RESUMEN

Two cases of traumatic dislocation of the hip in childhood are presented. Both the dislocations were posterior and involved the left hip joints. Closed reduction was carried out in both the cases with a click within one and a half hours and within eight hours of injury respectively. No complication occurred in either of the patients. The second case was associated with an undisplaced greenstick fracture of the lower third of the ipsilateral femur. The results are graded as excellent in both the cases at follow up as both the patients have normal hip joints

RESUMEN

Thirtynine fractures of the femoral shaft in 38 patients were treated with compression plating using the posterolateral approach. These fractures were situated from below the lesser trochanter to about 7.5 cms above the articular surface of the medial femoral condyle. After operative fixation of the fracture, no external form of splinting was needed as reasonably long compression plates with screws biting a minimum of 8 to 10 cortices in each of the main fractured fragments were fixed. Quadriceps and knee bending exercises followed by non-weight-bearing mobilisation on bilateral axillary crutches was started immediately after the operation. 38 fractures united well and in only one case, where there was breakage of the plate with non-union, reosteosynthesis with longer plate and bone grafting was done. The results were excellent in 36 fractures in 35 patients, fair in 2 patients in whom the last 20 degrees of flexion was restricted, and poor in 1 case

RESUMEN

This study was conducted on 29 severely displaced unstable supracondylar fractures of the humerus in Nizwa Hospital from June 1992 to March 1997. These fractures were treated by open reduction and internal fixation by a bicipital approach to the elbow joint using criss-cross K-wires. The advantages of opening the elbow from both sides provide for an exact anatomic reduction of the fracture which can be appreciated by the naked eye from the antero-posterior, medial and lateral sides; by using criss-cross K-wires the chances for medial or lateral angulation of the distal fragment are minimized and this thus prevents cubitus varus or cubitus valgus; and lastly, it obviates the need for an image intensifier where it is not available. In this series all the patients, except one, finally had excellent results with full range of movements of the elbow joint with no cubitus varus or valgus deformity. The one patient who developed late cubitus varus had also loss of terminal 15° of flexion
