RESUMEN

Objective:To investigate the efficacy and safety of DOS regimen in adjuvant chemotherapy after D2 radical resection for advanced gastric adenocarcinoma.Methods:The clinical data of 130 patients who received adjuvant chemotherapy after D2 radical resection for advanced gastric adenocarcinoma in Jincheng People's Hospital from January 2017 to January 2019 were retrospectively analyzed. According to treatment regimens, the patients were divided into DOS regimen chemotherapy group (DOS group, 63 cases) and SOX (oxaliplatin, S-1) regimen chemotherapy group (SOX group, 67 cases). The short-term efficacy and adverse reactions of the two groups were compared.Results:Due to the irregular treatment time, 2 cases in each group were removed. Finally, there were 61 cases in DOS group and 65 cases in SOX group. The 1-, 2- and 3-year disease-free survival (DFS) rates of the DOS group were 80.33%, 73.77% and 62.30%, and the DFS rates of the SOX group were 73.85%, 55.38% and 41.54%. The difference in DFS between the two groups was statistically significant ( χ2 = 5.43, P = 0.022). The 1-, 2- and 3-year overall survival (OS) rates of the DOS group were 93.44%, 80.33% and 70.50%, and the OS rates of the SOX group were 96.92%, 73.85% and 52.31%. The difference in OS between the two groups was statistically significant ( χ2 = 4.38, P = 0.045). There were no statistical differences in the incidence rates of grade Ⅰ-Ⅱ nausea, vomiting, diarrhea, bone marrow suppression and fatigue between the two groups (all P > 0.05). There were no statistical differences in the incidence rates of grade Ⅲ-Ⅳ nausea, vomiting and fatigue (both P > 0.05), but there were statistical significances in the incidence rates of grade Ⅲ-Ⅳ diarrhea and bone marrow suppression (both P < 0.05). Conclusions:Adjuvant DOS regimen in the treatment of advanced gastric adenocarcinoma after D2 radical resection has good curative efficacy and can improve the survival of patients, and the adverse reactions can be tolerated.

Objective:To explore the method of ultrasonography for detecting the fetal umbilical vein diameter, blood flow volume and normalized volume blood flow and establish normal reference ranges with umbilical vein diameter, blood flow volume and normalized blood flow and Z-scores for umbilical vein diameter and blood volume flow.Methods:This was a prospective study on 907 normal fetuses in the Second Xiangya Hospital, Central South University and Women and Children Healthcare Hospital of Zhuzhou from March 2019 to December 2020. The umbilical vein diameter (Duv), umbilical vein blood flow volume (Quv) and normalized volume blood flow (nQ = Quv/estimated fetal weight) of the free loop of umbilical vein (FUV) and fetal intra-abdominal umbilical vein (IUV) were collected. And the mean values and 90% confidence intervals of Duv, Quv and nQ in two segments of umbilical veins at different gestational ages were calculated. Regression analysis of Duv, Quv and nQ were performed with gestational age (GA), and the parameters of umbilical vein in different segments were compared. Finally, with gestational age (GA) as the independent variable, Z-scores of the Duv and Quv were built.Results:The mean values and 90% confidence intervals of Duv, Quv, and nQ in 858 (94.6%) normal fetal umbilical veins were successfully obtained. The Duv, Quv of the FUV and IUV increased as pregnancy progressed. The Quadratic curve of Duv and Linear curve of Quv were of the highest fitnesses, respectively( r=0.951, 0.941, 0.986, 0.982; all P<0.001). While nQ increased with GA followed by a decreased trend, and the Quadratic curve was the highest fitting curve of nQ( r=0.610, 0.611; all P<0.001). Duv-FUV was greater than Duv-IUV( P<0.001), nQ-FUV was bigger than Quv-IUV( P=0.001), and he difference was not statistically significant between Quv-FUV and Quv-IUV( P=0.133). Z-scores models of Duv and Quv were successfully established, and all Z-scores were Gaussian distribution. Conclusions:The normal ranges and Z-scores of umbilical vein parameters are useful to improve the evaluation of placental circulation and provide a strong basis for the monitoring of fetus-related diseases and the evaluation of pregnancy prognosis. The choice of FUV or IUV umbilical vein to evaluate placental circulation may depend on the actual situation in clinical application.

Objective@#To investigate the therapeutic effect and prognosis of gastrectomy combined with chemotherapy and chemotherapy alone for treatment of advanced gastric cancer with abdominal para-aortic (the 16th) lymph node metastasis.@*Methods@#The clinical data of 236 patients with advanced gastric cancer with abdominal para-aortic (the 16th) lymph node metastasis in Jincheng People's Hospital of Shanxi Province from January 2003 to December 2012 were retrospectively analyzed. According to the treatment method, the patients were divided into chemotherapy alone group (121 cases) and gastrectomy + chemotherapy group (115 cases), and mFOLFOX6 regimen was used for chemotherapy. The efficacy and adverse reactions of the two groups were compared.@*Results@#The incidence of upper gastrointestinal adverse events in the gastrectomy+chemotherapy group was higher than that in the chemotherapy alone group [8.7% (10/115) vs. 1.7% (2/121)], and the difference between the two groups was statistically significant (χ 2 = 3.881, P = 0.049). The median overall survival was 14.8 months (10.9-17.8 months) in the chemotherapy alone group and 11.7 months (9.8-15.6 months) in the gastrectomy+chemotherapy group, and there was no significant difference between the two groups (P > 0.05). The 2-year overall survival rate was 32.3% in the chemotherapy alone group and 24.3% in the gastrectomy+chemotherapy group, and there was no significant difference between the two groups (χ 2 = 3.105, P = 0.078). The 2-year progression-free survival rate was 16.5% in the chemotherapy alone group and 20.0% in the gastrectomy+chemotherapy group, and the difference between the two groups was statistically significant (χ 2 = 3.917, P = 0.047).@*Conclusion@#Compared with chemotherapy alone, advanced gastric cancer patients with abdominal para-aortic (the 16th) lymph node metastasis could not benefit from gastrectomy combined with chemotherapy.

AIM To observe the expression of inducible nitric ox ide synthase (iNOS) in the rats' hippocampus of kainic acid (KA) induced-seizur es and the effect of L-arginine(L-Arg) or L-nitroarginine(L- NNA) chronic intervention before KA. METHODS The expression of iN OS mRNA by RT-PCR and behaviour were observed after administration of convulsan t dose of KA (10 mg?kg -1 ) and pretreatment with NO predecessor (L-Ar g, 40 mg?kg -1 ) or a inhibitor of NOS(L-NNA, 50 mg?kg -1 ) befo re KA. RESULTS The time-dependent seizures were induced on rats after giving KA, and were improved and serious in the animals with L-NNA pr etreatment, but they were alleviated by L-Arg pretreatment before KA. Compa red with control, iNOS mRNA expression was feebly at KA 3 h, continuative improv ement accompanying KA time prolonged, and it was markedly enhanced at KA 24 h. I t couldn't be examined at KA 2 d or 3 d, and was obviously improved at KA 7 d a gain. iNOS mRNA appeared weak in the rats hippocampus with L-Arg pretreatme nt after KA 1 h, whereas it wasn't found in L-NNA pretreatment animals. CONCLUSION iNOS mRNA expression appeared in the hippocampus of seizu re rats at a few time after KA, and L-Arg chronic intervention before KA ha s a little effect on it.

RESUMEN

AIM To explore the role of nitric oxide (NO) mediators in seizure behavior and the related expression of interlukin 6(IL 6). METHODS The IL 6 immunoreactivity (IL 6 ir) and behaviour were observed in kainic acid (KA) induced seizure following pretreatment with L nitroarginine( L NNA), a inhibitor of nitric oxide synthase(NOS), or the same number of moles of L arginine( L Arg). RESULTS The results showed that time dependent seizures were induced on animals after administration of KA (10 mg?kg -1 ), accompanied by the immediate enhancement of IL 6 ir in hippocampal structure, piriform area and cortex. The behaviors of rats were not markedly altered by chronic pretreatment with L NNA (50 mg?kg -1 ) or L Arg (40 mg?kg -1 ). However, after the administration of KA, the seizure behaviors were obviously different in the group of L NNA and L Arg respectively. Seizure was agrevated in the animals with L NNA pretreatment and many rats died at KA 3 hours, whereas seizure was alleviated after KA in the group of L Arg. IL 6 expression was apparently up regulated in some brain areas such as hippocampus in the group of KA pretreated with L NNA, but opposite effects appeared in the group pretreated with L Arg before KA injection. CONCLUSION These results indicate that endogenous NO mediators may alleviate the seizure behaviors and may down regulated the early expression of IL 6 in KA challenged seizure, but it the mechanism of the early expression of IL 6 with the homeostasis of endogenous NO mediator merits further study.nismoftheearlyexpressionofIL 6withthehome
