RESUMEN

La prótesis parcial removible (PPR) es una alternativa de tratamiento en pacientes que perdieron algunos elementos dentarios, debido al costo relativamente bajo y a la generación de una función masticatoria satisfactoria. Además, es una buena opción cuando hay pérdida de los tejidos óseos, dado que impide la instalación de implantes dentarios. La PPR convencional presenta una estructura metálica que ofrece retención y resistencia a la prótesis, convirtiéndola en poco agradable a los pacientes, por el hecho de dejar a la vista el metal en la cavidad oral. A partir de investigadores que decidieron ir más allá del simple estudio de la función y la estética de las PPR para adentrase en sus diversas formas de presentación, surgió la prótesis parcial flexible (PFlex). La PFlex es una prótesis parcial confeccionada con resina termoplástica, sin metal y con flexibilidad. Si bien, muchos profesionales la emplean como prótesis temporal, otros tantos la han usado como alternativa a la PPR convencional. Así, uno de los frecuentes interrogantes de los clínicos es si realmente es posible utilizar la PFlex como alternativa protética definitiva. Por tanto, el propósito de este trabajo fue realizar una revisión de la literatura para evaluar si el uso de prótesis parciales removibles con resina termoplástica puede ser una alternativa a las prótesis parciales convencionales, exponiendo las ventajas, desventajas, indicaciones, contraindicaciones y las propiedades del material protético.


Removable partial denture (RPP) is an alternative treatment when it comes to patients who have lost some dental elements, as they have the advantages of a relatively low cost and a satisfactory masticatory function. Furthermore, it is a good option when there is bone tissue loss, making dental implants placement unfeasible. Conventional PPR has a metallic frame that provides retention and resistance to the prosthesis, which makes it unpalatable to patients because it leaves the metal in the oral cavity evident. Many researchers, aiming to give back beyond function and aesthetics, are studying forms of presentation of PPR, thus emerging the flexible partial prosthesis (PFlex). PFlex is a partial denture made with thermoplastic, resin metal-free and with flexibility, however its use, for many professionals, is limited only as temporary prostheses, but there are some clinicians who use PFlex as an alternative to conventional PPR. Thus, one of the questions constantly asked by scientists is whether it is possible to use PFlex as a definitive prosthetic alternative. Therefore, the aim of this study was to conduct a literature review to assess whether the use of removable partial dentures with thermoplastic resin can be an alternative to conventional partial dentures, exposing the advantages, disadvantages, indications, contraindications, and the properties of the prosthetic material.

Purpose: To evaluate the effect of fibromucosa height on the stress distribution and displacement of mandibular total prostheses during posterior unilateral load, posterior bilateral load and anterior guidance using the finite element analysis (FEA). Material and methods: 3D virtual models were made to simulate the stress generated during different mandibular movements in a total prosthesis. The contacts were simulated according to the physiology, being considered perfectly bonded between cortical and medullar bones; and between cortical bone and mucosa. Non-linear frictional contact was used for the total prosthesis base and fibromucosa, allowing the prosthesis to slide over the tissue. The cortical bone base was fixed and the 100 N load was applied as unilateral load, posterior bilateral load and anterior guidance simulation. The required results were for maximum principal stress (MPa), microstrain (mm/mm) and total displacement (mm). The numerical results were converted into colorimetric maps and arranged according to corresponding scales. Results: The stress generated in all situations was directly proportional to the fibromucosa height. The maximum principal stress results demonstrated greater magnitude for anterior guidance, posterior unilateral and posterior bilateral, respectively. Only posterior unilateral load demonstrated an increase in bone microstrain, regardless of the fibromucosa height. Prosthesis displacement was lower under posterior bilateral loading. Conclusion: Posterior bilateral loading is indicated for total prosthesis because it allows lower prosthesis displacement, lower stress concentration at the base of the prosthesis and less bone microstrain. Objetivo: Avaliar o efeito da altura da fibromucosa na distribuição de tensões e deslocamento das próteses totais mandibulares durante a carga unilateral posterior, carga bilateral posterior e orientação anterior usando a análise por elementos finitos (FEA). Material e métodos: Modelos virtuais 3D foram confeccionados para simular a tensão gerada durante diferentes movimentos mandibulares em uma prótese total. Os contatos foram simulados de acordo com a fisiologia, sendo considerados perfeitamente unidos entre os ossos corticais e medulares; e entre o osso cortical e a mucosa. O contato friccional não linear foi utilizado entre a base total da prótese e a fibromucosa, permitindo que a prótese deslizasse sobre o tecido. A base do osso cortical foi fixada e a carga de 100 N aplicada como carga unilateral, carga bilateral posterior e simulação de guia incisiva anterior. Os resultados foram calculados em tensão máxima principal (MPa), microdeformação (mm/mm) e deslocamento total (mm). Os resultados numéricos foram convertidos em mapas colorimétricos e organizados de acordo com as escalas correspondentes. Resultados: A tensão gerada em todas as situações foi diretamente proporcional à altura da fibromucosa. Os resultados de tensão máxima principal demonstraram maior magnitude durante a carga anterior, posterior unilateral e posterior bilateral, respectivamente. Apenas a carga unilateral posterior demonstrou aumento da microdeformação óssea, independente da altura da fibromucosa. O deslocamento da prótese foi menor sob carga bilateral posterior. Conclusão: A carga bilateral posterior é indicada para prótese total, pois permite menor deslocamento da prótese, menor concentração de tensões na base da prótese e menor microdeformação óssea (AU)

Abstract Non-human teeth have been commonly used in research as replacements for human teeth, and potential dissimilarities between the dental tissues should be considered when interpreting the outcomes. Objective: To compare the proteolytic activity and degradation rate of bovine and human dentin matrices. Methodology: Dentin beam specimens were obtained from human molars (n=30) and bovine incisors (n=30). The beams were weighed hydrated and after complete dehydration to obtain the mineralized wet and dry masses. Then, the beams were demineralized in 10 wt% phosphoric acid. Next, 15 beams from each substrate were randomly selected and again dehydrated and weighed to obtain the initial demineralized dry mass (DM). Then, the beams were stored in saliva-like buffer solution (SLBS) for 7, 14 and 21 days. SLBS was used to evaluate hydroxyproline (HYP) release after each storage period. The remaining beams of each substrate (n=15) were tested for initial MMP activity using a colorimetric assay and then also stored in SLBS. DM and MMP activity were reassessed after 7, 14 and 21 days of incubation. The data were subjected to two-way ANOVA tests with repeated measures complemented by Bonferroni's tests. Unpaired two-tailed t-tests were also used (p<0.05). Results: Similar water and inorganic fractions were found in human and bovine dentin, while human dentin had a higher protein content. The most intense proteolytic activity and matrix deterioration occurred short after dentin was demineralized. Both substrates exhibited a sharp reduction in MMP activity after seven days of incubation. Although human dentin had higher MMP activity levels, greater HYP release and DM loss after seven days than bovine dentin, after 14 and 21 days, the outcomes were not statistically different. Conclusion: Bovine dentin is a suitable substrate for long-term studies involving the degradation of dentin matrices.

Os fotopolimerizadores vêm sendo utilizados como dispositivos fundamentais na Odontologia para confecção de restaurações. No entanto, a etapa de fotopolimerização da resina composta inadequada pode afetar a união entre o material restaurador e o substrato. No caso dos agentes cimentantes resinosos a dificuldade está na limitação do local de incidência da luz. Em cimentações de retentores intrarradiculares pré-fabricados, normalmente são utilizados os cimentos de ativação química ou dupla ativação, devido a limitações anatômicas. Portanto, o objetivo deste trabalho foi avaliar o efeito da emissão de luz com fibra óptica, na cimentação de retentor intrarradicular experimental capaz de fotoativar em toda a extensão do conduto radicular. No estudo foi confeccionado um protótipo de ponta acessória em fibra óptica para fonte transmissora de luz que acoplado a um aparelho fotoativador (Bluephase, Ivoclar, Brasil) transmite a luz pelo interior do conduto radicular. Desta forma foram confeccionados em parceria com a empresa Angelus, retentores experimentais vazados ao longo dos seus eixos em fibra de vidro parcialmente opacos. Para simulação do conduto radicular foram utilizadas 80 amostras de resina G10 seccionadas no tamanho de 10 mm de largura por 14 mm de altura (n=80). Cada amostra foi perfurada ao centro nas seguintes dimensões: diâmetro de 3,0 mm e 10 mm de altura. Para cimentação dos retentores intrarradiculares foram utilizados cimento resinoso fotoativado ou dual. A fotoativação foi realizada diretamente com a ponta do aparelho fotoativador ou utilizando o dispositivo experimental transmissor de luz. As amostras foram analisadas imediatamente ou após o envelhecimento térmico de 10.000 ciclos. Para análiseda resistência de união as amostras foram seccionadas em 3 partes, com 2 mm cada (coronal, média e apical) e submetidas aos testes de resistência de união por push out, seguido da análise estereomicroscópica e análise espectroscópica no infravermelho transformada por Fourier (FT-IR). O dispositivo experimental transmissor de luz foi avaliado quanto a sua irradiância, e esta obteve 50% da energia quando comparada com obtida diretamente da ponteira do fotoativador. Após testes mecânicos, os resultados encontrados foram submetidos aos testes estatísticos, Anova com 3 fatores (cimento x fotoativação x profundidade) e Anova 2 fatores (envelhecimento x cimento), seguida de teste Tukey com nível de significância de 5%. Nos resultados de resistência houve diferença estatística entre os tipos de cimentos, profundidade, fotoativação e interação entre oscimentos e a profundidade, sendo que os grupos cimentados com os cimentos fotoativados e com o dispositivo houve uma maior resistência de união na porção média e apical. Com relação ao grau de conversão houve uma maior conversão e a análise estereomicroscópica apresentou maior falha adesiva na porção apical, independente da realização do envelhecimento. Portanto, a utilização do dispositivo obteve resultados promissores e o retentor intrarradicular experimental contribuiu para aumentar a adesão entre o cimento e o substrato em sua porção mais apical.


Photopolymerizers have been used as a fundamental device in dentistry for making restorations. Nonetheless, the stage for the photopolymerization of the composite resin is critical and can affect the adhesion between the restorative material and the substrate. In the case of resin cementing agents, the difficulty lies in limiting the location of the incidence of light. In cementations of prefabricated intraradicular retainers, chemical activation or double activation cements are normally used, due to anatomical limitations. Therefore, the objective of this work was to evaluate the effect of light emission with optical fiber in the cementation of experimental intraradicular retainer capable of photoactivating in the entire length of the root canal. In the study, a prototype of fiber optic accessory tip for light transmitting source was made, which coupled to a photoactivator device (Bluephase, Ivoclar, Brazil) transmits the light inside the root canal. Thus, they were made in partnership with the company Angelus, experimental retainers cast along their axes in partially opaque fiberglass. For simulation of the root canal were used 80 samples of resin G10 sectioned in size 10mm wide by 14mm high (n=80). Each sample was drilled in the center in the following dimensions: diameter 3.0 mm and height 10 mm. For the cementation of the intraradicular seals, light-cured or dual resin cement was used. The photoactivation was performed directly with the tip of the photoactivator device or using the light transmitting experimental device. The samples were analyzed immediately or after thermal aging of 10,000 cycles. For bond strength analysis, the samples were sectioned into 3 parts, with 2 mm each (coronal, mean and apical) and submitted to the bond strength tests by push out, followed by the stereomicroscopic analysis and infrared spectroscopic analysis by Fourier (FTIR). The Light Transmitting Experimental Device was evaluated for its irradiance and obtained 50% of the energy when compared to that obtained directly from the tip of the photoactivator. After mechanical tests, the results found were submitted to the statistical tests Anova with 3 factors (cement x photoactivation x depth) and Anova 2 factors (aging x cement), followed by Tukey's test with a significance level of 5%. In resistance results, there was a statistical difference between the types of cements, depth, photoactivation and interaction between the cements and depth, and the groups cemented with the photoactivated cements and the device had a greater bond strength in middle and apical portion. Regarding the degree of conversion, there was a greater conversion and the stereomicroscopic analysis, it showed greater adhesive failure in the apical portion, regardless of the performance of aging. Therefore, the use of the device obtained promising results and the experimental intraradicular retainer contributed to increase the adhesion between the cement and the substrate in its most apical portion.
