RESUMEN

BACKGROUND@#The duration of immobilization after arthroscopic rotator cuff repair and the optimal time to commence rehabilitation are still the subject of ongoing debates. This study was undertaken to evaluate the functional outcome and rotator cuff healing status after arthroscopic rotator cuff repair by comparing early and delayed rehabilitation.@*METHODS@#Totally, 76 patients with small, medium, and large sized rotator cuff tears underwent arthroscopic repair using the suture-bridge technique. In early rehabilitation group, 38 patients commenced passive range of motion at postoperative day 2 whereas 38 patients assigned to the delayed rehabilitation group commenced passive range of motion at postoperative week 3. At the end of the study period, clinical and functional evaluations (Constant score, the University of California, Los Angeles [UCLA] shoulder score) were carried out, subsequent to measuring the range of motion, visual analogue scale for pain, and isokinetic dynamometer test. Rotator cuff healing was confirmed by magnetic resonance imaging at least 6 months after surgery.@*RESULTS@#No significant difference was obtained in range of motion and visual analogue scale between both groups. Functional outcomes showed similar improvements in the Constant score (early: 67.0–88.0; delayed: 66.9–91.0; p<0.001) and the UCLA shoulder score (early: 20.3–32.3; delayed: 20.4–32.4; p<0.001). Furthermore, rotator cuff healing showed no significant differences between the groups (range, 6–15 months; average, 10.4 months).@*CONCLUSIONS@#Delayed passive rehabilitation does not bring about superior outcomes. Therefore, early rehabilitation would be useful to help patients resume their daily lives.

RESUMEN

Arteriovenous hemangiomas are found mainly in the limbs and scalp, and are common in women and children. However, there have been very limited reports concerning posttraumatic arteriovenous hemangioma, and none was reported in the medical literature regarding posttraumatic arteriovenous hemangioma of hypothenar eminence causing neurologic symptoms. We report a case of arteriovenous hemangioma of hypothenar eminence after recurrent trauma that was successfully treated by an excision of the mass and vein graft.

RESUMEN

Intramuscular schwannomma is unusual and rare cases were reported in the literature in the gluteus maximus muscle. We present a case of an intramuscular schwannoma arising from the gluteus maximus muscle in a sixty-five-year-old woman. An oval in shape and well encapsulated tumor was found embedded with the gluteus maximus muscle, and then the lesion was excised surgically. Two years following excision of the lesion, the patient remained asymptomatic, with no evidence of local recurrence.

RESUMEN

Calcific tendinitis is most common seen within the rotator cuff of the shoulder, although it may develop around the hip, wrist, elbow, knee, forefoot, and neck. However, there has been no report in the medical literature regarding calcific tendinitis of the common extensor tendon. We present a case of a 26-year-old woman who had calcific tendinitis of the common extensor tendon. Intraoperatively, partial rupture and calcific deposit at the insertion of the common extensor tendon were seen. We were removed calcific deposit and ruptured tissue of common extensor tendon, and then ruptured common extensor tendon was sutured. The patient showed excellent result two years postoperatively with return to range in a degree of activity levels.

RESUMEN

Although calcaneal fracture is relatively common in ankle injury, open intraarticular calcaneal fracture with dorsal dislocation of the navicular from talus is extremely rare and severe injury. There are few data which are available concerning the injury mechanism and treatment options. The purpose of this report is to describe a case with bilateral open transcalcaneal fracture with talonavicular dislocation and to discuss the prevalence, mechanism of this injury, and treatment options.

RESUMEN

Bisphosphonates are widely used in the management of metastatic bone disease and in the prevention of osteomalacia and osteoporosis. In particular, oral preparations are more commonly used for the prevension and treatment of osteoporosis. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) has been well documented recently in relation to intravenous preparations of the drug. But, a few cases have been reported of oral bisphosphonate-associated osteonecrosis. We could not find any risedronate cases in the Korean medical literature. Here we report a case of BRONJ in a 91-year-old woman patient receiving an oral bisphosphonate (risedronate) for the treatment of osteoporosis.

RESUMEN

Subchondral cyst is a benign cystic lesion, and it is often found in patients with rheumatoid arthritis or osteoarthritis. However, its occurrence in those joints without preexisted disease is rare. Furthermore, there has been no report in the medical literature regarding pathologic fracture in the bony plate of a subchondral cyst at the superior acetabulum. We report here on a case of the 44-year-old woman for whom a pathologic fracture was found in the bony plate of a subchondral cyst at the superior acetabulum.

RESUMEN

Spontaneous bilateral rupture of the quadriceps tendons without a significant history of trauma is an uncommon disease. It is generally associated with mechanical factors and coexisting systemic and local factors are taken into consideration in the pathogenesis of these ruptures. In patients with some chronic systemic diseases, simultaneous rupture can occur spontaneously or with minor trauma. We present a case of patient with chronic renal failure on hemodialysis for the past 9 years, who sustained this injury, and subsequently had surgical repair of both tendons. We performed a preoperative magnetic resonance imaging to confirm the diagnosis, then we repaired the ruptured quadriceps tendons and performing MRI postoperatively 1 yr later. He regained his normal knee joint functions 1 yr after the operation.

RESUMEN

PURPOSE: We wanted to evaluate the relating factors for deformities of the foot after tibial fracture and the outcome of respective surgical treatment. MATERIALS AND METHODS: We studied thirty-two patients (thirty-five feet) who had foot deformities after tibia fracture. The age, gender, shape, location, concurrent soft tissue injury and operative treatment of tibial fracture were analysed and outcome of the surgical treatment was investigated. RESULTS: Deformities of the foot mostly occurred in open fractures of Gustilo type 3 or closed fracture with severe soft tissue injury. Variable surgical treatments such as simple release, lengthening, Z-plasty and osteotomy were performed. The AOFAS ankle-hind foot scale was improved from average of 37.3 points preoperatively to an average of 77.2 points at the last follow up. The Maryland foot score was from 42.9 to 90.2 and AOFAS Lesser Toe Metatarso-phalangeal interphalangeal scale was from 42.9 to 90.5. Radiological correction was also improved from 21.9 degrees to 7.9 degrees (Meary angle), from 112.2 to 138.5 (Hibb's angle) and from 33.8 to 25.9 (Calcaneal pitch). CONCLUSION: We proposed that great care should be taken of treatment for the tibial fracture and early detection and proper management of the foot deformities are crucial.

RESUMEN

PURPOSE: To evaluate the clinical and radiologic efficacy of posterior lumbar interbody fusion (PLIF) using a threaded fusion cage (TFC) alone with autogenous iliac corticocancellous bone graft for single-level degenerative lumbar spinal disorders. MATERIALS AND METHODS: Thirty-eight consecutive patients receiving PLIF with TFC alone were evaluated with at least three years of follow-up. Clinical outcome and satisfaction were assessed using Ragab's measurements. Radiological outcomes were analyzed in terms of lumbar lordosis, segmental lordosis, disc height, and fusion rates. RESULTS: Disc height was 6.18+/-2.54 mm, 11.71+/-1.60 mm and 9.94+/-1.94 mm on average for preoperative, postoperative, and final follow-up, respectively. Lumbar lordosis was 27.46+/-11.47degrees, 31.41+/-8.89degrees on average for preoperative and at final follow-up, respectively. Segmental lordosis was 11.51+/-9.35degrees preoperatively, and 10.86+/-7.49degrees at final follow-up. Satisfactory clinical outcome were obtained in 89.2% of patients with successful bone fusion in 73.0% of patients. CONCLUSION: Stand alone TFC is an effective treatment modality in maintenance of disc height and lumbar lordosis, with satisfactory long term clinical outcomes in the treatment of single-level degenerative lumbar spinal disorders.

RESUMEN

PURPOSE: To investigate the effect of human joint fluid media on the survival and proliferation of bone marrow derived precusor cell, and to provide the basic data of the intraarticular injection with scaffold-free progenitor cell in advanced degenerative osteoarthritis (OA). MATERIALS AND METHODS: We obtained the joint fluid and bone marrow from 15 patients who had total knee arthroplasty due to degenerative OA, and isolated the mesenchymal progenitor cells (MPCs) from bone marrow by washing and ten times subculture. We devided the control and experiment groups according to the addition of joint fluid at various ratios (1/100, 1/10, 1, 10, 100, 1000 microliter), and statistically analyzed the numbers of mesenchymal progenitor cell proliferated according to the culture period. RESULTS: The experiments using joint fluid without centrifuge showed the increase of MPCs as the culture poriod was extended and was independent to the existence of fetal bovine serum, the dose dependent pattern in the increase of MPCs in proportion to the dose of joint fluid, and statistically significant increase MPCs in 10, 100, 1000 microliter of serum contained groups, 1000 microliter of serum-free groups (p=0.039, p=0.017, p=0.077, p=0.004). The experiments using joint fluid with centrifuge showed the increase of MPCs as the culture period was extended and was independent of fetal bovine serum, and the dose dependent pattern in the increase of MPCs in proportion to the dose of joint fluid, and statistically significant increase MPCs in 1000 microliter of both serum contained and serum-free groups (p=0.006, p=0.024). CONCLUSION: MSCs not only can survive, but also proliferate in human joint fluid. The rate of proliferaton is increased faster by the adding of joint fluid than only using common media in cell culture. And the experiment shows the dose dependent pattern in the increase of MPCs in proportion to the dose of joint fluid.

PURPOSE: In this study, the effects of dexamethasone, retinoic acid and vitamin D on the transdifferentiation of adipocyte and osteoblast were investigated using 3T3-L1 and KS483 cells, pre-adipocyte and pre-osteoblast cell lines, respectively. MATERIALS AND METHODS: Both cells were induced to differentiate in adipogenic or osteogenic conditions for 10 days, and the enzyme activities of lipoprotein lipase (LPL) and alkaline phosphatase (AP), well-known marker enzymes for adipogenesis and osteogenesis, respectively, were determined. Oil red-O and the AP activity staining of the cells were also determined. RESULTS: The presence of dexamethasone during the differentiation period did not cause any specific alterations in the LPL on AP activities in either cell line. Retinoic acid increased AP activity in both cell lines, whereas no change was observed in LPL activity in 3T3-L1 cells. Vitamin D increased AP activity but decreased LPL activity in 3T3-L1 cells. CONCLUSION: The fates of both cell lines in differentiation lineage have been already determined. However, the presence of retinoic acid and vitamin D during the differentiation period can cause the transdifferentiation of both committed cell lines.
