RESUMEN

Defibrillation is no longer universally recommended as initial intervention for the reversal of ventricular fibrillation (VF) after a prolonged and untreated cardiac arrest. We sought to examine this issue in an animal model where a prolonged untreated VF was induced. The aim of this study was to investigate the potential mechanism of the detrimental effect of defibrillation prior to cardiopulmonary resuscitation (CPR) in prolonged cardiac arrest model. VF was electrically induced in 32 domestic male swine weighing 40+/-3 kg and remained untreated for 15 minutes. The animals were then randomly allocated to either the initial defibrillation group or the chest compression group. Mean aortic pressure, right atrial pressure and coronary perfusion pressure (CPP) were continuously measured during the performance. The dimensions of the left ventricle (LV) were assessed by echocardiographic methods. The CPP induced by CPR after defibrillation was significantly lower in the initial defibrillation group than in the chest compression group; 1 minute after defibrillation (9+/-3 mmHg vs. 14.8+/-7 mmHg (P<0.05)), and after 5 minutes 16+/-5 mmHg vs. 21.7+/-1 mmHg (P<0.05). The LV volumes were reduced from 18+/-2 mmHg to 14+/-1 mmHg after defibrillation (P<0.05). In brief, this study showed that the conducting defibrillation prior to chest compression may cause a contracture of the LV, resulting in lowering CPP, thus dropping the efficiency of chest compression in a prolonged cardiac arrest model.

RESUMEN

Distinguishing wild plants in spring from ingestible plants is difficult. Differentiation of budding plants from other plants is particularly difficult. Many people want to find edible plants for health during the spring season. Scopolia japonica can cause symptoms of mania. The author experienced three cases of poisoning by Scopolia japonica. We reported on the cases with literature reviews.

RESUMEN

Meningioma's account for around 15% of all primary brain tumors with some 10% of meningiomas arising in the posterior fossa. In rare cases, a meningioma can form around the endolymphatic sac. When formed in the posterior fossa, meningioma tumors can produce vague, non-specific vertiginous symptoms. Research has observed that a subset of these lesions could produce symptoms indistinguishable from those of Meniere's disease. Therefore, we described the clinical features of a case of posterior petrous meningioma with recurrent vertigo as well as the substantial resolution of symptoms after tumor removal via transmastoid approach.

RESUMEN

Glechoma longituba is a common medicinal plant that is common in wet and sunny land. This plant is used to treat various illnesses including jaundice and edema. But, ingestion of the plant can cause severe pain and swelling on lip and mouth. We experienced three cases that had oral pain and sensory change after ingestion of G. longituba, which we report with a literature review.

RESUMEN

Superwarfarin, such as brodifacoum, is a highly lethal vitamin K antagonist used as a rodenticide. Brodifacoum has a particularly long half-life in the body, which ranges to several months, and therefore requires prolonged treatment with antidotal vitamin K. We experienced a case whereby an 18-year-old male was presented to the hospital with a severe bleeding disorder. It was discovered that he had ingested brodifacoum rodenticide with intent to commit suicide. Despite continual treatment with vitamin K, the bleeding disorder persisted for several months before he recovered. We report this case with literature review.

BACKGROUND AND OBJECTIVES: Hepatocyte growth factor (HGF) is known to stimulate motility, invasiveness, proliferation, and morphogenesis of endothelial cells. Recent reports revealed that this growth factor is also related to tumor invasion and metastasis. We examined the role of HGF/c-Met on the proliferation, dispersion and migration of FaDu cell, a hypopharyngeal squamous cell carcinoma cell line. MATERIALS AND METHOD: We performed RT-PCR and Western blot in FaDu. Proliferation of the FaDu cells was assayed by counting the number of cells after treatment by HGF of different concentrations of 0, 10, 30 ng/mL. Dispersion of the cells was observed by measuring the separation and morphologic changes of cells after the colony of FaDu cells was formed in the media and then treated with HGF of 10 ng/mL or 30 ng/mL for 24 hours. Tumor cell migration was assessed by wound healing assay. Lastly, we examined the enhancement of HGF production in human fibroblast (MRC-5) by putative inducer secreted from FaDu cells. RESULTS: The expresssion of c-Met mRNA and protein was detected in the hypopharyngeal cell line while that of HGF was not. Exogenous HGF significantly enhanced the growth of FaDu in a dose-dependent manner 30 ng/ml (p<0.05). HGF stimulated the dispersion and enhanced the migration of cancer cells in a dose-dependent manner (p<0.05). HGF produced by human stromal fibroblast (MRC-5) was increased by a certain inducer originated from FaDu cells (p<0.05). CONCLUSION: These results suggest that HGF may play an important role in the progression of hypopharyngeal cancer through the enhancement of proliferation, dispersion and migration.

Teratomas are the tumor derived from the three germ cell layers. Teratomas presented in the middle ear and eustachian tube are extremely rare. Eustachian tube teratoma extends either up to the middle ear, external ear, and mastoid or down to the nasopharynx. Tumors extending up to the middle ear usually present with recurrent ear drainage and rarely present with facial palsy. Our patient underwent transoral removal of pedunculated nasopharyngeal mass prior to excision of the ear canal mass. This lesion could be suspected as a "dumb-bell" teratoma involving the nasopharynx, eustachian tube, the middle ear and the external ear canal. This case demonstrates the importance of ear examination when managing nasopharyngeal lesions that originate from the eustachian tube and middle ear, even if the patient is an infant. We report a case of eustachian mature teratoma in a female child.

RESUMEN

Behcet's disease is multi-systemic vasculitis affecting all sizes of arteries and veins. The prevalence of coronary involvement in Behcet's disease is extremely rare. A 35-year-old woman with one-year history of Behcet's disease was in hospitalized with pseudoaneurysm of left anterior descending coronary artery, which was detected by echocardiography. The patient was successfully treated by implantation of an endovascular graft-stent.

Hepatocyte growth factor (HGF), a potent stimulator of hepatocyte growth, stimulates motility, invasiveness, proliferation, and morphogenesis of epithelium, and may be involved in physiologic and pathologic processes such as embryogenesis, wound healing, organ regeneration, inflammation, and tumor invasion. So we examined the role of the HGF/c-Met on invasion and metastasis of hypopharyngeal squamous cell carcinoma (SCC). We performed immunohistochemical stains on 40 normal mucosas and 40 SCCs of hypopharynx with HGF antibody and c-Met antibody. For RT-PCR and Western blot, fresh normal tissues and cancer tissues in hypopharynx obtained from five patients were used. The positive rates of HGF and c-Met expression in hypopharyngeal SCC were 77.5% and 70%, respectively. HGF staining was significantly correlated with lymph node metastasis and pathologic stage (p<0.05). c-Met staining was only significantly correlated with lymph node metastasis (p<0.05). The increased expresssion of c-Met mRNA (RT-PCR) and protein (Western Blot) were detected in hypopharyngeal cancer tissue. These results suggest that HGF may play an important role in the progression of hypopharyngeal cancer.
