RESUMEN

Cardiac myxoma is the most common primary tumor of the heart. Although skin symptoms in patients with cardiac myxoma are common, cutaneous manifestations of cardiac myxoma are usually nonspecific. So only very rarely have the cutaneous signs been clue to the correct diagnosis. We present a 44-year-old-man with a history of episodes of migratory acral erythematous maculopapular lesions on his right hand and foot including the palm and sole, who also had episodes of myalgia in his right side upper and lower extremities. Histological examination revealed an embolus within the vessels in the dermis. Alcian blue staining was positive, consistent with a diagnosis of myxomatous embolus. Two-dimensional echocardiogram revealed a 4 (3cm-sized mobile mass in the left atrium. A diagnosis of left atrial myxoma was strongly suspected and the tumor was excised by open-heart surgery. No more cutaneous lesions or other symptoms of systemic embolism have appeared to date.

RESUMEN

Fusarium species are a group of soil, saprobic and phytopathogic fungi. The pathogen generally affects immunocompromised individuals and infection of immunocompetent people is rarely reported. Fusarium species frequently have been found in immunocompetent people's burned tissue and skin ulcers. We describe a case of cutaneous infection by Fusarium solani in an immunocompetent 67-year-old patient associated with arteriosclerosis obliterans. To the best of our knowledge, this has not been previously reported in the Korean dermatological literature.

RESUMEN

Woolly hair is the presence of Negroid hair on the scalp of non-Negroid people. We report a rare case of woolly hair on a 14-year-old Korean girl who presented with tightly-curled, fine and light brown hair which she had had since birth. Light microscopy disclosed flattening of the hair shafts with partial twists at irregular intervals. On scanning electron microscopy, the patient's hair revealed weathered cuticles, and twisted and flattened hair shafts.

RESUMEN

A healthy 4-year-old boy presented with a linear erythematous eruption, along with some pearly papules on the right cheek. The distribution of the lesions corresponded to that of Blaschko?s lines. Histopathological examination showed necrotic keratinocytes and vacuolar changes on the basal layer in the epidermis, and a superficial and deep perivascular, perieccrine, and perifollicular lymphocytic infiltrate in the dermis. In addition, a small keratin-filled cyst was observed in the upper dermis. These findings were consistent with a diagnosis of milium developing on lichen striatus. We applied 0.03% topical tacrolimus ointment twice daily. On follow-up 12 months later, the linear erythematous eruption cleared, but the pearly papules persisted.

RESUMEN

Leukoedema of the oral mucosa is a whitish or whitish-gray edematous lesion of the buccal and labial oral mucosa. The condition is seen most frequently among black people, and has not yet been reported in the Korean dermatologic literature. We report a 28-year-old Korean woman affected by leukoedema of the oral mucosa. She presented with a 3 year history of white plaques on both buccal mucosae. The diagnosis was clinically based on the presence of white plaques on both buccal mucosae which disappeared when the lesion was stretched, and histologically based on the acanthosis and intracellular edema.

RESUMEN

Multiple minute digitate hyperkeratosis (MMDH) is a rare disorder of keratinization characterized clinically by multiple, tiny, spiky, keratotic papules. Herein, we report a new case of MMDH. There was no family history of similar lesions or a personal history of underlying disease. The lesions were limited to the face and associated with acne vulgaris. We consider this case as a postinflammatory type of MMDH.

BACKGROUND: Cutaneous melanoma is a malignant neoplasm that has the tendency to metastasize through lymphatic vessels. However, the mechanism of lymphatic spread in malignant melanoma is not fully understood due to lack of lymphatic-specific markers. OBJECTIVE: The purpose of this study was to determine the intra- and peritumoral lymphatic vessel density (LVD) using a novel monoclonal antibody D2-40, and whether increased expression of lymphatic vessel correlated with malignancy grading in a series of melanocytic lesions and prognosis of malignant melanoma. METHODS: The intra- and peritumoral LVD were examined by immunohistochemistry using D2-40 antibody in a series of melanocytic lesions. RESULTS: We found significantly higher intra- and peritumoral LVD in malignant melanoma as compared with either benign melanocytic nevus, dysplastic nevus, or melanoma in situ (p<0.05), and the intratumoral LVD was significantly related to Breslow depth (p<0.05) and the development of lymphatic metastasis (p<0.05). CONCLUSION: The higher intra- and peritumoral LVD in malignant melanomas suggests that melanoma cells might promote lymphangiogenesis. In addition, our data suggests that increased lymphatics in melanoma is an independent prognostic factor and important for the development of lymphatic metastasis.

RESUMEN

Verrucous carcinoma is a low-grade squamous cell carcinoma. It usually occurs in the oral cavity, genital or palmoplantar area. A 55-year-old woman presented with an asymptomatic, erythematous, round, hyperkeratotic nodule, of 0.8cm in diameter, on the left cheek. A skin biopsy revealed a typical verrucous carcinoma. We report herein an unusual case of verrucous carcinoma occurring on the face.

RESUMEN

Xanthoma may or may not be associated with abnormal serum lipids. Normolipidemic xanthomas can occur in association with underlying lymphoproliferative disease, or they can result from lipid deposition with damaged or altered skin. A 56-year-old woman presented with multiple, yellowish to erythematous papules and plaques on the periocular area and forehead. She had no evidence or history of hyperlipidemia. The biopsy specimen of the forehead showed a diffuse histiocytic infiltrate with abundant bubbly cytoplasm, associated with lobular lymphocytic panniculitis and mucin deposition. Based on the clinical and histopathologic findings, we diagnosed this case as lupus erythematosus panniculitis with xanthomatous reaction. The patient was treated with low dose oral steroids and antimalarials, and her skin lesions were improved after 2 weeks.
