RESUMEN

Rhus is a common cause of allergic contact dermatitis (ACD), particularly in East Asia. Rhus can cause localized contact dermatitis following direct contact with the skin, and systemic contact dermatitis following its ingestion, causing pruritus, erythema, and occasionally blistering. Bullous pemphigoid (BP) is an acquired subepidermal autoimmune bullous disease that typically occurs in the elderly. Although BP usually occurs sporadically, some precipitating factors of BP have been suggested, such as other autoimmune diseases, drugs, and malignancies. We report a rare case of BP induced by ingestion of Rhus in a 54-year-old man who presented with itchy erythematous rash and blisters on the whole body, initially misdiagnosed as ACD, and treated with systemic steroids.

Background@#Causative antigens in allergic contact dermatitis (ACD) can be changed by several variables, including time and place. Change in antigens over a period of time in the same institution is one way to reduce the variables; however, these investigations have rarely been performed in Korea. @*Objective@#To investigate the change in patch test results by comparing with the results obtained 10 years ago at the same hospital. @*Methods@#Patch test results were analyzed retrospectively through the medical records of patients with suspected ACD who underwent the patch test using Korean standard series between September 2015 and September 2018. The institutional Review Board of the Dongguk University Ilsan Hospital approved this study. The results of the patch test were compared with those obtained 10 years ago. @*Results@#A total of 360 patients (males: 142, females: 218) were included. Compared with the results obtained 10 years ago, the positivity rates of metal-related allergens (nickel sulfate, cobalt chloride, and potassium dichromate), fragrance allergens, and 4-phenylenediamine base were increased, whereas the positivity rates of mercury ammonium chloride, thimerosal, and formaldehyde were decreased. @*Conclusion@#The patch test results obtained at the same hospital over a 10-year interval suggest that causative allergens in ACD could change with time lapse. In the future, collaborative studies including a number of participating hospitals are needed for obtaining more reliable results.

RESUMEN

Orofacial granulomatosis is a rare granulomatous inflammatory disease, characterized by recurrent orofacial swelling. Infectious, genetic, and immunologic etiologies are suggested, but not fully understood. Herein, we report a case of synchronous orofacial granulomatosis with brain cavernous hemangioma in a 44-year-old female patient, which may be considered paraneoplastic syndrome.

Arterial hypertension following blunt abdominal trauma is a rare complication that can be induced by compression of the renal parenchyma due to a perirenal or subcapsular hematoma or adrenal hemorrhage. A 63-year-old woman was admitted after blunt abdominal trauma. Abdominal computed tomography (CT) revealed a right renal subcapsular hematoma and right adrenal gland hematoma. The patient developed hypertension during conservative treatment. Pheochromocytoma, renovascular hypertension, adrenal adenoma, thyroid disease, and other causes of secondary hypertension were excluded. On following the patient, her blood pressure normalized spontaneously without prescribing any antihypertensive medications. The renal subcapsular hematoma and adrenal hemorrhage regressed progressively as the blood pressure normalized. We report this case to make physicians aware that transient hypertension can develop in patients with blunt abdominal trauma, and to prevent the use of unnecessary antihypertensive medications.

The aim of this study was to examine the relationship of complications related to diverticulitis and visceral obesity. The study was based on a retrospective case note review conducted at the Hanyang University Hospital. Patients were diagnosed with diverticulitis based on clinical symptoms and abdominal computed tomography (CT) findings and divided into two groups: those admitted with complicated diverticulitis and those with a simple diverticulitis episode. We compared the body mass index (BMI) and degree of visceral obesity, measured by abdominal CT. The study included 140 patients, 87 (62.1%) were simple diverticulitis and 53 (37.9%) were complicated diverticulitis. In the complicated diverticulitis group, 9 (6.4%) cases were recurrent, 29 (20.7%) were perforation or abscess patients, and 28 (20%) were patients with systemic inflammatory response syndrome (SIRS). Of the SIRS patients, 13 were involved in other complication groups. When comparing in the two groups, the complicated diverticulitis group had a significantly higher visceral fat area (128.57 cm2 vs 102.80 cm2, P = 0.032) and a higher ratio of visceral fat area/subcutaneous fat area (0.997 vs 0.799, P = 0.014). Visceral obesity is significantly associated with complications of diverticulitis.

RESUMEN

Adefovir dipivoxil (ADV) effectively inhibits lamivudine-resistant hepatitis B virus replication. Hypophosphatemia and elevated serum creatinine are ADV-related nephrotoxicity caused by high-dose ADV. Hypophosphatemic osteomalacia is very rare and is induced by low-dose ADV. A 61-year-old man suffering from chronic hepatitis B manifested with generalized myalgia and bone pain, especially in both ankles and knees. He had been administered ADV for 56 months, since lamivudine-resistant HBV was detected. He developed severe hypophosphatemia and elevated serum alkaline phosphatase levels with a high bone fraction. Bone densitometry and a whole-body bone scan revealed osteoporosis and multiple hot uptake lesions. Blood chemistry and clinical symptoms improved after discontinuing the ADV.

RESUMEN

Although gout often initially affects the peripheral joints, gout may also involve the axial joints. The radiologic changes of axial gout are more common than are clinically recognized. According to a recent report, when the spine CT images of peripheral gout were reviewed for features of axial gout, there was about a 14% frequency of suspected axial gout. The vertebral level and the finding with the most common spinal gouty changes were L4 and lumbar facet joint erosions. We describe here the case of a 36-year-old gout patient with low back and right buttock pain and his lesions were unexpectedly diagnostic of lumbar facet joint arthritis and right sacroiliitis.
