RESUMEN

PURPOSE: To assess the affinity of large head total hip arthroplasty in Korean lifestyle sitting position, and the limitation of measurement for post operative function score system. And we assess the usefulness of Korean Lifestyle Score System on Korean patients who get large head total hip arthroplasty operation. MATERIALS AND METHODS: 23 patient performed by one operator during for 11 month. We use ceramic - ceramic bearing, 36mm sized head implants, of Lima corporation and we workup at least 6 month. We check Korean Lifestyle Checklist Score. Among the 6 position of the checklist, we check each angles of flexion, external rotation and abduction angles of crossleg, bended knee, squatting position. And we compare each data with normal person's data. RESULTS: Average flexion is 93degrees, external rotation is 67degrees, and abduction is 37degreesat crossed leg position. Flexion is 85degrees, external rotation is 15degrees, abduction is 0degreesat bended knee. At squatting position, flexion is 109degrees, external rotation is 45degrees, abduction is 42degrees. Each Korean Lifestyle Checklist Score : use of toilet : 4.9 point, wearing socks is 3.82 points, crossed leg position is 2.48 points. Squatting position is 3.57 points. Tie of shoe is 3.74 points. Bended knee position is 3.7 points. CONCLUSION: Large head total hip arthroplasty is a good method for Korean Life style without dislocation. Previously used functional score of Harris Hip Score and Iowa score are just focused on function not on positional aspect. So we recommend Korean Lifestyle Checklist System for Korean arthroplasty patients.

PURPOSE: To analyse the efficacy of MRI in assessing fracture configuration of tibial plateau. MATERIALS AND METHODS: The subjects were consisted of the MR images and the plain radiographs of the thirty one patients with tibial plateau fracture. With the plain radiographs, the fractures were classified according to the Schatzker's system and the amount of depression and displacement, location of the major fragment were measured. Then the same parameters were measured with MR images. Additionally new fracture lines and resulting fragments which can be fixed with a screw or transfixing wire were described. The two groups were compared with the each parameters. RESULTS: Fracture classifications were changed in 4 cases. The locations of main fragment were changed in 10 cases. Significant difference was noted in the amount of articular depression (p<0.05) but no significant difference was found in displacement (p=0.332) . Especially in Schatzker-VI cases, articular step offs over 2mm and articular fragments over 2x2cm were revealed in detail by MRI (p<0.05) . CONCLUSION: Fracture type, amount of depression, location, and fracture configuration can be better visualized by MRI, especially in highly comminuted plateau fractures (Schatzker-VI) . The MR imaging seems to be recommended as a primary mode for evaluation of plateau fractures, especially in Schatzker-VI fractures.

RESUMEN

Twenty nine cases of segmental tibial fracture were treated at Ewha Womans University Hopital, Department of Orthopedic Surgery, during the period from January, 1970 to December, 1984. The following results were obtained. 1. Most common fracture level was type I, high middle segment, occurred about 12 cases (41.4%). 2. In view of the higher incidence of open fracture (16 cases, 55.1%) and comminuted fracture (22 cases, 75.9%), fracture was caused by high velocity. 3. Various external and internal fixation methods were applied, but higher union rate was seen in the cases of Küntscher nailing and plating with bone graft. 4. Excluding the cases of type V, average union rate was 28.5 weeks and open fracture (ave. 29.6 weeks) required more longer union rate than closed fracture (ave. 26 weeks). 5. Higher rate of complication (21 cases, 72.4%) was noted than other types of tibial fracture. 6. Good result was obtained for treatment of 5 cases of delayed union or non-union by plating and bone graft than any other methods.
