RESUMEN

Here we report the diagnosis and treatment of a rare IgG4-related kidney disease with nephrotic syndrome as the first manifestation. A 62-year-old male patient, presented with edema in both lower limbs and foam urine, had a history of "lung malignant tumor with brain and lymph node metastasis". The increase of IgG4 and decrease of glomerular filtration rate were detected at admission, and the pathological consideration of renal biopsy was membranous nephropathy with IgG4-related tubulointerstitial nephritis. After the combination of low-dose glucocorticoids therapy and rituximab treatment, the patient showed good prognosis in a 9 month follow-up.

RESUMEN

Primary age-related tauopathy (PART) is characterized by tau neurofibrillary tangles (NFTs) in the absence of amyloid plaque pathology. In the present study, we analyzed the distribution patterns of phosphorylated 43-kDa TAR DNA-binding protein (pTDP-43) in the brains of patients with PART. Immunohistochemistry and immunofluorescence double-labeling in multiple brain regions was performed on brain tissues from PART, Alzheimer's disease (AD), and aging control cases. We examined the regional distribution patterns of pTDP-43 intraneuronal inclusions in PART with Braak NFT stages > 0 and ≤ IV, and a Thal phase of 0 (no beta-amyloid present). We found four stages which indicated potentially sequential dissemination of pTDP-43 in PART. Stage I was characterized by the presence of pTDP-43 lesions in the amygdala, stage II by such lesions in the hippocampus, stage III by spread of pTDP-43 to the neocortex, and stage IV by pTDP-43 lesions in the putamen, pallidum, and insular cortex. In general, the distribution pattern of pTDP-43 pathology in PART cases was similar to the early TDP-43 stages reported in AD, but tended to be more restricted to the limbic system. However, there were some differences in the distribution patterns of pTDP-43 between PART and AD, especially in the dentate gyrus of the hippocampus. Positive correlations were found in PART between the Braak NFT stage and the pTDP-43 stage and density.

RESUMEN

Through the addition of discussion course associated with judicial expertise during the pre medical education, integration of true and typical forensic pathological cases into basic medical theory and experimental education, further addition of optional course of forensic medicine,and guiding the medical students applying the scientifically training projects about forensic pathology, students may improve their learning interesting and clinical thought, and are made early warning and increase the abilities of preventing and dealing with the suddenly medical tangles in the future, at the same time, the medical teachers also increase their professional levels and teaching qualities.These benefit the growing up of high quality medical doctors, decrease and even prevent the happening of medical tangles.

AIM: To explore the effect of low concentration of N-methyl-N-nitro-N-nitroguanidine (MNNG) on p38MAPK, and the function of glutathione (GSH) on p38MAPK. METHODS: Western blotting was applied to detect the p38MAPK phosphorylation in MNNG treatment group and control group. To study the effect of GSH on MNNG-regulated p38MAPK activity, the intracellular GSH level was reduced by pretreatment of L-buthionine-S, R-sulfoximine (BSO). Assuming the absorbance of band in control group as 1.0, the relative P/T values of the treatment groups were calculated, with the “P” served as absorbance values of phospho-p38MAPK and the “T” as absorbance values of total p38MAPK. RESULTS: In BSO pretreated groups,the relative P/T in samples treated with MNNG for 2.5 h was 0.84, and became 2.19 with 3 h more incubation in the fresh medium. CONCLUSION: GSH deletion increases the activity of p38MAPK in MNNG treatment.

AIM: To identify the effect of alkylating agent N-methyl-N'- nitro-N-nitrosoguanidine (MNNG) with low concentration on JNK/SAPK and p38MAPK and the origins of JNK/SAPK and p38MAPK cascade.METHODS: p38 and JNK kinase activity were detected by immunoprecipitation and Western immunoblotting in intact and enucleated Vero cells.RESULTS: With the same experimental conditions, low concentration of MNNG inhibited JNK kinase in both intact cell and enucleated Vero cell. MNNG activated p38 kinase in intact cell while no effect on p38 kinase in enucleated cell was observed.CONCLUSION: Inhibition of JNK/SAPK by low concentration of MNNG was independ of a nuclear signal while MNNG activation of p38MAPK may depend on a nuclear signal.
