BACKGROUND: Generalized granuloma annulare (GGA) is a benign granulomatous disease of an unknown etiology. Although numerous studies about GGA have been reported, publications that describe the general clinical features of the disease are very sparse. OBJECTIVE: This study aimed to identify the clinical characteristics of Korean GGA cases. METHODS: We reviewed and analyzed the clinical data derived from four patients diagnosed with GGA at our hospital and the clinical data from 58 patients diagnosed with GGA at other Korean hospitals between 1995 and 2011. RESULTS: The cutaneous lesions could be divided into the annular (n=30, 48%) and nonannular (n=32, 52%) types, and the lesions were more common in males than in females, with 33 males and 29 females affected. The GGA incidence showed a bimodal distribution with respect to age at disease onset. Twenty-six cases (42%) presented within the first decade of life and 29 cases (47%) presented when they were in the fifth decade of life or older. Twelve patients (19%) had systemic diseases. Of note, diabetes mellitus (DM) occurred only in adult GGA patients who were aged over 40 years. CONCLUSION: In contrast to previously reported studies, this study shows that the age at GGA onset has a bimodal distribution, and that GGA occurs more often in males. The prevalence of DM among GGA-affected individuals was higher than that found in the general Korean population. Therefore, a DM workup should be undertaken for GGA-affected patients who are over 40 years of age.

RESUMEN

Focal acral hyperkeratosis is a rare skin disorder, initially described by Dowd et al., which is sporadic or inherited in an autosomal dominant way. Clinically, the condition presents with small yellowish to white papules located on lateral aspects of the hands and feet. Histopathological alterations are limited to the epidermis and there are no changes in the elastic fibers of the dermis. In this case report, a 14-year-old girl had a two-year history of persistent, asymptomatic, multiple papules along the border of the hands and feet. A histologic section of papules from the left hand and foot showed marked hyperkeratosis and hypergranulosis of epidermis.

Trichophyton tonsurans is an anthropophilic dermatophyte that is not yet truly prevalent in Korea. Especially, cases of toe nail infection during childhood are very rare, and no cases have been reported prior to this report. We report a single case of tinea unguium due to T. tonsurans in a healthy child. A 7 year-old patient visited an outpatient clinic presenting with subungual hyperkeratosis of the 1st and 3rd Rt. toe nails, which had first presented at an age of 28 months, and no other symptoms. Numerous hyphae were evident on the KOH smear examination. A confirmative diagnosis of tinea unguium cased by T. tonsurans was made after culturing scales from the toe nail lesion. The patient is currently undergoing follow-up, after being treated with oral terbinafine hydrochloride, without a relapse in symptoms.

In the WHO-EORTC (World Health Organization-European Organization for Research and Treatment of Cancer) classification, the primary cutaneous B-cell lymphomas were divided into 4 groups: primary cutaneous marginal zone B-cell lymphoma, primary cutaneous follicle center cell lymphoma, primary cutaneous diffuse large B-cell lymphoma, other, and primary cutaneous diffuse large B-cell lymphoma, leg type (PCLBCL LT). PCLBCL LT is known to have more aggressive clinical behavior and a worse prognosis than the other groups of primary cutaneous B-cell lymphomas. We report here on a case of PCLBCL LT in a 55-year-old male who presented witha multiple erythematous nodules on the right leg. After chemotherapy, the skin lesions had almost cleared, but afterward the tumor was revealed to have metastasized to the brain during 1 year.

The skin lesions of herpes zoster are classically limited to a single dermatome and most cases of multidermatomal herpes zoster have contiguous skin lesions. Simultaneous involvement of two noncontiguous dermatomes is very rare and it has been referred to as zoster duplex unilateralis or bilateralis, depending whether one or both halves of the body are involved. A 67-year-old woman presented with a group of painful vesicles on the right buttock and thigh, and left anterior chest and back. The Tzanck smear and skin biopsy were consistent with a herpetic infection. We report a rare case of zoster duplex bilateralis.

BACKGROUND: Signal transducers and activators of transcription (STAT) proteins are a family of latent cytoplasmic transcription factors that are expressed in many cell types and they regulate the transcription of a broad spectrum of genes that are critically involved in cytokine signaling, cell proliferation and development. Among them, STAT3 has been most strongly implicated in tumorigenesis. OBJECTIVE: This study was designed to measure the expression of STAT3 in different skin tumors. METHODS: The expression of STAT3 was examined by immunohistochemical staining of 61 specimens of skin tumors; 15 cases of seborrheic keratosis, 15 cases of keratoacanthomas, 18 cases of basal cell carcinomas (BCC) and 13 cases of squamous cell carcinomas (SCC). RESULTS: Immunohistochemical analysis of the 61 skin tumor tissue samples revealed a high expression of STAT3 in the SCCs, which are the more dysplastic and invasive skin tumors. Keratoacanthomas and BCCs showed a mild to moderate expression of STAT3 and a low expression was seen in the specimens of seborrheic keratosis. The degree of the expression of STAT3 in SCCs was much stronger than that in the keratoacanthomas and BCCs, with statistical significance (p<0.05). CONCLUSION: These findings provide the roll of STAT3 in the loss of differentiation and the dysplasia of skin tumors. STAT3 is also thought to have an association with the invasion and metastasis of SCC.

RESUMEN

Cutaneous mucinous eccrine carcinoma is a rare malignant tumor that is presumably of an eccrine gland origin. We observed a 53-year-old man with an erythematous nodule on his left infraorbital area. A large mucinous pool and characteristic tumor cells were shown on H&E stain, and further specific immunohistochemical stains confirmed the diagnosis. We report here on a case of mucinous eccrine carcinoma and we review the relevant literature.

Burn scar can give rise to different types of malignant tumors, and malignant fibrous histiocytoma (MFH) is very rare among these different tumors. We herein report on a rare case of MFH that developed on the burn scar. A 58-year-old male patient who had been scalded on the chest and abdomen at the age of 8 years was hospitalized with exudative ulcer in the scar area. He was diagnosed with MFH and then he underwent wide excision and flap coverage. No sign of metastatic recurrence was noted during the 6 month follow-up period.

RESUMEN

A glomus tumor, is a benign neoplasm, which is derived from a normal glomus body, and participates in thermal regulation. This tumor is relatively uncommon and usually occurs in the peripheral soft tissue, most frequently in the digits, where the glomus bodies are normally present. We experienced a case of a glomus tumor in a 60-year-old man who had a solitary, dome-shaped, grayish blue-colored papule on his back. This was present in an area where a normal glomus body might not usually occur.

RESUMEN

Dopamine is a catecholamine used in the treatment of circulatory shock of various etiologies. The usual dose range used is from 1 to 25 ug/kg/min. When used at higher doses, the prominent effect of dopamine on the vasculature is vasoconstriction induced by stimulation of alpha-adrenergic receptors. We report a case of dopamine gangrene. The gangrene developed on the injection site in the right dorsum of foot in a 43-year-old man during infusion of dopamine for treatment of septic shock. The gangrenous lesion healded after change of injection site to subclavian vein. The dosage of dopamine was not higher, we think that the gangrene developed due to the local vasoconstriction caused by dopamine leakage from the vessels.
