Correção da ruptura de cordas tendíneas na insuficiência mitral degenerativa pelo emprego de cordas padronizadas de pericárdio bovino / Surgical repair of chordae tendineae rupture after degenerative valvular regurgitation using standardized bovine pericardium

OBJETIVO: Avaliar, clinicamente e pelo ecodopplercardiograma, o funcionamento da valva mitral em 22 pacientes submetidos à correção do refluxo valvar com substituição das cordas tendíneas nativas por cordas padronizadas de pericárdio bovino. MÉTODOS: Os pacientes apresentavam insuficiência mitral degenerativa. Quatorze (63,6%) pacientes eram do gênero masculino e a idade variou de 19 a 76 anos (média 56,8±13,8 anos). As cordas de pericárdio bovino foram tratadas com glutaraldeído, com reforço de suas extremidades transversais formando um trapézio. RESULTADOS: Um (4,5%) paciente faleceu no pós-operatório imediato em síndrome de baixo débito cardíaco e três (13,6%) no pós-operatório tardio. Uma (4,5%) paciente foi reoperada. As curvas atuariais de sobrevivência livre de óbitos por causa cardiovascular e livres de reoperações para os pacientes que deixaram o hospital (21) demonstraram taxas de 82,0±9,8% e 83,9±10,4%, aos 70 meses de pós-operatório, respectivamente. Dezessete (77,3%) pacientes estão vivos com a própria valva. Dos 17 pacientes vivos com a própria valva 16 (94,1%) estão em classe funcional I. O ecodoppler pós-operatório (média de 41 meses; 4 a 70 meses) demonstrou ausência de regurgitação mitral em 11 (64,7%) pacientes e regurgitação discreta em cinco (29,4%). CONCLUSÃO: A técnica de implante de cordas padronizadas de pericárdio bovino para substituição de cordas tendíneas da valva mitral em pacientes com insuficiência mitral degenerativa demonstrou resultados bastante satisfatórios.

OBJECTIVE: To evaluate clinically and by Doppler Echocardiography 22 patients submitted to mitral valve repair after valvular regurgitation using standardized bovine pericardium chordae. METHODS: The patients had degenerative mitral regurgitation. Fourteen (63.6%) patients were male and the age ranged from 19 to 76 years (mean 56.8 ± 13.8 years). The strings of bovine pericardium treated with glutaraldehyde were reinforced in its transverse ends forming a trapezoid. RESULTS: One patient (4.5%) died in the immediate postoperative period with in low cardiac output syndrome and three (13.6%) in the late postoperative period. One patient (4.5%) was reoperated. The actuarial curves for survival free of death from cardiovascular causes and free from reoperation for patients who left the hospital (21), showed rates of 82.0 ± 9.8% and 83.9 ± 10.4% at 70 months postoperatively, respectively. Seventeen patients (77.3%) are alive with native valves. Of the 17 patients alive with native valves 16 (94.1%) were in functional class I. The Doppler Echocardiography postoperatively (mean 41 months, 4-70 months), showed no mitral regurgitation in 11 (64.7%) patients and mild regurgitation in five (29.4%). CONCLUSION: The technique of standard cords of bovine pericardium implantation to replace chordae tendineae of the mitral valve in patients with degenerative mitral regurgitation showed satisfactory results.

Correçäo cirúrgica simultânea da fibrilaçäo atrial e da insuficiência mitral em criança / Simultaneous surgical correction of the atrial fibrillation and the mitral insufficiency in child

Plastia valvar mitral combinada com cirurgia de COX (labirinto) foi realizada em uma paciente de 12 anos, para correçao de insuficiência mitral reumática e fibrilaçao atrial crônica. Um ano após a cirurgia, a paciente encontra-se assintomática e em ritmo sinusal. O ecodopplercardiograma mostrou reduçao importante do átrio esquerdo e o cateterismo, assim como o ecodopplercardiograma confirmaram sístole atrial efetiva.

Correçäo cirúrgica da conexäo venosa anômala do pulmäo esquerdo sem circulaçäo extracorpórea / Surgical correction of anomalous venous connection of the left lung without extracorporeal circulation

Surgical correction of anomalous pulmonary venous connection from the left lung to the innominate vein (through a vertical vein) without cardiopulmonary bypass, was performed in two patients, with excellent evolution. After median sternotomy, the ascending vertical vein was cut obliquely close to the vein, and connected to the left atrial appendage. In the postoperative period, the patients were in NYHA class I and the hemodynamic study showed normal venous drainage from the left lung to the left atrium

Técnica de Cox sem crioablaçäo para tratamento cirúrgico da fibrilaçäo atrial / Cox technique without cryoablation for surgical treatment of atrial fibrillation

Seis pacientes foram submetidos ao tratamento cirúrgico da fibrilaçao atrial (FA) pela técnica do labirinto (Cox), sem a utilizaçao da crioblaçao. Quatro pacientes apresentavam estenose mitral (EM) associada, 1 insuficiência mitral e 1 dupla lesao mitral. Um dos pacientes apresentava estenose tricúspede e 2 insuficiênicia tricúspede associadas. o diâmetro médio do átrio esquerdo, medido pelo ecocardiograma era de 6,0 cm (5-7,3). A etiologia era reumática em 5 pacientes e degenerativa em 1. Plastia valvar mitral foi realizada em 3 pacientes, comissurotomia mitral em 2 e troca valvar mitral em 1. Em 4 pacientes havia tormbos no átrio esquerdo, um deles também no átrio direito. o tempo médio de circulaçao extracorpórea foi de 123 minutos 9110-142) e de anóxia do miocárdio de 91 minutos (80-108). Na sala de operaçoes o ritmo inicial era juncional em 4 e sinusal em 2. No pós-operatório imediato, todos apresentavam ritmo juncional, passando para ritmo sinusal em todos, exceto em 1. Os pacientes receberam alta hospitalar (8-27 dias) em boas condiçoes, sem utilizaçao de drogas antiarrítimicas e o Holter de 24 horas mostrava ritmo sinusal em 5 pacientes e ritmo atrial ectópico em 1 paciente. O estudo hemodinâmico e o ecodoppler revelaram contraçao atrial sincrônica em todos os casos, exceto 1. O diâmetro atrial esquerdo médio era de 4,8 cm (3,7-5,2).

Nova técnica reconstrutora na insuficiência mitral por alongamento das cordas tendíneas da cúspide anterior: relato de caso / A new reconstructive technique in cases of mitral insufficiency and elongated cordae tendineae of the anterior leaflet: report of a case

Uma nova técnica de encurramento de cordas tendíneas alongadas é proposta. Através de pequena incisäo longitudinal na cúspide anterior próximo ao bordo livre ae à inserçäo da corda alongada, esta é puxada para cima com um tracionador de nervos. Desta forma a cúspide anterior prolapsada desce em direçäo ao mesmo nível da cúspide posterior. Em seguida sutura-se o orifício fixando a porçäo alongada da corda sobre a face atrial da cúspideanterior. Esta técnica foi aplicada em uma paciente com insuficiência mitral importante corrigindo-a satisfatoriamente. Devido à presença de diâmetro ântero-posterior aumentado do anel mitral foi implantado anel Gregori IMC para sua correçäo. A excelente evoluçäo clínica, assim como os bons resultados avaliados pelo estudo hemodinâmico e ecodopplercardiografia, demonstram tratar-se de procedimento que reproduz as condiçöes anatômicas próprias da valva mitral, indicado na insuficiência mitral por alongamento de cordas da cúspide anterior.

A new technique is proposed for mitral incompetence due to elongated chordae tendineae of the anterior leaflet. It consists in shortening the chordae tendineae of the leaflet level. First, we make a longitudinal orifice in the anterior leaflet just near its free edge and the elongated chordae tendineae. Through the orifice and with the help of a micro nerve hook we pull the elongated chordae the necessary to keep the anterior leaflet in the same level of the posterior leaflet. The orifice is then closed with a polipropilene 5-0 suture. The mitral annuloplasty was performed with a Gregori IMC prosthetic ring. The excellent clinical and laboratorial data suggest that mitral insufficiency due to elongated chordae tendineae can be corrected by this technique.
