RESUMEN

The keratinocyte culture has been used for the reconstruction of artificial skin or as in vitro skin model. For these purpose, keratinocytes should be cultured for long time (usually 10 cumulative population doublings) and it is important to evaluate the state of replicative senescence or cell senescence during this time. This study was undertaken to investigate senescence associated beta-galactosidase (SA-beta-gal) activity for the senescent cell and keratin 19 immunohistochemistry for the skin stem cell in keratinocytes on ten times serial subculture (referred to 10 cumulative population doublings). Keratinocytes were isolated from foreskin of a 2 day old child, and cultured in KSFM and then in DMEM/F-12. In both keratinocytes cultured in KSFM and DMEM/F-12, lysosomal beta-galactosidase activity, which is detected at pH 4.0, was present in all cells regardless of their replicative age. In contrast, SA-beta-gal activity, which is detected at pH 6.0, was present in a few cells (from 0.1% to 3%) during whole subcultures. These data suggest that most of keratinocytes did not undergo replicative senescence in this culture. Furthermore, in keratin 19 skin stem cell staining, a lot of keratinocytes (13.8%) showed strong positive reaction on the 10th subculture. Together with the results of beta-galactosidase activity, the persistence of high proportion of keratin 19 positive skin stem cells implies further increment of keratinocyte populations by continued subcultures.

RESUMEN

Megalourethra is a rare congenital abnormaly characterized by severe dilatation of urethra, the disorder at first reported by Nesbitt in 1955. The scaphoid type is due to poor development of the corpus spongiosum, whereas in the more severe fusiform variety the corpora cavernosa also are affected. We present a case of congenital scaphoid megalourethra associated with acute renal failure, sepsis and grade IV bilateral vesicoureteral reflux. Initial management of this patient was vesicocutaneostomy and urethrocutaneostomy followed later by urethral reconstruction.

RESUMEN

We performed surgical treatment in 18 cases of torsion of the spermatic cord during the past four years. Patient age ranged from 2 to 54 years old : 13 cases were 16 or less. Torsion involved the left side in 11 cases. Surgical detorsion was performed in 10 cases, but in the other 8 cases orchiectomy was unavoidable. Of 8 cases treated with orchiectomy, the diagnosis was delayed in 7 cases due to mistaken diagnosis, primarily epididymitis by doctors in local clinics and general hospitals. The time of delay in the orchiectomy group ranged from 54 hours to 2 weeks with the average of 6.28 days. For one case, the visit to our hospital was too late. In 10 cases treated with surgical detorsion, all of the patients visited our hospital within 48 hours. Improvement of the testis salvage rate requires that adolescents, their families and teachers should learn to recognize this disease, and particularly doctors of other departments should recognize it as acute scrotum.

RESUMEN

The following result were obtained through the clinical review of 78 cases with severe tuberculosis under the age of 5 years admitted at Department of Pediatrics, busan National University Hospital during the period of january, 1974 through December, 1980. 1) Incidence of severe tuberculosis in infancy and early chilhood is noticed as 49.3% in tuberculous menigitis, 16.0% in miliary tuberculosis and 34.9% in both. 2) There was no significant annual difference and the higher incidence by age noticed as 48.7% in patients under age of 2 Years. Seasonal distribution showed slightly increased incidence in spring and early summer. 3) Family history of tuberculosis was found in 50.0% of cases and most sources of infection were their parents. 4) In tuberculin skin test, 36.8% of cases were noticed as positive reaction. BCG Vaccination was not done in 53.8% of cases. 5) Treatment was done in two divided group : one group with INH. SM. And PAS. And another group with INH. SM. And RMP. Effectiveness of treatment was better in the group treated with INH.SM. and RMP. Than the group treated with INH.SM. and PAS. Liver function abnormalities were noticed in the group treated with INH.SM. and RMP. After 2-3 weeks but most casease were recovered by redused doses.
