RESUMEN

PURPOSE: This study was performed to evaluate the relationship of the expressions of clusterin with Ki-67 and the clinicopathological factors and significance of clusterin expression in human renal cell carcinoma (RCC). MATERIALS AND METHODS: Normal kidney (n=26) and RCC tissues (n=111) were obtained from 111 patients who had undergone a radical or partial nephrectomy. The expressions of clusterin and Ki-67 protein were analysed using immunohistochemical staining. The medical records of the patients were retrospectively reviewed to evaluate the clinicopathological factors. RESULTS: In contrast to the clusterin and Ki-67 expressions of 30.8 and 11.5%, respectively in the normal kidney (n=26), those in the RCC tissues were 93.7 and 28.8% (n=111), respectively (p0.05). Furthermore, the recurrence-free survival in patients with strong clusterin expression was significantly lower than in those with weak expression (p<0.05). CONCLUSIONS: These findings suggest that clusterin may be involved in the progression of RCC and; therefore, a useful prognostic variable in patients with an RCC.

RESUMEN

PURPOSE: Radical cystectomy with pelvic lymphadenectomy is an effective therapeutic modality in invasive bladder cancer. The development of another technique for a radical cystectomy for the early recovery and reduction of complications was attempted. MATERIALS AND METHODS: Between March 1997 and June 2003, an extraperitoneal radical cystectomy was attempted in 18 patients with invasive bladder cancer (pT1-pT4). Not only standard or extended pelvic lymphadenectomy, but also re-positioning of the ureter to the opposite side, could be performed extraperitoneally. An intestinal segment could be taken out through a small window within the peritoneum and manipulated as desired. The clinical safety and complications of other surgical approaches for radical cystectomy were also investigated. RESULTS: The procedure was successful in 15 of the 18 patients (4 studer pouches, 3 ileal conduits and 8 ureterocutaneostomies). Failure to peel out the peritoneum occurred in 3 cases, 1 each due to technical difficulties, tumor invasion at the bladder dome and adhesion due to a previous radical prostatectomy. Complications occurred in 8 cases (53.1%). The intestinal obstruction progressed in 1 case (12.5%) due to a high positioned mesentery of the Studer pouch. There were no significant gastrointestinal complications in any of the 8 patients that underwent the extraperitoneal radical cystectomy with ureterocutaneostomy. CONCLUSIONS: An extraperitoneal radical cystectomy seems to reduce the gastrointestinal complications and be worthwhile in certain cases with advanced bladder cancer. Despite the visual limitation, it is an acceptable surgical technique compared to the conventional transperitoneal methods.

RESUMEN

Adenomatoid tumor is an uncommon neoplasm found mainly near or in the epididymis in the male and the fallopian tubes and ovary in the female. We observed a case of adenomatoid tumor of the epididymis, which was found incidentally 20 years ago by patient self examination. A rubbery, non-tender, 2 X 1-cm mass was found in the tail portion of the right epididymis. Scrotal ultrasonography showed a 2.1 X 1.1 X 1.2-cm hypoechoic solitary mass. Mass excisional biopsy with partial epididymectomy was done under local anesthesia. Pathologic examination showed an adenomatoid tumor.

RESUMEN

PURPOSE: The clinical efficacy and safety of apomorphine hydrochloride SL, a new centrally acting oral agent for the management of the male erectile dysfunction (ED), were evaluated. MATERIALS AND METHODS: Sixty consecutive patients with ED, who visited the Male Sexual Dysfunction Clinic at Pusan National University Hospital, between April and December 2002, were evaluated by open labelled and uncontrolled randomized methods. Apomorphine hydrochloride SL (Uprima(r)), 4-8 3mg tablets, was prescribed on each visit, which was administered sublingually according to the standard patient guidelines. An overall evaluation of erectile function was performed using the international index of erectile function (IIEF) and global efficacy assessment questions (GAQ). RESULTS: The age distribution of the 51 cases was 22 to 76 years, with a mean of 47.6+/-10.6 years. During the mean follow-up period of 26.2+/-18.6 days, ranging from 7 to 156 days, the sum of the IIEF symptom scores changed significantly, from 11.2+/-6.7, at the baseline, to 17.0+/-8.2, on the 3rd visit (p>0.05). The IIEF scores were significantly improved in the Q3 and Q4, at 45.7% and 59.8%, respectively(p0.05). The improvements in the IIEF scores revealed in psychogenic and mild grade ED were statistically significance (p<0.05). Selectivity as a treatment regimen was shown in only 5 cases (8.3%). Adverse reactions were noted in 7 cases (13.7%), but were self-limited, and resulted in only 1 (1.9%) drop-out due to dizziness. CONCLUSIONS: Apomorphine hydrochloride SL is suggested would be a proper oral agent in patients with psychogenic or mild degree organic ED.
