RESUMEN

This paper expounds that the construction of a clinical teacher's teaching ability model is an urgent problem to be solved in medical colleges and universities, and analyzes that the current clinical teaching concepts and methods are constantly improving, and the clinical teaching environment is more informatized and intelligent. This paper summarizes the clinical teachers' teaching ability models at home and abroad, such as the ability and quality iceberg model, teacher growth model, inquiry-based teaching model, Molenaar three-dimensional teaching ability model, etc., and discusses the practice research progress of current clinical teacher teaching ability models such as student-centered guided teaching, bedside teaching, micro-teaching and BOPPPS (bridge-in, objective, pre-assessment, participatory-learning, post-assessment, and summary) method, medical simulation teaching, etc., hoping to provide guidance for further constructing models of teacher's teaching ability suitable for Chinese medical colleges and universities.

RESUMEN

A novel blood pressure measurement system was designed which based on auscultatory method. And the electret sensor that embedded into the internal instrument can detect the Korotkoff-sound signal directly which is coupled by the cuff and transmitted in the cross connection. The BP values identification algorithm is based on combined detection of Korotkoff-sound and pulse signal, and the products of amplitudes and energies are calculated as the characteristic values of Korotkoff-sound, and the Korotkoff-sound phases are classified and detected by means of clustering of characteristic values, and then BP parameters are determined. The contrast test and statistical analysis showed good consistency and accuracy between the new BP detection method and conventional mercury sphygmomanometer.

Job-transfer training for general practitioner is the important channel of primary medical talents team construction.This article elaborated on existing problems in the general practitioner job-transfer training in our country from several aspects,such as training base,teachers,teaching materials,actual work need and discipline construction,etc.Based on the existing problems and combined with practical work requirements of general practitioner job-transfer training in our country,the article put forward corresponding suggestions and countermeasures including strengthening teachers' construction,textbook construction and speeding up general medical discipline construction,etc.

RESUMEN

<p><b>OBJECTIVE</b>To investigate the origin and the recombinant model of H7N9 virus prevailing in China by sequence analysis.</p><p><b>METHODS</b>The sequences of H7N9 virus were collected and analyzed with the software BLAST and MEGA 5.0. The phylogenetic trees were constructed after multiple sequences alignment. The homologous sequences of H7N9 segments were determined and the model was inferred according to the origin of H7N9 segments.</p><p><b>RESULTS</b>The most relevant sequences of HA, NA, NS and PB2 segments were located at one branch of the phylogenetic tree, while the closest relevant sequences of PB1, PA, NP and MP contained two H9N2 virus origins. According to the analysis of sequence origin, H7N9 viruses might be divided into 5 genotypes: namely A, B, A/Shanghai/1/2013-H7N9, A/Pigeon/Shanghai/S1069-H7N9 and A/Zhejiang/HZ1/2013-H7N9, and the genotype A consisted of A1 and A2 subtypes.</p><p><b>CONCLUSION</b>The prevailing H7N9 virus might be derived from 5 different viruses after 4 times of recombination, which resulted in the two major types of A and B. The subtypes of A1 and A2 were two different derivatives from one reassortant. The A/Pigeon/Shanghai/S1069-H7N9 strain might be the recombinant of type A H7N9 virus with a local H9N2 virus during the H7N9 epidemics. The A/Zhejiang/HZ1/2013-H7N9 strain could be the re-arrangement of subtype A2 with type B H7N9 virus.</p>

RESUMEN

<p><b>OBJECTIVE</b>To propose a new method for acquiring pulmonary function parameters based on measurement of volume changes of thoracic impedance.</p><p><b>METHODS</b>We studied the relationship between the volume changes of thoracic impedance and pulmonary function parameters during forced breathing based on bioimpedance measurement, and developed an instrument for measuring thoracic impedance. Using this instrument and a MRI spirolab III lung function test instrument, both based on flowmeter measurement, we measured such pulmonary function parameters including forced vital capacity (FVC), forced expiratory volume in one second/FVC (FEV1/FVC), and peak expiratory flow in 10 healthy volunteers and compared the measurement results.</p><p><b>RESULTS</b>The differences in the parameters measured using the two instruments were all within two folds of the positive and negative standard deviations of the average values, demonstrating good consistency in the measurement between the two methods.</p><p><b>CONCLUSIONS</b>The measurement results of the bioimpedance-based instrument we developed show good consistency with those by the commercially available pulmonary function test instrument.</p>

RESUMEN

<p><b>OBJECTIVE</b>To assess mental fatigue by noninvasive monitoring of the physiological signals.</p><p><b>METHODS</b>The changes in the physiological parameters including the electrodermal activity, heart rate and heart rate variability were analyzed in 14 subjects performing the reaction-time tasks when fatigue and changes in the reaction time occurred.</p><p><b>RESULTS</b>The average skin conductance level, average heart rate, and heart rate variability parameters including the total power density, percentage of the very low power density, percentage of high power density all differed significantly between the sober state and the mental fatigue state.</p><p><b>CONCLUSION</b>Monitoring the physiological parameters including the electrodermal activity, heart rate and heart rate variability is a noninvasive, effective and practical approach to mental fatigue assessment.</p>

RESUMEN

This paper is aimed to study the feasibility of myocardial infarction risk assessment by noninvasive multivariable trend analysis, including heart rate variability (HRV), deceleration capacity (DC) of heart rate and pulse wave velocity (PWV). Thirty five patients with cardiovascular diseases (11 Myocardial infarction (MI), 8 coronary artery disease (CAD), 6 artery atherosclerosis (AA) and 10 hypertension), and 31 healthy subjects were randomly selected into a control group for this research as comparison. 15 min ECG and 1 min pulse wave data were collected based on the analysis workbench developed by our Lab. HRV, DC and PWV between the cardiovascular group and the control group were analyzed and compared. The results showed that the HRV indices, DC and PWV were significantly higher than those in the control group. The DC and the HRV indices including NN50, PNN50 and TINN especially presented a decline trend that was consistent with the regularity of cardiovascular development process. This noninvasive multivariable trend analysis of HRV, DC and PWV can be a reference for the earlier risk prediction of MI.

RESUMEN

This article describes a design of physiological signal storage format and transfer protocol for the tele-medical system between home and community. The protocol is based on ASCII character, with frames as its basic structure. There are two kinds of frames: control frames and data frames. Control frames can start and stop data transfer, confirm the order, and ask for start. There are seven kinds of data frames, according to the different data types. Data is transferred in data frames. The protocol described in this article is simple and extensible. The design target has been accomplished in real system.

In order to increase the positive detection rate of HCV RNA in the patients with chronic hepatitis C (CHC), RT-PCR was used to synchronously detect HCV RNA in the plasma and peripheral blood mononuclear cells (PBMC) of 583 CHC patients with a continuously elevated level of ALT for more than one year. The results showed that the positive detection rate of HCV RNA in the plasma of the CHC patients was 19.2 %, while 24.5 % in PBMC. It was demonstrated that the positive detection rate for HCV RNA in PBMC was obviously higher than that detected in plasma. To synchronously detect HCV RNA in PBMC by using RT-PCR can increase the positive detection rate of HCV RNA in the CHC patients.

RESUMEN

Objective To evaluate interventional therapy for Budd-Chiari syndrome. Methods IVC venography was first carried out, the obliteration or stenosis in the IVC was opened or dilated with the hard tip of guid wire or puncture needle and balloon, then a stent was implanted. Results The procedure was successful in 271 out of 312 cases including IVC intervention in 260 cases, and hepatic vein intervention in 11 cases. IVC stent was used in 195 cases and hepatic vein stent in 1 case. There was no pulmonary embolism happened, acute renal failure occurred in 6 cases, hepatic coma in 1 case and acute heart failure in 21 cases. One patient died in this group and 2 were complicated with acute IVC thrombosis. Follow up of 6 to 104 mos was made in 203 cases with recurrence found in 21 cases. Conclusion Interventional therapy is indicated for most types of BCS with safe and effective results.
