BACKGROUND@#Neuroendocrine neoplasms (NENs) are rare tumors characterized by variable biology and delayed diagnosis. However, the nationwide epidemiology of NENs has never been reported in China. We aimed to estimate the incidence and survival statistics of NENs in China, in comparison to those in the United States during the same period.@*METHODS@#Based on the data from 246 population-based cancer registries covering 272.5 million people of China, we calculated age-specific incidence on NENs in 2017 and multiplied by corresponding national population to estimate the nationwide incidence in China. The data of 22 population-based cancer registries were used to estimate the trends of NENs incidence from 2000 to 2017 through the Joinpoint regression model. We used the cohort approach to analyze the 5-year age-standardized relative survival by sex, age group, and urban-rural area between 2008 and 2013, based on data from 176 high-quality cancer registries. We used data from the Surveillance, Epidemiology, and End Results (SEER) 18 program to estimate the comparable incidence and survival of NENs in the United States.@*RESULTS@#The overall age-standardized rate (ASR) of NENs incidence was lower in China (1.14 per 100,000) than in the United States (6.26 per 100,000). The most common primary sites were lungs, pancreas, stomach, and rectum in China. The ASRs of NENs incidence increased by 9.8% and 3.6% per year in China and the United States, respectively. The overall 5-year relative survival in China (36.2%) was lower than in the United States (63.9%). The 5-year relative survival was higher for female patients than male patients, and was higher in urban areas than in rural areas.@*CONCLUSIONS@#The disparities in burden of NENs persist across sex, area, age group, and site in China and the United States. These findings may provide a scientific basis on prevention and control of NENs in the two countries.

RESUMEN

Objective:To observe the clinical efficacy of addition and subtraction therapy of Jinkui Shenqiwan combined with Buzhong Yiqitang to postmenopausal osteoporosis (PMO) with deficiency of spleen and kidney, and to investigate its regulation effect on immune inflammatory factors. Method:One hundred and sixty patients were randomly divided into observation group and control group, with 80 cases in each group. Both groups got comprehensive western medicine treatment measures. Patients in control group additionally got Zhuanggu Zhitong capsule, 4 capsules/time, 3 times/day. Patients in observation group additionally got addition and subtraction therapy of Jinkui Shenqiwan combined with Buzhong Yiqitang, 1 dose/day. The treatment was continued for 24 weeks. Before and after treatment, lumbar L2-4 bone mineral density (BMD) was detected by Dual energy X-ray absorptiometry (DXA) and lumbar BMD was detected by quantitative CT (QCT). Scores of traditional Chinese medicine(TCM) syndromes and Chinese osteoporosis-targeted quality of life questionnaire (COQOL) were graded. Levels of Estradiol (E<sub>2</sub>), type Ⅰ procollagen amino terminal pro peptide (PINP), serum osteocalcin (OC), osteoprotegerin (OPG), type Ⅰ collagen cross-linked C-terminal peptide (S-CTX), tartrate resistant acid phosphatase (TRACP) and urinary pyridinoline (PYD) were detected. Levels of CD4<sup>+</sup> T cells, CD8<sup>+</sup> T cells, interleukin-17 (IL-17), tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), <italic>γ-</italic>interferon(IFN-<italic>γ</italic>) and interleukin-4 (IL-4) were calculated. The proportion of T helper cell (Th)17 and regulatory T cell (Treg) in CD4<sup>+</sup> T cells was calculated. Besides, the safety was evaluated. Result:Bone density was detected by DXA in observation group, and its T-value and bone density detected by QCT were all higher than those in control group (<italic>P</italic><0.01). After treatment, scores of TCM syndrome and COQOL were lower than those in control group (<italic>P</italic><0.01). Levels of PINP, OC, S-CTX, TRACP and PYD/Cr were all lower than those in control group (<italic>P</italic><0.01). Levels of OPG, CD8<sup>+</sup> and Treg were higher than those in control group (<italic>P</italic><0.05), levels of Th17, Th17/Treg, CD4<sup>+</sup>/CD8<sup>+</sup>, IL-17, TNF-<italic>α</italic> and IFN-<italic>γ </italic>were lower (<italic>P</italic><0.01), and levels of IL-4 and E<sub>2</sub> were higher than those in control group (<italic>P</italic><0.01). The clinical efficacy in observation group was better than that in control group (<italic>Z</italic>=2.103, <italic>P</italic><0.05). Conclusion:On the basis of calcium and vitamin D supplementation, Jinkui Shenqiwan combined with Buzhong Yiqitang can improve levels of E<sub>2</sub> and bone density, reduce clinical symptoms, improve quality of life, regulate bone metabolism index and immune inflammation reaction, with better clinical efficacy and safety.

Objective:To study the correlation of eight chemical components in Miao medicine <italic>Periploca forrestii</italic> from different producing areas with the ecological and soil factors. Method:The contents of neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, isochlorogenic acid B, isochlorogenic acid A, isochlorogenic acid C, procyanidin A<sub>2</sub>, and periplocin were simultaneously determined by ultra-performance liquid chromatography (UPLC). The root soil samples from various producing areas were collected for testing various soil factors, followed by climate data extraction with ArcGIS and topographic data recording using GPS. SPSS 24.0 was employed to conduct the bivariate analysis and stepwise regression analysis of the eight chemical components in <italic>P. forrestii</italic> from different producing areas with the ecological and soil factors. Result:Stepwise regression equations of the content of eight chemical components against ecological and soil factors were established. The findings demonstrated that neochlorogenic acid was negatively correlated with precipitation in the coldest season and chlorogenic acid negatively correlated with precipitation in the driest month. Cryptochlorogenic acid was negatively correlated with precipitation in the coldest season and average temperature in the warmest season, but positively with selenium. Isochlorogenic acid B was mainly affected by soil factors. Specifically, it was positively correlated with available iron and molybdenum but negatively with total phosphorus and available phosphorus. Isochlorogenic acid A was positively correlated with molybdenum but negatively with the coefficient of variation of precipitation. Isochlorogenic acid C showed a positive correlation with exchangeable magnesium. Procyanidin A<sub>2</sub> exhibited a positive correlation with molybdenum and a negative correlation with available potassium. Periplocin was negatively correlated with the coefficient of variation of precipitation. Conclusion:The correlation between the eight chemical components of <italic>P. forrestii</italic> and the ecological and soil factors has been clarified, which will provide reference for the introduction, cultivation, and standardized planting of <italic>P. forrestii </italic>and also a theoretical basis for further research on its ecological and soil factors and quality formation mechanism.

Objective:To evaluate effect of addition and subtraction therapy of Buyang Huanwutang combined with Si Junzitang and acupuncture to poststroke fatigue (PSF) and syndrome of Qi deficiency and blood stasis, at the same time we studied the antioxidant and anti-inflammatory effects. Method:One hundred and forty-four patients were randomly divided into control group and observation group (1∶1) by random number table. 66 patients in control group completed the therapy (4 patients were falling off or missing visit, 2 patients were eliminate), 67 patients in observation group completed the therapy (2 patients were falling off or missing visit, 3 patients were eliminate). In control group, patinets got acupuncture, 1 time/day, 6 times/week, they also got Geqi Tongmai grain, 10 g/time, 3 times/day. Patients in observation group got acupuncture (the same as which in control group), and addition and subtraction therapy of Buyang Huanwutang combined with Si Junzitang, 1 dose/day, and courses of treatment in two groups were 4 weeks. Before and after treatment, fatigue severity scale (FSS), NIH stroke scale (NIHSS), syndrome of Qi deficiency and blood stasis, stroke specific quality of life scale (SS-QOL), and scores of ability of daily life (ADL) were recorded. And levels of superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px), homocysteine (Hcy), interleukin-1β (IL-1β), IL-6, tumor necrosis factor-α (TNF-α) and C-reactive protein (CRP) were detected. And the safety evaluated. Result:Disease effect in observation group was better than which in control group (Z=2.118, P<0.05). And effect after using traditional Chinese medicine (TCM) was also better than that in control group (Z=2.046, P<0.05). Scores of FSS, syndrome of Qi deficiency and blood stasis, NIHSS, and levels of IL-1β, IL-6, Hcy, CRP, TNF-α and MDA in observation group were all lower than those in control group (P<0.01), and scores of SS-QOL, ADL, and levels of GSH-Px and SOD were all higher than those in control group (P<0.01). Then there was no related safety issues caused by drug. Conclusion:Addition and subtraction therapy of Buyang Huanwutang combined with Si Junzitang and acupuncture had effect of anti-oxidation and anti-inflammatory, and can significantly reduce fatigue and degree of neurological impairment and can improve patients' quality of life and daily life ability. The clinical effect is significant and safe, which is worthy of further research and application.

Objective: To investigate the effects of neochlorogenic acid, chlorogenic acid, cryptochlorogenic acid, isochlorogenic acid B, isochlorogenic acid A, and isochlorogenic acid C in effective parts of rhizoma arthritis against rhizoma arthritis from different sources of Miao medicine Periploca forrestii Schltr. The determination of 6 kinds of caffeoacylquinic acid chemical components and comprehensive analysis of their quality combined with chemometrics provide a basis for a reasonable and effective evaluation of the efficacy of Miao Medicine Periploca forrestii Schltr against rheumatoid arthritis. Methods: Simultaneous determination was performed by HPLC-UV method. The column was Xtimate C18 (250 mm×4.6 mm, 5 μm), the mobile phase was 0.1% formic acid-acetonitrile, gradient elution, the injection volume was 10 μL, and the flow rate was 1 mL/min. The column temperature is 25 ℃ and the detection wavelength is 327 nm. SPSS24.0 and SIMCA14.1 software are used for comprehensive analysis and evaluation of the quality of medicinal materials. Results: The content of 21 batches of Periploca forrestii Schltr samples from different sources was determined. Through the cluster analysis, the 21 batches of samples were classified into type III, of which the quality of types II and III was better; principal component analysis and partial least squares discriminant analysis The results were consistent with the clustering results. The chlorogenic acid and isochlorogenic acid C were screened to be the markers that caused the differences in the quality of Periploca forrestii Schltr from different sources. Conclusion The determination of the contents of six caffeoacylquinic acid components combined with stoichiometry can further evaluate the efficacy and quality of the anti-rheumatoid arthritis effect of Periploca forrestii Schltr, and provide a reference basis for the quality control of Periploca forrestii Schltr.

Objective@#Using updated population-based cancer registration (PBCR) data, we estimated nation-wide liver cancer statistics overall, by sex and by areas in China.@*Methods@#Qualified PBCR data of liver cancer in 2015 which met the data quality criteria were stratified by geographical locations, sex, and age groups. Age-specific incidence and mortality rates by sex and area were calculated. The burden of liver cancer was evaluated by multiplying these rates by the year of 2015 population. Chinese standard population in 2000 and World Segi′s population were used for the calculation of age-standardized rates (ASR) of incidence and mortality.@*Results@#Qualified 368 cancer registries covered a total of 309 553 499 populations in China, accounting for 22.52% of the national population. It is estimated that there were 370 000 new cases (274 000 males and 96 000 females) of liver cancer in China. The age-standardized incidence rates by Chinese standard population (ASR China) and World Segi′s population (ASR World) were 17.64 per 100 000 and 17.35 per 100 000, respectively. Rural areas showed higher incidence (ASR China: 20.07 per 100 000, ASR World: 19.67 per 100 000) than urban areas (ASR China: 15.90 per 100 000, ASR world: 15.67 per 100 000). Subgroup analysis showed that western areas of China had highest incidence rate of liver cancer, with the ASR China of 20.65 per 100 000 and 20.22 per 100 000 for ASR world, respectively. For new cases of liver cancer deaths, there were 326 000 new deaths (242 000 males and 84 000 females) in China, with age-standardized mortality rate by Chinese standard population and World Segi′s population of 15.33 per 100 000 and 15.09 per 100 000, respectively. Rural areas showed higher mortality (ASR China: 17.17 per 100 000, ASR world: 16.86 per 100 000) than urban areas (ASR China: 14.00 per 100 000, ASR World: 13.81 per 100 000).@*Conclusions@#There is still a heavy burden of liver cancer in China. Rural residents have higher incidence and mortality of liver cancer compared with urban counterparts. It is likely that many factors such as hepatitis virus infection, and aflatoxin exposure play a dominating role. Prevention and control strategies should be enhanced in the future.

RESUMEN

PURPOSE: This study was done to evaluate the effects of a short-term life review on spiritual well-being, depression, and anxiety in patients with terminal cancer. METHODS: The study used a pre posttest quasi experimental design with a nonequivalent control group. Measurement instruments included the Functional Assessment of Chronic Illness Therapy-Spiritual scale (FACIT-Sp12) and the Hospital Anxiety and Depression Scale (HADS). Participants were 32 patients with terminal cancer who were receiving chemotherapy or palliative care at hospitals or at home. Eighteen patients were assigned to the experimental group and 14 to the control group. A sixty minute short-term life review session was held twice a week as the intervention with the experimental group. RESULTS: There was a statistically significant increase in spiritual well-being in the experimental group compared to the control group. There were also significant decreases in depression and anxiety in the experimental group compared to the control group. CONCLUSION: The results indicate that a short-term life review can be used as a nursing intervention for enhancing the spiritual well-being of patients with terminal cancer.
