RESUMEN

Objective@#To investigate the effect of micro steel plate internal fixation combined with traditional Chinese medicine medicated bath in the treatment of proximal phalanx fractures.@*Methods@#From January 2012 to December 2012, 100 patients with proximal phalanx fracture in Ningbo Mingzhou Hospital were selected and randomly divided into control group and observation group, according to the digital table, with 50 cases in each group.The control group was treated by simple miniature steel plate internal fixation, the observation group was treated by microplate internal fixation combined with traditional Chinese medicine medicated bath.The clinical treatment effect was compared between the two groups.@*Results@#The hospitalization time and fracture healing time in the observation group were (8.35±1.62)d and (49.35±3.85)d, respectively, which were significantly shorter than those in the control group [(8.95±1.48)d, (57.82±6.12)d], the differences were statistically significant (t=5.032, 6.114, all P<0.05). The postoperative total active flexion degree and incidence rate of adverse reactions of the observation group (16.00%) was lower than that in control group (34.00%), the difference was statistically significant (χ2=18.485, P<0.05).@*Conclusion@#Microplate internal fixation combined with traditional Chinese medicine medicated bath in the treatment of proximal phalanx fractures has better treatment effect, and it has shorter treatment time and less complications.

Objective:To investigate the effects of Purendan Superfine Powder (PRD) on the retinal hemodynamics and the expressions of cytokines in the diabetic rats, and to elucidate its protective effect on diabetic retinopathy (DR) .Methods:Thirty-six male SD rats were randomly divided into control group, diabetes group, and PRD group (n=12) .The diabetes rat models were established by intraperitoneal injecting streptozotocin (STZ) in diabetes group and PRD group.After successful modeling, the diabetic rats in PRD group were given PRD (1.8g·kg-1) by gavage for 3months.The hemodynamics parameters of central retinal artery (CRA) of the rats were measured with color Doppler ultrasound.The expression levels of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF) mRNA in retina tissue were detected by reverse transcription polymerase chain reaction (RT-PCR) .Immunohistochemical staining and Western blotting methods were used to detect the expression levels of VEGF and bFGF proteins in retina tissue of the rats.Results:Compared with control group, the blood glucose, urine volume, liver index and kidney index of the rats in diabetes group were significantly increased (P<0.05) , the peak systolic velocity (PSV) , end diastolic velocity (EDV) and mean velocity (MV) of CRA were significantly decreased (P<0.05) , the resistant index (RI) and pulsatility index (PI) of CRA were significantly increased (P<0.05) , and the expression levels of VEGF and bFGF mRNA and protein in retina tissue were significantly increased (P<0.05) .Compared with diabetes group, the blood glucose, urine volume, liver index and kidney index of the rats in PRD group were significantly decreased (P<0.05) , the PSV, EDV and MV of CRA were increased (P<0.05) , the RI and PI of CRA were decreased (P<0.05) , the expression levels of VEGF and bFGF mRNA and protein in retina tissue were significantly reduced (P<0.05) .Conclusion:PRD can protect the diabetic retinal microvascular injury by reducing the blood glucose level, increasing the blood supply of retinal tissue, increasing the amount of microcirculation blood perfusion, reducing the vascular bed perfusion resistance, and reducing the expression of VEGF and bFGF.

RESUMEN

Objective To investigate the effect of SVV guided fluid therapy on blood gas and lipopolysaccharide (LPS),procalcitonin (PCT) in patients undergoing resection of gastrointestinal tumor.Methods Sixty patients aged 60 85 years from Jan,2016 to Feb,2017 falling into ASA physical status Ⅱ or Ⅲ,scheduled for elective radical operations for gastrointestinal cancers,were includ ed and randomly divided into two groups (n =30 each) using a random number table:ScvO2 guided fluid therapy (group C),SVV guided fluid therapy (group G).MAP,HR and CVP of patients were recorded at the same time before anesthesia (T0),tumor removal (T1) and the end of surgery (T2)respectively.PH,BE,HCO3-and Lac of venous blood samples and artery blood samples at T0-T2,6 hours after surgery(T3) were recorded.Venous blood samples were collected at T0,T3 to detect LPS and PCT.Crystalloid requirements,colloid requirements,total volume,bleeding volume,peritoneal fluid volume and the use of dopamine were recorded.The time of PACU,time when the patients first exhausted and was fed after operation,length of hospital stay after operation were recorded.Results Compared with group C,BE of artery blood was obviously increased at T2,T3 in group G (P<0.05);the Lac of artery at T2 and the Lac of artery and venous blood at T3 in group G was obvi ously decreased.LPS and PCT were decreased at T3 in group G (P<0.05).Compared with group C,the needed colloid was increased in group G,the needed crystalloid and total volume of fluid infused were decreased in group G (P<0.05).Compared with group C,the time of PACU starting to exhaust and feed,length of hospital was shortened in group G (P<0.05).Conclusion SW guided goal directed fluid therapy is more conductive to maintain the acid base and reduce the incidence of in fection for the patients with gastrointestinal tumor operation,promote the recovery of gastrointestinal function and decrease the length of hopital after operations.

RESUMEN

Objective To investigate whether brachial-ankle pulse wave velocity(baPWV),independently and jointly with the level of body mass index(BMI),affect the risk of incident diabetes with hypertension. Methods The effects of baPWV and BMI level on incident diabetes with hypertension were evaluated using Kaplan-Meier curves (log-rank test) and multi logistical regression models after adjustment for various confounding factors. Results A total of 2 222 participants were recruited in this study. Higher baPWV (Q4) was associated with a greater risk of incident diabetes with hypertension simultaneously compared with baPWV in the lower quartiles(Q1-Q3),with HR 4.16(95% CI 2.14,8.09)in the crude model,2.52(95% CI 1.10,5.78) in modelI,and 2.45(95% CI 1.05, 5.70)in modelⅡ.The highest risk of diabetes with hypertension simultaneously was in the group with high baPWV and BMI≥25 kg/m2,with HR 11.50(95% CI 4.5,29.9)in the crude model,4.20(95% CI 1.4,13.1)in modelI,and 4.30(95% CI 1.4,13.3) in modelⅡ. Conclusions The joint effect of baPWV and BMI level on incident diabetes with hypertension simultaneously is a better predictor than their independent effect. The risk of incident diabetes with hypertension markedly increases in the subjects with BMI≥25 kg/m2and higher baPWV.

This study is to investigate the effect of (2-methyl-n-butyl) shikonin (MBS) on inducing apoptosis of human gastric cancer cell line SGC-7901 and the role of ERK1/2 signal pathway in the apoptosis. MTT assay was used to detect SGC-7901 cell proliferation. DNA condensation was measured by DAPI stain. Cell apoptosis was analyzed by flow cytometry. Mitochondrial membrane potential (MMP) was analyzed by JC-1 staining. The protein expressions of Bcl-2, Bax, Survivin, cleaved caspase-9, cleaved caspase-3, cleaved PARP, p-ERK1/2, ERK1/2, p-JNK, JNK, p-p38 and p38 were detected by Western blotting. The results showed that MBS reduced the cell viability of SGC-7901 cells in a dose- and time-dependent manner. The IC50 at 24 h and 48 h for SGC-7901 cells was 10.113 and 4.196 micromolL(-1), respectively. After being treated with MBS, the typical nuclear condensation was observed in SGC-7901 cells by DAPI stain. Apoptosis in SGC-7901 cells was induced by MBS in a dose dependent manner. The protein expression of Bcl-2 was down-regulated, while the protein expressions of cleaved caspase-9, cleaved caspase-3, cleaved PARP, p-ERK1/2 and p-JNK were up-regulated after MBS treatment. U0126, a specific MAP kinase (MEK1/2) inhibitor, blocked the ERK1/2 activation by MBS. MMP was decreased by MBS treatment. It can be concluded that MBS could inhibit SGC-7901 cell proliferation and induce apoptosis. Mitochondrial apoptosis pathway, ERK1/2 signal pathway and JNK signal pathway might be involved in this process.

Objective To investigate the effects of hyperbaric oxygen (HBO) therapy following spinal cord injury (SCI) and its mechanisms. Methods Thirty Sprague-Dawley rats were randomly divided into two groups after inducing SCI models using a modified version of Allen's method. The HBO group received HBO treatment 2 h after the procedure and were then treated 100 min every day for 5 consecutive days. All the rats were evaluated 1 h before the operation, and 1 h, 10 d and 20 d afterward using BBB scores and inclined plane experiments. Superoxide dismutase (SOD) and malondialdehyde (MDA) levels were measured. At twenty days, all the rats were sacrificed and their spinal cords were examined pathologically using HE staining. Results Average BBB scores and climbing ability in the HBO group were better than in the control group at the 10th and 20th day after the operation. Compared to the control group, SOD increased significantly and MDA decreased significantly in the HBO group at the 2nd and 5th day after the operation. There was less cystic degeneration of the spinal cord in the HBO group. Conclusions HBO demonstrated a positive effect after SCI. Oxygen free radicals might be one of the mechanisms for the better recovery.

RESUMEN

The non-contact life parameters detecting systems can detect many important life parameters such as heartbeat and breathing at certain distance. In practice, the self-dithering interference of the system is produced by some factors; it makes the signal-to-noise ratio (SNR) of the recorded signal lower. This paper is discussed an adaptive technology based on a variable step-size LMS algorithm, which is used to eliminate the self-dithering interference of the system and hence to raise the SNR of the detecting signal. A method to obtain the reference signal by vibration sensor is brought forward and the optimum parameters settings are discussed. The results show that the reference signal is an integral part of output signal of the vibration sensor and has a relationship with the setting direction of the sensor. The conclusion is that the method is effective and feasible; also the algorithm is well convergent.
