universities in Hefei, Anhui Province, were selected to participate in this study. The Conflict Tactics Scales Short Form (CTS2SF) and the World Health Organization Quality of Life-Brief (WHOQOL-BREF) were used to assess the effect of witnessing domestic violence in childhood on the QOL of college students. Multiple linear regression was used to analyze the relationship between the different frequencies of witnessing domestic violence in childhood and the QOL of college students.@*Results@#The mean scores of the subjective assessment of QOL, health subjective assessment, physiology, psychology, social relationships, and environment were (14.06±3.25) (13.42±3.47) (12.61±2.02) (14.09±2.62) (13.72±2.71) (13.77±2.46), respectively. The multiple linear regression model showed that, witnessing domestic violence in childhood was negatively correlated with the QOL of college students (P<0.01). Although occasional expose to severe family violence in childhood was not associated with the health subjective assessment and the environmental dimensions, the different frequencies of witnessing domestic violence in childhood were negatively correlated with the QOL of college students (P<0.05).@*Conclusion@#Frequency of domestic violence witness in childhood is associated with the mean scores of the QOL of college students. It is necessary to explore effective prevention and intervention programs for exposure to domestic violence in childhood.

The aim of this study was to investigate the effects of Avastin on aquaporin4 (AQP4) expression in human retinal Müller cells in vitro under hypoxia, so as to explore the mechanism of Avastin treating retinal edema. The human Müller cells were cultured using the enzymatic digestion method. Müller cells were identified under the transmission electron microscopy and by using immunofluorescence staining. By using semi-quantitative reverse transcription polymerase chain reaction (RT-PCR), the expression of AQP4 mRNA and VEGF mRNA in Müller cells cultured with 500 μmol/L CoCl(2) for 0, 3, 6, 12 and 24 h, and with 0, 100, 300, 500 and 700 μmol/L CoCl(2) for 24 h was detected. The expression of AQP4 mRNA in Müller cells cultured with 50 ng/mL exogenous vascular endothelial growth factor (VEGF) for 0, 0.5, 1, 2 and 4 h, and with 0, 25, 50 and 75 ng/mL VEGF for 24 h was detected. Amplified cDNA products of AQP4 mRNA in Müller cells cultured with 500 μmol/L CoCl(2) and 200 μg/mL Avastin for 24 h were detected. The results showed that more than 95% cells displayed positive immunofluorescence reaction. Characteristic 8-10 nm intracellular filaments could be seen in the cytoplasm under the transmission electron microscopy. In the CoCl(2) experimental groups, the expression of AQP4 mRNA and VEGF mRNA in Müller cells was increased as compared with the control group. Alteration of AQP4 mRNA and VEGF mRNA levels showed a significantly positive correlation (r (2)=0.822, P<0.05). The expression of AQP4 mRNA in Müller cells was increased by VEGF. The expression of AQP4 mRNA was significantly decreased by Avastin as compared with the control group. It is suggested that Avastin can decrease the expression of AQP4 mRNA in human Müller cells under chemical hypoxic conditions partially via VEGF path, which may be one of the mechanisms of Avastin treating retinal edema.

Objective To analyze clinical and laboratory features, viral load and viral shedding period of patients with mild or severe H1N1 influenza A infection. Methods Seventy mild cases and 16 severe cases with concurrent pneumonia were included from Shcnzhen area for analysis.Nasopharyngeal-swab specimens of patients were collected and viral load was detected by real-time quantitative polymerase chain reaction (PCR) assay during their hospitalization. The viral load and viral shedding period were compared between patients over 14 years old and less than 14 years old, and between 70 mild cases without pneumonia and 16 severe cases with pneumonia. The statistic analysis was performed using t test and chi square test. Results The most common symptoms and signs of the patients were fever, cough and enlargement of tonsils. However, the severe cases suffered more frequently from cough, dyspnea and high fever compared with the mild cases (x2 = 10. 9 and 14.3, respectively, t=3.65; both P<0.01 ). The levels of white blood cell (WBC) count and alanine arninotransferase (ALT) of severe patients were both significantly higher than those of mild patients(t= 3.2, 2.4,respectively; both P<0.05). The chest radiology of the severe cases showed interstitial pneumonia,mostly with ground glass image. The viral load of patients under 14 years was significantly higher than those over 14 years [(4.86± 1.23) lg vs (4. 17±0.89) lg; t=2.3, P<0.05], and the viral shedding period of patients under 14 years was significantly longer than those over 14 years [(5.33±0. 49) d vs(3. 63±0.28) d; t=3.4, P<0.01]. The severe patients also displayed significantly higher viral load and prolonged viral shedding period than the mild patients [(6. 36±1. 44) lg vs (4. 35±0.99) lg, t=6.1,P<0.01; (5.75±1.77) d vs (4. 24±1. 96) d, t=3.2, P<0.01]. Conclusion Age anddisease severity of patients with H1N1 influenza A infection are significantly associated with viral load and viral shedding period.

Vitamin B1 self-assembled monolayer modified electrode (VB1-Au/SAMs/CME) was prepared and its electrochemical behavior was studied by cyclic voltammetry. The results showed that a self-assembled monolayer (SAM) was formed on the surface of gold electrode. \3-/4-redox couple was used as the electrochemical probe to study the electrochemical characteristics of SAM modified gold electrode and the results indicated that the existence of Vitamin B1 membrane on gold surface hindered the electron transfer. The electrochemical behaviors of dopamine(DA) and uric acid (UA) were investigated at VB1-Au/SAMs/CME. The results showed that the electrochemical oxidation of DA and UA could be electrocatalyzed. There were linear relationships between the anodic peak current and the concentration of DA(8.0×10-6-4.0×10-4 mol/L) and UA (6.0×10-5-2.2×10-4mol/L) respectively in differential pulse voltammograms. This modified electrode was effective to detect DA and UA in simultaneous determination of these species in a mixture.

Objective To study the psychological health state of parents of children patients with hand foot mouth disease and provide scientific basement for clinical nursing care. Methods Zung self- rating anxi-ety scale (SAS) and self-designed investigation scale were used to survey 98 parents of children patients with hand foot mouth disease,and selective nursing intervention was adopted. Results The anxiety scores of the parents were significantly higher than those of the normal mode, and their psychological symptom was influ-eneed by various factors. After the nursing intervention, the anxiety degree was significant different from that of before intervention. Conclusions Psychological health levels of the parents of children with hand foot mouth disease were markedly lower than those of healthy individuals. Psychological nursing care should not only in-clude children but also their parents in order to alleviate their bad mental state.
