RESUMEN

BACKGROUND: Production of nitric oxide (NO) by inducible NO synthase (iNOS) has been implicated in the pathology of autoimmune disease. It is unknown whether iNOS expression is increased within testes and whether iNOS and NO have essential roles in the pathogenesis of EAO. METHODS: EAO was induced in guinea pig testes at 17 days after secondary immunization by administration of crude extract (CE) and purified glycoprotein 1 (GP1) from normal guinea pig testes. iNOS gene expression was assessed by RT-PCR and Northern blot analysis in testes. Localization of iNOS and Mac-1 and the indicator of NO-mediated tissue injury, nitrotyrosine, were detected in the testicular lesion by immunohistochemistry. RESULTS: In control testes, inflammation and iNOS gene expression were not detected, whereas, in CE- and GP1-injected testes, inflammation and marked iNOS gene expression were evident at day 17 after secondary immunization. Immunohistochemistry of Mac-1 showed the colocalization with iNOS protein and nitrotyrosyl proteins in intertubules, suggesting that NO produced by infiltrated macrophages may be involved in inflammatory lesions of intertubules. Intraperitoneal administration of aminoguanidine significantly prevented EAO with reduction of inflammation, iNOS expression and nitrotyrosine formation. CONCLUSION: These results suggest that NO production by macrophages may be important in the pathogenesis of CE- and GP1-induced EAO. Furthermore, this study demonstrated the therapeutic potential of iNOS inhibitor in the treatment of inflammatory and autoimmune mediated-diseases.

We analyzed the prognostic factors, such as sex, age, tumor shape, number, 1ocation, size, TNM stage, WHO grade, modified Bergkvist grade, treatment modality, cytology, type of progression, mitotic activity index(MAI), volume corrected mitotic index(M/V index) and pathologic grade, in a chort of 83 advanced transitional cell carcinoma of bladder who was followed for more than 3 years. N stage of TNM,WHO grade, modified Bergkvist grade and pathologic stage were related to mitotic indices(p<0.05), but cytology was related to MAI only (p<0.05).In univariate analysis of prognostic factors, sex, N and M stage of TNM, progression type, MAI & M/ V index and pathologic stage were related to 5-year survival during follow up. Multivariate analysis of prognostic factors, which were significantly related in univariate analysis, identified MAI as the most important factor(p<0.05), followed by M/V index, N stage of TNM, sex, M stage of TNM and progression type. The result show that advanced transitional cell bladder tumors can be efficiently categorized into prognostic groups by quantitative mitotic frequency analysis.

RESUMEN

Most of bladder tumors are transitional cell type and leiomyoma of the urinary bladder is a rare, benign mesothelial tumor. The clinical importance of leiomyomas is usually slight, but patients have shown symptoms that reflect the tumor's size and location within the bladder. Herein, we present a case of leiomyoma of the bladder in a 62-year-old woman. IVP, ultrasonography and CT suggested bladder mass and cystoscopic finding demonstrated intraluminal protruding mass near bladder neck with intact mucosa. The tumor was removed bytransurethral resection and diagnosis was confirmed by histologic examination.

RESUMEN

Spontaneous subcapsular hematoma of the kidney is an uncommon, but there are only few reports on bilateral subcapsular hematoma associated with non-traumatic origin in the world. Several commonest causes for this phenomenon have been described : including disease of the kidney , nephritis, tumor, hydronephrosis, infection, tuberculosis. lithiasis and cystic disease, blood vessel disease ; arteriosclerosis, aneurysm, periarteritis nodosa and renal infarcts, blood dyscrasia. No definite causative factor was found in this case : that a 49-year-old woman was done the decortication of left kidney and hematoma remove, due to no specific abnormality in preoperative coagulation test, 24hr urine AFB stain, immunoserologic test and renal angiography.

RESUMEN

Our experience of surgical management of 20 adrenal tumors was present during the period from June, 1988 to May, 1993. The results of clinical study were as follows ; 1. Average age of patients was 40.9 years with the highest incidence in sixth decade occupying 40% and male to female ratio was 1:1 and right to left ratio was equal also. 2. The adrenal tumors consist of 8 adrenocortical adenoma( including 3 primary aldosteronisms and 1 Cushing syndrome), 4 adrenocortical carcinoma(including 1 Cushing syndrome), 7 pheochromocytomas and l neuroblastoma. 3. The 11 functioning adrenal tumors( including 4 adrenocortical adenomas, 1 adrenocortica1 carcinoma and 6 pheochromocytomas) showed specific symptoms and signs and the majority symptom and sign of the 9 non-functioning adrenal tumors( including 4 adrenocortical adenomas, 3 adrenocortical carcinomas, 1 neuroblastoma and 1 pheochromocytoma) showed abdominal discomfort and pain. 4. For initial diagnosis of the adrenal tumors, USG, CT, MRI and angiography are reliable but IVP was reliable in large adrenal tumor 5. The surgical approaches were transperitoneal in 18 patients and retroperitoneal in 2 patients and postoperative surgical complication occurred in 3 patients of transperitoneal approaches that were prolonged ileus, spleen injury and pancreatic injury with pleural effusion.6. The tumor masses were 12gm to 4,000gm in weight (mean: 616gm). The adrenal benign and malignant tumor were significantly different in weight ( 120gm : 1817gm).
