RESUMEN

Objective To analyze the prevalence and risk factors of sleep apnea syndrome (SAS) in patients with Alzheimer's disease (AD), and to provide theoretical basis for the prevention of SAS in AD patients. Methods A total of 130 AD patients admitted to the Department of Neurology of Guang'an People's Hospital of Sichuan Province from January 2019 to September 2022 were selected and divided into control group (without SAS) and observation group (with SAS) according to whether the patients were complicated with SAS{AHI ≥5 times/h}. Snoring, waking at night, dry mouth in the morning, AHI and SaO2 values were compared between the two groups. Clinical data of AD patients, including age, gender, body mass index (BMI), AD course, tobacco and alcohol history, and neurodegenerative diseases, were collected by self-made questionnaire and consulting the patient's electronic medical record bed. Univariate analysis and logistic regression were used to analyze the independent risk factors for SAS in AD patients. Results Among 130 AD patients, 43 cases (33.08%) of SAS occurred. The proportion of snoring, awakening at night, dry mouth in the morning and AHI value in the observation group were significantly lower than those in the control group (P2, PSQI score >16 points and CDR score ≥2 points were independent risk factors for SAS in AD patients (P2, PSQI score >16 and CDR score. Polysomnosis monitoring should be performed regularly to prevent SAS.

Objective Toinvestigatethechangesofiron,fatandwatercontentinspleentissuesforacutepancreatitis(AP).Methods Atotal of44patientswithAP(experimentalgroup)and21healthysubjects(controlgroup)wererecruitedinthisstudy.RoutineupperabdominalMR scansandIDEAL-IQsequencescanwereperformed.TheR2?,Water,FatandFFvaluesofspleenwererespectivelymeasuredinthe experimentalgroupandcontrolgroup,andthedataofthetwogroupswereanalyzedstatistically.Results TheR2?value(P=0.011),Water value(P=0.003)andFatvalue(P=0.022)ofspleenintheexperimentalgroupandthecontrolgrouphadsignificantdifferences, whiletheFFvalue(P=0.861)didn’t.TherewerenosignificantdifferencesinR2?,WaterandFatvaluesinthemild,moderateand severeAP (P>0.05).aswellasintheyounggroup (14-44yearsold),themiddle-agedgroup (45-59yearsold)andtheelderly group (≥60yearsold)inAP (P>0.05).Conclusion APcanleadtothechangesofirondeposition,fatandwatercontentinspleen tissue,andIDEAL-IQtechnologycanquantitativelyevaluatethechangeofthem.

RESUMEN

Objective To investigate the difference of clinical curative effect of Apatinib combined with second line chemotherapy between AFP positive gastric cancer and AFP negative gastric cancer.Methods The da-ta of 78 patients that met the inclusion criteria with advanced gastric cancer from January 2015 to June 2017 were collected from the Second Affiliated Hospital of Zhengzhou University.According to the blood serum AFP levels be-fore treatment(including chemotherapy,surgery)the patients were divided into the AFP positive group(AFPGC) and the AFP negative group(NAFPGC),excluding the impact of other factors.Analysing the difference of the clin-ical effect with apatinib combined with second line chemotherapy in the two groups. Results Getting rid of other factors that may affect the efficacy of chemotherapy,we administered apatinib combined with docetaxel or irinote-can regimen as second line therapy for gastric cancer.The AFP positive group was treated for 2 cycles,4 cycles,in which ORR and DCR were significantly better than that of the AFP negative group(P<0.05).The OS of the AFP positive group and the AFP negative group was 5.5 months,6 months(P = 0.747). No significant adverse reac-tions occurred in the two groups. Conclusion Chemotherapy curative effect of the AFP positive group of apatinib combined with docetaxel or irinotecan is obviously better than that of the AFP negative group in short term,but no obvious difference in long term effect.

RESUMEN

Objective To observe the effects of curcumin on B-type natriuretic peptide (BNP),cardiac troponin Ⅰ (cTnI) and D-dimer in rats with acute pulmonary embolism (APE).Methods The irregular cbemotactic factor (CX3CL1)-shRNA and overexpression vector were designed and synthesized,and adenovirus packages were respectively carried out.Sixty Spargue-Dawley (SD) rats were divided into normal control group,sham-operation group,model group,curcumin group,curcumin+CX3CL1-shRNA group and curcumin+CX3CL1 overexpression vector group by random number table (each group 10 rats).The APE rat models were duplicated by autologous blood embolus method.in the pharmacological intervention groups,1 day and again 40 minutes before operation,a dose of curcumin 100 mg/kg was administered by gavage respectively;in virus intervention groups,3 days before modeling,intravenous injection of 109 pfu into a tail vein was performed once;in sham-operation group and model group,an equivalent dose of normal saline 2 mL daily was administered by gavage;the normal group did not accept any intervention.Six hours after modeling,the change of lung tissue were observed under confocal laser scanning microscope,the lung tissue was taken for hematoxylin-eosin (HE) staining,histopathological changes of lung tissue were observed,and the levels of serum BNP,cTnI,D-dimer were detected by enzyme-linked immunosorbent assay (ELISA).Results Virus infected lung tissue was green,nucler was blue.The HE staining in lung tissue showed:histopathological changes in the model group were more obvious than those in normal group and sham-operation group,after curcumin intervention,some emboli in pulmonary arteries in all treatment groups were partially dissolved,and the inflammation was alleviated.The results of ELISA shows:the contents of cTnI,BNP,D-dimer were significantly higher in model group than those in shamoperation group [cTnI (ng/L):224.97 ± 37.96 vs.149.13 ± 36.65,BNP (ng/L):53.66 ± 21.72 vs.24.62 ± 10.44,D-dimer (ng/L):78.68 ± 13.10 vs.42.00 ± 9.30,all P < 0.05];the contents of cTnI,BNP,D-dimer were significantly lower in curcumin group,curcumin+shRNA group and curcumin+CX3CL1 overexpression vector group than those in model group (cTnI was 162.98 ± 28.65,143.32 ± 34.55,159.50 ± 37.80 vs.204.97 ± 37.96,all P < 0.05,BNP was 27.45 ± 11.00,26.95 ± 6.66,33.05 ± 7.05 vs.53.66 ± 21.72,all P < 0.05,D-dimer was 54.34 ± 4.57,53.38 ± 11.01,56.33 ± 6.80 vs.78.68 ± 13.10,all P < 0.05);there were no statistical significant differences in comparisons among all intervention groups (all P > 0.05).Conclusion Curcumin is able to improve serum BNP,cTnI,D-dimer in APE rats,but its relation with CX3CL1 is not obvious.

AIM:To investigate the molecular mechanism of nontypeable Haemophilus influenzae ( NTHi)-in-duced MUC5AC expression in the human airway NCI-H292 cells.METHODS:Bronchial epithelial NCI-H292 cells were cultured in vitro.The production of MUC5AC and matrix metalloproteinase 9 (MMP-9) after stimulation with NTHi was analyzed by enzyme-linked immunosorbent assay (ELISA).The enzymatic activity of MMP-9 was detected by gelatin zy-mography.In addition, the cells were pretreated with different inhibitors such as AG 1478, LY294002, DPI, NAC and GM6001 before stimulation, which specifically inhibit epidermal growth factor receptor (EGFR), phosphatiadylinositol 3-ki-nase (PI3K), NADPH oxidase, reactive oxygen species (ROS) and MMP-9, respectively, and then the production of MUC5AC or MMP-9 was determined.RESULTS:NTHi-stimulated NCI-H292 cells exhibited a time-dependent increase in MMP-9 secretion and activity .NTHi also increased the activity of Rac 1 and the production of ROS .Pretreatment of AG1478 and LY294002 decreased the Rac1 activity, and preincubation of DPI or Rac 1 inhibitor significantly abrogated ROS production.In addition, secretion of MMP-9 and the enzymatic activity was decreased by treatment of NAC and NSC23766.Furthermore, inhibition of the MMP-9 activity by GM6001 significantly inhibited MUC5AC production.CON-CLUSION:EGFR/PI3K/Rac1/NADPH oxidase/ROS/MMP-9 regulates MUC5AC production in NTHi-challenged NCI-H292 cells.

RESUMEN

Objective To explore the clinical curative effect of traditional Chinese medicine enema,amino salicylic acid preparation and probiotics in the treatment of ulcerative colitis.Methods 168 patients were selected as the research objects which were confirmed by endoscopic examination and the diagnosis of ulcerative colitis patients,randomly divided into the control group of 84 cases and observation group of 84 cases.The control group was given with sulfasalazine(SASP) treatment,a dose of 1g,4 times/d,period of treatment was 28d.In the observation group,oral SASP treatment combined with microecological preparation often meian and traditional Chinese medicine enema,Chang An capsule,2 capsules (500mg)/time,3 times/d,postprandial oral; Herbal Enema with angelica,white lentils,flavescent sophora root,cicada,windproof suffering after filtering,every night before bed retention enema.The clinical efficacy was compared between the two groups.Results After the treatment,the Sutherland index of the observation group was (1.82 ± 1.65),which was significantly lower than (5.51 ± 1.59) of the control group (t =10.436,P < 0.05).In observation group,the clinical total effective rate,the total efficiency of endoscopic and histological total effective rate was 88.1%,81%,76.2%,the control group were 66.7%,57.1%,50%,the two groups had significant difference(x2 =4.353,4.512,5.113,P<0.05).Conclusion The traditional Chinese medicine enema,amino salicylic acid preparation and probiotics in the treatment of ulcerative colitis has good curative effect,can relieve the clinical symptoms of patients,improve the degree of intestinal diseases.
