RESUMEN

As one of the pathogenic mechanisms contained in The Inner Canon of Yellow Emperor (《黄帝内经》), “disease with latent pathogen induced by a new pathogen” means that the induced new pathogen resulted to a combination of the latent previous pathogen and the new pathogen, which caused the disease. Based on this, it is believed that the change of “nodule-cancer transformation” of pulmonary nodules is actually based on the deficiency of original qi, and the new pathogen induces the latent pathogens like phlegm coagulation, qi stagnation, blood stasis, toxicity, so healthy qi can not drive the pathogens out, and the long-time detention generated into cancerous turbidity, and deve-loped into cancerous tumour at the end. Therefore, based on the three-stage treatment of unformed cancer, dense cancerous toxin, and developed cancer, the clinical practice applied six methods of clearing, expelling, dissipating, tonifying, harmonizing, and transforming, taking into account both the manifestation and root cause, moving the treatment window of pulmonary nodules forward, attacking the pathogens when the toxin was not yet overbearing, supporting the healthy qi before declining, delaying the process of nodules-cancer transformation, and providing ideas for the prevention and treatment of pulmonary nodules “nodule-cancer transformation” in traditional Chinese medicine.

RESUMEN

Objective@#To observe the efficacy of the combination of chemotherapy and Ginseng Rg3 on advanced non-small cell lung cancer(NSCLC).@*Methods@#In the multi-center, large-sample, randomized, double blind trial, 414 patients with Ⅲ-Ⅳ NSCLC were enrolled.199 were in the experimental group and 215 the control group. The patients in the experimental group were treated with the standard first-line chemotherapy combined with Ginseng Rg3. The patients in the control group were treated with the same chemotherapy combined with placebo. Median overall survival (OS), Karnofsky performance scale (KPS), Traditional Chinese Medicine (TCM) symptoms score and side effects of two groups were observed as main indexes.@*Results@#The median OS were 12.03 months in the experimental group, which was significantly better than that in the control group (8.46 months, P<0.05). Hemoglobin and white blood cells were decreased after the first and second cycle of treatment in both groups. Both adverse events were significantly milder in the treatment group (P<0.05). In addition, after two courses of treatment, the KPS of patients was 78.95±9.14 in the experimental group and 76.77±9.15 in the control group, while the TCM symptoms score was 2.45±1.73 in the experimental group and 2.92±2.06 in the control group, with significant difference (P<0.05).@*Conclusions@#Combination of TCM with Western medicine such as chemotherapy could prolong the survival of patients with advanced NSCLC. The combined therapy improved patients′ symptoms and reduced chemotherapy induced myelosuppression.

Objective To investigate the mechanism of inducing production of vascular endothelial growth factors (VEGF) by recombinant human S100 calcium binding protein A4 (rhS100A4) in rheumatoid arthritis fibroblast-like synoviocytes (RAFLSs).Methods Synovial tissue was sampled from the patients with rheumatoid arthritis (RA) undergoing knee arthroplasty for in vitro culture of RAFLSs.CCK-8 assay was conducted to detect the effect of rhS100A4 and the effect of its interaction with Rapamycin (Rap),an inhibitor of mammalian rapamycin target 1 (mTORC1) signaling pathway,on the proliferation of RAFLSs.The effects of rhS100A4 and its interaction with Rap on the expression of VEGF in RAFLSs were detected by immunofluorescence.After rhS100A4 and its cooperation with Rap stimulated the conditioned medium (CM)produced by RAFLSs,the effect of CM on formation of lumen in human unbilical vein endothelial cells (HUVECs) in vitro was observed to detect the angiogenic ability of rhS100A4.Western blot was used to detect the effect of rhS100A4 on the phosphorylation of downstream ribosomal protein S6 (S6) in the mTORC1 signaling pathway in RAFLSs and to analyze the effects of rhS100A4 and Rap on phosphorylation of S6 protein and expression of VEGF protein in RAFLSs.Results rhS100A4 promoted cell proliferation and expression of VEGF protein in RAFLSs,and the CM formed by rhS100A4 promoted HUVECs to form blood vessels in vitro.Rap inhibited the above biological effects of rhS100A4,rhS100A4 activated the downstream protein S6 in the mTORC1 signaling pathway in RAFLSs cells to increase their phosphorylation levels.The effects of rhS100A4 on the phosphorylation of S6 protein and on the expression of VEGF protein in RAFLSs were inhibited by Rap.Conclusion rhS10OA4 promotes production of VEGF in RAFLSs by activating the mTORC 1 signaling pathway.

Objective:To study the curative efficacy of functional training focusing on muscle group around shoulder for subacromial impingement syndrome.Method:Forty five patients with subacromial impingement syndrome (SIS) were randomly grouped into three groups (fifteen patients for each group).Patients in Group Ⅰ were trained only on muscles around scapula.Patients in Group Ⅱ were treated on rotator cuff muscles.While patients in Group Ⅲ were treated on both.The training goes every 2 days and last for 4 weeks.The indexes for evaluating curative efficacy included:the Neer-test,the Hawkins-test and the Peen Shoulder Score.These indexes were measured immediately and three months after the four week treatment.Result:After treatment of the functional training focusing on muscle group around shoulder,Neer-tests and Hawkins-tests of all of patients were changed from positive to negative,the degree of pain compared with pain before the treatment declined apparently(P<0.01),at the same time,the degree of satisfaction and the function went up remarkablely(P<0.01).Compared with group Ⅰ and group Ⅱ,it dropped the most notably on the degree of pain in the group Ⅲ(P<0.01),however,the degree of satisfaction and the function rose most apparently (P<0.01) Conclusion:The curative efficacy of functional training focusing on muscle group around shoulder for sub-acromial impingement syndrome was effective apparently.But the efficacy of treated focusing on both muscles around scapula and rotator cuff was the most remarkably.

AIM:To study the expression level of S100 calcium-binding protein A4 (S100A4) in synovial tissue of the knee joint in rheumatoid arthritis (RA) patients and normal persons, and the effect of S100A4 on the angiogenesis induced by rheumatoid arthritis fibroblast-like synoviocytes (RAFLSs).METHODS:The synovial tissue was taken from the knee joint of the RA patients (RA group) and the normal persons (control group).The protein expression of S100A4 and vascular endothelial growth factor (VEGF) in the synovial tissue of the 2 groups was observed by immunohistochemistry.RAFLSs were isolated from synovial tissue of patients with active RA.ELISA was used to detect the effect of S100A4 on the secretion of VEGF by RAFLSs.The effect of S100A4 on the angiogenesis of HUVECs cultured with conditioned medium from RAFLSs was also detected.RESULTS:The protein of S100A4 and VEGF was highly expressed in the synovial tissues of RA group (P<0.05).rhS100A4 significantly stimulated the secretion of VEGF in RAFLSs in a time-and dose-dependent manner (P<0.05).Cultured with conditioned medium from RAFLSs, rhS100A4 significantly promoted HUVECs to form tube-like structures in vitro.CONCLUSION:S100A4 protein is highly expressed in synovial tissue of the knee joint in RA patients, and S100A4 stimulates synovial angiogenesis by promoting RAFLSs to generate VEGF, indicating that S100A4 may be used as a potential target for the treatment of RA.

Objective To observe the effects of dredging collaterals and activating blood worm Chinese materia medica on angiogenesis related factors of lung cancer in hypoxic environment. Methods The lung cancer A549 cells were cultured in vitro to simulate tumor hypoxia microenvironment by the hypoxia workstation, and different concentrations of Scorpio, Scolopendra and Gecko medicated serum were added. MTT method was used to detect cell proliferation and screen the best medicine concentration and duration of action. Lung cancer A549 cells were administrated by the three kinds of medicated serum, and cells were collected and supernatant was cultured. Contents of VEGF, TGF-β1, and bFGF were detected by ELISA. Results Three kinds of medicated serum had the inhibitory effect on both added normoxia and hypoxia in cultured A549 lung cancer cells. 7.5% concentration of medicated serum was selected, and 24 h later were used in later experiments. Scorpio, Scolopendra and Gecko medicated serum can more reduce the contents of VEGF, TGF-β1 and bFGF in the supernatant of A549 cell compared with the control group (P<0.05, P<0.01). Conclusion Dredging collaterals and activating blood worm Chinese materia medica had inhibitory effect on cancer cells and the regulation of angiogenesis related cytokines in the condition of normoxia and hypoxia.

Objective To observe the initial mechanism investigation in immune escaping correlated T cell proportion of Lewis lung cancer by Fuzheng-Peiyuan formula. To provide a theoretical basis for the application of the Chinese medicine of strengthening the body resistance in clinics. Methods The healthy C57BL/6J mice were divided into six groups by the random number table: the Chinese medicine group, the Chinese medicine plus chemotherapy group, the chemotherapy group, the model group and the normal control group. There were 6 mice in each group. All the groups expect the normal control group were inoculated subcutaneously with Lewis lung cancer cell suspension 0.2 ml. The medicine given to the Chinese medicine group were orally Fuzheng-Peiyuan formula with 8.17 g/kg. The combination group was given intraperitoneal injection of cyclophosphamide 60 mg/kg and orally Fuzheng-Peiyuan formula 8.17 g/kg. The chemotherapy group received the injection of cyclophosphamide 60 mg/kg and intragastric administration of normal saline. The model group and normal control group were administered with saline. After continuous administration for 13 days, the expressions of CD4, Foxp3, CD8 and CD28 in spleen cells of tumor bearing mice were observed by flow cytometry. Results Compared with the model group, the chemotherapy group and the chemotherapy combined with Chinese traditional medicine group showed that tumor weight (1.76 ± 0.42 g, 1.40 ± 0.43 g vs. 4.37 ± 0.59 g) significantly decreased (P<0.01); and the CD4+ Foxp3+ cells of mouse spleen cells in the Chinese medicine group and Chinese medicine plus chemotherapy group (11.25% ± 1.69%, 9.30% ± 2.68% vs. 14.21% ± 1.50 %) significantly decreased (P<0.05 or P<0.01). Conclusions The result initial proved that the Chinese medicine could strengthen the body resistance, adjust the proportion of Treg and CTL in spleen T cell in tumor-bearing mouse.

The preoperative medication can improve lung cancer patients’ surgical tolerance, and postoperative medication can facilitate patients’ postoperative rehabilitation. And if combined with chemotherapy, they cound reduce toxicity and enhance efficiency, enhance the patients’ immunity and improve their life quality. Research shows that classical prescriptions can improve the immunity of lung cancer model mice, facilitate apoptosis of tumor cell and control its migration. This paper sumed up the clinical and experimental research of lung cancer treatment with classical prescriptions in recent 5 years.

BACKGROUND:Atorvastatin has been shown to reduce bone loss and fracture, but its effects on implant osseointegration remain unknown. OBJECTIVE:To investigate the effects of atorvastatin on implant osseointegration in osteoporotic rats and the underlying mechanisms. METHODS:Forty-eight Sprague-Dawley rats were randomized into sham-surgery, ovariectomy, and atorvastatin (10 and 20 mg/kg per day) treatment groups, respectively. Al rats received ovariectomy and implant surgery except those in the sham-surgery group. Bone mineral density of the lumbar vertebra, osseointegration ratio and pul-out strength of implants were measured after 12-week treatment.Levels of bone formation and resorption markers in osteoblasts treated with atorvastatin were determined by ELISA. Wnt pathway-relatedgene expression was detected by RT-PCR. RESULTS AND CONCLUSION:Bone mineral density, osseointegration ratio and pul-out strength of implants were significantly increased in 20 mg/kg per day of atorvastatin treatment group compared with ovariectomy group (P< 0.05). Levels of alkaline phosphatase, osteocalcinand osteoprotegerinwere significantly increased in osteoblasts treated with atorvastatinin vitro(P<0 .05), and the level of osteoclast differentiation factor RANKL was significantly inhibited (P< 0.05). Meanwhile, atorvastatin significantly promoted the mRNA expression of low-density lipoprotein associated protein 5and β-catenin, and inhibited the mRNA expression of dickkopfWnt signal pathway inhibitor 1and sclerostin. Our results suggest that atorvastatin promotes implant osseointegration in osteoporotic rats by activating Wnt/β-catenin signal pathway.

This article was aimed to study medication rules of traditional Chinese medicine (TCM) in the treatment of gastric cancer. The history information of 111 hospitalized cases from 2008 to 2011 was collected from the Department of Oncology of Guang'anmen Hospital with the clinical collection system. The medication rules in the treatment of gastric cancer by TCM were explored by the scale-free network analysis from several aspects, such as effects, classifications and compatibility relations of Chinese medicine. The analysis result of 111 gastric cancer cases showed that strengthening the body resistance was the main treatment principle. Liujunzi decoction was the most frequently used prescription. In TCM compatibility, deficiency-nourishing herbs were the main ingredients, especiallyqi-supplementing herbs which take the first place in the prescription. And the heat-reducing herbs, dampness-eliminating herbs, blood-activating and stasis-eliminating herbs, food stagnation resolving herbs and qi regulation herbs were also used in the combination. Core herbs in the prescription for the treatment of gastric cancer were Tuckahoe, Codonopsis pilosula, Astragalus root, Pericarpium Citri Reticulatae, Fructus Aurantii, Nidus Vespae and etc. TCM treatment, syndrome differentiation and medication are varied according to different therapeutic stages and disease stages. The result showed that scale-free network analysis had certain significance in the understanding of medication rules of TCM treatment of gastric cancer.

AIM:To explore the effect of Toll-like receptor ( TLR) 2 and TLR4 in Mycobacterium bovis Bacil-lus Calmette-Guérin (BCG)-induced human proximal renal tubule epithelial cell (HK-2) injury.METHODS:HK-2 cells were stimulated by BCG, and the expression of TLR2, TLR4, chemokine (C-X3-C motif) ligand 1 (CX3CL1) and trans-forming growth factor beta 1 ( TGF-β1 ) was detected by quantitative real-time PCR and Western blotting .TLR2 monoclonal antibody and TLR4 inhibitor were used to treat the HK-2 cells 1 h before BCG stimulation.The expression of CX3CL1 and TGF-β1 was evaluated by quantitative real-time PCR and Western blotting .RESULTS: BCG increased the expression of TLR2, TLR4, CX3CL1 and TGF-β1 in the HK-2 cells.Additionally, the expression of CX3CL1 and TGF-β1 was inhibited partly by TLR2 monoclonal antibody or TLR4 inhibitor.CONCLUSION:BCG is able to increase the production of TLR 2, TLR4, CX3CL1 and TGF-β1 in the HK-2 cells.TLR2 and TLR4 signaling pathways play important roles in tubule epitheli-al cell injury induced by BCG .

This article was aimed to study the characteristics of intestinal cancer by traditional Chinese medicine (TCM) based on the scale-free network analysis method. History information of 145 hospitalized cases from 2008 to 2011 was collected from the Oncology Department, Guang A nmen Hospital, in the clinical collection system. The rule of medication in the treatment of intestinal cancer by TCM was explored by scale-free network analysis method from several aspects, such as effects, classification and compatibility relations. The analysis results of 145 intestinal cancer cases showed that strengthening the body resistance was the main treatment principle. Si-Jun-Zi (SJZ) decoc-tion was used with the highest frequency. In the prescription design, qi-supplementing herbs were the most. The heat-clearing, dampness-eliminating, qi-regulating, blood-activating and stasis-removing, food stagnation removing and phlegm-removing herbs were also used frequently. The TCM treatment and syndrome differentiation will be changed due to the treatment and disease stages. It was concluded that scale-free network analysis method is able to relatively show the TCM treatment rule of intestinal cancer quantificationally and intuitively.

Objective To explore the effects of matrine and oxymatrine on apoptosis in human hepatocarcinoma SMMC-7721 cells.Methods The MTT assay and double staining of annexin V-fluorescein isothiocyanate(annexin V-FITC)/propidium iodide(propidium iodide, PI)were used to detect proliferation and apoptosis of SMMC-7721 cells, respectively.Results When the concentrations of matrine and oxymatrine were 0.50 mg/ml, 1.00 mg/ml and 2.00 mg/ml, the proliferation inhibition rates in SMMC-7721 cells was gradually increased in a dose- and time-dependent manner,and the inhibition of matrine on proliferation were greater than that of oxymatrine in the same concentration(1.00 mg/ml)(24 h:42.39%±0.04%vs. 21.36%±0.02%;48 h: 51.69%±0.03%vs. 36.16%±0.02%;72 h: 78.98%±0.05%vs. 61.24%±0.13%;allP<0.05). When the concentrations of matrine and oxymatrine were 0.25 mg/ml, 0.50 mg/ml and 1.00 mg/ml, the apoptosis rates of SMMC-7721 cells were significantly increased;and induction of matrine in apoptosis in SMMC-7721 cells was greater than that of oxymatrine at the same time point(48 h)(apoptosis rates in 0.25 mg/ml, 4.08%±0.20%vs. 2.20%±0.18%;0.50 mg/ml: 4.32%±0.19%vs. 3.08%±0.26%;1.00 mg/ml: 9.93%±0.18%vs. 9.01%±0.20%;allP<0.05).Conclusion Matrine and oxymatrine can inhibit proliferation and promote apoptosis human hepatocarcinoma SMMC-7721 cells.

Objective To explore the compound matrine injection on human hepatoma smmc-7721 cells' proliferation.Methods Human hepatoma smmc-7721 cells were sterilely cultured.Assayed the effects of compound matrine injection on smmc-7721 cells by MTT.The concentrations of the compound matrine injection were 3.34,2.38,1.43 and 0.48 mg/ml; Used the double staining method of annexin V-fluorescein isothiocyanate(Annexin V-FITC)/propidium iodide (propidium iodide,PI) to measure the apoptosis of hepatoma cell.The concentrations of the compound matrine injection were 0.86,1.72,and 3.43 mg/ml.Results The apoptosis rates of compound matrine injection with different concentration of 3.34,2.38,1.43 and 0.48 mg/ml on liver cancer cell SMMC-7721 were51.03%、53.67%、49.83% and 0.03% respectively after 24 hours; and 67.14%、65.35%、62.25% and 0.05% respectively after 48 hours;and 74.16%、68.77%、66.04% and 26.02%respectively after 72 hours.The apoptosis rates of compound matrine injection with different concentration of 0.86,1.72 and 3.43 mg/ml on liver cancer cell SMMC-7721 were(2.86±0.35)%,(15.16± 1.15)%and(13.17±0.40)% respectively after 24 hours; and (8.57±0.44)%,(28.07±0.76)% and (29.81±8.10)%respectively after 48 hours,which were significantly higher than the control group (1.05±0.09)%(P<0.05).Conclusion The compound matrine injection has a significant inhibition on human hepatoma smmc-7721 cells' proliferation and the mechanism was its inducing cell apoptosis.

BACKGROUND: Though there were many experiments addressing repairing osteochondral defects before, faulty restoration occurred at coupling interfaces. OBJECTIVE: To investigate the feasibility of repairing of osteochondral composite defects in rabbit knees with animal-origin osteochondral scaffold combined with bone marrow mesenchymal stem cells (BMSCs)/chondrocytes.METHODS: New Zealand white rabbits were randomly divided into the experimental, control and blank groups and prepared for unilateral knee joint osteochondral defects. Animal-origin osteochondral scaffold combined with BMSCs/chondrocytes, animal-origin osteochondral scaffold and no material was implanted to repair the defects in the experimental, control and blank groups, respectively. Healing condition was evaluated by gross observation, hematoxylin-eosin staining, and toluidine blue staining at 4, 8, and 12 weeks after operation. RESULTS AND CONCLUSION: At 12 weeks after operation, gross observation showed the defects were repaired completely without local depression and the regenerated tissues were fused with surrounding tissues in the experimental group. Hematoxylin-eosin staining and toluidine blue staining revealed that there were many new hyaline cartilages in the cartilage defects in which columnar cells were lined well and cartilage lacuna was obviously, also, there were many bony tissues in the bone defects. The regeneration cartilage, the underlying subchondral bone and host bone were coupled completely. The toluidine blue positive rate and histologic scores of the experimental group were superior to those of the control and blank groups (P < 0.05). It is demonstrated that animal-origin osteochondral scaffold combined with BMSCs/chondrocytes is an ideal method to repair defects between cartilage and the underlying subchondral bone.

Objective Observing the effect of Shuganyishen formula on hormone-dependant metastatic breast cancer and the mechanisms of anti-resistance and endocrine therapy. Methods 226 ITT cases were divided into two cohorts by treatment. 126 cases were treated with the combination of Shuganyishen formula and standard western medicine including endocrine therapy, and 100 cases were treated with western medicine alone. The end points were time to progress (TTP) and overall survival and quality of life (QOF). The effect of Shuganyishen formula on growth of TAM-resistance cell line in vitro was observed. Results There were statistical significances of median TTP between combination and western medicine in Luminal A (ER+ /PR+, HER2-) as well as Luminal B (ER+/PR+, HER2+), but no significances of OS; The extent and incidence of hot flush, sweat and syndromes related to bone were decreased significantly in combination treated group. Shuganyishen formula could reverse resistance to TAM. Conclusion Patients with LuminalA and LuminalB metastatic breast cancer benefit from Shuganyishen formula, which maybe related with its reverse resistance to endocrine therapy.

In addition to kill tumor cells through direct cytotoxicity, the immunomodulatory activity is another facet of chemotherapy. The major molecular mechanisms of immunogenicity of cytotoxic drugs are elimination of CD_4~+ CD_(25)~+ regulatory T cells or Myeloid-Derived Suppressor Cells, and sensitization of tumor cells to immune effectors through modulation of death receptor expression. These studies provide scientific evidence for combinatorial treatments including vaccines and Traditional Chinese Medicine for cancer therapy.

Objective To investigate the clinical value and relating problems in treating atlantoaxial instability by using transpedicular instrumentation with fusion. Methods The study reviewed 18 patients (11 males and 7 females, at age range of 13-82 years, mean 46.5 years) with atlantoaxial instability undergone transpedicular screw internal fixation with bone grafts fusion. There were 15 patients with type Ⅱ odontoid fractures, two with traumatic disruption of transverse atlantal ligament and one with fracture of the anterior ring of C1, all of which were associated with atlantoaxial subluxation or obvious instability. Preoperative JOA score was 6-13 points (average 9.5 points). Results The operation lasted for mean 115 minutes (range 75-180 minutes), with intraoperative blood loss of mean 235 ml (range 130-450 ml). One patient presented intraoperative plexus venous bleeding during removal of lower edge of the posterior arch of atlas and was treated with hemostasis using compession of gelatin sponge. All the patients were followed up for a mean period of 13.5 months (6-38 months), which showed no complications including infection, loosening or breakage of internal fixators or neurovascular injury related to internal fixation. Postoperative JOA score was 12-17 points (average 14.5 points). Reduction and solid fusion of the bone graft were achieved satisfactorily in all patients. Conclusions Posterior transpedicular screw internal fixation with bone grafts fusion can achieve good reduction, reliable fixation, high fusion rate and is a reliable method to manage atlantoaxial instability. Correct selection of the indication, familiarity with local anatomy and mastery of the operation technique are key to a satisfactory curative effect.

RESUMEN

Objective The prospective study was trying to explore the altering tendency of plasma concentration of E2, P and PAS after TCM intervention for post-surgery breast cancer patient who received endocrine therapy (tamoxifen).Methods By randomization, parallel, coincidence measures and the lab. technologies of AXSYM registered at Abbott lab. in the USA, plasma concentration of E2, P and FAS were tested before and after treatment. Results The results of biochemical test of plasma E2, P and FAS concentration of the two arms showed no significance difference before and after the intervention,indicating the side effects relating to TAM had nothing to do with plasma E2, P and FAS level. Conclusion It could be predicable that there must be some other by-pass ways initiating breast cancer rather than the way directly altering the level of plasma concentration of E2, P and FAS.
