RESUMEN

Objective:To explore the different types and characteristics of rehabilitation exercise compliance of patients with first stroke at 2 weeks of onset, and analyze its influencing factors, so as to provide reference for the formulation of targeted health education.Methods:A cross-sectional investigation was conducted in Neurology Department of the Second Affiliated Hospital of Zhejiang University School of Medicine from January to June, 2021. 276 patients with first-episode stroke were investigated by the Questionnaire of Exercise Adherence at 2 weeks of the onset. The potential profile analysis was conducted to explore characteristics classification of the rehabilitation exercise compliance. And the chi-square test was used to compare demographic differences among different categories and ordered multi classification Logistic regression was used to explore the influencing factors of rehabilitation exercise compliance.Results:The patients were divided into 109 cases with high rehabilitation exercise compliance (39.5%), 114 cases with moderate rehabilitation exercise compliance (41.3%), and 53 cases with low rehabilitation exercise compliance (19.2%). There were statistically significant differences in education level, consciousness level at admission, complications and limb muscle strength among the three types of patients ( χ2 values were 6.17-31.50, all P<0.05). Ordered multi classification Logistic regression showed that the patient′s education level, the patient′s consciousness level at admission, whether there were complications and limb muscle strength would affect the rehabilitation exercise compliance of stroke patients ( P<0.05). Conclusions:There are three potential categories of rehabilitation exercise compliance in patients with first stroke. Patients with illiteracy, drowsiness, complications and poor limb muscle strength have poor rehabilitation exercise compliance.

BACKGROUND@#Severe hand-foot-and-mouth disease (HFMD) is a life-threatening contagious disease among young children and infants. Although enterovirus A71 has been well acknowledged to be the dominant cause of severe HFMD, there still remain other unidentified risk factors for severe HFMD. Previous studies mainly focused on identifying the individual-level risk factors from a clinical perspective, while rare studies aimed to clarify the association between regional-level risk factors and severe HFMD, which may be more important from a public health perspective.@*METHODS@#We retrieved the clinical HFMD counts between 2008 and 2014 from the Chinese Center for Disease Control and Prevention, which were used to calculated the case-severity rate in 143 prefectural-level cities in mainland China. For each of those 143 cities, we further obtained city-specific characteristics from the China City Statistical Yearbook (social and economic variables) and the national meteorological monitoring system (meteorological variables). A Poisson regression model was then used to estimate the associations between city-specific characteristics (reduced by the principal component analysis to avoid multicollinearity) and the case-severity rate of HFMD. The above analysis was further stratified by age and gender to examine potential modifying effects and vulnerable sub-populations.@*RESULTS@#We found that the case-severity rate of HFMD varied dramatically between cities, ranging from 0 to 8.09%. Cities with high case-severity rates were mainly clustered in Central China. By relating the case-severity rate to city-specific characteristics, we found that both the principal component characterized by a high level of social and economic development (RR = 0.823, 95%CI 0.739, 0.916) and another that characterized by warm and humid climate (RR = 0.771, 95%CI 0.619, 0.960) were negatively associated with the case-severity rate of HFMD. These estimations were consistent across age and gender sub-populations.@*CONCLUSION@#Except for the type of infected pathogen, the case-severity rate of HFMD was closely related to city development and meteorological factor. These findings suggest that social and environmental factors may also play an important role in the progress of severe HFMD.

Objective To evaluate the compliancy of HIV pre-exposure prophylaxis (PREP) in men who have sex with men (MSM) in Chengdu,Sichuan province,and explore the influencing factors.Methods From 1 July 2013 to 30 September 2015,a random,open,multi-center and parallel control intervention study was conducted in 328 MSM enrolled by non-probability sampling in Chengdu.The MSM were divided into 3 groups randomly,i.e.daily group,intermittent group (before and after exposure) and control group.Clinical follow-up and questionnaire survey were carried out every 3 months.Their PrEP compliances were evaluated respectively and multivariate logistic regression analysis was conducted to identify the related factors.Results A total of 141 MSM were surveyed,in whom 59(41.8%) had good PrEP compliancy.The PrEP compliancy rate was 69.0% in daily group,higher than that in intermittent group (14.3%),the difference had significance (x2=45.29,P<0.001).Multivariate logistic analysis indicated that type of PrEP was the influencing factors of PrEP compliancy.Compared with daily group,the intermittent group had worse PrEP compliancy (OR=0.07,95%CI:0.03-0.16).Conclusion The PrEP compliance of the MSM in this study was poor,the compliancy would be influenced by the type of PREP.

RESUMEN

Time perspective therapy (TPT) which belongs to positive psychology is a new psychological intervention to post-traumatic stress disorder (PTSD).A large number of studies abroad have testified that TPT is very long-term effective,what′s more, TPT has been popularized and improved by foreign scholars but not been applied in China.TPT has been a study issue in mental and psychological fields,but that how to apply TPT rightly in China needs to explore.Therefore, this paper systematically introduced and analyzed TPT theory, the problems of applying TPT and the enlightenment of nursing PTSD,so as to help Chinese scholars apply TPT to solve problems and status of nursing PTSD in China.

RESUMEN

Objective To study the prevalence rates of Rash and Febrile Illnesses (RFIs) including measles,rubella,scarlet fever,exanthema subitum and the differences among measles and other RFIs to tentatively formulate the logistic regression model through clinical manifestation.Methods All the suspected cases of measles,rubella,scarlet fever,exanthema subitum reported by the county/prefecture lever hospitals at four counties were collected during March 2011 to February 2012.When setting laboratory confirmed measles as dependent variable and existed symptoms as independent variable,a logistic regression model was formulated and optimal operational point (OOP) chosen,according to the ROC curve.Results A total number of 551 cases were collected but the consistency of measles diagnosis between clinical and laboratory was not satisfied,with Kappa value=0.349,same to the diagnosis of rubella.As for the result from the two-lever logistic regression model,symptoms that related to the confirmation of measles would include cough (OR=5.75),conjunctivitis (OR=3.00),Koplik spot (OR =7.52),lymphadenectasis (OR =0.07),rash after fever (OR=0.07).The area under ROC curve was 0.97 and the optimal operational point was 0.249.Conclusion A logistic regression model was formulated using the clinical symptoms which was resulted in better performance on prediction.As the sample size of this survey was small,the expansion on the scale of investigation and laboratory testings were needed before the types and components of measles-related RFIs be clarified.

RESUMEN

Objective To evaluate the screening effect of OSTA in Chinese post-menopausal women, to provide suggestions for the further development of OP screening in mainland China. Methods With the data from Chinese Database of Bone Mineral Density of Chinese women aged 50 years old and above, each subject was calculated a OSTA score by the formula of OSTA and classified into three risk levels (the normal, the osteopenia and the osteoporosis) or two groups (the low and the high risk) by the original cutoffs, and compared with results by T-score. A series of screening indexes, such as sensitivity, specificity, kappa value and ROC etc., were employed to compare and analyze the diagnosis results of OSTA with T-score from the femoral neck bone density. Objective To evaluate the therapeutic efficiency of reversed lamellar keratoplasty for bullous keratopathy, Terrien's marginal degeneration and ocular chemical burns.Methods Twenty eyes of patients with bullous keratopathy, twelve eyes with Terrien's marginal degeneration, ten eyes with chemical burns were recruited in this study.Diseased tissues were removed by means of dissection.Reversed lamellae corneal tissues which had been prepared by cryopreservation were thawed and rinsed. Then the prepared posterior corneal lamellae was turned over, endothelium facing upwards. The diameter of the graft should be 0.5~1.0mm larger than that of the recipient bed. The reversed donor posterior lamellae was sutured into the host keratectomy bed with 10-0 nylon sutures continuously or interruptedly.Results Allograft reaction episode occurred only one (chemical burn) of the total 42 eyes enrolled.The grafts of the rest 41 eyes maintained clear or semitransparent for 1~5 years. Graft edema could be observed within 2~4 weeks postoperatively. Postoperative interface separation was not found. For all 20 patients with bullous keratopathy, the symptoms of pain disappeared. The visual acuity improved 2~3 lines of Snellen among eleven of twelve patients with Terrien's marginal degeneration. The visual acuity improved 2~3 lines of Snellen with four patients with chemical burns.Conclusion The technique of reversed lamellar keratoplasty can be applied successfully for tectonic reconstruction of corneal surface such as bullous keratopathy, Terrien' s marginal degeneration and also chemical burns.The effects of therapy was satisfactory.

Objective To analyze factors affecting the expenses of inpatients and their comprehensive force of influence so as to check irrational increases of medical expenses. Methods Data on hospitalization expenses for disease entities ranking in the first ten places in a third tier hospital in 1997 were taken as samples and a statistical analysis was conducted of the medical expenses incurred in the discharged cases by means of the nerve network model. Results Major factors affecting hospitalization expenses were days of hospitalization, pre hospitalization conditions, pre operation length of stay and age, the comprehensive force of influence of which was respectively 0 4879 , 0 2124 , 0 1847 and 0 0612 . Conclusion To check the irrational increases of medical expenses, we must focus our efforts on the reduction of expenses for medicine and shorten days of hospitalization and pre operation length of stay.

Yeast rosette rate of RBC C_3b receptor, as an active index of immunity of RBC, can serve as an indicator of patient's immune fuction and also can be reference for judging treatment effect and prognosis of a disease just as active rosette rate of T lymphocyte can do.
